South Africa was an early leader in multidisciplinary studies of marine resources, particularly with the Benguela Ecology Programme in the 1980s and 1990s and catch records are available for some species dating back more than a century. Resources data have focused on trends in catches, fishing effort and changes in distribution and abundance of harvested resources, which often account for a major part of the structure and functioning of marine ecosystems. Data have been collected by means of fishery-dependent and fishery-independent survey techniques appropriate to each particular stock and analysed using traditional single species stock assessment techniques. The data are complemented by comprehensive information on the environment and top predators and have been analysed using trophodynamic models such as Ecopath with Ecosim. Future approaches include a shift to an ecosystem approach to fisheries monitoring and management, in an attempt to reconcile utilisation and biodiversity conservation objectives. Despite these scientific achievements, the single species approach to the management of most resources still persists, with only limited interactions between competing species or predators and prey being formally taken into account when modelling the stock dynamics and providing management advice to the authorities.

South Africa uniquely lies at the junction of two major currents, the Agulhas and the Benguela. The waters overlying the continental shelf exhibit exceptionally high short-, medium- and long-term (days to inter-decadal) variability compared with most other shelf areas, and strongly contrasting oceanographic conditions are observed on the east and west coasts. South Africa is rich in fisheries resources and associated environmental data collected over more than a century. The South African marine scientific community has a history of multidisciplinary studies of marine foodwebs, from the driving forces such as wind, currents and solar heating, to the top predators, with the development of kelp bed, sub-tidal reefs and estuarine ecosystem studies in the 1970s; the Benguela Ecology Programme, which ran through four successive five-year stages, focused on the pelagic marine resources. Various approaches have been used to observe the continental shelf at different time and space scales, including: macroscale but frequent satellite imagery, mesoscale environmental and fishery surveys, dedicated crossshelf transects in key areas, measurements of dynamic processes, use of moored buoys and coastal weather stations, and integrated monitoring approaches, including modelling and simulation studies. Between 30 and 50 years of comprehensive marine data now exist, which are proving useful in the application of an ecosystem approach to fisheries monitoring and management, as decadal changes become discernible. These observations need to continue; even though the single-species stock assessment and operational management procedures have not yet formally used environmental factors for fisheries management advice, they help us to understand the factors affecting fish population fluctuations and early life histories and to identify large-scale regime shifts where marine trophic structure and functioning alter to a new state.

Background: Persistent viruses such as murine cytomegalovirus (MCMV) and adenovirus-based vaccines induce strong, sustained CD8+ T-cell responses, described as memory “inflation”. These retain functionality, home to peripheral organs and are associated with a distinct transcriptional program. Methods: To further define the nature of the transcriptional mechanisms underpinning memory inflation at different sites we used single-cell RNA sequencing of tetramer-sorted cells from MCMV-infected mice, analyzing transcriptional networks in virus-specific populations in the spleen and gut intra-epithelial lymphocytes (IEL). Results: We provide a transcriptional map of T-cell memory and define a module of gene expression, which distinguishes memory inflation in spleen from resident memory T-cells (TRM) in the gut. Conclusions: These data indicate that CD8+ T-cell memory in the gut epithelium induced by persistent viruses and vaccines has a distinct quality from both conventional memory and “inflationary” memory which may be relevant to protection against mucosal infections.

Purpose of Review: To summarise the current evidence and clinical practices for patients with fragility fractures of the pelvis (FFP)., Recent Findings: FFPs are an increasingly prevalent and recognised problem in the elderly population. Recent evidence indicates they have a significant impact on function, morbidity and mortality. While traditional management of FFPs was predominantly non-surgical, surgical options have been increasingly used, with a range of surgical methods available. To date, limited consensus exists on the optimal strategy for suitable patient selection, and clinical trials in this population have proved problematic. The management of FFPs requires a multi-faceted approach to enhance patient care, including adequate pain control, minimisation of complications and optimisation of medical management. Early return to mobilisation should be a key treatment goal to maintain functional independence. The selection of patients who will maximally benefit from surgical treatment, and the most appropriate surgical strategy to employ, remains contentious., (© 2022. Pharmacy practice continues to advance, allowing professionals to contribute further to patient care and the healthcare system. Pharmacists are authorized to perform point-of-care testing (POCT) in seven out of ten Canadian provinces. In considering the potential for enhanced clinical decision-making with the opportunity to gain patient data at the site of care, the New Brunswick College of Pharmacists (NBCP) proceeded to draft regulatory amendments and a policy to enable POCT scope in New Brunswick. Policy writing is a core function of Provincial Regulatory Authorities in Canada as the process determines principles that direct pharmacy practice. Each province has a differing scope of practice and method for developing documents. This paper highlights the approach, analysis, and findings of the NBCP pursuant to drafting a POCT policy. The policy development process included a literature search and environmental scan of the ten Canadian provincial regulatory authorities along with other countries. The findings highlighted in this paper describe the use of POCT, quality assurance, regulatory framework, educational opportunities, and the role of pharmacy technicians in relation to POCT in a pharmacy setting. The approach NBCP took to engage professionals and decisions on the direction of the policy are described. As point-of-care services continue to expand in pharmacies, the insights by the NBCP can be utilized by other regulatory bodies or pharmacy professionals who are implementing or enhancing POCT policies or procedures within their organizations.

We have identified a generally effective Pd catalyst for the highly enantioselective cooperative Lewis base/Pd-catalyzed α -allylation of aryl acetic esters using electron-deficient electrophiles. Changing between aldehyde, ketone, ester, and amide substituents at the terminus of intermediate cationic π -(allyl)Pd species affects both the efficiency of the reaction and, in the case of amides, control over the stereochemistry of the product alkene, as a function of the ligand. Tris[tri(2-thienyl)phosphino]Pd(0) serves as a broadly effective catalyst and overcomes these challenges to provide a general, high-yielding, and operationally simple C(sp 3 )-C(sp 3 ) bond-forming method that gives products with high levels of enantioselectivity., Competing Interests: The authors declare no competing financial interest.

Background: Postinjury multiple organ failure (MOF) remains a significant cause of morbidity and mortality. A large number of scoring systems have been proposed to define MOF, with no criterion standard. The purpose of this study was to compare three commonly used scores: the Denver Postinjury Multiple Organ Failure Score, the Sequential Organ Failure Assessment (SOFA), and the Marshall Multiple Organ Dysfunction Score, by descriptive analysis of the populations described by each score, and their predictive ability for mortality., Methods: An observational cohort study was performed at a UK trauma center on major trauma patients requiring intensive care unit admission from 2003 to 2011. A novel trauma database was created, merging national audit data with local electronic monitoring systems. Data were collected on demographics, laboratory results, pharmacy, interventions, and hourly physiological monitoring. The primary outcome measure was mortality within 100 days from injury. Sensitivity analyses and receiver operating characteristic curves were used to assess the predictive ability of MOF scores for mortality., Results: In total, 491 patients were included in the trauma database. MOF incidence ranged from 22.8% (Denver) to 40.5% (Marshall) to 58.5% (SOFA). MOF definition did not affect timing of onset, but did alter duration and organ failure patterns. Overall mortality was 10.6%, with Denver MOF associated with the greatest increased risk of death (hazard ratio 3.87, 95% confidence interval, 2.24-6.66). No significant difference was observed in area under the receiver operating characteristic curve values between scores. Marked differences were seen in relative predictors, with Denver showing highest specificity (81%) and SOFA highest sensitivity (73%) for mortality., Conclusion: The choice of MOF scoring system affects incidence, duration, organ dysfunction patterns, and mortality prediction. We would recommend use of the Denver score since it is simplest to calculate, identifies a high-risk group of patients, and has the strongest association with early trauma mortality., Level of Evidence: Epidemiological study, level III.

Several commercially and ecologically important species in the southern Benguela have undergone southward and eastward shifts in their distributions over previous decades, most notably the small pelagic fish sardine Sardinops sagax and anchovy Engraulis encrasicolus. Understanding these changes and their implications is essential in implementing an ecosystem approach to fisheries in the southern Benguela and attempting to appreciate the potential impacts of future environmental change. To investigate possible impacts of these shifts at an ecosystem level, distribution maps for before (1985-1991), during (1997-2000) and after (2003-2008) the shift in small pelagic fish were constructed for 14 key species from catch and survey data, and used to calculate spatial indicators including proportion east and west of Cape Agulhas, relative overlap in biomass and area, index of diversity, connectivity. Potential interactions on the south and west coasts were also compared. For several species (redeye; chub mackerel; kingklip; chokka squid; yellowtail), previously unidentified increases in the proportion of biomass east of Cape Agulhas were shown to have occurred over the same period as that of small pelagic fish, although none to the same degree. On average, overlap with small pelagic fish increased over time and overall system connectivity was lowest in the intermediate period, possibly indicating a system under transition. Connectivity declined over time on the west coast while increasing on the east coast. Distributions of other species have changed over time, with the region east of Cape Agulhas becoming increasingly important in terms of potential trophic interaction. Variations in distribution of biomass and structural complexity affect the trophic structure and hence functioning of the system, and implications should be considered when attempting to identify the possible ecosystem impacts of current and future system-level change.

A good deal of scholarship examines the effects of prejudice against blacks on public opinion and vote choice in the United States. Despite producing valuable insights, this research largely ignores the attitudes of Latinos-a critical omission, since Latinos constitute a rapidly growing share of the population. Using two nationally representative survey data sets, we find that the level of racial prejudice is comparable for Latinos and non-Hispanic whites. Equally comparable are associations between prejudice and political preferences: policy opinion and support for Obama in the 2008 presidential election. Our findings suggest that despite demographic changes, efforts to enact policies intended to assist blacks and elect black candidates will continue to be undermined by prejudice. That said, Latinos are more likely than non-Hispanic whites to support policies intended to assist blacks, because Latinos are more Democratic than non-Hispanic whites, more egalitarian, and less committed to the value of limited government.

We investigated whether, in the management of stable paediatric fractures of the forearm, flexible casts that can be removed at home are as clinically effective, cost-effective and acceptable to both patient and parent as management using a cast conventionally removed in hospital. A single-centre randomised controlled trial was performed on 317 children with a mean age of 9.3 years (2 to 16). No significant differences were seen in the change in Childhood Health Assessment Questionnaire index score (p = 0.10) or EuroQol 5-Dimensions domain scores between the two groups one week after removal of the cast or the absolute scores at six months. There was a significantly lower overall median treatment cost in the group whose casts were removed at home (£150.88 (sem 1.90) vs £251.62 (sem 2.68); p < 0.001). No difference was seen in satisfaction between the two groups (p = 0.48).

Objectives: Self-criticism and reassurance are important mechanisms for regulating negative emotions but relatively little attention has been paid to interventions aimed at improving them., Design: This study explored the use of an expressive writing task to increase self-reassurance and reduce self-criticism using a randomized controlled design., Method: A total of 46 participants wrote either about life goals (the expressive writing task, n= 23) or a control topic (a review of a recent book or film, n= 23) for 15 min, three times within an hour. Measures of self-criticism/self-reassurance, stress, and positive affect were completed at baseline and at 2-week follow-up. The Linguistic Inquiry and Word Count (LIWC) was used to analyse the writing of participants in the 'life goals' condition to identify psychological processes that might differentiate those who improved and those who did not., Results: While there were no significant changes in self-reported stress or positive affect, participants writing about life goals decreased in their levels of self-criticism at 2-week follow-up relative to participants writing about control topics. Text analysis showed that experimental participants using words that imply the possibility of doubt or failure, including use of the subjunctive tense (e.g., could, would, should), were least likely to decrease their self-criticism. CONCLUSION. Expressive writing shows promise as a means by which people may decrease in their self-criticism. Future research should determine whether such experimentally induced changes in self-criticism lead to the improvements in psychological health that is implied by previous cross-sectional research., (© 2012 The British Psychological Society.)

Introduction: Patients with proximal femoral fractures present a difficult problem to health care systems in view of their complex presentations and co-morbidities. Traditionally, the focus of outcome measurement for this patient group has been on mortality and surgical implant success. Increasing recognition of the need to diversify outcome measurements has led to the creation and use of a number of outcome scales. We sought to examine how these scales are being used in the current literature., Methods: Abstracts to over 4000 papers related to proximal femoral fracture research were screened to identify commonly used scales in the five main categories of general quality of life measures (QoL), Activities of Daily Living scales (ADL), mobility and physical performance scales, disease-specific scales and hip-specific scales. The 14 identified scales were then searched for directly, and papers analysed for scale usage, timing and interpretation., Results: ADL scales were the most commonly used group, followed by QoL measures, which are validated for elderly patients. Scale timing and use varied widely between studies. A large number of scales were found in addition to the 14 identified scales. None of the 14 identified scales were validated for the proximal femoral fracture population., Discussion: A good scale must be appropriate in content, method and clinical utility. Its method of application must be reliable, responsive, and validated for the population in question. Outcome scale usage was difficult to assess in proximal femoral fracture research due to difficulties in isolating the relevant research, and in differences in scale timing and interpretation. Scale prevalence was skewed by use by specific research groups., Conclusion: There is no single unifying scale in widespread use for proximal femoral fracture patients. We would recommend the validation of commonly used scales for this population, and would advise the use of scales from more than one category to assess outcome., (Copyright © 2010 Elsevier Ltd. Background: Spinal injury in pediatric trauma is associated with significant morbidity and mortality, but no current consensus exists on the safest and most effective method of clearance in the high-risk pediatric trauma patient., Methods: A retrospective analysis was performed on the records of 115 pediatric patients who had suffered major trauma and required admission to the pediatric intensive care unit of a United Kingdom level I trauma centre during a 7-year period from January 2000 to December 2006. The spinal imaging performed, and methods of clearance for each spinal region were obtained from analysis of written and electronic medical documentation., Results: In the cohort of 115 patients, there was a male predominance (63%) with motor vehicle accidents as the major mechanism of injury (63.5%). Ten patients (8.7%) were identified with spinal injuries, all of whom had sustained closed head injuries. Two of these patients had spinal cord injuries; one subsequently died. Spinal injury resulted in longer intubation times and intensive care stays, but no difference in new injury severity score or outcome. Clearance methods ranged from clinical examination to imaging with radiographs, computed tomography (CT), and dynamic screening. Magnetic resonance imaging was used as a secondary modality in two cases only, and in neither case was it used for clearance. CT demonstrated 100% specificity and sensitivity with positive and negative predictive values of 1 for all spinal regions. There were no cases of Spinal Cord Injury WithOut Radiologic Abnormality and no evidence of missed injuries., Conclusions: There is a need for an evidence-based protocol for the clearance of the spine in the obtunded and high-risk pediatric trauma patient. High-resolution CT with sagittal and coronal reconstructions should be the basis for cervical spinal clearance, in combination with the interpretation of films by an expert radiologist. All spinal regions should be imaged, and clearance should be formally documented. The role of magnetic resonance imaging in routine clearance remains controversial. Multicenter prospective studies are needed to develop consensus for an evidenced-based protocol for clearance in this high-risk group.

Background: Dispersal plays a key role in shaping biological and ecological processes such as the distribution of spatially-structured populations or the pace and scale of invasion. Here we have studied the relationship between long-distance dispersal behaviour of a pest-controlling money spider, Erigone atra, and the distribution of maternally acquired endosymbionts within the wider meta-population. This spider persists in heterogeneous environments because of its ability to recolonise areas through active long-distance airborne dispersal using silk as a sail, in a process termed 'ballooning'., Results: We show that there is spatial heterogeneity in the prevalence of two maternally acquired endosymbiont infections within the wider E. atra meta-population and we demonstrate through several independent approaches a link between the presence of one of these endosymbionts, Rickettsia, and the tendency for long-distance movement., Conclusion: This novel finding that particular endosymbionts can influence host dispersal is of broad importance given the extremely widespread occurrence of similar bacteria within arthropod communities. A bacterial phenotype that limits dispersal has the potential not only to reduce gene flow and thus contribute to degrees of reproductive isolation within species, but also to influence species distribution and thus overall community composition.

Purpose: Laboratory studies confirm that constraint-induced movement therapy (CIMT) improves upper-extremity (UE) function after stroke. Due to strict patient criteria and the intensive resources required, CIMT has been slow to become part of rehabilitation practice. Our purpose was to determine the feasibility and effectiveness of an adapted experimental protocol within an outpatient clinical setting for a patient with moderate to severe UE impairment who did not meet traditional CIMT criteria., Patient Description: AJ, a 16-year-old male, experienced a left middle cerebral artery ischemic stroke due to carotid artery dissection one year before beginning CIMT. He demonstrated some proximal movement but no wrist or finger extension. He had received intensive rehabilitation for 12 months prior to beginning CIMT., Intervention: Two occupational therapists and two physiotherapists collaborated to provide CIMT task training for 6 hours daily for 2 weeks. A knitted mitten extending to the elbow restrained the less-involved UE during 90% of waking hours. Tasks were tailored to AJ's interests, with the goal of integrating his affected UE into his behavioural repertoire. MEASURES AND OUTCOMES: After 2 weeks of CIMT, AJ improved in all measures (grip and lateral pinch strength, Action Research Arm Test [ARAT], and Box and Block Test) except the Chedoke McMaster Impairment Inventory. Greatest gains were seen at 6 months in the ARAT and Box and Block Test, which coincided with patient and family reports of AJ's using his arm in everyday functional tasks., Implications: Shared workload, emphasis on relevant functional tasks, and complete family participation likely influenced the success of CIMT. Our findings suggest that the strict CIMT criteria used in previous studies may exclude patients who might benefit from the treatment. Controlled trials should be undertaken to examine the effects of CIMT in patients with moderate to severe UE impairment.

Interindividual variation in acclimatization to altitude suggests a genetic component, and several candidate genes have been proposed. One such candidate is a polymorphism in the angiotensin converting enzyme (ACE) gene, where the insertion (I-allele), rather than the deletion (D-allele), of a 287 base pair sequence has been associated with lower circulating and tissue ACE activity and has a greater than normal frequency among elite endurance athletes and, in a single study, among elite high altitude mountaineers. We tested the hypothesis that the I-allele is associated with successful ascent to the extreme high altitude of 8000 m. 141 mountaineers who had participated in expeditions attempting to climb an 8000-m peak completed a questionnaire and provided a buccal swab for ACE I/D genotyping. ACE genotype was determined in 139 mountaineers. ACE genotype distribution differed significantly between those who had successfully climbed beyond 8000 m and those who had not (p = 0.003), with a relative overrepresentation of the I-allele among the successful group (0.55 vs. 0.36 in successful vs. unsuccessful, respectively). The I-allele was associated with increased maximum altitudes achieved: 8079 +/- 947 m for DDs, 8107 +/- 653 m for IDs, and 8559 +/- 565 m for IIs (p = 0.007). There was no statistical difference in ACE genotype frequency between those who climbed to over 8000 m using supplementary oxygen and those who did not (p = 0.267). This study demonstrates an association between the ACE I-allele and successful ascent to over 8000 m.

The compatibility and stability of 2B combinations of diamorphine hydrochloride (5-100 mg/ml) with cyclizine lactate (5-50 mg/ml), eight combinations of diamorphine (10-100 mg/ml) with haloperidol (2-4 mg/ml) and eight combinations of all three drugs was assessed after storage in 1 ml polypropylene syringes. Samples were stored for periods up to seven days in the light and at room temperature (22 degrees C). Five combinations of diamorphine with cyclizine precipitated immediately upon preparation. After analysis and determination of t90% values (the time taken for 10% degradation). 16 of the remaining 23 combinations were judged to be compatible (no signs of crystallization or precipitation) and stable (less than 10% loss of potency of either drug) after storage for 24 h. After seven days storage only four remained compatible and stable. The results indicate that ratios of diamorphine to cyclizine of 1:1 are stable at concentrations up to 20 mg/ml. An increase in diamorphine concentration necessitates a reduction in cyclizine to 10 mg/ml, and an increase in cyclizine concentration necessitates a reduction in concentration of diamorphine to 15 mg/ml to maintain stability over 24 h. All the combinations of diamorphine with haloperidol remained compatible and stable for seven days. The addition of haloperidol (2 mg/ml) to the diamorphine and cyclizine combinations had no detrimental effect on their compatibility and stability. A stability curve is included as an easy way for palliative care personnel to avoid potential problems with incompatibilities and reduced stability when using these combinations. Furthermore, to reduce the possibility of precipitation with mixtures containing cyclizine, the use of 0.9% sodium chloride should be avoided.

The properties of gene conversion as measured in fungi that generate asci containing all the products of meiosis imply that meiotic recombination initiates at specific sites. The HIS2 gene of Saccharomyces cerevisiae displays a high frequency of gene conversion, indicating that it is a recombination hotspot. The HIS2 gene was cloned and sequenced, and the cloned DNA was used to make several different types of alterations in the yeast chromosome by transformation; these alterations were used to determine the location of the sequences necessary for the high levels of meiotic conversion observed at HIS2. Previous work indicated that the gene conversion polarity gradient is high at the 3' end of the gene, and that the promoter of the gene is not necessary for the high frequency of conversion observed. Data presented here suggest that at least some of the sequences necessary for high levels of conversion at HIS2 are located over 700 bp downstream of the end of the coding region, extend over (at least) several hundred base pairs, and may be quite complex, perhaps involving chromatin structure. Additional data indicate that multiple single base heterologies within a 1-kb interval contribute little to the frequency of gene conversion. This contrasts with other reports about the role of heterologies at the MAT locus.
