ObjectiveTo determine the frequency of chronic joint pain and stiffness 3 years after infection with chikungunya virus (CHIKV) in a Latin American cohort.MethodsA cross-sectional followup of 120 patients from an initial cohort of 500 patients who reported joint pain 2 years after infection from the Atlántico Department, Colombia. Patients were clinically diagnosed as having CHIKV during the 2014-2015 epidemic, and baseline and followup symptoms at 40 months were evaluated in serologically confirmed cases.ResultsOf the initial 500 patients enrolled in the study, 482 had serologically confirmed chikungunya infection. From this group, 123 patients reported joint pain 20 months after infection, and 54% of those patients reported continued joint pain 40 months after infection. Therefore, 1 out of every 8 people who tested serologically positive for CHIKV infection had persistent joint pain 3 years after infection. Participants who followed up in person were predominantly adult (mean ± SD age 51 ± 14 yrs) and female (86%). The most common type of pain reported in these patients at 40 months post-infection was pain with periods of relief and subsequent reoccurrence, and over 75% reported stiffness after immobility, with 39% experiencing morning stiffness.ConclusionTo our knowledge, this is the first report to describe persistent joint pain and stiffness 40 months after viral infection. The high frequency of chronic disease highlights the need to develop prevention and treatment methods. Further studies should be conducted to understand the similarities between post-chikungunya joint pain and rheumatoid arthritis.

Low-/negative-pressure hydrocephalus (LPH/NePH) is uncommon in clinical practice, and doctors are unfamiliar with it. LPH/NePH is frequently caused by other central nervous system diseases, and patients are frequently misdiagnosed with other types of hydrocephalus, resulting in delayed treatment. LPH/NePH therapy evolved to therapeutic measures based on "external ventricular drainage below atmospheric pressure" as the number of patients with LPH/NePH described in the literature has increased. However, the mechanism of LPH/NePH formation is unknown. Thus, understanding the process of LPH/NePH development is the most important step in improving diagnosis and treatment capability. Based on case reports of LPH/NePH, we reviewed theories of transcortical pressure difference, excessive cerebral venous drainage, brain viscoelastic changes, and porous elastic sponges. A novel fluorinated diamine monomer (FPMDA ) was synthesized from p-fluorobenzaldehyde and 2, 6-dimethylaniline, and then reacted with pyromellitic anhydride to obtain a fluorinated polyimide (FPI). The structures of the synthesized FPMDA diamine monomer and polyimide were characterized by FTIR, nuclear magnetic hydrogen spectroscopy, and mass spectrometry (MS) ;the thermal properties of the polyimide film were characterized using differential scanning calorimetry (DSC) and thermogravimetric analysis (TGA). The results showed that FPI has excellent solubility in solvents such as DMF, NMP, and THF. FPI also shows good thermal stability, under nitrogen environment, with thermal decomposition temperature Td5% of 550 °C, Td10% of 580 °C, and char yield of 58.7% at 1 000 °C, its glass transition temperature is greater than 400 °C . To analyze the dynamic response of a rigid M-shaped jumper subjected to combined internal and external flows, a one-way coupled fluid–structure interaction process is applied. First, CFD simulations are conducted separately for the internal and external fluid domains. The pressure histories on the inner and outer walls are exported and loaded into the finite element model using inverse distance interpolation. Then, FEA is performed to determine the dynamic response, followed by a fatigue assessment based on the obtained stress data. The displacement, acceleration, and stress distribution along the M-shaped jumper are obtained. External flow velocity dominates the displacements, while internal flow velocity dominates the vibrations and stresses. The structural response to the combined effect of internal and external flows, plus the response to gravity alone, equals the sum of the structural responses to internal flow alone and external flow alone. Fatigue damage is calculated for the bend exhibiting the most intense vibration and higher stress levels, and the locations with significant damage correspond to areas with high maximum von Mises stress. This paper aims to evaluate multiple flow fields acting simultaneously on subsea pipelines and to identify the main factors that provide valuable information for their design, monitoring, and maintenance. Numerical simulations of fluid-structure interaction (FSI) on an elastic foil heaving with constant amplitude in freestream flow are carried out at a low Reynolds number of 20,000. The commercial software STAR-CCM+ is employed to solve the flow field and the large-scale passive deformation of the structure. The results show that introducing a certain degree of flexibility significantly improves the thrust and efficiency of the foil. For each Strouhal number St considered, an optimal flexibility exists for thrust; however, the propulsive efficiency keeps increasing with the increase in flexibility. The visualisation of the vorticity fields elucidates the improvement of the propulsive characteristics by flexibility. Furthermore, the mechanism of thrust generation is discussed by comparing the time-varying thrust coefficient and vortex structure in the wake for both rigid and elastic foils. Finally, in addition to sinusoidal motions, we also consider the effect of non-sinusoidal trajectories defined by flattening parameter S on the propulsive characteristics for both rigid and elastic foils. The non-sinusoidal trajectories defined by S=2 are associated with the maximum thrust, and the highest values of propulsive efficiency are obtained with S=0.5 among the cases considered in this work.

The cytokine profile during acute chikungunya infection that predicts future chronic arthritis has not yet been investigated. We conducted a nested case-control study comparing serum cytokine concentrations during acute chikungunya infection in cases (n = 121) that reported the presence of chronic joint pain versus age- and gender-matched controls (n = 121) who reported recovery at 20 months post infection. We observed that a robust cytokine response during acute infection was correlated with a decreased incidence of chronic joint pain and that low TNFα, IL-13, IL-2, and IL-4 during acute infection was predictive of chronic joint pain. These data suggest that a robust cytokine response is necessary for viral clearance and cytokines that are related to immune tolerance during acute infection may be protective for chronic arthritis pathogenesis.

A bulbous bow is a typical ship structure. Due to the influence of the bulbous bow, complex flow separation and gas capture phenomena may appear during the water entry of ship-like sections. In this paper, experimental and numerical studies on the water entry of a ship-like section with an obvious bulbous bow are carried out. Two thin plates are installed at both ends of the test model to ensure that the flow field during the impact process is approximately two-dimensional. The free-fall drop test is carried out in the test rig equipped with guide rails. By changing drop heights, impact pressure on the model surface with different initial impact velocities is measured. A numerical model for simulating the water entry of the ship-like section is established by using the Computational Fluid Dynamics (CFD) method, based on the Navier-Stokes equations. Reasonable time steps and mesh size are determined by convergence analysis. Four different flow models are used in the numerical analysis. It is found that the K-Epsilon turbulence model can present the most reasonable numerical prediction by comparing numerical results with the experimental data. Furthermore, the influence of the bulbous bow on the impact loads is numerically studied by using the validated numerical model. It suggests that the bulbous bow has little effect on the impact force acting on the bow-flared area but, in the position near the bulbous bow, the pressure will be affected by the second slamming and the air cushion.

Underwater gliders are winged, autonomous underwater vehicles that are broadly applied in physical and biological oceanography. The position of the wing has an important effect on the movement performance of the underwater glider. In this paper, the dynamic motion of a series of underwater glider models with different longitudinal wing positions are simulated, which provides guidance for the design of underwater gliders. The results show that when the net buoyancy is constant, the wing position affects the gliding angle, but does not affect the relationship between the gliding angle and the gliding speed. In addition, the farther the wing position of the glider is from the buoyancy centre, the longer it takes for the attitude of a glider to change, whether the wing is in front of, or behind, the buoyancy centre.

Background: SP gene family, consisting of SP100, SP110, SP140, and SP140L, has been implicated in the initiation and advancement of numerous malignancies. Nevertheless, their clinical significance in glioma remains incompletely understood. Method: Expression levels and prognostic significance of SP family members were evaluated in the TCGA and CGGA datasets. Multifactorial analysis was used to identify SP gene family members that can independently impact the prognosis of glioma patients. A SP140‐based predictive risk model/nomogram was developed in TCGA dataset and validated in CGGA dataset. The model's performance was evaluated through receiver operating characteristic (ROC) curves, calibration plots, and decision curve analyses. Phenotypic associations of SP140 and TRIM22 were examined through CancerSEA and TIMER. The effect of SP140 inhibitor in glioma progress and TRIM22/PI3K/AKT signaling pathway was confirmed in U251/U87 glioma cells. Results: The SP family members exhibited elevated expression in gliomas and were negatively correlated with prognosis. SP140 emerged as an independent prognostic factor, and a SP140‐based nomogram/predictive risk model demonstrated high accuracy. SP140 inhibitor, GSK761, lead to the suppression of TRIM22 expression and the PI3K/AKT signaling pathway. GSK761 also restrain glioma proliferation, migration, and invasion. Furthermore, SP140 and TRIM22 coexpressed in glioma cells with high level of vascular proliferation, TRIM22 is closely associated with the immune cell infiltration. Conclusion: SP140‐based nomogram proved to be a practical tool for predicting the survival of glioma patients. SP140 inhibitor could suppress glioma progress via TRIM22/PI3K/AKT signaling pathway. Basalt fiber (BF) is deemed a new environmentally friendly and high-performance fiber material due to its high strength, electrical insulation, corrosion resistance and high temperature resistance. Yet, the surface inertness restricts its practical application. In this work, the BF was irradiated and activated by electron beam, followed by in situ growth of SiO2 using a hydrothermal method, then composites with polypropylene (PP) were prepared by microinjection molding. According to the results of scanning electron microscopy (SEM) and Fourier transform infrared (FTIR), more active sites can be formed after irradiation, thus more SiO2 nanoparticles were generated on the surface of BF. Consequently, the rough surface of modified BF could provide stronger shear force during melt processing and resulted in a higher orientation of the molecular chains, increasing the lamellar thickness and generating more highly ordered β crystals in the composites. I400BF-gSiO2 exhibited the highest content of β crystals with the crystallinity of 53.62% and orientation of β (300) crystal plane of 0.91, which were 8.66% and 0.04 higher than those of the composite with pristine BF. Furthermore, due to the perfection of crystals, increased interfaces and interfacial interlocking between PP molecules and modified BF, I400BF-gSiO2 showed good overall performance, with storage modulus of 8000 MPa at −100 °C, glass transition temperature of 23.03 °C and tensile strength of 62.2 MPa, which was 1900 MPa, 1.23 °C and 29.6 MPa higher than neat PP. Hence, the surface roughing strategy proposed in this work is expected to provide some insight and promote the application of BF reinforced thermoplastic composites. [ABSTRACT FROM AUTHOR]

Severe cases of COVID‐19 present hyperinflammatory condition that can be fatal. Little is known about the role of regulatory responses in SARS‐CoV‐2 infection. In this study, we evaluated the phenotype of regulatory T cells in the blood (peripheral blood mononuclear cell) and the lungs (broncho‐alveolar) of adult patients with severe COVID‐19 under invasive mechanical ventilation. Our results show important dynamic variation on Treg cells phenotype during COVID‐19 with changes in number and functional parameters from the day of intubation (Day 1 of intensive care unit admission) to Day 7. We observed that compared with surviving patients, non‐survivors presented lower numbers of Treg cells in the blood. In addition, lung Tregs of non‐survivors also displayed higher PD1 and lower FOXP3 expressions suggesting dysfunctional phenotype. Further signs of Treg dysregulation were observed in non‐survivors such as limited production of IL‐10 in the lungs and higher production of IL‐17A in the blood and in the lungs, which were associated with increased PD1 expression. These findings were also associated with lower pulmonary levels of Treg‐stimulating factors like TNF and IL‐2. Background: Recombinant Schistosoma mansoni Tetraspanin-2 formulated on Alhydrogel (Sm-TSP-2/Alhydrogel) is being developed to prevent intestinal and hepatic disease caused by S. mansoni. The tegumentary Sm-TSP-2 antigen was selected based on its unique recognition by cytophilic antibodies in putatively immune individuals living in areas of ongoing S. mansoni transmission in Brazil, and preclinical studies in which vaccination with Sm-TSP-2 protected mice following infection challenge. Methods: A randomized, observer-blind, controlled, Phase 1b clinical trial was conducted in 60 healthy adults living in a region of Brazil with ongoing S. mansoni transmission. In each cohort of 20 participants, 16 were randomized to receive one of two formulations of Sm-TSP-2 vaccine (adjuvanted with Alhydrogel only, or with Alhydrogel plus the Toll-like receptor-4 agonist, AP 10–701), and 4 to receive Euvax B hepatitis B vaccine. Successively higher doses of antigen (10 μg, 30 μg, and 100 μg) were administered in a dose-escalation fashion, with progression to the next dose cohort being dependent upon evaluation of 7-day safety data after all participants in the preceding cohort had received their first dose of vaccine. Each participant received 3 intramuscular injections of study product at intervals of 2 months and was followed for 12 months after the third vaccination. IgG and IgG subclass antibody responses to Sm-TSP-2 were measured by qualified indirect ELISAs at pre- and post-vaccination time points through the final study visit. Results: Sm-TSP-2/Alhydrogel administered with or without AP-10-701 was well-tolerated in this population. The most common solicited adverse events were mild injection site tenderness and pain, and mild headache. No vaccine-related serious adverse events or adverse events of special interest were observed. Groups administered Sm-TSP-2/Alhydrogel with AP 10–701 had higher post-vaccination levels of antigen-specific IgG antibody. A significant dose-response relationship was seen in those administered Sm-TSP-2/Alhydrogel with AP 10–701. Peak anti-Sm-TSP-2 IgG levels were observed approximately 2 weeks following the third dose, regardless of Sm-TSP-2 formulation. IgG levels fell to low levels by Day 478 in all groups except the 100 μg with AP 10–701 group, in which 50% of subjects (4 of 8) still had IgG levels that were ≥4-fold higher than baseline. IgG subclass levels mirrored those of total IgG, with IgG1 being the predominant subclass response. Conclusions: Vaccination of adults with Sm-TSP-2/Alhydrogel in an area of ongoing S. mansoni transmission was safe, minimally reactogenic, and elicited significant IgG and IgG subclass responses against the vaccine antigen. These promising results have led to initiation of a Phase 2 clinical trial of this vaccine in an endemic region of Uganda. Trial registration: NCT03110757. New tools, such as vaccines, are needed due to the inadequacy of current control strategies. Tetraspanin-2 of S. mansoni (Sm-TSP-2) is one of the lead vaccine candidates for hepatic/intestinal schistosomiasis. Antibodies induced by this vaccine are postulated to interfere with the development of the tegument of adult S. mansoni worms, thereby impairing their development and survival. We conducted a Phase 1 trial of recombinant Sm-TSP-2 adjuvanted with Alhydrogel in 60 healthy adults living in Brazil. Each participant received three vaccinations every 2 months by intramuscular injection of the vaccine administered with or without an aqueous solution of the Toll-like receptor-4 agonist, Glucopyranosyl Lipid A (AP 10–701). Sm-TSP-2/Alhydrogel was well tolerated in schistosomiasis-exposed adults; no vaccine-related severe or serious adverse events were observed. Antigen-specific IgG antibodies were induced in a dose-dependent fashion with increasing levels observed after each vaccination. The addition of AP 10–701 to the vaccine resulted in significantly higher antibody responses. A novel luminescent Zn(II)-based metal–organic framework with twofold interpenetrating [Zn(DHT)(BPP)]n (GUPT-2) {GUPT = Guangdong University of Petrochemical Technology, H2DHT = 2,5-dihydroxyterephthalic acid, and BPP = 1,3-di(4-pyridyl)propane} was successfully synthesized using the solvothermal method, and displayed a double-interleaved 2D framework. GUPT-2 not only revealed excellent stability in water and multiple organic solvents but also retained its structural integrity at pH values in the range of 3–11. According to electronic structure calculations, electronic transitions in GUPT-2 were governed by the ligand-to-ligand charge transfer (LLCT) mechanism. GUPT-2 exhibited significant luminescence and selectively detected Fe3+ and Al3+ ions, with limit of detection (LOD) values of 0.446 and 0.269 μM, respectively. The lower LOD values for Fe3+ and Al3+ of GUPT-2 are excellent compared with those of most of reported MOF-based sensors. It is interesting to note that GUPT-2 revealed a prominent luminescence color change from faint yellow to bright green under UV irradiation at 360 nm, with an enhancement in the detection of Al3+ ions. More importantly, the fluorescence enhancement (turn on) and blue shift effects on Al3+ were obvious even in the presence of other interfering ions. These excellent fluorescence properties make GUPT-2 a rare selective luminescent probe in the detection of Fe3+ and Al3+ ions in pure water systems. [ABSTRACT FROM AUTHOR]

The bow-flared section may be simplified in the prediction of slamming loads and whipping responses of ships. However, the difference of hydrodynamic characteristics between the water entry of the simplified sections and that of the original section has not been well documented. In this study, the water entry of several different bow-flared sections was numerically investigated using the computational fluid dynamics method based on Reynolds-averaged Navier–Stokes equations. The motion of the grid around the section was realized using the overset mesh method. Reasonable grid size and time step were determined through convergence studies. The application of the numerical method in the water entry of bow-flared sections was validated by comparing the present predictions with previous numerical and experimental results. Through a comparative study on the water entry of one original section and three simplified sections, the influences of simplification of the bow-flared section on hydrodynamic characteristics, free surface evolution, pressure field, and impact force were investigated and are discussed here. A new ionic pentanuclear FeIII cluster, namely, triethylazanium tetrakis(μ2‐5‐amino‐1,2,3,4‐tetrazolido)tetrakis(μ3‐4‐chloro‐2‐{[(1H‐tetrazol‐1‐id‐5‐yl)imino]methyl}phenolato)di‐μ3‐oxido‐pentairon(III) acetonitrile monosolvate monohydrate, (C6H16N)[Fe5(C8H4ClN5O)4(CH2N5)4O2]·CH3CN·H2O, was synthesized using microvial synthesis methods and characterized by elemental analysis, FT–IR spectroscopy, single‐crystal X‐ray diffraction and thermogravimetric analysis. Magnetic studies reveal that the complex displays dominant antiferromagnetic intracluster interactions between the FeIII ions through the μ3‐oxide bridges. [ABSTRACT FROM AUTHOR]
