Organizational culture in sports affects well-being, performance, and overall success. Although team culture has been explored within able-bodied teams, little is known about culture development within parasport. Using a descriptive case study approach, our study examined the culture development and experiences of an elite parasport team. Two semistructured individual telephone interviews were conducted with nine participants (athletes and staff), and one athlete-participant completed one interview. An inductive thematic analysis revealed two themes: approach to culture development and team culture components. Culture development was linked with facilitative leadership and important resources. This supported the team to create their shared values and mechanisms, including a behavioral framework, other artifacts (e.g., shared language and team motto), and a relaxed environment. These helped to maintain their agreed culture and benefited their well-being, progress, and team cohesion. The use of conventional scan bodies (SBs) with an intraoral scanner (IOS) to capture the position of a complete arch of dental implants has proven to be challenging. The literature is unclear about the accuracy of intraoral scanning techniques using SBs that are connected vertically to multiunit abutments (MUAs) for numerous adjacent implants in the same arch. Recently, there has been a paradigm shift from vertical SBs to horizontal SBs, which are positioned perpendicular to the long axis of the MUAs. Most IOSs available today can capture these horizontal SBs, called scan gauges (SGs), with better accuracy and consequently acquire the position of multiple adjacent implants using an effective scan path, thus reducing stitching and the number of images. The key to implementing this novel technology is to strategically arrange the SGs to optimize horizontal overlap of multiple adjacent SGs without touching each other. By superimposing two high-resolution intraoral scans of the SGs, an artificial intelligence (AI) algorithm is employed to produce a calibrated digital best-fit model on which a passive complete-arch prosthesis can be designed and fabricated. Objectives: Care for community‐dwelling people with dementia is frequently delegated to relatives, who find themselves in the role of informal caregivers with no practical management knowledge. This situation exposes caregivers to increased risk for emotional wellbeing. The current study aims to test whether the integration of the efficacy of an immersive virtual reality (VR) experience into an online psychoeducational program impacts caregiver empathy and therefore emotional wellbeing. Methods: One‐hundred informal caregivers of mild‐to‐moderate Alzheimer's disease (AD) patients will be enrolled and randomly assigned to (i) an online psychoeducational program (control arm); or (ii) an online psychoeducational program integrated with VR (experimental arm). VR will consist of 360‐degree videos involving the caregivers to an immersive experience of dementia symptoms from the patient's perspective. Before, after the intervention and after 2 months, all participants will complete validated clinical scales for caregiver burden and anxiety (primary outcomes) and sense of competence and dispositional empathy (secondary outcomes). A subsample of 50 participants will also undergo MRI exam, including structural and functional (resting‐state and task‐functional MRI [fMRI]) sequences. The fMRI task paradigm will use emotional stimuli to evaluate the neural correlate of empathy, by stressing its cognitive and affective components. The main outcome will be the change in the clinical assessment; the secondary outcome will be the change in brain connectivity of networks subserving the empathic and emotional functioning. Results: We expect that the psychoeducational program will decrease anxiety and stress, enabling caregivers to perceive themselves capable of managing AD patients at home, educating them on symptom handling and boosting their cognitive empathy. In the experimental intervention, the VR‐based experience will act as an add‐on to psychoeducation, leading to greater improvement in the assessed clinical dimensions. VR should, in fact, enable a deeper understanding of disease symptoms and improve caregivers' cognitive empathy. We expect that the experimental intervention will result in deeper comprehension of disease symptoms and further strengthen caregivers' cognitive empathy. At the neural level, we expect to observe increased activation in circuits subserving cognitive empathy and decreased activation in circuits underlying affective empathy. Conclusions: To the best of our knowledge, this will be the first randomized controlled trial assessing the effect of combining psychoeducational interventions with VR‐based experience in caregivers, and assessing both clinical and imaging outcomes. Simple Summary: It is known that there are sex differences in adverse events experienced following cancer treatment. However, little is known about sex differences in adverse events experienced in individuals with metastatic cancer. Here, using SEER-Medicare data, we investigate sex differences in adverse events in lung cancer individuals with brain metastasis who are 66 years old and older. Sex differences in adverse events were observed and were dependent on lung cancer histology, age at diagnosis, year of diagnosis and treatment as well as potential interplay between these variables. Sex differences are evident in adverse events (AEs) related to brain tumors, yet sex differences in AEs specific to brain metastases (BrMs) are underexplored. Lung cancer BrMs dominate among BrM, comprising over half of cases. This study examined sex differences in AEs associated with lung cancer BrMs in individuals aged 66 or older using the SEER-Medicare dataset. Multivariable logistic regression, adjusted for demographic factors and comorbidities, stratified by histological subtype, treatment, age, and year of diagnosis were used to analyze AEs among those with BrMs from primary lung tumors. Year of diagnosis was grouped into prior/post-2013, to account for shifts in treatment paradigms. The results showed nuanced sex-specific AEs. Females diagnosed post-2013 with small-cell, squamous-cell, or other non-small-cell carcinoma BrMs had a higher headache likelihood than males. Males with adenocarcinoma post-2013 were more likely to experience brain herniation. Females aged 76 and older with small-cell BrM exhibited increased vision difficulty risk compared to males of the same age, with no significant difference in other age groups. Males treated for adenocarcinoma faced heightened hemorrhagic stroke risk. Cet article concerne les défis actuels pour la psychanalyse que constituent le genre et les incongruences de genre. En effet, bien que le terme genre soit de plus en plus utilisé, il reste un concept plus difficile à définir et à circonscrire qu'il n'y paraît au premier abord. Afin d'apporter un éclairage sur ces sujets, il sera exposé le changement au combien important, qu'est le passage du paradigme du syndrome transsexuel à celui de la santé transgenre. Cet exposé sera alors étayé par deux vignettes cliniques : celle de Claudia dont la transition s'inscrit dans un parcours transsexuel et celle de Rose dont la transition paraît plus s'apparenter à un parcours transgenre. Les vignettes cliniques de Claudia et de Rose permettent d'illustrer ce qui se rapporte à une manière différente d'appréhender la différence des sexes : entre une abstraction qui épouse les contours d'une différence des sexes conçue comme binaire pour Claudia, et un genre fluide pour Rose. Suite à ces remarques, il est proposé de revenir sur le concept de genre afin de mieux le cerner et de le concevoir, notamment dans sa rencontre clinique et théorique avec la psychanalyse. Le genre est alors conçu comme un concept frontière, renvoyant à un troisième corps social au croisement du sentiment personnel et subjectif et du sociopolitique. En référence à la narratologie, le genre est défini comme une construction personnelle et/ou collective visant à donner une cohérence à la différence des sexes, et étayé par une réalisation par la perception, aide possible à la liaison de l'excitation sexuelle associée aux questions relatives à la différence anatomique des sexes d'un point de vue psychanalytique, qui concerne autant l'analysant que le psychanalyste. This article is concerned with the current challenges to psychoanalysis that gender and gender discordance represent. Indeed, although the term gender is increasingly used, it remains a concept that is more difficult to define and circumscribe than it first appears. In order to shed light on these issues, the important shift from the transsexual syndrome paradigm to the transgender health paradigm will be discussed. This presentation will then be supported by two clinical vignettes: that of Claudia, whose transition is part of a transsexual journey, and that of Rose, whose transition seems to be more akin to a transgender journey. Claudia's and Rose's stories allow us to illustrate a different way of apprehending the difference of the sexes: for Claudia, an abstraction that follows the contours of a difference of the sexes considered as binary, and, for Rose, a conception of gender as fluid. Following these reflections, it is suggested that we return to the concept of gender in order to better define and conceptualize it, particularly in its clinical and theoretical encounter with psychoanalysis. Gender is thus considered as a frontier concept, referring to a third social body at the crossroads of personal and subjective feeling and the socio-political. In reference to narratology, gender is defined as a personal and/or collective construction aiming to give coherence to the difference of the sexes and supported by a realization through perception. Gender can be thought of as an aide to the binding of sexual excitation associated with questions relating to the anatomical difference of the sexes from a psychoanalytical point of view, which concerns both the analysand and the psychoanalyst. A core assumption in the current neurosemantic research is that meanings of concrete words (object nouns, action verbs) are at least partially grounded in modality-specific representations implemented by perceptual and motor cortices. Nevertheless, increasing evidence from the multisensory research suggests that extensive portions of what are traditionally considered modality-specific cortices are in fact supramodal in nature – that is, they can process specific information in multiple sensory modalities and in both normal and sensory-deprived individuals. My central argument in this paper is that the amount of data collected within the supramodal paradigm, although certainly not conclusive, is already robust enough to foster reflections about the role of modality-specificity in the semantic brain and to prompt considerations about the possibility of promoting a property-specific and modality-invariant turn in neurosemantics. Historically, specific mutations in WT1 gene have been associated with distinct syndromes based on phenotypic characteristics, including Denys-Drash syndrome (DDS), Frasier syndrome (FS), Meacham syndrome, and WAGR syndrome. DDS is classically defined by the triad of steroid-resistant nephrotic syndrome (SRNS) onset in the first year of life, disorders of sex development (DSD), and a predisposition to Wilms tumor (WT). Currently, a paradigm shift acknowledges a diverse spectrum of presentations beyond traditional syndromic definitions. Consequently, the concept of WT1-related disorders becomes more precise. A genotype–phenotype correlation has been established, emphasizing that the location and type of WT1 mutations significantly influence the clinical presentation, the condition severity, and the chronology of patient manifestations. Individuals presenting with persistent proteinuria, with or without nephrotic syndrome, and varying degrees of kidney dysfunction accompanied by genital malformations should prompt suspicion of WT1 mutations. Recent genetic advances enable a more accurate estimation of malignancy risk in these patients, facilitating a conservative nephron-sparing surgery (NSS) approach in select cases, with a focus on preserving residual kidney function and delaying nephrectomies. Other key management strategies include kidney transplantation and addressing DSD and gonadoblastoma. In summary, recent genetic insights underscore the imperative to implement individualized, integrated, and multidisciplinary management strategies for WT1-related disorders. How to collect relevance judgment has long been an important problem in Information Retrieval (IR). A popular method is to collect relevance judgment in a point‐wise manner, in which assessors examine and give an absolute relevance score for each item independently of the others. As an alternative, pair‐wise relevance judgment, also named preference judgment, allows an assessor to compare two items side‐by‐side and express their preference for one over the other. Previous work has explored the differences between these two paradigms of relevance judgments from many different aspects. Most of these works are conducted through explicit/implicit feedback. However, few works investigate the underlying neurological mechanisms of the two paradigms. In this paper, we conduct a lab study to investigate and compare point‐wise and pair‐wise relevance judgment in image search scenarios. We study the neurological mechanisms of the two paradigms through an event‐related potential (ERP) analysis of the users' brain signals while viewing images during a search process. We have obtained several observations, such as search engine users tend to pay more attention to preferred items in the point‐wise paradigm but unpreferred items in the pair‐wise paradigm. Furthermore, we test the adoption of brain signals as implicit feedback for predicting pair‐wise relevance judgment, highlighting the feasibility of leveraging brain signals to understand users' relevance judgments. This article will briefly introduce the traditional disability studies project through the social model of disability and how its inherent theoretical limitations became the impetus to perpetuate a need to adopt critical disability theory (CDT) to expand the intellectual arsenal of the discipline. Being a reflexive inquiry that resists domination that oppresses people with disabilities, CDT's reach produced work in conversation with other areas of critical social thought and inspired this paper to explore how post-colonial work – particularly Homi Bhabha's hybridity theory – can be incorporated into CDT's paradigmatic views of power. This paper concludes by addressing some of the debates and cultural implications that stem from conceptually merging CDT and post-colonial hybridity theory to frame how it can reimagine the possibilities and trajectories of a global disability movement. Points of interest: This article addresses the two main ideas in disability studies, starting with the social model of disability, which helped put forth critical disability theory. Critical disability theory suggests that people with disabilities are controlled by others using the idea of 'impairment' to justify their power. The article suggests that critical disability theory can benefit from an approach called post-colonial hybridity, which combines global North and South ideas to help represent people with disabilities in the Southern part of the world. The article also concludes by exemplifying how mental health and traditional healing practices are combined to support well-being. The rampant use of antibiotics has led to the emergence of multidrug resistance and is often coupled with gut dysbiosis. To circumvent the harmful impact of antibiotics, probiotics have emerged as an effective intervention. However, while the new probiotics are being added to the list, more recently, the nature and role of their counterparts, viz. prebiotics, postbiotics and parabiotics have also drawn considerable attention. As such, intricate relationships among these gut-biotics vis-à-vis their role in imparting health benefits is to be delineated in a holistic manner. Prebiotic dietary fibers are selectively fermented by probiotics and promote their colonization in the gut. The proliferation of probiotics leads to production of fermentation by-products (postbiotics) which affect the growth of enteropathogens by lowering the pH and producing inhibitory bacteriocins. After completing life-cycle, their dead remnants (parabiotics e.g. exopolysaccharides and cell wall glycoproteins) also inhibit adhesion and biofilm formation of pathogens on the gut epithelium. These beneficial effects are not just endemic to gut but a systemic response is witnessed at different gut-organ axes. Thus, to decipher the role of probiotics, it is imperative to unravel the interdependence between these components. This review elaborates on the recent advancements on various aspects of these gut-biotics and the mechanism of potential attributes like anti-oxidant, anti-inflammatory, anti-neoplastic, anti-lipidemic and anti-hyperglycemic benefits. Healthcare inequity is a persistent systemic problem, yet many solutions have historically focused on "debiasing" individuals. Individualistic strategies fit within a competency‐based medical education and assessment paradigm, whereby professional values of social accountability, patient safety, and healthcare equity are linked to an individual clinician's competence. Unfortunately, efforts to realise the conceptual linkages between medical education curricula and goals to improve healthcare equity fail to address the institutional values, policies, and practices that enable structural racism. In this article, we explore alternative approaches that target collective and structural causes of health inequity. We first describe the structural basis of healthcare inequity by identifying the ways in which institutional culture, power and privilege erode patient‐centred care and contribute to epistemic injustice. We then outline some reasons that stereotypes, which are a culturally supported foundation for discrimination, bias and racism in healthcare, cannot be modified effectively through individualistic strategies or education curricula. Finally, we propose a model that centres shared values for leadership by individuals and institutions with consistency in goal setting, knowledge translation, and talent development. Revitalization has emerged as an innovative treatment approach for immature permanent teeth with necrotic pulp. This article presents a comprehensive analysis of revitalization, focusing on its principles, clinical protocols and outcomes. The article highlights the importance of thorough diagnosis and assessment of the pulp and peri-apical condition to determine the suitability of revitalization. Various factors influencing the success of revitalization, such as the use of scaffolds, growth factors and stem cells, are discussed. Additionally, a case that was treated with a PRP scaffold is also presented. Overall, revitalization shows promise in promoting pulp regeneration and improving treatment outcomes in selected cases of pulp necrosis. This article uses the Ubuntu paradigms to reflect on the ways in which community-level values and frameworks can be recognised, strengthened and used to promote children's welfare. We present a literature review and documented practical examples from African Ubuntu communities in Global South that strongly emphasise the collectivist approach in their ways of life. With a further examination of the Ubuntu philosophy, we identify common values and patterns that can be instrumental in fostering child welfare. These are volunteerism, solidarity, vigilance, morality and hospitality. We underline these informal standards as not only crucial to social workers providing interventions for children that belong to families and communities that are interdependent in nature, but as well present an Ubuntu social work model that can be a benchmark for social work practice with children and families in multicultural communities in Global North. Introduction: Geriatric fracture is a pressing global health issue, marked by elevated mortality and morbidity rates and escalating health care costs. The evolving health care system from fee-for-service to quality-based reimbursement has led to externally driven reward and reimbursement systems that may not account for the complexity of caring for older adults with fracture. Significance: The aim of this review is to highlight the need for a shift towards meaningful metrics that impact geriatric fracture care and to issue a call to action for all medical societies to advocate for national reimbursement and ranking systems that focus on metrics that truly matter. Results: Traditional metrics, while easier to capture, may not necessarily represent high quality care and may even have unintentional adverse consequences. For example, the focus on reducing length of stay may lead to older patients being discharged too early, without adequately addressing pain, constipation, or delirium. In addition, a focus on mortality may miss the opportunity to deliver compassionate end-of-life care. Existing geriatric fracture care metrics have expanded beyond traditional metrics to include assessment by geriatricians, fracture prevention, and delirium assessments. However, there is a need to further consider and develop patient-focused metrics. The Age-Friendly Health Initiative (4 Ms), which includes Mobility, Medication, Mentation, and what Matters is an evidence-based framework for assessing and acting on critical issues in the care of older adults. Additional metrics that should be considered include an assessment of nutrition and secondary fracture prevention. Conclusion: In the realm of geriatric fracture care, the metrics currently employed often revolve around adherence to established guidelines and are heavily influenced by financial considerations. It is crucial to shift the paradigm towards metrics that truly matter for geriatric fracture patients, recognizing the multifaceted nature of their care and the profound impact these fractures have on their lives. Perceived stress, a global health problem associated with various mental disorders, is assumed to be influenced by dysfunctional beliefs. It can be hypothesized that these beliefs can be modified with the help of approach‐avoidance modification trainings (AAMTs). In the present study (conducted 2020–2022), we aimed to clarify whether the efficacy of AAMTs can be enhanced by utilizing the expression of emotions to move AAMT stimuli. For this purpose, we tested the feasibility and acceptability of a new AAMT paradigm in which the expression of disgust is used to move stress‐increasing beliefs away from oneself and the expression of positive emotions is used to move stress‐reducing beliefs towards oneself (AAMT‐DP). Additionally, we explored the therapeutic potential of the AAMT‐DP intervention by comparing it to an inactive control condition and to a conventional AAMT in which stimuli are moved by swipe movements (n = 10 in each condition). The primary outcome was perceived stress 1 week after the training as assessed with the Perceived Stress Scale. Findings indicate sufficient feasibility and acceptability of the intervention and that the decrease in perceived stress in the AAMT‐DP condition was greater than in the inactive control condition (g = 0.72 [0.10, 1.72]) and than in the swipe control condition (g = 0.64 [0.01, 1.41]). In sum, findings provide preliminary evidence for the feasibility, acceptability, and the therapeutic potential of the AAMT‐DP intervention. [ABSTRACT FROM AUTHOR]

Purpose: To propose a paradigm change for the validation procedures of medication adherence questionnaires. Methods: A total of 121 validation procedures of unique questionnaires for medication adherence were analyzed. Results: "Construct validity" and "internal consistency" were most often assessed, and test results varied largely. A more in-depth analysis indicated that the assessment of medication non-adherence included distinct but related constructs, such as the extent to which doses are missed, and the attempt to identify different facets of medication-taking behavior. Consequently, each construct requires a different measurement approach with different psychometric tests for establishing its validity and reliability. Conclusion: Results show that assessing the validity and reliability of adherence questionnaires with standard procedures including statistical tests is inconclusive. Refinement of the constructs of non-adherence is needed in pharmacy and medical practice. We suggest a distinction between the (i) extent of missed doses over the past 2 weeks, (ii) modifiable reasons for non-adherence behavior, and (iii) unmodifiable factors of non-adherence. Friendship is a unique and underexplored area of human sociality. Research suggests that humans have preferences for characteristics in their friends that maximize the benefits of these relationships. Yet, whereas more friends might increase friendship benefits, humans also have limited time, resources, and energy to invest in finding high-quality friends, making it likely that the nature of these preferences differs depending on the resources an individual has available to invest in this goal. Across two studies (total N = 693), we investigated how this trade-off may function by investigating the nature of friendship preferences. In Study 1, we utilized the budget paradigm method from behavioral economics to investigate the necessities and luxuries in friendship preferences. In Study 2, we replicated these preferences with a novel method and extended our investigation into understanding the hierarchical nature of these preferences. Recent research has examined the effectiveness of inclusion tools aimed at encouraging greater involvement of disadvantaged social groups in local deliberative institutions. Most research, quantitative and qualitative, tends to examine positive outcomes, although results are frequently ambivalent. This research begins by arguing that analytical frameworks should consider the limitations of inclusion tools and that greater attention should be given to alternative approaches to inclusion (material/symbolic, formal/informal, focused/general). Based on these theoretical points, the study focuses on the views and experiences of working-class people and smallholder farmers in relation to their participation in open rural assemblies (concejos abiertos) in the Basque Country (Spain). The research employed an ethnographic methodology involving 20 observations of assemblies/events and 55 in-depth interviews conducted between 2012 and 2015. Three inclusion tools were identified as key to the functioning of the assemblies: "administrative and technical support" for engaging with the bureaucratic processes, a legal "right to paid work leave" for board members, and the use of "multi-disciplinary boards." The study examines the effectiveness of these tools in motivating and facilitating the involvement of participants from disadvantaged social class positions, presenting novel findings in the case of paid work leave and multi-disciplinary boards. The importance of informal practices and procedures for reducing material constraints are emphasized as well as the identification of some limitations of inclusion tools in a context of predominant elitist paradigms. The key aim of diabetes management is to prevent complications, which are a major cause of morbidity and mortality. At an individual level, people with diabetes are less likely than they were several decades ago to experience classical macrovascular and microvascular complications as a result of improvements in modifiable cardiovascular risk factors and preventive healthcare. However, a significant burden of diabetes complications persists at a population level because of the increasing incidence of diabetes, as well as longer lifetime exposure to diabetes because of younger diagnosis and increased life expectancy. Trials have shown that the most effective strategy for preventing complications of diabetes is a multifactorial approach focussing simultaneously on the management of diet, exercise, glucose levels, blood pressure and lipids. In addition to the cornerstone strategies of addressing diet, exercise and lifestyle measures, the introduction of newer glucose‐lowering agents, including sodium‐glucose transport protein 2 inhibitors and glucagon‐like peptide‐1 agonists, have brought about a paradigm shift in preventing the onset and progression of complications of type 2 diabetes, particularly cardiovascular and renal disease. The improvement in rates of classical complications of diabetes over time has been accompanied by a growing awareness of non‐traditional complications, including non‐alcoholic fatty liver disease. These emerging complications may not respond to a glycaemic‐centred approach alone and highlight the importance of foundational strategies centred on lifestyle measures and supported by pharmaceutical therapy to achieve weight loss and reduce metabolic risk in patients living with diabetes. It is well known that social interaction enhances learning processes, improving abilities such as attention and memorization. However, it is not clear whether similar advantages may be obtained even in virtual environments. Here, we investigate whether virtual interactions in a video game, similarly to real-life social interactions, may improve individuals' performance in a subsequent implicit learning task. Twenty-one healthy participants were asked to play a cooperative video game for 20 minutes in three different gaming modalities: alone (Single-Player); together with someone without verbal interactions (Multi-Player −); and with someone with verbal interactions (Multi-Player +). After each gaming session, participants were presented with an EEG paradigm directed to measure mismatch negativity (MMN) responses, a well-validated index of implicit learning. MMN responses were significantly larger following Multi-Player +, as compared with Single-Player, and Multi-Player − conditions. No significant difference was found between Single-Player, and Multi-Player − conditions. These results indicate that implicit learning processes are enhanced following communicative virtual interactions. Verbal interaction in a virtual environment seems necessary to elicit social copresence and its positive effects on learning performances. Background: One of the criticisms of the operational/diagnostic criteria, generalised since DSM‐III, has been that they were shaped solely to achieve the best inter‐peer reliability with no considerations for validity. This does not fully reflect reality since throughout the development of the criteria, there was an effort to define and fulfil some validity requirements. However, despite several attempts to create alternative diagnostic systems, there is still a widespread misunderstanding of the epistemological foundations that support this paradigm. Methods: In this article, we intend to analyse the epistemological context in which the operational criteria (OC) emerged and some of the validation processes they have undergone since their conception. Results: On the epistemological basis of these operational criteria (OC) the influence of Hempel has been widely discussed. However, the group from St. Louis and, also the DSM‐III editors, never openly acknowledged his role and his contribution and revealed other influences such as other medical specialties (that used and validated several OC in the diagnosis of their diseases). On the other hand, contrary to what has often been mentioned there has been a continuous attempt to validate the OC since their conception. In the implementation and development of the operational paradigm, a more instrumental trend was followed, focused on utility, but with successive attempts to achieve realistic validity by searching for biological or psychological causality. The methodologies were initially expert‐driven and gradually more data‐driven and included some variables external to the construct itself, such as familial aggregation, diagnostic consistency over time, prognostic and other psychometric measures. The purpose of this exploratory study was to learn more about the social experiences of people with chronic lymphocytic leukemia (CLL), with respect to diagnosis disclosure, communicating one's health risks to others in the context of COVID-19, and managing positive and negative support. Communication privacy management theory framed this study. Thirty-eight participants took part in interviews. Phenomenological analysis yielded 4 themes: strategizing disclosure; reconciling others' paradigms of cancer; living with CLL during the COVID-19 pandemic; and seeking, receiving, and managing support. By learning more about these experiences, cancer navigators, oncologists, and mental health professionals will be better equipped to support the psychosocial needs of patients and their families. The use of Environmental Enrichment (EE) has been widely studied in animal models. However, the application of the same in humans is limited to rehabilitation settings. To investigate the feasibility of a community-based EE paradigm in adults with brain injury. Six individuals diagnosed with traumatic brain injury enrolled in the study. The Go Baby Go Café instrumented with a body weight harness system, provided physical and social enrichment as participants performed functional tasks for 2 hours, three times a week, for 2 months. Feasibility and safety outcomes were recorded throughout sessions. Clinical measures including 10-meter walk, timed up and go, jebsen hand function, 6-minute walk, and trail making tests were obtained pre and post intervention. All participants completed the study. The attendance was 100% and adherence was 87%. Positive changes in clinical measures were statistically significant for the timed up and go (p = 0.0175), TUG-cognitive (p = 0.0064), 10-meter walk (p = 0.0428), six-minute walk (p = 0.0196), TMT-A (p = 0.034). Changes in JHFT were not significant (p = 0.0506), with one subject recording values counter to the trend. The Café was a comprehensive EE-based intervention that was feasible, safe, and has the potential to enhance motor and cognitive function in individuals with brain injury. [ABSTRACT FROM AUTHOR]

Refractory chronic cough is a disabling disease with very limited therapeutic options. A better understanding of cough pathophysiology has led to the development of emerging drugs targeting cough receptors. Recent strides have illuminated novel therapeutic avenues, notably centred on modulating transient receptor potential (TRP) channels, purinergic receptors, and neurokinin receptors. By modulating these receptors, the goal is to intervene in the sensory pathways that trigger cough reflexes, thereby providing relief without compromising vital protective mechanisms. These innovative pharmacotherapies hold promise for improvement of refractory chronic cough by offering improved efficacy and potentially mitigating adverse effects associated with current recommended treatments. A deeper comprehension of their precise mechanisms of action and clinical viability is imperative for optimising therapeutic interventions and elevating patient care standards in respiratory health. Classic studies of human categorization learning provided evidence that high-variability training in the prototype-distortion paradigm enhances subsequent generalization to novel test patterns from the learned categories. More recent work suggests, however, that when the number of training trials is equated across low-variability and high-variability training conditions, it is low-variability training that yields better generalization performance. Whereas the recent studies used cartoon-animal stimuli varying along binary-valued dimensions, in the present work we return to the use of prototype-distorted dot-pattern stimuli that had been used in the original classic studies. In accord with the recent findings, we observe that high-variability training does not enhance generalization in the dot-pattern prototype-distortion paradigm when the total number of training trials is equated across the conditions, even when training with very large numbers of distinct instances. A baseline version of an exemplar model captures the major qualitative pattern of results in the experiment, as do prototype models that make allowance for changes in parameter settings across the different training conditions. Based on the modeling results, we hypothesize that although high-variability training does not enhance generalization in the prototype-distortion paradigm, it may do so when participants learn more complex category structures. Background: Self‐regulation in early childhood develops within a social context. Variations in such development can be attributed to inter‐individual behavioral differences, which can be captured both as facets of temperament and across a normal:abnormal dimensional spectrum. With increasing emphasis on irritability as a robust early‐life transdiagnostic indicator of broad psychopathological risk, linkage to neural mechanisms is imperative. Currently, there is inconsistency in the identification of neural circuits that underlie irritability in children, especially in social contexts. This study aimed to address this gap by utilizing a functional magnetic resonance imaging (fMRI) paradigm to investigate pediatric anger/frustration using social stimuli. Methods: Seventy‐three children (M = 6 years, SD = 0.565) were recruited from a larger longitudinal study on irritability development. Caregivers completed questionnaires assessing irritable temperament and clinical symptoms of irritability. Children participated in a frustration task during fMRI scanning that was designed to induce frustration through loss of a desired prize to an animated character. Data were analyzed using both general linear modeling (GLM) and independent components analysis (ICA) and examined from the temperament and clinical perspectives. Results: ICA results uncovered an overarching network structure above and beyond what was revealed by traditional GLM analyses. Results showed that greater temperamental irritability was associated with significantly diminished spatial extent of activation and low‐frequency power in a network comprised of the posterior superior temporal sulcus (pSTS) and the precuneus (p <.05, FDR‐corrected). However, greater severity along the spectrum of clinical expression of irritability was associated with significantly increased extent and intensity of spatial activation as well as low‐ and high‐frequency neural signal power in the right caudate (p <.05, FDR‐corrected). Conclusions: Our findings point to specific neural circuitry underlying pediatric irritability in the context of frustration using social stimuli. Results suggest that a deliberate focus on the construction of network‐based neurodevelopmental profiles and social interaction along the normal:abnormal irritability spectrum is warranted to further identify comprehensive transdiagnostic substrates of the irritability. Following a racialized encounter, especially one that is racist or discriminatory, individuals often turn to those in their racial ingroup for social support. However, ingroup racial membership lines may be blurred for those with one Black parent and one White parent. The purpose of this study is to examine Black-White multiracial individuals' experiences navigating the monoracial paradigm of race when seeking racialized support. Guided by Critical Multiracial Theory, I take a self-reflexive Interpretative Phenomenology Approach to explore the racialized support-seeking experiences of 15 Black-White biracial adults in the U.S. Results reveal a unique support seeking process that is influenced by the monoracial paradigm of race. As the limits of fetal viability have increased over the past 30 years, there has been a growing body of evidence supporting the idea that chronic disease should be taken into greater consideration in addition to survival after preterm birth. Accumulating evidence also suggests there is early onset of biologic aging after preterm birth. Similarly, chronic kidney disease (CKD) is also associated with a phenotype of advanced biologic age which exceeds chronologic age. Yet, significant knowledge gaps remain regarding the link between premature biologic age after preterm birth and kidney disease. This review summarizes the four broad pillars of aging, the evidence of premature aging following preterm birth, and in the setting of CKD. The aim is to provide additional plausible biologic mechanisms to explore the link between preterm birth and CKD. There is a need for more research to further elucidate the biologic mechanisms of the premature aging paradigm and kidney disease after preterm birth. Objective: Anorexia nervosa (AN) is a pernicious psychiatric disorder which is principally characterized by a fear of weight gain. Notwithstanding the centrality of fear in the psychopathology of AN, controlled assessments of negative valence systems are lacking. Herein we assess fear conditioning in adolescent females with AN. Method: Adolescent girls (Mage = 14.6 years, ±1.57) with DSM‐5 diagnoses of AN (N = 25) and age‐matched control girls (Mage = 14.8 years, ±1.46) with no DSM‐5 diagnoses (N = 25) completed structured clinical interviews and participated in a classical three‐phase Pavlovian fear conditioning paradigm. Participants with comorbid anxiety disorders were excluded. Skin conductance response (SCR) was measured, alongside self‐reported fear, valence, and fear expectancy ratings. Results: Both groups demonstrated significant differential acquisition across all four measures. Regarding group comparisons, no differences emerged for self‐reported fear, valence, and fear expectancy ratings during acquisition, although for SCR, those with AN demonstrated reduced physiological arousal relative to controls. Both groups demonstrated significant differential extinction for unconditioned stimuli (US) expectancy, self‐report fear, and self‐report valence. No statistically significant group differences were evident during extinction to the conditioned stimuli (CS)+, on any outcome measure. However, controls reported more positive valence to the CS− than those with AN. Conclusions: Contrary to our hypotheses, our preliminary assessment did not find support for elevated fear responding among adolescent girls with AN with regards to fear acquisition or extinction. These data suggest that AN in adolescent girls may not be associated with a heightened propensity to acquire fear, but conversely, may suggest that exposure treatments for AN may be helpful, since extinction learning is intact in AN. Public significance: AN is characterized by fear‐related symptoms, including food and weight‐related fear, and behavioral avoidance, yet controlled studies assessing fear learning are limited. Our preliminary assessment of adolescent AN indicates no abnormalities in fear learning among adolescents with AN. Simple Summary: Colorectal cancer is a common illness. It can spread to the liver in about a quarter of people with colorectal cancer, known as metastases. Previously, people with liver metastases did not survive for long. Thankfully, this is changing. As we understand more about the underlying causes and genetics of the disease, we can tailor treatments to patients on a more individual basis. Treatments like chemotherapy have made a difference in patients' survival, and now newer treatments like immunotherapy can have even greater benefits. Surgery is also changing, with more advanced techniques allowing for better recovery for patients and more aggressive surgery. It is important for surgeons to consider a large number of individual factors when making decisions with patients about their treatment—this is what we mean by "precision surgery". The incidence of colorectal cancer and colorectal liver metastases (CRLM) is increasing globally due to an interaction of environmental and genetic factors. A minority of patients with CRLM have surgically resectable disease, but for those who have resection as part of multimodal therapy for their disease, long-term survival has been shown. Precision surgery—the idea of careful patient selection and targeting of surgical intervention, such that treatments shown to be proven to benefit on a population level are the optimal treatment for each individual patient—is the new paradigm of care. Key to this is the understanding of tumour molecular biology and clinically relevant mutations, such as KRAS, BRAF, and microsatellite instability (MSI), which can predict poorer overall outcomes and a poorer response to systemic therapy. The emergence of immunotherapy and hepatic artery infusion (HAI) pumps show potential to convert previously unresectable disease to resectable disease, in addition to established systemic and locoregional therapies, but the surgeon must be wary of poor-quality livers and the spectre of post-hepatectomy liver failure (PHLF). Volume modulation, a cornerstone of hepatic surgery for a generation, has been given a shot in the arm with the advent of liver venous depletion (LVD) ensuring significantly more hypertrophy of the future liver remnant (FLR). The optimal timing of liver resection for those patients with synchronous disease is yet to be truly established, but evidence would suggest that those patients requiring complex colorectal surgery and major liver resection are best served with a staged approach. In the operating room, parenchyma-preserving minimally invasive surgery (MIS) can dramatically reduce the surgical insult to the patient and lead to better perioperative outcomes, with quicker return to function. Background: Behavioral economic theory suggests that the value of alcohol depends upon elements of the choice context, such that increasing constraints on alternatives (e.g., price) or increasing the benefits of alcohol (e.g., social context) may result in greater likelihood of heavy drinking. The P3 event‐related potential elicited by alcohol‐related cues, a proposed marker of incentive salience, may be an electrophysiological parallel for behavioral economic alcohol demand. However, these indices have not been connected in prior research, and studies typically do not disaggregate social influences in the context of alcohol cue reactivity. Method: The current study recruited heavy drinking young adults (N = 81) who completed measures of alcohol use and alcohol demand, in addition to a 2 (social/nonsocial) × 2 (alcohol/nonalcohol) visual oddball task to elicit the P3. Results: In multilevel models controlling for demographic characteristics, P3 reactivity was greater to alcohol (p < 0.001) and social (p < 0.001) cues than to nonalcohol and nonsocial cues, but without a significant interaction. Higher alcohol consumption (p = 0.02) and lower elasticity of demand (p = 0.01) were associated with greater P3 response to alcohol than nonalcohol cues. Conclusions: The results highlight a brain‐behavior connection that may be an important marker for alcohol reward across units of analysis and may be sensitive to changes in the economic choice contexts that influence the likelihood of alcohol use. Objectives Age deficits in memory are widespread, this affects individuals at a personal level, and investigating memory has been a key focus in cognitive aging research. Age deficits occur in memory for an episode, where information from the environment is integrated through the senses into an episodic event via associative memory. Associating items in memory has been shown to be particularly difficult for older adults but can often be alleviated by providing support from the external environment. The current investigation explored the potential for increased sensory input (multimodal stimuli) to alleviate age deficits in associative memory. Here, we present compelling evidence, supported by Bayesian analysis, for a null age-by-modality interaction. Methods Across three preregistered studies, young and older adults (n  = 860) completed associative memory tasks either in single modalities or in multimodal formats. Study 1 used either visual text (unimodal) or video introductions (multimodal) to test memory for name-face associations. Studies 2 and 3 tested memory for paired associates. Study 2 used unimodal visual presentation or cross-modal visual-auditory word pairs in a cued recall paradigm. Study 3 presented word pairs as visual only, auditory only, or audiovisual and tested memory separately for items (individual words) or associations (word pairings). Results Typical age deficits in associative memory emerged, but these were not alleviated by multimodal presentation. Discussion The lack of multimodal support for associative memory indicates that perceptual manipulations are less effective than other forms of environmental support at alleviating age deficits in associative memory. Background: Racial/ethnic disparities in maternal and infant morbidity and mortality (MIM&M) is a wicked problem that is reinforced and perpetuated by our system[s] of care. Life Course Theory (LCT) helps to explain drivers of health disparities, but its application is challenged. An upstream approach that promotes systemic change requires the implementation of an expanded prevention framework that includes primordial and quaternary prevention. Research Design: We developed an innovative expanded Whole System Prevention Framework (WSPF) that incorporates LCT, prevention (including primordial and quaternary prevention) and systems thinking. Study Sample: We implemented this new conceptual Framework with two Healthy Start community partnerships through training, service mapping, and strategic planning to address upstream drivers of MIM&M. Data Collection and Analysis: Service mapping revealed few Healthy Start upstream activities/services with the predominance being delivered downstream at the program (microsystem) level. Results: Service mapping provided a snapshot of the current service distribution of services across the systems. The preponderance were primary, secondary and tertiary prevention activities (75.5% and 65.6%) delivered at the program level (58.2% and 68%), revealing opportunities for upstream strategies to promote equity. The implementation process provided a new way to frame strategic planning and develop upstream strategies to promote health equity and reduce MIM&M. The FABC recommends the exercise of dialogue in three different areas of life in Asia: (i) dialogue with the Asian poor, (ii) intercultural dialogue and (iii) interreligious dialogue. Interreligious dialogue, which is the subject of our discussion, is classified into four types: Dialogue of life, Dialogue of action, Dialogue of theological exchange and Dialogue of religious experience. Employing the analytical and critical methodology, this paper aims at investigating various paradigms of interreligious dialogue that have been practiced down through the centuries, in view of developing a most feasible paradigm, based on the writings of Hans-Georg Gadamer. Accordingly, we have critically analysed the following models: (i) the ideal of both/and (Hegel), and (ii) the ideal of either/or (Kierkegaard), (iii) the metaphor of two books (Bonaventure), (iv) the intercultural hermeneutics (Schleiermacher and Gadamer), and (v) the fusion of horizons (Heidegger and Gadamer). Finally, our discussions boil down to Hans-Georg Gadamer’s understanding of “fusion of horizons” and its implications in interreligious dialogue. According to Gadamer, “to acquire a horizon means that one learns to look beyond what is close at hand.” A genuine fusion of horizons through dialogue always involves rising to a higher universality that overcomes our own particularity and that of the other. A fusion of horizons thus envisages respect for the differences of different religions. Here, one does not try to acquire mastery over the other and other religions based on our pre-judgments, rather strive to partake in the other and share the other’s alterity. In order to foster interreligious dialogue as “fusion of horizons, we have to shun “ethnocentrism,” and cultivate certain essential qualities like (i) openness to change, (ii) interreligious consciousness, and (iii) empathetic understanding. To conclude, the Pontifical Council for Interreligious Dialogue, in “Dialogue and Proclamation,” advocates the people living in religious pluralistic societies to engage in meaningful dialogue for “the integral development and liberation of all people.” [ABSTRACT FROM AUTHOR]

Previous studies have suggested that deafness could lead to deficits in motor skills and other body-related abilities. However, the literature regarding motor skills in deaf adults is scarce and existing studies often included participants with heterogeneous language backgrounds and deafness etiologies, thus making it difficult to delineate the effects of deafness. In this study, we investigated motor learning in deaf native signers and hearing nonsigners. To isolate the effects of deafness and those of acquiring a signed language, we additionally tested a group of hearing native signers. Two well-established paradigms of motor learning were employed, in which participants had to adapt their hand movements to a rotation of the visual feedback (Experiment 1) or to the introduction of a force field (Experiment 2). Proprioceptive estimates were assessed before and after adaptation. Like hearing nonsigners, deaf and hearing signers showed robust adaptation in both motor adaptation paradigms. No significant differences in motor adaptation and memory were observed between deaf signers and hearing nonsigners, as well as between hearing signers and hearing nonsigners. Moreover, no discernible group differences in proprioceptive accuracy were observed. Background: The integration of information technology into educational systems represents a dynamic educational paradigm that delineates modern education for effective engagement in the third millennium. However, prior to its implementation, efforts should be directed towards identifying the factors influencing its acceptance and utilization. Objectives: The present study aimed to investigate the factors influencing the acceptance of e-learning among professors and students of macro medical sciences universities in the third region based on the Davis model in the year 2023. Methods: This study employed a cross-sectional design. The research instrument consisted of a 25-item Davis questionnaire utilizing a 5-point Likert scale, which was distributed to the samples both in person and virtually following validation and reliability confirmation by experts and a pre-test. The study population comprised 308 professors and 400 students from macro medical sciences universities in Macro Region 3. Data analysis utilized descriptive statistics (frequency, percentage, mean, and standard deviation) and inferential statistics (correlation, analysis of variance, and related tests), using SPSS software version 19 and LISREL software for Structural Equation Modeling.Modeling. Results: Data analysis revealed that the structural model of acceptance of electronic learning technology among the students of the University of Medical Sciences in Macro Region 3 is appropriate and applicable in the academic community. The variable of ease of use on attitude towards use demonstrated a coefficient of 0.41 and a T score of 7.16, while the variable of usefulness on decision to use showed a coefficient of 0.46 and a T-score of 6.31. Additionally, usefulness on attitude towards use displayed a coefficient of 0.49 and a T score of 5.92. Moreover, the variable of attitude towards use on decision to use had a coefficient of 0.47 and a T score of 5.65. Finally, the variable of decision to use on use exhibited a coefficient of 0.49 and a T score of 6.42. Conclusions: Conducting preliminary studies to comprehend the current landscape and elucidate the influencing factors in this regard can facilitate the implementation of related programs and pave the way for their efficacy. Moreover, it is advisable for officials and policymakers to employ technology acceptance theories to deploy new systems and technologies, thereby understanding the factors contributing to their acceptance and application, mitigating the risk of failure and resource wastage, and enhancing the quality of education and service delivery. This review delves into the most recent therapeutic approaches for pediatric chronic heart failure (HF) as proposed by the International Society for Heart and Lung Transplantation (ISHLT), which are not yet publicly available. The guideline proposes an exhaustive overview of the evolving pharmacological strategies that are transforming the management of HF in the pediatric population. The ISHLT guidelines recognize the scarcity of randomized clinical trials in children, leading to a predominance of consensus-based recommendations, designated as Level C evidence. This review article aims to shed light on the significant paradigm shifts in the proposed 2024 ISHLT guidelines for pediatric HF and their clinical ramifications for pediatric cardiology practitioners. Noteworthy advancements in the updated proposed guidelines include the endorsement of angiotensin receptor-neprilysin inhibitors (ARNIs), sodium-glucose cotransporter 2 inhibitors (SGLT2is), and soluble guanylate cyclase (sGC) stimulators for treating chronic HF with reduced ejection fraction (HFrEF) in children. These cutting-edge treatments show potential for enhancing outcomes in pediatric HFrEF. Nonetheless, the challenge persists in validating the efficacy of therapies proven in adult HFrEF for the pediatric cohort. Furthermore, the proposed ISHLT guidelines address the pharmacological management of chronic HF with preserved ejection fraction (HFpEF) in children, marking a significant step forward in pediatric HF care. Movement priming may be a useful tool to improve auditory short-term memory in children with ADHD in challenging retention conditions. What is the repercussion of the main findings? Our results could have repercussions for the management of children with ADHD at school, particularly in terms of the educational framework imposed during school time. More freedom of movement could facilitate learning for children with ADHD in the classroom. Aim: The aim was to evaluate the effect of body actions on learning process, particularly semantic memory capabilities in drug-naïve children with attention deficit hyperactivity disorder (ADHD). Method: Thirty children had to listen to a story which was repeated three times in a row and then a fourth time five minutes later. After each listen, the child was asked what she/he remembered from the story. The whole sample was split randomly into three subgroups of equal IQ (mean 102.2 ± 12.7), age (mean age 8 ± 0.6 years), sex (ratio female to male 1:5) and severity of ADHD symptoms (34.2 ± 7.4); a G1 "Freeze" subgroup, which implied listening to the story while sitting on a chair without moving; a G2 "Minimal" subgroup, which implied listening to the story while sitting on a chair but free movement was allowed; a G3 "Prescribed movement" subgroup, which implied listening to the story standing up, while copying the experimenter movements that mimicked the actions told in the story. Results: Although our sample was limited in size, interestingly, children in the G3 subgroup showed the highest short-term semantic memory retention compared to G1. In all subgroups, repetition allowed an increase in performance. Conclusions: Our exploratory findings stress the positive role of movement in children with ADHD to increase semantic memorization. Hyperactivity may counteract the deficit of memorization related to attention impairment in children with ADHD. Cough is a powerful, protective expulsive behavior that assists in maintaining respiratory health by clearing foreign material, pathogens, and mucus from the airways. Therefore, cough is critical to survival in both health and disease. Importantly, cough protects the airways and lungs from both antegrade (e.g., food, liquid, saliva) and retrograde (e.g., bile, gastric acid) aspirate contents. Aspiration is often the result of impaired swallowing (dysphagia), which allows oral and/or gastric contents to enter the lung, especially in individuals who also have cough dysfunction (dystussia). Cough hyposensitivity, downregulation, or desensitization- collectively referred to as hypotussia- is common in individuals with dysphagia, and increases the likelihood that aspirated material will reach the lung. The consequence of hypotussia with reduced airway clearance can include respiratory tract infection, chronic inflammation, and long-term damage to the lung parenchyma. Despite the clear implications for health, the problem of managing hypotussia in individuals with dysphagia is frequently overlooked. Here, we provide an overview of the current interventions and treatment approaches for hypotussic cough. We synthesize the available literature to summarize research findings that advance our understanding of these interventions, as well as current gaps in knowledge. Further, we highlight pragmatic resources to increase awareness of hypotussic cough interventions and provide support for the clinical implementation of evidence-based treatments. Health-related quality of life (HRQoL) is a comprehensive measure that evaluates an individual's well-being across physical, mental, and social dimensions. Enhancing HRQoL, particularly in individuals with chronic conditions like inflammatory bowel diseases (IBD), necessitates a holistic approach. Mindfulness, a scientifically supported strategy for managing anxiety, has shown promise in improving both physical and mental health. Its benefits may be partly explained through its effects on the gut–brain axis (GBA), a bidirectional communication link between the gastrointestinal system and the central nervous system. By exploring the interplay between mindfulness and the GBA, this study aims to uncover how these elements collectively influence HRQoL in both healthy individuals and those with IBD, offering insights into potential therapeutic pathways. A cross-sectional investigation involved 338 adults, including 50 IBD patients, utilizing validated Greek scales for Mindfulness (MAAS-15), Mediterranean Diet (14-MEDAS), and HRQoL (EQ-5D-5L). The questionnaire gathered demographic, anthropometric, and lifestyle data. Among healthy participants, EQ-5D-5L showed a moderate correlation with the MAAS-15 scale (r = 0.389, p < 0.05) and a low correlation with 14-MEDAS (r = 0.131, p < 0.05). IBD patients exhibited significantly lower mean EQ-5D-5L scores than healthy individuals (0.75 vs. 0.85, p < 0.05). MAAS-15 demonstrated a robust correlation (r = 0.414, p < 0.001) with EQ-5D-5L in IBD patients. Elevated mindfulness levels emerged as predictive factors for higher HRQoL in IBD patients (OR: 1.101, 95% CI: 1.008, 1.202, p < 0.05, compared to low mindfulness). In summary, factors influencing the GBA, including mindfulness and the Mediterranean diet, exhibit positive associations with HRQoL. Increased mindfulness levels predict better HRQoL in IBD patients, emphasizing the potential for clinical trials to validate these cross-sectional study findings. [ABSTRACT FROM AUTHOR]

In this narrative review, we evaluate the myriad applications of ChatGPT in diverse healthcare domains. We discuss its potential role in clinical decision-making, exploring how it can assist physicians by providing rapid, data-driven insights for diagnosis and treatment. We review the benefits of ChatGPT in personalized patient care, particularly in geriatric care, medication management, weight loss and nutrition, and physical activity guidance. We further delve into its potential to enhance medical research, through the analysis of large datasets, and the development of novel methodologies. In the realm of medical education, we investigate the utility of ChatGPT as an information retrieval tool and personalized learning resource for medical students and professionals. There are numerous promising applications of ChatGPT that will likely induce paradigm shifts in healthcare practice, education, and research. The use of ChatGPT may come with several benefits in areas such as clinical decision making, geriatric care, medication management, weight loss and nutrition, physical fitness, scientific research, and medical education. Nevertheless, it is important to note that issues surrounding ethics, data privacy, transparency, inaccuracy, and inadequacy persist. Clinical trials of pharmacological candidates targeting the core features of autism have largely failed. This is despite evidence linking differences in multiple neurochemical systems to brain function in autism. While this has in part been explained by the heterogeneity of the autistic population, the field has largely relied upon association studies to link brain chemistry to function. The only way to directly establish that a neurotransmitter or neuromodulator is involved in a candidate brain function is to change it and observe a shift in that function. This experimental approach dominates preclinical neuroscience, but not human studies. There is little direct experimental evidence describing how neurochemical systems modulate information processing in the living human brain. Thus, our understanding of how neurochemical differences contribute to neurodiversity is limited, impeding our ability to translate findings from animal studies into humans. Here, we introduce our 'shiftability' paradigm, an approach to bridge the translational gap in autism research. We provide an overview of the guiding principles and methodologies we use to directly test the hypothesis that neurochemical systems function differently in autistic and non‐autistic individuals.
