Abstract

IntroductionType 2 diabetes (T2D) is a global epidemic, and nations are struggling to implement effective healthcare strategies to reduce the burden. While efficacy studies demonstrate that metformin can reduce incident T2D by half among younger, obese adults with prediabetes, its real-world effectiveness are understudied, and its use for T2D prevention in primary care is low. We describe the design of a pragmatic trial to evaluate the incremental effectiveness of metformin, as an adjunct to a simple lifestyle counseling.MethodsThe "Prevención de la Diabetes con Ejercicio, Nutrición y Tratamiento" [Diabetes Prevention with Exercise, Nutrition and Treatment; PRuDENTE, (Spanish acronym)] is a cluster-randomized trial in Mexico City's public primary healthcare system. The study randomly assigns 51 clinics to deliver one of two interventions for 36 months: 1) lifestyle only; 2) lifestyle plus metformin, to 3060 patients ages 30-65 with impaired fasting glucose and obesity. The primary endpoint is incident T2D (fasting glucose ≥126 mg/dL, or HbA1c ≥6.5%). We will also measure a range of implementation-related process outcomes at the clinic-, clinician- and patient-levels to inform interpretations of effectiveness and enable efforts to refine, adapt, adopt and disseminate the model. We will also estimate the cost-effectiveness of metformin as an adjunct to lifestyle counseling in Mexico.DiscussionFindings from this pragmatic trial will generate new translational knowledge in Mexico and beyond, both with respect to metformin's real-world effectiveness among an 'at-risk' population, and uncovering facilitators and barriers to the reach, adoption and implementation of metformin preventive therapy in public primary care settings.Trial registrationThis trial is registered at Clinicaltrials.gov (NCT03194009).

26. Analysis of Educational Leadership at Rural Early-Childhood and Primary Schools: A Case Study in Teruel (Aragon, Spain)

Abstract

Rural schools are a distinctive feature in the autonomous community of Aragon, as state-run rural schools are the main cultural institution in small towns. This research was conducted in four grouped rural schools (hereafter referred to by their Spanish acronym, CRAs) in the province of Teruel in 2017. Our objectives included analising the views of management team members and teachers on educational leadership with a special emphasis on the rural school's differentiating elements. Four CRAs were chosen in the province of Teruel. Three teachers and six members of management teams participated and the study data were collected using a qualitative interview. The content analysis of the interview was conducted using the NVivo program and was organised into two fundamental areas: views of educational leadership and the skills this leadership includes. The general perception of management teams and teachers is positive; they value the effort put into performing the work and understand that both management and CRAs are complex. They also appreciate the management teams' organisation of training and its contents. This analysis suggests a set of conditions that provides a qualitative insight into management possibilities and limitations in the CRA model.

Abstract

This paper examines transit migration through the Darién Gap and the Mexico–Guatemala border, focusing on the commodification of migration, the transformative role of digital platforms, and the socio-economic impacts on local economies. Based on ethnographic fieldwork and in-depth interviews conducted from 2021 to 2023, we explore how migration has evolved into a commodified journey where services and safety are bought and sold, often exploiting migrants' vulnerabilities. We analyze how digital platforms have become essential tools for navigation and community building among migrants, yet also introduce new risks and inequalities. Our findings highlight the dual nature of local economic adaptations, which both capitalize on and are reshaped by the migratory flows, leading to significant economic and social transformations. The paper argues for a reevaluation of migration policies that integrates these elements and supports a migration governance framework that prioritizes human dignity and rights. Through exposing these dynamic and interlinked phenomena, we aim to contribute to a better understanding of migration that goes beyond traditional paradigms of border security and control, advocating for policies that are just, equitable, and respectful of human experiences and challenges. This review comprehensively explores some of the most prominent KGs, detailing their structure, data sources, and how they facilitate the repurposing of drugs. In addition to KGs, this paper delves into various artificial intelligence techniques that enhance the process of drug repurposing. These methods not only accelerate the identification of viable drug candidates but also improve the precision of predictions by leveraging complex datasets and advanced algorithms. Furthermore, the importance of explainability in drug repurposing is emphasized. Explainability methods are crucial as they provide insights into the reasoning behind AI-generated predictions, thereby increasing the trustworthiness and transparency of the repurposing process.
