The article presents a study conducted to 469 emotionally disturbed children from 17 child treatment centers in the U.S. and from a treatment center in Canada to compare the season of conception with the season of birth. The method was based on the grouping of the calendar year into cold months of December, January, and February; the hot months of June, July, and August; and the remaining other months. Results depict consistency with the often-cited relationship between the seasonality of birth and the psychiatric disturbance. However, as indicated, temperature or temperature-related influence near the time of conception was found to be a relevant factor, rather than the time of birth. Thus, further study of their behavioral characteristics during conception in hot months are required.

Epiploic appendagitis (EA) is an uncommon intraabdominal pathology resulting in transient, localized pain. The condition is caused by ischemia of one of the epiploic appendages, which are distributed axially along the length of the colon. EA is often mistaken for other more common etiologies of an acute abdomen. Generally, the patients experience focal abdominal pain with no further symptoms or laboratory abnormalities. The authors encountered a 79-year-old male with severe sepsis and acute respiratory failure requiring intubation. He recovered rapidly after the identification and removal of a single EA. This paper reports the first case of EA leading to the systemic dysregulation of sepsis.

Scapular dyskinesis, the impairment of optimal scapular position and motion, is common in association with shoulder injury. A comprehensive evaluation process can show the causative factors and lead to effective treatment protocols. The complexity of scapular motion and the integrated relationship between the scapula, humerus, trunk, and legs suggest a need to develop rehabilitation programs that involve all segments working as a unit rather than isolated components. This is best accomplished with an integrated rehabilitation approach that includes rectifying deficits in mobility, strength, and motor control but not overtly focusing on any one area., (Copyright © 2022 Elsevier Inc. Sex and gender differences have long been a research topic of interest, yet few studies have explored the specific differences in neurological responses between men and women during and after spaceflight. Knowledge in this field is limited due to the significant disproportion of sexes enrolled in the astronaut corps. Research indicates that general neurological and sensory differences exist between the sexes, such as those in laterality of amygdala activity, sensitivity and discrimination in vision processing, and neuronal cell death (apoptosis) pathways. In spaceflight, sex differences may include a higher incidence of entry and space motion sickness and of post-flight vestibular instability in female as opposed to male astronauts who flew on both short- and long-duration missions. Hearing and auditory function in crewmembers shows the expected hearing threshold differences between men and women, in which female astronauts exhibit better hearing thresholds. Longitudinal observations of hearing thresholds for crewmembers yield normal age-related decrements; however, no evidence of sex-related differences from spaceflight has been observed. The impact of sex and gender differences should be studied by making spaceflight accessible and flying more women into space. Only in this way will we know if increasingly longer-duration missions cause significantly different neurophysiological responses in men and women.

Sequenced physiologic muscle activations in the upper and lower extremity result in an integrated biomechanical task. This sequencing is known as the kinetic chain, and, in upper extremity dominant tasks, the energy development and output follows a proximal to distal sequencing. Impairment of one or more kinetic chain links can create dysfunctional biomechanical output leading to pain and/or injury. When deficits exist in the preceding links, they can negatively affect the shoulder. Rehabilitation of shoulder injuries should involve evaluation for and restoration of all kinetic chain deficits that may hinder kinetic chain function. Rehabilitation programs focused on eliminating kinetic chain deficits, and soreness should follow a proximal to distal rationale where lower extremity impairments are addressed in addition to the upper extremity impairments. A logical progression focusing on flexibility, strength, proprioception, and endurance with kinetic chain influence is recommended.

Background and Purpose: Head stability is the dynamic process of maintaining an equilibrium position of the head-in-space. Individuals with vestibular deficits restrict head movements during dynamic activities in an effort to adapt to vestibular loss. However, this strategy does not provide them with a successful means for adaptation during dynamic tasks where head movements are required. Therefore, identification of successful head stabilization strategies is the first step towards improving the rehabilitation of these patients. The purpose of the present study was twofold: to characterize the sagittal plane head stabilization response during walking; and to identify successful head stabilization strategies during normal walking and during a walking task that challenged head stability., Method: The study used a repeated-measures design. Eight healthy volunteers walked normally (normal condition) and walked whilst swinging their arms at twice the natural frequency (frequency condition). Head and trunk angular velocities were measured to determine head velocity magnitudes, and head-on-trunk, with respect to trunk gains and phases across the frequency spectrum of walking., Results: The frequency condition increased the challenge to head stabilization and produced phases indicative of increased head stability (p < 0.05). Post hoc analyses revealed that the shift in phase occurred at higher frequencies (> 8 Hz) (p < 0.05). Increased head velocity magnitudes (p < 0.05) accompanied by decreased variability (p < 0.05) were also found at higher frequencies for the frequency condition., Conclusions: The results were indicative of a tightly controlled movement strategy that ensured head stabilization under conditions where head stability was challenged. This strategy was characterized by head-on-trunk movement that was equal and opposite to trunk motion.

Clinically depressed (n = 20), previously depressed (n = 28), and nondepressed control (n = 27) individuals, classified according to a structured clinical diagnostic interview, participated in a study employing a modified prior entry (Titchener, 1908) procedure to investigate interrelationships among word (adjective) valence, visual attention, and cerebral hemispheric activity. Overall, positive words were selected more quickly when presented to the right, versus left, visual field (RVF, LVF); the opposite pattern was observed for negative words. While there was no significant group X Valence X Visual Field interaction, planned comparisons revealed that the aforementioned Valence X Visual Field interaction was significant only for the nondepressed control group. Although the remitted group exhibited an overall pattern similar to the control group, the depressed group evinced a pattern in the opposite direction for positive words (i.e., quicker in the LVF than the RVF).

Study Design: Descriptive study examining kinematic and electromyographic (EMG) patterns of the upper body during walking., Objective: To examine trunk, neck, and head movements to determine a mechanism for upper body stabilization during walking., Background: Dynamic balance of the upper body during walking provides a stable base for function of sensory systems. Prior investigations of upper body motion during walking were limited to examination of isolated segments, or examination of the upper body as a single unit. In our study, the upper body is examined as 3 segments: the trunk, neck, and head., Methods and Measures: Sagittal plane walking patterns were examined in 8 unimpaired young adults. Markers placed on the trunk, neck, and head segments were recorded on videotape. Angles were calculated with respect to an external horizontal reference to determine segment position relative to space. EMG measures were obtained from erector spinae, rectus abdominus, semispinalis capitis, and sternocleidomastoid muscles., Results: Results showed dynamic stability was accomplished through maintenance of a posture where the trunk was flexed, the neck was extended and the head was flexed. The trunk segment demonstrated greatest stability with the neck being the least stable of the 3 segments. Movements of upper body segments showed a tendency for the head and neck to move opposite to the trunk. EMG data demonstrated erector spinae muscle activity occurring near heel contact of each limb followed by trunk extension. The remaining muscles exhibited variable patterns of activity., Conclusions: These data indicate that movements of the upper body help to maintain a posture that promotes stability of these segments during walking. The trunk was the most stable of the three segments thereby, providing a stable platform for head and neck movement. Erector spinae muscle activity contributed to upper body movements by extending the trunk to maintain balance at heel contact. These results provide a basis for studying changes in dynamic stability that occur with age.

Frequency characteristics of head stabilization were examined during locomotor tasks in healthy young adults(N = 8) who performed normal walking and 3 walking tasks designed to produce perturbations primarily in the horizontal plane. In the 3 walking tasks, the arms moved in phase with leg movement, with abnormally large amplitude, and at twice the frequency of leg movement. Head-in-space angular velocity was examined at the predominant frequencies of trunk motion. Head movements in space occurred at low frequencies (< 4.0 Hz) in all conditions and at higher frequencies (> 4.0 Hz) when the arms moved at twice the frequency of the legs. Head stabilization strategies were determined from head-on-trunk with respect to trunk frequency profiles derived from angular velocity data. During natural walking at low frequencies (< 3.0 Hz), head-on-trunk movement was less than trunk movement. At frequencies 3.0 Hz or greater, equal and opposite compensatory movement ensured head stability. When arm swing was altered, compensatory movement guaranteed head stability at all frequencies. Head stabilization was successful for frequencies up to 10.0 Hz during locomotor tasks. Maintaining head stability at high frequencies during voluntary tasks suggests that participants used feedforward mechanisms to coordinate head and trunk movements. Maintenance of head stability during dynamic tasks allows optimal conditions for vestibulo-ocular reflex function.

Objective: This article describes a series of studies designed to evaluate the reliability and validity of the Adult Sensory Profile., Method: Expert judges evaluated the construct validity of the items. Coefficient alpha, factor analysis, and correlations of items with subscales determined item reliability, using data from 615 adult sensory profiles. A subsample of 20 adults furnished skin conductance data. A heterogeneous group of 93 adults completed the revised Adult Sensory Profile, and item reliability was reexamined., Results: Expert judgment indicated that items could be categorized according to Dunn's Model of Sensory Processing. Results suggested reasonable item reliability for all subscales except for the Sensation Avoiding subscale. Skin conductance measures detected distinct patterns of physiological responses consistent with the four-quadrant model. Revision of the Adult Sensory Profile resulted in improved reliability of the Sensation Avoiding subscale., Conclusion: The series of studies provides evidence to support the four subscales of the Adult Sensory Profile as distinct constructs of sensory processing preferences.

The purposes of this study were to 1) characterize head stabilization in older adults across the frequency spectrum of walking, and 2) assess the ability of older adults to adapt head-trunk coordination to maintain head stability in the absence of vision. Sagittal plane head and trunk angular velocities in space were measured for 17 healthy older adults while walking with eyes open (EO) and eyes closed (EC). Average walking velocity and cadence were also determined. Frequency analyses were used to examine head velocities in space and head-on-trunk with respect to trunk gain and phase values across the frequency spectrum of walking. Average walking velocity decreased with EC. Head stability was maintained during EO, but decreased during EC as indicated by increased head velocities across the frequency spectrum. Gain values increased while phase values remained similar during EC walking demonstrating diminished coordination of head-trunk movements. When relying on vestibular and proprioceptive information, older adults were unable to adapt head-trunk movements suggesting impaired plasticity of these systems with age.

Background and Purpose: Head stabilization in space provides a stable reference frame for visual and vestibular systems during locomotor tasks. The purpose of the present study was to examine upper body movement patterns used to maintain head stability in response to the increased challenge of stair ambulation., Method: A repeated-measures study design was used and the order of conditions was randomized for each subject. Eight healthy, non-disabled, young adults walked 10 metres on a level surface, ascended and descended four steps. Head, neck and trunk angular positions in space were obtained for three trials of each condition. Significant differences in mean head, neck and trunk angular orientation in space and segment excursions were determined. Cross-correlation analyses between head and neck, neck and trunk, and head and trunk determined movement patterns used for maintaining head stability. The chi-square test examined the homogeneity of movement pattern distributions for each segment pair across conditions., Results: Head in space flexion orientation and angular excursions were greater during stair ascent and greater still during stair descent as compared to level walking. This indicates a progressively significant increase in the challenge to head stability. Neck flexion orientation in space and angular excursion were greater during stair descent as compared to level walking. Movement patterns between segment pairs demonstrated phase leads and lags between segments when threats to head stability were minimal. With increased challenge to head stability, segments moved synchronously in opposite directions to maintain balance. With further challenges to head stability, segments moved synchronously in the same direction to maintain body segment alignment., Conclusions: Head stabilization was significantly challenged when ascending stairs and is further challenged while descending stairs. Movement patterns reflected changes associated with challenges to head stability. Increased head excursion during stair ambulation may reflect the need to orient vision for foot placement. Results suggest that individuals with visual or vestibular deficits may be at risk for falls during stair ambulation.

Thirteen depressed and 13 non-depressed college students attended to valence-loaded word pairs (euphoric/dysphoric, euphoric/neutral, and neutral/dysphoric) on a computer screen. Each pair was observed through a viewing box with a vertical partition, each word in a different visual field. As a prior-entry task (Titchener, 1908) the words were simultaneously replaced by colored bars. Participants indicated which color bar (left or right) was seen first. As predicted, identifications of color bars following euphoric words in the right visual field (left hemisphere) exceeded their identification in the left visual field (right hemisphere). Also, as predicted, the non-depressed participants made identifications following the euphoric word of a pair more often than did the depressed participants. No interaction occurred between laterality and participant classification. Implications for research and therapy are discussed.

Objectives: Non-depressed and depressed persons were hypothesized to input and recall self-referential information that was consistent with their self theories., Design: Non-depressed and depressed persons were compared., Methods: Non-depressed (14) and depressed (14) students chose between positive or negative audio statements, and then recalled the statements., Results: Non-depressed relative to depressed participants: (i) listened more to the positive than the negative tapes; and (ii) fabricated (i.e. incorrectly recalled) less negative messages., Conclusion: Partial support is given to reality negotiation processes.

Twenty college undergraduates (10 depressed, 10 nondepressed, in terms of the Beck Depression Inventory and the Structured Clinical Interview for the DSM-III-R) chose freely from a positive or negative tape-recorded message. They also rated the tonal quality of each tape. The nondepressed group selected the positive message more often than the negative message. However, the depressed group showed no difference in tape preference. The nondepressed individuals rated the negative tape as inferior in tonal quality, whereas no difference in tonal quality was reported by the depressed individuals. Results appear to support a response set not only toward positive cognitions among nondepressed individuals, which is absent among depressed individuals, but also toward perceived tonal quality differences.

Irrelevant stimuli that flank a fixated target may cause either facilitation or interference with target classification. Twenty schizophrenic patients, 20 depressed control patients, and 20 normal control subjects were compared on a flanker priming task that involved the linear display of a target surrounded by two flanking letters or digits. Choice reaction time between letter and digit targets was examined as a function of flanker condition and onset asynchrony between flankers and target. Facilitative priming occurred only with prior exposure of flankers compatible with the response required and was greater in degree with schizophrenic and depressed than with normal subjects. Interference from flankers incompatible with the response required less among schizophrenics than among other groups. Several different processes may be involved in the inhibition of irrelevant information by schizophrenics.

Use of clinical diagnosis as the phenotype for genetic study is discussed, and criteria for appropriate alternative phenotypes are presented. Then, recent research on four alternatives is discussed: (a) resistance to interference from an associative prime, (b) reaction time crossover, (c) creativity, and (d) socioeconomic achievement. It is concluded that resistance to interference from an associative prime and reaction time crossover at 3-sec. preparatory interval level are the most promising of these candidates.

On the basis of an earlier study that reported depressed patients to be superior to normal control subjects in visual iconic integration, the present investigation sought to determine if this finding was replicable and if medication produced it. Newly admitted depressed patients entering drug treatment, depressed patients already in drug treatment, and healthy control subjects were twice tested, with a 5-week interval between sessions. Fully medicated depressed patients, whether newly admitted or already in drug treatment, were superior to normal control subjects in visual integration, and healthy control subjects were superior to depressed patients just entering treatment. At the first testing, the number of days of medication treatment was significantly correlated with the adequacy of visual integration.

Twenty schizophrenic patients, 10 depressed control patients, and 20 normal control subjects were compared in a forced-choice, target-detection method for assessing the span of apprehension. The detection task required the subject to report which of 2 target letters was presented among 7 other (distractor) letters. Performance accuracy was examined as a function of target location and whether the distractor letters were masked after their presentation. The backward masking of the distractors improved target-detection accuracy of both control groups but reduced accuracy of the schizophrenic group. In addition, schizophrenics performed particularly poorly on targets located in the left half or lower half of the display. These results suggest that response to the masking of distractors may be a new index of attentional shortcoming in schizophrenia. Various theoretical explanations for the target location findings are also discussed.

Metadiagnostic guidelines are delineated for evaluating the utility of both existing diagnostic criteria and recently proposed revisions (Magaro 1980). Among the metadiagnostic guidelines are the demonstration of differential treatment-outcome relationships for different disorders and the applicability of the characteristics of a superordinate diagnosis to its subordinate (or subtype) diagnoses. The authors note the conceptual development of the schizophrenia construct. Attempts to verify the existence of subtypes of schizophrenia in psychoanalytic theory, demographic traits, psychological assessment, and behavioral research are reviewed. Data accumulated from these efforts provide evidence regarding the validity of the distinction between paranoid and nonparanoid schizophrenia proposed by Magaro (1980). The authors conclude that a number of alternate relationships between paranoid and nonparanoid subtypes may by hypothesized. Only the notion that paranoid and nonparanoid subtypes are representative of differences in severity of schizophrenia may be rejected. As yet, little evidence exists for choosing one of the remaining alternatives as correct. Integrating research from diverse fields of study is advocated in the effort to refine conceptions of psychiatric disorders. In addition to continued attempts to make improvements in psychiatric diagnosis by changing the clinical symptoms used to define disorders, refinements may be accelerated by assessing the potential utility of other sources of information as the basis for psychiatric diagnosis. Performance on tasks employed in behavioral psychopathology research may represent such an example.

A clarification and response to a critique of the Multisystem-Multimethod (MS-MM) family assessment is provided in this paper. This approach consists of a logic for integrating family assessment, systems thinking, and clinical interventions. In contrast to the wholistic, or systemic, focus of MS-MM family assessment, the critique of these procedures recommends a more reductionistic approach that contradicts basic elements of systems theory. The critique advocates the isolation and measurement of a few family phenomena, whereas MS-MM assessment recognizes the interdependence among the levels of organization and components of family systems.

The goal of this study was to estimate the loads internal to the lumbar trunk that arise during level walking. To do this, (a) trunk muscle activities were calibrated in terms of muscle contraction force levels in a set of isometric exertions; (b) trunk muscle myoelectric activities were measured during level walking; and then (c) the muscle contraction forces that arose during walking were calculated from these measurements and calibrations. Lumbar trunk muscle myoelectric activities were quantified in 10 healthy young males. Myoelectric activities were monitored using eight bipolar surface electrode pairs placed around the trunk at the level of the third lumbar vertebrae. The subjects first performed four static weight-resisting tasks to calibrate muscle force/activity relationships. They then traversed a 8.25 m walkway three times each at cadences of 72 and 120 steps/min. A biomechanical model incorporating 22 lumbar trunk muscles was used to predict muscle contraction forces for the calibration tasks. Predicted forces were linearly correlated with the measured myoelectric activities for these tasks. The regression equations were then interpolated to estimate the muscle contraction forces from the myoelectric data during gait. Peak contraction forces for the iliocostalis muscles were calculated to be approximately 55 N per side, corresponding to total erector spinae peak contractions on the order of 140 N per side. For the other six muscles that were monitored, contraction forces were less than 15 N per side. This suggests that peak net reaction moments and peak spine compressions on the lumbar trunk during these walking tasks were on the order of 15 Nm and 1.2 times body weight, respectively.

The authors propose that time be considered as a variable in family process. Previous theory and research on human temporal patterning and experience are reviewed, and empirical generalizations from an initial pilot study concerned with marital and family members' orientation to "morningness" and "nightness" are presented. Further implications from this work-in-progress are delineated.

The authors describe a program to reduce the use of full-leather restraints on an acute adult inpatient psychiatric ward of a large community mental health center. It included the introduction of a multipurpose pipe-frame bed, which can be used for seclusion only or seclusion with restraints. Use of the bed, coupled with a seclusion-first policy and rapid tranquilization programs, reduced the time the patients spent in full-leather restraints from 360 hours in the month before the program was introduced to 30 hours two months later.
