Abstract

Introduction The ERAS protocol (Enhanced Recovery After Surgery) is a multimodal pathway aimed to reduce surgical stress and to allow a rapid postoperative recovery. Application of the ERAS protocol to colorectal cancer surgery has been limited to a minority of hospitals in Italy. To promote the systematic adoption of ERAS in the entire regional hospital network in Piemonte an Audit and Feedback approach (A&F) has been adopted together with a cluster randomised trial to estimate the true impact of the protocol on a large, unselected population.Methods A multicentre stepped wedge cluster randomised trial is designed for comparison between standard perioperative management and the management according to the ERAS protocol. The primary outcome is the length of hospital stay (LOS). Secondary outcomes are: incidence of postoperative complications, time to patients’ recovery, control of pain and patients’ satisfaction. With an A&F approach the adherence to the ERAS items is monitored through a dedicated area in the study web site. The study includes 28 surgical centres, stratified by activity volume and randomly divided into four groups. Each group is randomly assigned to a different activation period of the ERAS protocol. There are four activation periods, one every 3 months. However, the planned calendar and the total duration of the study have been extended by 6 months due to the COVID-19 pandemic.The expected sample size of about 2200 patients has a high statistical power (98%) to detect a reduction of LOS of 1 day and to estimate clinically meaningful changes in the other endpoints.Ethics and dissemination The study protocol has been approved by the Ethical Committee of the coordinating centre and by all participating centres. Study results will be timely circulated within the hospital network and published in peer-reviewed journals.Trial registration number NCT04037787.

Abstract

Industrial by-products and wastes can be very challenging substrates for anaerobic digestion, since their composition in terms of nutrient content can be strongly unbalanced. Tannery by-products, for instance, are biomasses of animal origin with high nitrogen content since their composition is dominated by proteins, peptides and amino acids. The use of this biomass as main substrate to feed anaerobic digestion is limited because of the high concentrations of ammonia formed during fermentation. Ammonia is originated from the biological degradation of proteinaceous materials in anaerobic environment. Free ammonia permeation into bacterial cells can cause intracellular pH change, inhibition of specific enzymatic activities and increased energy requirement for cell maintenance. Therefore, high ammonia amount cause severe inhibition or failures of anaerobic digestion, with important consequences for both process stability and productivity. The aim of this study was the development of a multiphase anaerobic digestion process for tannery by-products, characterized by a C/N ratio below 5, in order to convert them into biogas. The process needed an acclimation phase of anaerobic microorganisms due to high concentrations of ammonia (NH4+/NH3), above 9,000 mg/L, and the low C/N ratio. Implication for process stability, microbial activity and for the achievement of the target performances are discussed.

Abstract

Although the postantibiotic effect (PAE) is a well recognized phenomenon, the mechanism by which it is induced has not fully elucidated yet. It has been suggested that PAE is the time required by bacteria to synthesize proteins or mRNA characterized by a short half-life that are consumed during antibiotic treatment.This phenomenon is widely studied on Gram-positive cocci and Gram-negative rods, while information about Gram-positive rods and Gram-negative cocci are scanty.To gain new insights on the PAE, this study was addressed to evaluated the time required by Moraxella catarrhalis and Lactobacillus planctarum to resume their physiological growth rate after exposure to various antibiotics. Methods PAE was estimated in accordance with the method of Craig and Gudmundsson using the following drugs: penicillin, piperacillin-tazobactam, cefalotin, ceftazidime, imipenem, ciprofloxacin, gentamycin and azithromycin. Log-phase bacteria were exposed to drug at a concentration corresponding to 4 times the MIC value for 1h.The drug was inactivated by 1:1000 dilution. Bacterial counts were determined at time zero, immediately after drug dilution, and at each hour after removal for 6 - 7h by a pour-plate technique. The PAE was defined as the difference in time required by test and control cultures to increase by 1 log in CFU number. Results All drugs tested induced a PAE on the strains studied. M. catarrhalis registered PAE values ranging between 0,5 (gentamycin) and 2 (ceftazidime, imipenem and azithromycin).With respect to L. plantarum a PAE between 0,8 (cefalotin) and 3 hours (ciprofloxacin) were detected. Conclusion. These findings demonstrated that all the drugs tested were able to induce a PAE on the strains tested.This observation differs from that observed on Gram-negative rods characterised by negative PAE values induced by penicillins and cephalosporins.This results might reflect the different target of these compounds on these Gram-positive rods or the different affinity of the antibiotics to several PBPs that required more time to restore the physiological fitness of the bacterial cell.
