Abstract Objective Lower socioeconomic status (SES) is a risk factor for poorer pain-related outcomes. Further, the neighborhood environments of disadvantaged communities can create a milieu of increased stress and deprivation that adversely affects pain-related and other health outcomes. Socioenvironmental variables such as the Area Deprivation Index, which ranks neighborhoods based on socioeconomic factors could be used to capture environmental aspects associated with poor pain outcomes. However, it is unclear whether the ADI could be used as a risk assessment tool in addition to individual-level SES. Methods The current study investigated whether neighborhood-level disadvantage impacts knee pain-related outcomes above sociodemographic measures. Participants were 188 community-dwelling adults who self-identified as non-Hispanic Black or non-Hispanic White and reported knee pain. Area Deprivation Index (ADI; measure of neighborhood-level disadvantage) state deciles were derived for each participant. Participants reported educational attainment and annual household income as measures of SES, and completed several measures of pain and function: Short-form McGill Pain Questionnaire, Western Ontario and McMaster Universities Osteoarthritis Index, and Graded Chronic Pain Scale were completed, and movement-evoked pain was assessed following the Short Physical Performance Battery. Hierarchical linear regression analyses were used to assess whether environmental and sociodemographic measures (i.e., ADI 80/20 [80% least disadvantaged and 20% most disadvantaged]; education/income, race) were associated with pain-related clinical outcomes. Introduction: Medical cannabis (MC) is available upon certification for one of several qualifying conditions in Florida, USA. Previous studies suggested that some people seek cannabis for medical conditions/symptoms beyond those legally permitted. However, data remain limited on patient motives for seeking MC and their experiences around its impact on their health. We aimed to compare reported qualifying conditions for MC certification with the most frequently self-reported reasons for using MC while assessing the alignment between the two and understanding the perceived impacts of MC on self-reported conditions and symptoms. Methods: We conducted a cross-sectional study using survey data from the Medical Marijuana and Me (M3) Data Bank of individuals receiving MC in Florida, USA, in 2022. Participants were recruited via convenience sampling from nine MC clinics/clinic networks across Florida and were asked to fill out an online survey. The study measures included sociodemographic variables, self-reported health conditions, self-reported main reasons for using MC, self-reported qualifying conditions for MC certification, and self-reported perceived impact of MC on health conditions. We cross-tabulated reported qualifying conditions and reasons for MC use and reported the perceived impact per condition. Results: A total of 632 participants completed the survey, of whom 396 (62.66%) were female and 471 (74.53%) were non-Hispanic white. The median (IQR) age was 45 (35, 58). The most frequently reported qualifying conditions were post-traumatic stress disorder (PTSD) (n = 187, 29.59%), a condition not on the qualifying conditions list (n = 175, 27.69%), medical conditions of the same kind/comparable to those listed (n = 140, 22.15%), and chronic nonmalignant pain (n = 62, 25.63%). The top ten most frequently reported reasons for using MC were anxiety (n = 383, 60.60%), chronic pain (n = 278, 43.99%), depression (n = 252, 39.87%), PTSD (n = 220, 34.81%), headaches/migraine (n = 134, 21.20%), fibromyalgia (n = 67, 10.60%), attention-deficit hyperactivity disorder (ADHD) (n = 59, 9.34%), bipolar disorder (n = 53, 8.39%), high blood pressure (n = 41, 6.49%), and cancer (n = 18,2.85%). Of respondents, 70–90% with each qualifying condition reported it as one of the main reasons for using MC. Most respondents reported improvement of anxiety (n = 430/451, 95.34%), depression (n = 381/392, 97.20%), chronic pain (n = 305/310, 98.39%), insomnia/sleeping problems (n = 225/295, 86.44%), PTSD (n = 247/270, 91.48%), headaches/migraine (n = 172/218, 78.90%), ADHD (n = 82/123, 66.67%), bipolar disorder (n = 79/89, 88.76%), and fibromyalgia (n = 77/82, 93.90%). Most respondents were unsure/reported no change in blood pressure (n = 93/162, 57.41%). A small percentage reported perceived worsening impacts on their conditions. Conclusion: Qualifying conditions and self-reported reasons for using MC aligned for most respondents. Yet, a notable proportion of respondents sought MC for broader treatment effects beyond those delineated by the officially recognized qualifying conditions in Florida, USA. Most patients perceived positive effects, including those with limited available evidence on efficacy.

Abstract: This study aimed to estimate the prevalence and identify social factors and preventive strategies associated with the coronavirus disease 2019 (COVID-19) in socio and economically vulnerable people (recyclable waste pikers, immigrants/refugees, and homeless people) in Goiânia, Goiás State, Central-Western Brazil. A cross-sectional study was conducted from July 2020 to October 2020. COVID-19 positivity was defined as a positive total anti-SARS-COV-2 antibody test and/or RNA test for SARS-COV-2. Univariable and multiple regression analyses were performed to identify the variables associated with COVID-19. Of the 594 participants, 47.3% were recyclable waste pickers, 29.6% were immigrants/refugees, and 23.1% were homeless people. The positivity for SARS-CoV-2 RNA was 14.1%, whereas for anti-SARS-CoV-2 a total of 30.8% were positive, and 39.4% were positive for at least one COVID-19 marker. Among the 541 individuals, being immigrants/refugees, not wearing a surgical mask, and having three or more people sleeping in the same room were associated with SARS-CoV-2 infection, while using TV news as the main source of information about the pandemic was a protective predictor of COVID-19. This study revealed ethnic and socioeconomic inequalities in the prevalence of COVID-19 among impoverished people in Brazil. Additionally, a high prevalence of COVID-19 was detected in all three groups. Developing new strategies to combat and prevent communicable diseases affecting this population is essential for mitigating future and ongoing pandemics.

Introduction: Chronic obstructive pulmonary disease and asthma are significant respiratory conditions that contribute to substantial morbidity and mortality globally. Chronic obstructive pulmonary disease remains the third most prevalent cause of death worldwide, with 90% of chronic obstructive pulmonary disease deaths attributed to combustible cigarette smoking. Asthma, although often less fatal, leads to considerable health burdens, particularly among marginalized populations who are at higher risk for both more severe chronic obstructive pulmonary disease and asthma outcomes. This study investigates the association between race/ethnicity and current cigarette smoking among adults diagnosed with chronic obstructive pulmonary disease and/or asthma in the U.S. before, during, and after the COVID-19 pandemic. Methods: The authors analyzed data from 10,763 adults (aged ≥40 years) with a history of chronic obstructive pulmonary disease and/or asthma surveyed in the 2019, 2021, and 2023 National Health Interview Survey. The authors estimated predicted probabilities derived from multiple logistic regression and negative binomial regression models to examine changes in (1) the prevalence of current cigarette smoking in the target population, (2) the mean number of days of cigarette smoking in the past 30 days, and (3) the mean number of cigarettes smoked in the past 30 days across racial/ethnic subgroups and prepandemic, pandemic, and postpandemic periods. Results: Current cigarette smoking was reported by about one fifth (20%) of U.S. adults diagnosed with chronic obstructive pulmonary disease and asthma. Among them, the mean number of days of cigarette smoking in the past 30 days was 27 days, and the mean number of cigarettes smoked in the past 30 days was 14. Hispanic individuals had the lowest prepandemic rates of current cigarette smoking (10.5%); however, it was the only racial/ethnic group showing a significant change during the pandemic, with rates increasing to 14.9% during the pandemic. Non-Hispanic White individuals, who showed the highest rates of current cigarette smoking (21.1%) during the pandemic, were the only racial/ethnic group showing a significant decline in the postpandemic period (19.8%). No significant changes were observed in the mean number of days or cigarettes smoked in the past 30 days, except for a marginally significant increase in cigarette consumption among non-Hispanic Black individuals. Conclusions: These analyses suggest that the pandemic had a differential impact on the rates of current smoking across racial/ethnic subgroups in adults with chronic obstructive pulmonary disease and/or asthma, with a detrimental effect in Hispanic individuals and an improvement among non-Hispanic White individuals. Given the dangers of smoking in chronic lung conditions, future research should investigate the factors behind these to develop targeted interventions.

Background Prescription pain reliever (PPR) overdoses differentially affect ‘American Indian/Alaskan Natives’ in the United States (US). Here, studying onset of extra-medical PPR use in 12-24-year-olds, we examine subgroup variations in rates of starting to use prescription pain relievers extra-medically (i.e., to get ‘high’ or for other reasons outside boundaries of prescriber’s intent). Risk differences (RD) are estimated for US-born versus non-US-born young people, stratified by American Indian/Alaskan Natives versus other ethnic self-identities. Methods Between 2002–2009, nationally representative cross-sectional samples of 12–24-year-old non-institutionalized civilians completed interviews for the US National Surveys of Drug Use and Health. Analysis-weighted annual incidence estimates, RD, and confidence intervals (CI) are from the Restricted-use Data Analysis System, an online software tool for US National Surveys of Drug Use and Health. Results Each year, an estimated 2.5% of 12-24-year-olds in the US start using PPR extra-medically (95% CI [2.1%–3.0%]). Background Cannabis use and cannabis regulatory policies recently re-surfaced as noteworthy global research and social media topics, including claims that Mexicans have been sending cannabis and other drug supplies through a porous border into the United States. These circumstances prompted us to conduct an epidemiological test of whether the states bordering Mexico had exceptionally large cannabis incidence rates for 2002–2011. The resulting range of cannabis incidence rates disclosed here can serve as 2002–2011 benchmark values against which estimates from later years can be compared. Methods The population under study is 12-to-24-year-old non-institutionalized civilian community residents of the US, sampled and assessed with confidential audio computer-assisted self-interviews (ACASI) during National Surveys on Drug Use and Health, 2002–2011 (aggregate n ∼ 420,000) for which public use datasets were available. We estimated state-specific cannabis incidence rates based on independent replication sample surveys across these years, and derived meta-analysis estimates for 10 pre-specified regions, including the Mexico border region. Results From meta-analysis, the estimated annual incidence rate for cannabis use in the Mexico Border Region is 5% (95% CI [4%–7%]), which is not an exceptional value relative to the overall US estimate of 6% (95% CI [5%–6%]). Geographically quite distant from Mexico and from states of the western US with liberalized cannabis policies, the North Atlantic Region population has the numerically largest incidence estimate at 7% (95% CI [6%–8%]), while the Gulf of Mexico Border Region population has the lowest incidence rate at 5% (95% CI [4%–6%]). Within the set of state-specific estimates, Vermont’s and Utah’s populations have the largest and smallest incidence rates, respectively (VT: 9%; 95% CI [8%–10%]; UT: 3%; 95% CI [3%–4%]). Discussion Based on this study’s estimates, among 12-to-24-year-old US community residents, an estimated 6% start to use cannabis each year (roughly one in 16). Relatively minor variation in region-wise and state-level estimates is seen, although Vermont and Utah might be exceptional. As of 2011, proximity to Mexico, to Canada, and to the western states with liberalized policies apparently has induced little variation in cannabis incidence rates. Our primary intent was to create a set of benchmark estimates for state-specific and region-specific population incidence rates for cannabis use, using meta-analysis based on independent US survey replications. Public health officials and policy analysts now can use these benchmark estimates from 2002–2011 for planning, and in comparisons with newer estimates.

Objetivos. Identificar los factores asociados con la membresía a las barras bravas, la incidencia del uso de drogas y la asociación entre membresía y uso de drogas. Materiales y métodos. Se analizan los datos reportados por 1303 adolescentes escolares que participaron en dos etapas (T1 y T2) de un estudio longitudinal realizado en 23 colegios de Bogotá, Colombia entre 2006 y 2007, seleccionados a partir de un muestreo probabilístico estratificado por conglomerados. Se implementaron modelos de regresión logística para estudiar las asociaciones de interés. Resultados. En T1, 8,2% de los encuestados reportaron membresía a las barras bravas. Luego de un año de seguimiento, el 14,2% y el 4,6% de los estudiantes que afirmaron y negaron respectivamente pertenecer a una barra brava, iniciaron el consumo de drogas. La asociación entre membresía a las barras bravas e inicio del uso de drogas permanece después de ajustar por múltiples factores de confusión (RRA: 2,4; IC 95%: 1,2 - 4,7). En el análisis estratificado por sexo, esta asociación permanece significativa únicamente en las mujeres (RRA: 6,1; IC 95%: 2,5 - 15,3). Conclusiones. La membresía a las barras bravas mostró incrementar el riesgo de inicio del uso de drogas, particularmente en las mujeres. Los hallazgos implican que se deben focalizar los esfuerzos preventivos hacia las mujeres en dichos contextos.Objectives. To identify the factors associated with barras bravas (soccer fans clubs) membership, the incidence in drug use and the association between membership and drug use. Materials and methods. This report analyzed data from 1303 adolescent scholars who participated in two stages (T1 and T2) of a longitudinal study done in 23 schools of Bogota during 2006- 2007, selected through a probabilistic sampling stratified by clusters. Logistic regression models were implemented in order to evaluate the associations of interest. Results. In T1, 8,2% of the surveyed students reported barras bravas membership. After one year of follow-up, 14,2% of barras bravas members, and 4,6% of non-members, initiated drug use. The association between barras bravas membership and drug use onset remained significant after adjusting for multiple confounding factors (ARR=2,4; 95%CI=1,2-4,7). Sex-stratified analysis indicated that this association remained statistically significant only for females (ARR=6,1; 95%CI=2,5-15,3). Conclusions. Barras bravas membership increases the risk of drug use onset, particularly among females. The findings imply that preventive efforts should be focused on females in these contexts.
