Abstract

Diagnostics-related errors made by medical students represent a considerable issue for the field, as they very often have negative consequences that reduce learning effectiveness. This study proposes a means of ameliorating problems with diagnostic reasoning and improving the learning process. The description of a virtual case, containing necessary information, is used within a knowledge-based system for evaluation of medical students' diagnostic abilities. This system is grounded in a branch-oriented model, where every decision reveals part of the information about the patient in each iteration, depending on the diagnostician's choice, which supports evaluation of the flow of students' thoughts. By fostering the acquisition of "negative knowledge" about typical cognitive errors in the medical reasoning process, the system helps eliminate learners' errors specific to any type of situation that the system might be set up to address in any given case. Descriptions of teachers' inputs, the process model, the evaluation procedure for student's performance, and a method for configuring virtual patient cases are provided. Apart from the accuracy of the diagnosis, other aspects such as patients' comfort and the cost of the diagnosis are also taken into consideration when evaluating students' diagnostic process.

Abstract

More powerful and challenging laser systems are built to meet the need of evolving technology, reflecting demands coming from both industry and research areas. Laser damage is a key parameter for all components in such high-power laser system, establishing limits of maximum achievable energy in laser beam, and consequently the power of the system. As a value for all optical components, this is represented by laser induced damage threshold (LIDT). LIDT is a value determining highest quantity of laser radiation incident upon the optical component for which the extrapolated probability of damage is zero. In following paper is described testing station build and used at HiLASE centre to determine such value, which is necessary to build reliable and stable laser sources and produce high quality optics. Abstract

Vincenz Alexander Bochdalek was a skilful and modest anatomist and pathologist who studied and worked at Charles-Ferdinand University in Prague from the 1820s to the early 1870s. He was born on 11 February 1801 in Skřipov (Austrian Silesia). In 1833 he defended his Thesis (A Guide to the Practical Dissection of the Human Brain), from the Faculty of Medicine in Prague. He worked there from 1831 as an Assistant at the Department of Anatomy and from 1837 as the first Pathological Prosector ever in the General Hospital in Prague. In 1840 he was promoted to the Associate Professorship of Pathological Anatomy and in 1845 was appointed Professor of General, Comparative and Surgical Anatomy and Head of the Department of Anatomy. His life's work contributed substantially to the birth and rise of the so-called 'Prague medical school'. He was elected Dean of the Medical Faculty thrice. He retired in 1871 and died on 3 February 1883 in Litomĕřice (Leitmeritz, Bohemia). His name is forever connected with anatomical and pathological structures, especially the lumbocostal triangle in the posterior part of the diaphragm (Bochdalek Foramen) and the hernia at the same site (Bochdalek Hernia). 14. MISTAKES IN THE USAGE OF ANATOMICAL TERMINOLOGY IN CLINICAL PRACTICE.

Abstract

Background: Anatomical terminology serves as a basic communication tool in all the medical fields. Therefore Latin anatomical nomenclature has been repetitively issued and revised from 1895 (Basiliensia Nomina Anatomica) until 1998, when the last version was approved and published as the Terminologia Anatomica (International Anatomical Terminology) by the Federative Committee on Anatomical Terminology. Methods and results: A brief history of the terminology and nomenclature development is mentioned, along with the concept and contributions of the Terminologia Anatomica including the employed abbreviations. Examples of obsolete anatomical terms and their current synonyms are listed. Clinicians entered the process of the nomenclature revision and this aspect is demonstrated with several examples of terms used in clinical fields only, some already incorporated in the Terminologia Anatomica and a few obsolete terms still alive in non-theoretical communication. Frequent mistakes in grammar and orthography are stated as well. Conclusion: Authors of the article strongly recommend the use of the recent revision of the Latin anatomical nomenclature both in theoretical and clinical medicine. [ABSTRACT FROM AUTHOR]

15. 175TH ANNIVERSARY OF BOCHDALEK'S INAUGURAL DISSERTATION.

Abstract

Background: In 2008, it has been 175 years since the publication of Vincenc Alexandr Bochdalek's thesis. Results: On October 22, 1833 he defended his inaugural dissertation in the Greater Hall of Carolinum in Prague and six days later he got his degree in the Lesser Hall of Carolinum. The dissertation was called Anleitung zur praktischen Zergliederung des menschlichen Gehirnes (A guide of the practical dissection of the human brain). It consisted of an introduction (11 pages), main section (208 pages) and an appendix with 19 defended theses. They was aimed not only at anatomy, but also at various fields of medicine, e.g. atmosphere study, treatment of tetanus by usage of antiphlogistics, comparison of the effects of venesection and drainage of the ascites, blockage of the lacrimal duct etc. Bochdalek pointed out that resorption did not occur via blood veins but via lymph vessels; presented his own views of the nerve function, denied the statement that both eye chambers were lined with the Demours' membrane; warned of overestimating the results of experiments upon animals; contemplated over the indication of the Cesarean section of the dead pregnant women; appealed not to separate the lunatic asylums from hospitals and recommended to dissect brains of the dead lunatics. Conclusions: Although some of Bochdalek's opinions mentioned in the defended theses have been unconfirmed by modern science, this only Bochdalek's monography remains to serve as a proof of one of his findings (flower basket) which later acquired an eponym. Abstract

Aims: Biological incidents jeopardising public health require decision-making that consists of one dominant feature: complexity. Therefore, public health decision-makers necessitate appropriate support., Methods: Based on the analogy with business intelligence (BI) principles, the contextual analysis of the environment and available data resources, and conceptual modelling within systems and knowledge engineering, this paper proposes a general framework for computer-based decision support in the case of a biological incident. At the outset, the analysis of potential inputs to the framework is conducted and several resources such as demographic information, strategic documents, environmental characteristics, agent descriptors and surveillance systems are considered., Results: Consequently, three prototypes were developed, tested and evaluated by a group of experts. Their selection was based on the overall framework scheme. Subsequently, an ontology prototype linked with an inference engine, multi-agent-based model focusing on the simulation of an environment, and expert-system prototypes were created., Conclusions: All prototypes proved to be utilisable support tools for decision-making in the field of public health. Nevertheless, the research revealed further issues and challenges that might be investigated by both public health focused researchers and practitioners.
