BACKGROUND/OBJECTIVES: Occupational psychosocial stress has been identified as a risk factor for obesity, whereas dietary habits have a key role in weight control. We examined whether dietary habits modify the association between occupational psychosocial factors and waist circumference. SUBJECTS/METHODS: Data comprised 31-year-old men (n = 2222) and women (n = 2053) in the Northern Finland Birth Cohort 1966. Waist circumference was measured and data on occupational psychosocial factors (demands, control and social support) and other characteristics were obtained through questionnaires. Healthy and unhealthy diet indices were constructed according to the current dietary guidelines. Associations were examined using analysis of variance adjusted for body mass index at age 14, basic education level, leisure-time physical activity, alcohol consumption, smoking, stress-related eating behaviour and parity. RESULTS: Among men, high job demands and high job control were associated with greater waist circumferences, and there were interactions between unhealthy diet and job demands (P = 0.043) and job control (P = 0.036) in relation to waist circumference. The waist of men with high demands or high control and low consumption of unhealthy foods (red/processed meat, hamburgers and pizzas, fried potatoes, sugar-sweetened soft drinks and white bread) was smaller than that of men with high demands or high control and high consumption of such foods. No associations were found among women. CONCLUSIONS: A diet based on the current dietary guidelines seems to cancel out the adverse effects of occupational psychosocial factors on waist circumference among young men. Longitudinal studies are needed to assess the risks for obesity-related diseases arising from psychosocial work environments and dietary habits. We utilized citizen scientist photographs of subauroral emissions in the upper atmosphere and identified a repeatable sequence of proton aurora and subauroral red (SAR) arc during substorms. The sequence started with a pair of green diffuse emissions and a red arc that drifted equatorward during the substorm expansion phase. Simultaneous spectrograph and satellite observations showed that they were subauroral proton aurora, where ion precipitation created secondary electrons that illuminated aurora in green and red colors. The ray structures in the red arc also indicated existence of low‐energy electron precipitation. The green diffuse aurora then decayed but the red arc (SAR arc) continued to move equatorward during the substorm recovery phase. This sequence suggests that the SAR arc was first generated by secondary electrons associated with ion precipitation and may then transition to heat flux or Joule heating. Proton aurora provides observational evidence that ion injection to the inner magnetosphere is the energy source for the initiation of the SAR arc. Plain Language Summary: Photographs that were taken by citizen scientists at Strathmore (Canada), Orimattila (Finland) and Ikaalinen (Finland) revealed peculiar green bands and blobs and red arcs in the night sky equatorward of the auroral oval. Those are different from STEVE (Strong Thermal Emission Velocity Enhancement) due to the lack of the purple/mauve arc and green picket fence. Using concurrent scientific instruments, we identified that those are a type of proton aurora that is illuminated by electrons induced by proton precipitation from space. The proton aurora transitions to a stable auroral red (SAR) arc. A series of photographs suggests that the SAR arc is initiated by proton precipitation, which has been suggested but never been demonstrated previously. The citizen scientist photographs played a critical role in unveiling the interaction between the proton aurora and SAR arc. We present an overview of the modelling of particle number concentrations (PNCs) in five major European cities, namely Helsinki, Oslo, London, Rotterdam, and Athens, in 2008. Novel emission inventories of particle numbers have been compiled both on urban and European scales. We used atmospheric dispersion modelling for PNCs in the five target cities and on a European scale, and evaluated the predicted results against available measured concentrations. In all the target cities, the concentrations of particle numbers (PNs) were mostly influenced by the emissions originating from local vehicular traffic. The influence of shipping and harbours was also significant for Helsinki, Oslo, Rotterdam, and Athens, but not for London. The influence of the aviation emissions in Athens was also notable. The regional background concentrations were clearly lower than the contributions originating from urban sources in Helsinki, Oslo, and Athens. The regional background was also lower than urban contributions in traffic environments in London, but higher or approximately equal to urban contributions in Rotterdam. It was numerically evaluated that the influence of coagulation and dry deposition on the predicted PNCs was substantial for the urban background in Oslo. The predicted and measured annual average PNCs in four cities agreed within approximately ⩽26% (measured as fractional biases), except for one traffic station in London. This study indicates that it is feasible to model PNCs in major cities within a reasonable accuracy, although major challenges remain in the evaluation of both the emissions and atmospheric transformation of PNCs. The predictions of two road dust suspension emission models were compared with the on-site mobile measurements of suspension emission factors. Such a quantitative comparison has not previously been reported in the reviewed literature. The models used were the Nordic collaboration model NORTRIP (NOn-exhaust Road TRaffic Induced Particle emissions) and the Swedish-Finnish FORE model (Forecasting Of Road dust Emissions). These models describe particulate matter generated by the wear of road surface due to traction control methods and processes that control the suspension of road dust particles into the air. An experimental measurement campaign was conducted using a mobile laboratory called SNIFFER, along two selected road segments in central Helsinki in 2007 and 2008. The suspended PM10 concentration was measured behind the left rear tyre and the street background PM10 concentration in front of the van. Both models reproduced the measured seasonal variation of suspension emission factors fairly well during both years at both measurement sites. However, both models substantially under-predicted the measured emission values. The article illustrates the challenges in conducting road suspension measurements in densely trafficked urban conditions, and the numerous requirements for input data that are needed for accurately applying road suspension emission models. Ospemifene is a tissue-selective estrogen agonist/ antagonist that was recently approved for the treatment of dyspareunia associated with vulvar and vaginal atrophy, which occurs in up to approximately 50 % of postmenopausal women. The current analyses were conducted to determine whether ospemifene exhibits estrogenic activity in the mammary glands of ovariectomized rats and to compare potential estrogenic activity with selective estrogen receptor modulators (tamoxifen, raloxifene, and toremifene). Three separate studies with differing durations (6, 9, and 28 days) were conducted using similar procedures in ovariectomized Sprague-Dawley rats. Estradiol treatment and sham-treated ovariectomized rats were used as positive and negative controls, respectively. Cell proliferation was examined using labeled 5-bromo-2-deoxyuridine; cytoplasmic prolactin was characterized with antibody staining. The morphology of the mammary gland was studied by histological staining of sections from the right fourth mammary glands, and the excised gland from the left side was used for counting the lobulus number. Neither ospemifene nor selective estrogen receptor modulators substantially induced 5-bromo-2-deoxyuridine staining, altered the morphology of the mammary glands, or changed prolactin immunostaining in ovariectomized rats compared with the ovariectomized controls. With the exception of toremifene, the selective estrogen receptor modulators did not cause a substantial induction in mammary gland lobuli. Estradiol had effects opposite to those of the selective estrogen receptor modulators in these studies. Ospemifene exhibited no substantial estrogenic activity in the mammary gland of ovariectomized rats. Ospemifene is a non-estrogen agent that exerts tissue-specific estrogen agonistic and weak antagonistic effects (i. e., is a selective estrogen receptor modulator [SERM]). The effects of various once-daily oral doses of ospemifene on bone are examined across 3 studies for 4 or 52 weeks after surgery in the ovariectomized (OVX) rat model of postmenopausal bone loss. Ospemifene treatment reduced the loss of bone mineral content and density observed in untreated OVX rats, significantly increased distal femur bone mineral content at 51 weeks at 25 mg/kg dose compared with untreated OVX rats (p < 0.01), and significantly increased trabecular bone mineral density of the distal femur and proximal tibia with 1, 5, or 25 mg/kg doses after 52 weeks. Ospemifene 5 and 25 mg/kg preserved distal femur trabecular structure; trabecular number was significantly increased, whereas trabecular separation and eroded surface values were significantly decreased (all p < 0.01). Structural changes associated with ospemifene were accompanied by increased mechanical strength of femurs and 4 th lumbar vertebra compared with untreated OVX rats. Ospemifene 10 mg/kg prevented OVX-induced bone loss; trabecular bone volume of distal femurs was increased after 4 weeks. Further, histomorphometric measures revealed decreased bone resorption after 4 weeks of ospemifene treatment, with effects similar to other SERMs (raloxifene and droloxifene). Ospemifene 3 and 10 mg/kg significantly inhibited OVX-induced increases in osteoclast number, and doses ⩾ 0.3 mg/kg dosedependently reversed the OVX-induced increase in the double-labeled volume:bone volume ratio. These results demonstrate antiresorptive, selective agonist effects of ospemifene on bone that appear similar to raloxifene in this in vivo animal model of estrogen deficiency. STUDY DESIGN: Cohort study with 5-, 10-, and 28-year follow-up. OBJECTIVES: To examine associations between baseline serum lipid concentrations and later low back pain (LBP). SUMMARY OF BACKGROUND DATA: Atherosclerosis of the lumbar vessels has been suggested as a mechanism leading to disc degeneration and LBP. Cholesterol is considered essential for atherosclerosis development. METHODS: A sample (n = 902) of employees in an engineering company was examined for serum total cholesterol and triglycerides, body mass index (BMI), smoking, exercise, work history, and LBP in 1973. By November 2000, 232 subjects had died. In 1978, 748 (84% of the survivors), in 1983, 654 (76%), and in 2000, 546 (81%) responded to a follow-up questionnaire. RESULTS: In men, baseline serum total cholesterol predicted new cases of radiating LBP in the 5-year follow-up (highest tertile vs. lowest: odds ratio [OR], 2.5; 95% confidence interval [CI], 1.1-5.9) and in the 10-year follow-up (OR, 2.8; 95% CI, 1.3-6.1), adjusted for age, occupational class, work history, BMI, smoking and exercise. Also, serum triglycerides predicted new cases in the latter examination (OR, 2.6; 95% CI, 1.2-5.8). In women, no associations were seen until the 10-year follow-up, when their results were similar to those in men. In the total material, serum total cholesterol predicted radiating LBP reported both at the 10- and the 28-year follow-up. Study Design: A prospective cohort study.Objective: To study the relationship of sleep disturbances with severe back disorders leading to hospitalization.Summary Of Background Data: Sleep disturbances are associated with persistent pain syndromes, but little is known about their relationship with back disorders.Methods: The first hospital admission for back disorders from 1973 to 2000 was studied in a cohort of metal industry workers (n = 902). The occurrence of sleep disturbances at baseline was categorized as: none; 1 type (either difficulties in falling asleep/waking up at night or nightmares); or both types. Cox proportional hazards regression was used to estimate the time between the assessment of risk factors and first hospital admission for back disorders.Results: Those individuals who had 1 type of sleep disturbance had a 2.1-fold (95% confidence interval 1.1-3.8) risk of back-related hospitalization, and those with both types of disturbance a 2.4-fold (1.2-4.6) risk, compared with those with no sleep disturbances. The hazard ratios were 2.1; 1.0-4.6 and 2.9; 1.2-7.1, respectively, when patients with chronic back disease or recurrent back symptoms at baseline were excluded from the analyses.Conclusion: These findings suggest that sleep disturbances are predictive of hospitalization for back disorders. Study Design: Prospective cohort study.Objective: To study symptoms, chronic disorders, and clinical findings in the low back, and work absenteeism, as predictors of hospitalization.Summary Of Background Data: Socioeconomic and lifestyle factors are associated with back-related hospitalization, but the significance in the working normal population of low back symptoms and clinical findings are not known.Methods: The cohort (n = 902) was drawn in 1973 from among employees in the metal industry (n = 2,653). The data were collected by questionnaire and a structured clinical assessment by a physiotherapist. Weight was measured. A sum score of local and radiating low back symptoms (frequency during the past year on a 4-point Likert scale) was categorized as no/yes and no/infrequent/frequent. Local and radiating symptoms were considered also separately. The data were linked with those from the Finnish Hospital Discharge Register during 1973 to 2000. Logistic regression and the Cox proportional hazard models were used.Results: As compared with persons without low back pain, those with frequent or radiating low back pain had an increased risk of hospitalization due to low back disorders (hazard rate ratio (HRR), 3.0; 95% confidence interval (CI), 1.4-6.5, and 3.7; 1.8-7.7, respectively) after adjustment for age, gender, and occupational class. Similarly, clinical findings (HRR, 2.4; 95% CI, 1.3-4.7), back-related absenteeism (HRR, 3.3; 95% CI, 1.6-6.7), and chronic low back disorders (HRR, 2.8; 95% CI, 1.5-5.4) predicted hospitalization. The associations persisted when further adjusted for smoking, body mass index, and distress symptoms at baseline.Conclusion: Frequent or radiating low back symptoms, chronic low back disorders, back-related work absenteeism, and having clinical findings in the low back predicted inpatient hospital care for low back disorders. Information on hospital admissions for back disorders from the Finnish Hospital Discharge Register in 1973-2000 was linked to the data. Cox proportional hazards regression was used to estimate the time between the assessment of risk factors and the first hospital admission for intervertebral disc disorders (36 cases) and other back disorders (47 cases).Results: In a model including psychosocial factors and potential confounders, low job control versus high control was associated with a 3.2-fold risk (95% confidence interval, 1.3-7.8) of hospitalization for back disorders other than those of the intervertebral disc. The corresponding rate ratio for low versus high supervisor support was 2.9 (95% confidence interval, 1.3-6.3). Job demands, coworker support, and distress were not independently associated with these disorders. The result did not change when patients with chronic back disorder at baseline were excluded from the analysis. There was no association between psychosocial factors at work and hospitalizations for intervertebral disc disorders.Conclusion: Low job control and low supervisor support seem to increase the risk of hospitalization for back disorders other than intervertebral disc disorders. SUMMARY OF BACKGROUND DATA: Many epidemiological studies have shown an association between smoking or overweight and back pain, but the results are still equivocal. Longitudinal studies are few. METHODS: A cohort of metal industry employees (n = 902) was studied for lifestyle, work history, and health in 1973 by questionnaire and interview. The weight of the subjects was measured and body mass index (kg/m2) was calculated. Based on intensity and duration, smoking was categorized as: never smoked (reference), stopped smoking, smoked 9 pack-years. Information on hospital admissions from 1973 to 2000 from the Finnish Hospital Discharge Register was linked to the data. Seventy-five individuals had been admitted to hospital because of back disorders. Intervertebral disc disorders and other common back disorders were analyzed separately. Cox proportional hazards regression was used to estimate the time between the assessment of potential risk factors and the first hospitalization for a back disorder. RESULTS: The rate ratio of heavy smokers (>9 pack-years) for hospitalization because of intervertebral disc disorders was 3.4 (95% confidence interval 1.3-9.0) as compared with never-smokers, allowing for other risk factors. Accordingly, the rate ratio of body mass index >27.5 kg/m2 was 2.7 (1.1-6.45) as compared with people with normal weight. The results retained when patients with chronic back disease at baseline were excluded from the analyses. Other back-related diagnoses of hospitalization were not consistently associated with smoking or overweight. After a single intramuscular dose of flunitrazepam 0.015 mg kg-1 (n = 14) in women 37 to 41 weeks pregnant, the concentrations in the umbilical artery and amniotic fluid were significantly lower than in maternal venous plasma. Although the difference between the maternal venous and umbilical venous plasma concentrations was not significant, the mean fetomaternal ratio was 0.7. The plasma protein binding of flunitrazepam was 80 +/- 4% in the mother and 79 +/- 5% in the umbilical circulation.
