Los bosques secos tropicales (BST) son uno de los ecosistemas más amenazados de Colombia, con remanentes de estos concentrados en bosques riparios. Este trabajo estudió la vegetación de un bosque ripario ubicado dentro de un remanente de BST en la cuenca alta del río Magdalena mediante el uso de transectos en tres estaciones a lo largo del arroyo La Avería (Paicol, Huila, Colombia). Se encontraron 199 individuos distribuidos en 47 especies. La composición florística fue semejante a la de otros BST, mientras que el número de individuos fue menor. Fabaceae fue la familia más representada y Zygia longifolia y Guadua angustifolia las especies con mayor Índice de Valor de Importancia (IVI). Se concluyó que el bosque ha sido sometido a perturbaciones antrópicas y se encuentra en una etapa sucesional temprana. Además, se encontraron diferencias en el grado de conservación del bosque entre las tres estaciones muestreadas.

La desinfección del agua con cloro, genera subproductos como los trihalometanos (THM´s), a los cuales se les atribuyen propiedades cancerígenas y con un potencial efecto adverso en el sistema reproductivo. El nivel máximo permitido de THM´s en Estados Unidos es 80 µg/L, mientras que en Colombia es 200 µg/L. En este estudio un método simple, rápido y libre de solventes fue estandarizado para la determinación de THM´s en aguas para consumo humano utilizando microextracción en fase sólida con espacio de cabeza combinada con cromatografía de gases con detector de microcaptura de electrones (HS-SPME-GC-mECD) y utilizado para la cuantificación de THM´s en aguas de consumo humano. Fueron estudiados parámetros experimentales así como el tiempo de adsorción, la temperatura de adsorción y desorción, la agitación de la muestra y la adición de NaCl. Se determinaron los límites de detección y cuantificación del método y se realizó el análisis de los trihalometanos de 75 muestras de agua de la planta de tratamiento de Aguas y Aguas de la ciudad de Pereira. Las condiciones optimizadas fueron 20 min de extracción a 37 °C en presencia de 25% de NaCl y con una agitación de 200 rpm; con tiempo de desorción de 4 min a 250 °C. El rango lineal de 5-100 mg/L fue establecido con una desviación estándar relativa (%RSD) en el rango 6,1-10,1 %. Los límites de detección estuvieron en el rango 3,8-7,8 mg/L. El promedio de la concentración de THM´s fue de 55,5 mg/L el cual cumple con los límites máximos permitidos en la regulación internacional y nacional.

Major depressive disorder (MDD) during adolescence is a common and disabling psychiatric condition; yet, little is known about its neurobiological underpinning. Evidence indicates that MDD in adults involves alterations in white and gray matter; however, sparse research has focused on adolescent MDD. Similarly, little research has accounted for the wide variability of symptom severity among depressed teens. Here we aimed to investigate white matter (WM) microstructure between seventeen adolescents with MDD compared to sixteen matched healthy controls (HC) using diffusion tensor imaging (DTI). We further assessed within the MDD group relationships between WM integrity and depression severity, as well as anhedonia and irritability—two core symptoms of adolescent MDD. As expected, adolescents with MDD manifested decreased WM integrity compared to HC in the anterior cingulum and anterior corona radiata. Within the MDD group, greater depression severity was correlated with reduced WM integrity in the genu of corpus callosum, anterior thalamic radiation, anterior cingulum, and sagittal stratum. However, anhedonia and irritability were associated with alterations in distinct WM tracts. Specifically, anhedonia was associated with disturbances in tracts related to reward processing, including the anterior limb of the internal capsule and projection fibers to the orbitofrontal cortex. Irritability was associated with decreased integrity in the sagittal stratum, anterior corona radiata, and tracts leading to prefrontal and temporal cortices. Overall, these preliminary findings provide further support for the hypotheses that there is a disconnect between prefrontal and limbic emotional regions in depression, and that specific clinical symptoms involve distinct alterations in WM tracts.

Calculation methods of spur and helical gear drives with a transverse contact ratio higher than 2, accurate enough for preliminary designs or standardization purposes, are not available in technical literature. If the load is assumed to be uniformly distributed along the line of contact, simple equations given by the linear theory of elasticity and the Hertzian contact model are not in good agreement with experimental results. However, these simple equations may work quite accurately with a more realistic model of load distribution accounting the changing rigidity of the pair of teeth along the path of contact. In this paper a nonuniform model of load distribution along the line of contact, obtained from the minimum elastic potential criterion, has been considered for stress calculation of spur and helical gear drives with transverse contact ratio between 2 and 2.5. Introduction: Compared with the operating room, tracheal intubations in the intensive care unit (ICU) are associated with worsened glottic view, decreased first-time success rate and increase in the technical difficulty of intubation and incidence of complications. Videolaryngoscopes (VLs) have been proposed to improve airway management, and while recent studies have confirmed that VLs improve intubation conditions in this patient population, there remains a lack of clarity regarding the selection between a standard Macintosh blade or a hyperangulated one, to determine which yields the best outcomes. The purpose of this study was to compare successful intubation on the first attempt with the Macintosh VL versus the hyperangulated VL during tracheal intubation in ICU patients. We hypothesise that tracheal intubation using the hyperangulated VL will improve the frequency of successful intubation on the first attempt., Methods and Analysis: The INtubation VIdeolaryngoscopy BLADE-ICU trial is a prospective, multicentre, open-label, interventional, randomised, controlled superiority study conducted in 29 ICUs in Spain. Patients will be randomly assigned in a 1:1 ratio to undergo intubation using a Macintosh VL (control group) or a hyperangulated VL (experimental group) for the first intubation attempt. The primary outcome is successful intubation on the first attempt. The secondary outcomes include the time to intubation, attempts for successful intubation, laryngoscopic vision assessed with the modified Cormack-Lehane scale, the need for adjuvant airway devices for intubation, difficulty assessed by the anaesthesiologist and complications during tracheal intubation. Introduction and Objectives: Corticosteroids are among the drugs demonstrating a mortality benefit for coronavirus disease 2019 (COVID-19). The RECOVERY trial highlighted that dexamethasone reduced 28-day mortality for hospitalized COVID-19 patients requiring either supplemental oxygen or mechanical ventilation. It is noted that approximately 30% of COVID-19 patients, initially presenting with mild symptoms, will advance to acute respiratory distress syndrome (ARDS), especially those with detectable laboratory markers of inflammation indicative of disease progression. Our research aimed to explore the efficacy of dexamethasone in preventing the progression to ARDS in patients hospitalized with COVID-19 pneumonia who do not yet require additional oxygen but are at high risk of developing ARDS, potentially leading to a reduction in morbimortality., Methods: In this multicenter, randomized, controlled trial, we evaluated the impact of dexamethasone on adult patients diagnosed with COVID-19 pneumonia who did not need supplementary oxygen at admission but were identified as having risk factors for ARDS. The risk of ARDS was determined based on specific criteria: elevated lactate dehydrogenase levels over 245 U/L, C-reactive protein levels exceeding 100 mg/L, and a lymphocyte count below 0.80 × 10 9 /L. Participants were randomly allocated to either receive dexamethasone or the standard care. The primary endpoints included the incidence of moderate or severe ARDS and all-cause mortality within 30 days post-enrollment., Results: One hundred twenty-six patients were randomized. Among them, 41 were female (30.8%), with a mean age of 48.8 ± 14.4 years. Ten patients in the dexamethasone group (17.2%) and ten patients in the control group (14.7%) developed moderate ARDS with no significant differences. Mechanical ventilation was required in six patients (4.7%), with four in the treatment group and two in the control group. There were no deaths during hospitalization or during follow-up. An intermediate analysis for futility showed some differences between the control and treatment groups (Z = 0.0284). We study the probability distribution function of the long-time values of observables being time-evolved by Hamiltonians modeling clean and disordered one-dimensional chains of many spin-1/2 particles. In particular, we analyze the return probability and its version for a completely extended initial state, the so-called spectral form factor. We complement our analysis with the spin autocorrelation and connected spin-spin correlation functions, both of interest in experiments with quantum simulators. We show that the distribution function has a universal shape provided the central limit theorem holds. Explicitly, the shape is exponential for the return probability and spectral form factor, meanwhile it is Gaussian for the few-body observables. We also discuss implications over the so-called many-body localization. Remarkably, our approach requires only a single sample of the dynamics and small system sizes, which could be quite advantageous when dealing specially with disordered systems.

Synthetic opioids such as fentanyl account for over 71,000 of the approximately 107,000 overdose deaths reported in the United States in 2021. Fentanyl remains the fourth most identified drug by state and local forensic laboratories, and the second most identified drug by federal laboratories. The unambiguous identification of fentanyl-related substances (FRS) is challenging due to the absence or low abundance of a molecular ion in a typical gas chromatography-mass spectrometry (GC-MS) analysis and due to a low number of fragment ions that are similar among the many potential isomers of FRS. This study describes the utility of a previously reported gas chromatography-infrared (GC-IR) library for the identification of FRS within a blind, interlaboratory study (ILS) involving seven forensic laboratories. Twenty FRS reference materials, including those with isomer pairs in the library, were selected based on either their presence in the NIST library and/or some similarity of the mass spectra information produced. The ILS participants were requested to use the Florida International University (FIU) GC-MS and GC-IR libraries supplied by FIU to search for matches to their unknown spectra generated from in-house GC-MS and GC-IR analysis. The laboratories reported improvement in the positive identification of unknown FRS from ~75% using GC-MS alone to 100% correct identification using GC-IR analysis. One laboratory participant used solid phase IR analysis, which produced spectra incompatible with the vapor phase GC-IR library to generate a good comparison spectrum. Objective: This study aimed to evaluate whether early vitamin C and thiamine administration was associated with a lower 28-day and in-hospital mortality in surgical critically ill patients with refractory septic shock., Methods: We performed a retrospective before-and-after study on patients with refractory septic shock. According to local protocol, hydrocortisone is initiated in case of refractory septic shock. In January 2017, the protocol was changed and vitamin C and thiamine were included. Patients who were admitted in 2015-2016 and 2017-2018 were included in the control and treatment groups, respectively. The primary end point was 28-day and in-hospital mortality. Secondary end points were ICU mortality, ICU and hospital length of stay, duration of vasopressors and mechanical ventilation, use of renal replacement therapy (RRT), and the modification in serum procalcitonin and SOFA score during the first 72 h., Results: A total of 120 patients were included (58 in the treatment group and 62 in the control group). Log-rank test in Kaplan-Meier curves showed lower 28-day and in-hospital mortality over time in the treatment group (p=0.021 and p=0.035, respectively) but it not reached statistical significance in ICU mortality over time (p=0.100). The need of RRT was less frequent in treatment group (17.2% vs. 37.1%, p=0.024). There were no differences in other secondary outcomes., Conclusions: Intravenous vitamin C and thiamine administration in surgical patients with refractory septic shock may be associated with a lower 28-day and in-hospital mortality. This position paper describes the most relevant and essential aspects of a comprehensive, multidimensional assessment of hospitalized elderly people. The change in demographic patterns and the epidemiological profiles of diseases makes it necessary for internal medicine departments to adapt in order to take into account the vulnerabilities of the elderly in this context. A comprehensive, multidimensional assessment and the multidisciplinary development of a care plan during hospitalization can have an impact in terms of preventing mortality, disability, and institutionalization at discharge. It is necessary for all internists to acquire skills to improve the hospitalization experience in the elderly and obtain better health outcomes in our patients. In a disordered system, a quantity is self-averaging when the ratio between its variance for disorder realizations and the square of its mean decreases as the system size increases. Here, we consider a chaotic disordered many-body quantum system and search for a relationship between self-averaging behavior and the properties of the distributions over disorder realizations of various quantities and at different timescales. An exponential distribution, as found for the survival probability at long times, explains its lack of self-averaging, since the mean and the dispersion are equal. Gaussian distributions, however, are obtained for both self-averaging and non-self-averaging quantities. Our studies show also that one can make conclusions about the self-averaging behavior of one quantity based on the distribution of another related quantity. This strategy allows for semianalytical results, and thus circumvents the limitations of numerical scaling analysis, which are restricted to few system sizes.

Over the course of 2018 and 2019, the Spanish Society of Internal Medicine carried out a project called "The Future Hospital." Based on cumulative knowledge on the Spanish National Health System, this project seeks to transfer the observations on the organization of healthcare in future hospitals made by the Royal College of Physicians in the United Kingdom to the context of the Spanish healthcare system. The project's participants included numerous scientific and medical societies, professional associations in the health sector, and patient associations. Very limited labelled indications have been approved for the newer antimicrobials. Data on the clinical uses, efficacy and safety of dalbavancin are scarce, thus here we sought to describe our clinical experience. 16-month observational prospective study was performed. 19 (86%) were used under off-label indications. 10 (46%) for osteoarticular infections, 5 (23%) bloodstream infections and 3 (14%) endocarditis. To highlight, one patient received dalbavancin as long-term suppressive therapy. Most frequent use reasons were promptly hospital discharge, 11 (65%), and the presence of resistant organisms involving limited treatment options, 5 (23%). Successful outcome was observed in >95% of the patients and only 1 (4.5%) adverse event was reported. Further evidence beyond labelled indications is urgently needed. In this study we apply two methods for data collection that are relatively new in the field of atmospheric science. The two developed methods are designed to collect essential geo-localized information to be used as input data for a high resolution emission inventory for residential wood combustion (RWC). The first method is a webcrawler that extracts openly online available real estate data in a systematic way, and thereafter structures them for analysis. The webcrawler reads online Norwegian real estate advertisements and it collects the geo-position of the dwellings. Dwellings are classified according to the type (e.g., apartment, detached house) they belong to and the heating systems they are equipped with. The second method is a model trained for image recognition and classification based on machine learning techniques. The images from the real estate advertisements are collected and processed to identify wood burning installations, which are automatically classified according to the three classes used in official statistics, i.e., open fireplaces, stoves produced before 1998 and stoves produced after 1998. The model recognizes and classifies the wood appliances with a precision of 81%, 85% and 91% for open fireplaces, old stoves and new stoves, respectively. Emission factors are heavily dependent on technology and this information is therefore essential for determining accurate emissions. The collected data are compared with existing information from the statistical register at county and national level in Norway. The comparison shows good agreement for the proportion of residential heating systems between the webcrawled data and the official statistics. The high resolution and level of detail of the extracted data show the value of open data to improve emission inventories. Polypathological patients have specific clinical, functional, psychoaffective, social, family and spiritual characteristics. These patients are generally elderly and frail and have frequent decompensations. They frequently use healthcare resources, have significant functional impairment and have a high index of dependence. This results in a significant social impact, high mortality and a high consumption of resources. The current healthcare models have not answered these needs, which causes problems with accessibility to healthcare services, a lack of coordination among these services, a higher probability of adverse events related to polypharmacy and a high consumption of resources. In the past decade, the healthcare models have changed and are characterized by work in multidisciplinary and interlevel teams, patient self-care, the availability of tools for decision making, information and communication systems and prevention. The goal is to have prepared and proactive health teams and an informed and active patient population. The assessment of health results, processes and the costs for these programs is still based on moderate to low evidence. Myelosuppression secondary to chemotherapy remains a serious adverse effect of cancer therapy that causes high morbidity and mortality. Several current European and American guidelines recommend consideration of primary prophylaxis with colony-stimulating factors (CSFs) when the risk of febrile neutropenia is higher than 20 %. The main factors associated with a high risk of febrile neutropenia include the chemotherapy regimen, tumor type, and patient-related factors such as old age and/or comorbidities. The purpose of this paper is to summarize the most relevant clinical trials and updated recommendations of the main guidelines on the role of granulocyte colony-stimulating factors (G-CSFs) in febrile neutropenia, examining whether the combination of G-CSF with chemotherapy improves overall survival. Future directions for G-CSF use are also discussed.

Adequate radioprotection of the environment requires the identification of biomonitors sensitive to the variation of its radionuclide content. Due to the chemical similarities between calcium and strontium, calcified tissues of mammals are considered to be good ⁹⁰Sr biomonitors. This work considered Cervus elaphus antlers which, being shed annually, can give information about the importance of radiostrontium contamination in an ecosystem in the time period required for the growth of the antler. The samples were collected at various points of W and SW Spain. The mean value of their ⁹⁰Sr content was (70 ± 43 (S.D.)) Bq/kg d.w., range (16-218) Bq/kg d.w., and the radionuclide was evenly distributed in the different parts of the antler. There was a good correlation between the antlers' ⁹⁰Sr content and the ⁹⁰Sr deposited in the soil. The antlers' content of ²²⁶Ra (from the natural uranium series) and the contents of some stable elements (Ca, Mg, Sr, and K) were also determined. Objective: To analyze adverse drug reaction (ADR) detection using the Minimum Basic Data Set (MBDS) at hospital discharge and to compare the ADR reporting rate to the Pharmacovigilance Referral Centre with other similar hospitals that do not use this reporting system. Setting 650-bed University Hospital serving a population of 294,000 inhabitants in Spain., Method: A retrospective descriptive study was conducted between January 2006 and December 2007. All reports of ADRs gathered in MBDS (a tool that encodes all administrative and clinical information generated for each patient during a hospitalization episode) with International Classification Disease codes between E930 and E949.9 were analyzed to assess the appropriateness of their referral to the pharmacovigilance centre. Finally, we compared our reporting rate with other hospitals that do not use this system for ADR identification., Main Outcome Measure: The incidence of ADRs detected in hospitalized patients and the reporting rate (per thousand inhabitants) to the referral pharmacovigilance centre using the Yellow Card system., Results: Out of 43,282 hospital discharges, 386 ADR were recorded (0.89% of hospitalized patients). The mean (+/-SD) age of patients with reported ADR was 61.9 years (+/-19.2), median age was 65 years, and 55.2% were female. The Department of Pharmacy reported 276 (71.5%) of ADR using the Yellow Card system. The most frequently reported drugs were anti-cancer agents (42.5%) and cardiovascular drugs (23.8%), with a high frequency of digitalis glycosides (18.4%). ADR were most frequently recorded by the Departments of Oncology (41.7%) and Internal Medicine (17.9%)., Conclusion: The MBDS is a useful and accessible instrument to determine the incidence of ADR in a hospital, resulting in the notification of severe events that might otherwise not be reported. Its use also improves identification of the main drugs responsible for ADR and of the patient populations at greatest risk, facilitating the implementation of alert systems and the development of prevention and detection strategies.

The patients being treated in our health care system are becoming increasingly older and have a greater prevalence of chronic diseases. Due to these factors, these patients require greater and easier accessibility to the system as well as continuity of medical care. Collaboration between the different levels of health care has been instrumental in the success of the system and has produced changes in the hospital medical care protocol. Our hospital has developed a care model oriented towards the patient's needs, resulting in a higher grade of satisfaction among the medical professionals. In this paper, we have given a detailed description of part of our medical model, illustrating its different components and indicating several parameters of its evaluation. We have also reviewed the current state of the various models published on this topic. In summary, we believe that this medical care model presents a different approach to management that benefits patients, medical professionals and the health system alike.

Botrytis cinerea is a phytopathogenic fungus causing disease in a substantial number of economically important crops. In an attempt to identify putative fungal virulence factors, the two-dimensional gel electrophoresis (2-DE) protein profile from two B. cinerea strains differing in virulence and toxin production were compared. Protein extracts from fungal mycelium obtained by tissue homogenization were analyzed. The mycelial 2-DE protein profile revealed the existence of qualitative and quantitative differences between the analyzed strains. The lack of genomic data from B. cinerea required the use of peptide fragmentation data from MALDI-TOF/TOF and ESI ion trap for protein identification, resulting in the identification of 27 protein spots. A significant number of spots were identified as malate dehydrogenase (MDH) and glyceraldehyde-3-phosphate dehydrogenase (GAPDH). The different expression patterns revealed by some of the identified proteins could be ascribed to differences in virulence between strains. Our results indicate that proteomic analysis are becoming an important tool to be used as a starting point for identifying new pathogenicity factors, therapeutic targets and for basic research on this plant pathogen in the postgenomic era.

Botrytis cinerea is a phytopathogenic fungi causing disease in a number of important crops. It is considered a very complex species in which different populations seem to be adapted to different hosts. In order to characterize fungal virulence factors, a proteomic research was started. A protocol for protein extraction from mycelium tissue, with protein separation by 2-DE and MS analysis, was optimised as a first approach to defining the B. cinerea proteome. Around 400 spots were detected in 2-DE CBB-stained gels, covering the 5.4-7.7 pH and 14-85 kDa ranges. The averages of analytical and biological coefficients of variance for 64 independent spots were 16.1% and 37.5%, respectively. Twenty-two protein spots were identified by MALDI-TOF or ESI IT MS/MS, with some of them corresponding to forms of malate dehydrogenase and glyceraldehyde-3-phosphate dehydrogenase. Two more spots matched a cyclophilin and a protein with an unknown function.

The C825T polymorphism of the beta-3 subunit of the protein G (GNB3) has been related to an increased activity of the Na+/H+ exchanger (NHE-1) through the synthesis of an anomalous hyperactive protein. Because of the important role of this system in essential hypertension (EH), we analysed the distribution of the different genotypes of this polymorphism in normotensive subjects (NS) and essential hypertensive patients (EHP), their relationship with the condition of salt sensitivity, plasma sodium and potassium concentrations and plasma renin activity (PRA) in EHP. 144 subjects (78 EHP and 76 NS) were studied. Salt sensitivity was assessed by the rapid protocol of Weinberger and genotype determination for GNB3 C825T polymorphism was performed by PCR. The distribution of the different genotypes was similar among EHP (CC 37.2%; CT 41.1%; TT 16.7%) and NS (CC 32.9%; CT 55.3%; TT 11.8%). In regard to general characteristics of EHP (including blood pressure levels) and the condition of salt sensitivity, there were no differences among the different genotypes. Plasma sodium concentration was higher and plasma potassium was lower in TT patients (141.0+/-1.7 and 3.7+/-0.1) than in CC patients (139.1+/-1.9 and 4.0+/-0.3) p<0.05. CT patients had intermediate values (139.9+/-1.9 and 3.9+/-0.2). PRA values were similar in the three genotypes as were the rest of analytical parameters studied. Our data demonstrate an association between the C825T polymorphism of the GNB3 and plasma sodium and potassium concentrations in EHP, as expression of an increase in NHE-1 activity, without modifications in PRA nor relationship with salt sensitivity.

Background and Objective: The HELLP syndrome is a rare form of preeclampsia with a variable presentation with substantial maternal and perinatal morbidity and mortality. The aim of this study was to determine its clinical and biological characteristics in our hospital., Patients and Method: An observational study of all cases of confirmed HELLP syndrome from 1999 to 2002 was carried out., Results: There were 54 cases of HELLP syndrome. Mean maternal age was 30.06 (16-41) years. 57.4% were primiparous. Mean gestational age at diagnosis of HELLP syndrome was 31.75 (20-41) weeks. Symptoms and signs were dominated by the digestive ones. Hypertension was observed in all cases. 77.8% pregnancies were delivered by caesarean section. Mean birth weight was 1674.52 g (150-3800 g). There were 12 perinatal deaths and one maternal death., Conclusions: HELLP syndrome is an uncommon but potentially serious complication of pregnancy which is associated with an increased risk of adverse maternal and fetal outcomes. Management should be multidisciplinary and based on strict control by maternal and fetal symptoms.

Approximately 50% of hypertensive patients are salt sensitive (they increase their Blood Pressure in response to sodium intake or volume expansion). Mechanisms underlying salt sensitivity are not completely elucidated although there is evidence that they may be genetically determined. The aim of this study is to establish the relation among some genetic polymorphisms of the renin-angiotensin system (RAAS) and the beta-3 subunit of the protein G and salt sensitivity. We studied 102 essential hypertensive patients, stage 1-2 and without target organ damage. Salt sensitivity was assessed by the rapid protocol of Weinberger. We determined by polymerase Chain reaction techniques the following polymorphisms: insertion/deletion (I/D) of the angiotensin-converting enzyme (ACE), A1166C of the angiotensin II type 1 receptor (AT1R), -344C/T and intron 2 conversion (IC) of the aldosterone synthase (CYP11B2), and C825T of the beta-3 subunit of the protein G (GNB3). 41 patients (40.19%) were salt sensitive. The distribution of the different polymorphisms was similar in both groups of patients, but subjects carriers of the W allele of the CYP11B2 IC polymorphism had a greater risk for salt sensitivity as compared with no carriers (37 of 41, 90.2% vs 4 of 41, 9.8%, OR 3.02, P<0.05). Although there is no association between salt sensitivity and the different studied genotypes of the RAAS and of the GNB3, our data show a greater risk for salt sensitivity among carriers of the W allele of the CYP11B2 1C polymorphism.

Ciliary neurotrophic factor and bone morphogenetic proteins induce astrocytogenesis in the developing rat brain by stimulating STAT- and Smad-dependent signaling, respectively. We previously found that stimulation of the cAMP-dependent signaling pathway also triggers differentiation of cerebral cortical precursor cells into astrocytes, providing an additional mechanism to promote astrocyte differentiation. In this study, we show that pituitary adenylate cyclase-activating polypeptide (PACAP), but not the related vasoactive intestinal peptide, induces astrocyte differentiation of cortical precursor cells, even after a transient exposure. Cortical precursors were found to express predominantly the short isoform of the PACAP-specific PAC1 receptor, which couples to adenylate cyclase. Consistent with this notion, we determined that exposure of cortical precursors to PACAP resulted in a dose-dependent increase in cAMP production. Pretreatment of cells with the cAMP antagonist Rp-cAMPS prevented astrocyte differentiation. Thus, PACAP acts as an extracellular signal to trigger cortical precursor cell differentiation into astrocytes via stimulation of intracellular cAMP production.

Background: Glucose effectiveness (SG) is a parameter that indicates the glucose ability to clearing itself from the plasma independently of insulin's action. Our purpose was to analyze the cluster characteristics associated with the metabolic syndrome in a group of non-obese, recent-onset hypertensives and to test if there was a correlation with SG and the effectiveness of glucose at basal insulin point (GEZI)., Patients and Method: We studied 36 patients with mild hypertension with normal basal glucose levels. We determined plasma lipid subfractions, apolipoproteins and urate levels. An intravenous glucose tolerance test (TTGI) and minimal model analysis according to Bergman was performed and SG, GEZI and insulin sensitivity (SI) were calculated., Results: Patients with lower SG and GEZI had higher levels of total triglycerides (Tg) (r = 0.42; p = 0.01 and r = 0.48; p = 0.002, respectively) and triglycerides bind to VLDL (Tg-VLDL) (r = 0.40; p < 0.01 and r = 0.49; p = 0.002, respectively). When the cluster of metabolic syndrome was analyzed, SG was inversely related to uric acid levels and to the waist-hip index. However, SI was only related to the uric acid levels (r = 0.38; p = 0.01)., Conclusions: In non-obese, recently diagnosed hypertensive patients, the SG parameter seems to be an early marker for the development of metabolic syndrome.

Botrytis cinerea is a plant-pathogenic fungus that produces the disease known as grey mould in a wide variety of agriculturally important hosts in many countries. Ten strains from different locations collected on different years have been isolated and characterized by several methods (morphological, biochemical, genetical and molecular). Results showed that clear morphological differences exist between strains, and showing a relationship between the presence of sclerotia and pathogenicity. The conidial size and the nuclear number were highly variable between different strains. Pulsed-field gel electrophoresis showed a unique karyotype for each strain, highly polymorphic between strains and with a number of bands ranging from 4 to 8. An efficient transformation system has been achieved through the plasmid pAMPF21, containing the region AMA1 of Aspergillus nidulans. Lastly, from a genomic library the gdhA gene has been cloned. This gene produces an RNAm of 1.7 Kb and complements the deficiency on glutamate dehydrogenase activity of A. nidulans.

In the developing brain, differentiation of neural precursors into neurons or glial cells occurs in response to neurotrophic factors acting on the cell surface. Intracellular signaling mechanisms that relay information to initiate differentiative responses of neural precursor cells are poorly understood. To investigate whether stimulation of the cAMP-dependent signaling pathway participates in differentiative responses of cells in the developing CNS, we performed experiments using both conditionally immortalized neural precursor cells (RC2.E10 cells) and primary cultures of cells from developing rat cortex. Initially, we determined that RC2.E10 cells retain phenotypic features of neural precursors after inactivation of the immortalizing oncogene, a temperature-sensitive mutant of the simian virus 40 large-T antigen (SV40T). We found that, once SV40T is inactivated, RC2.E10 cells cease to divide and die. However, RC2. E10 cells can proliferate in the presence of basic fibroblast growth factor. In addition, they express nestin, a marker of neural precursor cells. Both RC2.E10 cells and primary cortical precursor cells undergo astroglial differentiation in response to cAMP stimulation by treatment with 8-bromo-cAMP. In both cases, cAMP-induced astrocyte differentiation is characterized by morphological changes, stimulation of glial fibrillary acidic protein expression, downregulation of nestin expression, and decreased proliferation. No increases in the expression of neuronal or oligodendrocytic markers were observed. Our results support the notion that the developing CNS contains neural precursor cells with the capacity of undergoing astrocyte differentiation in response to increased intracellular cAMP concentrations.

Objective: Analysis of colorectal anastomosis stricture incidence after anterior resection of the rectum performed with the EEA-Stapler. To find out if differences existed in stricture incidence considering factors such, as age, neoplasia, postoperative radiotherapy, tumor stage and anastomic level., Design: Longitudinal descriptive study., Patients and Method: 67 patients who underwent rectal anterior resection using the EEA-Stapler were evaluated. Data from sex, age, indication for operation, postoperative radiotherapy, tumor staging and anastomic level were recorded and compared with presence of stricture anastomosis. Stenosis was evaluated and graded as follows: grade O, no stenosis; grade I, no symptoms, X-ray or endoscopic finding; grade II, symptoms, the patients require ballon catheter dilation; and grade III, invalidant symptoms, the patients require surgery., Results: Twelve patients (20%) were recorded as grade II and 3 patients (5%) as grade III. There were no statistically significant differences between prevalence of stricture and sex, age, neoplasic or non-neoplasic conditions, previous radiotherapy, level of anastomosis, and tumor stage., Conclusion: Stenosis after colorectal anastomosis with stapler devices must not be considered as an uncommon complication. In 20% of patients it may be a serious state that may require repeated catheter balloon dilations or surgery. Such condition is not dependent on diverse factors studied.

Visnadine, an active principle extracted from the fruit of Ammi visnaga, exhibits peripheral and coronary vasodilator activities and has been used for the treatment of angina pectoris. The present study was undertaken to further characterize the inhibitory effects of visnadine on the contractile responses in rat isolated aortic rings and portal vein segments. Visnadine (< 10(-5) M) selectively inhibited the contractions induced by depolarization with 80 mM KCl or by CaCl2 in KCl-depolarized aorta and the spontaneous activity of the portal vein. Its inhibitory effects were not increased as the time of depolarization was prolonged and were similar in aorta incubated in 5 or 40 mM KCl. At concentrations higher than 10(-5) M, visnadine also inhibited the contractile responses induced by noradrenaline and phorbol 12-myristate 13-acetate (PMA), being equipotent to inhibit noradrenaline-induced contractions in either Ca(2+)-containing or Ca(2+)-free medium and PMA-induced contractions. In conclusion, the present results suggest that visnadine preferentially inhibited the contractile responses mediated by Ca2+ entry through L-type Ca2+ channels, whereas at high concentrations it may also interfere with other sites involved in vascular smooth muscle contraction.

A transformation system has been developed for the pathogen fungus Botrytis cinerea, based on the utilization of the wide host plasmid pUT737 that contains the Sh ble gene, conferring resistance to phleomycin. Transformed protoplasts were regenerated at 10-25 micrograms ml(-1) of phleomycin, at a frequency of 25-40 transformants per microgram of DNA, and they were resistant up to 50 micrograms ml(-1). Southern hybridization using undigested and digested total DNA showed the presence of circular autonomously replicating plasmid pUT737 in the transformants. Reisolated plasmid from transformed fungus transformed E. coli and rescued plasmid was identified as PUT737. Transformants were grown for four generations under non-selective conditions and replicative plasmids were still detected. Plasmids present in all transformants at this stage had been modified from native pUT737 and showed the same size and configuration indicating that selection through stabilizing plasmid forms has happened.

Objectives: To study cardiopulmonary function in 30 ASA I and II patients undergoing laparoscopic cholecystectomy with CO2., Patients and Methods: Fifteen patients were given total anesthesia with propofol (group PRO) and 15 inhaled isoflurane (group ISO). In addition to the usual monitoring, we used esophageal Doppler ultrasonogram (ED) to study the hemodynamic status after pneumoperitoneum (NP) at 15 mmHg, after the patient had been placed in anti-Trendelenburg (AT) position, and 10, 20 and 30 minutes after NP (series 1). We also studied response to reduction of NP from 15 mmHg to 12, 10, 8 and 6 mmHg (series 2), repeating this sequence when duration of surgery permitted (series 3)., Results: In group PRO, cardiac index (CI) decreased 17.96% (NS) after NP and 24.90% (p = 0.015; r = 0.71) after AT. In group ISO, the decreases were 15.86% (p = 0.02; r = 0.69) and 22.34% (p = 0.02; r = 0.80), respectively. Correlated flow time (FTc) and peak velocity (PV) decreased, while the index of total peripheral resistance (TPRI) increased with NP and AT. Recovery of CI was gradual and spontaneous (series 1). The decreases in NP pressure did not produce significant improvement in IC., Conclusions: Induction of NP and placement in AT position causes significant decreases in CI as measured by ED in ASA I and II patients, whether they inhale the anesthetic agent or are given total intravenous anesthesia. This effect seems to be related to the increase in afterload, measured in this study by TPRI and the ratio PV/FTc.
