Abstract

Hepatitis B is one of the main cause of chronic liver disease, and potentially cause cirrhosis, liver failure and hepatocellular carcinoma in 15-40% population. Globally, 2 billion people are infected by Hepatitis B. Two hundred and forty million people are suffering from chronic hepatitis B, and more than 780 000 people are dying from both acute and chronic complication of hepatitis B. In developed countries, horizontal transmission is the main mode of virus transmission, while in developing countries vertical transmission occured in 90% population with hepatitis B. Immunoglobulin administration as a prevention strategy for vertical transmission has 95% success rate, but it left 5-10% of those who has immunoprophylaxis failure to be at risk to develop chronic hepatitis B. Antiviral administration during pregnancy is considered to prevent hepatitis B vertical transmission in population at risk on developing immunoprophylaxis failure.

Abstract

This study has analyzed a rainfall estimation using a modified convective stratiform technique (CSTm). Unlike the original convective stratiform technique (CST), which only utilizes infrared (IR) data, CSTm applies not only IR data but also passive microwave (PMW) data. Two major modifications contained in CSTm are: (1) the application of a variability index (VI) method that uses PMW data to perform convective and stratiform separation, and (2) the ability to determine the average extensive coverage of the new areas of each pixel point as a result of the utilization of the PMW data. In this study, rainfall estimation was conducted for 23 points spread over four major islands in Indonesia. The estimation was performed based only on IR and PMW data obtained from coincident observations. For verification, the estimation results were compared with real observations. The main verification action conducted in this study used multi-category contingency tables for 4 categories and this action was supported by the root mean square error (RMSE) method. The verification results of the hourly estimation conducted for 4 days in early November 2011 showed that CSTm can effectively improve the performance quality of CST.

Abstract

Abstract The 4.0 industrial revolution created opportunities for MSMEs in rural areas to build and develop marketing networks on a wide and efficient scale. Applying digitization in a village will have an impact on the potential of what is not yet known in general. This study discusses the design of Android applications that are expected to provide new experiences to visitors to find out the potential of the village that has not been exposed. The contribution of this research is to examine the application design model that evaluates in two villages with different cultures and potentials, namely Ponggok Klaten Village and Kadubungbang Pandeglang Village. The results of QFD and AHP can help applications that are flexible to the village as well as visitor application needs and application priority features. Of the different features, because there are indeed differences between villages with different topography. Abstrak Revolusi industri 4.0 menciptakan peluang bagi UMKM di daerah pedesaan untuk dapat membangun dan mengembangkan jaringan pemasaran pada skala yang luas dan efisien. Penerapan digitalisasi pada suatu Desa akan memberikan dampak luias terhadap potensi apa yang belum diketahui secara umum. Penelitian ini bertujuan untuk mengusulkan desain aplikasi android yang diharapkan dapat memberikan pengalaman baru kepada para pengunjung untuk mengetahui potensi desa yang belum terekspos. Kontribusi dari penelitian ini adalah untuk mempelajari perbandingan pada desain model aplikasi yang dievaluasi pada dua desa dengan budaya dan potensi yang berbeda, yaitu Desa Ponggok Klaten dan Desa Kadubungbang Pandeglang. Hasil QFD dan AHP dapat diketahui bahwa aplikasi bersifat fleksibel terhadap penerapan dua desa serta prioritas kebutuhan pengunjung dan fitur prioritas aplikasi. Hasil erbandingan terdapat kesamaan dalam kebutuhan pelanggan dan perbedaan dalam desain fitur, karena kedua desa memiliki budaya dan topografi yang berbeda.

Abstract

IMPORTANCE:. Factors associated with mortality in coronavirus disease 2019 patients on invasive mechanical ventilation are still not fully elucidated. OBJECTIVES:. To identify patient-level parameters, readily available at the bedside, associated with the risk of in-hospital mortality within 28 days from commencement of invasive mechanical ventilation or coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Prospective observational cohort study by the global Coronavirus Disease 2019 Critical Care Consortium. Patients with laboratory-confirmed coronavirus disease 2019 requiring invasive mechanical ventilation from February 2, 2020, to May 15, 2021. MAIN OUTCOMES AND MEASURES:. Patient characteristics and clinical data were assessed upon ICU admission, the commencement of invasive mechanical ventilation and for 28 days thereafter. We primarily aimed to identify time-independent and time-dependent risk factors for 28-day invasive mechanical ventilation mortality. RESULTS:. One-thousand five-hundred eighty-seven patients were included in the survival analysis; 588 patients died in hospital within 28 days of commencing invasive mechanical ventilation (37%). Cox-regression analysis identified associations between the hazard of 28-day invasive mechanical ventilation mortality with age (hazard ratio, 1.26 per 10-yr increase in age; 95% CI, 1.16–1.37; p < 0.001), positive end-expiratory pressure upon commencement of invasive mechanical ventilation (hazard ratio, 0.81 per 5 cm H2O increase; 95% CI, 0.67–0.97; p = 0.02). Time-dependent parameters associated with 28-day invasive mechanical ventilation mortality were serum creatinine (hazard ratio, 1.28 per doubling; 95% CI, 1.15–1.41; p < 0.001), lactate (hazard ratio, 1.22 per doubling; 95% CI, 1.11–1.34; p < 0.001), Paco2 (hazard ratio, 1.63 per doubling; 95% CI, 1.19–2.25; p < 0.001), pH (hazard ratio, 0.89 per 0.1 increase; 95% CI, 0.8–14; p = 0.041), Pao2/Fio2 (hazard ratio, 0.58 per doubling; 95% CI, 0.52–0.66; p < 0.001), and mean arterial pressure (hazard ratio, 0.92 per 10 mm Hg increase; 95% CI, 0.88–0.97; p = 0.003). CONCLUSIONS AND RELEVANCE:. This international study suggests that in patients with coronavirus disease 2019 on invasive mechanical ventilation, older age and clinically relevant variables monitored at baseline or sequentially during the course of invasive mechanical ventilation are associated with 28-day invasive mechanical ventilation mortality hazard. Further investigation is warranted to validate any causative roles these parameters might play in influencing clinical outcomes.

Abstract

The Pacific islands and Island Southeast Asia have experienced multiple waves of human migrations, providing a case study for exploring the potential of ancient microbiomes to study human migration. We perform a metagenomic study of archaeological dental calculus from 102 individuals, originating from 10 Pacific islands and 1 island in Island Southeast Asia spanning ~3000 years. Oral microbiome DNA preservation in calculus is far higher than that of human DNA in archaeological bone, and comparable to that of calculus from temperate regions. Oral microbial community composition is minimally driven by time period and geography in Pacific and Island Southeast Asia calculus, but is found to be distinctive compared to calculus from Europe, Africa, and Asia. Phylogenies of individual bacterial species in Pacific and Island Southeast Asia calculus reflect geography. 47. Factors on medical clerkship students' quality of life: A survey study.

Abstract

Introduction: Due to the higher demand for healthcare provision in Indonesia, increasing the number of medical schools in Indonesia was highly encouraged. Meanwhile, medical students were considered a susceptible population to a variety of issues that negatively impact their quality of life (QOL) in comparison to the general community., Objectives: The study aims to assess the QOL of Indonesian medical students during the clinical clerkship program (years 4-5) of their medical training while also exploring the association between the results and their ongoing rotation., Methods: The survey was conducted online to 311 students at a private medical school in Jakarta using the Indonesian version of WHOQOL-BREF. Participants' characteristics, sleep duration, physical activity, and smoking history were also assessed to see if there was any association. Results: 82% of 311 registered clerkship students responded to the survey. 48. Veillonella orientalis sp. nov., an anaerobic Gram-stain-negaitve coccus isolated from saliva of a Thai child.

Abstract

Objective: The genus Veillonella is one of the major and important constitutions of the oral microbiome. A novel anaerobic, Gram-strain-negative coccus belonging to the genus Veillonella was isolated from the saliva of a Thai child coming from the east of Thailand. In the present study, the characterization of one strain, S12025-13 T is described with the comparison of established species of the genus Veillonella and proposed a novel oral species of the genus Veillonella., Methods: Strain S12025-13 T was conclusively identified using phenotypic, phylogenetic, genomic and biochemical approach. In phylogenetic analysis, multi-locus species trees (MLST) analysis was also conducted in this study., Results: The strain S12025-13 T showed typical phenotypic characteristics of members of the genus Veillonella. Under anaerobic conditions, the strain produced acetic acid and propionic acid as metabolic end products in a trypticase-yeast extract-heamin medium containing 1% (w/v) glucose, 1% (w/v) fructose and 1% (v/v) sodium lactate. Sequencing of their 16S rRNA genes confirmed that it belongs to the genus Veillonella. Comparative analysis of the 16S rRNA, dnaK and rpoB gene sequences indicated that phylogenetically the strain comprised a distinct novel branch within the genus Veillonella. The novel strain showed 99.77, 97.29, and 99.02% similarity to partial 16S rRNA, dnaK and rpoB gene sequencing, respectively, to the type strains of the most closely related species. In MLST (selected 79 genes) analysis, strain S12025-13 T formed a distinct taxon with robust bootstrap support (100%) within the genus Veillonella. Furthermore, strain S12025-13 T shared the highest average nucleotide identity (ANI) value (95.97%) with the type strain of the most closely related species, "Veillonella faecalis" which is not validly published under the International Code of Nomenclature of Prokaryotes (ICNP). Likewise, strain S12025-13 T showed the highest digital DNA-DNA hybridization (dDDH) value (65.00%) with the type strain of "V. faecalis". In addition, strain S12025-13 T formed a distinct branch in this clade with a bootstrap value of 72% with Veillonella nakazawae and 51% with "V. faecalis" related species of the genus Veillonella in the genome based phylogenetic tree., Conclusion: The novel strain could be discriminated from previously reported species of the genus Veillonella based on partial dnaK gene sequencing, MLST analysis, ANI and dDDH values, and genome-based phylogeny. Based on these observations, this strain represents a novel species, for which the name Veillonella orientalis sp., nov. is proposed. Abstract

Background: The selling of Jamu Gendong (an Indonesian traditional herbal medicine), is closely associated with the informal work of women who migrate to different regions. In Sukoharjo Regency and Wonogiri Regency, Central Java Province, the pressing need to meet household necessities in their places of origin compels women to assume the role of breadwinners. Therefore, this research aimed to identify the livelihood capital and resilience of migrant women selling jamu gendong., Methods: The study was conducted in Ciamis Regency, West Java Province, Indonesia, in November 2022, using quantitative and qualitative methods. Primary data were collected through structured questionnaires and in-depth interviews. The sample comprised 51 women selected through snowball sampling and actively involved in selling Jamu Gendong, along with six key individuals from relevant agencies., Results: The research findings indicate that the households of migrant women who sell jamu gendong can survive and adapt during their migration to prevent poverty by implementing diverse livelihood strategies. They are the only ones who move without their family members renting a place to live with them. This research has identified various capital sources, including productive age and skills in preparing jamu gendong; physical capital, including road infrastructure, markets, and access to health and education facilities; and natural, social, and financial capital. This research provides an in-depth understanding of women's roles in family economic resilience, diversification of life strategies, the importance of social capital in migrant networks, economic empowerment through migration, and the influence of cultural values on livelihood strategies., Conclusions: The findings contribute to a comprehensive understanding of the resilience demonstrated by migrant women selling jamu gendong. Abstract

Cyclooxygenase (COX) and lipoxygenase (LOX) enzymes play a pivotal role in producing pro-inflammatory eicosanoids, including prostaglandins (PGs) and leukotrienes (LTs), in the inflammation process. Mitragynine is a primary alkaloid contained in the kratom's leaves and has been reported to show anti-inflammatory activity by suppressing COX-2 mRNA translation to lowering PGs synthesis. In this study, the Kratom's alkaloid extract containing ~ 46% mitragynine was found to exhibit dual inhibition activity towards COX-2/5-LOX enzymes at concentrations below 25 ppm in the LPS-induced RAW 264.7 macrophage cells. At these levels, no cell toxicity was observed while the cells became death (e.g., 10-46% viability at 50-100 ppm) and only COX-2 inhibition activity was observed after exposed with more than 25 ppm of alkaloid extract. In contrast, the methanolic-crude extract of Kratom's leaf containing ~ 5% mitragynine showed no inhibition toward COX-2/5-LOX enzymes and did not toxic onto the cells, even after treated at 100 ppm. The alkaloid extract suppressed several antiinflammation parameters, including ROS (64% reduction at 25 ppm), NO (30% reduction at 25 ppm), TNF-α (~ 50% reduction at 25 ppm), and IL-6 production (60% reduction at 6.25 ppm). In silico molecular studies indicated strong binding affinity of Kratom alkaloids to COX-2 and 5-LOX active sites, supporting the Kratom's alkaloids to have great potential dual inhibition activity towards COX-2/5-LOX enzymes and to be developed as a safer NSAIDs with fewer side effects., Competing Interests: Declarations.
