Aim of the study: Identification of factors which determine effective communication between a doctor and a patient. Development of a tool for the evaluation of doctors’ skills of breaking bad news to patients’ relatives. Material and methods: The study was conducted with the survey method, and covered a group of 94 doctors of medicine from three hospitals in the Małopolska region in Poland. The Breaking Bad News Skills questionnaire, developed by Wioletta Szwed-Łopata and Jakub Lickiewicz, describes a doctor’s behaviour in contact with a patient’s family. Results: Exploratory factor analysis was conducted to establish the factor structure of Breaking Bad News Skills questionnaire. The scree plot was used prior to determining the number of factors. Internal consistency was evaluated with Kaiser–Meyer–Olkin (KMO) measure, which should be above 0.6, and Bartlett’s test of sphericity. The factor analysis identified five factors referring to the most characteristic types of doctors’ behaviour. These are: Communication (COM), Emotional Barriers (EMO), Partnership (PAR), Support (SUP) as well as Empathy and Compassion (EMP). The first of the constructed factors (8 items) focuses on statements concerning a doctor’s communication with a patient (COM), while the second factor (8 items) pertains to statements connected with the emotional context of breaking bad news by the physician (EMO). Next factor (5 items) includes statements about partnership (PAR). The fourth factor (5 items) concerns the ability to build a supportive environment (SUP). The last factor (8 items) comprises statements concerning partnership, refers to building a supportive environment and the doctor’ s empathy and compassion (SUP). Conclusions: Results of the Breaking Bad News Skills tool analyses indicate that it should be recommended for further scientific research and used in many areas of psychological and social practice.

Abstract Background Recently, veno-arterial extracorporeal membrane oxygenation (VA-ECMO) has become the rewarming treatment of choice in hypothermic cardiac arrest. The detailed indications for extracorporeal rewarming in non-arrested, severely hypothermic patients with circulatory instability have not been established yet. The primary purpose of the study was a preliminary analysis of all aspects of the treatment process, as well as initial identification of mortality risk factors within the group of severely hypothermic patients, treated with arteriovenous extracorporeal membrane oxygenation (VA-ECMO). The secondary aim of the study was to evaluate efficacy of VA-ECMO in initial 6-h period of treatment Methods From July 2013 to June 2016, thirty one hypothermic patients were accepted for extracorporeal rewarming at Severe Accidental Hypothermia Center, Cracow. Thirteen patients were identified with circulatory instability and were enrolled in the study. The evaluation took into account patients’ condition on admission, the course of therapy, and changes in laboratory and hemodynamic parameters. Results Nine out of 13 analyzed patients survived (69%). Patients who died were older, had lower both systolic and diastolic pressure, and had increased creatinine an potassium levels on admission. In surviving patients, arterial blood gases parameters (pH, BE, HCO3) and lactates would normalize more quickly. Their potassium level was lower on admission as well. The values of the core temperature on admission were comparable. Although normothermia was achieved in 92% of patients, none of them had been weaned-off VA-ECMO in the first 6 h of treatment. Discussion and Conclusions In our preliminary study more pronounced markers of cardiocirculatory instability and organ hypoperfusion were observed in non-survivors. Future studies on indications to extracorporeal rewarming in severely hypothermic, non-arrested patients should focus on the extent of hemodynamic disturbances. Short term (

Celem badania było przedstawienie dylematów dotyczących diagnozy zaburzeń odżywiania się na przykładzie alternatywnego do kryteriów klinicznych podziału grupy dziewcząt z rozpoznaniem tych zaburzeń, uwzględniającego wyniki kwestionariuszy samooceny. Badaniem objęto 116 dziewcząt z rozpoznaniem któregoś z zaburzeń odżywiania się według DSM-IV, konsultowanych po raz pierwszy w latach 2002–2004 w ambulatorium Oddziału Klinicznego Psychiatrii Dzieci i Młodzieży Szpitala Uniwersyteckiego w Krakowie. Z uwagi na ograniczenia statystyczne modelu jedynie zmienne zależne, takie jak: samoocena obecności objawów depresyjnych (Kwestionariusz Depresji Becka, BDI), samoocena obecności problemów z odżywianiem się (Kwestionariusz Postaw wobec Odżywiania, EAT26), obraz siebie (Ja Społeczne Kwestionariusza Obrazu Siebie Offera, JaSpoł QSIA) i relacje rodzinne (skala Intymności Skali Rodziny Pochodzenia, INT FOS), zostały poddane analizie skupień metodą k-średnich. Przeprowadzone analizy doprowadziły do wyłonienia pięciu skupień niepokrywających się z podziałem na poszczególne diagnozy kliniczne. Wszystkie skupienia różnią się wynikami tworzących je skal kwestionariuszowych. Pierwsze skupienie okazało się charakteryzować osoby o niskim w swojej ocenie nasileniu występowania problemów z odżywianiem się i depresyjności oraz korzystnym obrazie swojego funkcjonowania społecznego i relacji rodzinnych. Zależność obserwowana w skupieniu 5. była odwrotna. Przeważająca liczba pacjentek w skupieniu 1. miała rozpoznaną anoreksję restrykcyjną, a w skupieniu 5. bulimię. Najbardziej odmienne od pozostałych okazało się skupienie 3. Pozytywnemu obrazowi siebie, relacji rodzinnych w badanych obszarach towarzyszyło duże nasilenie występowania problemów z odżywianiem się i niskie depresyjności w samoocenie. Otrzymane wyniki wskazują, iż podział kliniczny zaburzeń odżywania się proponowany w ICD i DSM nie jest jedynym możliwym. Inne sposoby porządkowania pacjentów nie tylko mogą być interesujące poznawczo, ale też mają pewną wartość kliniczną.
