Abstract Paper, an inexpensive material with natural biocompatibility, non‐toxicity, and biodegradability, allows for affordable and cost‐effective substrates for unconventional advanced electronics, often called papertronics. On the other hand, polymeric elastomers have shown to be an excellent success for substrates of soft bioelectronics, providing stretchability in skin wearable technology for continuous sensing applications. Although both materials hold their unique advantageous characteristics, merging both material properties into a single electronic substrate reimagines paper‐based bioelectronics for wearable and patchable applications in biosensing, energy generation and storage, soft actuators, and more. Here, a breathable, light‐weighted, biocompatible engineered stretchable paper is reported via coaxial nonwoven microfibers for unconventional bioelectronic substrates. The stretchable papers allow intimate bioconformability without adhesive through coaxial electrospinning of a cellulose acetate polymer (sheath) and a silicone elastomer (core). The fabricated cellulose‐silicone fibers exhibit a greater percent strain than commercially available paper while retaining hydrophilicity, biocompatibility, combustibility, disposable, and other natural characteristics of paper. Moreover, the nonwoven stretchable cellulose‐silicone fibrous mat can adapt conventional printing and fabrication process for paper‐based electronics, an essential aspect of advanced bioelectronic manufacturing.

Coronavirus disease 2019 (COVID-19), is an ongoing issue in certain populations, presenting rapidly worsening pneumonia and persistent symptoms. This study aimed to test the predictability of rapid progression using radiographic scores and laboratory markers and present longitudinal changes. This retrospective study included 218 COVID-19 pneumonia patients admitted at the Chungnam National University Hospital. Rapid progression was defined as respiratory failure requiring mechanical ventilation within one week of hospitalization. Quantitative COVID (QCOVID) scores were derived from high-resolution computed tomography (CT) analyses: (1) ground glass opacity (QGGO), (2) mixed diseases (QMD), and (3) consolidation (QCON), and the sum, quantitative total lung diseases (QTLD). Laboratory data, including inflammatory markers, were obtained from electronic medical records. Rapid progression was observed in 9.6% of patients. All QCOVID scores predicted rapid progression, with QMD showing the best predictability (AUC = 0.813). In multivariate analyses, the QMD score and interleukin(IL)-6 level were important predictors for rapid progression (AUC = 0.864). With >2 months follow-up CT, remained lung lesions were observed in 21 subjects, even after several weeks of negative reverse transcription polymerase chain reaction test. AI-driven quantitative CT scores in conjugation with laboratory markers can be useful in predicting the rapid progression and monitoring of COVID-19.

Abstract Rationale The long-acting β2-agonist/long-acting muscarinic antagonist combination indacaterol/glycopyrronium (IND/GLY) elicits bronchodilation, improves symptoms, and reduces exacerbations in COPD. Magnetic resonance imaging (MRI) of the lung with hyperpolarized gas and gadolinium contrast enhancement enables assessment of whole lung functional responses to IND/GLY. Objectives The primary objective was assessment of effect of IND/GLY on global ventilated lung volume (%VV) versus placebo in COPD. Lung function, regional ventilation and perfusion in response to IND/GLY were also measured. Methods This double-blind, randomized, placebo-controlled, crossover study assessed %VV and pulmonary perfusion in patients with moderate-to-severe COPD after 8 days of once-daily IND/GLY treatment (110/50 µg) followed by 8 days of placebo, or vice versa, using inhaled hyperpolarized 3He gas and gadolinium contrast-enhanced MRI, respectively. Lung function measures including spirometry were performed for each treatment after 8 days. Measurements and main results Of 31 patients randomized, 29 completed both treatment periods. IND/GLY increased global %VV versus placebo (61.73% vs. 56.73%, respectively, least squares means treatment difference: 5.00% [90% CI 1.40 to 8.60]; P = 0.025). IND/GLY improved whole lung index of ventilation volume to perfusion volume (V/Q) ratio versus placebo; 94% (90% CI 83 to 105) versus 86% (90% CI 75 to 97; P = 0.047), respectively. IND/GLY showed a trend to improve diffusing capacity for carbon monoxide (DLCO) (+ 0.66 mL/min/mmHg; P = 0.082). Deep neural networks (DNNs) detect patterns in data and have shown versatility and strong performance in many computer vision applications. However, DNNs alone are susceptible to obvious mistakes that violate simple, common sense concepts and are limited in their ability to use explicit knowledge to guide their search and decision making. While overall DNN performance metrics may be good, these obvious errors, coupled with a lack of explainability, have prevented widespread adoption for crucial tasks such as medical image analysis. The purpose of this paper is to introduce SimpleMind, an open-source software environment for Cognitive AI focused on medical image understanding. It allows creation of a knowledge base that describes expected characteristics and relationships between image objects in an intuitive human-readable form. The knowledge base can then be applied to an input image to recognize and understand its content. SimpleMind brings thinking to DNNs by: (1) providing methods for reasoning with the knowledge base about image content, such as spatial inferencing and conditional reasoning to check DNN outputs; (2) applying process knowledge, in the form of general-purpose software agents, that are dynamically chained together to accomplish image preprocessing, DNN prediction, and result post-processing, and (3) performing automatic co-optimization of all knowledge base parameters to adapt agents to specific problems. SimpleMind enables reasoning on multiple detected objects to ensure consistency, providing cross-checking between DNN outputs. This machine reasoning improves the reliability and trustworthiness of DNNs through an interpretable model and explainable decisions. Proof-of-principle example applications are provided that demonstrate how SimpleMind supports and improves deep neural networks by embedding them within a Cognitive AI environment.

Chronic non-healing wounds challenge tissue regeneration and impair infection regulation for patients afflicted with this condition. Next generation wound care technology capable of in situ physiological surveillance which can diagnose wound parameters, treat various chronic wound symptoms, and reduce infection at the wound noninvasively with the use of a closed loop therapeutic system would provide patients with an improved standard of care and an accelerated wound repair mechanism. The indicating biomarkers specific to chronic wounds include blood pressure, temperature, oxygen, pH, lactate, glucose, interleukin-6 (IL-6), and infection status. A wound monitoring device would help decrease prolonged hospitalization, multiple doctors' visits, and the expensive lab testing associated with the diagnosis and treatment of chronic wounds. A device capable of monitoring the wound status and stimulating the healing process is highly desirable. In this review, we discuss the impaired physiological states of chronic wounds and explain the current treatment methods. Specifically, we focus on improvements in materials, platforms, fabrication methods for wearable devices, and quantitative analysis of various biomarkers vital to wound healing progress.

The goal of this study was to develop a computer-aided therapeutic response (CADrx) system for early prediction of drug treatment response for glioblastoma multiforme (GBM) brain tumors with diffusion weighted (DW) MR images. In conventional Macdonald assessment, tumor response is assessed nine weeks or more post-treatment. However, we will investigate the ability of DW-MRI to assess response earlier, at five weeks post treatment. The apparent diffusion coefficient (ADC) map, calculated from DW images, has been shown to reveal changes in the tumor’s microenvironment preceding morphologic tumor changes. ADC values in treated brain tumors could theoretically both increase due to the cell kill (and thus reduced cell density) and decrease due to inhibition of edema. In this study, we investigated the effectiveness of features that quantify changes from pre- and post-treatment tumor ADC histograms to detect treatment response. There are three parts to this study: first, tumor regions were segmented on T1w contrast enhanced images by Otsu’s thresholding method, and mapped from T1w images onto ADC images by a 3D region of interest (ROI) mapping tool using DICOM header information; second, ADC histograms of the tumor region were extracted from both pre- and five weeks post-treatment scans, and fitted by a two-component Gaussian mixture model (GMM). The GMM features as well as standard histogram-based features were extracted. Finally, supervised machine learning techniques were applied for classification of responders or non-responders. The approach was evaluated with a dataset of 85 patients with GBM under chemotherapy, in which 39 responded and 46 did not, based on tumor volume reduction. We compared adaBoost, random forest and support vector machine classification algorithms, using ten-fold cross validation, resulting in the best accuracy of 69.41% and the corresponding area under the curve (Az) of 0.70.

High-resolution imaging studies on large cohorts of patients generate vast data sets, which are infeasible to analyze through manual interpretation. In this article we describe a comprehensive architecture for computer-aided detection (CAD) and surveillance on lung nodules in CT images. Central to this architecture are the analytic components: an automated nodule detection system, nodule tracking capabilities and volume measurement, which are integrated within a data management system that includes mechanisms for receiving and archiving images, a database for storing quantitative nodule measurements and visualization, and reporting tools. We describe two studies to evaluate CAD technology within this architecture, and the potential application in large clinical trials. The fi rst study involves performance assessment of an automated nodule detection system and its ability to increase radiologist sensitivity when used to provide a second opinion. The second study investigates nodule volume measurements on CT made using a semi-automated technique and shows that volumetric analysis yields significantly different tumor response classifications than a 2D diameter approach. These studies demonstrate the potential of automated CAD tools to assist in quantitative image analysis for clinical trials.
