ObjectivesThis systematic review aimed to verify if diabetes affects vertical and horizontal ridge augmentation in individuals undergoing dental implant treatment with guided bone regeneration (GBR).MethodsFive databases were systematically screened up to September 2023, according to predefined eligibility criteria. The methodological risk of bias of the included studies was assessed using the ROBINS-I tool, and GRADE was used to evaluate the certainty of evidence. Random-effects pairwise meta-analyses were used to compare changes in vertical (height) and horizontal (width) alveolar bone dimensions between individuals exposed and unexposed to diabetes, through standardized mean differences (SMDs).ResultsThree non-randomized controlled trials met the inclusion criteria, all of which showed a serious risk of bias. The results showed, with overall very low certainty on evidence, that individuals with diabetes did not exhibit a significant difference in horizontal (SMD = −0.41, 95% CI: −0.92–0.10) and vertical (SMD = 0.06, 95% CI: −0.43–0.56) ridge augmentation compared to the those without diabetes.ConclusionsThe available evidence, albeit of very low certainty, suggests that diabetic individuals with moderate or good glycemic control undergoing dental implants and GBR show comparable horizontal and vertical bone gains to their unexposed counterparts. Abstract Laboratory evidence has demonstrated the antimicrobial effect of Melaleuca alternifolia (MEL) against oral microorganisms. This randomized, double-blind, crossover clinical trial, compared the anti-biofilm and anti-inflammatory effects of MEL nanoparticles with 0.12% chlorhexidine gluconate (CHX) on biofilm-free (BF) and biofilm-covered (BC) surfaces. Before each experimental period, the participants refrained from all oral hygiene practices for 72 hours. The 60 participants were randomly assigned to professional prophylaxis in two quadrants (Q1–Q3 or Q2–Q4), and rinsed with MEL or CHX for four days. The Quigley & Hein plaque index (QHPI), gingival crevicular fluid (GCF) volume, and participants’ perceptions were assessed. CHX showed significantly lower mean QHPI on BF (2.65 ± 0.34 vs. 3.34 ± 0.33, p < 0.05) and BC surfaces (2.84 ± 0.37 vs. 3.37 ± 0.33, p < 0.05). Intragroup comparisons indicated reductions in GCF in all the groups, with significant differences only for CHX on BF surfaces (p < 0.05). Intergroup comparisons revealed no significant differences (p > 0.05). Based on individual perceptions, CHX had better taste and biofilm control, but resulted in a greater change in taste. Nevertheless, MEL demonstrated anti-inflammatory effects similar to those of CHX. Further clinical trials testing different protocols, concentrations and follow-up periods are required to establish its clinical application.

ABSTRACT OBJECTIVE To describe the sampling strategy of an epidemiological survey with the aid of satellite images, including details of the multistage probability sampling process. METHODS A probability sample of individuals living in the rural area of Rosário do Sul, state of Rio Grande do Sul, Brazil, aged 15 years old or more, was evaluated. Participants answered questionnaires (medical history, sociodemographic characteristics, habits, alcohol use, quality of life, stress, rumination, and self-perceived periodontal diseases), and were subjected to clinical oral examinations as well as anthropometric measurements (blood pressure, height, weight, abdominal and waist circumferences). Oral evaluation comprehended a complete periodontal exam at six sites per tooth, including the following assessments: furcation involvement; dental abrasion; tooth decay, including the indexing of missing and filled surfaces; O’Brien index; gingival abrasion; oral cavity and lip lesions; complete periapical radiographic exam, and use of prostheses. Besides this oral clinical approach, subgingival plaque, crevicular gingival fluid, saliva, and blood samples were collected. Examiners were trained and calibrated during previous evaluations. A pilot study allowed the logistic of the performed exams to be adjusted as needed. RESULTS Among 1,087 eligible individuals, 688 were examined (63.3%). Age, sex, and skin color data were compared to data from the last demographic census (2010) of the Brazilian Institute of Geography and Statistics, which served to validate the sampling strategy. CONCLUSIONS The careful methods used in this study, in which satellite images were used in the delimitation of epidemiological areas, ensure the quality of the estimates obtained and allow for these estimates to be used in oral health surveillance and health policies improvements.

El presente estudio tuvo como objetivo identificar factores asociados a violencia en estudiantes de escuelas públicas de la región central del Río Grande do Sul. Estudio observacional, analítico y transversal. La muestra estuvo conformada por 435 estudiantes de quinto grado de escuela primaria hasta el nivel secundario. Los datos fueron recolectados por medio de entrevistas utilizando cuestionario estructurado. Las asociaciones entre variables dependientes e independientes se tomaron del modelo de regresión de Poisson. El análisis de datos indicó un aumentó en la prevalencia entre violencia y variables de sexo masculino, mayor número de hermanos y bajos ingresos, mientras que la religión se asoció como indicador de protección de violencia psicológica. Se concluye que los enfoques intersectoriales, interdisciplinarios y multidimensionales se revelan como estrategias para acciones de enfrentamiento a la violencia. Algunas estrategias específicas de intervenciones de enfermería son indicadas para prevenir los riesgos asociados a violencia.

OBJECTIVE: The aim of this study was to investigate the association among gingival enlargement (GE), periodontal conditions and socio-demographic characteristics in subjects undergoing fixed orthodontic treatment. METHODS: A sample of 330 patients undergoing fixed orthodontic treatment for at least 6 months were examined by a single calibrated examiner for plaque and gingival indexes, probing pocket depth, clinical attachment loss and gingival enlargement. Socio-economic background, orthodontic treatment duration and use of dental floss were assessed by oral interviews. Associations were assessed by means of unadjusted and adjusted Poisson's regression models. RESULTS: The presence of gingival bleeding (RR 1.01; 95% CI 1.00-1.01) and excess resin around brackets (RR 1.02; 95% CI 1.02-1.03) were associated with an increase in GE. No associations were found between socio-demographic characteristics and GE. CONCLUSION: Proximal anterior gingival bleeding and excess resin around brackets are associated with higher levels of anterior gingival enlargement in subjects under orthodontic treatment.

ABSTRACT This study evaluated the clinical diagnosis of proximal gingivitis by comparing two methods: dental flossing and the gingival bleeding index (GBI). One hundred subjects (aged at least 18 years, with 15% of positive proximal sites for GBI, without proximal attachment loss) were randomized into five evaluation protocols. Each protocol consisted of two assessments with a 10-minute interval between them: first GBI/second floss, first floss/second GBI, first GBI/second GBI, first tooth floss/second floss, and first gum floss-second floss. The dental floss was slid against the tooth surface (TF) and the gingival tissue (GF). The evaluated proximal sites should present teeth with established point of contact and probing depth ≤ 3mm. One trained and calibrated examiner performed all the assessments. The mean percentages of agreement and disagreement were calculated for the sites with gingival bleeding in both evaluation methods (GBI and flossing). The primary outcome was the percentage of disagreement between the assessments in the different protocols. The data were analyzed by one-way ANOVA, McNemar, chi-square and Tukey’s post hoc tests, with a 5% significance level. When gingivitis was absent in the first assessment (negative GBI), bleeding was detected in the second assessment by TF and GF in 41.7% (p < 0.001) and 50.7% (p < 0.001) of the sites, respectively. In the absence of gingivitis in the second assessment (negative GBI), TF and GF detected bleeding in the first assessment in 38.9% (p = 0.004) and 58.3% (p < 0.001) of the sites, respectively. TF and GF appears to be a better diagnostic indicator of proximal gingivitis than GBI.

There is little material in the literature that compares biological width measurements in periapical and bite-wings radiographs with clinical measurements. The purpose of this study was to compare measurements of biological width taken by three different methods which are frequently used for planning periodontal surgery - periapical radiograph, bite-wing radiograph and transperiodontal probing - with the trans-surgical measurements. Thirty-four sites from twenty-one subjects were analyzed. The intra-class correlation coefficients between measurements obtained trans-surgically (gold standard) and those obtained by transperiodontal probing, periapical radiography and bite-wing radiography were determined. Average measurements were compared using the Wilcoxon test at a significance level of 0.05. Also, the frequency distribution of differences between test measurements and the gold standard was calculated. The results showed that transperiodontal probing (mean 2.05 mm) was the most accurate measurement, as compared to the gold standard (mean 1.97 mm), with no statistically significant difference observed. On the other hand, periapical and bite-wing radiographic mean values (1.56 mm and 1.72 mm, respectively) were smaller than the gold standard, with statistically significant differences (p < 0.05). It was concluded that transperiodontal probing was the most accurate measurement, as compared to the gold standard, followed by that obtained with the bite-wing radiograph. The clinical relevance of these results could be that planning for crown lengthening surgery should, preferably, include transperiodontal probing.

When dealing with patients with periodontal disease of variable severities, dentists must often choose between treating and restoring the involved tooth or indicating its extraction. Different criteria have been adopted in this decision-making process. The purpose of this study was to evaluate the criteria adopted by dentists to indicate the extraction of teeth with periodontitis. Dentists were interviewed at their private practices in three cities of the state of Rio Grande do Sul, Brazil. The evaluated criteria included severity of attachment loss, tooth mobility, furcation involvement, prosthetic planning, periodontal-endodontic lesion, possible systemic involvement due to the presence of periodontitis, referral to a periodontist for evaluation, radiographic bone loss greater than 50%, presence of extensive caries, socio-economic and cultural status of the patient, among others. The most often adopted criteria to indicate the extraction of periodontally affected teeth were the presence of mobility (37.5%), severity of attachment loss (24.3%) and radiographic bone loss greater than 50% (21.2%). The results of the present study demonstrated the difficulties faced by dentists to indicate the extraction of teeth with severe attachment loss, in addition to the establishment of an adequate prognosis. Aspects associated with the past disease were still the most often reported to indicate the extraction of teeth for periodontal reasons.

Aim: Previous studies have shown excellent prognosis with short implants. However, evidence of short implants rehabilitated with single-unit restorations is scarce. The purpose of this study was to evaluate the percentage of success of short implants in posterior maxillary and mandibular regions with single-unit restorations. Methods: This study comprised a retrospective analysis of data of 41 patients that received 54 short implants between 2004 and 2009. All implants were placed by the same surgeon and all restorations were performed by experienced professionals. All implants were placed in sites with reduced bone volume; 48 implants were evaluated by computed tomography and 6 implants by radiography. Failures and biological complications were analyzed. Results: It was observed a success rate of 96.3% in the osseointegration period and 100% in the load period that ranging from 7 to 38 months. Two complications were observed, which were successfully treated without risk of implant loss. Conclusions: Short implants in posterior regions rehabilitated with single-unit restorations are a treatment option with high success rate and thus may be indicated in posterior regions with reduced bone height.

Abstract Objective To compare the antimicrobial efficacy of a 0.12% chlorhexidine (CHX) and herbal green tea (Camellia sinensis) solution on established biofilms formed at different oxygen tensions in an in situ model. Method Twenty-five dental students were eligible for the study. In situ devices with standardized enamel specimens (ES) facing the palatal and buccal sides were inserted in the mouths of volunteers for a 7 day period. No agent was applied during the first four days. From the fifth day onward, both agents were applied to the test ES group and no agent was applied to the control ES group. After 7 days the ES fragments were removed from the devices, sonicated, plated on agar, and incubated for 24 h at 37 °C to determine and quantify the colony forming units (CFUs). Result CHX had significantly higher efficacy compared to green tea on the buccal (1330 vs. 2170 CFU/µL) and palatal (2250 vs. 2520 CFU/µL) ES. In addition, intragroup comparisons showed significantly higher efficacy in buccal ES over palatal ES (1330 vs. 2250 CFU/µL for CHX and 2170 vs, 2520 CFU/µL for CV) for both solutions. Analysis of the ES controls showed significantly higher biofilm formation in palatal ES compared to buccal ES. Conclusion CHX has higher efficacy than green tea on 4-day biofilms. The efficacy of both agents was reduced for biofilms grown in a low oxygen tension environment. Therefore, the oxygen tension environment seems to influence the efficacy of the tested agents.

ABSTRACT Objective: This two-centre cross-sectional study aimed to evaluate whether xerostomia occurrence is associated with oral health-related quality of life (OHRQoL) in patients with end-stage renal disease (ESRD) after the adjustment for potential confounders. Methods: Oral examinations were performed by calibrated examiners for untreated dental caries, periodontitis and tooth loss in 180 adults with ESRD. The presence of xerostomia was determined using the global question “How often does your mouth feel dry?”. OHRQoL was evaluated by the simplified version of the Oral Health Impact Profile (OHIP14) questionnaire. Multivariate zero-inflated negative binomial regression analysis was used to calculate the incidence rate ratios (IRR) for the nonzero scores and odds ratios (OR) of having no impact in OHIP14 scores according to the presence of exposure. Results: In the adjusted model, xerostomia (IRR = 1.57; 95% CI: 1.12 to 2.20) was associated with poorer OHRQoL. The adjusted domain-specific analysis revealed that xerostomia occurrence significantly impacted the psychological disability and social disability, and the chance of having no impact was lower for the psychological discomfort domain (OR = 0.84; 95% CI: 0.12 to 0.98). Conclusion: Xerostomia exert an impact on OHRQoL in patients with ESRD, mainly in the psychological and social disabilities constructs.

OBJECTIVES: Studies concerning side effects of chlorhexidine as related to the presence of plaque are scarce. The purpose of this study was to compare the side effects of 0.12% chlorhexidine gluconate (CHX) on previously plaque-free (control group) and plaque-covered surfaces (test group). METHODS: This study had a single-blind, randomized, split-mouth, 21 days-experimental gingivitis design, including 20 individuals who abandoned all mechanical plaque control methods during 25 days. After 4 days of plaque accumulation, the individuals had 2 randomized quadrants cleaned, remaining 2 quadrants with plaque-covered dental surfaces. The aim of this double-blind, controlled, split-mouth designed clinical trial was to assess the effect of a single application of Gluma Desensitizer® on alleviating dentin hypersensitivity. Twelve subjects entered the study and ten completed the protocol. Each subject had two teeth treated: one with Gluma Desensitizer® according to the manufacturer's instructions and one with water. The assessment of pain was performed with the VAS (Visual Analogue Scale), after tactile (probe), thermal (cold blast of water) and thermal/evaporative (cold blast of air) stimuli at baseline, immediately after treatment, after 1 week and after 4 weeks. The mean VAS values for the test and control teeth were compared by the paired t test (alpha = 0.05). Repeated measurements ANOVA was used to compare the different experimental times. The results showed that for test teeth, at baseline, mean VAS values were 1.76 (± 2.82), 7.10 (± 2.10) and 4.75 (± 2.65), and, after 4 weeks, the mean values were 1.70 (± 2.31), 5.50 (± 3.30) and 4.61 (± 3.14), respectively for probe, water and air stimuli. For the control teeth, at baseline, the mean VAS values were 1.86 (± 2.92), 6.61 (± 2.31) and 4.08 (± 2.91) and, after 4 weeks, 2.66 (± 3.07), 6.32 (± 2.94) e 4.76 (± 3.26).There were no statistically significant differences between test and control teeth at any time. No intra-group differences were demonstrated either. It was concluded that Gluma Desensitizer® had no effect on hypersensitive teeth from periodontally treated patients for a period up to 4 weeks.O objetivo deste estudo clínico duplo-cego, controlado, de boca-dividida foi avaliar o efeito de uma aplicação única do Gluma Desensitizer® no alívio da hipersensibilidade dentinária. Doze sujeitos participaram do estudo, sendo que destes dez completaram o estudo. Cada um dos participantes teve dois dentes tratados, um com o Gluma Desensitizer®, de acordo com as orientações do fabricante, e o outro com água. Para a avaliação da dor foi utilizada a EVA (Escala Visual Analógica), após os estímulos táctil (sonda), térmico (jato de água frio) e térmico/evaporativo (jato de ar frio) no início do experimento, imediatamente após o tratamento, uma e quatro semanas após. Os valores médios encontrados na EVA foram comparados pelo teste-t pareado (alfa = 0,05). Para os diferentes tempos experimentais utilizou-se ANOVA para medidas repetidas. Os resultados mostram que, no início do tratamento, os valores médios foram de 1,76 (± 2,82), 7,10 (± 2,10) e 4,75 (± 2,65) para os dentes teste e, após quatro semanas, os valores médios foram de 1,70 (± 2,31), 5,50 (± 3,30) e 4,61 (± 3,14) respectivamente para os estímulos sonda, água e ar. Para os dentes controle, no início do tratamento, os valores médios segundo a EVA foram de 1,86 (± 2,92), 6,61 (± 2,31) e 4,08 (± 2,91) e, após 4 semanas, 2,66 (± 3,07), 6,32 (± 2,94) e 4,76 (± 3,26). Não ocorreram diferenças estatisticamente significativas entre os dentes teste e controle em nenhum dos tempos experimentais. Também não ocorreram diferenças intragrupo. Concluímos que o Gluma Desensitizer® não tem efeito sobre a hipersensibilidade dentinária de pacientes periodontalmente tratados num período de até quatro semanas.

Abstract The aim of the present infodemiological study was to evaluate whether the COVID-19 outbreak has influenced the volume of content related to the dental treatment needs of Brazilian Twitter users to summarize the trends, and to identify the perceptions of the treatment needed. We collected tweets related to dental care needs of individuals exposed to the COVID-19 outbreak scenario between March 23 to May 4, 2020 and of those not exposed to the COVID-19 pandemic (unexposed group) on the same reported days of 2019 using the terms “dentista (dentist), dente (tooth), siso (third molar), and aparelho (orthodontic appliance).” Descriptive analysis was performed to provide summary statistics of the frequencies of tweets related to different dental treatment needs and also the differences in volume content between the years 2019 and 2020. Moreover, the data were analyzed by qualitative analysis using an inductive approach. A total of 1,763 tweets from 2020 and 1,339 tweets from 2019 were screened. Those tweets posted by non-Brazilian users, duplicates, and those unrelated to dental treatment needs were removed and, therefore 1,197 tweets from 2020 and 719 tweets from 2019 were selected. Content volume related to dental treatment needs greatly increased during the COVID-19 outbreak. Findings from the word cloud and content analysis suggest that dental pain, related or not to the third molar, and problems with orthodontic appliances were the topics most commonly related to dental treatment needs discussed during the COVID-19 outbreak, mainly conveying anxiety and distress. The volume of tweets related to dental treatment needs posted by Brazilian users increased during the COVID-19 outbreak and self-reported pain and urgencies were the most popular topics.
