In the Eastern Anatolia Region, where traces of life have been seen since prehistoric times, many communities have settled in the historical process and many states have established sovereignty. The region, which is on the migration routes between the Caucasus and Anatolia, has experienced intense activity since the Bronze Age. The effect of these migration movements is seen in the transfer of many cultures, especially the Karaz Culture, to Anatolia. In the region, which was the scene of intense political struggles in the 2nd millennium BC, the dominance of the Urartian State, which was the important political formation of the 1st millennium BC, is seen. Ardahan province, located in the northeast of the Eastern Anatolia Region, is in a strategic position on the transition route from Anatolia to the Caucasus. Due to this location, it has been inhabited since prehistoric times. The settlement, which was concentrated in the Bronze Age, continued increases in the Iron Age. The inscriptions that prove that they reached Ardahan, which was under the sovereignty of the Urartian State, were found in Hanak/Ortakent and Taşköprü. In addition, Sengertepe Castle, Taşköprü Castle and, Darboğaz Dam are other pieces of evidence of the Urartian presence in the region. Castles are built for administrative, military and, economic purposes. In particular, the security and control of strategic locations such as water resources, road routes and, fertile agricultural lands are provided by the construction of these castles. Brain metastasis is observed in increasing frequency along with the progress in treatment of epithelial ovarian cancer. When compared with brain parenchymal metastases, isolated cerebellum involvement has been reported more rarely. In our patient with advanced stage serous ovarian cancer, solitary cerebellum metastasis and leptomeningeal involvement were observed in the 50th month after the patient's diagnosis of ovarian cancer. when this metastasis was detected, the patient's ca 125 levels were normal. The patient has no systemic metastases. Owing to effective chemotherapeutic agents in ovarian cancer, such isolated metastases will be observed more. Multicentric studies on this subject are needed. Treatment of coronavirus disease-2019 (COVID-19) in quarantine at home may cause some disruptions in various other health problems. If surgical intervention for deep neck abscess is delayed, morbidity and mortality increase. A 45-year-old male patient was admitted with persistent fever, sweating, weakness; inability to open his mouth due to 80x100 mm centrally softened hyperemic mass on the left submandibular region. In neck tomography; it was extending from the inferior of the mandibular arch to the parapharyngeal and peritonsillar area obliterating airway asymmetrically on the left side of the vallecula. Huge purulent material with thrombus drained from the abscess under general anesthesia. During COVID-19 disease, it is possible for upper respiratory tract and tooth infections to rapidly transform into abscess formation due to the possible reduced immunity. To avoid these situations with high mortality and morbidity, such as deep neck infection, is only possible to closely monitor the patient during the COVID-19 disease with the help of technology. Introduction: Although atherosclerotic cardiovascular diseases and cardiovascular risks are known to increase in patients with end-stage renal disease, it is not clear whether these risks increase in the geriatric patient population as well. This study aims to evaluate these risks in geriatric patients with end-stage renal disease by evaluating epicardial fat and carotid and femoral intima-media thicknesses, known as markers, for subclinical atherosclerosis and cardiovascular risks. Materials and Methods: This cross-sectional study included 52 patients who started to receive chronic hemodialysis treatment after the age of 65 years (mean age 73.92±5.63) years with end-stage renal failure and 51 healthy volunteers (mean age: 74.49±4.63 years). Epicardial fat and carotid and femoral intima-media thicknesses were measured and compared between these groups. Results: Carotid intima-media and epicardial fat thicknesses were significantly higher in the patient group than in the control group (0.91±0.08 vs. 0.71±0.1 mm, p<0.001 and 0.84±0.17 vs. 0.75±0.17 cm, p=0.01, respectively). However, no significant difference was observed in femoral intima-media thickness between the two groups (0.58±0.07 vs. 0.56±0.97 mm, p=0.266). Correlation analysis revealed a significant positive correlation between the duration of dialysis and epicardial fat and carotid intima-media thicknesses (r=0.611, p<0.001 and r=0.337, p=0.015, respectively). Furthermore, regression analysis revealed a significant relationship between the duration of dialysis and carotid intima-media thickness (β=0.657, p=0.001). Conclusion: Epicardial fat and carotid intima-media thicknesses increase in geriatric patients with end-stage renal disease but with no significant changes in femoral intima-media thickness, indirectly suggesting that subclinical atherosclerosis and cardiovascular risks are increased in these patients. Neuromyelitis optica (NMO) is an autoimmune disorder of the central nervous system, that predominantly affects the spinal cord and the optic nerve. Its key features include transverse myelitis, commonly associated with extensive inflammation spanning three or more consecutive vertebral segments. Longitudinal extensive spinal cord lesions can also occur in systemic autoimmune diseases, infections, vascular and metabolic disorders, subsequent to irradiation, intramedullary tumors and paraneoplastic myelopathies. We present a case study of an 8-year-old girl seropositive for antibodies against the aquaporin 4 who displayed longitudinal extensive spinal cord lesions, that was initially misdiagnosed as an intramedullary tumor. Spondylodiscitis is an infection of the intervertebral disc and adjacent vertebral bodies. Magnetic resonance imaging is the method of choice for the spondylodiscitis. The characteristic findings in the spondylodiscitis are hypointense on T1-weighted (W) image and hyperintense on T2W and fat-saturation T2W images, contrast enhancement on contrast- enhanced T1W with fat saturation images in the disc space and adjacent vertebral bodies, and phlegmon or abscess of the paraspinal soft tissues and epidural space. Phlegmon shows homogenous contrast enhancement, while abscess shows peripheral ring-enhancement on contrast- enhanced T1W with fat saturation images. Differentiation of tuberculous, brucellar and pyogenic spondylodiscitis is radiological difficult. Features that also favor tuberculosis infection include multilevel disease, large paravertebral abscess, meningeal involvement and subligamentous spread. Brucellar spondylodiscitis most commonly affects the lumbar spine. Bone destruction is less severe than in tuberculous spondylodiscitis. High-grade glial tumors are the most common primary brain tumor in adults. The current standard of care for high-grade glial tumors includes surgical resection followed by combination of radiation with temazolomide treatment and adjuvant temozolomide. The Macdonald Criteria are currently the most widely used guideline for assessing response to therapy in patients with high-grade glial tumors. These are based on the size of the contrast-enhancing lesion in MR imaging. Recently, nontumoral changes in enhancement have been found. It is now clear that evaluation of gadolinium enhancement alone is not adequate to characterize tumor regression or progression. MR diffusion, MR perfusion, MR spectroscopy, and PET imaging will be important adjuncts to traditional imaging for tumor assessment. Purpose: Diffusion-weighted MR imaging (DW-MRI) is generally applied to diagnose intra-axial brain pathologies such as parenchyma metastasis, abscesses, and infarcts in patients with systemic cancer. This study aimed to evaluate the DW-MRI findings in cranial bone marrow metastases to facilitate diagnosis. Material and Methods: Conventional MR imaging and DW-MRI findings of 57 patients with cranial bone marrow metastases were evaluated retrospectively. These images were evaluated by two neuroradiologists for lesion detection and lesion consciousness. Reader agreement was assessed by ? statistics. Results: Fifty-seven patients exhibited 113 cranial bone marrow metastases. The sensitivities of DW-MRI and the apparent diffusion coefficient to detect cranial bone metastasis for all types of primary malignancy were 86% and 90%, respectively. The sensitivities of T1-weighted imaging (T1WI), fluid attenuation inversion recovery, contrast-enhanced fat-suppressed (CE-FS) T1WI, and T2WI sequences to detect cranial bone metastasis were 93%, 90%, 89%, and 84%, respectively. Regarding lesion conspicuousness, DW-MRI was equivalent to CE-FS T1WI and equivalent or superior to non-contrast conventional sequences in most of the cranial bone marrow metastases. Interobserver agreement was excellent. Conclusions: DW-MRI is generally indicated not only to assess the diagnosis of parenchyma involvement, but also to discover the cranial bone marrow metastasis in patients with systemic cancer. A broad range of neurologic disorders has been described in children infected with Mycoplasma pneumoniae, of which encephalitis is among the most common. In contrast, the association between optic neuritis and Mycoplasma pneumoniae infection has been rarely described in children. We report a case of a 12-year-old girl who was seropositive for antibodies against Mycoplasma pneumoniae and presented with optic neuritis without respiratory symptoms or other neurologic findings. Aim: This study was planned with the aim of retrospectively reviewing the clinical and laboratory findings and therapies of our patients diagnosed with tuberous sclerosis and redefining the patients according to the diagnostic criteria revised by the 2012 International Tuberous Sclerosis Complex Consensus Group and comparing them with the literature. Material and Methods: Twenty patients diagnosed with tuberous sclerosis complex in the Pediatric Neurology Clinic were examined retrospectively in terms of clinical findings and therapies. The diagnoses were compared again according to 1998 and 2012 criteria. Results: It was observed that the complaint at presentation was seizure in 17 of 20 patients and hypopigmented spots on the skin in 3 of 20 patients. On the initial physical examination, findings related with the disease were found in the skin in 17 of the patients, in the eye in 5, in the kidneys in 7 and in the brain with imaging in 17. No cardiac involvement was observed in the patients. Infantile spasm was observed in 7 of the patients who presented because of seizure (n=17), partial seizure was observed in 7 and multiple seizure types were observed in 3. It was found that sirolimus treatment was given to 9 of 20 patients because of different reasons, 7 of these 9 patients had epileptic seizures and sirolimus treatment had no effect on epileptic seizures. According to 2012 diagnostic criteria, no marked change occured in the diagnoses of our patients. Conclusions : It was observed that the signs and symptoms of our patients were compatible with the literature. Molecular genetic examination was planned for the patients who were being followed up because of probable tuberous sclerosis complex. Creutzfeldt-Jakob disease (CJD) is a rare dementing disease and is thought to caused by a prion. It is characterized by rapidly progressive dementia, ataxia, myoclonus, akineticmutism and eventual death. Brain biopsy or autopsy is required for a definitive diagnosis of CJD. Diffusion-weighted imaging became an important tool for early diagnosis of CJD because of the high sensitivity. Spontaneous intracranial hypotension is an important cause of new daily persistent headaches in young and middle-aged individuals. The diagnosis is made based on low cerebrospinal fluid pressure with characteristic findings upon brain and spinal magnetic resonance imaging (MRI). We present the case of a 15-year-old boy with spontaneous intracranial hypotension. Although his brain MRI was normal, his lumbar spinal MRI showed clustering of the nerve roots characteristic of arachnoiditis. Radionuclide cisternography revealed an epidural leak, which was treated with an epidural blood patch. The patient reached a near-full recovery within 24 h, and the lumbar spinal MRI findings mimicking arachnoiditis disappeared. [ABSTRACT FROM AUTHOR]

The prevalence of abnormalities on brain magnetic resonance imaging (MRI) varies in primary Sjögren's syndrome (pSS) patients and they are generally multiple hyperintense areas in the subcortical and periventricular white matter on T2-weighted and fluid-attenuated inversion recovery sequences. Here, we report brainstem involvement in a patient with pSS that was extensive on MRI, but reversible. The patient's outcome was positive. Acute cerebellitis is an inflammatory syndrome characterized by cerebellar dysfunction with acute onset. Here, the clinical findings, conventional and diffusion-weighted MR imaging features of five cases are reported. The most common clinical findings were headache, vertigo, nausea and vomiting. Neurological examination revealed cerebellar ataxia, nystagmus, dysmetria and dysdiadokinesia. Analysis of the CSF showed an elevated protein content and pleocytosis. Bilateral cerebellar cortical involvement and leptomeningeal enhancement along the folia represent the most common imaging presentations. Diffusionweighted MR imaging showed restricted diffusion in two patients. In one patient, diffusionweighted MR imaging did not show restricted diffusion. All patients were treated medically. Additionally, external ventricular drainage was performed in one patient. Follow-up MR imaging revealed cerebellar atrophy in 1 patient and gliosis in 2 patients. Two patients completely resolved after medical therapy. In conclusion, brain MR imaging and diffusionweighted MR imaging are valuable imaging methods in diagnosis of cerebellitis, assessment of complications and sequel lesions. [ABSTRACT FROM AUTHOR]

Hypothalamic hamartomas (HH) are rare non-neoplastic congenital malformations. The aims of this study are to demonstrate the brain MR findings and clinical manifestations in children with HH and the literature is reviewed. We have evaluated 6 children with HH diagnosed by brain MR imaging. We analyzed age at seizure onset, seizure types, neuropsychological assessment, and the presence of precocious puberty, electroencephalogram and MR findings. Among 6 HH patients, 2 manifested precocious puberty, 2 had experienced seizures, and 1 presented with both precocious puberty and seizures, and 1 presented with hyperactivity. MR imaging showed hypothalamic mass that was isointense to brain parenchyma on T1- weighted images and isointense-or hyperintense on T2-weighted images, without contrast enhancement. The size of the lesions does not change in the course of follow-up. Typical clinical presentation, peculiar location and MR imaging features strongly favour the HH diagnosis. Colonic herniation through the esophageal hiatus is an unusual condition. Isolated trans-hiatal herniation of the transverse colon is very rare and only five cases have been reported. All these reported patients were symptomatic, and had dysphagia, epigastralgia, vomiting, dyspnea, cough, or palpitations. Hiatal hernia is an important clinical problem. Because of the severe complications of hiatal hernia, including bleeding, strangulation, and perforation, early diagnosis and treatment are important. A 75-year-old man who had prostate carcinoma was evaluated with multidetector computed tomography (MDCT). Inflammatory bowel disease (IBD) is an idiopathic inflammatory disease of the gastrointestinal tract in which the pathogenesis is not known wholly. Thrombosis is an important risk that may be encountered in this disease and it is thought to be associated with the pathogenesis of IBD. Deep venous thrombosis and pulmonary emboli are the most common thrombotic complications in IBD patients, however, cerebral vascular involvement is rare. In these patients, although thromboembolic complications are uncommon, they are important because of their high mortality. In this report, the clinical picture with a fatal course of a patient, newly diagnosed with ulcerative colitis, who developed a cerebral venous sinus thrombosis is discussed. [ABSTRACT FROM AUTHOR]

Variants in the myogenesis-regulating glycosidase (MYORG) gene which is known as the first autosomal recessive gene that has been associated with primary familial brain calcification (AR-PFBC). Although adult patients have been reported, no pediatric case has been reported until now. Herein, we review the clinical and radiological features of all AR- PFBC patients with biallelic variants in the MYORG gene who were reported until now, and we report the youngest patient who has a novel homozygous variant. Since the first identification of the MYORG gene in 2018, 74cases of MYORG variants related to AR-PFBC were evaluated. The ages of symptom onset of the patients ranged between 7.5 and 87 years. The most frequent clinical courses were speech impairment, movement disorder and cerebellar signs. All patients showed basal ganglia calcification usually bilaterally with different severities. Conclusion; herein, we reported the first pediatric patient in the literature who had a novel homozygous variant in the MYORG gene with mild clinic findings. [ABSTRACT FROM AUTHOR]

Gallstone-induced ileus is a rare complication of cholelithiasis, and gastric outlet obstruction is even rarer. We describe the multidetector computed tomographic diagnosis of small bowel obstruction resulting from a gallstone impacted in the distal ileum and of gastric outlet obstruction from a gallstone impacted in the pyloric antrum (Bouveret syndrome).

Background/aims: Spontaneous intestinal hematoma is a rare complication of anticoagulant therapy, and small bowel obstruction caused by intramural hematoma secondary to anticoagulant therapy is even rarer. The first symptom is usually abdominal pain, frequently accompanied by nausea and vomiting. A history of anticoagulant use with prolonged international normalized ratios in patients presenting with abdominal pain should alert physicians to search for this entity. Typical findings on abdominal computed tomography yield the diagnosis. Early diagnosis is crucial because most patients are treated non-operatively with a good outcome. Herein, we present the non-enhanced and enhanced computed tomography findings of two cases who were admitted to the emergency clinic with acute abdominal pain and diagnosed as spontaneous intramural ileal hematomas and partial small bowel obstruction secondary to intramural ileal hemorrhage.
