Introducción: la educación ambiental debe constituirse en un eje transversal de la formación de profesionales en áreas de la salud, los huertos universitarios de plantas medicinales son el espacio para aprender-haciendo en el fomento y protección de la salud y la vida respetando experiencias y cosmovisiones. Materiales y métodos: investigación acción participativa, bajo el paradigma sociocrítico para la recuperación del huerto medicinal de la Facultad de Ciencias Médicas de la Universidad de Cuenca. Discusión, reflexión y sistematización de experiencias. Resultados: previa fase de capacitación con expertos en temas de agronomía y huertos, cosmovisión andina y plantas medicinales y, medio ambiente; gestión y mantenimiento del huerto medicinal. Conclusiones: la investigación acción-participativa permite la implementación de un huerto universitario fomentando la sensibilización ambiental, la agricultura sostenible y el cuidado del medio ambiente.

La vainilla es originaria de las regiones húmedas tropicales de México y América Central, pero también se encuentra en forma silvestre en las selvas de América del sur. Es una de las especies aromáticas más importantes a nivel mundial. La esencia extraída de su fruto es ampliamente utilizada en la industria alimentaria y farmacéutica. Dada su importancia, en esta investigación se evaluaron diversas características físicas y químicas de un cultivo de Vanilla planofolia A. que se encuentra en etapa inicial y que fue sometido a dos tipos de tratamientos, convencional y orgánico con el objetivo de conocer, con cuál de ellos se desarrolla satisfactoriamente. El trabajo de campo se llevó a cabo en el municipio de Tihuatlán, Ver. México. Se seleccionaron 162 esquejes de un metro de largo y fueron trasplantados bajo malla sombra para optimizar las necesidades de sombra que requiere el cultivo. Las variables de respuesta que se utilizaron fueron: número de hojas, número de entrenudos, grosor de planta a los 30 cm de altura, clorofila y número de entrenudos. La recolección de datos se llevó a cabo cada 30 días. No se encontraos diferencias significativas entre ambos.

The growing challenges of healthcare systems pose a unique opportunity to leverage evidence-based digital health interventions. The WHO's SMART (Standards-based, Machine-readable, Adaptive, Requirements-based, and Testable) guidelines represent a significant advancement in this domain. This paper aims to summarize SMART guidelines authoring and implementation process, drawing on a comprehensive literature analysis. Our findings highlight critical success factors for national implementation, including stakeholder engagement, customization to local contexts, and leveraging international standards and digital technologies. We conclude with recommendations for countries aiming to implement WHO SMART guidelines, underscoring the need for a multi-disciplinary approach and the potential challenges to be navigated. El objetivo del proyecto fue evaluar la viabilidad de un modelo de negocio diversificando la ganadería tradicional que se considera un instrumento exitoso de conservación y desarrollo regional. En el norte de Veracruz México, existen varias unidades de producción y conservación de venado cola blanca debidamente autorizadas y en funcionamiento. Esta investigación consideró las variables social, económica y ambiental. Los factores evaluados fueron las condiciones de manejo, aprovechamiento, aspectos sociales y económicos, mediante información recopilada en entrevistas, recorridos a campo y obtenida de talleres. Los resultados indican que actualmente existe la desigualdad en el acceso y distribución de los recursos, falta de manejo y asesoría técnica para el aprovechamiento adecuado de la especie, por ello se determinó que se requiere de estrategias basadas en conocimientos para lograr un buen manejo y aprovechamiento del venado en la zona norte de Veracruz, esto desde la perspectiva de ganadería diversificada como una alternativa productiva que genere ingresos a los propietarios, para mejorar el bienestar social, económico y ambiental de la región, impulsando la formación de una red de colaboración para el intercambio de ejemplares, plan de manejo y comercialización en grupo para potencializar el modelo de negocio de ganadería diversificada con Venado (Odocoileus virginianus veraecrucis) que en base a los resultados obtenidos posee mucha viabilidad.

Este artículo pretende profundizar, a través de las fuentes impresas presentes en la Biblioteca Histórica «Marqués de Valdecilla», en la concepción de la imagen regia que legaron a la posteridad Carlos V y Felipe II y en los vínculos de ambos monarcas con el mundo de las artes, prestando especial atención a la obra de Tiziano, a los viajes de juventud de Felipe II y a su Monasterio de San Lorenzo de El Escorial.Based on printed sources preserved at Biblioteca Histórica «Marqués de Valdecilla», this paper focuses on analysing the conception of royal majesty which Charles V and Philip II pass on to posterity. Links of both monarchs to arts are also studied, giving special attention to Tiziano's paintings, youth travels of Philip II and to his Monastery of El Escorial.

Approach to the figure of the Venetian man of letters Lodovico Dolce (ca. 1510-1568) through the works kept at the Biblioteca Histórica Marqués de Valdecilla, paying special attention to his Dialogo della memoria (1st ed. 1562) and to the connections between image and text present in it, which are also traced in some of his other publications.

Trichopria drosophilae Perkins (Hymenoptera: Diapriidae) is a cosmopolitan endoparasitoid of the pupal stages of Drosophila suzukii Matsumura (Diptera: Drosophilidae) and other Drosophila species around the world. It is also considered as a potential biocontrol agent of this pest. Here we describe the effects of three hosts (D. suzukii, Drosophila melanogaster Meigen, and Drosophila immigrans Sturtevant) on the demographic parameters of T. drosophilae. Adult female T. drosophilae that emerged from D. immigrans and D. suzukii were larger than those that emerged from D. melanogaster. Fertility was 18% greater for T. drosophilae that developed on D. immigrans and D. suzukii, compared with those developing on D. melanogaster. The net reproductive rate of T. drosophilae was higher on D. immigrans and D. suzukii (R0 = 25.25 and 26.51) than on D. melanogaster (R0 = 22.53). However, the generation time was longer on D. immigrans and D. suzukii (T = 20.38 and 19.59 days, respectively) than on D. melanogaster (T = 17.20 days), perhaps due to their larger size. This favored the intrinsic rate of natural increase of T. drosophilae reproducing on D. melanogaster (rm = 0.181) compared with those reproducing on D. immigrans and D. suzukii (rm = 0.158 and 0.167, respectively). Parasitism rate of D. suzukii by T. drosophilae was 53% regardless of the host species from which the parasitoid had emerged. Despite recommendations for the routine HIV testing of all sexually active individuals, a significant percentage of HIV-positive adults are unaware of their HIV status. Therefore, a number of strategies have been implemented to expand HIV testing, which in turn makes it necessary to develop tools for identifying patients with unknown HIV status. This study presents the results of an external validation of an electronic phenotyping algorithm for identifying HIV status and its application on a retrospective cohort in order to explore temporal trends of HIV knowledge status and associated factors. The continuity of care is the relationship between a physician and a patient that extends throughout different episodes of illness. This care includes that in some opportunities the patient must consult a medical specialist. In this qualitative study, we set out to explore and describe the different requests for referral authorization to a specialist by patients affiliated with private health insurance, in order to understand the different patterns of interaction between general practitioners (GP) and their patients. The conversation analysis let us identify categories (structural and functional) after a critical reading of messages between GPs and their patients through an Electronic Health Records (EHR) messaging system. This work allowed us to know more in depth the process of continuity of care. In most cases, the doctor answered affirmatively to the request for a new referral to a specialist. Background: In recent years, there has been growing concern about the overuse of colonoscopy (CC). Our objective was to evaluate the incidence rate and cumulative probability of having a potentially inadequate CC (PI-CC, e.g. a CC that was performed earlier that recommended) and the association between the report of a hyperplastic polyp in the baseline CC report and the probability of having a PI-CC.Methods: A retrospective cohort of adults 50y/o or older with a complete baseline CC between January 1st and December 31st 2005, without reported lesions or with hyperplastic polyps, based on secondary data extracted from the electronic medical record of the Hospital Italiano of Buenos Aires. The outcome consisted of time until a PI-CC, defined as the time measured between basal colonoscopy and a colonoscopy performed earlier than the inter-screening interval recommended by the USPSTF and the USMSTF.Results: 389 patients were included. The cumulative probability of receiving a PI-CC over 10 years was 0.29 (95% CI 0.241, 0.342). The incidence rate resulted in 30.91 PI-CC per 1000 person-years (95% CI 25.14, 38). The crude analysis of the association between the outcome and the presence of hyperplastic polyps in the baseline CC, showed a statistically significant difference between both groups (log rank, p 0.036). The multivariate analysis yielded a hazard ratio of 1.67 (95% CI 1.02-2.73).Conclusion: We observed that 3 in every 10 patients treated in our health system received a PI-CC during the first ten consecutive years after a normal complete CC. Furthermore, this could be in part attributed to the presence of a hyperplastic polyp in the baseline CC. [ABSTRACT FROM AUTHOR]

During the implementation of an electronic health record (EHR) system in the public system of the city of Buenos Aires, Argentina, different patient indexing strategies were devised and implemented. We sought to assess the association between these strategies and effective indexing (proportion of patients who are indexed and have a consultation registered in the EHR). Strategies were grouped into three modalities (High, Intermediate, and Low intensity). We estimated hazard ratios (HR) comparing the High and Low intensity to the Intermediate strategies. The crude analyses showed a significant difference between the curves (p < 0.0001). In the multivariate analysis, the HRs of High and Low intensity interventions showed on average, values above 1 from 0 to 90 days compared to the Intermediate intensity strategy (High: 2.08 (1.65, 2.52); Low: 2.59 (2.29, 2.9)). Background: The implementation of electronic medical records (EMR) is becoming increasingly common. Error and data loss reduction, patient-care efficiency increase, decision-making assistance and facilitation of event surveillance, are some of the many processes that EMRs help improve. In addition, they show a lot of promise in terms of data collection to facilitate observational epidemiological studies and their use for this purpose has increased significantly over the recent years. Even though the quantity and availability of the data are clearly improved thanks to EMRs, still, the problem of the quality of the data remains. This is especially important when attempting to determine if an event has actually occurred or not. We sought to assess the sensitivity, specificity, and agreement level of a codes-based algorithm for the detection of clinically relevant cardiovascular (CaVD) and cerebrovascular (CeVD) disease cases, using data from EMRs. Methods: Three family physicians from the research group selected clinically relevant CaVD and CeVD terms from the international classification of primary care, Second Edition (ICPC-2), the ICD 10 version 2015 and SNOMED-CT 2015 Edition. These terms included both signs, symptoms, diagnoses and procedures associated with CaVD and CeVD. Terms not related to symptoms, signs, diagnoses or procedures of CaVD or CeVD and also those describing incidental findings without clinical relevance were excluded. The algorithm yielded a positive result if the patient had at least one of the selected terms in their medical records, as long as it was not recorded as an error. Else, if no terms were found, the patient was classified as negative. This algorithm was applied to a randomly selected sample of the active patients within the hospital's HMO by 1/1/2005 that were 40-79 years old, had at least one year of seniority in the HMO and at least one clinical encounter. Thus, patients were classified into four groups: (1) Negative patients (2) Patients with CaVD but without CeVD; (3) Patients with CeVD but without disease CaVD; (4) Patients with both diseases. To facilitate the validation process, a stratified sample was taken so that each of the groups represented approximately 25% of the sample. Manual chart review was used as the gold standard for assessing the algorithm's performance. One-third of the patients were assigned randomly to each reviewer (Cohen's kappa 0.91). Both coded and un-coded (free text) sections of the EMR were reviewed. This was done from the first present clinical note in the patients chart to the last one registered prior to 1/1/2005. Results: The performance of the algorithm was compared against manual chart review. It yielded high sensitivity (0.99, 95% CI 0.938-0.9971) and acceptable specificity (0.86, 95% CI 0.818-0.895) for detecting cases of CaVD and CeVD combined. A qualitative analysis of the false positives and false negatives was performed. Conclusions: We developed a simple algorithm, using only standardized and non-standardized coded terms within an EMR that can properly detect clinically relevant events and symptoms of CaVD and CeVD. Background: The widespread and growing use of alternative and complementary medicine (CAM) worldwide has been thoroughly described. In Argentina the limited information on the use of CAM has been reported between 40 and 55 %. However, the rate of use is extremely variable worldwide. For this purpose the international questionnaire on the use of complementary and alternative medicines (I-CAM - Q), was developed. The implementation of a translated and cross-culturally adapted version of the questionnaire would allow for a reliable and standardized evaluation of the rate of use of CAM in Argentina. It would be a great step towards improving what we know about the healing habits of our population. Methods: The forward and back-translation method was used. Four translators were involved. A committee was commissioned to reconcile the different versions. The process of cross-cultural adaptation was made by consulting 17 alternative and complementary medicine experts using the DELPHI method. The retrieved questionnaire was evaluated in 18 patients sampled by convenience (9 men, different educational and self-reported health levels). The interviews consisted of three parts: an initial demographics questionnaire; the administration of the I-CAM-Q and finally the cognitive interview, which included reviewing the questionnaire and reexamining questions that generated doubts during the interview. The comprehension of the questions was also evaluated. As a last step, using the information obtained from the interviews, the final version of the questionnaire was drafted. Results and conclusion: The questionnaire seems to have been accepted by most patients during the interviews. Conflictive elements that emerged did not seem to have an impact on its administration. The flexibility of the questionnaire allowed to add professionals and practices which contributed to a more accurate local adaptation. Microparticles (MPs) could be considered biomarkers of cell damage and activation as well as novel signalling structures. Since rheumatoid arthritis (RA) is characterized by immune and endothelial activation, the main aim of the present study was to analyse MP counts in RA patients. Citrated-blood samples were obtained from 114 RA patients, 33 healthy controls (HC) and 72 individuals with marked cardiovascular (CV) risk without autoimmune manifestations (CVR). MPs were analysed in platelet-poor plasma (PPP) and different subsets were identified by their surface markers: platelet-(CD41+), endothelial- (CD146+), granulocyte- (CD66+), monocyte- (CD14+) and Tang- (CD3+CD31+) derived. Disease activity score (DAS28), clinical and immunological parameters as well as traditional CV risk factors (diabetes, hypertension, dyslipidaemia and obesity) were registered from clinical records and all data were integrated using Principal Component Analysis (PCA). Absolute MP number was increased in RA patients compared with HC and positively correlated with traditional CV risk factors, similar to that of CVR subjects. In addition, frequency of the different MP subsets was different in RA patients and significantly associated with disease features. Moreover, in vitro assays revealed that MPs isolated from RA patients were able to promote endothelial activation and exhibited detrimental effects on human microvascular endothelial cells (HMEC-I) endothelial cell functionality. Circulating MPs from RA patients displayed quantitative and qualitative alterations that are the result of both disease-specific and traditional CV risk factors. Accordingly, this MP pool exhibited in vitro detrimental effects on endothelial cells, thus supporting their role as biomarkers of vascular damage. [ABSTRACT FROM AUTHOR]

Through the development of a dashboard with participative methodology we present a centralized strategy to analyze and visualize EMRs data for the management of 15 public hospitals from Buenos Aires City.
