In India, there are marked variations in resources for cervical cancer screening. For the first time, resource‐stratified screening guidelines have been developed that will be suitable for low middle‐income countries with similar diversities. The current article describes the process and outcomes of these resource stratified guidelines for screening and treatment of preinvasive lesions of cervix. Evidence from literature was collated and various guidelines were reviewed by an expert panel. Based on the level of evidence, guidelines were developed for screening by human papillomavirus (HPV) testing, cytology and visual inspection after application of acetic acid (VIA), and management of screen positive lesions in different resource settings. Expert opinion was used for certain country‐specific situations. The healthcare system was stratified into two resource settings – good or limited. The mode of screening and treatment for each was described. HPV testing is the preferred method for cervical cancer screening. VIA by trained providers is especially suitable for low resource settings until an affordable HPV test becomes available. Healthcare providers can choose the most appropriate screening and treatment modality. A single visit approach is encouraged and treatment may be offered based on colposcopy diagnosis ('see and treat') or even on the basis of HPV test or VIA results ('screen and treat'), if compliance cannot be ensured. The Federation of Obsterician and Gynaecologists of India Good Clinical Practice Recommendations (FOGSI) GCPR are appropriately designed for countries with varied resource situations to ensure an acceptable cervical cancer prevention strategy. Background: One in every five women suffering from cervical cancer belongs to India. Objectives: In this study, an attempt has been made to screen for cervical cancer in all sexually active females attending Gynaecology Out Patient Department (OPD), segregate operable and inoperable cases, study varying operative management of diagnosed cases of cervical cancer, and study the outcome and complications of the same. Materials and Methods: This was a prospective study carried out in the Department of Obstetrics and Gynaecology in a tertiary care referral hospital in Mumbai amidst the largest urban slum in Asia, Dharavi from January 2011 to December 2016. A total number of 36,100 patients attended the Gynecology OPD during the study period. Being an observational study, only percentage calculations were used in this study. Results: Incidence of neoplasia in this cohort by Pap smear cytology was 0.48%, thus proving the sensitivity of Pap smear to be 84.57%. Most of patients with cervical cancer belonged to the age group 30-39 years (35.9%), lower socioeconomic class (78.18%), and were multiparous (66.36%). Most common presenting complaint was abnormal vaginal discharge (including bleeding) occurring in 87%. Most common histopathological type was squamous cell carcinoma (81.81%). Total abdominal hysterectomy was done in 12 patients (Stage 1a1), Type 2 hysterectomy with pelvic lymph node dissection was done in 16 patients (Stage 1a2, Stage 1b1), and Type 3 hysterectomy with pelvic lymph node dissection was done in 15 patients (Stage 1b2, Stage 2a). Conclusion: Women continue to be afflicted by a disease that is potentially preventable and treatable when detected early. India is a largest producer of pomegranate with high export value. The cultivation is affected with the oily spot disease caused byXanthomonas axonopodis pv. punicaeinfection. The present study aims to control the disease with newer biocontrol methods. Thirty-six isolates ofX. axonopodiswere isolated from different varieties of infected pomegranates fruits from Maharashtra. Forty strains of actinomycete were also isolated from natural sources and screened for their antagonistic activity againstX. axonopodisisolates. Eight strains of actinomycete were screened out for their high antagonistic activity and were optimized for maximizing antibiotic production. The extracted compound from A5 strain exhibited maximum inhibitory activity against all the pathogenic isolates with a MIC in the range of 0.625 to 1.25 mg mL−1. It was identified asStreptomyces violaceusnigeby 16SrRNA gene sequencing (Accession number KP208943). The extracted compound belonged to aminoglycosides with a molecular formula C22H28N3O6determined by thin layer chromatography, high-performance liquid chromatography, gas chromatography-mass spectroscopy, nuclear magnetic resonance spectroscopy, fourier transform infrared spectroscopy and carbon hydrogen nitrogen ratio analysis. In vivo biocontrol studies with strain A5 and its extracted compound effectively prevented the growth 36Xanthomonasisolates inoculated on pomegranate fruits, illustrating its biocontrol potential against the oily spot disease of pomegranate. A series of novel superporous hydrogels for regenerative medicine were prepared by oil-in-water (o/w) or inverse high internal phase emulsion (i-HIPE) copolymerization of glycerol monomethacrylate (GMMA), 2-hydroxy ethyl methacrylate (HEMA) and glycerol dimethacrylate (GDMA) as a cross-linker using a non toxic solvent and a redox initiator system at the physiological temperature (37 °C). The monomer GMMA was synthesized from glycidyl methacrylate (GMA) by an alternative facile method using Amberlyst-15. The described i-HIPEs showed a significantly wider stability window. The polyHIPE hydrogels were characterized by FTIR, BET method for surface area, mercury porosimetry, SEM, DSC, TGA, XRD, compressive strain and strain recovery. The swelling ratio of the hydrogels and their degradation in 0.007 M NaOH and lipase B (Candida antarctica) solutions were determined gravimetrically and the rate of degradation was explained in terms of the molecular structure of the hydrogels. The morphological studies showed that the pore diameter varied between 20 and 30 μm and the pore throats (interconnecting windows) diameter was in the range of 4–8 μm. The described polyHIPE hydrogels were found to have an open cell morphology and interconnected pore architecture, which are important characteristics for scaffold applications. The initial cytotoxicity study performed according to ISO-10993-5 indicated cytocompatibility (97% cell viability) and the subsequent cell seeding and proliferation study exhibited 55–88% cell viability (increased monotonously from GHG-1 to GHG-5), which could be attributed to modulation of the physical and chemical properties of the hydrogels. Photocopolymerization of heterocyclic monomer namely, tetrahydrofurfuryl methacrylate with bulky bicyclic monomer, isobornyl methacrylate with different feed ratios was carried out in bulk with low concentration of an α-hydroxyl ketone based photoinitiator. The ambient temperature photocopolymerization was carried out by using a UV-Visible lamp with fixed low intensity of 0.4 mW cm−2for a period of 6 min. The residual monomer remained in the polymerization process were determined by using gas chromatography. The reactivity ratio values for the two monomers were calculated from the copolymer composition data by using Fineman-Ross, Kelen-Tudos, Extended Kelen-Tudos and Mao-Huglin methods. Individually, as well as the average of all the methods revealed that the monomer reactivity ratios of tetrahydrofurfuryl methacrylate were higher than isobornyl methacrylate. The dyad sequence distribution and dyad sequence lengths were calculated using the Igarashi and Pyun method and the sequence length distribution for tetrahydrofurfuryl methacrylate was observed to be higher with an increase in its feed content. This supports the reactivity ratio studies that a higher monomer reactivity ratio value for tetrahydrofurfuryl methacrylate was observed as compared to its comonomer. Silica nanoparticles were modified with aromatic amino groups and modified-silica/polyimide composite films were prepared using them. 3,3',4,4'-Benzophenone tetracarboxylic dianhydride (BTDA) and 4,4'-oxydianiline (ODA) were used as precursors for polyimide matrix. The structures of the modified nanoparticles and hybrid nanocomposites were identified using Fourier Transform Infrared (FTIR) spectrometry. The hybrid composite films were evaluated for mechanical, thermal and morphological characteristics. Morphological results describe a uniform dispersion of silica particles in the polymer matrix. The thermal stability and mechanical properties of polyimide composite were improved, and the decomposition temperature was increased when the amount of silica nanoparticles was increased. Synthesis of some new functionalized thiazolidin-4-ones and α-amino phosphonate derivatives has been reported. The imines were synthesized from the reaction of various substituted anilines with 1-phenyl-3-(pyridine-4-yl)-1H-pyrazole-4-carbaldehyde in ethanol at reflux condition. The corresponding thiazolidin-4-ones and α-aminophosphonates were prepared by reaction of imines with mercaptoacetic acid and triethyl phosphite, respectively. The structures of the newly synthesized compounds were confirmed by IR, 1H NMR, and mass spectral data and were evaluated for their antimicrobial activities. The objective of this study was to investigate the influence of type of cross-linking method used on the properties of ambroxol hydrochloride microspheres such as encapsulation efficiency, particle size, and drug release. Microspheres were prepared by solvent evaporation technique using chitosan as a matrix-forming agent and cross-linked using formaldehyde and heat treatment. Morphological and physicochemical properties of microspheres were then investigated by scanning electron microscopy (SEM), X-ray diffractometry (XRD), differential scanning calorimetry (DSC), and Fourier-transform infrared spectroscopy (FTIR) spectroscopy. The cross-linking of chitosan takes place at the free amino group because of formation of imine bond as evidenced by FTIR. The DSC, XRD, and FTIR analysis showed that chitosan microspheres cross linked by heating were superior in properties and performance as compared to the microspheres cross-linked using formaldehyde. SEM results revealed that heat-treated microspheres were spherical, discrete having smooth, and porous structure. The particle size and encapsulation efficiencies of the prepared chitosan microspheres ranged between 10.83-24.11 μm and 39.73-80.56%, respectively. The methanolic extract of young and mature leaves of nine coastal plants from West coast of Maharshtra were analysed to evaluate free radical scavenging activity. The antioxidant activity was evaluated using 1,1-diphenyl-2-picryl hydrazyl radical (DPPH) and the reductive potential of the methanolic extract. The highest antioxidant activity was recorded in young leaves of Hibiscus tiliaceus (76 %), followed by Syzigium corymbosa (71 %) Calophyllum inophyllum (68 %) and Colubrina asiatica (55 %). The plant species also posseses appreciable reductive potential. The leaves of these species were found to be rich in Flavanoids (6.03 to 16.63 mg /g of dry weight) and total polyphenol (12.12 to 26.23 mg /g of dry weight) and these compounds mainly contributed the antioxidant potential of these plants. Background: Need for undertaking prostate biopsies for detection of prostate cancer is often decided on the basis of serum levels of prostate specific antigen (PSA). Aim: To evaluate the case detection rate of prostate cancer among patients presenting with lower urinary tract symptoms (LUTS) on the basis of PSA levels and to assess the scope of prostate biopsy in these patients. Setting and Design: A retrospective study from a tertiary care center. Materials and Methods: The clinical and histopathological data of 922 patients presenting with LUTS in the last five years was obtained from the medical record section. They had been screened for prostate cancer using PSA and /or digital rectal examination examination followed by confirmation with prostate biopsy. Statistical Analysis Used: Detection rate and receiver operating characteristic curve were performed using SPSS 16 and Medcalc softwares. Results: The detection rate of prostate cancer according to the PSA levels was 0.6%, 2.3%, 2.5%, 34.1% and 54.9% in the PSA range of 0-4, 4-10, 10-20, 20-50 and >50 ng/ml, respectively. Maximum prostate cancer cases were detected beyond a PSA value of 20 ng/ml whereas no significant difference in the detection rate was observed in the PSA range of 0-4, 4-10 and 10-20 ng/ml. Conclusion: A low detection rate of prostate cancer observed in the PSA range of 4-20 ng/ml in LUTS patients indicates the need for use of higher cutoff values of PSA in such cases. Therefore we recommend a cutoff of 20 ng/ml of PSA for evaluation of detection rate of prostate cancer among patients presenting with LUTS. [ABSTRACT FROM AUTHOR]

Engineering transcription factors (TF) hold promise in enhancing abiotic stress tolerance in plants. In this study, one of the popular rice varieties of South India, namely ADT 43, was engineered with a TF AtDREB1A driven by a stress-inducible rd29A promoter. PCR and Southern hybridization were employed to confirm the integration and copy number of the transgene. Transgenic lines (T1) of ADT 43 showed enhanced tolerance to drought and salinity compared to the non-transgenic ADT 43. Transgenic lines were found to maintain higher RWC %, lower leaf temperature, and partially closed stomata, enabling better survival under stress conditions. qRT-PCR analysis revealed the strong induction of AtDREB1A transcripts during drought. Transgenic lines of ADT 43 exhibited increased germination and retention of chlorophyll in their leaves under salinity. Evaluation of transgenic lines under transgenic screen house conditions revealed that line # A16 exhibited on par agronomic performance against its non-transgenic counterpart under normal conditions. Under drought, non-transgenic ADT 43 showed >20% reduction in the total number of spikelets per panicle, whereas transgenic line # A16 registered only a 2% reduction. Non-transgenic ADT 43 recorded 80% yield reduction under drought, whereas line # A16 recorded only 54% yield loss. The above results demonstrated the effectiveness of controlled expression of DREB1A in regulating dehydration responses in rice.

In this investigation additives are used to improve the performance, combustion and emission characteristics of Mahua biodiesel (B) and Diesel (D) blends. B20 blend is used as basic blend to compare the performance.Ethanol (E) and Propanol (P) used as additives indiesel-biodiesel blend with 5%,10% and 15%and tested in VCR diesel engine. Ethanol shown better performance in particular as compare to Propanol. The reduction of NOx, CO, CO2, HC emission in diesel engine and also increase performance in addition of 10% ethanol. From this conclude the result ethanol is alternative to improve efficiency of diesel engine by using the blended biodiesel.

We have used surgical gauze under the prepuceal skin as a pack in 20 cases prior to marking incision for circumcision. The prepuceal adhesions were first dissected and seperated. The method allows a stable, well-supported prepuceal surface for marking incisions and avoids injuries to the glans. Because the prepuceal surface is taut and stable, hemostasis is easier and quicker and the operating time is reduced.

Aim: National data registries provide a valuable source of data for epidemiological research but may be subject to inaccuracies. Whilst studies have compared agreement between cause of death (COD) data from Australia and New Zealand's dialysis and transplant registry (ANZDATA) to other databases, no studies have manually compared agreement with electronic medical records (EMR). This study aimed to assess the agreement between COD for dialysis patients in the Western Sydney renal service according to ANZDATA and EMR., Methods: We conducted a retrospective cohort study on dialysis patients from the Western Renal Service who died between January 1, 2016 and July 31, 2022, inclusive. We matched ANZDATA patient records to our service's EMR, extracted data on COD from EMR and coded COD according to ANZDATA's categorisation system. Agreement between COD from EMR and ANZDATA was assessed using kappa statistics., Results: There were 709 deaths in the study period. 449 (63.3%) were male and 260 (36.7%) were female. Of the 539 patients with available records, the most common COD categories were cardiovascular (201, 37%) and infection (154, 29%). There was poor agreement between individual COD from EMR and ANZDATA (kappa of 0.39). However, this improved when comparing broader COD categories (kappa of 0.53). Subgroup analysis showed no difference in agreement between males and females or patients who died between 2016-2018 and 2019-2022., Conclusion: There is poor agreement between ANZDATA and EMR for individual COD. Abstract: An R-code, which allows the calculation of the time dependent activity distribution based on ICRP reference models, the number of decays in a commitment period, and the dose coefficients for tissues and organs of the human body, has been developed. R Language was chosen due to its powerful mathematical and statistical modeling features, as well as its graphical capabilities. The developed set of functions and constants (called "INTDOSKIT") can be sourced in R-scripts that define or import the models and calculations to be performed. The code has been tested on models of several radionuclides and was successfully validated against reference data taken from ICRP OIR Data Viewer software. Furthermore, the code has been tested and verified on the modeling of the radioactivity of decay chains using data of the 233Ra model presented by Höllriegl and colleagues. The results of calculations with INTDOSKIT demonstrated that the code is able to reproduce the ICRP bioassay data and dose coefficients. Deviations are a few percent only and are due mainly to rounding in the original data. Lastly, the code is able to handle uncertainty and sensitivity studies as demonstrated by the results in a pilot study of injection of 241Am, which estimated geometric standard deviations (GSD) for dose coefficients ranging between 1.25 (bone-surface) and 1.66 (testes); these results are consistent with those obtained from similar studies done by other researchers who reported GSD values ranging from 1.13 to 1.73., (Copyright © 2024 Health Physics Society.)

A cerebellar infarct occurs when blood flow to the cerebellum, located in the posterior cranial fossa, is disrupted. This diminished blood supply leads to decreased oxygen delivery, resulting in motor and balance control impairments. One prevalent sign of neurodegenerative diseases is dysphagia, which is typically linked to a higher death rate. No systematic and uniform assessment of dysphagia is used in the clinical care environment of individuals with ataxia. Its effect on the quality of life associated with health in patients is little understood. Therefore, this case report works to address dysphagia in cerebellar ataxia. This case report examines the physiotherapy management of a 41-year-old male who had cerebellar ataxia secondary to an infarct in the bilateral cerebellar hemisphere and vermis. The rehabilitation period lasted for six weeks. On examination, the patient had difficulty swallowing and showed symptoms of cerebellar dysfunctions, such as nystagmus, dyssynergia, dysmetria, and dysdiadochokinesia. Neuro-physiotherapy interventions, like conventional physiotherapy, trunk, and pelvis proprioceptive neuromuscular facilitation (PNF), Kinesio taping for dysphagia, interventions to treat gait, balance training interventions, and Frenkel's exercises were commenced. The outcome measures were evaluated using standardized outcome measures like the Swallowing Quality of Life Scale (SWAL-QOL), Severity of Ataxia Scale (SARA), Trunk Impairment Scale (TIS), Berg Balance Scale (BBS), Barthel Index, and World Health Organization Quality of Life (WHO-QOL). We conclude that a properly structured physiotherapy program subsequently improved the symptoms of patients. Erb's palsy usually commonly arises from incidents such as falls, collisions, birth trauma, and shoulder injuries in children. It impairs upper extremity muscle function, which has an impact on the quality of life and social interaction. Physical therapy is beneficial in preserving and enhancing upper extremity function, improving the quality of life. The study thoroughly explores the chemical components derived from various natural sources, thus emphasizing their unique structural characteristics and therapeutic potential as an anticancer agent. The review contains the critical chemical constituents' in-depth molecular mechanisms, their source's chemical structures and the categories. The review also comprises an exhaustive and comprehensive analysis of different chemical spacing parameters of the anticancer agents derived from natural products. It compares them with USFDA-approved synthetic anticancer drugs up to 2020, thus providing a meaningful understanding of the relationship between natural and synthetic compounds portraying the anticancer assets. The review also delves more deeply into the chemical analysis of the heterocyclic moieties from the natural product arena, illustrating the anticancer mechanisms. Myelopathy manifests in childhood and can be clinically categorized according to the site of injury (which may result in spinal syndrome) or the source (which may be nontraumatic or widely traumatic). Nontraumatic myelopathy can be caused by inflammatory, infectious, nutritional, metabolic, or ischemic factors. It may also be associated with systemic illnesses such as demyelinating disease, multiple sclerosis, or systemic lupus. Nonintentional harm is a significant factor to take into account in instances of traumatic myelopathy, which can frequently be linked to additional injuries. MRI and CT radiography help identify compressive myelopathy. We present the case of a 12-year-old girl who is right-hand dominant. She was in good health six months ago but recently began experiencing weakness in both of her lower limbs. An MRI of the brain revealed basilar invagination with stenosis of the foramen magnum, causing compressive myelopathy at the cranio-vertebral junction. Acute ischemic stroke (AIS) affecting the posterior cerebral artery (PCA) represents a unique clinical challenge, necessitating a multifaceted approach to rehabilitation. This review aims to provide a comprehensive overview of physiotherapeutic interventions tailored specifically for individuals with AIS involving the PCA territory. The PCA supplies critical areas of the brain responsible for visual processing, memory, and sensory integration. Consequently, patients with PCA infarcts often exhibit a distinct set of neurological deficits, including visual field disturbances, cognitive impairments, and sensory abnormalities. This case report highlights evidence-based physiotherapy strategies that encompass a spectrum of interventions, ranging from early mobilization and motor training to sensory reintegration and cognitive rehabilitation. This case report documents the comprehensive management of a 21-year-old female resident of Gadchiroli presenting with a 10-day history of fever, altered consciousness, and neurological sequelae following a traumatic incident. The patient exhibited a Glasgow Coma Scale score of 6/15, hypotonia in both upper and lower limbs, diminished deep tendon reflexes, and respiratory complications. This case study describes a thorough physiotherapeutic strategy that focuses on tone facilitation and muscle weakness improvement. The intervention used Rood's facilitative approaches as well as neuromuscular electrical stimulation (NMES). Rood's treatments, which emphasized mobilizing touch and tactile stimulation, brushing, quick icing, quick stretching, tapping, massaging the skin, heavy joint compression, and rolling, were used deliberately to move the patient from flaccidity to better muscle tone. These techniques' repetitive and task-specific nature coincided with motor learning principles, enabling adaptive modifications in brain networks. The ganglio-capsular region consists of the basal ganglia nuclei (caudate nucleus and lentiform nucleus), thalamus, and internal capsule. A disorder of the ganglio-capsular region typically presents with movement disturbance and cognitive impairment. This report presents the case of a 52-year-old male who was diagnosed with acute non-hemorrhagic infarct in the right parietal-occipital-temporal region predominantly involving the cortex and in the right ganglio-capsular region. The patient exhibited typical symptoms, which include impaired reflexes, decreased strength, reduced range of motion, and tone abnormalities. Targeted early physiotherapy intervention (TERI) was initiated from the bedside in the intensive care unit (ICU). Modified constraint-induced movement therapy (mCIMT) along with conventional therapy was selected as the rehabilitation approach for the case as it deals with "forced use" of the affected extremities, which addresses "learned non-use." The internal capsule (IC) is a vital brain structure housing descending and ascending fiber tracts, with traditional assumptions about the corticobulbar and corticospinal tracts descending through the genu and anterior third of the posterior limb of internal capsule (PLIC), respectively. However, observations of IC infarctions reveal that symptoms often deviate from the expected fiber pattern, prompting a deeper exploration of these complexities. The posterior limb of the IC receives its blood supply from the lenticulostriate branches of the middle cerebral artery and the anterior choroidal artery (AChA). AChA infarctions present a diverse array of symptoms beyond the classic triad, reflecting the intricate vascular supply and lesion patterns within this region. We present a case of a 74-year-old male farmer with right-hand dominance, who experienced a fall resulting in head and right lower limb injuries. Subsequently, he developed weakness in his left upper and lower limbs, facial deviation, slurred speech, and swelling in the right lower limb. Following these symptoms, his family promptly brought him to the hospital on November 30, 2023. Extensive investigations, including magnetic resonance imaging (MRI), revealed a hyper-acute infarct in the posterior limb of the left IC. The patient was admitted to the intensive care unit (ICU) for three days and later shifted to the neurology ward where medical management was commenced, including physiotherapy protocol that was started on December 2, 2023. Physiotherapy interventions were designed to address the patient's weakness, altered sensation, and diminished reflexes. Therapeutic goals focused on preventing complications, improving posture, enhancing range of motion (ROM), and mitigating breathing difficulties and mobility issues. The physiotherapy aimed to enhance the patient's overall physical and mental well-being, emphasizing independence and improved quality of life. Regular assessments and adjustments to the therapeutic interventions were made based on the patient's progress. Coxsackievirus-A6 (CV-A6) is responsible for more severe dermatological manifestations compared to other enteroviruses such as CV-A10, CV-A16, and EV-A71, causing HFMD in children and adults. Between 2005 and 2007, the recombinant subclade D3/RF-A started to expand globally, and a CV-A6 pandemic started. The study aimed to conduct whole-genome sequencing (WGS) of an isolated CV-A6 strain from currently circulating HFMD cases from India in 2022. Gene-specific RT-PCR and sequencing were used to perform molecular characterization of the isolated virus. Confirmation of these isolates was also performed by transmission electron microscopy and WGS. Among eleven positive clinical enterovirus specimens, eight CV-A6 strains were successfully isolated in the RD cell line. Isolates confirmed the presence of the CV-A6 strain based on VP1 and VP2 gene-specific RT-PCR. Sequences of isolates were clustered and identified as the novel CV-A6 strain of the D3/Y sub-genotype in India. The studies revealed that the D3/Y sub-genotype is being introduced into Indian circulation. The predicted putative functional loops found in VP1 of CV-A6 showed that the nucleotide sequences of the amino acid were a remarkably conserved loop prediction compatible with neutralizing linear epitopes. Therefore, this strain represents a potential candidate for vaccine development and antiviral studies.
