Plant Growth-Promoting Rhizobacteria (PGPR) and compost present promising solutions for bolstering agricultural sustainability by stimulating plant growth and yield while lessening dependence on chemical inputs. Our investigation delved into the combined effects of PGPR and compost on spinach cultivation, conducted at Amity University’s Organic Farm. Employing a Randomized Block Design (RBD) with six treatments and three replications, we evaluated how these treatments influenced spinach growth parameters and photosynthetic pigment levels. Results showed that treatment T5 (AIOA Strain + Compost) outperformed others, significantly boosting plant height, leaf breadth, root length, and leaf count per plant. Furthermore, T5 exhibited elevated chlorophyll and carotenoid levels, indicating enhanced photosynthetic function and overall plant vigor. These outcomes underscore the potential synergy between PGPR and compost in fostering sustainable crop production. By diminishing reliance on chemical fertilizers and pesticides while enhancing soil health and nutrient cycling, integrating PGPR and compost-based biofertilizers offers an economical and environmentally friendly avenue for augmenting agricultural productivity and mitigating environmental hazards. Further exploration into optimization techniques and underlying mechanisms is crucial to fully harness the potential of these microbial and organic interventions across varied agroecological settings. Advanced pregnancy is associated with a higher risk of complicated aortopathies owing to the physiologic changes in pregnancy. The diagnosis can be elusive due to its rare incidence. The optimal treatment strategy is chosen based on the clinical condition of the patient, gestational age, and the severity of the aortic disease. A healthy young primigravida presented with acute chest pain in the early second trimester, diagnosed as a thoracic aortic aneurysm that had ruptured causing hemothorax. She underwent emergency endovascular repair under general anesthesia. Aortic disease should always be ruled out early in acute chest pain in pregnancy. Aeridas rosea and Eria lasiopetala are the species of rare and endangered orchids of uttrakhand region. These species deal with the in vitro propagation methods. For green pod culture, three Nutrient media Murashige and skoog (MS Media), Knudson Media (KC Media) and stock Solution of MS Media were tested by adding different concentration of growth regulators. MS Media supplemented with growth regulators 1.5mg/L NAA and 1.5mg/L BAP were found to be the most effective media for the development of protocorn like body(PLBs). This study is helpful in the propagation of these rare Orchids Plants at lower elevation, which could be a successful effort of conservation of rare Orchid plants and this could also reduce pressure on its population in the Wild. Purpose: This research paper aims to explore the challenges associated with employee retention in the telemedicine sector and identifying various strategies to enhance retention rates taking into consideration job satisfaction as a mediator. Design/Methodology: The primary data for this study was collected through questionnaires distributed to healthcare professionals, with 185 fully completed responses out of a total of 205 participants. Employing a combination of purposive and stratified random sampling techniques, this empirical study utilized PLS-SEM for the path analysis. The study revealed that Monetary benefits in comparison to non-monetary benefits play a major role in contributing job satisfaction which in turn leads to employee retention. Findings: The findings reveal that the monetary benefits have greater influence on job satisfaction which leads to better retention rates in the healthcare industry, based on the analysis of survey data from doctors, nurses, and other healthcare staff. These insights can inform retention strategies in healthcare organizations to improve employee satisfaction and reduce turnover rates. Pulmonary Alveolar Proteinosis (PAP) is a rare disease of the lungs characterized by deposition of lipo-proteinaceous material in the alveoli. This leads to reduced lung compliance and V/Q mismatch, causing respiratory failure. Patients with PAP may be posted for Whole Lung Lavage (WLL) under anesthesia, which is a challenging case for anesthetists. Preexisting respiratory failure is complicated by requirement of single lung ventilation with the help of double lumen tube during the procedure. Intraoperative ventilation is difficult and is managed with blood gas and airway pressure monitoring. Some degree of hypoxemia and hypercapnia may have to be tolerated. Large volumes of fluid are instilled into the lung which may cause hypothermia, hypervolemia, and electrolyte disturbances. These patients frequently require elective ventilation and intensive care admission in the postoperative period. We report the case of a young woman with PAP, posted for unilateral WLL under anesthesia. Objective: Financial toxicity has been associated with several clinical outcomes like early mortality and poor quality of life. The aim of this study was to evaluate the magnitude of FT among radiation oncology patients and its association with health related quality of life (HRQOL) in Indian health care setting.Methodology: This cross-sectional study was conducted among cancer patients who have completed radiation therapy either as stand-alone or part of a multimodal treatment. The financial toxicity and HRQOL were assessed using Comprehensive Score for Financial Toxicity (COST) and Functional Assessment of Cancer Therapy: General (FACT-G) measures respectively. Association between financial toxicity and HRQOL was assessed using Pearson correlation. Univariate and multivariate regression analyses were conducted to identify the factors associated with financial toxicity.Results: A total of 350 patients were included in this study. Of the 350 participants, 57.7% were male, 95.7% had no health insurance and 61% were diagnosed with HNC cancers. The average COST score was 15.38 ± 9.18 (range 2-35) and the average FACT-G score was 69.63 ± 12.25 (range 33-99). Based on total COST score, 7.4% participants reported grade-3 and 44.9% reported grade-2 financial toxicity. A significant positive correlation was observed between COST Score and FACT-G Score, with a correlation coefficient of 0.58, (p < .001) indicating a large effect size. COST Score also significantly predicted the FACT-G Score, β = 0.77, 95%CI (0.66 - 0.88) p < .001. The results of multivariate linear regression identified annual household income (β: 3.9, 95%CI: 3.29 to 4.57, p < .001) and cancer type (β: 3.74, 95%CI: 2.33 to 5.14, p < .001) as significant predictors of the COST score.Conclusion: More than 80% of the participants experienced financial toxicity in this study. Background and Objectives: Alloimmunization can lead to difficulty in arranging compatible, antigen-negative blood units for the patients. Alloimmunization by coexisting “c” and “E” antibodies, though common, is frequently missed. Both “c” and “E” antigens are highly immunogenic and have the potential to cause hemolytic disease of newborn and hemolytic transfusion reactions. The objective of this study is to discuss different clinical scenarios of concomitant and singular presence of anti-c and anti-E along with the diagnostic approach and transfusion management in resource-limited settings. Methods: Column agglutination gel technology in low ionic strength solution phase was used for initial antibody identification. Detailed immunohematological workup was done by the use of select cells (c+, E− and c−, E+) and adsorption elution studies using a commercially available acid elution kit. Results: Out of 16 patients, detailed immunohematological workup was available for 14 patients, whereas two patients were lost to follow-up. Among 14 patients, 12 had CCDee (R1R1) phenotype, whereas two patients had CcDee phenotype (possible R1r) with anti-E antibody. In 12 patients with R1R1 phenotype, 6/12 (50%) had dual coexisting anti-c and E, whereas 3/12 (25%) had only anti-c and 3/12 (25%) had only anti-E. In R1R1 patients having anti-E, coexisting anti-c was found in 6/9 (66.66%) of patients. Conclusion: The study emphasizes the use of both “c” and “E” negative red cells (R1R1) in R1R1 patients having either anti-c or anti-E. Thus, in India, there is a need to develop our own red cell panels having an adequate representation of indigenous antigens and phenotypes. [ABSTRACT FROM AUTHOR]

Moksha, or spiritual liberation, is the ultimate goal of mankind, based on the Hindu Value System. For this objective to be attained the ancient Indian civilization was split into four fundamental stages of life known as ashramas. The term 'ashrama 'is originated from the roots i.e. 'shram 'which denotes efforts. The derivative term 'ashrama' refers to a condition in which an individual undertakes ones' own self efforts. Brahmacharya, Grihastha, Vanprastha, and the Sanayasa ashrama, every ashrama symbolized a certain life phase in addition to the roles and responsibilities that one was expected to accomplish. On the other hand, if one adhered to and followed the laws and regulations provided in the ashrama system, one would inevitably lead a prosperous and fulfilling life throughout all stages of life. This research isfocused on thefinancial component and its significance to the well-established vedic idea of the ashrama system, among other dimensions such as academic, societal, and emotive. This methodology relies on the secondary analysis, and no prior research in this field has indeed been undertaken. These guidelines werefounded on a solid basis and, ifnotfollowed, would result in a plethora of social vices. Gallbladder cancer (GBC) is an aggressive and difficult to treat biliary tract carcinoma with a poor survival rate. The aim of this study was to design a peptide-based multi-epitope vaccine construct against GBC using immunoinformatics approaches. Three proteins implicated in the progression of GBC were selected for B and T cell epitope prediction and the designing of the potential vaccine construct. Seven CTL, four HTL and six Bcell epitopes along with a suitable adjuvant were selected and connected using linkers for designing the vaccine construct. The secondary and tertiary models of the designed vaccine were generated and satisfactorily validated. A Ramachandran plot of the final 3D model showed more than 90% of the residues in allowed regions and only 0.4% in disallowed regions. The binding affinity of a vaccine construct with TLR 2, 3 and 4 receptors was assessed through molecular docking and simulation. The average numbers of hydrogen bonds for vaccine-TLR 2, 3 and 4 complexes in the simulation were 15.36, 16.45, and 11.98, respectively, and remained consistent over a 100 ns simulation period, which is critical for their function. The results of this study provide a strong basis for further evaluation through in vitro/in vivo experimental validation of the safety and efficacy of the designed vaccine construct. Introduction: Colorectal cancer has been primarily considered a disease of the elderly, but recent data have shown an alarming rise among young people. It has been also suggested that young age is associated with aggressive histopathological characteristics and advanced stages of the disease at diagnosis. As there are few studies and none from our part of the country evaluating the clinicopathological profile of early-onset versus late-onset rectal cancer patients, this analysis was conducted to assess and compare the clinical and pathological characteristics of patients with rectal cancer diagnosed with ages over and below 50 years. Materials and method: The relevant details of all biopsy proven rectal cancer patients undergoing radiotherapy at a tertiary cancer hospital, from January 2017 to December 2019, were collected. All the data were categorised into two groups, an early-onset group (age <50 years) and a late-onset group (age ≥50 years), and comparison of the clinicopathological characteristics between the two groups was made. Results: A total of 224 patients with rectal cancer, 150 male and 74 female, were included in the study. About two-thirds of the patients were less than 50 years of age, with an average age of 42 years. The comparative analysis showed a significantly higher number of young patients presenting with bleeding and pain. Patients below 50 years also had a significantly higher number of adenocarcinoma grade III and clinical stage III than those in the late-onset group. Conclusion: Our study revealed a significant number of early-onset rectal cancer patients. There should be a high index of suspicion in any young patient presenting with symptoms suggestive of rectal malignancy and they should be evaluated promptly. [ABSTRACT FROM AUTHOR]

Introduction: Mucormycosis, an aggressive fungal infection may result in a difficult airway owing to its inflammation. King Vision Video Laryngoscope (KVVL) is a useful addition to a difficult airway armamentarium. McCoy laryngoscope with hinged tip is well-known equipment of difficult airway cart. Conventional Macintosh laryngoscope dominates in anaesthesia practice. Standard, existing and contemporary devices were compared in difficult airways resulting from mucormycosis. Aim: To compare the relative performance of KVVL, McCoy, and Macintosh laryngoscopes based on the ease of intubation and haemodynamic response in patients with mucormycosis. Materials and Methods: The present study was a randomised clinical trial done during June-August 2021 on 90 consenting patients of 18-65 years age, belonging to either sex with microbiologically confirmed mucormycosis undergoing debridement surgery. Group A was intubated with a non channeled KVVL, while group B had McCoy and group C had Macintosh laryngoscope. Primary outcome parameters were Cormack Lehane (CL) grade, time from laryngoscopy to successful intubation, number of attempts, any adjuncts or optimisation maneuvers, and any mucosal injury. Secondary outcome parameters were Heart Rate (HR), Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), Mean Blood Pressure (MBP) measured on arrival, before induction, after induction, and at 1, 2, 3, 5, and 7 minutes after intubation. Comparison of quantitative variables not normally distributed were analysed using Kruskal Wallis test. Post-hoc analysis by Dunn's multiple pairwise comparison test. Friedman test followed by pairwise comparison was done to compare haemodynamic parameters within each group. Chi-square test was used for qualitative variables. The p-value =0.05 was considered to be statistically significant. Results: CL grade was lowest in group B (1.83±0.38) against group A (1.93±0.25), group C (2.13±0.35) with p-value of 0.029. Time from laryngoscopy to successful intubation was the least in group B (19.5±3.98) against group A (26.07±9.8), group C (21.33±3.74) with p-value of 0.002. No significant difference was there in the number of attempts, airway adjuncts/optimisation maneuvers, mucosal injury. Haemodynamic variables were comparable. Conclusion: McCoy laryngoscope was found to perform best in difficult airways resulting from mucormycosis. It was most effective for glottic visualisation, with the shortest time to successful intubation and, haemodynamic parameters were comparable to KVVL and Macintosh laryngoscopes. Background: Financial toxicity is a consequence of subjective financial distress experienced by cancer patients as a result of treatment expenditures. Financial toxicity has been associated with poor quality of life, early mortality, and non-adherence. It is evident from the literature that the currently available instruments for the assessment of financial toxicity do not measure coping and support seeking domains. The aim of this study was to develop an instrument for the assessment of financial toxicity among radiation oncology patients that captures and integrates all the relevant domains of subjective financial distress. Materials and Methods: The study was conducted among Head & Neck cancer (HNC) patients (age ≥18 years) who have completed the radiotherapy either as stand-alone or part of a multimodal treatment. Literature review, expert opinion, and patient interviews were used for scale item generation. The validity and underlying factor structure were evaluated by Exploratory Factor Analysis (EFA) and Confirmatory Factor Analysis (CFA). The reliability and internal consistency of the final scale was assessed using Cronbach's alpha coefficient. Results: A total of 17 items were identified for scale development. The preliminary 17-item instrument was administered to 142 HNC patients. Among 142 participants, 85.9% were male and 98.6% were from rural areas. EFA was performed on 17 items and three items were removed (factor loadings <0.5). The remaining 14 items loaded onto three factors (eigenvalue >1) explaining 62.0% of the total variance. The Chi-square goodness of fit test in CFA and the values of other model fit indices, namely, RMSEA = 0.045, SRMR = 0.014, GFI = 0.92, CFI = 0.98, and TLI=0.97 indicate a good model fit suggesting the three-factor model adequately fits the data. The Cronbach's α for the final 14-item scale was 0.87 indicating excellent reliability and the Cronbach's α coefficient of all the individual 14 items was ≥0.85 (range 0.85–0.88). Conclusion: The SFDQ showed excellent validity and reliability. SFDQ captures and integrates all the relevant domains of financial toxicity. However, the provisional SFDQ instrument warrants further larger sample studies for validation and psychometric evaluation in different primary cancer subsites and treatment modalities from multiple cancer centers to improve the generalizability of this instrument. Background and Purpose: Presurgical prediction of H3K27M mutation in diffuse midline gliomas (DMGs) on MRI is desirable. The purpose of this study is to elaborate conventional MRI (cMRI) features of H3K27M‐mutant DMGs and identify features that could discriminate them from wild‐type (WT) DMGs. Methods: CMRI features of 123 patients with DMG were evaluated conforming to the institutional research protocols. Multimodality MRI was performed on 1.5 or 3.0 Tesla MR Scanners with imaging protocol, including T1‐weighted (w), T2w, fluid‐attenuated inversion recovery, diffusion‐weighted, susceptibility‐weighted, and postcontrast T1w sequences. Pertinent cMRI features were annotated along the lines of Visually AcceSAble Rembrandt Images features, and Intra Tumoral Susceptibility Signal score (ITSS) was evaluated. R software was used for statistical analysis. Results: Sixty‐one DMGs were H3K27M‐mutant (mutant DMGs). The patients in the H3K27M‐mutant DMG group were younger compared to the WT‐DMG group (mean age 24.13 ± 13.13 years vs. 35.79±18.74 years) (p = 0.016). The two groups differed on five cMRI features––(1) enhancement quality (p = 0.032), (2) thickness of enhancing margin (p = 0.05), (3) proportion of edema (p = 0.002), (4) definition of noncontrast‐enhancing tumor (NCET) margin (p = 0.001), and (5) cortical invasion (p = 0.037). The mutant DMGs showed greater enhancement and greater thickness of enhancing margin, while the WT DMGs exhibited significantly larger edema proportion with poorly defined NCET margins and cortical invasion. ITSS was not significantly different among the groups. Conclusion: CMRI features like enhancement quality, the thickness of the enhancing margin, proportion of edema, definition of NCET margin, and cortical invasion can discriminate between the H3K27M‐mutant and WT DMGs.
