Abstract

Breast cancer risk associated with germline likely pathogenic/pathogenic variants (PV) varies by gene, often by penetrance (high >50% or moderate 20–50%), and specific locus. Germline PVs in BRCA1 and BRCA2 play important roles in the development of breast and ovarian cancer in particular, as well as in other cancers such as pancreatic and prostate cancers and melanoma. Recent studies suggest that other cancer susceptibility genes, including ATM, CHEK2, PALB2, RAD51C and RAD51D confer differential risks of breast and other specific cancers. In the era of multigene panel testing, advances in next-generation sequencing technologies have notably reduced costs in the United States (US) and enabled sequencing of BRCA1/2 concomitantly with additional genes. The use of multigene-panel testing is beginning to expand in Europe as well. Further research into the clinical implications of variants in moderate penetrance genes, particularly in unaffected carriers, is needed for appropriate counselling and risk management with data-driven plans for surveillance and/or risk reduction. For individuals at high risk without any pathogenic or likely pathogenic variant in cancer susceptibility genes or some carriers of pathogenic variants in moderate-risk genes such as ATM and CHEK2, polygenic risk scores offer promise to help stratify breast cancer risk and guide appropriate risk management options. Cancer patients whose tumours are driven by the loss of function of both copies of a predisposition gene may benefit from therapies targeting the biological alterations induced by the dysfunctional gene e.g. poly ADP ribose polymerase (PARP) inhibitors and other novel pathway agents in cancers with DNA repair deficiencies. A better understanding of mechanisms by which germline variants drive various malignancies may lead to improvements in both therapeutic and preventive management options. • The interpretation of genetic testing results requires careful attention. • ATM, CHEK2, RAD51D and BARD1 correlated with breast and other cancers risk. • European and American guidelines discrepancies. • Support European healthcare providers in interpreting and managing female carriers. Abstract

Abstract: Maize (Zea mays L.) silage is of major importance for milk production in the Northwest of Portugal. Farmers typically have a variety of maize hybrids to choose from according to cycle length and sowing date. The general recommendation regarding cultivar selection is to use long cycle cultivars for early sowing dates and vice versa. Cycle length, sowing date and temperature regime will determine the harvest date. Because weather regime is unknown at sowing date, there is a need to develop decision support based on historical weather series to help farmers optimize silage production. Production optimization occurs through a better matching of cycle length to sowing date to produce more and better silage at optimal harvest dates. The CERES-Maize crop model was used to establish decision support to help farmers identify the best cultivar and sowing date combinations. Cultivar parameters were estimated from 3-year field experiments involving five planting dates and six cycle lengths (FAO 200 to 700). The model was run with 39 years of historical weather data, simulating 18 sowing dates and 6 cycle lengths. Decision support was developed based on the analysis of simulation outputs and three integrated risk management strategies. Tactical use of guidelines is illustrated with examples. Abstract

The aim of this study was to conduct a preliminary investigation on the occurrence of Salmonella spp. in eggs from chickens raised in backyards in Portugal and Romania. A lack of compliance with safety practices by chicken owners, was demonstrated, especially in Portugal, as 96% of the eggs were visibly dirty and 92.5% were stored at room temperature. In Romania the 202 analysed eggs were Salmonella free, whereas in Portugal six of the 200 eggs sampled were positive for Salmonella spp. (3%). A positive egg for Salmonella spp. was found in 10.7% of the 56 backyard flocks sampled in Portugal. One egg exhibited contamination both in the shell-membrane mixture and in its content, while in the remaining eggs, the pathogen was found either in the shell-membrane (n = 2) or in the yolk and white mixture (n = 3). The serotypes S. Typhimurium (with identical PFGE patterns) and S. Enteritidis were isolated from five eggs and one egg, respectively. Whilst S. Enteritidis was sensitive to the 14 antibiotics tested, S. Typhimurium isolates presented divergent antimicrobial resistant phenotypes and three were classified as multi-drug resistant. • In Portugal six of the 200 eggs were positive for Salmonella spp. (3%). • 10% of backyards sampled with an egg positive for Salmonella spp. • Contamination in shell (n = 2), in eggs content (n = 3), or both (n = 1). • One isolate identified as S. Abstract

We dedicate this manuscript in memory of a dear friend and colleague Bella Kaufman. The fifth International Consensus Symposium for Breast Cancer in Young Women (BCY5) took place virtually in October 2020, organized by the European School of Oncology (ESO) and the European Society of Medical Oncology (ESMO). Consensus recommendations for the management of breast cancer in young women were updated from BCY4 with incorporation of new evidence to inform the guidelines. Areas of research priorities as well as specificities in different geographic and minority populations were identified. This manuscript summarizes the ESO–ESMO international consensus recommendations, which are also endorsed by the European Society of Breast Specialists (EUSOMA). • Breast cancer in young women encompasses unique treatment and survivorship considerations. • Fertility, ovarian function suppression and premature menopause are major issues that need to be addressed. • The presence of a germline pathogenic variant impacts risk reduction measures, local management and systemic therapy. • Research on how new systemic therapies impact fertility and ovarian function is urgently needed.
