Different variants of the selected-response (SR) item type have been developed for various reasons (i.e., simulating realistic situations, examining critical-thinking and/or problem-solving skills). Generally, the variants of SR item format are more complex than the traditional multiple-choice (MC) items, which may be more challenging to test takers and thus may discourage their test engagement on low-stakes assessments. Low test-taking effort has been shown to distort test scores and thereby diminish score validity. We used data collected from a large-scale assessment to investigate how variants of the SR item format may impact test properties and test engagement. Results show that the studied variants of SR item format were generally harder and more time consuming compared to the traditional MC item format, but they did not show negative impact on test-taking effort. However, item position had a dominating influence on nonresponse rates and rapid-guessing rates in a cumulative fashion, even though the effect sizes were relatively small in the studied data.

Machine learning (ML) techniques are increasingly prevalent in education, from their use in predicting student dropout to assisting in university admissions and facilitating the rise of massive open online courses (MOOCs). Given the rapid growth of these novel uses, there is a pressing need to investigate how ML techniques support long-standing education principles and goals. In this work, we shed light on this complex landscape drawing on qualitative insights from interviews with education experts. These interviews comprise in-depth evaluations of ML for education (ML4Ed) papers published in preeminent applied ML conferences over the past decade. Our central research goal is to critically examine how the stated or implied education and societal objectives of these papers are aligned with the ML problems they tackle. That is, to what extent does the technical problem formulation, objectives, approach, and interpretation of results align with the education problem at hand? We find that a cross-disciplinary gap exists and is particularly salient in two parts of the ML life cycle: the formulation of an ML problem from education goals and the translation of predictions to interventions. We use these insights to propose an extended ML life cycle, which may also apply to the use of ML in other domains. Our work joins a growing number of meta-analytical studies across education and ML research as well as critical analyses of the societal impact of ML. Specifically, it fills a gap between the prevailing technical understanding of machine learning and the perspective of education researchers working with students and in policy.

Multiparametric magnetic resonance imaging (mpMRI) is an emerging standard for diagnosing and prognosing prostate cancer, but ~ 20% of clinically significant tumors are invisible to mpMRI, as defined by the Prostate Imaging Reporting and Data System version 2 (PI-RADSv2) score of one or two. To understand the biological underpinnings of tumor visibility on mpMRI, we examined the proteomes of forty clinically significant tumors (i.e., International Society of Urological Pathology (ISUP) Grade Group 2)-twenty mpMRI-visible and twenty mpMRI-invisible, with matched histologically normal prostate. Normal prostate tissue was indistinguishable between patients with visible and invisible tumors, and invisible tumors closely resembled the normal prostate. These data indicate that mpMRI-visibility arises when tumor evolution leads to large-magnitude proteomic divergences from histologically normal prostate.

Prostate cancer is the second most frequently diagnosed non-skin cancer in men worldwide. Patient outcomes are remarkably heterogeneous and the best existing clinical prognostic tools such as International Society of Urological Pathology Grade Group, pretreatment serum PSA concentration and T-category, do not accurately predict disease outcome for individual patients. Thus, patients newly diagnosed with prostate cancer are often overtreated or undertreated, reducing quality of life and increasing disease-specific mortality. Biomarkers that can improve the risk stratification of these patients are, therefore, urgently needed. The ideal biomarker in this setting will be non-invasive and affordable, enabling longitudinal evaluation of disease status. Prostatic secretions, urine and blood can be sources of biomarker discovery, validation and clinical implementation, and mass spectrometry can be used to detect and quantify proteins in these fluids. Protein biomarkers currently in use for diagnosis, prognosis and relapse-monitoring of localized prostate cancer in fluids remain centred around PSA and its variants, and opportunities exist for clinically validating novel and complimentary candidate protein biomarkers and deploying them into the clinic.

BackgroundPatients with human papillomavirus-related oropharyngeal cancers have excellent outcomes but experience clinically significant toxicities when treated with standard chemoradiotherapy (70 Gy). We hypothesized that functional imaging could identify patients who could be safely deescalated to 30 Gy of radiotherapy.MethodsIn 19 patients, pre- and intratreatment dynamic fluorine-18-labeled fluoromisonidazole positron emission tomography (PET) was used to assess tumor hypoxia. Patients without hypoxia at baseline or intratreatment received 30 Gy; patients with persistent hypoxia received 70 Gy. Neck dissection was performed at 4 months in deescalated patients to assess pathologic response. Magnetic resonance imaging (weekly), circulating plasma cell-free DNA, RNA-sequencing, and whole-genome sequencing (WGS) were performed to identify potential molecular determinants of response. Samples from an independent prospective study were obtained to reproduce molecular findings. All statistical tests were 2-sided.ResultsFifteen of 19 patients had no hypoxia on baseline PET or resolution on intratreatment PET and were deescalated to 30 Gy. Of these 15 patients, 11 had a pathologic complete response. Two-year locoregional control and overall survival were 94.4% (95% confidence interval = 84.4% to 100%) and 94.7% (95% confidence interval = 85.2% to 100%), respectively. No acute grade 3 radiation-related toxicities were observed. Microenvironmental features on serial imaging correlated better with pathologic response than tumor burden metrics or circulating plasma cell-free DNA. A WGS-based DNA repair defect was associated with response (P = .02) and was reproduced in an independent cohort (P = .03).ConclusionsDeescalation of radiotherapy to 30 Gy on the basis of intratreatment hypoxia imaging was feasible, safe, and associated with minimal toxicity. A DNA repair defect identified by WGS was predictive of response. Intratherapy personalization of chemoradiotherapy may facilitate marked deescalation of radiotherapy.

Whole-genome sequencing can be used to estimate subclonal populations in tumours and this intra-tumoural heterogeneity is linked to clinical outcomes. Many algorithms have been developed for subclonal reconstruction, but their variabilities and consistencies are largely unknown. We evaluate sixteen pipelines for reconstructing the evolutionary histories of 293 localized prostate cancers from single samples, and eighteen pipelines for the reconstruction of 10 tumours with multi-region sampling. We show that predictions of subclonal architecture and timing of somatic mutations vary extensively across pipelines. Pipelines show consistent types of biases, with those incorporating SomaticSniper and Battenberg preferentially predicting homogenous cancer cell populations and those using MuTect tending to predict multiple populations of cancer cells. Subclonal reconstructions using multi-region sampling confirm that single-sample reconstructions systematically underestimate intra-tumoural heterogeneity, predicting on average fewer than half of the cancer cell populations identified by multi-region sequencing. Overall, these biases suggest caution in interpreting specific architectures and subclonal variants.

The long-term impact of algorithmic decision making is shaped by the dynamics between the deployed decision rule and individuals' response. Focusing on settings where each individual desires a positive classification-including many important applications such as hiring and school admissions, we study a dynamic learning setting where individuals invest in a positive outcome based on their group's expected gain and the decision rule is updated to maximize institutional benefit. By characterizing the equilibria of these dynamics, we show that natural challenges to desirable long-term outcomes arise due to heterogeneity across groups and the lack of realizability. We consider two interventions, decoupling the decision rule by group and subsidizing the cost of investment. We show that decoupling achieves optimal outcomes in the realizable case but has discrepant effects that may depend on the initial conditions otherwise. In contrast, subsidizing the cost of investment is shown to create better equilibria for the disadvantaged group even in the absence of realizability.

Tumor DNA sequencing data can be interpreted by computational methods that analyze genomic heterogeneity to infer evolutionary dynamics. A growing number of studies have used these approaches to link cancer evolution with clinical progression and response to therapy. Although the inference of tumor phylogenies is rapidly becoming standard practice in cancer genome analyses, standards for evaluating them are lacking. To address this need, we systematically assess methods for reconstructing tumor subclonality. First, we elucidate the main algorithmic problems in subclonal reconstruction and develop quantitative metrics for evaluating them. Then we simulate realistic tumor genomes that harbor all known clonal and subclonal mutation types and processes. Finally, we benchmark 580 tumor reconstructions, varying tumor read depth, tumor type and somatic variant detection. Our analysis provides a baseline for the establishment of gold-standard methods to analyze tumor heterogeneity.

We developed and evaluated the Digital Platform for Exercise (DPEx): a decentralized, patient-centric approach designed to enhance all aspects of clinical investigation of exercise therapy. DPEx integrated provision of a treadmill with telemedicine and remote biospecimen collection permitting all study procedures to be conducted in patient's homes. Linked health biodevices enabled high-resolution monitoring of lifestyle and physiological response. Here we describe the rationale and development of DPEx as well as feasibility evaluation in three different cohorts of patients with cancer: a phase 0a development study among three women with post-treatment primary breast cancer; a phase 0b proof-of-concept trial of neoadjuvant exercise therapy in 13 patients with untreated solid tumors; and a phase 1a level-finding trial of neoadjuvant exercise therapy in 53 men with localized prostate cancer. Collectively, our study demonstrates the utility of a fully digital, decentralized approach to conduct clinical trials of exercise therapy in a clinical population. [ABSTRACT FROM AUTHOR]

Introduction: Contrast-induced nephropathy (CIN), defined as an increase in serum creatinine (SCr) greater than 25% or ≥0.5 mg/dL within 3 days of intravenous (IV) contrast administration in the absence of an alternative cause, is the third most common cause of new acute renal failure in hospitalized patients. It is known to increase in-hospital mortality up to 27%. The purpose of this study was to investigate the rate of outpatient follow up and the occurrence of CIN in patients who presented to the emergency department (ED) and were discharged home after computed tomography (CT) of the abdomen and pelvis (AP) with IV contrast.Methods: We conducted a single center retrospective review of charts for patients who required CT of AP with IV contrast and who were discharged home. Patients’ clinical data included the presence of diabetes mellitus, hypertension, chronic kidney disease (CKD) and congestive heart failure (CHF).Results: Five hundred and thirty six patients underwent CT of AP with IV contrast in 2011 and were discharged home. Diabetes mellitus was documented in 96 patients (18%). Hypertension was present in 141 patients (26.3%), and 82 patients (15.3%) were on angiotensin-converting-enzyme inhibitors (ACEI). Five patients (0.9%) had documented CHF and all of them were taking furosemide. Seventy patients (13%) had a baseline SCr >1.2 mg/dL. One hundred fifty patients (28%) followed up in one of the clinics or the ED within one week after discharge, but only 40 patients (7.5%) had laboratory workup. Out of 40 patients who followed up within 1 week after discharge, 9 patients (22.5%) developed CIN. One hundred ninety patients (35.4%) followed up in one of the clinics or the ED after 7 days and within 1 month after discharge, but only 71 patients (13.2%) had laboratory workup completed. Out of 71 patients who followed up within 1 month, 11 patients (15%) developed CIN. The overall incidence of CIN was 15.3% (17 out of 111 patients).Conclusion: There was a poor outpatient follow up after CT of AP with IV contrast and biochemically CIN appears to be present in some patients. Unlike previous reports that CKD is the major risk factor for CIN, our results demonstrated that risk factors such as advanced age, DM and hypertension seem to predispose patients to CIN rather than abnormal baseline SCr. Relatively little is known about differences in the quality of engineering education within and across countries because of the lack of valid instruments that allow for the assessment and comparison of engineering students' skill gains. The purpose of our study is to develop and validate instruments that can be used to compare student skill gains in mathematics and physics courses in undergraduate engineering programmes across countries. The approach includes procedures to establish construct validity and other necessary psychometric properties. Drawing on data collected from over 24 engineering experts and 3600 engineering students across Russia and China, we establish that it is possible to develop valid, equitable and cross-nationally comparable instruments that can assess and compare skill gains.

What happens to philosophy when philosophical activities migrate to the AI lab? My article explores the philosophical work that has gone into the machine simulations of language and understanding after Alan Turing. The early experiments by AI practitioners such as Karen Spärck Jones, Richard Richens, Yorick Wilks, and others at the Cambridge Language Research Unit (CLRU) led to the creation of the machine interlingua, semantic networks, and other technological innovations central to the development of AI in the 1950s–1970s. I attempt to show how, in the midst of their computational work, the CLRU pioneers engaged with Ludwig Wittgenstein, Ferdinand de Saussure, Rudolf Carnap, and other philosophers and developed startling new ways of formulating fundamental questions about language and human understanding. More significantly, their philosophical activities on the machine present an inclusive and culturally diverse picture of the world that profoundly negates the ethnocentric metaphysics of human-machine conundrums that John Searle and his critics represent in the Chinese Room debate. The familiar legacy of that debate has long distorted the narrative of AI origins through its simultaneous reiteration and repudiation of the Turing test. My study seeks to clarify those origins, but my primary goal is to demonstrate what it is like to practice philosophy on the machine and how the critique of metaphysics is made possible in the AI lab. This article brings to light how AI research has benefited from post-Wittgensteinian philosophy. My research shows that Wittgenstein's work began to engage the attention of AI researchers not only in the 1970s down to the present but right from the early beginnings of computational research in the 1950s. More specifically, his later philosophy inspired a group of researchers called the Cambridge Language Research Unit (CLRU) to start one of the first programs in machine translation, information retrieval, mechanical abstracting, and knowledge representation technologies in the early 1950s, all of which have later been claimed for AI and cognitive science. I focus on the philosophical work of CLRU founder Margaret Masterman and her extraordinary but forgotten contributions to ordinary language philosophy. [ABSTRACT FROM AUTHOR]

We study two-sided matching markets in which one side of the market (the players) does not have a priori knowledge about its preferences for the other side (the arms) and is required to learn its preferences from experience. Also, we assume the players have no direct means of communication. This model extends the standard stochastic multi-armed bandit framework to a decentralized multiple player setting with competition. We introduce a new algorithm for this setting that, over a time horizon T, attains O(log(T)) stable regret when preferences of the arms over players are shared, and O(log(T)2) regret when there are no assumptions on the preferences on either side. Moreover, in the setting where a single player may deviate, we show that the algorithm is incentive compatible whenever the arms' preferences are shared, but not necessarily so when preferences are fully general. In photon-limited imaging, the pixel intensities are affected by photon count noise. Many applications require an accurate estimation of the covariance of the underlying 2-D clean images. For example, in X-ray free electron laser (XFEL) single molecule imaging, the covariance matrix of 2-D diffraction images is used to reconstruct the 3-D molecular structure. Accurate estimation of the covariance from low-photon-count images must take into account that pixel intensities are Poisson distributed, hence the classical sample covariance estimator is highly biased. Moreover, in single molecule imaging, including in-plane rotated copies of all images could further improve the accuracy of covariance estimation. In this paper we introduce an efficient and accurate algorithm for covariance matrix estimation of count noise 2-D images, including their uniform planar rotations and possibly reflections. Our procedure, steerable $e$ PCA, combines in a novel way two recently introduced innovations. The first is a methodology for principal component analysis (PCA) for Poisson distributions, and more generally, exponential family distributions, called $e$ PCA. The second is steerable PCA, a fast and accurate procedure for including all planar rotations when performing PCA. The resulting principal components are invariant to the rotation and reflection of the input images. We demonstrate the efficiency and accuracy of steerable $e$ PCA in numerical experiments involving simulated XFEL datasets and rotated face images from Yale Face Database B. [ABSTRACT FROM AUTHOR]

Coffee intake is suggested to have a positive impact on chronic diseases, yet its role in urological diseases such as erectile dysfunction (ED) remains unclear. We investigated the association of coffee intake with incidence of ED by conducting the Health Professionals Follow-Up Study, a prospective analysis of 21,403 men aged 40-75 years old. Total, regular, and decaffeinated coffee intakes were self-reported on food frequency questionnaires. ED was assessed by mean values of questionnaires in 2000, 2004 and 2008. Multivariable adjusted Cox proportional hazards models were used to compute hazard ratios for patients with incident ED (n = 7,298). No significant differences were identified for patients with incident ED after comparing highest (≥4 cups/day) with lowest (0 cups/day) categories of total (hazard ratio (HR) = 1.00, 95% confidence interval (CI): 0.90,1.11) and regular coffee intakes (HR = 1.00, 95% CI: 0.89,1.13). When comparing the highest category with lowest category of decaffeinated coffee intake, we found a 37% increased risk of ED (HR = 1.37, 95% CI: 1.08,1.73), with a significant trend (P trend = 0.02). Stratified analyses also showed an association among current smokers (P trend = 0.005). Overall, long-term coffee intake was not associated with risk of ED in a prospective cohort study. Objectives: To investigate the associations of beneficial parenting behaviours with adaptive and maladaptive offspring personality traits that persist into adulthood among individuals in the community. Methods: Families (n = 669) participating in the Children in the Community Study were interviewed during the childhood, adolescence, emerging adulthood, and adulthood of the offspring at the mean ages of 6, 14, 16, 22, and 33 years. Results: Twelve types of beneficial maternal and paternal child-rearing behaviour, reported by offspring at the mean age of 16 years, were associated with elevated offspring personality resiliency, at the mean ages of 22 and 33 years, and with low offspring personality disorder trait levels. These longitudinal associations remained significant when histories of childhood behaviour problems and parental psychiatric disorder were controlled statistically. Similar linear (that is, dose-dependent) associations were observed between the number of beneficial parenting behaviours during childhood and adaptive and maladaptive offspring traits at the mean ages of 22 and 33 years. Maternal and paternal behaviours were independently associated with both adaptive and maladaptive offspring traits. Conclusions: Beneficial maternal and paternal child-rearing behaviours may promote the development of adaptive offspring personality traits that endure into adulthood, and they may be prospectively associated with reduced levels of maladaptive offspring traits. These associations may not be attributable to childhood behaviour problems or parental psychiatric disorders, and they may be equally evident during early and middle adulthood. This essay seeks to reopen the space of interpretation between psychoanalysis and the current studies of image, media, and machine. W. J. T. Mitchell's important work on animated picture in the book What Do Pictures Want? puts a fresh demand on theorists to contemplate the pictorial uncanny. It also means that Freud's speculations about the uncanny and his rejection of Ernst Jentsch's stance on the same must be re-evaluated with respect to the place of automata in psychoanalytical studies. Mitchell has been critical of some of the central Freudian assumptions about the human psyche and, in particular, how images and texts address our visual and cognitive perception. At the same time, Mitchell often alludes to the uncanny and related psychoanalytical concepts and seems to find them indispensable to his analysis of animated picture. Are such ambivalences indicative of the fundamental difficulty of doing without Freudian insights? This essay probes Mitchell's relationship with psychoanalysis by making a double interpretative move surrounding Freud's classic essay 'Das “Unheimliche”'. It begins with the following questions: in what sense can the Freudian uncanny be said to frame Mitchell's engagement with the psychic powers of image, object, and media? Will Mitchell's work on media and animated picture help reveal some aspects of Freud's original insights that would have escaped our attention? The essay concludes by reflecting on the current developments in biocybernetics and neuroscientific experiments to explain why the pictorial uncanny must be thought in our time. Summary The majority of newly diagnosed prostate cancers are slow growing, with a long natural life history. Yet a subset can metastasize with lethal consequences. We reconstructed the phylogenies of 293 localized prostate tumors linked to clinical outcome data. Multiple subclones were detected in 59% of patients, and specific subclonal architectures associate with adverse clinicopathological features. Early tumor development is characterized by point mutations and deletions followed by later subclonal amplifications and changes in trinucleotide mutational signatures. Specific genes are selectively mutated prior to or following subclonal diversification, including MTOR , NKX3-1 , and RB1 . Patients with low-risk monoclonal tumors rarely relapse after primary therapy (7%), while those with high-risk polyclonal tumors frequently do (61%). The presence of multiple subclones in an index biopsy may be necessary, but not sufficient, for relapse of localized prostate cancer, suggesting that evolution-aware biomarkers should be studied in prospective studies of low-risk tumors suitable for active surveillance. Abstract: Background: Approximately 2% of angioedema (AE) patients have a hereditary or an acquired deficiency of the complement 1 (C1) esterase inhibitor (C1 INH) gene. Some case reports indicate an association between angiotensin-converting enzyme inhibitor (ACEI) use and exacerbation of hereditary AE (HAE). Objective: The aim of this retrospective study is to investigate the association between HAE and ACEI use in a larger patient population. Methods: A retrospective chart review of patients who presented with AE and patients with diagnostic serum assays for functional C1 INH, C1 INH antigenic protein, C1q, C1q immune complex (C1q IC), and complement 4 (C4) regardless of medical complaint. Descriptive statistics were used to analyze the data. Results: A total of 1594 patients had complement levels measured (136 C1 INH, 55 C1q, 10 C1q IC, and 1500 C4), of which 156 (9.7%) patients presented with AE. Angiotensin-converting enzyme inhibitor use was documented in 747 (47%) patients. Low C1 INH was detected in one patient with recurrent AE who was not taking ACEI. Another patient who presented with recurrent AE was found to have systemic lupus erythematosus with abnormal C4, C1q, and C1q IC, but normal C1 INH. A low C4 level was present in 94 patients, 4 of which had AE. Conclusions: The risk of exacerbating HAE by ACEI might be present, but we did not find any association in this retrospective study.
