Objective: In this study, the aim was to compare the results of mini and standard percutaneous nephrolithotomy (PCNL) for the treatment of pediatric kidney stones. Materials and Methods: Data for 128 patients < 18 years of age who underwent mini and standard PCNL due to pediatric kidney stones were retrospectively examined. Patients were divided into two groups: mini-PCNL (16-20 Fr) and standard PCNL (26 Fr). Surgery time, number of punctures to the pelvicalyceal system, hospital stay, postoperative hemoglobin drop, complications and stone-free status (SFR) were compared between the groups. Additional surgical intervention (double-J stent, ureterorenoscopy, secondary PCNL) performed after the surgery was recorded. The absence of residual stones or < 3 mm residual stones on kidney, ureter and bladder radiography (KUB) and ultrasonography (USG) performed in the third postoperative month were accepted as success criteria. Results: There were 32 (43.8%) patients in the mini-PCNL group and 41 (56.2%) patients in the standard PCNL group. The mean age was 9.3 ± 4.1 years in the mini-PCNL group and 10.1 ± 5.4 years in the standard PCNL group. Mean stone size in the mini-PCNL group was 2.1 ± 1.2; while for standard PCNL it was 2.3 ± 1.4. The mean surgery time was statistically significantly higher in the mini-PCNL group (p = 0.005). There was no difference between the groups in terms of intraoperative double J stent use, postoperative complications and SFR. A double J stent was inserted in two patients in the mini-PCNL group and in one patient in the standard PCNL group due to urine leakage from the nephrostomy tract in the postoperative period. Although the postoperative hemoglobin drop was found to be significantly higher in standard PCNL (p = 0.001), hematuria and blood transfusion rates were low in both groups. Mean hospital stay was shorter in the mini-PCNL group compared to standard PCNL (3.6 ± 1.2 days vs. 2.5 ± 1.1; p = 0.018). Conclusions: Although mini-PCNL has longer surgery time compared to standard PCNL, it should be preferred for the treatment of pediatric kidney stones due to advantages such as similar success and complication rates to standard PCNL, short hospital stay and less postoperative hemoglobin drop.

Aim: The aim of this study was to reveal the effect of manual detorsion on testicular salvage rates and the factors affecting the success of manual detorsion in adult testicular torsion. Material and Methods: The records of patients who applied to the emergency department with pre-diagnosis of acute scrotum were examined. A total of 62 adult patients diagnosed with testicular torsion were included in the study. Manual detorsion was attempted in all patients before surgery. Demographic characteristics of the patients, time from onset of scrotal pain to hospital admission (24 hours), physical examination and ultrasonography findings, type of surgery performed, and long-term results were evaluated. The data were statistically compared between patients with manual detorsion considered successful and unsuccessful. Results: Manual detorsion was considered successful in 36 (58.1%) patients and unsuccessful in 26 (41.9%) patients. Purpose Comparing the two different drainage methods of percutaneous nephrostomy (PCN) versus retrograde ureteral double – J (DJ) stent insertion in patients with obstructive pyelonephritis cases. Material and methods In this retrospective study, patients with obstructive pyelonephritis secondary to urolithiasis who underwent PCN or DJ insertion were included in the study. Patients were divided into two groups according to drainage method. After exclusion criteria, the study included 105 patients. The groups were compared for intraoperative outcomes, duration of fluoroscopy usage, the time needed for normalization of infection parameters (white blood cells (WBC), C – reactive protein (CRP), procalcitonin), and complications were observed. Results From 105 patients, 56 patients were in DJ stent group and 49 patients were in PCN group. According to intraoperative data, operative time and fluoroscopy duration were significantly shorter in the DJ ureteral stent group (P < .001). WBC returned to normal range in mean 3.5 ± 1.3 days in the DJ stent group and 3.2 ± 1.1 days in the PCN group (95% CI: −0.76–0.21, P = .268). There were no statistically significant differences identified for the duration for CRP, PCT and fever to return to normal range between the drainage methods. The complication rates was 51.8% for DJ stent group and, 30.6% in PCN group (P = .028). Conclusion Both methods are effective and safe in obstructive pyelonephritis. Despite the higher complication rate in DJ stent group, these complications were minor.

Purpose: The aim of this study was to discuss the progression results of patients with muscle invasive bladder cancer (MIBC) who delayed their treatment by not making hospital visits on time for fear of contamination during the COVID-19 pandemic. Materials and Methods: Records of patients diagnosed with MIBC and recommended radical cystectomy (RC) between March 2020 and June 2020 were reviewed. Whether patients were operated on the scheduled time, and if not, the time elapsed after the first diagnosis or the end of neoadjuvant chemotherapy (NAC) were recorded. The patients who applied to our clinic late were restaged and examined whether there was any progression during the pandemic period. Results: NAC was recommended before RC in 9 of 24 patients, RC was recommended directly to the remaining 15 patients. After NAC, RC was applied to four patients in the planned period, and 4 patients applied delayed. The mean admission period of the patients who applied late after NAC was 197.8±68.5 days. Lung metastases and pelvic lymphadenopathies were found in two (50%). RC was applied to eight of fifteen patients who did not receive NAC in the planned period. Five of these patients applied for RC in the late period and the average application time after diagnosis was 202.8±31.9 days. In the staging of these five patients who presented late, two (40%) had progression (cT2, N0,M1b). Conclusion: In times of widespread COVID-19 outbreak, physicians should be alert to potential stage progression and worse survival outcomes resulting from delays in diagnosis and treatment.

Agresif anjiyomiksoma; sıklıkla pelvik yumuşak dokular ve perinede ortaya çıkan, nadir görülen benign bir tümördür. Agresif anjiyomiksoma ilk kez 1983 yılında Steeper ve Rosai tarafından tanımlanmıştır. Ana tedavi metodu geniş lokal eksizyodur. Yüksek rekürrens oranı vardır. Bu vakada 32 yaşında skrotal anjiomiksoma olgusunu literatür eşliğinde sunmayı amaçladık.

Amaç: Bu çalışmamızda Plasma Kinetik Doku Tedavi Sistemi ile transüretral rezeksiyon (TURP) yapılan, semptomatik benign prostat hiperplazili (BPH) hastaların kısa dönem sonuçlarını inceleyerek, Plasma Kinetik Doku Tedavi Sisteminin etkinlik ve yan etki profilini ortaya koymaya çalıştık. Materyal ve Metod: Mart 2011 ile şubat 2012 tarihleri arasında kliniğimize başvuran benin prostat hiperplazi (BPH) tanısı ile ameliyat kararı alınan 86 hasta çalışma kapsamına alındı. Hastaların; IPPS semptom skorlaması, üroflowmetri, artık idrar miktarı tayini, prostat hacimleri, belirlendi. Tüm hastaların ameliyat sonrası üretra darlığı, mesane boyun darlığı, inkontinans gibi erken ve geç istenmeyen yan etkileri kaydedildi. Hastalar ameliyat süreleri, ameliyat sırasında kullanılan irrigasyon sıvısı miktarı ve transfüzyon gerekliliği, ameliyat sonrası hastanede kalış ve katerizasyon süreleri, irrigasyon sıvısı miktarları kaydedildi. 6.ayda hastalardan elde edilen veriler ameliyat öncesi verilerle kıyaslandı. Ameliyatı gerçekleştiren doktorlara görüntü kalitesinin 0"dan 10"a doğru artış gösterdiği söylendikten sonra, skala üzerinde kişisel derecelendirme yapmaları istendi. Bulgular: Çalışmaya dahil edilen 86 hastadan 24"ünün başvuru nedeni akut üriner retansiyona bağlı olarak gelişen glob vezikale idi. Bir hastada yatış endikasyonu hematüri nedeniyle olmuştu. Çalışmaya alınan hastaların operasyon öncesi IPSS değerlerinin genel ortalaması 22,2 idi. Postoperatif 6.aydaki sorgulamalarında ortalama IPSS değeri 6,8 olarak tespit edilmiştir. Operasyondan hemen sonra, rezektoskopun çıkarılmasını takiben, cerrahın cerrahi alan görüntü kalitesini ölçmeyi hedefleyen VAS skoru ortalaması 7,41, cihazın kanama kontrolü yeteneğini ölçmeyi hedefleyen VAS skoru ortalaması 7,62 bulunmuştur. Sonuç: Çalışmamızda ortaya çıkan sonuçlar ve literatür verileri kılavuzluğunda; bipolar TURP teknolojisinin, mesane çıkım obstrüksiyonuna yol açmış benign prostat hiperplazisinin endoskopik cerrahi tedavisinde daha iyi hemodinamik stabilite ve cerrahi işlem sırasında daha yüksek konfor sağlaması nedeniyle monopolar TURP uygulamalarına iyi bir alternatif olduğu kanısına vardık.

Abstract. Objectives:. This study aimed to investigate the effect of the coronavirus disease 2019 (COVID-19) pandemic on the treatment of children with primary monosymptomatic nocturnal enuresis (MNE) with desmopressin melt versus an enuresis alarm. Materials and methods:. This study included 56 children with primary MNE who were taking desmopressin melt or using an alarm. Their anxiety levels were evaluated using the Social Anxiety Scale for Children-Revised. For both treatment methods, data from a 3-month bedwetting diary between the third and sixth months of the pre-pandemic treatment were compared with those assessed during the same period during the pandemic. Objectives: To compare the efficacy and reliability of Ho:YAG laser lithotripsy (HLL) and pneumatic lithotripsy (PL) in the treatment of bladder stones in patients with benign prostatic hyperplasia and stones ≥ 20 mm who were transurethrally treated in the same surgical session. Methods: We studied the data of patients with benign prostatic hyperplasia and ≥20 mm bladder stones who were treated with transurethral resection of the prostate and cystolithotripsy in the same session, obtained between January 2010 and February 2014 from three urology clinics. All patients underwent bipolar plasmakinetic (PK) transurethral resection of the prostate. For treatment of the bladder stone, either HLL or PL was applied. A total of 62 patients were divided into two groups: PK-PL (Group 1, n = 29) and PK-HLL (Group 2, n = 33). The data of both groups were analyzed for stone dimensions, stone fragmentation time, total operating time, hospitalization duration, prostate dimensions, success rates, and complications. Results: Group 1 included 29 patients with a mean age of 70 ± 7.6 (range, 57–85) years, whereas Group 2 included 33 patients with a mean age of 67.5 ± 10.5 (range, 45–84) years. In Group 1, five patients had mucosa injury, one patient had residual stone, and one patient had bladder perforation. In Group 2, three patients had mucosa injury, three patients had postoperative fever, and one patient had residual stone. Total operation time and stone fragmentation time were significantly lower in Group 2 (p 0.05). Conclusion: PK-HLL using a single shaft without the need for repeated access has the advantages of shorter fragmentation and operation time.

Objective: This study was conducted to evaluate clinical experience and learning curve associated with laparoscopic ureterolithotomy performed for upper ureteral stones. Materials and methods: The medical data of 50 patients who had undergone retroperitoneal laparoscopic ureterolithotomy between June 2010 and March 2013 were retrospectively analyzed. To assess the learning curve, patients were divided into two groups: Group A (the first 25 cases) and Group B (the last 25 cases). In Group A, double J stents were placed in 17 patients, whereas in Group B 15 patients received double J stents. In Group A, three ports were placed in nine patients and four ports in 16 patients. In Group B, three ports were placed in 20 patients and five patients had four ports. The patients were compared according to demographics, operative time, stone size, complications, hospital stay, and transfusion. Results: The mean age for Group A was 47.8 ± 14.13 (21–72) years and that for Group B was 44.2 ± 14.98 (22–78) years. Mean operative times were 106.4 ± 38 (55–210) minutes and 70.76 ± 30.4 (30–180) minutes for Groups A and B, respectively (p 0.05). Conclusion: In our study, as staff experience (in performing laparoscopic retroperitoneal ureterolithotomy) increased, operative time, length of hospital stay, and complication rates have correspondingly declined.

Aggressive angiomyxoma is often a rare benign tumor, arising in the pelvic soft tissues and perineum. Aggressive angiomyxoma was described by Steper and Rosai first time in 1983. The main treatment method is wide-local excision. There is a high recurrence rate. In this case report, we aimed to present a case of scrotal agresif angiomyxoma in a 32-year-old man with literature. Aim: To evaluate short term outcomes of patients undergone transurethral resection with plasmakinetic energy for benign prostate hyperplasia and to present safety and efficacy of the procedure. Material and Method:86 patients applied to our clinic between March 2011 and February 2012 were enrolled into the study. PSS scores, uroflowmetry, post voiding residual urine, prostate volumes of the patients were recorded. Post operative urethral stricture, bladder neck stricture, incontinance rates were noted. Data assessed after six months were compared to peroperative data. After the operations, the surgeons evaluated their comfort of surgery with visual analog scale from 0 to 10. Results: 24 of 86 patients were hospitalized for acute urinary retention. One patient was hospitalized because of hematuria. Peroperative IPSS was 22.2. at post operative sixth month, mean IPSS was 6.8. VAS scores of surgeons was; to assess visual comfort was 7.41, to assess coagulation was 7.62. Conclusion:After evaluating the data and literature, we believe bipolar plasmakinetik TUR is a safe and comforting method to treat bladder outlet obstruction with goog hemodynamic stability.
