Abstract This systematic review aims to determine whether the presence of human papillomavirus (HPV) influences the immunohistochemical expression of programmed cell death-1 ligand (PD-L1) in oropharyngeal squamous cell carcinoma (OPSCC). PD-L1 immunohistochemical expression varies in OPSCC, and the presence of HPV is a plausible explanation for this variability. Comprehending these findings is crucial, as high PD-L1 expression in the tumor microenvironment of OPSCC can help identify patient subgroups that could be suitable for immunotherapy. Therefore, a systematic review was conducted following PRISMA guidelines (CRD42023437800). An electronic literature search was performed without time or language restrictions. The search included PubMed/MEDLINE, Embase, Scopus, Web of Science, https://clinictrials.gov, and relevant journals. A meta-analysis was performed using RStudio. Fourteen studies involving 1,629 participants were included. The sample consisted predominantly of males (81.26%) with a mean age of 58.3 years. Concerning clinical and pathological characteristics, the most frequently described anatomical location was the tonsils (68.54%), and most participants were either current or former smokers (78%) and alcohol users (79%). Advanced TNM IV was the most common stage. Regarding histopathological characteristics, HPV 16 was the only type mentioned, and half of the cases were detected through immunohistochemistry. The SP142 clone (35.7%) and the pattern of membrane immunostaining in tumor cells (71%) were the most commonly employed methods. The most prevalent findings were positive expression of PD-L1 (64.28%) and negative HPV status (57.14%). The association between PD-L1 positivity and HPV positivity (78.57%) was confirmed by meta-analysis. The conclusion was that HPV-positive status has an impact on immunohistochemical expression of PD-L1 in OPSCC.

Abstract Temporomandibular disorders (TMD) is a term used to describe a set of clinical conditions that may compromise the temporomandibular joint (TMJ) and masticatory muscles and/or associated structures, considered the most frequent cause of orofacial pain of non-dental origin. In recent years, many forms of physical therapy have been used in the treatment of TMD to reduce pain and improve the range of mandibular movement present in this impairment. Among these resources are kinesiotherapy (exercise), electrothermal and manual therapy, acupuncture, training posture, mobilizations, and biofeedback. Objectives To determine if exercises with or without occlusal splints are effective in reducing pain in patients with temporomandibular disorders (TMD) of myogenic origin. Methodology This systematic review was registered in the International Prospective Register of Systematic Reviews (CRD 42019134244). Controlled trials published in PubMed, Scopus, and Cochrane Library following PRISMA guidelines up to April 2022 were randomized and included. The population above 18 years, which evaluated the effectiveness of exercise with or without occlusal splints in reducing pain in patients with TMD of myogenic origin, diagnosed through the Research Diagnostic Criteria for Temporomandibular Disorders, was also included. There was no restriction on the period of publication. Cochrane risk of bias analysis was performed. Results Of the five included articles, all showed a reduction of pain, but without significant differences between the interventions performed. Additionally, studies that evaluated the quality of life and mandibular movements showed a reduction in pain, but no significant differences between therapies. Conclusion The analyzed studies showed no difference in the improvement of pain, quality of life, and mandibular movements between the groups that performed only exercises or the associated treatments.

Swallowing syncope is a neurological eating disorder associated with severe transient heart disorders. This case report aimed to describe a rare case of swallowing syncope after orthognathic surgery.

ABSTRACT Despite being a benign tumor of the maxillofacial region, some cases of ameloblastoma can be categorized as malignant ameloblastoma (or metastasizing) when metastases occur. The aim of this study is to report a rare case of lung metastasis from mandibular ameloblastoma, in order to review its risk and analyze the main anatomic sites that can occur with this disease. The case of a 48-year-old woman is described. She presented a metastatic pulmonary ameloblastoma 7 years after the removal of a mandibular ameloblastoma. During routine exams, a tumor in the left lung was observed. It was asymptomatic, near to the mediastinum, measured 7x5.5 cm. Transthoracic needle biopsy revealed ameloblastoma with the same histological characteristics of the primary tumor. After radiotherapy, the patient presented regression of the tumor. The patient has been under follow-up for 5 years and there is no presence of tumor. Ameloblastoma is an aggressive tumor not only in the region of origin, but also in distant regions, mainly in cases of recurrence. Metastases can cause high rates of morbidity, a fact that requires early treatment.

Abstract This study aimed to investigate if SNP rs6313, SNP rs2770304, SNP rs4941573, and SNP rs1923884 of the 5-HT2A receptor gene and SNP rs6295 of the 5-HT1A receptor gene are associated with bruxism etiology. Methodology This systematic review was registered in PROSPERO (CRD42018094561). A search was conducted for articles published in or before May 2021. To qualify for eligibility in this review, the studies had to be case-controls, cohort or cross-sectional. The inclusion criteria were the articles with a group of patients with bruxism and a control group in which the presence of these SNPs was evaluated. The exclusion criteria were the investigations of other polymorphisms, the studies that did not consider a control group for comparison, case reports, and reviews. The NOS and JBI were used to evaluate the methodological quality of studies. Results We conducted this study with databases, such as Web of Science, Scopus, Embase, PubMed/MEDLINE, and ProQuest. We considered four studies eligible. A total of 672 participants were included,187 with sleep bruxism, 105 with awake bruxism, 89 with sleep and awake bruxism, and 291 controls. One study found a strong association between the SNPs rs6313, rs2770304 and rs4941573 of the 5-HT2A receptor gene and sleep bruxism. In one study, we considered the C allele of the SNP rs2770304 a risk factor for sleep bruxism. We found no significant results of other SNPs in sleep bruxers compared to controls. We found no positive association concerning the SNPs and groups of awake bruxism and sleep and awake bruxism. Conclusion The different results regarding the SNPs in sleep bruxers could be explained by the genetic distinction between Chilean, Mexican, Japanese, and Polish population. More clinical trials and prospective studies must be conducted with larger sample size and in different ethnicities to confirm the results of this review.

Abstract The objective of this study was to apply elliptic Fourier analysis (EFA) to find shape differences among skeletal growth patterns in both radiographic and tomographic panoramic views, controlling for asymmetry. Lateral and panoramic images were obtained from 350 patients. After screening patients with asymmetric linear and angular values and natural asymmetric hemimandibular shape, 240 patients were included in the study: 48 with tomographic information and 192 with radiographic information. The images were classified according to the mandibular plane angle and the ANB angle. Mandibular contours were digitized on the panoramic images and EFA was performed with 20 harmonics, filtering rotation, translation and size properties. As there were no differences between radiographic and tomographic panoramic mandibular contours and normal distribution was found in all groups, MANOVA was conducted to determine differences using a Hotelling’s p-values with Bonferroni correction and an XY graph tool was applied to visualize these differences graphically. A 95% confidence level was used. Significative differences were found among hypodivergent, normodivergent, and hyperdivergent patterns in Class I, II, and III (p < 0.05), located mainly in the symphyseal region. The results of this study suggest that EFA is a useful tool to mathematically analyze mandibular contours and their morphological differences given by facial biotypes. This method could improve the precision of the mandibular prediction models.

Abstract The aim of the present overview was to evaluate the outcomes of systematic reviews to determine the incidence of condylar resorption in patients submitted to orthognathic surgery and analyze whether the risk of developing this condition is related to a specific type of surgery. Searches were conducted in the PubMed/MEDLINE, Embase, and Cochrane electronic databases for systematic reviews with quantitative data on condylar resorption due to any type of orthognathic surgery for dentoskeletal deformities published up to May 25, 2019. The AMSTAR 2 and Glenny tools were applied for the quality appraisal. Five systematic reviews were included for analysis. Only one article was considered to have high quality. Among a total of 5128 patients, 12.32% developed condylar resorption. From those patients, 70.1% had double jaw surgery, 23.4% had mandibular surgery alone, and in 6.5% a Lefort I technique was used. Based on these findings, bimaxillary surgery could be considered a risk factor for condylar resorption. However, these results should be interpreted with caution, since other factors, such as pre-operative skeletal deformities, type of movement, and type of fixation, can contribute to the development of this condition. Further studies should consider reporting main cephalometric data, temporomandibular diagnosis, hormonal levels, and tomographic measures before and after the surgery at least every 6 months during the firsts two years to identify accurately risk factors for condylar resorption.

Pacientes com insuficiência óssea alveolar representam um problema comum na reabilitação de edêntulos. O levantamento do seio maxilar é o procedimento cirúrgico que permite através de enxerto ósseo na cavidade do seio, o aumento do volume vertical da porção posterior da maxila, possibilitando a reabilitação oral através de implantes dentários. Apesar de ser um procedimento com comprovado potencial de resolução das limitações causadas pelas atrofias maxilares posteriores, não está livre de complicações. Esta revisão teve como objetivo resgatar as informações disponíveis acerca das complicações associadas a cirurgias de levantamento de seio maxilar publicadas na base de dados MEDLINE/PUBMED nos últimos cinco anos. Após leitura de título, resumo e em seguida texto completo foram incluídos 22 estudos. A perfuração de membrana do seio maxilar foi a ocorrência mais comum durante a cirurgias de levantamento de seio maxilar. Mais estudos precisam ser realizados para identificar as repercussões destas complicações.

Abstract Polymethyl methacrylate (PMMA) has been considered a suitable material for cranioplasty. However, no consensus has been reached concerning the best material for cranioplasty with regard to minimizing complications. Thus, this systematic review and meta-analysis aimed to compare the complication rates of PMMA with those of autologous bone and titanium mesh. This review was registered with PROSPERO (CRD42016042725). Systematic searches were conducted on PubMed/MEDLINE, Scopus, and Web of Science. The focus question was, “Do PMMA prostheses used in cranioplasty have complications rates similar to those of autologous bone and titanium mesh?” A meta-analysis of complication rates was performed on the basis of dichotomous outcomes assessed by risk ratio (RR) with corresponding 95% confidence intervals (CI). From 1014 data sources, 11 articles were selected according to eligibility criteria. These articles involved 1,256 individuals and 1,278 cranioplasties using autologous bone (n = 408), PMMA (n = 379), or titanium (n = 151). The follow-up period ranged from 63 days to 54.3 months. No difference was observed between the complication rates of PMMA and autologous bone (p = 0.94; RR, 0.98; 95%CI, 0.54–1.75) or between PMMA and titanium (p = 0.38; RR, 1.59; 95%CI, 0.57–4.48). Sub-analysis of the reasons for craniotomy (trauma/non-trauma) was conducted, which revealed no significant difference (p = 0.91; RR, 0.95; 95%CI, 0.37–2.42). The meta-analysis indicated that the use of PMMA yields complication rates that are near those of autologous bone and titanium mesh.

PURPOSE: To evaluate the effects of particulate (granule-shaped) SCB on bone repair relating it to its biocompatibility and bone neoformation. METHODS: Thirty Wistar rats were submitted to a one 7-mm-diameter defect and divided equally into three experimental groups, with two different postoperative times of evaluation, 90 and 120 days. Each calvaria defect was filled up with clot (control group), particulated autogenous bone or granulated SCB. Five animals of each group were assessed at 90 and 120 days after surgery. In these two periods, histological and histometric analysis were obtained. RESULTS: The clot group showed a bone resorption trend while the autogenous bone group a bone repair trend. However in the SCB group, the critical defect filled up only with fibrous connective tissue and presented none bone neoformation. CONCLUSION : The sugarcane biopolymer when used in critical size defects was a biocompatible material and proved to be a good material to fill bone cavities, keeping them as uniform areas filled with soft tissue and avoiding the tissue shrinkage.

Introduction: Patients with dentofacial deformities may benefit from orthognathic surgery in the maxilla. Maxillary osteotomy may include procedures in the bone, cartilaginous, and soft tissues of the nose, leading to shape alterations. Objective: To evaluate the anatomic alterations of the nasal region in patients undergoing a Le Fort I osteotomy for advancement or superior impaction. Methods: This is a clinical prospective study. Twenty-one patients were evaluated during the preand postoperative periods. The positioning of the nasal tip and the modification of the nasal base were evaluated. Results: The results showed that the nasal tip was superiorly positioned in 85% of the cases, advanced in 80%, rotated in 80%, and there was a wide nasal base in 95%, resulting in esthetic improvement. Conclusions: Surgeries of maxillary advancement and superior reposition tend to cause elevation and advancement of the nasal tip, as well as enlargement of the nasal base.

The aim of this study was to evaluate the effectiveness and level of satisfaction of patients who underwent TMJ arthrocentesis under local anesthesia by considering the following parameters: joint pain, mouth opening, mastication, and satisfaction. Fourteen patients (13 women and one man; mean age, 37.6 years with TMJ arthralgia were selected. The patients underwent arthrocentesis under local anesthesia, and pain intensity was measured before, during, and 1 year after treatment using a visual analog pain scale (VAS 0–100 mm). A Likert scale was used to assess patients’ opinion regarding the improvement of mouth opening and mastication. After treatment, patients were questioned on levels of satisfaction provided by arthrocentesis in regard to their quality of life. At the end of 1 year, patients showed a marked improvement in their pain clinical picture (p < 0.0001). Mouth opening and mastication, evaluated separately and then correlated (R2 = 0.925, p < 0.0001), also showed a marked improvement. When questioned, all patients reported being satisfied with the treatment outcome.

PURPOSE: To evaluate bone regeneration in critical defects in the rats' calvarium. METHODS: Eighteen rats Wistar were divided into three groups of six animals each according to the time of evaluation (15, 30 and 60 days). One calvarium defect of 5mm was made in the parietal bone of each animal under general anesthesia. After the time of evaluation, the animals were killed, when the bone was histological studied and classified according to the type of tissue found: fibrosis or bone. RESULTS: The results showed that in the group of 15 days, in five animals there was only fibrosis. In the group of 30 days, the process of regeneration was growing and in four animals was found bone, in three with partial filling and in the other one with complete filling of the defect. In the group of 60 days, out of the three animals with bone, two had a complete filling of the defect. CONCLUSIONS: There was no bone regeneration in critical defects in 15 days. There was regeneration in the most part of the animals in 30 and 60 days.OBJETIVO: Avaliar a regeneração óssea de defeitos críticos em calota craniana de ratos. MÉTODOS: Foram utilizados 18 ratos Wistar que foram distribuídos em três grupos de acordo com o tempo de avaliação (15, 30 e 60 dias). Na calota craniana desses animais foi realizado defeito de 5mm, após anestesia geral prévia. Após o tempo de avaliação, os animais foram submetidos à eutanásia e a calota foi enviada para estudo histológico, quando foi classificada de acordo o tecido encontrado: fibroso ou ósseo. RESULTADOS: Diante dos achados deste estudo, pode-se observar que para o grupo de 15 dias na maioria dos animais, n=5, foi encontrado apenas fibrose. Com o passar do tempo de avaliação, no grupo de 30 dias, o processo de reparo foi evoluindo e em quatro animais já foi encontrado osso, sendo em três com preenchimento parcial e em um completo. No grupo de 60 dias, o processo praticamente permaneceu o mesmo, onde em três animais foi encontrado osso, e, desses, em dois o preenchimento foi completo. CONCLUSÕES: Não houve regeneração óssea do defeito crítico para o período de avaliação de 15 dias. Houve regeneração óssea em grande parte dos animais para o período de avaliação de 30 e 60 dias.

PURPOSE: To evaluate the bone healing of mandibular fractures following the use of Portland cement. METHODS: Thirty-two male Wistar rats were divided into control and experimental groups. In the control group the rats were submitted to a mandibular fracture, which was reduced, and the soft tissues were sutured. In the experimental group the rats had the mandibular fracture reduced and maintained with the Portland cement. The animals were euthanized 7 and 21 days after surgery by injecting a lethal dose of anesthetic. The following variables were studied: weight of the animals, radiographic images, histopathological features and time of surgery. RESULTS: A weight loss was observed in the specimens of both groups at the different times of evaluation, a greater difference in weight before and after surgery being found in the experimental group, which was statistically significant (p OBJETIVO: Avaliar a reparação óssea de fratura mandibular após o uso do cimento Portland (CP). MÉTODOS: Trinta e dois ratos machos Wistar foram divididos em grupo controle e grupo experimental. No grupo controle os ratos foram submetidos à fratura, redução e manutenção dos seguimentos com sutura dos tecidos moles. No grupo experimental foram submetidos a fratura, redução e manutenção dos segmentos fraturados com CP e sutura dos tecidos. Os animais foram eutanasiados com sete e 21 dias de pós-operatório através da injeção de dose letal dos anestésicos adotados. As variáveis estudadas foram: peso dos animais, avaliação tomográfica, avaliação histológica e tempo cirúrgico. Because of its biocompatibility and osteoconductive properties, calcium-phosphate cement has been used as bone surrogate. OBJECTIVE: The purpose of this study was to do a histomorphometric comparison of bone regeneration using hydroxyapatite biphasic ceramic, calcium-phosphate cement and autogenous bone graft. STUDY DESIGN: Prospective and laboratorial experiment. MATERIALS AND METHODS: Two 5mm in diameter cavities (left and right - test and control) were made in the parietal bone of 72 rats. The GI, GII, GIII and GIV test cavities were filled with calcium-phosphate cement, biphasic ceramic hydroxyapatite, autogenous bone graft and blood. The animals were killed at 30, 60 and 90 days and the specimens underwent histomorphometric analysis. RESULTS: The results showed that autogenous bone graft treated defects had significantly more new bone at 30 days compared to other test groups. Within 60 and 90 days, bone formation was more significant in the test groups GI, GII and GIII; GI and GII encompassed larger areas. Throughout the evaluation periods, GII tests showed more bone formation when compared to GI. CONCLUSIONS: Biomaterials depicted a significantly increase in bone content, when compared to autogenous bone graft, concerning bone regenerationOs materiais de fosfato de cálcio, pelas características de biocompatibilidade e osteocondução, estão sendo estudados como substitutos ósseos. OBJETIVO: Comparar histomorfometricamente o reparo ósseo promovido pela cerâmica bifásica de hidroxiapatita, cimento fosfato de cálcio e enxerto ósseo autógeno. FORMA DE ESTUDO: Prospectivo e experimental em laboratório. MATERIAL E MÉTODO: Foram criadas duas cavidades (esquerda e direita - teste e controle) de 5mm de diâmetro nos ossos parietais de 72 ratos Wistar. As cavidades testes GI, GII, GIII e GIV foram preenchidas com cimento fosfato de cálcio, cerâmica bifásica de hidroxiapatita, enxerto ósseo autógeno e coágulo sanguíneo. Os animais foram sacrificados aos 30, 60 e 90 dias e os espécimes avaliados histomorfometricamente. RESULTADOS: Os resultados mostraram maior formação óssea no período de 30 dias para o GIII quando comparado aos outros grupos testes. Nos períodos de 60 e 90 dias, a neoformação óssea foi mais acentuada nos grupos testes GI, GII e GIII, alcançando maiores áreas em GI e GII. Em todos os períodos de avaliação, as cavidades testes GII apresentaram maiores áreas frente às GI. CONCLUSÃO: Os biomateriais foram superiores ao enxerto autógeno no reparo ósseo

PURPOSE: To describe and evaluate normal rat temporomandibular joints from anatomic and histopathologic point of view and make a comparison between this joint in rats and humans. METHODS: Twelve male adult Wistar rats (12 same side joints) were used in this procedure. The following anatomical structures were histologically evaluated in a qualitative fashion: condyle, disc, temporal bone, retrodiscal tissue and synovia. The macroscopical and microscopic study of the human TMJ was based on the current literature. RESULTS: The TMJ is surrounded by a thin capsule, consisting of fibrous tissue, and a synovial lining. The mandibular angle has a prominent shape. The glenoid fossa is flat, with no eminences. Histologically, the TMJ is composed of different tissues that comprise the mandibular head, mandibular fossa and fibrocartilaginous disc. A layer of hyaline cartilage covers the articulating cortical condyle and temporal bone. CONCLUSION:Morphologically and histologically, the articular structure of rats is, on the whole, similar to that of humans. In these animals there is no articular eminence.OBJETIVO: Descrever e avaliar a articulação temporomandibular de ratos sob o aspecto anatômico e histológico e realizar uma comparação entre esta articulação e a de humanos. MÉTODOS: Foram utilizados doze ratos adultos Wistar (12 articulações do mesmo lado). As seguintes estruturas anatômicas foram avaliadas de forma qualitativa: côndilo, disco, osso temporal, tecido retrodiscal e sinóvia. O estudo macroscópico e o microscópico da ATM humana foram baseados na literatura atual. RESULTADOS: A ATM é envolvida por uma fina cápsula, formada por tecido fibroso e cobertura sinovial. O ângulo mandibular é proeminente. A fossa glenoide é rasa, sem eminência articular. De acordo com os achados histológicos, a ATM é composta por diferentes tecidos, são eles a cabeça da mandíbula, a fossa mandibular e o disco fibrocartilaginoso. Uma camada de cartilagem hialina recobre a superfície articular do côndilo e do osso temporal. CONCLUSÃO: De acordo com os achados morfométricos e histológicos, a estrutura articular em ratos é, no geral, similar a articulação humana. Exceto que nesses animais não há eminência articular.

PURPOSE: To compare tissue cicatrization in the tongue of rats using electrosurgery and the cold blade. METHODS: Ten adult male Wistar rats were divided into two groups of 5 animals each, according to the time of sacrifice (3 and 7 days). Each animal had two incisions, one made with a cold blade and the other with an electric blade, both of which were approximately 0.5 mm in length. Following sacrifice of the animals, the tongues were submitted to a histological study in order to classify the presence of angiogenesis, fibroblastic proliferation, epithelial proliferation and inflammatory cells as good, moderate and weak. RESULTS: The wound made with electric blade presented a delay in the healing process and a greater inflammatory response as compared with the cold blade, despite the fact that there was no statistically significant difference (pOBJETIVO: Comparar a cicatrização tecidual com bisturi elétrico e frio em língua de ratos. MÉTODOS: A amostra foi de 10 ratos Wistar, divididos em dois grupos com 5 animais em cada, de acordo com o tempo de sacrifício (3 e 7 dias).Cada animal teve duas incisões transversais, com 0,5 mm de comprimento, no dorso da língua, sendo a mais anterior com a eletrocirurgia (experimento), e a mais posterior, com bisturi convencional (controle). Após o sacrifício dos animais, as línguas foram submetidas a análise histológica para classificação da presença de angiogeneses, proliferação fibroblástica e epitelial e células inflamatórias em boa, moderada e fraca. RESULTADOS: Os resultados indicaram que as feridas realizadas com bisturi elétricos apresentaram um retardo no processo cicatricial e uma maior resposta inflamatória quando comparadas com as feridas do bisturi frio, apesar de não ter havido diferença estatisticamente significante (p

Information contained in the present study can better explain the type of care performed in this service, which is paramount in order to plan, organize and improve medical care here. AIM: the goal of the present study was to carry out a retrospective epidemiological study concerning facial trauma from August 2006 to August 2007. MATERIALS AND METHODS: 211 charts were studied in order to collect patient data regarding the number of patients seen, gender, age, year and their origin, surgical approaches and cases. RESULTS: Among the surgical cases, facial fractures were the most prevalent (73.9%). Male patients prevailed (81.5%), in the ages between 11 and 40 years from the arid Paraíba mesoregion. The most frequent causes we motorcycle accidents, making up 64.5% of the sample, and the zygomatic-orbital complex was the most affected site. CONCLUSION: The most affected patients were males, and those from the Paraíba arid mesoregion were the ones who benefited the most from the service. Among the most frequent etiological factors we list: motorcycle accidents, physical fights and falls from one's own height.As informações contidas na presente pesquisa provêem dados para um melhor esclarecimento do tipo de atendimento realizado pelo serviço, sendo de fundamental importância para o planejamento, organização e melhoria do atendimento. OBJETIVO: Este trabalho teve como finalidade realizar um estudo epidemiológico retrospectivo dos traumas de face, no período compreendido entre agosto de 2006 a agosto de 2007. MATERIAL E MÉTODO: Foram examinados 211 prontuários e coletados dados relacionados ao número de pacientes atendidos, gênero, idade, ano e procedência, modalidades e acessos cirúrgicos realizados. RESULTADO: Dentre as modalidades cirúrgicas, as fraturas faciais foram as mais prevalentes (73,9%). Houve uma predominância de pacientes do gênero masculino (81,5%), na faixa etária compreendida entre 11 e 40 anos e oriundos da mesorregião do agreste paraibano. As causas mais frequentes foram os acidentes motociclísticos, com 64,5% da amostra, sendo o complexo zigomático-orbitário a região mais acometida. CONCLUSÃO: Os pacientes mais acometidos foram os do gênero masculino, sendo os oriundos da mesorregião do Agreste Paraibano os que mais se beneficiam com o serviço. Dentre os fatores etiológicos mais frequentes foram: acidente de trânsito motociclístico, agressões físicas e queda de própria altura.

Summary: Surgical access to the skull base is always difficult, especially because of the noble anatomic structures present there. Maxillary osteotomy provides direct view to the clivus region and the neck spine, and it also bears less morbidity when compared to the many other accesses described in the literature. Aim: to assess 11 patients submitted to transmaxillary osteotomy, describing the surgical technique and postoperative results and complications. Materials and Methods: A retrospective study involving eleven patients submitted to transmaxillary approach to the brainstem. We studied dental occlusion, trans and postoperative bleeding, bone necrosis and soft tissue alterations. All followed the same surgical protocol and were followed up for two years. Results: after treatment, all the patients improved in their clinical status and had no neurological complication, trans and postoperative hemorrhage or major complications were seen. Among the complications, two patients had incomplete maxilla fracture, two had laceration of their nasal mucosa and one had, as late complication, an oral-sinusal fistula. Conclusion: Transmaxillary osteotomy provided proper access to the clivus for brainstem decompression with low rate of complications in this series. Keywords: maxilla, osteotomy

PURPOSE: To analyze the action of gangliosides in peripheral nerve regeneration in the sciatic nerve of the rat. METHODS: The sample was composed of 96 male Wistar rats. The animals were anaesthetized and, after identification of the anaesthesic plane, an incision was made in the posterior region of the thigh, followed by skin and muscle divulsion. The right sciatic nerve was isolated and compressed for 2 minutes. Continuous suture of the skin was performed. The animals were randomly divided into two groups: the experimental group (EG), which received subcutaneous injection of gangliosides, and the control group (CG), which received saline solution (0.9%) to mimic the effects of drug administration. RESULTS: No differences were observed between the experimental and control groups evaluated on the eighth day of observation. At 15 and 30 days the EG showed an decrease in Schwann cell activity and an apparent improvement in fibre organization; at 60 days, there was a slight presence of Schwann cells in the endoneural space and the fibres were organized, indicating nerve regeneration. At 15 and 30 days, the level of cell reaction in the CG had diminished, but there were many cells with cytoplasm in activity and in mitosis; at 60 days, hyperplastic Schwann cells and mitotic activity were again observed, as well as nerve regeneration, but to a lesser extent than in the EG. CONCLUSION: The administration of exogenous gangliosides seems to improve nerve regeneration.OBJETIVO: Avaliar os efeitos de gangliosídeos na regeneração nervosa periférica em nervo isquiático de ratos após axonotmese. MÉTODOS: Foram utilizados 96 ratos machos albinos (Wistar). Os animais foram anestesiados e após constatação do plano anestésico, foi realizada incisão na face posterior da coxa direita do animal. Em seguida, foi realizada a dissecção cirúrgica da pele e do músculo e divulsão dos músculos. O nervo isquiático direito foi isolado e sofreu compressão por 2 minutos. Efetuou-se a sutura contínua da pele. Os animais foram distribuídos aleatoriamente em 2 grupos: experimental (GE) que receberam gangliosídeos pela via sub-cutânea e controle (GC) que receberam soro fisiológico 0,9% com a finalidade de mimetizar os efeitos de administração da droga de estudo. A análise histopatológica foi realizada em 8, 15, 30 e 60 dias. RESULTADOS: Não se evidenciaram diferenças significantes entre os grupos controle e experimental avaliados com 8 dias. Nos grupos experimentais de 15 e 30 dias observou-se uma diminuição da atividade das células de Schwann e aparente melhora na organização das fibras nervosas; com 60 dias havia discreta presença células de Schwann no espaço endoneural e as fibras nervosas estavam organizadas sinalizando a regeneração nervosa. Nos grupos controles de 15 e 30 dias o padrão de reação celular diminuiu, entretanto havia muitas células com citoplasmas em atividade e mistose; com 60 dias observou-se ainda a presença de hiperplasia de células de Schwann, atividade mitótica ainda presente e regeneração nervosa presente, porém em menor grau comparando-se com aquele visto no grupo experimental. CONCLUSÃO: A administração de gangliosídeos exógenos parece incrementar a regeneração nervosa.

Cancer is the terminology applied to a set of more than one hundred diseases that have disorderly cell growth in common. The treatmentof cancer can be performed by surgery, radiotherapy, chemotherapy, or bone marrow transplant. In the region of the head and neck,radiotherapy results in a series of complications in the patient, such as hypovascularization of the irradiated tissues, with reduced demand of oxygen and cells, and there is risk of the occurrence of osteoradionecrosis after tooth extraction, leading to the need for a differentiated approach to these patients. Of the therapeutic conduct adopted in the approach to irradiated patients, hyperbaric oxygen therapy is a well established resource, as the therapeutic principle of this modality enables the restoration of an adequate process of tissue repair. In this study, a clinical case is related, of the use of hyperbaric oxygen therapy as an adjuvant procedure, before and after multiple tooth extractions, in a patient irradiated in the head and neck region, obtaining success with cicatricial repair without complications.

OBJETIVOS: Neste estudo realizamos a análise clínica e histopatológica da reação tecidual dos fios de nylon e poliglecaprone 25 monofilamentares nas suturas interna e externa em ratos. MÉTODOS: Foram utilizados 40 Rattus norvegicus (Wistar) machos. O ato operatório consistiu de incisão e divulsão dos planos muscular e cutâneo realizadas na região posterior das coxas dos animais. As suturas internas e externas da coxa direita foram realizadas com o fio de nylon n°5-0, e na coxa esquerda aplicamos o poliglecaprone 25 n°5-0. Os animais foram divididos em 4 grupos (n=10) de acordo com o tempo pós-operatório G1 (7 dias), G2 (14 dias), G3 (21 dias) e G4 (28 dias). Para a avaliação cínica foi considerada a ocorrência de deiscência, de exsudato e edema. Na análise histopatológica objetivou-se avaliar reação inflamatória, células gigantes de corpo estranho, proliferação fibroblástica e fibrose. RESULTADOS: Clinicamente, não foram observadas alterações nos grupos estudados. Histopatologicamente a reação inflamatória, presença de células gigantes de corpo estranho, proliferação fibroblástica e fibrose foram maiores nas suturas internas realizadas com Nylon. Nas suturas realizadas com poliglecaprone 25 essa reação declinou com o passar do tempo pós-operatório. CONCLUSÃO: De acordo com a metodologia empregada podemos concluir que as suturas externas realizadas com nylon induziram menor reação tecidual, enquanto que nas suturas internas este fio contribuiu para perpetuar a reação tecidual. As suturas externas realizadas com poliglecaprone 25 apresentaram maior reação tecidual, e suturas internas realizadas com o mesmo fio a reação tecidual declinou na medida em que o fio estava sendo absorvido.

Being overweight is recognised as a significant risk factor for several morbidities; however, the experience of the dentistry faculties focusing on this population is still low. The aim of the present study was to determine the occurrence of adverse events during removal of impacted lower third molars in overweight patients. A prospective cohort study was carried out involving overweight patients subjected to surgical removal of impacted lower third molar as part of a line of research on third molar surgery. Predictor variables indicative of the occurrence of adverse events during surgery were classified by their demographic, clinical, radiographic, and surgical aspects. Descriptive and bivariate statistics were computed. In total, 140 patients fulfilled the eligibility criteria, and 280 surgeries were performed. Patients’ mean age was 25.1 ± 2.2 years, and the proportion of women to men was 3 : 1. Eight different adverse events during surgery were recorded.
