24. Plasma lipidomics and fungal peptide-based community analysis identifies distinct signatures of early mortality in acute liver failure.

Abstract

Background and Aims: Acute liver failure (ALF) has high mortality predominantly due to compromised immune system and increase vulnerability to bacterial and fungal infections., Method: Plasma lipidome and fungal peptide-based-community (mycobiome) analysis were performed in Discovery cohort (40-ALF, 5-healthy) and validated in a validation cohort of 230-ALF using High-resolution-mass-spectrometry, artificial-neural-network (ANN) and machine-learning (ML)., Results: Untargeted lipidomics identified 2,013 lipids across 8 lipid-groups. 5 lipid-species (Phosphatidylcholine, PC(15:0/17:0), PC(20:1/14:1), PC(26:4/10:0), PC(32:0) and TG(4:0/10:0/23:6)) significantly differentiated ALF-NS (FC>10, p<0.05, FDR<0.01). Mycobiome (alpha/beta diversity) was significantly higher and showed 4 phyla and >20 species significantly dysregulated in ALF-NS linked with lipid-metabolism, fatty-acid-elongation in ER, and others (p<0.05). Lipid and mycobiome diversity in ALF-NS were strongly correlated(r2>0.7, p<0.05). Multi-modular correlation network showed striking associations between lipid, Fungal-peptides modules, and Clinical parameters specific to ALF-NS (p<0.05). Cryptococcus amylolentus CBS6039 and Penicillium oxalicum1142 directly correlated with Phosphatidylcholine, Triglycerides, and severity in ALF-NS(r2>0.85, p<0.05). POD-fungus and POD-lipids showed direct association with infection, necrosis, and hepatic encephalopathy (Beta>1.2, p<0.05). POD-lipid (AUC=0.969) and HR=1.99(1.02-2.04) superseded POD-fungus and severity indices for early-mortality prediction. Finally, significant increase in PC(15:0/17:0) level showed highest normalized importance and predicted early-mortality with >95% accuracy/sensitivity/specificity using ANN and ML. Interestingly, fungal surveillance protein Clec7a was significantly downregulated (>2-fold), leading to a notable rise in fungal infection-mediated choline/phosphatidylcholine and associated enzymes (FC>1.5; Kennedy cycle). This contributed to phosphatidic acid-mediated hyper-inflammation in ALF-non-survivors., Conclusion: In ALF plasma lipidome and mycobiome are dysregulated. Increase circulating phosphatidylcholine could stratify ALF predisposed to early-mortality or require emergency liver transplantation.

Abstract

The aim of the study was to investigate similarities and differences in health beliefs, experiences and educational needs by type of osteoporosis (OP), particularly in people with glucocorticoid-induced OP (GIOP) and men. A qualitative study was conducted via focus groups involving post-menopausal women with or without osteoporotic fractures, osteoporotic men and people with GIOP. Fifty-three participants were included in eight groups. A wide range of health beliefs was found for all types of OP. Osteoporosis was considered a natural consequence of ageing except in men or conversely a serious disease associated with risk of new fractures and disability. GIOP patients had heterogeneous knowledge of OP and reported fewer prevention behaviours, and their quality of life was affected by the causal illness. Men had difficulties coping with the loss of their functional abilities and felt that OP was a "women's" disease. Beliefs about treatments ranged from confidence to fear of adverse effects or doubt about efficacy in all types of OP. Participants were interested in physical activity, fall prevention and diet, and preferred group sessions. GIOP patients and men had an interest in face-to-face education. Men were also interested in brief information including via the Internet. Patients' beliefs about OP differed by type of OP. Specific populations such as men or people with GIOP need particular care owing to experiences and needs. Offering group sessions in educational interventions is of interest to allow for sharing experiences and also face-to-face education for men and GIOP patients or the Internet for men.

Abstract

Purpose: Though recommended, participation of patients with specific expertise in therapeutic education programs (TEP) is rare. This work reports the experience of a national reference centre for rare systemic diseases., Patients and Methods: Involvement of "expert patients" (EP) has been planned from the development of a TEP dedicated to systemic lupus: patients' roles and required expertise have been defined and linked to the pedagogical tools. Such patients have been recruited during individual interviews and called to participate to specific pedagogical training. EP intervention have been evaluated by questionnaire to EP and health care providers., Results: Three EP's functions have been identified: sharing experiences, giving "tips and tricks" and promoting dialogue. EP's interventions has been organised into a hierarchy (from sharing to co-animation). Among 298 patients enrolled in the TEP, 25 (8.4%) have been identified as possible EP. Eight of them (32%) benefited from a specific training of 12 hours. Among these patients, two (25%) regularly participate to the education sessions. For EP as well as for health care providers, EP's intervention seems beneficial (visual scale scores of 7.5 and 9.5, respectively)., Conclusions: Though difficult to organise, EP's intervention in TEP dedicated to rare systemic diseases seems useful and would earn to be increase., (Copyright © 2015 Société nationale française de médecine interne (SNFMI). Abstract

Asthma management requires adequate adherence to many recommendations, including therapy, monitoring of asthma control, avoidance of environmental triggers, and attending follow-up appointments. Poor adherence is common in patients with asthma and is often associated with increased health care use, morbidity, and mortality. Many determinants of poor adherence have been identified and should be addressed, but there is no clear profile of the nonadherent patient. Interventions to improve adherence therefore demand tailoring to the individual by including patient-specific education, addressing patient fears and misconceptions, monitoring adherence, and developing a shared decision process., (Copyright © 2012. Abstract

Introduction: Spouses play a major role as care givers for their partners with Parkinson's disease. This de facto part of family nursing turns out to be so demanding that they often feel isolated. While spouses may have access to financial and technical aids, but no specific psychological support is available to assist them in coping with the difficulties they have to face. Supporting and educating spouses thus appears today to be a real need., Methods: Wishing to create an appropriate support program responding to the needs and expectancies of spouses of Parkinson's disease, we conducted a study designed to measure the effects of Parkinson's disease on spouses' quality-of-life and identify the priority needs in terms of information and support. This study included the spouses of 14 patients who participated in semi-directive individual interviews and a focus group., Results: The data collected shows that spouses experience great disarray when faced with the disease. Their perception of Parkinson's disease has a strong anxiogenic effects. Caring for their spouse on a day to day basis creates a permanent atmosphere of stress with an insecure feeling generating tensions and major frustrations. Most of the spouses do not allow themselves any break and are overwhelmed with ambivalent feelings. They experience a kind of hostility towards their spouse and at the same time feel guilty for their attitude and also for their helplessness. The disease also leads to an impoverishment of the couples' social network, due to reduced autonomy and fear of other people's way of looking at them., Conclusion: Our study confirms the usefulness of organizing an educational support program for these spouses who often feel very lonely and helpless when confronted with their partner's disease.

Abstract

There is increasing number of cases of acquired immune deficiency syndrome (AIDS) and in France there have been about 100 cases. The authors report one case who died within 10 months. The patient presented a Pneumocystis carinii pneumonia, extensive peri-anal herpes, gastro-intestinal candidiasis and, in the final stage, severe diarrhoea. In the context of this case, the authors review the principal manifestations of this new clinical entity.
