Abstract

Background Surgical complications in pancreas transplantation are associated with increased morbidity and graft loss. The purpose of our study was to determine the frequency of re-operation in early and late postoperative of pancreas transplantation and its impact in relation to graft survival, patient survival, and hospital mortality. Methods We conducted a retrospective study of 238 pancreas transplants performed from 2001 to 2011. We analyzed surgical complications that led to the early and late re-operations by type of transplantation and its relation to pancreas graft survival, patient survival, and hospital mortality. Results We studied 61 re-operated patients (25.63%), 58 patients with early re-operation and 3 with late re-operation. The median of re-operations per patient was 1.48 (range, 1 to 5). Seventy-five surgical complications required 90 re-operations. The causes of early re-operations were bleeding (8.8%), pancreas graft thrombosis (6.3%), pancreatitis (2.9%), bowel obstruction (2.1%), leaks (2.1%), and evisceration (1.7%). The hospital mortality rate was 0.43%. Pancreas graft survival at 1 and 5 years is higher in the group of non–re-operated than in the re-operated group (98% vs 74% and 93% vs 57%; P = .0001). Patient survival at 1 year is slightly higher in the non–re-operated group, but with no significant differences (99% vs 97%), and at 5 years the patient survival in the non–re-operated group is higher than in the re-operated group (98% vs 91%; P = .011). Conclusions Surgical complications required re-operations that were associated with higher morbidity and lower pancreas graft and patient survival. [ABSTRACT FROM AUTHOR]

37. Endothelin mediated nitric oxide effects in ischemia--reperfusion associated with pancreas transplantation.

Abstract

Formation of nitric oxide (NO) in ischemia-reperfusion (I-R) associated with pancreas transplantation could modulate the inflammatory response. In this sense, previous studies have demonstrated the action of NO on vasoactive substances like prostacyclin or endothelin. The present study was designed to evaluate the contribution of endothelin to the inflammatory events induced by NO in the I-R process associated with pancreas transplantation. For this purpose, pancreatic levels of endothelin, neutrophil infiltration, and prostacyclin were evaluated in an experimental model of pancreas transplantation after inhibition of NO synthesis or after NO inhibition plus addition of endothelin. Results show significant posttransplantation increases in endothelin, neutrophil infiltration, and prostacyclin production. These increases were prevented by NO inhibition. Endothelin administration plus nitric oxide inhibition reversed this effect, resulting in an increase in myeloperoxidase and 6-keto-prostaglandin F1alpha. These results suggest that the proinflammatory effects of NO in I-R associated with pancreas transplantation are mediated by the induction of endothelin generation. [ABSTRACT FROM AUTHOR]

Abstract

The role of nitric oxide, produced during reperfusion as a function of preservation time, in the development of the inflammatory process in pancreas transplantation has been explored. For this purpose, the effect of nitric oxide synthase inhibition, as well as 6-keto-prostaglandin F1alpha, leukotriene B4, and lipoperoxidation levels were evaluated in an experimental model of rat pancreas transplantation after different periods of cold preservation. The results show posttransplantation increases in 6-keto-prostaglandin F1alpha, leukotriene B4, and lipoperoxidation levels in pancreatic tissue and in plasma lipase. When ischemia was induced for 30 min, nitric oxide synthase inhibition prevented these increases, and L-arginine was able to reverse this effect. By contrast, nitric oxide synthase inhibition has no effect when ischemia was prolonged for 12 hr. In summary, this study suggests that, during reperfusion, nitric oxide modulates 6-keto-prostaglandin F1alpha synthesis, lipoperoxidation levels, and the development of pancreatic injury but only when the ischemic period is quite short. 43. Has double-phase MIBI scintigraphy usefulness in the diagnosis of hyperparathyroidism?

Abstract

The usefulness of double-phase parathyroid technetium-99m-MIBI scintigraphy for the detection of hyperplastic parathyroid tissue has been described. The aim of the present study was to establish the effectiveness of this new technique in the morphological and functional assessment of parathyroid glands in patients with different types of hyperparathyroidism. We performed 99mTc-MIBI scintigraphy (MIBI) and neck ultrasonography in 38 patients with primary (n=16) or secondary (n=22) hyperparathyroidism. All patients underwent surgical neck exploration, removing a total of 84 parathyroid glands. Before and after surgery, blood intact parathyroid hormone (iPTH) was measured peripherally and in both the right and left internal jugular veins. In patients with primary hyperparathyroidism, ultrasonography showed one enlarged gland in 11 cases (69%), while MIBI was positive in 15 (94%) (including two ectopic glands). The sensitivity of MIBI (93%) was greater than that of ultrasonography (68%), with a similar specificity (100 and 97%. respectively). In patients with secondary hyperparathyroidism, there was a discrepancy between both imaging modalities in 29 glands (33%). The sensitivity of both techniques was similar (41 and 54%, respectively), with the same specificity (89%). There were more difficulties in detecting the upper than the lower pathological glands. MIBI reflected more accurately the functionality of the glands, and ultrasonography has a better correlation with the volume and weight. In conclusion, Tc-99m-MIBI scintigraphy is a good technique to identify parathyroid hyperfunctioning tissue in cases of primary hyperparathyroidism and to detect ectopic glands, but it does not give significantly better results than conventional ultrasonography in patients with secondary hyperparathyroidism.
