Abstract Background Policy makers, clinicians and researchers are demonstrating increasing interest in using data linked from multiple sources to support measurement of clinical performance and patient health outcomes. However, the utility of data linkage may be compromised by sub-optimal or incomplete linkage, leading to systematic bias. In this study, we synthesize the evidence identifying participant or population characteristics that can influence the validity and completeness of data linkage and may be associated with systematic bias in reported outcomes. Methods A narrative review, using structured search methods was undertaken. Key words "data linkage" and Mesh term "medical record linkage" were applied to Medline, EMBASE and CINAHL databases between 1991 and 2007. Abstract inclusion criteria were; the article attempted an empirical evaluation of methodological issues relating to data linkage and reported on patient characteristics, the study design included analysis of matched versus unmatched records, and the report was in English. Included articles were grouped thematically according to patient characteristics that were compared between matched and unmatched records. Results The search identified 1810 articles of which 33 (1.8%) met inclusion criteria. There was marked heterogeneity in study methods and factors investigated. Characteristics that were unevenly distributed among matched and unmatched records were; age (72% of studies), sex (50% of studies), race (64% of studies), geographical/hospital site (93% of studies), socio-economic status (82% of studies) and health status (72% of studies). Conclusion A number of relevant patient or population factors may be associated with incomplete data linkage resulting in systematic bias in reported clinical outcomes. Readers should consider these factors in interpreting the reported results of data linkage studies.

Co-design, co-development, and co-delivery (Co-3D for short) are activities within the co-production research pathway that are increasingly being used in climate change science and adaptation projects. However, the research community is still coming to understand how best to incorporate Co-3D in practice, as each project has a specific context around stakeholder relationships, governance arrangements, and capacity to actively participate. This paper outlines five case studies from Australia as examples of different projects engaging with Co-3D in different ways in order to explore how Co-3D is being used and might be improved. Crucially, we include the perceptions and experiences of researchers, funders and end users, as well as our own critical reflections. Each of the projects self-describes as using ‘co-production’, but the extent and format varies widely with different combinations of co-design, co-development and/or co-delivery used in each. Our findings show that without clear understanding of Co-3D within the co-production process, aspects of Co-3D may not be properly considered in planning or implementation. Co-3D activities are not completely distinct, rather they form a continuum of engagement and integration across phases of project work. Thus, the specific definitions and delineations between these terms may not be required for them to be applied. However, practical and explicit negotiation of what ‘co-production’ means in different project contexts is needed so that all parties understand their roles and responsibilities. Further, more evaluations of outcomes and stakeholder experiences are required. We provide seven principles of Co-3D that should be considered when embarking on co-production projects.

Abstract Background Comprehensive national joint replacement registries with well-validated data offer unique opportunities for examining the potential future burden of hip and knee osteoarthritis (OA) at a population level. This study aimed to forecast the burden of primary total knee (TKR) and hip replacements (THR) performed for OA in Australia to the year 2030, and to model the impact of contrasting obesity scenarios on TKR burden. Methods De-identified TKR and THR data for 2003–2013 were obtained from the Australian Orthopaedic Association National Joint Replacement Registry. Population projections and obesity trends were obtained from the Australian Bureau of Statistics, with public and private hospital costs sourced from the National Hospital Cost Data Collection. Procedure rates were projected according to two scenarios: (1) constant rate of surgery from 2013 onwards; and (2) continued growth in surgery rates based on 2003–2013 growth. Sensitivity analyses were used to estimate future TKR burden if: (1) obesity rates continued to increase linearly; or (2) 1–5% of the overweight or obese population attained a normal body mass index. Results Based on recent growth, the incidence of TKR and THR for OA is estimated to rise by 276% and 208%, respectively, by 2030. The total cost to the healthcare system would be $AUD5.32 billion, of which $AUD3.54 billion relates to the private sector. Projected growth in obesity rates would result in 24,707 additional TKRs totalling $AUD521 million. A population-level reduction in obesity could result in up to 8062 fewer procedures and cost savings of up to $AUD170 million. Conclusions If surgery trends for OA continue, Australia faces an unsustainable joint replacement burden by 2030, with significant healthcare budget and health workforce implications. Strategies to reduce national obesity could produce important TKR savings.

The sustainable development and food security of islands in the Asia-Pacific region is severely compromised by climate change, sea level rise and compounding socio-economic issues. To achieve a step-change in food production and climate adaptation, livelihoods must rapidly transform. Food security programs continue to apply the “pipeline” model of scaling-out technological innovations, but do not account for the social-ecological complexity of islands. We tested the feasibility of scaling-out adaptation strategies in two provinces in the region: Nusa Tenggara Barat in Indonesia, and West New Britain in Papua New Guinea. Guided by a sub-district typology of resource use, we trialled a participatory, systems-based livelihood adaptation pathways approach in sub-district case studies. The process aimed to mainstream social learning and future uncertainty into community development decision-making, yielding ‘no regrets' adaptation strategies to transform livelihoods. We tested two assumptions: first, that because the contexts of all villages were homogenous, strategies were sufficiently similar to enable scaling-out across the provinces; second, that the sub-district typologies would assist scaling-out within each type. The results showed that the first assumption was untenable: there was very little similarity amongst villages' strategies; only sustainable fisheries management was scalable amongst coastal villages. The second was marginally tenable, because there were strong similarities amongst villages in an off-shore island type. When pooled into classes of adaptation strategy, most related to practice and behaviour change, and addressed systemic social issues; very few were technological. Our results suggest that scaling-out livelihood and food system innovations is not feasible due to the complex social-ecological contexts within islands, caused by steep climate gradients, natural resource and cultural diversity. We discuss the limitations of a resource use typology that aimed to mitigate this complexity and guide scaling-out. Instead we argue that appropriate social learning approaches akin to livelihood adaptation pathways must be mainstreamed into existing community development decision cycles, thereby “scaling-up” and “scaling-deep” to tackle institutional, political and cultural barriers to transformation. We discuss the implications of our recommendations for government and donor support for food security programs in islands of the Asia-Pacific region, and future research priorities.

Abstract Background Femoroacetabular impingement syndrome (FAI), a hip disorder affecting active young adults, is believed to be a leading cause of hip osteoarthritis (OA). Current management approaches for FAI include arthroscopic hip surgery and physiotherapy-led non-surgical care; however, there is a paucity of clinical trial evidence comparing these approaches. In particular, it is unknown whether these management approaches modify the future risk of developing hip OA. The primary objective of this randomised controlled trial is to determine if participants with FAI who undergo hip arthroscopy have greater improvements in hip cartilage health, as demonstrated by changes in delayed gadolinium-enhanced magnetic resonance imaging (MRI) of cartilage (dGEMRIC) index between baseline and 12 months, compared to those who undergo physiotherapy-led non-surgical management. Methods This is a pragmatic, multi-centre, two-arm superiority randomised controlled trial comparing hip arthroscopy to physiotherapy-led management for FAI. A total of 140 participants with FAI will be recruited from the clinics of participating orthopaedic surgeons, and randomly allocated to receive either surgery or physiotherapy-led non-surgical care. The surgical intervention involves arthroscopic FAI surgery from one of eight orthopaedic surgeons specialising in this field, located in three different Australian cities. The physiotherapy-led non-surgical management is an individualised physiotherapy program, named Personalised Hip Therapy (PHT), developed by a panel to represent the best non-operative care for FAI. It entails at least six individual physiotherapy sessions over 12 weeks, and up to ten sessions over six months, provided by experienced musculoskeletal physiotherapists trained to deliver the PHT program. The primary outcome measure is the change in dGEMRIC score of a ROI containing both acetabular and femoral head cartilages at the chondrolabral transitional zone of the mid-sagittal plane between baseline and 12 months. Secondary outcomes include patient-reported outcomes and several structural and biomechanical measures relevant to the pathogenesis of FAI and development of hip OA. Interventions will be compared by intention-to-treat analysis. Discussion The findings will help determine whether hip arthroscopy or an individualised physiotherapy program is superior for the management of FAI, including for the prevention of hip OA. Many challenges posed by the current Anthropocene epoch require fundamental transformations to humanity's relationships with the rest of the planet. Achieving such transformations requires that humanity improve its understanding of the current situation and enhance its ability to imagine pathways toward alternative, preferable futures. We review advances in addressing these challenges that employ systematic and structured thinking about multiple possible futures (futures-thinking). Over seven decades, especially the past two, approaches to futures-thinking have helped people from diverse backgrounds reach a common understanding of important issues, underlying causes, and pathways toward optimistic futures. A recent focus has been the stimulation of imagination to produce new options. The roles of futures-thinking in breaking unhelpful social addictions and in conflict resolution are key emerging topics. We summarize cognitive, cultural, and institutional constraints on the societal uptake of futures-thinking, concluding that none are insurmountable once understood. [ABSTRACT FROM AUTHOR]

26. Scenario planning to leap-frog the Sustainable Development Goals: An adaptation pathways approach

Few studies have examined how to mainstream future climate change uncertainty into decision-making for poverty alleviation in developing countries. With potentially drastic climate change emerging later this century, there is an imperative to develop planning tools which can enable vulnerable rural communities to proactively build adaptive capacity and ‘leap-frog’ the Sustainable Development Goals (SDGs). Using an example from Indonesia, we present a novel participatory approach to achieve this. We applied scenario planning to operationalise four adaptation pathways principles: (1) consideration of climate change as a component of multi-scale social-ecological systems; (2) recognition of stakeholders’ competing values, goals and knowledge through co-learning; (3) coordination of responses across multiple decision-making levels; and (4) identification of strategies which are ‘no regrets’, incremental (tackling proximate drivers of community vulnerability) and transformative (tackling systemic drivers). Workshops with stakeholders from different administrative levels identified drivers of change, an aspirational vision and explorative scenarios for livelihoods in 2090, and utilised normative back-casting to design no regrets adaptation strategies needed to achieve the vision. The resulting ‘tapestry’ of strategies were predominantly incremental, and targeted conventional development needs. Few directly addressed current or possible future climate change impacts. A minority was transformative, and higher level stakeholders identified proportionately more transformative strategies than local level stakeholders. Whilst the vast majority of strategies were no regrets, some were potentially mal-adaptive, particularly for coastal areas and infrastructure. There were few examples of transformative innovations that could generate a step-change in linked human and environmental outcomes, hence leap-frogging the SDGs. We conclude that whilst effective at integrating future uncertainties into community development planning, our approach should place greater emphasis on analysing and addressing systemic drivers through extended learning cycles.

The claim that in natural resource management (NRM) a change from anthropocentric values and ethics to eco-centric ones is necessary to achieve sustainability leads to the search for eco-centric models of relationship with the environment. Indigenous cultures can provide such models; hence, there is the need for multicultural societies to further include their values in NRM. In this article, we investigate the environmental values placed on a freshwater environment of the Wet Tropics by a community of indigenous Australians. We discuss their environmental values as human values, and so as beliefs that guide communities’ understanding of how the natural world should be viewed and treated by humans. This perspective represents a step forward in our understanding of indigenous environmental values, and a way to overcome the paradigm of indigenous values as valued biophysical attributes of the environment or processes happening in landscapes. Our results show that the participant community holds biospheric values. Restoring these values in the NRM of the Wet Tropics could contribute to sustainability and environmental justice in the area.
