BackgroundSelf-monitoring is key to successful behavior change in diabetes and obesity, and the use of traditional paper-based methods of self-monitoring may be time-consuming and burdensome. ObjectiveThis study aimed to explore participant experiences while using technology-assisted self-monitoring of lifestyle behaviors and health indicators among overweight or obese adults with type 2 diabetes. MethodsQualitative data collected from the intervention group of a 6-month, three-arm (control, paper diary, and technology-assisted self-monitoring groups) randomized clinical trial were analyzed. Study participants in the intervention group monitored their diet, exercise, and weight using the LoseIt! app, and their blood glucose levels using a glucometer and the Diabetes Connect app. Semistructured group discussions were conducted at 6 weeks (n=10) from the initiation of the behavioral lifestyle intervention and again at 6 months (n=9). All group interviews were audiotaped and transcribed verbatim. Using a combination of thematic and comparative analysis approaches, two trained professionals coded the transcriptions independently and then discussed and concluded common themes for the 6-week and 6-month discussions separately. ResultsThe sample (n=10), which primarily involved African American participants (n=7) and female participants (n=8), had a mean age of 59.4 years. The following eight themes emerged: (1) perceived benefits of technology-assisted self-monitoring; (2) perceived ease of use (eg, barriers: technical difficulties and lack of self-discipline; facilitators: help from family, friends, and the program); (3) use of technology-assisted self-monitoring; (4) facilitators of engaging in healthy lifestyle behaviors (eg, visualization and awareness of calorie input/expenditure); (5) positive lifestyle change; (6) barriers of engaging in healthy lifestyle behaviors (eg, event influence); (7) learning curve; and (8) monitored data sharing. The first six of these themes were shared between the 6-week and 6-month timepoints, but the codes within these themes were not all the same and differed slightly between the two timepoints. These differences provide insights into the evolution of participant thoughts and perceptions on using technology for self-monitoring and subsequent behavioral lifestyle changes while participating in lifestyle interventions. The findings from the 6-week and 6-month data helped to paint a picture of participant comfort and the integration of technology and knowledge overtime, and clarified participant attitudes, difficulties, behavioral processes, and modifications, as well as health indicators that were experienced throughout the study. ConclusionsAlthough there were some barriers, participants were able to identify various individual and external facilitators to adjust to and engage in technology-assisted self-monitoring, and it was concluded that the technology-assisted self-monitoring approach was beneficial, safe, and feasible to use for positive lifestyle change. These patient perspectives need to be considered in future research studies when investigating the effectiveness of using technology-assisted self-monitoring, as well as in clinical practice when recommending technology-assisted self-monitoring of lifestyle behaviors and health indicators to improve health outcomes.

BackgroundDiabetes and Alzheimer disease and related dementias (ADRD) are the seventh and sixth leading causes of death in the United States, respectively, and they coexist in many older adults. Caring for a loved one with both ADRD and diabetes is challenging and burdensome. ObjectiveThis study aims to explore diabetes-related topics in the Alzheimer’s Association ALZConnected caregiver forum by family caregivers of persons living with ADRD. MethodsUser posts on the Alzheimer’s Association ALZConnected caregiver forum were extracted. A total of 528 posts related to diabetes were included in the analysis. Of the users who generated the 528 posts, approximately 96.1% (275/286) were relatives of the care recipient with ADRD (eg, child, grandchild, spouse, sibling, or unspecified relative). Two researchers analyzed the data independently using thematic analysis. Any divergence was discussed among the research team, and an agreement was reached with a senior researcher’s input as deemed necessary. ResultsThematic analysis revealed 7 key themes. The results showed that comorbidities of ADRD were common topics of discussions among family caregivers. Diabetes management in ADRD challenged family caregivers. Family caregivers might neglect their own health care because of the caring burden, and they reported poor health outcomes and reduced quality of life. The online forum provided a platform for family caregivers to seek support in their attempts to learn more about how to manage the ADRD of their care recipients and seek support for managing their own lives as caregivers. ConclusionsThe ALZConnected forum provided a platform for caregivers to seek informational and emotional support for caring for persons living with ADRD and diabetes. The overwhelming burdens with these two health conditions were apparent for both caregivers and care recipients based on discussions from the online forum. Studies are urgently needed to provide practical guidelines and interventions for diabetes management in individuals with diabetes and ADRD. Future studies to explore delivering diabetes management interventions through online communities in caregivers and their care recipients with ADRD and diabetes are warranted.

BackgroundIn the United States, more than one-third of the adult population is obese, and approximately 25.2% of those aged ≥65 years have type 2 diabetes (T2D), which is the seventh leading cause of death. It is important to measure patient-reported outcomes and monitor progress or challenges over time when managing T2D to understand patients’ perception of health and quantify the impact of disease processes or intervention effects. The evaluation of patient-reported outcome measures (PROMs) is especially important among patients with multiple chronic conditions in which clinical measures do not provide a complete picture of health. ObjectiveThis study examined the feasibility of collecting Patient-Reported Outcome Measurement Information System (PROMIS) measures, and preliminarily evaluated changes in PROMIS scores and compared the scores with standard scores of the general US population. The parent study is a pilot randomized controlled clinical trial testing three different modes (mobile health [mHealth], paper diary, and control) of self-monitoring in a behavioral lifestyle intervention among overweight or obese patients with T2D. MethodsPatients with comorbid overweight or obesity and a diagnosis of T2D for at least 6 months were recruited from a diabetes education program. Participants were randomized to the following three groups: mHealth, paper diary, and control (standard of care) groups. Paper diary and mHealth experimental groups received additional behavioral lifestyle intervention education sessions, as well as tools to self-monitor weight, physical activity, diet, and blood glucose. All participants completed PROMIS-57 and PROMIS-Global Health (GH) version 1.0 questionnaires during visits at baseline, 3 months, and 6 months. The PROMIS-57 includes the following seven domains: anxiety, depression, fatigue, pain interference, physical function, satisfaction with participation in social roles, and sleep disturbance. The PROMIS-GH is composed of the following two domains: global mental health and global physical health. ResultsA total of 26 patients (mHealth, 11; paper diary, 9; control, 6) were included in our analysis. The study sample was predominantly African American (68%) and female (57%), with a mean age of 54.7 years and a mean BMI of 37.5 kg/m2. All patients completed the PROMIS-57 and PROMIS-GH questionnaires, and we compared the mean scores of the three groups to investigate potential differences. No relevant differences were noted across the groups. However, positive trends were noted in both intervention (mHealth and paper diary) groups in the middle (month 3) and end (month 6) of the study. ConclusionsOur pilot study provides evidence for the feasibility of using PROMIS questionnaires to record important components of T2D-related symptoms among overweight or obese individuals. The results from our study support the use of PROMIS questionnaires to provide clinicians and researchers with a benchmark for assessing the overall need for symptom management and determining the success or challenges of an intervention. BackgroundIndividuals with mild cognitive impairment and dementia have impaired physical and cognitive functions, leading to a reduced quality of life compared with those without such impairment. Exergaming, which is defined as a combination of exercise and gaming, is an innovative, fun, and relatively safe way to exercise in a virtual reality or gaming environment. Therefore, exergaming may help people living with mild cognitive impairment or dementia to overcome obstacles that they may experience regarding regular exercise and activities. ObjectiveThe aim of this systematic review was to review studies on exergaming interventions administered to elderly individuals with mild cognitive impairment and dementia, and to summarize the results related to physical and cognitive functions such as balance, gait, executive function, and episodic memory. MethodsWe searched Cochrane Central Register of Controlled Trials (CENTRAL), Medline, Embase, PsycINFO, Amed, and Nursing Database for articles published from the inception of the respective databases to January 2019. We included all clinical trials of exergaming interventions in individuals with mild cognitive impairment and dementia for review. The risk of bias was independently evaluated by two reviewers using the Cochrane Collaboration and Risk of Bias in Non-randomized Studies of Interventions tools. ResultsTen studies involving 702 participants were included for review. There was consistent evidence from 7 studies with a low risk of bias showing statistically significant effects of exergaming on cognitive functioning in people with mild cognitive impairment and dementia. With respect to physical function, 3 of 5 full-scale studies found positive results, and the intensity of most games was classified as moderate. ConclusionsOverall, exergaming is an innovative tool for improving physical and cognitive function in people with mild cognitive impairment or dementia, although there is high heterogeneity among studies in terms of the duration, frequency, and gaming platform used. The quality of the included articles was moderate to high. More high-quality studies with more accurate outcome indicators are needed for further exploration and validation of the benefits of exergaming for this population.

BackgroundType 2 diabetes mellitus (T2DM) is a major public health burden. Self-management of diabetes including maintaining a healthy lifestyle is essential for glycemic control and to prevent diabetes complications. Mobile-based health data can play an important role in the forecasting of blood glucose levels for lifestyle management and control of T2DM. ObjectiveThe objective of this work was to dynamically forecast daily glucose levels in patients with T2DM based on their daily mobile health lifestyle data including diet, physical activity, weight, and glucose level from the day before. MethodsWe used data from 10 T2DM patients who were overweight or obese in a behavioral lifestyle intervention using mobile tools for daily monitoring of diet, physical activity, weight, and blood glucose over 6 months. We developed a deep learning model based on long short-term memory–based recurrent neural networks to forecast the next-day glucose levels in individual patients. The neural network used several layers of computational nodes to model how mobile health data (food intake including consumed calories, fat, and carbohydrates; exercise; and weight) were progressing from one day to another from noisy data. ResultsThe model was validated based on a data set of 10 patients who had been monitored daily for over 6 months. The proposed deep learning model demonstrated considerable accuracy in predicting the next day glucose level based on Clark Error Grid and ±10% range of the actual values. ConclusionsUsing machine learning methodologies may leverage mobile health lifestyle data to develop effective individualized prediction plans for T2DM management. However, predicting future glucose levels is challenging as glucose level is determined by multiple factors. Future study with more rigorous study design is warranted to better predict future glucose levels for T2DM management.

[Objective] China's backbone natural gas pipeline network comprises numerous large-diameter and long-distance pipelines.Periodic pigging operations play a crucial role in ensuring the safe, stable and efficient operation of these pipelines. However, since current pigging operations rely heavily on operator experience and the establishment of monitoring points, the inability to simulate pigging schemes in advance prevents accurate prediction of parameters such as the operating speed, position and arrival time of the pig, and pressure changes in the pipe. [Methods] To address this challenge, a mathematical model for dynamic prediction of the pigging process was constructed and put into practice according to the characteristics of actual pigging operations. Subsequently, the software for dynamic prediction of the pigging process was developed and applied to the actual pigging operations of several sections(A-B, B-C and D-E) of a designated pipeline for predicting the pigging time and pigging speed. [Results] The improved two-phase flow transient pigging model and the dynamic prediction software accurately forecast critical parameters, including the running speed, position and arrival time at each station of the pig during the pigging process; The accuracy of the dynamic pigging prediction technology and the reliability of the software were verified using the monitoring data of 52 actual pigging operations; And compared with actual measurements, the software's predictions had average relative errors of 5.94% for pigging time and 6.56% for pigging speed. [Conclusion] The utilization of the dynamic prediction technology and software enables efficient preparation of pigging operation schemes for natural gas pipelines, reduces manual workload, ensures pigging process safety, and enhances pipeline management intelligence.

Brassica oleracea is one of the important vegetable crops in China. Its seeds are primarily imported. Hence, developing innovative varieties belonging to China is an urgent requirement. Since broccoli is a cross-pollinated crop systematic selection and cross-breeding are challenging, and the application of radiation mutagenesis breeding in broccoli has not been reported. In this study, the seeds of Brassica oleracea were irradiated using carbon ion beams to investigate the biological effects of these beams on broccoli. The growth indices, antioxidant enzyme activity, photosynthetic indices, and chlorophyll fluorescence were detected at the seedling stage. The results revealed that irradiation with 100–500 Gy exhibited no significant effect on the germination of Brassica oleracea, while the germination of seeds was significantly inhibited at 600 Gy. After irradiation with 100–600 Gy, the root length, shoot length, seedling height, and leaf area decreased with the increase in irradiation dose. The Median lethal dose (LD50) of Brassica oleracea irradiated using a carbon ion beam was 415.89 Gy, and the dose that halved the root length was 495.12 Gy. After irradiation, the activities of superoxide dismutase (SOD) and peroxidase (POD) were higher than that of the control plants, while the activity of catalase (CAT) was lower than that of the control plants. The content of malondialdehyde (MDA) increased significantly at a dose of 400 Gy. The photosynthetic pigments (chlorophyll a, chlorophyll b, and carotenoids) showed an increasing trend initially, and then a decreasing trend with an increasing irradiation dose, and the highest value was detected at a dose of 300 Gy. Net photosynthesis, transpiration rate, and stomatal conductance exhibited a negative correlation with irradiation dose, while the non-photosynthetic quenching coefficient exhibited a significant increase after irradiation. The findings indicated that heavy ion beam radiation inhibited the growth of broccoli plants and affected the activity of antioxidant enzymes and photosynthesis. The study provided basic data for the radiation mutation breeding of Brassica oleracea.

For a better understanding of the difference in functional components and functions among different parts of highland barley bran, the contents of protein, β-glucan and polyphenol, antioxidant activity, and alkyl radical intensity in highland barley bran with various milling rates were compared. The results showed that the contents of total protein, albumin, globulin, glutenin, free polyphenol, bound polyphenol, free flavone, bound flavone and condensed tannin first increased and then decreased with increasing milling rate. Protein mainly existed in bran flour with a milling rate of 17.74% and 19.22%. Bran flour with 9.7% milling rate had the highest phenol content, and the trend in antioxidant activity was consistent with that in the content of phenols. The contents of β-glucan and gliadin increased from outside to inside, and reached the highest value at a milling rate of 25.99%. Alkyl radical intensity was significantly higher at 1.54%–5.84% milling rates than at 7.67%–25.99% milling rates, indicating a higher susceptibility to oxidation. The findings of this study provide a theoretical basis for the deep processing of different layers of highland barley bran.
