Abstract

The triumphant way of the social state in the direction of the providing of a legitimacy constituted a new social and mental reality. This is for us much relevant because the decline of the social state also produced a new reality. Like during the construction of the social state, an accumulation of the miscellaneous legitimizations goes on, it was therefore necessary to bring on the scene the same entire pallet of the delegitimization. The Third Way is the answer of the social democracy to the new situation. An answer that offers a longterm partnership, if we want, a political marriage to the neoliberalism of an economical and political world order, which is already considered as hegemonic. The liberalism appears in every point and as a personification of every value in the current discussions. The greatest problem in the present to set the liberalism identical to the monetarist economical system. Precisely the really existing socialism of the 70-s and 80-s averred as the subject against which a classical political liberalism of the human rights and a new economical liberalism could unit. The real socialism has not "misunderstood" this new situation. It has simply not recognized it. We draw further up a hypothesis about three "societies" of actual global world. The first society would be the society of the good will itself, for which the value-oriented life of this year (1989) can no more become an open problem. We define the second society as the one, which is fundamentally not constituted by the above described good will. We see the third society in that political class or elite, which power has been built on the year 1989. The "third" society is the object of the self-identification of the "first", the "first" is the product of the year 1989. The "second" society lived after 1989 rather long in the shade, its emancipation began after the first elementary shakes of the new world order.

Our aim was to clarify the mechanism of immune complex-mediated inflammation and modulation of the disease. CII pre-immunized DBA/1 mice were intravenously boosted with extravidin coupled biotinylated monomeric CII-peptide epitope (ARGLTGRPGDA) and its complexes with biotinylated FcγRII/III specific single chain Fv (scFv) fragment. Disease scores were monitored, antibody titers and cytokines were determined by ELISA, and binding of complexes was detected by flow cytometry and immune histochemistry. Cytokine and chemokine secretion was monitored by protein profiler microarray. When intravenously administered into collagen-primed DBA/1 mice, both CII-peptide and its complex with 2.4G2 scFv significantly accelerated CIA and increased the severity of the disease, whereas the monomeric peptide and monomeric 2.4G2 scFv had no effect. FcγRII/III targeted CII-peptide complexes bound to marginal zone macrophages and dendritic cells, and significantly elevated the synthesis of peptide-specific IgG2a. Furthermore, CII-peptide containing complexes augmented the in vivo secretion of cytokines, including IL-10, IL-12, IL-17, IL-23, and chemokines (CXCL13, MIP-1, MIP-2). These data indicate that complexes formed by the CII-peptide epitope aggravate CIA by inducing the secretion of chemokines and the IL-12/23 family of pro-inflammatory cytokines. Taken together, these results suggest that the in vivo emerging immune complexes formed with autoantigen(s) may trigger the IL-12/23 dependent pathways, escalating the inflammation in RA. Thus blockade of these cytokines may be beneficial to downregulate immune complex-induced inflammation in autoimmune arthritis.Keywords: collagen-induced arthritis, Fc gamma receptor, immune complex, inflammation, targeting

5. Introducing Students to Experimental Design Skills

Abstract

The results of an earlier empirical research study on modifying 'step-by-step' instructions to practical activities requiring one or more steps of the experiments to be designed by students initiated a longitudinal study to investigate the effectiveness of the approach for younger students and over a period of time. The longitudinal study that followed took the form of a four year research project that began in September 2016. Over 900 students have been involved. All were 12-13 years old in the beginning of the study. Each year they spend six lessons carrying out practical activities using worksheets we provide. This paper reports the findings of the first year, when the participating classes were allocated to one of three groups. Group 1 was the control group. Students simply followed the step-by-step instructions. Groups 2 and 3 were experimental groups. Group 2 students not only followed the same instructions, but also had to complete experimental design tasks on paper. Group 3 students followed the same instructions, but one or more steps were incomplete and students were required to design these steps. The impact of the intervention on the students' experimental design skills, disciplinary content knowledge and attitude toward chemistry is measured by structured tests. After the first school year of the project it was clear that the type of instruction only had a weak significant positive effect on the results of the Group 2 students' disciplinary content knowledge. No significant effect of the intervention could be detected on the changes in the students' grades and attitudes toward the subject, which only seemed to depend on the ranking of their schools. This paper provides the interesting details of the results of the first year (pilot) of the research and discusses changes to the approach that have been made for the remaining three years of the project.

Abstract

Summary: Children with difficult tracheal intubation are at increased risk of severe complications, including hypoxaemia and cardiac arrest. Increasing experience with the simultaneous use of videolaryngoscopy and flexible bronchoscopy (hybrid) in adults led us to hypothesise that this hybrid technique could be used safely and effectively in children under general anaesthesia. We reviewed observational data from the international Pediatric Difficult Intubation Registry from 2017 to 2021 to assess the safety and efficacy of hybrid tracheal intubation approaches in paediatric patients. In total, 140 patients who underwent 180 attempts at tracheal intubation with the hybrid technique were propensity score‐matched 4:1 with 560 patients who underwent 800 attempts with a flexible bronchoscope. In the hybrid group, first attempt success was 70% (98/140) compared with 63% (352/560) in the flexible bronchoscope group (odds ratio (95%CI) 1.4 (0.9–2.1), p = 0.1). Eventual success rates in the matched groups were 90% (126/140) for hybrid vs. 89% (499/560) for flexible bronchoscope (1.1 (0.6–2.1), p = 0.8). Complication rates were similar in both groups (15% (28 complications in 182 attempts) hybrid; 13% (102 complications in 800 attempts) flexible bronchoscope, p = 0.3). The hybrid technique was more likely than flexible bronchoscopy to be used as a rescue technique following the failure of another technique (39% (55/140) vs. 25% (138/560), 2.1 (1.4–3.2) p < 0.001). While technically challenging, the hybrid technique has success rates similar to other advanced airway techniques, few complications and may be considered an alternative technique when developing an airway plan for paediatric patients whose tracheas are difficult to intubate under general anaesthesia. Abstract

Az elmúlt évtizedben, de különösen az utóbbi években a magyar ipar is számos kihívással nézett szembe, amelyek közül az egyik legfontosabb az Ipar 4.0 technológiák terjedése. A szakirodalmi tapasztalatok szerint ezt számos tényező befolyásolja. A tanulmányban azt vizsgáljuk evolúciós gazdaságföldrajzi megközelítésben, hogy milyen összefüggések vannak az eltérő technológiai intenzitású feldolgozóipari ágazatok területi koncentrációja és az Ipar 4.0 technológiák térbeli mintázata között a negyedik ipari forradalom elején. Különböző adatbázisok és indikátorok felhasználásával, amelyek a digitális infrastruktúrára és a társadalmi, gazdasági fejlettségre vonatkoztak, korreláció- és klaszterelemzést végeztünk megyei szinten. Megállapítottuk, hogy az Ipar 4.0 technológiák alkalmazása, és ezáltal a digitalizáció elsősorban azokban a térségekben előrehaladottabb, ahol a magas- és közepesen magas technológiai intenzitású ágazatok koncentrálódnak. A 2014–2019. között időszak, amely a negyedik ipari forradalom kezdeti szakaszának is tekinthető, egyik fontos trendje, hogy a feldolgozóiparban nőtt az alacsonyabb technológiai szintű ágazatok koncentrációja, ami viszont kevésbé kedvez a digitális technológiák alkalmazásának. Az Ipar 4.0-hoz kötődő technológiák térbeli megoszlása látványosan tükrözi a digitális megosztottságot az ország északi és déli fele között, mivel a fejlettebb és jelentősebb feldolgozóipari koncentrációval rendelkező északi országrészben sokkal markánsabb az előfordulásuk.
