First, this paper examines PPP's popularity. Second, we describe key characteristics of IAVI and explain why it differs from more traditional partnerships. Third, IAVI has had some success in bringing new science and technology closer to the world's poor and we look at how it has achieved this success and implications for theoretical and practical approaches to science and technology capacity building. Peer review of manuscripts is labour‐intensive and time‐consuming. Individual reviewers might feel themselves overburdened with the amount of reviewing they are requested to do. Aiming to explore how stakeholder groups perceive reviewing burden and what they believe to be the causes of a potential overburdening of reviewers, we conducted focus groups with early‐, mid‐, and senior career scholars, editors, and publishers. By means of a thematic analysis, we aimed to identify the causes of overburdening of reviewers. First, we show that, across disciplines and roles, stakeholders believed that the reviewing burden is distributed unequally across members of the academic community, resulting in the overburdening of small groups of reviewers. Second, stakeholders believed this to be caused by (i) an increase in manuscript submissions; (ii) inefficient manuscript handling; (iii) lack of institutionalization of peer review; (iv) lack of reviewing instructions and (v) inadequate reviewer recruiting strategies. These themes were assumed to relate to an inadequate incentive structure in academia that favours publications over peer review. In order to alleviate reviewing burden, a holistic approach is required that addresses both the increased demand for and the insufficient supply of reviewing resources. [ABSTRACT FROM AUTHOR]

Stakeholders might have diverging or conflicting expectations about the functions that peer review should fulfil. We aimed to explore how stakeholder groups perceive peer review and what they expect from it. We conducted qualitative focus group workshops with early‐, mid‐, and senior career scholars, editors, and publishers. We recruited participants following a purposive maximum variation sampling approach. To identify purposes of peer review, we conducted a thematic analysis. Stakeholders expected peer review (1) to assess the contributions of a manuscript, (2) to conduct quality control, (3) to improve manuscripts, (4) to assess the suitability of manuscripts for a journal, (5) to provide a decision‐making tool for editors, (6) to provide feedback by peers, (7) to curate a community, and (8) to provide a seal of accreditation for published articles. Stakeholders with different roles and tasks in the peer review process differed in the value they attached to the functions of peer review. Early‐ and mid‐career researchers valued social and feedback functions of peer review, while senior career researchers and editors expected it to instead perform a technical assessment of manuscripts and serve as a decision‐making tool. Publishers expected peer review to assess the suitability of manuscripts for their journals and to provide a seal of accreditation. This revealed a potential tension between functions of peer review. In a sector characterised by patenting, direct appropriations and returns from investment, the Structural Genomics Consortium (SGC) constitutes a radically different public-private and entirely open access approach to pre-competitive research. This paper discusses the significance of findings from the first independent review of the SGC. We argue that the SGC offers a shared knowledge resource for drug discovery which is distinctive from other types of knowledge production and, as such, provides a knowledge infrastructure for the wider scientific community. We distinguish three ways in which this infrastructure functions as a model for investing in, extracting value from, and generating knowledge for the field. Our analysis suggests there is a future for open science models such as the SGC in health research and innovation, but that such models raise a set of challenges over the role of different public and private institutional actors and the way in which value is extracted. Healthcare systems with limited resources face rising demand pressures. Healthcare decision-makers increasingly recognise the potential of innovation to help respond to this challenge and to support high-quality care. However, comprehensive and actionable evidence on how to realise this potential is lacking. We adopt sociotechnical systems and innovation systems theoretical perspectives to examine conditions that can support and sustain innovating healthcare systems. We use primary data focussing on England (with 670 contributions over time) and triangulate findings against globally-relevant literature. We discuss the complexity of factors influencing an innovating healthcare system's ability to support the development and uptake of innovations and share practical learning about changes in policy, culture, and behaviour that could support system improvement. Three themes are examined in detail: skills, capabilities, and leadership; motivations and accountabilities; and collaboration and coordination. Byline: Joanna Chataway, David Wield This paper examines R&D and strategic decision-making in firms, reviewing institutional and market constraints that act on the development of bio- and new materials technologies. The findings are assessed against the background of biotechnology and new materials as potential 'core technologies' that, combined with new socioeconomic norms, will make up a significantly different technoeconomic paradigm for the next century. The studies presented portray levels of complexity that highlight the variety of technological evolution. They also illustrate the need for improved methods of analysis that better integrate macro- and micro-levels of evolutionary theory.

Byline: Joanna Chataway, James Smith, David Wield Science and technology - and particularly biotechnology - is increasingly central to development agendas in Africa and elsewhere. Implicit within the centrality of science and technology lie a set of policy issues regarding how best to shape contextually appropriate, innovative and sustainable science and technological products in, with and for developing countries. The work of the Consultative Group on International Agricultural Research (CGIAR) is a case in point and we draw our empirical material from the evolution of two biotech bovine vaccine development programmes housed in a CG Centre. In the paper, we seek to show that broadening our understanding of scientific 'excellence' can lead to more innovative, systemic research that may produce more appropriate technological solutions. We believe this has key implications for science policy, development policy and the practice of science for development itself.

Questions linking social needs with new technologies have continued to be raised but only very recently have they begun to occupy more of the centre-stage in innovation policy debates. In this narrative review, we draw from innovation and health systems literature and thinking to trace and analyse the deployment and uptake of innovative point-of-care testing in the early infant diagnosis of HIV in Zimbabwe, one of 15 UNITAID supported early adopter countries. Following a literature review and discussions with policy-makers in Zimbabwe, we distil key lessons from this implementation context and delineate the implications of our findings for the debates on new technological and socio-economic approaches and the health systems strengthening agenda in low-middle income countries (LMICs). This article documents recent trends in science funding support in Sub-Saharan Africa (SSA). We analyse these trends at the SSA regional level alongside a summary of four case studies of science funding in four Science Granting Councils (SGCs) in East Africa. Our findings support the literature on science funding in SSA regarding low levels of funding, cross-country engagement, and the need for capacity building. However, we also find there are tensions among funding and policy actors around the perceived ways in which investment in science will benefit society. We argue that the narratives and logics of science funders and their roots in 'Republic of Science' vs. 'Embedded Autonomy' rationales for SGC activity must be more transparent to enable critical engagement with the ideas being used to justify spending. There is increasing policy demand for real-time evaluations of research and capacity-building programmes reflecting a recognition of the management, governance and impact gains that can result. However, the evidence base on how to successfully implement real-time evaluations of complex interventions in international development efforts is scarce. There is therefore a need for reflective work that considers methodologies in context. This article shares learning from the experience of conducting a participatory, real-time, ‘theory driven’ evaluation of the African Institutions Initiative, a Wellcome Trust-funded programme that aimed to build sustainable health research capacity in Africa at institutional and network levels, across seven research consortia. We reflect on the key challenges experienced and ways of managing them, highlight opportunities and critical success factors associated with this evaluation approach, compared with alternative evaluation approaches. Networked models are often proposed as a means to enhance health research capacity-building in Africa. This paper addresses a knowledge gap on what works and does not in capacity-building in African research settings. It provides an analysis of how multi-partner networks are built and how their success depends on building institutional level capacity-strengthening within partner institutions. To do this, the paper focuses on the Wellcome Trust's African Institutions initiative, drawing on initial learning and evaluation project data. We identify priority areas for policy attention and share emerging early insights on mechanisms and strategies being implemented by consortia to address key challenges. Open innovation has gained increased attention as a potential paradigm for improving innovation performance. This paper addresses crowdsourcing, an under-researched type of open innovation that is often enabled by the web. We focus on a type of crowdsourcing where financial rewards exist, where a crowd is tasked with solving problems which solution seekers anticipate to be empirically provable, but where the source of solutions is uncertain and addressing the challenge in-house perceived to be too high-risk. There is a growing recourse to crowdsourcing, but we really know little about its effectiveness, best practices, challenges and implications. We consider the shift to more open innovation trajectories over time, define crowdsourcing as an open innovation model, and clarify how crowdsourcing differs from other types of 'open' innovation (e.g. outsourcing and open-source). We explore who is crowdsourcing and how, looking at the potential diversity and core features and variables implicated in crowdsourcing models. [ABSTRACT FROM AUTHOR]

Agricultural biotechnology is typically analyzed critically by means of a political ecological focus on the science and its ecological implications – agbio science as a radical, and ‘non-natural’, break with ‘normal’ trajectories for ‘new plant science’. Surprisingly, less attention has been paid to a range of key political economic issues, many of which were important in the last big food production technology ‘revolution’, the Green Revolution. This paper will focus on three areas of political economy. First, we discuss the corporate drivers of agricultural biotechnology, and examine whether these drivers have already set the technology so that it cannot be changed. Second, we investigate the present economics and technology of genetic modification in plants, and its possible future. Third, we examine empirical evidence for alternative visions of the technology. 'Brain drain' is viewed as a curse for developing countries but analysis suggests it may provide crucial advantage to some countries. 'Reverse brain drain' of engineers and scientists trained in the USA or Europe can accelerate technological catch-up. Communities of returned scientists and engineers can provide skill and know-how to help local firms shift to higher value-added activities. However, transfer of knowledge through human mobility is not a straightforward process. This article is an attempt to think about participation not as an event but rather as part of an effort to build systems and institutions in science, technology and innovation in developing countries. It uses two notions of participation, one as a mechanism to facilitate democracy and broader control and another as a means to achieve development goals. The article looks at the International AIDS Vaccine Initiative (IAVI) and in particular considers the way in which more grassroots participation is linked to communications strategy in the IAVI's case. It argues that examining the extent to which participation efforts are embedded in broader communication strategies may be important in distinguishing between ways in which participation is deployed and used, and it distinguishes between 'communication for participation' and 'participation for communication'. The article also explores Hirschman's ideas around 'exit', 'voice' and 'loyalty' and innovation systems as frameworks, which help to situate participation in the broader context of institutional development. Some industrially advanced developing countries have, in the last decades, built substantial pharmaceutical scientific and industrial capabilities. The more advanced of these countries have built chemical and biological R&D, generic and reverse engineering, and drug discovery capabilities. There have been tremendous barriers to entry but nevertheless a range of imitative approaches have evolved, with some significant scientific, health and industrial advances. Opportunities in India are in part created by the problems of maturity being faced by big pharmaceutical firms in developed countries. Big pharmaceutical firms are searching for novel approaches to retain a high value-added model of innovation for drug development. These difficulties provide opportunities for innovative companies in developing countries. This paper explores the challenges faced under these circumstances by Indian pharmaceutical companies. Are they attempting to 'catch-up' by chasing a failing model of innovation? Do they have an alternative model? The authors reflect on the development of pharmaceutical industry before and after the implementation of the Agreement on Trade-Related Aspects of Intellectual Property Rights (TRIPS) in India. They cite that between 1950s to 1960s the industry has a weak growth due to multinational company subsidiaries while in 1970s it has massive changes because of the government policy. The TRIPS legislation has driven transformations in the industry that lead to global competitiveness.

This paper addresses the question of how Indian pharmaceutical firms have transformed themselves from reverse-engineering firms focused on the domestic market to research-driven firms with global presence. It analyses the crucial and changing role of national, then international, innovation and regulatory policies and their impact on growth, performance and technology evolution of Indian pharmaceutical firms. The paper argues that although public policy increased market turbulence, it also provided new opportunities for firms to expand and build new technology, knowledge and market capabilities. From the 1950s Indian firms have pursued a variety of research, marketing and internationalization strategies to tackle changes prompted by policy, markets and knowledge complexities. The paper traces firm strategies that vary both in time and by firm, and argues that the successful firms have evolved substantively towards knowledge-based strategies. Why do corporations develop technologies which can be associated with the generation of various environmental risks and how are the technologies that they develop governed by factors within and around firms? The authors examine the factors that have motivated and guided technological innovation, based on an examination of multinational companies developing genetically modified (GM) crops for the European market. The analysis is based on an inherently interdisciplinary approach to the study of innovation, which incorporates factors that are governed endogenously through the processes within companies (that is, strategic decisionmaking) and exogenously through interactions between firms and their external constituents (that is, government policies and regulations, and stakeholder and public perspectives and engagements). It is found that the introduction of GM technologies in Europe has been shaped significantly by public perception and societal reactions. It is also found that the aspects of industry strategies which contributed most to the course of European public opposition to GM crops were: (a) the choice of first-generation GM products; (b) interactions between pesticide-product and biotechnology-product strategies in different companies, and industry's efforts to present their sector and its products as contributing to sustainable development; (c) cultural and world-view differences between companies; and (d) company responses to European biotechnology policies and risk regulation. It is demonstrated that actions which seem rational to individual actors (corporations, governments, public interest groups) can have counterintuitive, and often counterproductive, outcomes in the longer term and when considered from the perspective of interactions within broader governance processes. The question of how to build science and technology (S&T) capacity in Africa has been on and off the agenda for decades, as has the issue of how to use partnerships to accelerate capacity building. Recent policy proposals have focused on expanding S&T capacity but have emphasized less the need for a rigorous rethink about how science, technology and innovation can be better organized for development. This paper aims to assess, using evidence from recent cases, the importance of new theories and practices based on the role of innovation and knowledge systems. The paper argues that lessons can be learned from transformations in research policy and from practices that better integrate new ideas from innovation, knowledge and development. Multinational companies in the life science sector are heavily influenced by government policies and regulations and in turn attempt to influence these actors nationally and internationally. This paper focuses on recent and on-going research, principally on the agro-biotechnology and, to a lesser extent, on the pharmaceutical industries, covering the evolution of policy and regulation in Europe, how policies are influenced by stakeholder pressures and how policy in turn influences company strategies for product development. We focus particularly on new ‘governance’ agendas in Europe and consider the relative impacts of enabling, constraining, discriminating and indiscriminate policies on company strategies as part of our development of an integrated approach to policy and governance. We also consider changes in external operating environments for multinational companies and compare past histories and present pressures on agro-biotechnology and pharmaceutical companies. The paper argues, with evidence, that a more enabling and discriminating policy and regulatory environment can achieve public goals more efficiently and effectively, taking account of impacts on innovation, than more blunt policy instruments. This type of environment takes into account the resources and capabilities available to firms and research laboratories rather than relying on broad brush carrot and stick approaches. Biotechnology represents hope for those who believe that new technology can contribute to overcoming a range of pressing productivity and environmental constraints facing poor farmers. Pouring money into the science and technology however without recognizing institutional and systemic complexities associated with creating ‘pro-poor’ technologies is unlikely to deliver much for those farmers. Moreover, there are a range of economic and political contextual factors which further complicate efforts to put the technology to use for poorer farmers. This paper reviews the relationship between knowledge management and development. First it highlights the ways knowledge has become a major issue and why it matters, arguing the danger of oversimplifying its application as the new panacea for development. The paper then emphasizes the need to understand better institutional and industrial change processes through detailed investigation of production transformations ‘on the ground’. The paper suggest that knowledge-based industrialization demands a change in the ways we think about development and in policies to promote industrialization, focusing on issues of knowledge transfer, local knowledge and institutions, and knowledge management. The article comments on the progress of the United Nations' Millennium Development Goals in eradicating poverty and hunger through crop biotechnology partnership in sub-Saharan Africa. This is one of the eight goals set by the organization with the specific target of alleviating poverty by 50 percent between 1990 and 2015. Reports and analyses conducted showed that food production is way behind its population growth due to poor governance, poverty, plant diseases and calamities.

Recent academic and policy debate on innovation indicates that there has been some shift from a more traditional systems approach to ecologies and ecosystems. The latter are concepts transferred from the world of biology to the social world in order to explain the evolutionary nature of interrelations between different individuals, their innovative activities, and their environment. We evaluate the concept of knowledge ecology and the theory of innovation ecosystem on two fundamental grounds; firstly, on the grounds of theoretical plausibility and conceptual consistency; secondly, on empirical grounds of the case of public-private interrelations of biotech innovation in Cambridge. The argument is that the concept of knowledge ecology and the theory of innovation ecosystems can lead to problems of reductionism and functionalism. This is due to their development in abstraction from more grounded analysis of historical processes of the social division of labour. The demand for health services in England is both growing and changing in nature, yet resources are limited in their ability to respond to the scale and scope of need. As a result, the NHS is under increasing pressures to realise productivity gains, while continuing to deliver high quality care. RAND Europe and the University of Manchester have been commissioned to conduct a study to examine the potential of innovation to respond to the challenges the NHS faces, and to help deliver value for money, efficient and effective services. "Innovation" in this study refers to any product, technology or service that is new to the NHS, or applied in a new way, aimed at delivering affordable and improved care. The learning we have gained adds considerable depth to the practical discussions presented regarding how innovation can be first nurtured and then made meaningful and actionable in a variety of settings. This is important given the complexity of health innovation systems and the diversity of elements that need to interact and work together for the overall system to function effectively. We share insights related to skills, capabilities and leadership; motivations and accountabilities; information and evidence; relationships and networks; patient and public engagement; and funding and commissioning. We will develop these detailed learning points into a more systematic analysis as the research evolves. The research is funded by the Department of Health Policy Research Programme, in close collaboration with NHS England and the Office of Life Sciences.

This study aimed to inform Hepatitis C (HCV) treatment by (1) better understanding the nexus of factors physicians consider when making HCV treatment decisions; (2) investigating the comparative influence and importance of specific factors and the trade-offs implicated in the decisionmaking process; and (3) examining how much thrombocytopenia impacts treatment decisions and how it impacts treatment. To meet this goal, we conducted five analyses, focusing on four European countries characterised by different approaches to healthcare organisation and financing, which alongside cultural differences may have potential implications for treatment pathways for patients with HCV infection. These were: France, Italy, Spain, and the United Kingdom. These analysis included: Review the academic literature and of relevant national and European guidelines;Conduct key informant interviews (KIIs) with national experts to contextualise the data from the literature review and further explore some emerging themes;Map the patient journey in the four countries to identify stages HCV patients pass through once they have entered the healthcare system and map, for each stage, potential points of departure from the typical journey;Design and conduct of Discrete Choice Experiments (DCEs) to quantitatively assess the importance of factors that influence treatment decisions;Conduct expert workshop to help build scenarios identifying challenges to HCV treatment.The five analyses build on one another, with the first three providing evidence that fed into the design of the DCEs and with the DCE results in turn serving as the key inputs into building the scenarios for the expert workshop.

The Department of Health's Innovation, Health and Wealth (IHW) strategy aimed to introduce a more strategic approach to the spread of innovation across the NHS. This study represents the first phase of a three-year evaluation and aims to map progress towards the IHW strategy and its component actions. This evaluation used a combination of quantitative and qualitative methods: document review, key informant interviews and stakeholder survey. This study also forms the basis for selecting case studies for phase two of the evaluation. Our findings from the interviews and survey suggest broad stakeholder support for the overarching ambitions of the IHW strategy. However, we found variable progress towards the overarching objectives of the eight IHW themes and an ambiguous relationship between many of the themes' objectives and their actions. It was difficult to assess progress on IHW's actions as commitment to the actions, implementation guidance and expected outcomes of the actions were not clearly articulated. The Academic Health Science Networks (AHSNs) and the Small Business Research Initiative (SBRI) were reported to be working well, which may be attributed to their clear structures of accountability and earmarked budgets. However, survey respondents and interviewees raised concerns that budgetary pressures may limit the impact of both AHSNs and the SBRI. The main challenges identified for ongoing action were the resources available for their implementation (e.g. Medtech Briefings), lack of awareness of the initiative (e.g. the NICE Implementation Collaborative) and the design of the actions (e.g. the Innovation Scorecard, web portal and High Impact Innovations).

The Department of Health and the Wellcome Trust, in co-operation with NHS England, asked RAND Europe to conduct a limited consultation with key stakeholders about the practicality of measures and incentives proposed as part of the NHS Accelerated Access Review (AAR), which aims to assess the pathways for the development, assessment, and adoption of innovative medicines and medical technology. Through a focused engagement exercise with key healthcare stakeholders this project explored the implications of selected interim AAR propositions and feasibility of implementation for key actors, in primary and secondary care as well as commissioners and academia. Specifically, the project investigated the feasibility of implementation of three specific propositions including: a new earmarked fund to encourage AHSNs and other key innovation actors to re-design systems to embrace innovation; mobilising the influence of clinical system leaders to champion change; and encouraging secondary care organisations to take on "innovation champion" roles linked to financial incentives and a new emphasis on accountable care organisations. Data was collected on the feasibility of the three AAR propositions from a workshop with AHSN CEOs and Commercial Directors and interviews with senior NHS staff in three AHSN regions (South West, University College London Partners, and North East, North Cumbria). The study concludes with reflections on the feasibility of each recommendation and identifies factors expected to facilitate or challenge their implementation, as well as considering the wider cross cutting issues that may influence the adoption and diffusion of innovation in the NHS.

In early 2012, the National Institute for Health Research (NIHR) leadership programme was re-commissioned for a further three years following an evaluation by RAND Europe. During this new phase of the programme, we conducted a real-time evaluation, the aim of which was to allow for reflection on and adjustment of the programme on an on-going basis as events unfold. This approach also allowed for participants on the programme to contribute to and positively engage in the evaluation. The study aimed to understand the outputs and impacts from the programme, and to test the underlying assumptions behind the NIHR Leadership Programme as a science policy intervention. Evidence on outputs and impacts of the programme were collected around the motivations and expectations of participants, programme design and individual-, institutional- and system-level impacts.
