Communities of Practice are an increasingly common tool used to support novice academics in higher education settings. Initiated in 2015 at a Western Australian University, TLABs is an acronym for 'Teaching and Learning for Level A and B' academic staff and was designed to build a community of practice to mentor junior academics; help them develop their teaching skills; and enhance academic careers. The paper describes the nature of TLABs; how it is experienced from the perspective of participants and provides recommendations for implementing a successful teaching and learning community of practice in a higher education setting.

This paper looks at a pilot of a strategy designed to help academic staff better respond to the inclusive teaching needs of its international students. The strategy was implemented by a School in a Western Australian University and offered academic staff evidence-based information on specific techniques for internationalising curriculum content and delivery. The extent to which international students perceived lecturers had implemented the suggested strategies was subsequently evaluated. While the evaluation found the School had performed adequately in the eyes of its international students across key areas of teaching practice, it also suggested that achieving greater inclusiveness required attention to things beyond classroom processes. These broader aspects included the University's climate of belongingness and the extent to which the curriculum content took account of international perspectives.

Osteoarthritis is one of the most frequent and disabling diseases of the elderly. Only few genetic variants have been identified for osteoarthritis, which is partly due to large phenotype heterogeneity. To reduce heterogeneity, we here examined cartilage thickness, one of the structural components of joint health. We conducted a genome-wide association study of minimal joint space width (mJSW), a proxy for cartilage thickness, in a discovery set of 13,013 participants from five different cohorts and replication in 8,227 individuals from seven independent cohorts. We identified five genome-wide significant (GWS, P≤5·0×10-8) SNPs annotated to four distinct loci. In addition, we found two additional loci that were significantly replicated, but results of combined meta-analysis fell just below the genome wide significance threshold. The four novel associated genetic loci were located in/near TGFA (rs2862851), PIK3R1 (rs10471753), SLBP/FGFR3 (rs2236995), and TREH/DDX6 (rs496547), while the other two (DOT1L and SUPT3H/RUNX2) were previously identified. A systematic prioritization for underlying causal genes was performed using diverse lines of evidence. Exome sequencing data (n = 2,050 individuals) indicated that there were no rare exonic variants that could explain the identified associations. In addition, TGFA, FGFR3 and PIK3R1 were differentially expressed in OA cartilage lesions versus non-lesioned cartilage in the same individuals. In conclusion, we identified four novel loci (TGFA, PIK3R1, FGFR3 and TREH) and confirmed two loci known to be associated with cartilage thickness.The identified associations were not caused by rare exonic variants. This is the first report linking TGFA to human OA, which may serve as a new target for future therapies.

ObjectivesTo identify genetic associations with hip osteoarthritis (HOA), we performed a meta-analysis of genome-wide association studies (GWAS) of HOA.MethodsThe GWAS meta-analysis included approximately 2.5 million imputed HapMap single nucleotide polymorphisms (SNPs). HOA cases and controls defined radiographically and by total hip replacement were selected from the Osteoporotic Fractures in Men (MrOS) Study and the Study of Osteoporotic Fractures (SOF) (654 cases and 4697 controls, combined). Replication of genome-wide significant SNP associations (p ≤5×10(-8)) was examined in five studies (3243 cases and 6891 controls, combined). Functional studies were performed using in vitro models of chondrogenesis and osteogenesis.ResultsThe A allele of rs788748, located 65 kb upstream of the IGFBP3 gene, was associated with lower HOA odds at the genome-wide significance level in the discovery stage (OR 0.71, p=2×10(-8)). The association replicated in five studies (OR 0.92, p=0.020), but the joint analysis of discovery and replication results was not genome-wide significant (p=1×10(-6)). In separate study populations, the rs788748 A allele was also associated with lower circulating IGFBP3 protein levels (p=4×10(-13)), suggesting that this SNP or a variant in linkage disequilibrium could be an IGFBP3 regulatory variant. Results from functional studies were consistent with association results. Chondrocyte hypertrophy, a deleterious event in OA pathogenesis, was largely prevented upon IGFBP3 knockdown in chondrocytes. Furthermore, IGFBP3 overexpression induced cartilage catabolism and osteogenic differentiation.ConclusionsResults from GWAS and functional studies provided suggestive links between IGFBP3 and HOA.

There has been a critical shortage of qualified Environmental Health Officers (EHOs) both globally and in Australia that has plagued the profession for decades. At Edith Cowan University (ECU), Perth, Western Australia, an undergraduate degree in Environmental Health (EH) was developed in 2006, however, viability was problematic, and the course was under threat of closure. In 2016 both the EH and occupational safety and health (OSH) majors were due for re-accreditation and since there was significant overlap between the two courses both majors were revised and combined into one (Occupational and Environmental Health and Safety). This new qualification has subsequently been accredited by Environmental Health Australia (EHA), as well as the Australian OHS Education Accreditation Board (AOHSEAB) and the British Institution of Occupational Safety and Health (IOSH). The new dual accredited course has had excellent graduate outcomes and provides graduates with the flexibility to move between traditional OHS and EH roles. In some smaller local governments, and in industry, ECU graduates have been employed to work in both roles. The curriculum was developed in consultation with industry and has an embedded year-long diploma course that consists of eight units (subjects/ courses) that prepare graduates to work in a support role to EHOs. The Diploma of Environmental Health course is a scaffolded course that is embedded in the degree, and it has been recognized by the State Government as a qualification leading to appointment as authorized officers under the Public Health Act 2016 (WA). The diploma is more than a pathway course as it is a stand-alone qualification that leads to gainful employment. Most students now enroll in the diploma, initially and gain employment as a Technical Officer, and then progress on to complete the EH degree on a part-time basis while employed in the profession. The diploma is also an exit option for students who struggle with the academic requirements of some of the more traditionally difficult units such as physics, chemistry, and statistics and the EH degree is now a viable university course with excellent graduate employment outcomes. Although medical mistrust (MM) may be an impediment to public health interventions, no MM scale has been validated across countries and the assessment of MM has not been explored using item response theory, which allows generalisation beyond the sampled data. We aimed to determine the dimensionality of a brief MM measure across four countries through Mokken analysis and Graded Response Modelling. Analysis of 1468 participants from UK (n = 1179), Ireland (n = 191), India (n = 49) and Malaysia (n = 49) demonstrated that MM items formed a hierarchical, unidimensional measure, which is very informative about high levels of MM. Possible item reduction and scoring changes were also demonstrated. This study demonstrates that this brief MM measure is suitable for international studies as it is unidimensional across countries, cross cultural, and shows that minor adjustments will not impact on the assessment of MM when using these items. A large-coverage pooled genome-wide association scan (GWAS) of knee osteoarthritis (OA) and the results from successive test of individual most highly associated single-nucleotide polymorphisms (SNPs) in five case-control studies are reported. Findings show that polymorphism in LD with variants affecting the expression of the PTGS2 gene are associated with susceptibility to knee OA in five independent populations and highlight the importance of inflammatory pathways in OA pathogenesis.

Abstract Background For large scale epidemiological studies clinical assessments and radiographs can be impractical and expensive to apply to more than just a sample of the population examined. The study objectives were to develop and validate two novel instruments for self-reported knee malalignment and foot rotation suitable for use in questionnaire studies of knee pain and osteoarthritis. Methods Two sets of line drawings were developed using similar methodology. Each instrument consisted of an explanatory question followed by a set of drawings showing straight alignment, then two each at 7.5° angulation and 15° angulation in the varus/valgus (knee) and inward/outward (foot) directions. Forty one participants undertaking a community study completed the instruments on two occasions. Participants were assessed once by a blinded expert clinical observer with demonstrated excellent reproducibility. Validity was assessed by sensitivity, specificity and likelihood ratio (LR) using the observer as the reference standard. Reliability was assessed using weighted kappa (κ). Knee malalignment was measured on 400 knee radiographs. General linear model was used to assess for the presence of a linear increase in knee alignment angle (measured medially) from self-reported severe varus to mild varus, straight, mild valgus and severe valgus deformity. Results Observer reproducibility (κ) was 0.89 and 0.81 for the knee malalignment and foot rotation instruments respectively. Self-reported participant reproducibility was also good for the knee (κ 0.73) and foot (κ 0.87) instruments. Validity was excellent for the knee malalignment instrument, with a sensitivity of 0.74 (95%CI 0.54, 0.93) and specificity of 0.97 (95%CI 0.94, 1.00). Similarly the foot rotation instrument was also found to have high sensitivity (0.92, 95%CI 0.83, 1.01) and specificity (0.96, 95%CI 0.93, 1.00). Abstract Background Sexual problems are common among people with coronary heart disease and can adversely affect patients' quality of life. GPs are ideally placed to deal with these problems. Research suggests that GPs are reluctant to address sexual problems but little is known about what currently takes place in practice. The aim of this study was to examine GPs' self-reported behaviour and attitudes to discussing sexual problems with people with coronary heart disease. Method Design: A cross-sectional survey which administered 230 postal questionnaires to a nationally representative, stratified random sample of GPs in the Republic of Ireland. GPs were asked about current practice, knowledge, awareness and confidence in dealing with sexual problems, barriers to addressing sexual problems, and about improving services in this area. Results Responses were available for 61 GPs (27% response rate). Seventy percent of GPs reported that they rarely or never discussed sexual problems with coronary patients. While all GPs believed addressing sexual problems was important, many GPs reported lacking awareness, knowledge and confidence in addressing sexual problems. The main barriers were lack of time, feeling the patient wasn't ready and lack of training in the area. GPs wanted more training and guidelines for practice. Conclusions There is currently no standardised protocol for GPs for dealing with sexual problems among coronary patients. Awareness of these issues appears to be low among GPs. Services could be improved by developing practice guidelines for brief, effective actions or assessments, providing training in the area and improving information resources and support services for referral.

Objectives To measure levels of occupational stress, burn-out, work-life balance, presenteeism, work ability (balance between work and personal resources) and desire to practise in trainee and consultant hospital doctors in Ireland. Design National cross-sectional study of randomised sample of hospital doctors. Participants provided sociodemographic data (age, sex), work grade (consultant, higher/basic specialist trainee), specialty, work hours and completed workplace well-being questionnaires (Effort-Reward Imbalance (ERI) Scale, overcommitment, Maslach Burnout Inventory) and single item measures of work ability, presenteeism, work-life balance and desire to practise. Setting Irish publicly funded hospitals and residential institutions. Participants 1749 doctors (response rate of 55%). All hospital specialties were represented except radiology. Results 29% of respondents had insufficient work ability and there was no sex, age or grade difference. 70.6% reported strong or very strong desire to practise medicine, 22% reported good work-life balance, 82% experienced workplace stress, with effort greatly exceeding reward, exacerbated by overcommitment. Burn-out was evident in 29.7% and was significantly associated with male sex, younger age, lower years of practice, lower desire to practise, lower work ability, higher ERI ratio and greater overcommitment. Apart from the measures of work ability and overcommitment, there was no sex or age difference across any variable. However, ERI and burn-out were significantly lower in consultants than trainees. Conclusions Hospital doctors across all grades in Ireland had insufficient work ability, low levels of work-life balance, high levels of work stress and almost one-third experienced burn-out indicating suboptimal work conditions and environment. Yet, most had high desire to practise medicine. Measurement of these indices should become a quality indicator for hospitals and research should focus on the efficacy of a range of individual and organisational interventions for burn-out and occupational stress. In an effort to achieve higher data-rates in disk-drives, it has been proposed that moving the preamp closer to the head will improve writer performance. This paper will show that the opposite is true: moving the writer closer to the head degrades write current rise-time and overshoot. Although settling time is improved by moving the preamp closer to the head, impedance-matching techniques are sufficient to address this. Ironically, reader considerations may motivate Chip-On-Suspension (COS), but only for MR sensors that are either very high or very low impedance. This paper will detail the noise peaking that results from an impedance-mismatched reader input network. Both reader and writer performances can be improved by using novel architectures and by optimizing their respective interconnects. In this article, we present an experimental study of the overwrite degradation in high speed recording. Recorded hard and easy transitions were directly characterized and correlated to medium speed, write current waveform and media properties. It has been found that, when high medium speed becomes greater than the bubble growth speed, the transition will be recorded when the field gradient is poor, which results in not only large hard transition shift, but also hard transition broadening. Slow field rise time causes further degradation due to the effective slowing down of the bubble growth speed. Write current overshoot can significantly suppress the degradation. Medium coercivity and its time dependence also have shown a direct effect on the overwrite degradation. Objectives To measure levels of psychological distress, psychological wellbeing and self-stigma in hospital doctors in Ireland. Design National cross-sectional study of randomised sample of hospital doctors. Participants provided sociodemographic data (age, sex, marital status), work grade (consultant, higher/basic specialist trainee), specialty and work hours and completed well-being questionnaires (the Depression Anxiety Stress Scale, WHO Well-being Index, General Health Questionnaire) and single-item scales on self-rated health and self-stigma. Setting Irish publicly funded hospitals and residential institutions. Participants 1749 doctors (response rate of 55%). All hospital specialties were represented except radiology. Results Half of participants were men (50.5%). Mean hours worked per week were 57 hours. Over half (52%) rated their health as very good/excellent, while 50.5% reported positive subjective well-being (WHO-5). Over a third (35%) experienced psychological distress (General Health Questionnaire 12). Severe/extremely severe symptoms of depression, anxiety and stress were evident in 7.2%, 6.1% and 9.5% of participants (Depression, Anxiety, Stress Scale 21). Symptoms of distress, depression, anxiety and stress were significantly higher and levels of well-being were significantly lower in trainees compared with consultants, and this was not accounted for by differences in sociodemographic variables. Self-stigma was present in 68.4%. Conclusions The work hours of doctors working in Irish hospitals were in excess of European Working Time Directive's requirements. Just over half of hospital doctors in Ireland had positive well-being. Compared with international evidence, they had higher levels of psychological distress but slightly lower symptoms of depression and anxiety. Two-thirds of respondents reported self-stigma, which is likely to be a barrier to accessing care. These findings have implications for the design of support services for doctors, for discussions on quality of patient care and for future research. [ABSTRACT FROM AUTHOR]
