Atrial fibrillation (AF) is the most frequent sustained cardiac arrhythmia in clinical practice and, although its importance has been underestimated even in recent years, we are now becoming aware of its clinical transcendence1,2,3. The classical treatment is pharmacological, but its efficacy is limited and it does have side effects4,5. Therefore, in recent years, there has been an increasing interest in other types of non-pharmacological treatments6,7. Physiologic cardiac pacing has proven to be more effective than VVI mode pacing to prevent the occurrence of AF during the follow-up of patients who have had a permanent pacemaker implanted 8,9,10. There are currently different lines of research that use different atrial pacing techniques to prevent and treat episodes of paroxysmal atrial fibrillation11,12. Techniques of multi-site pacing in the right atrium or both atria, new atrial pacing sites, prevention algorithms for paroxysmal atrial fibrillation episodes, and even high-frequency atrial tachyarrhythmia termination algorithms have all been proposed. In this article, we will try to synthesize the grounds for and findings of the different lines of research currently being developed.

The advent of generative networks and their adoption in numerous domains and communities have led to a wave of innovation and breakthroughs in AI and machine learning. Generative Adversarial Networks (GANs) have expanded the scope of what is possible with machine learning, allowing for new applications in areas such as computer vision, natural language processing, and creative AI. GANs, in particular, have been used for a wide range of tasks, including image and video generation, data augmentation, style transfer, and anomaly detection. They have also been used for medical imaging and drug discovery, where they can generate synthetic data to augment small datasets, reduce the need for expensive experiments, and lower the number of real patients that must be included in medical trials. Given these developments, we propose using the power of GANs to create and augment flow-based network traffic datasets. We evaluate a series of GAN architectures, including Wasserstein, conditional, energy-based, gradient penalty, and LSTM-GANs. We evaluate their performance on a set of flow-based network traffic data collected from 16 subjects who used their computers for home, work, and study purposes. The performance of these GAN architectures is described according to metrics that involve networking principles, data distribution among a collection of flows, and temporal data distribution. Given the tendency of network intrusion detection datasets to have a very imbalanced data distribution, i.e., a large number of samples in the "normal traffic" category and a comparatively low number of samples assigned to the "intrusion" categories, we test our GANs by augmenting the intrusion data and checking whether this helps intrusion detection neural networks in their task. Circular economy (CE) research has advanced significantly in recent years in areas like environmental science, engineering, and technology. Governance presents CE strategies as imperative and compelling, often linked to efforts to achieve sustainable business development. However, the financial quantification of a CE is still underexplored and heterogeneous, with multiple research streams exploring the financial dimension. The aims of this study are twofold: (i) to analyze the productivity of the scientific literature and the conceptual evolution of circularity as it relates to financial strategies; and (ii) to formulate future research directions addressing a CE in close connection with financial sustainability knowledge. Bilbiometrix R-package 2023.12.1 and VOSviewer version 1.6.20 were used to analyze a sample of 279 articles retrieved from WoS and Scopus using the PRISMA protocol. The analysis covered the period 2010–2023, with the diachronic thematic analysis revealing four key themes: (1) CE and green financial performance; (2) environmental impact of CE; (3) performance approach of CE and metrics; (4) CE and sustainable development. Building on previous knowledge, we developed an integrated and multidisciplinary framework of CE embedded in financial performance. Future research agenda shaped by the emerging themes and research gaps can support managers, shareholders, or governance in incorporating CE policies into operational activities and strengthening sustainable circularity knowledge. Myeloproliferative neoplasms (MPNs), namely, polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF), are clonal stem cell disorders defined by an excessive production of functionally mature and terminally differentiated myeloid cells. MPNs can transform into secondary acute myeloid leukemia (sAML/blast phase MPN) and are linked to alterations in the redox balance, i.e., elevated concentrations of reactive oxygen species and markers of oxidative stress (OS), and changes in antioxidant systems. We evaluated OS in 117 chronic phase MPNs and 21 sAML cases versus controls by measuring total antioxidant capacity (TAC) and 8-hydroxy-2′-deoxy-guanosine (8-OHdG) concentrations. TAC was higher in MPNs than controls (p = 0.03), particularly in ET (p = 0.04) and PMF (p = 0.01). MPL W515L-positive MPNs had higher TAC than controls (p = 0.002) and triple-negative MPNs (p = 0.01). PMF patients who had treatment expressed lower TAC than therapy-free subjects (p = 0.03). 8-OHdG concentrations were similar between controls and MPNs, controls and sAML, and MPNs and sAML. We noted associations between TAC and MPNs (OR = 1.82; p = 0.05), i.e., ET (OR = 2.36; p = 0.03) and PMF (OR = 2.11; p = 0.03), but not sAML. 8-OHdG concentrations were not associated with MPNs (OR = 1.73; p = 0.62) or sAML (OR = 1.89; p = 0.49). Background: Patients with myeloproliferative neoplasms (MPNs) experience a high disease-related symptom burden. A specific instrument to evaluate quality of life (QoL), i.e., the MPN Symptom Assessment Form Total Symptom Score (MPN-SAF TSS; MPN-10), was developed. We conducted the translation, cultural adaptation, and validation into Romanian of the MPN-10. Methods: We translated the MPN-10 and tested its psychometric properties. Results: We recruited 180 MPN patients: 66 polycythemia vera (36.67%), 61 essential thrombocythemia (33.89%), 51 primary and secondary myelofibrosis (SMF) (28.33%), and 2 MPN-unclassifiable (1.11%). The mean TSS was 19.51 ± 16.51 points. Fatigue, inactivity, and concentration problems were the most cumbersome symptoms. We detected scoring differences between MPN subtypes regarding weight loss (p < 0.001), fatigue (p = 0.006), early satiety (p = 0.007), night sweats (p = 0.047), pruritus (p = 0.05), and TSS (p = 0.021). There were strong positive associations between TSS and inactivity, fatigue, and concentration problems, and moderate negative correlations between QoL scores and all MPN-10 items. Cronbach's α internal consistency coefficient was 0.855. The Kaiser–Meyer–Olkin construct validity test result was 0.870 and the Bartlett Sphericity Test was significant (p < 0.001). Symptom scores were loaded into one single factor according to the exploratory factor analysis. Conclusions: The Romanian MPN-10 version displayed excellent psychometric properties and is a reliable instrument for assessing symptom burden and QoL in Romanian MPN patients. The use of Information Technology in organizations and in a business context has notably been associated with the manner in which transactions are carried out through recording, processing, and reporting of information. This often happens to organizations that have automated information systems to perform the aforesaid activities. In this regard, the study, therefore, aims to investigate the impact of the rapid growth of the Information technology to internal auditing activities and examine if there are changes that are identified in information technology evaluations that are undertaken in Lebanese Organizations. In carrying out this research, a quantitative methodology approach was deployed where a surveying technique was used to facilitate the study by use of a selfadministered questionnaire that aided in achieving these objectives. Five hundred copies of the questionnaire were randomly distributed to Lebanese organizations located across Beirut specifically to accountants and auditors. The survey results were analyzed using SPSS software to show the impact of information technology in auditing activities and the changes that have been experienced in Information technology and their implications on the accounting sector. The survey findings have been used to conclude if the internal auditors need to enhance their knowledge and skills of computerized information system (CIS) which can help improve planning, directing, supervising and reviewing the work performed. Factors that affect Auditors performance on the Information Technology evaluations were identified and their impacts assessed. The research conclusively highlighted the impact of the study on managers and auditors.

Public accounting is one of the industry's very growing sectors.The objective is to generate a large amount of Financial information that can be further used by stakeholders to make informed decisions false information or could jeopardize the businesses involved and therefore this type of information must be Protected from external and vulnerable attacks. As the transparency and accountability of the financial Statement is very important for the trust of both the stakeholder and the investment, the security of financial information is very important for the relevance, validity and reliability of financial data.however, there is always a persistent risk of Security breaches such as attacks by brute force to obtain the information. The main focus of this paper is to examine if the adoption of the IPASS in the public accounting sector in Lebanon Support and leverage the security systems and strategies used by Lebanon's public accountants to secure financial data and financial report relevance. A quantitative methodology was used to carry out this study and a survey was conducted with a representative sample of accountants currently employed in the government sector in Lebanon. The survey questionnaire was prepared to measure the extent where recently the IPASS was adopted in public accounting security in Lebanon and also to evaluate their efficiency in data protection and to detect threats. The research carried out is likely to determine the quality of financial data protection in Lebanon's public accounting in conjunction with the application of IPASS and suggest possible solutions to enhance security. As security measures are an integral part of the IPSAS implementation process, this study contributes significantly to both the IPSAS adoption theory and the practice of aligning local accounting practices with international standards through exorbitant efforts to enhance security management in the Public Accounting Sector of Lebanon.

Simple Summary: Acute promyelocytic leukemia (APL) stands as a remarkable success in achieving high cure rates with chemotherapy-free treatment regimens. Despite these remarkable outcomes, hematologists managing patients with APL take into consideration the distinct side effects associated with the two dual differentiation agents, i.e., all-transretinoic acid (ATRA) and arsenic trioxide (ATO). The objective of this paper is to review relevant literature data and discuss the recommended management strategies of non-hematologic early and late complications that arosein patients diagnosed with APL who were treated with ATO and ATRA-based regimens. The hallmark of acute promyelocytic leukemia (APL) is the presence of the characteristic fusion transcript of the promyelocytic leukemia gene with the retinoic acid receptor α gene (PML::RARA). The PML::RARA fusion is a molecular target for all-trans retinoic acid (ATRA) and arsenic trioxide (ATO). Therapies based on ATRA plus ATO have excellent outcomes in terms of complete remission rates, overall survival, and achievement of deep and durable molecular responses with a very low incidence of relapse. However, although the combination of ATRA and ATO has lower hematologic toxicity than standard chemotherapy, its use is associated with a spectrum of distinctive toxicities, such as differentiation syndrome, liver toxicity, QT interval prolongation, and neurotoxicity. Rigorous monitoring of patients' clinical evolution is indispensable for identifying and addressing each complication. The objective is to maintain an equilibrium between treatment-induced adverse events and therapeutic efficacy. This paper focused on non-hematologic complications associated with the combination of ATRA and ATO. Additionally, we discuss late-onset complications of this therapy. In summary, the majority of treatment-related adverse events are manageable, self-limiting, and reversible. More so, there seems to be a lower incidence rate of secondary neoplasms compared to standard chemotherapy. The credit risk is one of the main banking activity risks, with direct impact on the bank performance. Approaches based on internal rating models introduced by the Basel II agreement allow banks to use their own estimates for credit risk quantification, with direct effect on capital adequacy. This study aims to develop a scoring model for quantifying the probability of default dependent on the non-performing loans rate evolution based on quantitative information and determination of the power of prediction to determine non-reimbursement situations. Also, it was considered the determination of some qualitative variables impacting on the reimbursement capacity of companies. The financing sources, in essence, in-house or attracted, condition the profitability of any business and influence the financial position of the company, both in the short and long term. This study aims at an understanding of the inter-conditioning relationship between the financing sources, profitability and default risk. The estimation of the default probability is the first step to determining and assessing the credit risk. Major issues in the estimation of the default probability are generated by the limitation of the required information. The approach based on internal rating models relies on the accuracy of the default probability estimation.

The authors aim to “drill” into the complex issue of the testimonial evidence, through a fresher approach, thus being interested, besides the purely legal aspect, also in the historical, sociological and even psychological perspective.

In the 2016 revision of the World Health Organization classification the term therapy-related myeloid neoplasia (t-MN) defines a subgroup of acute myeloid leukemia (AML) comprising patients who develop myelodysplastic syndrome (MDS-t) or acute myeloid leukemia (AML-t) after treatment with cytotoxic and/or radiation therapy for various malignancies or autoimmune disorders. We report the case of a 36 year old patient with t-MN (t-MDS) after achieving complete remission (CR) of a PML-RARA positive acute promyelocytic leukemia (APL) at 32 months after diagnosis. Initially classified as low risk APL and treated according to the AIDA protocol - induction and 3 consolidation cycles - the patient achieved a complete molecular response in September 2013 and started maintenance therapy. On follow-up PML-RARA transcript remained negative. In January 2016 leukopenia and thrombocytopenia developed and a peripheral blood smear revealed hypogranular and agranular neutrophils. Immunophenotyping in the bone marrow aspirate identified undifferentiated blast cells that did not express cytoplasmic myeloperoxidase. The cytogenetic study showed normal karyotype. The molecular biology tests not identified PMLRARA transcript. A diagnosis of t-MDS (AREB-2 - WHO 2008) was established. Treatment of AML was started with 2 “3+7” regimens and 1 MEC cycle. Two months from diagnosis, while in CR, an allogeneic HSCT from an unrelated HLA compatible donor was performed after myeloablative regimen. An unfavorable clinical evolution was followed by death on day 9 after transplantation. The occurrence of t-MNs during CR of APL represents a particular problem in terms of follow-up and differential diagnosis of relapse and constitutes a dramatic complication for a disease with a favorable prognosis.

Chronic myelomonocytic leukemia (CMML) is a clonal hematopoietic stem cell disorder that is characterized by the presence of an absolute monocytosis (1 × 10^ 9/l) in the peripheral blood, the overlap of myelodisplastic aspects and myeloproliferative aspects in the bone marrow and tendency to transform into acute myeloid leukemia. CMML is considered to be the most aggressive chronic myeloid leukemia. We present the case of a 48 years old woman who was hospitalized in March 2013 in the Center of Hematology and Bone Marrow Transplantation for anemia related symptoms. Initial investigations showed anemia, relative monocytosis (10% monocytes of the WBC differential) with an increasing absolute number of monocytes (> 1,000/μl) in the following months. Initial exploration of the bone marrow (aspirate and bone marrow biopsy and immunohistochemistry IHC tests) revealed elements of trilinear dysplasia and an increased percentage of myeloblasts (11-14%). In the next four months myeloblasts percentage remained below 20% (8-14%) and it has been observed a gradually increasing of monocytoid elements (> 20%). Immunophenotyping in the bone marrow aspirate identified a monocytic proliferation with high percentage (8%) of immature cells. The karyotype reported the presence of clones with t (1;3). Initially diagnosed as RAEB-2 (WHO) the case was recomitted in CMML-type 2 with a progression to acute myeloid leukemia (AML). Allogeneic hematopoietic stem cell transplantation (allo-HSCT) has been performed after getting the best possible therapeutic response with AML chemotherapy type (complete remission). Allo-HSCT was performed using myeloablative conditioning, 12 months after diagnosis. The patient is now in complete remission, 24 months after allo-HSCT.

Fusarium, a common fungus, emerges as a pathogen in severely immunocompromised patients. We present a series of patients who developed invasive fusariosis (IF) during admission to an acute leukaemia ward: an outbreak of 12 cases in June and July 2018, followed by four sporadic cases until 2021. No case was reported earlier. All patients were clustered in the same location with indoor air and water installations found to be contaminated with Fusarium spp. thus a nosocomial outbreak was assumed. Following the water installation replacement, the number of Fusarium cases dramatically dropped to one or two isolated instances per year in the same location. All 16 patients had acute leukaemia and developed IF during severe neutropenia following induction therapy. IF diagnosis was based on positive blood cultures (14 patients) and/or on tissue biopsies (3 patients). The median time from admission to the IF onset was 20 days, and from the first day of severe neutropenia (≤500/mm3) was 11.5 days. All patients were febrile, eight had moderate‐to‐severe myalgias, eight had respiratory involvements: lung lesions and/or sinusitis and seven had characteristic skin lesions. Follow‐up: 12 out of 16 (75%) were alive on Day 90; nine out of 15 (60%) were alive on Month 6. All with intractable neutropenia died. In severely neutropenic febrile patients, the triad of respiratory involvement/skin lesions/severe myalgia may suggest Fusarium aetiology. The ability to recover from neutropenia is critical to surmount IF. The indoor environment in immunocompromised dedicated settings must be constantly controlled. [ABSTRACT FROM AUTHOR]

This paper delves into the impact of the automation-driven evolution of enterprise resource planning systems (ERPSs) on sustainability accounting and reporting and the associated challenges. By employing a holistic approach focusing on the current characteristics of both ERPSs and automation platforms and an inductive approach to perform a content analysis, this study highlights how the new generation of ERPSs can support the improvement of accounting in general and sustainability accounting and reporting quality in particular. The analysis was based on information provided by the developers of leading ERPSs and automation platforms with a significant worldwide market share. In this paper, we deepen the understanding of the role that ERPSs play in improving sustainability accounting and reporting, focusing on both the benefits and challenges derived from the impact generated by increasing the integration of robotic process automation and intelligent process automation solutions. The results obtained have academic and managerial implications, addressing a research gap concerning the understanding of the consequences of ERPSs evolution from the perspective of professionals and the competitive edge companies can take advantage of to improve sustainability reporting and accounting processes. Acute promyelocytic leukemia (APL) is a unique, highly curable subtype of acute myeloid leukemia, owing to the therapeutic advances of the last decades which led to high complete remission rates and excellent long-term survival. Nevertheless, it remains associated with high early mortality rates. Early death is the major cause of treatment failure in APL and is mainly attributed to coagulopathy, differentiation syndrome, and less commonly, infectious events. Timely recognition of each complication plays a crucial role in the management of patients diagnosed with APL. Coronavirus Infectious Disease 2019 (COVID-19) has shown great heterogeneity in patient presentation. Clinical manifestations range from asymptomatic disease to severe forms, mainly characterized by a hyperinflammatory syndrome leading to acute respiratory distress and multiorgan failure. Patients with acute leukemia and concomitant COVID-19- related hyperinflammatory syndrome have particularly poor outcomes. We hereby report the case of a 28-year-old male patient who was diagnosed with high-risk APL, with severe associated coagulopathy at presentation. He was treated with chemotherapy according to the AIDA regimen. The first week of induction therapy was complicated by a differentiation syndrome manifesting as fever not attributable to infection and respiratory distress with pulmonary infiltrates, resolved after ATRA discontinuation and corticotherapy. On the fourth week of treatment, he tested positive for acute respiratory syndrome coronavirus 2 (SARS-CoV-2) with minor pulmonary involvement. Clinical manifestations over the following days included tachycardia and hypotension, associated with elevated inflammatory markers and cardiac biomarkers (troponin I x58 upper NV). Cardiovascular magnetic resonance imaging was consistent with myocarditis. COVID-19-associated myocarditis was successfully treated with methylprednisolone, intravenous immunoglobulins and Anakinra. Differentiation syndrome and COVID-19-associated myocarditis are two lifethreatening complications that adversely impact survival. Digitalization and sustainable development represent two highly topical subjects, each of them being primarily debated in the literature. Although it is evident that digitalization brings new opportunities and challenges for the management of organizations and for meeting their sustainable strategies, there are relatively few studies analyzing the relationship between the two. Therefore, the objective of the present study is to analyze the relationship between digitalization and sustainable development in the European Union (EU) countries, between 2019 and 2021, before and during the COVID-19 pandemic. On this matter, the link between Digital Economy and Society Index (DESI), on one side, and Sustainable Development Goal Index (SDG Index) and Spillover Index (SS Index), on the other side, has been analyzed using correlations. While DESI refers to the monitoring of digital advancement of the EU member states, SDG Index and SS Index are based on many indicators and give a multidimensional perspective regarding sustainable development. Results show a positive and significant relationship between DESI and SDG Index, and the effect gradually decreases during the analyzed period. Regarding the relationship between DESI and SS Index, the correlation was negative, but significant in 2020 and 2019 only. At the level of each geographical region, the relationship between DESI and SDG Index was positive, with a larger effect in the Northern and Western region and the Southern region compared to the Central and Eastern region, but this effect was not statistically significant. In addition, the relationship between DESI and SS Index was negative and significant at the level of the Central and Eastern region and the Southern region but positive and not statistically significant at the level of the Northern and Western region. In addition to previous papers on this field, this study adds the analysis between digitalization and the Spillover Index. In addition, while previous studies seek to identify the relationship between digitalization and sustainability at a single country level or region, this study presents the correlation analysis at the EU level and compares the results for each EU region. Our study commemorates this event by presenting a retrospective of the publications related to the use of mathematical tools for the analysis of economic, ecological, and environmental phenomena. We analyzed 1257 scientific publications using bibliometric techniques to examine the most productive and influential authors and their contributions in the economic, ecological, and environmental fields. Co-authorship among the top authors and countries, co-occurrence of the keywords, bibliographic coupling of the documents and authors, and author co-citation were analyzed by applying network analysis techniques using VOSviewer software, identifying the intellectual structure of the research and the collaborative networks in the fields. The results show that mathematics has grown impressively in terms of publication and citation. The contributions come from all over the world, but the majority are from the People's Republic of China and Spain. The results also show themes and trends in the economic, environmental, and ecological fields and a predominant use of mathematical tools in optimization processes in order to rigorously substantiate the decisions of investors and policymakers. Enterprise Resource Planning systems (ERP systems) represent integrated IT systems that have the role of incorporating a series of modules and functions necessary for the development of the organization's activity. The digitalization process took off about ten years ago, when more and more organizations were looking for IT solutions to make their activities more efficient. The need arose as a result of the large volume of information that had to be processed daily and for which the time allocated for solving these tasks was much too large. The current work aims to analyze the digitalization method of the business using integrated ERP systems, based on the bibliometric analysis of the processed information from the specialized literature. The bibliometric analysis is based on a statistical (quantitative) analysis of written publications that address the two key concepts: ERP systems and business digitalization. Bibliometrics aims to "count" the scientific publications or the authors who wrote about the two concepts. The obtained results show that ERP systems can represent a source for digitalizing the processes of a business, integrating the organization's activity in ERP systems with the help of modules that can be implemented. ERP system modules can be implemented for each department, integrating all processed information in a single database common to all departments, access to sensitive information being controlled by each category of users. The digitalization process offers the possibility of the organization to develop sustainably, thus being able to obtain a series of financial advantages as a result of the implementation of a sustainable plan at the level of the organization. Protocols for clustering and routing in the Internet of Things ecosystem should consider minimizing power consumption. Existing approaches to cluster-based routing issues in the Internet of Things environment often face the challenge of uneven power consumption. This study created a clustering method utilising swarm intelligence to obtain a more even distribution of cluster heads. In this work, a firefly optimization method and an aquila optimizer algorithm are devised to select the intermediate and cluster head nodes required for routing in accordance with the NP-Hard nature of clustered routing. The effectiveness of this hybrid clustering and routing approach has been evaluated concerning the following metrics: remaining energy, mean distances, number of hops, and node balance. For assessing Internet of things platforms, metrics like network throughput and the number of the living node are crucial, as these systems rely on battery-operated equipment to regularly capture environment data and transmit specimens to a base station. Proving effective, the suggested technique has been found to improve system energy usage by at least 18% and increase the packet delivery ratio by at least 25%. [ABSTRACT FROM AUTHOR]

This paper aims to examine the main challenges encountered by mobile grocery-shopping applications' (MGSAs) users, wherein the analysis is based on the review comments for three popular MGSAs deployed by main grocery retailers in Europe. The research methodology used was qualitative in the form of a cross-sectional inductive approach, allowing for the identification of the main issues encountered by users and their classification into four categories for a more straightforward presentation. The research findings indicate that despite the below-average level of digital literacy, customers are trying to use MGSAs efficiently and are proposing different areas of improvement, such as the design, the general functionality of the applications, and other factors regarding the specific functionalities of MGSAs. The findings may be leveraged by grocery retailers to exploit this market efficiently in a post-pandemic context; moreover, the study's results could provide meaningful knowledge to the mobile retail industry, as the detailed insights offer adequate support for enhancing mobile-shopping (m-shopping) applications. Financial markets have always been a point of interest for automated systems. Due to their complex nature, financial algorithms and fintech frameworks require vast amounts of data to accurately respond to market fluctuations. This data availability is tied to the daily market evolution, so it is impossible to accelerate its acquisition. In this article, we discuss several solutions for augmenting financial datasets via synthesizing realistic time-series with the help of generative models. This problem is complex, since financial time series present very specific properties, e.g., fat-tail distribution, cross-correlation between different stocks, specific autocorrelation, cluster volatility and so on. In particular, we propose solutions for capturing crosscorrelations between different stocks and for transitioning from fixed to variable length time-series without resorting to sequence modeling networks, and adapt various network architectures, e.g., fully connected and convolutional GANs, variational autoencoders, and generative moment matching networks. Finally, we tackle the problem of evaluating the quality of synthetic financial time-series. We introduce qualitative and quantitative metrics, along with a portfolio trend prediction framework that validates our generative models' performance.
