Introduction: Men and women with psychosis have different courses and presentations of symptoms. Men with psychosis have an earlier onset of illness, more negative symptoms, and worse premorbid functioning. Women, on the other hand, have better social functioning and less substance abuse. Despite these evident differences, there are few studies that delve into these distinctions, especially from a subjective perspective. Objectives: The aim of this study is to understand the differences in the perception of psychosis between men and women. Methods: Five women and five men diagnosed with schizophrenia participated in the study. They were matched so that the age difference between them was no more than 5 years, with ages ranging from 40 to 56 years. Participants had not experienced acute decompensation of their underlying illness and had not required admission to an Acute Care Unit in the 6 months prior to inclusion in the study. Data collection was conducted through the Spanish translation of the Indiana Psychiatric Illness Interview, consisting of five parts: a narrative about their life, a narrative about the illness, questions related to how the illness has changed their life and what has not changed, the overall influence of the illness on their life, and lastly, expectations for the future. Results: Men expressed more concerns about work (4 men versus 2 women), while women expressed more concerns about not having become mothers (3 out of 5 women, compared to one man). All participants shared experiences of isolation in intimate relationships, including romantic relationships. Regarding stigma, three women believed that people treated them like children and dismissed their opinions. However, two of them viewed this behavior from their loved ones positively. Two women discussed the impact that psychosis and medications had on their bodies and how others had reacted to these changes Conclusions: The concerns and stigma associated with mental illness differ between genders. Introduction: Contrary to classical belief, people affected by this disease are at greater risk of developing organic pathologies.This risk has a very complex origin: a greater exposure to risk factors and specific socioeconomic conditions, a high prevalence of risk behaviors, the use of antipsychotics, and a potential common genetic background. (Reynolds et al.Int. J.Neuropsychopharmacol.2021; 24 854–855, Suvisaari J et al. Curr Diab Rep. 2016 16). Multiple studies demonstrate that Schizophrenia confers a high endogenous risk of Diabetes. Before patients diagnosed with Schizophrenia start taking antipsychotics (Andreassen OA et al. Am J Psychiatry. 2017;174 616-617), they have an approximately 3 times higher risk of developing Diabetes compared to the general population. The risk increases 3.6 times after the initiation of antipsychotic treatment compared to drug naive patients(Annamalai A et al World J Diabetes. 2017 390-396) Objectives: To study the association between Schizophrenia or other Psychotic Disorders and Diabetes Mellitus in a sample of patients diagnosed with Schizophrenia or other Psychotic Disorders. Methods: This is a Descriptive and Cross-sectional Observational Study. Clinical Histories were reviewed and a personal or telephone interview was established to expand data related to the objectives of the study. The patients were recruited among the patients seen in the specific Severe Mental Disorder consultation who had a diagnosis of schizophrenia or other Psychotic Disorders, according to DSM 5-TR criteria. Results: From a sample of 93 patients, 24 had Diabetes. The Prevalence of Diabetes in patients with Schizophrenia or other Psychotic Disorders was 25.8%. Of the patients without a diagnosis of Diabetes, 15 of them had values of Glycosylated Hemoglobin (HbA1c) for Prediabetes. Using the Chi-Square Test, statistically significant differences were found between the variable Main Psychiatric Medication and Diabetes. Patients treated with Clozapine, Aripiprazole and Olanzapine had a Prevalence of Diabetes of 40.9%, 33.3% and 28.5%, respectively. Conclusions: Prevalence of Diabetes in our sample was 3.4 times higher than the 7.51% of the general population in Spain. This presumes a significant importance and impact on the health of these patients. The diabetic patients in our sample were diagnosed with Diabetes years after the diagnosis of the mental illness, which seems to indicate that the causes have to do with lifestyle, dietary habits, weight, and exposure to chronic antipsychotics. Premature death in schizophrenia has several explanations, being of special importance the development of cardiovascular disorders and Diabetes This can be due to many reasons, but it is worth highlighting the metabolic side effects of some antipsychotics and lifestyle. Purpose To determinate biomechanical and functional changes in human keratoconic corneas after triangular section ring implantation in two designs. Methods Retrospective study of biomechanical properties of 46 patients (60 eyes) with keratoconus undergoing surgery Keraring® intracorneal ring segments divided into two groups according to 5 or 6 mm of diameter. Surgical technique to made the channel was with IntraLase femtosecond laser. UCVA and BSCVA were measured and biomechanical factors provided by the device ORA before surgery and postoperatively one month, 3 months and 6 months. Results After surgery, we obtained a significant increase (p<0.05) of the biomechanical parameters (CH and CRF) mean (0.87±2.59)mmHg for CH and (‐0.015±2.06)mmHg for CRF. This improvement is not homogeneous during the postoperative follow‐up in two groups in diameter. We found increased of CH top with rings of 6mm. The variations found in the CRF, are not significant at the descriptive level with any of the designs (p>0.05). At baseline‐ 6 months, the improvement in BSCVA (LogMAR) is 0.13 in 5mm diameter respect to 0.07 in 6mm in diameter. Among the changes in CH and BSCVA (LogMAR), there is a significant inverse correlation with 6mm rings at baseline‐ 1 month period (r=‐0.442) and 1 month‐ 3 month period 0.007 (r=‐0.511). No significant correlation was detected between CRF and BSCVA. Conclusion Intracorneal rings improve corneal biomechanics. The CH undergoes improvement in postoperative follow up with any of the ring designs, while the CRF is not affected significantly with any of the types. 5mm rings further improve best corrected visual acuity. We have designed an integrated device that couples input and output coaxial horn antennas with a self-assembled helical slow-wave structure cradled in a v-groove on a silicon-on-insulator wafer. These devices will potentially enable the characterization of cold parameters and beam-wave interaction in slow-wave structures on a wafer level, i.e., without having to dice and package individual devices. Background: Ischemia-reperfusion injury (IRI) upon transplantation is one of the most impactful events that the kidney graft suffers during its life. Its clinical manifestation in the recipient, delayed graft function (DGF), has serious prognostic consequences. However, the different definitions of DGF are subject to physicians' choices and centers' policies, and a more objective tool to quantify IRI is needed. Here, we propose the use of donor-derived cell-free DNA (ddcfDNA) for this scope., Methods: ddcfDNA was assessed in 61 kidney transplant recipients of either living or deceased donors at 24 h, and 7, 14 and 30 days after transplantation using the AlloSeq cfDNA Kit (CareDx, San Francisco, CA, USA). Patients were followed-up for 6 months and 7-year graft survival was estimated through the complete and functional iBox tool., Results: Twenty-four-hour ddcfDNA was associated with functional DGF [7.20% (2.35%-15.50%) in patients with functional DGF versus 2.70% (1.55%-4.05%) in patients without it, P = .023] and 6-month estimated glomerular filtration rate (r = -0.311, P = .023). At Day 7 after transplantation, ddcfDNA was associated with dialysis duration in DGF patients (r = 0.612, P = .005) and worse 7-year iBox-estimated graft survival probability (β -0.42, P = .001) at multivariable analysis. Patients with early normalization of ddcfDNA (<0.5% at 1 week) had improved functional iBox-estimated probability of graft survival (79.5 ± 16.8%) in comparison with patients with 7-day ddcfDNA ≥0.5% (67.7 ± 24.1%) (P = .047)., Conclusions: ddcfDNA early kinetics after transplantation reflect recovery from IRI and are associated with short-, medium- and long-term graft outcome. The SARS-CoV-2 (COVID-19) pandemic represents the infection with the highest lethality, but also the one that has caused the most sequelae and multi-organ consequences, especially respiratory, in the last century. Several actions have been required in the field of respiratory and intensive care medicine to reduce mortality and chronicity. The consequences of COVID-19 are multiple and encompass different physical, emotional, organizing, and economic aspects, which will require a multidisciplinary, transversal, and collaborative approach. This review includes the observations and results of published retrospective and prospective studies on post-COVID19 respiratory sequelae, especially after severe pneumonia with associated adult respiratory distress syndrome (ARDS).

A significant proportion of bronchiectasis patients are chronically infected by potentially pathogenic microorganisms which may lead to frequent exacerbations and worse clinical outcomes. Current bronchiectasis guidelines recommend long-term inhaled antibiotics and/or oral macrolides as a part of patient management. In recent years, an increasing amount of evidence assessing the impact of these treatments on patient outcomes has been collected. Inhaled antibiotics have demonstrated significant improvements in sputum bacterial load, but their impact on patient quality of life, lung function, and exacerbation rate has not been consistent across trials. In this regard, recent post hoc analyses of inhaled antibiotics trials in bronchiectasis patients have shown that sputum bacterial load may be a key biomarker to predict treatment response in these patients. Oral macrolides, on the other hand, have proven to reduce exacerbation frequency and improve quality of life, but potential drug-related adverse effects and the increase in bacterial resistance are relevant. Introduction: Many severe COVID-19 patients require respiratory support and monitoring. An intermediate respiratory care unit (IMCU) may be a valuable element for optimizing patient care and limited health-care resources management. We aim to assess the clinical outcomes of severe COVID-19 patients admitted to an IMCU. Methods: Observational, retrospective study including patients admitted to the IMCU due to COVID-19 pneumonia during the months of March and April 2020. Patients were stratified based on their requirement of transfer to the intensive care unit (ICU) and on survival status at the end of follow-up. A multivariable Cox proportional hazards method was used to assess risk factors associated with mortality. Results: A total of 253 patients were included. Of them, 68% were male and median age was 65 years (IQR 18 years). Ninety-two patients (36.4%) required ICU transfer. Patients transferred to the ICU had a higher mortality rate (44.6 vs. 24.2%; p < 0.001). Multivariable proportional hazards model showed that age ≥65 years (HR 4.14; 95%CI 2.31-7.42; p < 0.001); chronic respiratory conditions (HR 2.34; 95%CI 1.38-3.99; p = 0.002) and chronic kidney disease (HR 2.96; 95%CI 1.61-5.43; p < 0.001) were independently associated with mortality. High-dose systemic corticosteroids followed by progressive dose tapering showed a lower risk of death (HR 0.15; 95%CI 0.06-0.40; p < 0.001). Conclusions: IMCU may be a useful tool for the multidisciplinary management of severe COVID-19 patients requiring respiratory support and non-invasive monitoring, therefore reducing ICU burden. Swimming performance is a key feature that mediates fitness and survival in aquatic animals. Dispersal, habitat selection, predator-prey interactions and reproduction are processes that depend on swimming capabilities. Testing the critical swimming speed (U crit ) of fish is the most straightforward method to assess their prolonged swimming performance. We analysed the contribution of several predictor variables (total body length, experimental water temperature, time step interval between velocity increments, species identity, taxonomic affiliation, native status, body shape and form factor) in explaining the variation of U crit , using linear models and random forests. We compiled in total 204 studies testing U crit of 35 inland fishes of the Iberian Peninsula, including 17 alien species that are non-native to that region. We found that body length is largely the most important predictor of U crit out of the eight tested variables, followed by family, time step interval and species identity. By contrast, form factor, temperature, body shape and native status were less important. Results showed a generally positive relationship between U crit and total body length, but regression slopes varied markedly among families and species. By contrast, linear models did not show significant differences between native and alien species. In conclusion, the present study provides a first comprehensive database of U crit in Iberian freshwater fish, which can be thus of considerable interest for habitat management and restoration plans. The resulting data represents a sound foundation to assess fish responses to hydrological alteration (e.g. water flow tolerance and dispersal capacities), or to categorize their habitat preferences.

Pollution from human activities is a major threat to the ecological integrity of fluvial ecosystems. Microbial communities are the most abundant organisms in biofilms, and are key indicators of various pollutants. We investigated the effects some human stressors (nutrients and heavy metals) have on the structure and activity of microbial communities in seven sampling sites located in the Ter River basin (NE Spain). Water and biofilm samples were collected in order to characterize physicochemical and biofilm parameters. The 16S rRNA gene was analysed out from DNA and RNA extracts to obtain α and β diversity. Principal coordinates analyses (PCoA) of the operational taxonomic units (OTUs) in the resident microbial community revealed that nutrients and conductivity were the main driving forces behind the diversity and composition. The effects of mining have had mainly seen on the taxonomic composition of the active microbial community, but also at the OTUs level. Remarkably, metal-impacted communities were very active, which would indicate a close link with the stress faced, that is probably related to the stimulation of detoxification., (Copyright © 2019 The Authors. To assess the hospitalisation requirements for medical specialties (MS), admissions to a district hospital were studied during two subsequent years. Of the 196,800 inhabitants, 3% (6,089) were admitted which resulted in 7,841 admissions during this period. Four hundred and forty two different main diagnosis were performed but only 14 led to 50% of admissions and these same entities were also involved in any of the diagnosis associated (comorbidity). Mean age of the total group was 61 +/- 18 years being women patients three years older than men. More men than women were admitted. Pathology complexity was usually due to comorbidity, rather than a specific entity. Therefore we consider that the relationship between primary, general medicine and specialty attention should be reviewed and the involvement of MS in district hospitals should be revised so that the service may be efficient as well as improving cost-effectiveness ratio and thus achieving a remarkable economical saving.

Hereditary hemorrhagic telangiectasia (HHT) is a hereditary dominant autosoma clinical entity characterized by repeated hemorrhages (being epistaxis the more frequent) and telangiectasia in skin, mucosa and internal organs. Incidence in Europe is 1,2 per 100,000 pop. We discuss the experience of an Internal Medicine Department (Arnau de Vilanova Hospital), during 10 years, in which 12 patients were diagnosed. With the aim to provide more information about this rare disease. There was no predominant sex and the age of onset was very variable. All patients referred at admission past history of hemorrhages (either the patient him/herself or some relative), or were suffering it at admission. Except one patient, all of them showed telangiectasias in skin and mucosa. In five of them telangiectaias were found in upper gastrointestinal tract, in two there were localized on the tracheobronchial tree and one patient had teleangiectasia in liver. Three patients were diagnosed of pulmonary arteriovenous fistula (PAVF). The clinical, diagnostic and therapeutic aspects of this disease are reviewed.

A study of 5 patients diagnosed as having pulmonary tuberculosis during a period of 14 months is presented. 3 of them are members of the same family of 7 and the other 2 are closely related. This study calls our attention to the need for carrying out contacts evaluation, relative or not, and for chemoprophylaxis. The presentation of microepidemyes in relation to the decrease of the prevalence was also observed.

The causes of the maintained endemia of tuberculosis in the area of the "Hospital Arnau de Vilanova", Valencia, were studied in 162 cases treated over a 6 year period. The social conditions of the area, predisposition factors and the rare clinical and radiological first manifestations are factors influencing the maintained endemia. The introduction of invasive diagnosis techniques to confirm the disease, brought about the diagnoses of cases that with the standard techniques would have been impossible, giving the false impression of a higher incidence. We concluded that it is necessary to keep the disease in mind in the differential diagnosis of risk groups, maintaining the required epidemiological measures.
