High-speed dynamic centrifugal compressors are widely used both in the modern internal combustion engine design and in advanced pressurized cycles and innovative plant layouts as fuel cell systems. Surge strongly limits the stable operating region of centrifugal compressors in low mass flow rate conditions especially during fast transients. Therefore, it is of great importance to investigate transient system dynamic response of compressor surge evolution and early detection of incipient compressor surge. A specific experimental investigation on compressor surge was carried out at the University of Genoa turbocharger test facility and results are presented and analysed in this paper. The activity consists of steady state and transient measurements used to characterize and identify compressor behaviour in correspondence of surge inception conditions. The frequency and time frequency data analysis have been applied on inflow pressure, anemometric and vibrational signals to identify their contents and so to be able to classify compressor operation as stable or unstable. Synchronous averages performed in the time domain have been identified as a suitable algorithmic tool to detect incipient surge conditions. Anemometric signal analysis allowed to identify intermitting phenomena in deep surge conditions may be related to the rise of a rotation stall condition. The obtained results provide original diagnostic and predictive methods to be integrated in a monitoring system capable of preventing surge and extending compressor operating range, performance and reliability to allow the integration with the other plant components.

The reduction of CO2 and, more generally, GHG (Green House Gases) emissions imposed by the European Commission (EC) and the Environmental Protection Agency (EPA) for passenger cars has driven the automotive industry to develop technological solutions to limit exhaust emissions and fuel consumption, without compromising vehicle performance and drivability. In a mid-term scenario, hybrid powertrain and Internal Combustion Engine (ICE) downsizing represent the present trend in vehicle technology to reduce fuel consumption and CO2 emissions. Concerning downsizing concept, to maintain a reasonable power level in small engines, the application of turbocharging is mandatory for both Spark Ignition (SI) and Diesel engines. Following this aspect, the possibility to recover the residual energy of the exhaust gases is becoming more and more attractive, as demonstrated by several studies around the world. One method to recover exhaust gas energy from ICEs is the adoption of turbo-compounding technology to recover sensible energy left in the exhaust gas by-passed through the waste-gate valve. In the paper, an innovative option of advanced boosting system is investigated through a bladeless micro expander, promising attractive cost-competitiveness. The numerical activity was developed on the basis of experimental data measured on a waste-gated turbocharger for downsized SI automotive engines. To this aim, mass flow rate through the by-pass valve and the turbine impeller was measured for different waste-gate settings in steady-state conditions at the turbocharger test bench of the University of Genoa. The paper shows that significant electrical power can be harvested from the waste-gate gases, up to 94 % of compressor power, contributing to fuel consumption reduction.

Abstract Background For many women finding the right contraceptive method can be challenging and consistent and correct use over a lifetime is difficult. Even remembering to take a birth control pill every day can be a challenge. The primary objective of this study was to evaluate women's experience with a weekly ethinylestradiol/norelgestromin contraceptive patch (EE/NGMN patch), given new technologies recently developed in hormonal contraception to increase women's options in avoiding daily dosing. Methods In 24 Italian sites, 207 women received the EE/NGMN patch for up to 6 cycles. At study end, overall satisfaction and preference, as well as compliance, efficacy and safety, were evaluated. Results 175 women (84.5%) completed the study. The overall satisfaction rate was 88%; convenience and once-a-week frequency of the patch were especially appreciated. At baseline, 82 women (39.4%) were using a contraceptive method, mainly oral contraceptives and barrier methods, but only 45.1% were very satisfied/satisfied; after 6 months with the patch, 86.3% of this subset was very satisfied/satisfied. Considering the method used in the 3 months before the study entry, 78.1% strongly preferred/preferred the patch, for convenience (53.9%), ease of use/simplicity (28.9%), fewer (9.2%) and less severe (2.6%) side effects. Compliance was very high: 1034/1110 cycles (93.2%) were completed with perfect compliance and the mean subject's compliance score was 90%. One on-therapy pregnancy occurred. The patch was safe and well tolerated: adverse events frequency was low, with predominantly single reports of each event. Most of them started and subsided during cycle 1. Conclusion This study demonstrated that the EE/NGMN patch is associated with high satisfaction levels and excellent compliance. At study end, the majority of women indicated that they would continue using the patch.

Abstract Background As weight gain is one of the most frequently cited reasons for not using and for discontinuing hormonal contraceptives, in an open-label, single-arm, multicentre clinical study we evaluated the effect of the ethinylestradiol/norelgestromin contraceptive patch (EVRA, Janssen-Cilag International, Belgium) on body composition using bioelectrical impedance analysis (BIA). Methods Body weight and impedance vector components (resistance (R) and reactance (Xc), at 50 kHz frequency, Akern-RJL Systems analyzer) were recorded before entry, after 1, 3 and 6 months in 182 Italian healthy women aged 29 yr (18 to 45), and with BMI 21.8 kg/m2 (16 to 31). Total body water (TBW) was estimated with a BIA regression equation. Vector BIA was performed with the RXc mean graph method and the Hotelling's T2 test for paired and unpaired data. Results After 6 months body weight increased by 0.64 kg (1.1%) and TBW increased by 0.51 L (1.7%). The pattern of impedance vector displacement indicated a small increase in soft tissue hydration (interstitial gel fluid). Body composition changes did not significantly differ among groups of previous contraceptive methods. Arterial blood pressure did not significantly change over time. Conclusion After 6 months of treatment with the ethinylestradiol/norelgestromin contraceptive patch we found a minimal, clinically not relevant, increase in body weight less than 1 kg that could be attributed to an adaptive interstitial gel hydration. This fluctuation is physiological as confirmed by the lack of any effect on blood pressure. This could be useful in increasing women's choice, acceptability and compliance of the ethinylestradiol/norelgestromin contraceptive patch.

Due to the increasingly restrictive limits of pollutant emissions, electrification of automotive engines is now mandatory. For this reason, adopting hybrid boosting systems to improve brake specific fuel consumption and time-to-boost is becoming common practice. In this paper an advanced turbocharging system is analyzed, consisting in an electrically assisted radial compressor and a traditional turbocharger. As a first step, the steady-state performance of each component was measured at the University of Genoa test rig. Subsequently another experimental campaign was carried out to evaluate the transient response of the entire turbocharging system. Two different layouts were compared: upstream and downstream. In the upstream configuration the electrically assisted compressor was placed in front of the traditional turbocharger, in the downstream configuration the e-compressor was positioned after the traditional turbocharger. The two different coupling configurations, upstream and downstream, were then modeled in 1-D simulation software following the dimensions and characteristics of the experimental line from which the exploited data originates. The models were first validated by emulating the steady-state condition and subsequently the transient response was simulated and analyzed. Secondly, the transient response of the two layouts was compared, removing the constraints imposed by the experimental activity. The practical significance of the results is outlined, with reference to the transient response of the turbocharger. The adoption of the boosting system presented here allows a fast and stable transient response. Moreover, a reduction in the engine back pressure could be achieved through an optimization of the boosting system-engine matching calculation. [ABSTRACT FROM AUTHOR]

The use of centrifugal compressors has been increasing tremendously in the last decade as they are a key component in the present energy scenario both in the modern internal combustion engine design and in advanced cycles and innovative plant layouts as fuel cell systems. Instability phenomena limit the operating range of the whole compressor system, especially during fast transients. The target is therefore to extend the minimum flow limit in order to improve the operability of each unit while avoiding compressor surge operation and guaranteeing safe operation. For this reason, it is necessary to develop a monitoring system capable of preventing surge and extending the operating range of these machines, their performance, and reliability to allow the integration with the other plant components. The experimental investigation carried out at the University of Genoa turbocharger test facility and presented in this work, consists of steady-state and transient measurements used to characterize and identify compressor behavior in correspondence of surge inception conditions to determine different techniques which could represent surge precursors. The data analysis concentrates on pressure and vibro-acoustic signals by applying different signal processing techniques in the time and frequency domain to classify compressor operation as stable or unstable. The cross correlation function and wavelet analysis have been identified as techniques to define a precursor able to detect incipient surge conditions. Through cross correlation function analysis, it has been possible to identify the presence of propagation phenomena in the system and to evaluate how these events become more significant near an unstable low-mass flow rate condition. At low mass flow rate condition, spikes of significant amplitude are well detectable in the cross correlation function indicating the rise of significant random content in the system responses associated with the rise of incipient surge condition. Additionally, the continuous wavelet transform has been applied to operational signals to show how their time-dependent spectral structure responses can highlight the rise of unstable phenomena, not easily identifiable with traditional signal processing techniques. Exploiting its features in terms of good frequency and time resolution allowed us to identify different contents in system responses regarding phenomena that take place close to surge line and were able to detect their nature in conditions very close to deep surge ones (e.g., rotating stall with its intermitting characteristic nature). Moreover, system response was studied in high frequency range and through a demodulation technique, it was found how blade passes frequency energy content change interacting with rotating stall inception, moving close to surge line. The obtained results provide an interesting diagnostic and predictive solution to detect compressor instabilities at low mass flow rate operating conditions and to prevent compressor fails. This article describes an investigation of the unsteady behavior of turbocharger turbines by one-dimensional modeling and experimental analysis. A one-dimensional model has been developed to predict the performance of a vaneless radial-inflow turbine submitted to unsteady flow conditions. Different from other approaches proposed in the literature, the turbine has been simulated by separating the effects of casing and rotor on the unsteady flow and by modeling the multiple rotor entries from the volute. This is a simple and effective way to represent the turbine volute by a network of one-dimensional pipes, in order to capture the mass storage effect due to the system volume, as well as the circumferential variation of fluid dynamic conditions along the volute, responsible for variable admittance of mass into the rotor through blade passages. The method developed is described, and the accuracy of the one-dimensional model is shown by comparing predicted results with measured data, achieved on a test rig dedicated to the investigation of automotive turbochargers. A cooperative fuel research (CFR) engine was modified and instrumented in order to control operating conditions and to measure engine parameters and in-cylinder pressure diagrams. Aiming at the comparison of different alternative fuels, an experimental procedure was defined, including cetane number (CN) evaluation and the definition of engine operating quantities in different working points, for fixed levels of compression ratio (CR) and injection advance. An investigation was made considering several blends of methyl-esters of rapeseed oil (RME) and of a mix of vegetable oils (VOME) with conventional diesel oil. The defined experimental procedure was applied to assess CN, engine brake thermal efficiency (bte) and exhaust emissions. Results show that the biodiesel content has a positive influence on soot emissions, with strong reduction, while thermal efficiency and NOXemissions are negatively affected, which can be justified taking into account fuel properties and changes in combustion process. As observed outcomes are generally in line with those presented in literature, the facility proved to be a suitable tool for basic investigations on alternative fuels to be used in specific applications. In this paper, the unsteady-state behaviour of a turbocharger wastegated turbine (IHI-RHF3) is investigated using both an experimental approach and a numerical approach. First, an experimental campaign is performed in a specialized test rig operating at the University of Genoa, for different openings of the wastegate valve and under steady flow and unsteady flow operations. An appropriate configuration of the turbine outlet circuit fitted with a separating wall is used to carry out instantaneous measurements downstream of the turbine wheel and the wastegate valve. The above data constitute the basis for the tuning and validation of a one-dimensional turbine model recently developed at the University of Naples. Preliminary model tuning is carried out on the basis of the characteristic map measured for a completely closed wastegate valve under steady flow operations. A refined one-dimensional schematization of the experimental apparatus is implemented within the commercial GT-Power® software, including the turbine, the wastegate circuit and the upstream and downstream measuring stations. In particular, the classical map-based approach is suitably corrected with a sequence of pipes that schematizes each component of the turbine (the inlet and outlet ducts, the volute and the wheel) to account for the wave propagation and storage phenomena inside the machine. A detailed one-dimensional schematization of the wastegate circuit is also implemented and independently tuned. The turbine model capability under unsteady flow conditions is tested for different wastegate openings and pulse frequencies, by applying time-dependent boundary conditions. In particular, the pressures and temperatures measured upstream and downstream are imposed at the model ends, and the instantaneous mass flow rate and the actual power are numerically evaluated. The results are compared with the experimental data, demonstrating good accuracy and showing some improvements with respect to the standard turbine modelling in the case of the mass flow rate prediction. Background As illustrated by the Montreal classification, gastroesophageal reflux disease (GERD) is much more than heartburn and patients constitute a heterogeneous group. Understanding if links exist between patients' characteristics and GERD symptoms, and classify subjects based on symptom-profile could help to better understand, diagnose, and treat GERD. The aim of this study was to identify distinct classes of GERD patients according to symptom profiles, using a specific statistical tool: Latent class analysis. Methods An observational single-visit study was conducted in 5 European countries in 7700 adults with typical symptoms. A latent class analysis was performed to identify "latent classes" and was applied to 12 indicator symptoms. Results On 7434 subjects with non-missing indicators, latent class analysis yielded 5 latent classes. Class 1 grouped the highest severity of typical GERD symptoms during day and night, more digestive and non-digestive GERD symptoms, and bad sleep quality. Class 3 represented less frequent and less severe digestive and non-digestive GERD symptoms, and better sleep quality than in class 1. In class 2, only typical GERD symptoms at night occurred. Classes 4 and 5 represented daytime and nighttime regurgitation. In class 4, heartburn was also identified and more atypical digestive symptoms. Multinomial logistic regression showed that country, age, sex, smoking, alcohol use, low-fat diet, waist circumference, recent weight gain (>5 kg), elevated triglycerides, metabolic syndrome, and medical GERD treatment had a significant effect on latent classes. Conclusion Latent class analysis classified GERD patients based on symptom profiles which related to patients' characteristics. Although further studies considering these proposed classes have to be conducted to determine the reproducibility of this classification, this new tool might contribute in better management and follow-up of patients with GERD. The increasingly restrictive limits on exhaust emissions of automotive internal combustion engines imposed in recent years are pushing OEMs to seek new solutions to improve powertrain efficiency. Despite the increase in electric and hybrid powertrains, the turbocharging technique is still one of the most adopted solution in automotive internal combustion engines to achieve good efficiency with high specific power levels. Nowadays, turbocharged downsized engines are the most common solution to lower CO2 emissions. Pulse turbocharging is the most common boosting layout in automotive applications as the best response in terms of time-to-boost and exhaust energy extraction. In a high-fractionated engine with four or more cylinders, a twin entry turbine can be adopted to maximize pulse turbocharging benefits and avoid interaction in the discharge phase of the cylinders. The disadvantages of the twin entry turbine are mainly due to the complexity of the exhaust piping line and the high amount of information required to build a rigorous and reliable matching model. This paper presents a detailed experimental characterization of a twin entry turbine with particular reference to the turbine efficiency and the swallowing capacity under different admission conditions. The steady flow experimental campaign was performed at the turbocharger test bench of the University of Genoa, in order to analyze the behavior of the twin entry turbine in full, partial and unbalanced admission. These are the conditions in which the turbine must work instantaneously during its normal operation in engine application. New records of the Endangered Catagonus wagneri (Rusconi, 1930) are reported in Campo Grande Community of peasants, northwestern Santiago del Estero, Argentina. The new records were obtained through camera traps as well as the finding of a skull and footprints. We conducted interviews confirmed the historical presence of the species. The records indicate the presence of C. wagneri in the North Biological Corridor, expanding this species' distribution in Santiago del Estero province, providing initial evidence of potential connection between northern and southern populations. This work describes the development and the application of a quasi3D method for the simulation of turbochargers for automotive applications under unsteady flow conditions. The quasi3D approach is based on the solution of conservation equations for mass, momentum, and energy for unsteady flows and applied to zero-dimensional (0D) and one-dimensional (1D) elements arbitrarily oriented in the space. The compressor is divided into different regions, each one treated numerically in a different way. For the impeller region, a relative reference system has been used, and the presence of a centrifugal force field has been introduced both in the momentum and energy conservation equations. The direction of the ports at the inlet and outlet of the impeller are used to determine the design flow angles and therefore the deviation during off-design conditions. Conversely in the vaneless diffuser, the conservation of the angular momentum of the flow stream has been imposed in the tangential direction and then combined with the solution of the momentum equation in the radial direction. The model has been validated against measurements carried out on the test bench of the University of Genoa both in diabatic and adiabatic conditions. [ABSTRACT FROM AUTHOR]

In a medium term scenario hybrid powertrain and Internal Combustion Engine (ICE) downsizing represent the actual trend in vehicle technology to reduce fuel consumption and CO 2 emission. Concerning downsizing concept, to maintain a reasonable power level in small engines, the application of turbocharging is mandatory both for spark ignited (SI) and compression ignited (CI) engines. Following this aspect, the possibility to couple an electric drive to the turbocharger (electric turbo compound) to recover the residual energy of the exhaust gases is becoming more and more attractive, as demonstrated by several studies around the world and by the current application in the F1 Championship. The present paper shows the first numerical results of a research program in collaboration between the Universities of Pisa and Genoa. This first study is focused on the evaluation of the benefits resulting from the application of an ETC (Electric Turbo Compound) to a small twin-cylinder SI engine (900 cm 3 ). Starting from the experimental maps of two turbines and one compressor, the complete model of a turbocharged engine was created using the AVL BOOST one-dimension code. The numerical activity then moves to the whole vehicle/powertrain modelling, considering three driving cycles and two different vehicle configurations, in order to verify the effectiveness of the proposed ETC solution. Results show that the adoption of ETC is not advantageous if used for a conventional turbocharger turbine, if the target is to optimize the overall efficiency in one specific operating point of the ICE, like in the case of range-extended electric vehicles. Besides, ETC can slightly improve the average overall efficiency when the ICE must provide variable power output, as in the case of conventional or hybrid vehicles. However, the major benefits coming from ETC are the boost range extension in the lowest engine rotational speed region and a possible reduction of turbo lag, which are key points in parallel-hybrid and especially in conventional vehicles. Concerning the whole vehicle/powertrain simulation, first results show that the ETC does not improve fuel economy of the smaller vehicle, especially when employed in urban cycles. Objectives: To compare real-world persistence, effectiveness and tolerability of ustekinumab versus TNF inhibitors (TNFi) in psoriatic arthritis (PsA)., Methods: One-year data from Italian subjects enrolled in the PsABio study (PsA patients receiving 1st- to 3rd-line treatment with ustekinumab or TNFi) were evaluated. Treatment persistence was analysed using Kaplan-Meier curves; hazard ratios (HR) of stopping treatment, and the corresponding 95% confidence intervals (CI), were computed through Cox regression models. Proportions of patients reaching clinical effectiveness endpoints were analysed using logistic regression, including propensity score (PS) adjustment for imbalanced baseline covariates, and non-response imputation if treatment was stopped/switched., Results: Among 222 participants with follow-up data (effectiveness set), 101 received ustekinumab and 121 TNFi. In the ustekinumab group, 74.3% continued treatment up to 12±3 months compared to 63.6% in the TNFi group. Ustekinumab showed better persistence than TNFi, overall and in specific subgroups (females, monotherapy without methotrexate, BMI <25 or >30 kg/m2, patients receiving ustekinumab as 2nd-line treatment instead of a second TNFi). Overall, the PS-adjusted HR of treatment discontinuation was 0.46 (95% CI: 0.26-0.82) for ustekinumab vs. TNFi. cDAPSA LDA/remission was achieved in 43.5% of ustekinumab and 43.6% of TNFi-treated patients, while MDA was achieved in 24.2% and 28.0% of patients, respectively. After PS adjustment, odds ratios of clinical effectiveness did not differ significantly. Both treatments showed an acceptable safety profile., Conclusions: This prospective, real-life study found a better persistence of ustekinumab than TNFi in PsA patients. At 1 year, both treatments showed similar effectiveness.

Introduction: Data about the clinical presentation and management of early and mild spondyloarthritis (SpA) are limited. Objectives: The objective of this study was to describe the baseline characteristics of disease-modifying antirheumatic drug (DMARD)-naïve patients with axial or peripheral SpA. Methods: The Spondyloarthritis Italian Registry: Evidence from a National Pathway (SIRENA) study is an ongoing, Italian, multicenter, prospective registry of patients with a first or newly confirmed diagnosis of SpA according to the Assessment of SpondyloArthritis International Society (ASAS) criteria. To be included, patients had to be naïve to conventional, targeted, and biological DMARDs for SpA. Patients were enrolled between June 2017 and June 2019 and classified into groups according to disease presentation: predominantly axial or peripheral manifestations. The study is ongoing, and patients are being followed for 2 years, with an evaluation every 6 months according to clinical practice. Differences in baseline demographics, lifestyle, and clinical characteristics between axial and peripheral SpA were evaluated. Results: In this study, 350 patients were enrolled, of which 123 (35.1%) were axial and 227 (64.9%) were peripheral patients. Patients with axial SpA were significantly younger at enrollment (median age: 44 vs. 53 years), had significantly more anxiety/depression (13 vs. 2.6%), and expressed higher disease activity compared to patients with peripheral SpA. Patients with peripheral SpA had significantly more cardiometabolic disorders (33 vs. 18.7%), skin psoriasis (65.2 vs. 21.1%), and nail psoriasis (35.5 vs. 17.1%) than patients with axial SpA. Dactylitis, enthesitis, and fibromyalgia were observed, respectively, in 17.6, 51.2, and 5.7% of patients with axial SpA and 24.3, 40, and 3.1% of patients with peripheral SpA. In both disease groups, women tended to report depression, joint tenderness, and higher disease activity more frequently than their male counterparts. At inclusion, a new diagnosis of SpA was performed in 58% of axial and 77% of peripheral patients, with a median time from symptom onset to diagnosis of 36 and 24 months, respectively. At baseline, most patients with axial SpA (77%) started a biological DMARD, while over half of the peripheral patients started a conventional DMARD. Conclusions: Based on a well-characterized clinical registry of SpA, we provided real-world insights on the clinical features of DMARD-naïve SpA patients, pointing out major differences between axial and peripheral disease in terms of clinical characteristics and treatment pattern. Background: For many women finding the right contraceptive method can be challenging and consistent and correct use over a lifetime is difficult. Even remembering to take a birth control pill every day can be a challenge. The primary objective of this study was to evaluate women's experience with a weekly ethinylestradiol/norelgestromin contraceptive patch (EE/NGMN patch), given new technologies recently developed in hormonal contraception to increase women's options in avoiding daily dosing., Methods: In 24 Italian sites, 207 women received the EE/NGMN patch for up to 6 cycles. At study end, overall satisfaction and preference, as well as compliance, efficacy and safety, were evaluated., Results: 175 women (84.5%) completed the study. The overall satisfaction rate was 88%; convenience and once-a-week frequency of the patch were especially appreciated. At baseline, 82 women (39.4%) were using a contraceptive method, mainly oral contraceptives and barrier methods, but only 45.1% were very satisfied/satisfied; after 6 months with the patch, 86.3% of this subset was very satisfied/satisfied. Considering the method used in the 3 months before the study entry, 78.1% strongly preferred/preferred the patch, for convenience (53.9%), ease of use/simplicity (28.9%), fewer (9.2%) and less severe (2.6%) side effects. Compliance was very high: 1034/1110 cycles (93.2%) were completed with perfect compliance and the mean subject's compliance score was 90%. One on-therapy pregnancy occurred. The patch was safe and well tolerated: adverse events frequency was low, with predominantly single reports of each event. Most of them started and subsided during cycle 1., Conclusion: This study demonstrated that the EE/NGMN patch is associated with high satisfaction levels and excellent compliance. At study end, the majority of women indicated that they would continue using the patch.
