Abstract Background Hepatocellular carcinoma (HCC), a leading cause of cancer-related deaths globally, poses significant challenges in early detection. Improved diagnostic accuracy can drastically influence patient outcomes, emphasizing the need for innovative, non-invasive biomarkers. Methods: This study utilized a cohort of 402 participants, including healthy controls, chronic hepatitis patients, and HCC patients from Bangladesh, to evaluate DNA methylation signatures in peripheral blood mononuclear cells (PBMC). We performed targeted next-generation sequencing on selected genes previously identified to assess their methylation dynamics. The development of M8 and M4 scores was based on these dynamics, using Receiver Operating Characteristic (ROC) analysis to determine their effectiveness in detecting early-stage HCC alongside existing markers such as epiLiver and alpha-fetoprotein (AFP). Results: Integration of M8 and M4 scores with epiLiver and AFP significantly enhances diagnostic sensitivity for early-stage HCC. The M4+epiLiver score achieves a sensitivity of 79.4% in Stage A HCC, while combining M4 with AFP increases sensitivity to 88.2–95.7% across all stages, indicating a superior diagnostic performance compared to each marker used alone. Conclusions: Our study confirms that combining gene methylation profiles with established diagnostic markers substantially improves the sensitivity of detecting early-stage HCC. This integrated diagnostic approach holds promise for advancing non-invasive cancer diagnostics, potentially leading to earlier treatment interventions and improved survival rates for high-risk patients.

Abstract This study presents the findings of an extensive investigation to evaluate the precision and reliability of a non-destructive CAPO-Test in estimating the in situ compressive strength of concrete made from machine-crushed brick aggregates. To estimate the compressive strength of concrete built from brick and stone aggregates for several target strengths at various ages, the CAPO-Test, core test, and standard cylinder test were used. The results of the core test and cylinder test are correlated with the CAPO pullout force, demonstrating a strong relationship between pullout force and compressive strength. The results suggest that the CAPO-Test displays estimated strengths ranging from 5 to 17% of the cylinder strength and 0–6% of the core strength for concrete containing brick chips. The study validates the CAPO-Test's reliability in conducting in-situ concrete strength assessments.

Introduction: Breakthrough infection by SARS-COV-2 virus among vaccinated individuals has been reported from all over the world and it has created a substantial challenge in designing strategies to live with the virus in the post-pandemic era. Factors affecting the extent and nature of breakthrough infection are still not fully understood and those are known to vary depending on host and agent factors. Health Care Workers (HCWs), especially in hospital settings, are front-liners in combating the epidemic and, consequently, they are more vulnerable to breakthrough infection by SARS-COV-2. Like most of the countries of the world, Bangladesh went through several waves of COVID-19 and the last (3rd wave) was the widespread Omicron wave during the winter of 2022. HCWs in Bangladesh have been disproportionately affected by the virus. Under this context, the aim of the present study was to explore breakthrough infection (BTI) and its host-related covariates among HCWs of a COVID-dedicated city-based hospital during the Omicron wave in Bangladesh. Materials and methods: An observational cross-sectional study was conducted on 267 HCWs of the Narayanganj Tertiary (300-bed) hospital during February–March 2022 which coincided with the terminal part of the 3rd wave. Data were collected by trained Field Assistants using Interviewer-administered Data Collection Forms with Questionnaires as instruments. Previous COVID-19 status (any time after the onset of the pandemic and within last 3 months) was explored by the history of specific symptoms as well as by the confirmatory rtPCR test reports from DGHS approved laboratories Anti-nucleocapsid antibody (Anti-N-Ab) in venous blood samples, assayed by a chemiluminescent ELISA technique, was used as a seroprevalence-based marker of breakthrough infection during the preceding few months. Data were analyzed by bivariate as well as multivariate statistics using the IBM-SPSS software. Results: The median age (range) of the HCWs was 38 (21–65) years; Body Mass Index (BMI, kg/m2) 25 (15–49); and Waist-Hip Ratio (WHR) was 0.92 (0.46–1.21). The male subjects had significantly higher median age (p = 0.01) and higher WHR (p = 0.001) as compared to the female subjects. As per the BMI category, subjects with overweight and obesity constituted 83.3 % of the male subjects as compared to 61.6 % of the female subjects (p = 0.001). Objectives: The aim of this study was to investigate whether serum TNF-α and IL-1β levels are independent risk factors for depression in axSpA patients. Methods: All axSpA patients with BASDAI ≥4 were invited consecutively between March 2021 and August 2021 to participate. Depression was evaluated with the WHO-5 Well-Being scale. Disease activity was assessed using BASDAI (0–10), ASDAS-CRP (0.61–7.22), ASDAS-ESR (0.29–7.61), and health status by ASAS-HI (0–17). Serum TNF-α and IL-1β levels were measured by ELISA. An association between depression and cytokine levels was investigated with Spearman’s rank correlation coefficient test. Results: A total of 252 axSpA patients (155 men) could be included; of these, 123 (48.81%) were depressed, and of these, 75 were male. Serum TNF-α and IL-1β were not significantly associated with depression (r −0.041 and 0.110, respectively). Serum TNF-α levels were higher in depressed female axSpA patients (20.05 vs. 17.87; p = 0.03). Differences between depressed and non-depressed patients were respectively: TNF-α (19.7 vs.18.0; p= 0.84), IL-1β (32.3 vs. 21.2; p= 0.04), BASDAI (5.47 vs. 4.77; p = 0.000), ASDAS-CRP (4.17 vs. 3.78; p = 0.000), ASDAS-ESR (3.86 vs. 3.39; p = 0.000), CRP (48.43 vs. 37.93 mg/L; p = 0.000), and ASAS-HI (13.37 vs. 10.24; p = 0.000). Factors associated with depression were: peripheral joint involvement (OR = 1.073, 95% CI 1.012–1.138), BASDAI (OR = 1.534, 95% CI 1.011–2.335), and ASAS-HI (OR = 1.39, 95% CI 1.239–1.557). Only in depressed patients with peripheral SPA were higher IL-1β levels found, though the differences were probably not clinically relevant. Conclusions: Serum TNF-α and IL-1β were not independently related to depression in axSpA patients. Disease activity, peripheral joint involvement, and reduced health status showed the highest association with depression.

Brachyolmia is a heterogeneous group of developmental disorders characterized by a short trunk, short stature, scoliosis, and generalized platyspondyly without significant deformities in the long bones. DASS (Dental Abnormalities and Short Stature), caused by alterations in the LTBP3 gene, was previously considered as a subtype of brachyolmia.The present study investigated three unrelated consanguineous families (A, B, C) with Brachyolmia and DASS from Egypt and Pakistan. In our Egyptian patients, we also observed hearing impairment. Exome sequencing was performed to determine the genetic causes of the diverse clinical conditions in the patients. Exome sequencing identified a novel homozygous splice acceptor site variant (LTBP3:c.3629-1G > T; p. ?) responsible for DASS phenotypes and a known homozygous missense variant (CABP2: c.590T > C; p.Ile197Thr) causing hearing impairment in the Egyptian patients. In addition, two previously reported homozygous frameshift variants (LTBP3:c.132delG; p.Pro45Argfs*25) and (LTBP3:c.2216delG; p.Gly739Alafs*7) were identified in Pakistani patients.This study emphasizes the vital role of LTBP3 in the axial skeleton and tooth morphogenesis and expands the mutational spectrum of LTBP3. We are reporting LTBP3 variants in seven patients of three families, majorly causing brachyolmia with dental and cardiac anomalies. Skeletal assessment documented short webbed neck, broad chest, evidences of mild long bones involvement, short distal phalanges, pes planus and osteopenic bone texture as additional associated findings expanding the clinical phenotype of DASS. The current study reveals that the hearing impairment phenotype in Egyptian patients of family A has a separate transmission mechanism independent of LTBP3.

Objective: Natural substances found in dietary sources and medicinal plants have attracted con¬siderable attention in recent years as chemopreventive agents. Spirulina is a blue-green alga that possesses chemopreventive properties. The purpose of this study was to determine the effect of spirulina on rats with inorganic arsenic (As) [sodium arsenite (NaAsO2)]-induced lipid peroxidation. Materials and Methods: 120 rats were randomly assigned to 10 groups and designated T0, T1, T2, T3, T4, T5, T6, T7, T8, and T9. One group was kept as a control (T0) that received no treatment. The seven groups received 3.0 mg of NaAsO2/kg body weight in drinking water and were given spirulina ad libitum. T1 was treated with NaAsO2 but not with spirulina. Two groups of rats (T2 and T3), on the other hand, were treated with spirulina without receiving any As (NaAsO2). T2 received agro-based spirulina (Ab-Sp; grown in 1.5% soybean meal media and harvested on day 12 of seed inoculation) at 2.0 gm/kg feed, whereas T3 received commercially available spirulina (Com-Sp) at 2.0 gm/kg feed. T4, T5, and T6 were concurrently treated with Ab-Sp at 1.0, 1.5, and 2.0 gm/ kg of feed. On the other hand, T7, T8, and T9 induced by NaAsO2 were concurrently treated with Com-Sp at 1.0, 1.5, and 2.0 gm/kg feed. All groups received treatment for 90 days. Results: The efficacy of both spirulina in preventing lipid peroxidation caused by As was deter¬mined quantitatively by measuring the rats serum malondialdehyde (MDA). The results indicated that As supplementation increased serum MDA levels, whereas both types of spirulina signifi¬cantly decreased them. The highest dose of Ab-Sp (2.0 gm/kg feed) was found to be the most effective in preventing lipid peroxidation in rats treated with inorganic As. Conclusion: Ab-Sp could be a natural, cost-effective, and safe measure to mitigate As toxicity. [J Adv Vet Anim Res 2021; 8(2.000): 330-338]

Determination of accurate operating states of the power system is one of the most challenging tasks due to integration of large number of solar PVs into the power system. Since these sources of energy are inertialess generations, hence may cause instability issues if highly penetrated. Hence, precise monitoring and control of such systems with higher PV penetration is a critical issue to address both in terms of the total number of PV sources in the system and the percentage of penetration. Phasor Measurement Units (PMUs), with their time synchronizing abilities, have made this task a bit easier. However, traditional centralized data handling architecture at control centers are becoming redundant due to various limitations such as data handling, computational constraints etc. To overcome this, a distributed PMU-PDC architecture approach is utilized in this paper. Since local PDC (Phasor Data Concentrator) in an n-area power system can run separate optimization algorithms, by combining the results of these optimization algorithms of n-area PDCs, we can get a much more accurate global consensus about the current operating state of the system. The presented work is divided into two parts. First, the analysis of power system stability is performed in terms of the total number of Photovoltaic Sources (PV) in the system and the percentage of PV penetration. Then, the accuracy of the method is tested both in terms of the number of PMUs in each area along with their channel capacities. Low-frequency oscillations have been simulated on IEEE-68 standard bus system using MATLAB, and the modes of oscillation are estimated using the Alternating Direction Method of Multipliers (ADMM) algorithm.

Abstract Background Spread of malaria and antimalarial resistance through human movement present major threats to current goals to eliminate the disease. Bordering the Greater Mekong Subregion, southeast Bangladesh is a potentially important route of spread to India and beyond, but information on travel patterns in this area are lacking. Methods Using a standardised short survey tool, 2090 patients with malaria were interviewed at 57 study sites in 2015–2016 about their demographics and travel patterns in the preceding 2 months. Results Most travel was in the south of the study region between Cox’s Bazar district (coastal region) to forested areas in Bandarban (31% by days and 45% by nights), forming a source-sink route. Less than 1% of travel reported was between the north and south forested areas of the study area. Farmers (21%) and students (19%) were the top two occupations recorded, with 67 and 47% reporting travel to the forest respectively. Males aged 25–49 years accounted for 43% of cases visiting forests but only 24% of the study population. Children did not travel. Women, forest dwellers and farmers did not travel beyond union boundaries. Military personnel travelled the furthest especially to remote forested areas. Conclusions The approach demonstrated here provides a framework for identifying key traveller groups and their origins and destinations of travel in combination with knowledge of local epidemiology to inform malaria control and elimination efforts. Working with the NMEP, the findings were used to derive a set of policy recommendations to guide targeting of interventions for elimination.

Bovine rotavirus (BRV) is considered the leading cause of calf diarrhea worldwide, including Bangladesh. In this study we aimed to identify risk factors for BRV infection and determine the G and P genotypes of BRV strains in diarrheic calves. Fecal samples were collected from 200 diarrheic calves in three districts between January 2014 and October 2015. These samples were screened to detect the presence of BRV using rapid test-strips BIO K 152 (RTSBK). The RTSBK positive samples were further tested by polyacrylamide gel electrophoresis and the silver staining technique to detect rotavirus dsRNA. Risk factors were identified by multivariable logistic regression analysis. The G and P genotypes of BRV were determined by RT-PCR and sequencing. A phylogenetic tree was constructed based on the neighbor-joining method using CLC sequence viewer 8.0. About 23% of the diarrheic calves were BRV positive. The odds of BRV infection were 3.8- (95% confidence interval [95% CI]: 1.0-14.7) and 3.9-times (95% CI:1.1-14.2) higher in Barisal and Madaripur districts, respectively, than Sirrajganj. The risk of BRV infection was 3.1-times (95% CI: 1.5-6.5) higher in calves aged ≤ 5 weeks than those aged >5 weeks. Moreover, the risk of BRV infection was 2.6-times (95% CI:1.1-5.8) higher in crossbred (Holstein Friesian, Shahiwal) than indigenous calves. G6P[11] was the predominant genotype (94.4%), followed by G10P[11] (5.6%). The BRV G6 strains were found to be closest (98.9-99.9%) to Indian strains, and BRV G10 strains showed 99.9% identities with Indian strain. The VP4 gene of all P[11] strains showed >90% identities to each other and also with Indian strains. The most frequently identified BRV genotype was G6P[11]. About 23% of calf diarrhea cases were associated with BRV. To control disease, high-risk areas and younger crossbred calves should be targeted for surveillance and management. The predominant genotype could be utilized as the future vaccine candidate or vaccines with the dominant genotype should be used to control BRV diarrhea in Bangladesh.

It is crucial to explore knowledge, attitudes and perceptions (KAP) about rabies among the people in the community, the personnel dealing with animal bite management and suspected rabies patients, including humans and animals, to facilitate intervention in improving rabies elimination strategies. In 2016, we conducted an interactive face-to-face survey in three different districts of Bangladesh to understand the extent of KAP towards rabies in the community peoples (CPs), human healthcare professionals (HCPs) and veterinary practitioners (VPs). A set of prescribed questions was employed to measure what proportion of each group possessed sufficient knowledge, positive attitudes and adequate perceptions about rabies. A total of 1133 CPs, 211 HCPs and 168 VPs were interviewed by using a standard questionnaire comprising both closed and open-ended questions. Of the CPs, 49% identified the disease correctly (i.e. rabies is caused by an animal bite or a scratch). Only 29% of the CPs were aware that a wound should be washed immediately with soap and water after an animal bite or a scratch. However, only 49% of the CPs, 65% of the HCPs and 60% of the VPs felt that it is important to consult a physician and receive post-exposure vaccine as the first line of treatment following an animal exposure. Among the HCPs, 23% of the respondents did not possess sufficient knowledge about animal bites as categorised by the World Health Organization (WHO), and 12% of the respondents did not possess the knowledge on how to manage an animal bite properly. Out of 52% of the VPs who previously treated suspected rabid animals, only 29% had a history of taking rabies pre-exposure prophylaxis (PEP). Lack of formal education and rural subsistence were found to largely contribute to poor rabies KAP level among the CPs (P ≤ 0.01). There has been a high demand for proper training to be provided to HCPs and VPs for the effective management of an animal bite incidence in human and animals, respectively. Multi-sectoral collaboration through integrated One Health initiatives including community education, awareness programmes, facilitation of rabies PEP, and dog vaccination as well as its population control are critical in the way forward to control rabies in Bangladesh.

Arsenic (As) is a naturally occurring ubiquitous environmental toxicant. It has been reported that spirulina has protective effects against As toxicity. In the present study, we compared the prophylactic effects of spirulina [laboratory grown agro-based spirulina (Ab-Sp) and market spirulina (M-Sp)] against the histopathological changes in liver induced by inorganic arsenic (iAs) in male rats. Three doses (1.0g, 1.5g and 2.0g/kg feed) of both the Ab-Sp and M-Sp with feed and 3.0mg NaAsO2/kg body weight (BW) in drinking water were given simultaneously to six groups (T4, T5, T6, T7, T8 and T9) of rats daily for 90 days. Same dose of NaAsO2 (3.0mg/kg; T1) and highest dose (2.0g/kg) of each of Ab-Sp (T2) and M-Sp (T3) were given individually to other 3 groups keeping the rest one as control (T0) with normal feed and water. As feeding resulted in a variety of histopathological changes in liver, including congestion in the central veins, hemorrhage in the hepatic lobules and lobular tissues, higher numbers of hypertrophic hepatocytes with hypertrophic nucleus, hepatocytes with visible chromatin in the nucleus and vacuolated hepatocytes. The Ab-Sp treatment successfully improved all the histopathological conditions. In contrast, the M-Sp improved the conditions by combating all the histopathological conditions including vacuolated hepatocytes, erosions and hemorrhages in the liver. Taken together, the spirulina was found as an effective agent in prevention of the histopathological changes while we first clarified that Ab-Sp had better result than the M-Sp and finally, 2.0g Ab-Sp/kg feed was found as the best dose. In search of an effective biological control agent against the tomato pathogen Fusarium oxysporum f. sp. lycopersici, rhizospheric soil samples were collected from eight agro-ecological zones of Bangladesh. Among the bacteria isolated from soil, 24 isolates were randomly selected and evaluated for their antagonistic activity against F. oxysporum f. sp. lycopersici. The two promising antagonistic isolates were identified as Brevundimonas olei and Bacillus methylotrophicus based on morphological, biochemical and molecular characteristics. These two isolates were evaluated for their biocontrol activity and growth promotion of two tomato cultivars (cv. Pusa Rubi and Ratan) for two consecutive years. Treatment of Pusa Rubi and Ratan seeds with B. olei prior to inoculation of pathogen caused 44.99% and 41.91% disease inhibition respectively compared to the untreated but pathogen-inoculated control plants. However, treatment of Pusa Rubi and Ratan seeds with B. methylotrophicus caused 24.99% and 39.20% disease inhibition respectively. Furthermore, both the isolates enhanced the growth of tomato plants. The study revealed that these indigenous bacterial isolates can be used as an effective biocontrol agent against Fusarium wilt of tomato.

COVID-19 pandemic is now a great headache for the world population. Respiratory symptoms are the main presentation of COVID-19. However, musculoskeletal pain, headache, loss of taste and smell sense, and neurological manifestations may occur. Identification of patterns of musculoskeletal pain, fatigue and physical health status in COVID-19 is crucial. In this cross sectional study, a total 380 individuals with COVID-19 were recruited from the population following selection criteria. Pain varied widely in hip, neck, leg and calf muscles, back and spine, shoulder, arms and hand, and other parts of the body among the respondents. Inconstant, among respondents of younger age group (aged ≤50 year), 37.59% had moderate pain, 6.77% had severe pain, 13.91% had mild pain, and 41.17% had no pain. On the other hand, among older respondents (aged >50 year) 47.37% had moderate pain, 25.44% had severe pain, 13.15% had mild pain, and 14.03% had no pain. A 37-year-old Bangladeshi woman presented with low back and several joints pain and swelling for months together; there was significant morning stiffness for more than two hours. Repeated abortions, dry eye, hair fall, photosensitivity, and oral ulcer were the additional complaints. Clinical examination unveiled asymmetrical peripheral and both sacroiliac joint tenderness, positive modified Schober’s test, and limited chest expansion. Schirmer’s test was positive. The history of rheumatoid arthritis (RA) and ankylosing spondylitis (AS) among 1st-degree relatives was also significant. Biochemical analysis revealed pancytopenia, raised erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP), and mild microscopic proteinuria. The patient was seropositive for rheumatoid factor (RF), antibodies against cyclic citrullinated peptides (anti-CCP), antinuclear antibody (ANA), anti-Sm antibody, anti-Sjögren’s-syndrome-related antigen A and B (anti-SSA/SSB), antiphospholipid (aPL-IgG/IgM), and HLA B27; however, serum complement (C3 and C4) levels were normal. Basal cortisol level measured elevated. Besides, X-ray and MRI of lumbosacral spines demonstrated sacroiliitis. There was radiological cardiomegaly, echocardiography unveiled atrial regurgitation, and ascending aorta aneurysm. Based on the abovementioned information, RA, AS, and systemic lupus erythematosus (SLE) have been diagnosed. Moreover, the patient developed Sjogren’s syndrome (SS), antiphospholipid lipid syndrome (APS), Cushing syndrome, ascending aorta aneurysm, and atrial regurgitation. Her disease activity score for RA (DAS28), DAS for AS (ASDAS), SLE disease activity index (SLEDAI), and Systemic Lupus International Collaborating Clinics/American College of Rheumatology (SLICC/ACR) scores were 3.46, 2.36, 23, and 5, respectively. The patient received hydroxychloroquine (200 mg daily), pulsed cyclophosphamide, prednisolone (20 mg in the morning), and naproxen 500 mg (twice daily). To our best knowledge, this is the first report documenting RA, AS, and SLE with secondary SS and APS.

Identification of risk factors is crucial to find ways to reduce rabies deaths. We investigated the hospital records of rabies deceased through contact tracing of the relatives of the victims using enhanced verbal autopsies (VA) to identify why the people had to die from rabies in recent years in Bangladesh. Patients whose deaths were confirmed by physicians based on the history of animal exposure and clinical signs were taken into account for VA. Socio-demographic profile of the deceased, animal exposure, nature of the wound, and history of post-exposure prophylaxis (PEP) data were obtained and analysed. The study found 256 cases in which the cause of death was attributed to rabies, most of the victims were male (71.88%), resided in the rural community (80.47%), dependent (49.22%), and children below the age of 15 years (47.27%). Dogs were the single most responsible (81.64%); however, cats (12.11%), jackals (3.91%) and mongoose (2.34%) were also found accountable for rabies incidence (P < 0.05). Significantly, limbs were the most common (67.97%) site of exposure, and the shortest incubation period was identified in the case of bites to head and face (P < 0.05). The majority (86.72%) of the deceased did not receive any PEP; whereas, 66.80% sought treatment from traditional healers. Among the deceased (13.28%, n = 34) who had received PEP, only 8.82% of them completed the full course of the vaccination regimen (P < 0.01); however, none of them had history of taking rabies immunoglobulin (RIG). The study recommends extensive public health awareness programs in the rural community and establishing methods to improve healthcare-seeking behaviours, including receiving PEP instead of visiting traditional healers. Moreover, the availability and accessibility of PEP in the government hospital facilities are desirable, and laboratory-based surveillance with compatible rapid data reporting may be incorporated in the existing policy.

The present study was designed to see the level of awareness and practice of computer-related ergonomics among resident doctors. A total of 227 residents participated in this study who had used a computer for at least a year. They were selected by simple random sampling. A self-reporting questionnaire was used to conduct the study. For better understanding and convenience, the responses were categorized into good, fair, and poor. Awareness level was good in only 15% (n= 34) of residents, fair in 16.7% and the majority 68.3% (n= 155) of residents had a poor level of awareness about ergonomics.

Objective: Until recently, management of repeat breeding in cattle remains a major problem to dairy farmers. This study was carried out to improve the fertility in cows, those did not conceive after three or more consecutive services. Materials and methods: Twenty-three repeat breeding cows were selected from the Central Cattle Breeding Station and Dairy Farm (CCBS & DF), Savar, Dhaka, Bangladesh. Data of another 23 repeat breeding cows that conceived after four or more services were collected from the same farm as controls. The repeat breeders were synchronized for estrus with two injections of PGF2α 11 days apart and timed AI (TAI) was performed. Results: Seventeen of 23 cows had uterine infections. Intrauterine infusion of penicillin daily for three successive days from the following day of the first PGF2α injection resulted in 94.1% (16/17) recovery. Of 23 treated cows, four with estrus synchronization and TAI, and one with only AI after hormone treatment were detected pregnant by per rectal examination of the genital tract. By this pregnancy, the treated cows have advanced average of 131.6 days calving interval and days open, and thereby saved neat US $3,045.3 in comparison to control cows. Control group cows incurred losses of US $15,134.0 compared with average days open of the treated pregnant cows. Conclusion: Intrauterine infusion of penicillin can successfully recover the uterine infections, and estrus synchronization followed by TAI resulted in pregnancy in a proportion of repeat breeder cows with economic benefit that had a uterine infection. For countries aiming for malaria elimination, travel of infected individuals between endemic areas undermines local interventions. Quantifying parasite importation has therefore become a priority for national control programs. We analyzed epidemiological surveillance data, travel surveys, parasite genetic data, and anonymized mobile phone data to measure the spatial spread of malaria parasites in southeast Bangladesh. We developed a genetic mixing index to estimate the likelihood of samples being local or imported from parasite genetic data and inferred the direction and intensity of parasite flow between locations using an epidemiological model integrating the travel survey and mobile phone calling data. Our approach indicates that, contrary to dogma, frequent mixing occurs in low transmission regions in the southwest, and elimination will require interventions in addition to reducing imported infections from forested regions. Unlike risk maps generated from clinical case counts alone, therefore, our approach distinguishes areas of frequent importation as well as high transmission.

Background: The assessment of antibody responses to severe acute respiratory syndrome coronavirus-2 is potentially confounded by exposures to flaviviruses. The aims of the present research were to determine whether anti-dengue antibodies affect the viral load and the detection of anti-coronavirus nucleocapsid (N)-protein antibodies in coronavirus infectious disease 2019 (COVID-19) in Bangladesh. Methods: Viral RNA was evaluated in swab specimens from 115 COVID-19 patients by real-time reverse transcription polymerase chain reaction (rT-PCR). The anti-N-protein antibodies, anti-dengue virus E-protein antibodies and the dengue non-structural protein-1 were determined in serum from 115 COVID-19 patients, 30 acute dengue fever pre-COVID-19 pandemic and nine normal controls by ELISA. Results: The concentrations of viral RNA in the nasopharyngeal; Ct median (95% CI); 22 (21.9–23.3) was significantly higher than viral RNA concentrations in oropharyngeal swabs; and 29 (27–30.5) p < 0.0001. Viral RNA concentrations were not correlated with-dengue IgG levels. The anti-nucleocapsid antibodies were IgA 27% positive and IgG 35% positive at days 1 to 8 post-onset of COVID-19 symptoms versus IgA 0% and IgG 0% in dengue patients, p < 0.0001. The levels of anti- nucleocapsid IgA or IgG versus the levels of anti-dengue IgM or IgG revealed no significant correlations. Conclusions: Viral RNA and anti-nucleocapsid antibodies were detected in COVID-19 patients from dengue-endemic regions of Bangladesh, independently of the dengue IgG levels.

Background: Definitive diagnosis of visceral leishmaniasis (VL) by demonstrating parasites in tissue smears or by culture involves invasive procedures, technical expertise and adequate laboratory facilities. Endemic countries rely mainly on serological tests to diagnose VL. Currently, the immunochromatographic test incorporating the recombinant K39 antigen (rK39 ICT) is the reference test for rapid diagnosis of VL in the Indian subcontinent. The performance of serological tests using rK39 and other promastigote antigens can vary due to differences in antigen expression, the various hosts and environmental factors. To achieve elimination of VL, diagnostic accuracy will be necessary for active case detection especially in those who carry asymptomatic infections. We evaluated the performance of rK39 ICT, enzyme linked immunosorbent assay using mixed Leishmania promastigotes from different Leishmania species (p-ELISA) and indirect fluorescent antibody test (IFAT) utilizing whole promastigotes from the Leishmania donovani complex for sero-diagnosis of VL in Bangladesh. Methods: The sensitivity of each serological test was evaluated on 155 patients who were diagnosed to have VL by microscopy and/or by culture methods. Test specificities were calculated on 706 healthy blood donors, 91 diagnostic sera from patients with a febrile illness and sera from patients positive for malaria (n = 91) and Chagas disease (n = 91). All statistical calculations were at 95% confidence intervals. Results: The sensitivities of rK39 ICT, p-ELISA and IFAT were 100%, 86.5% and 92.3%, respectively. All three serological methods had a pooled sensitivity of 82.6%. The specificities of rK39 ICT, p-ELISA and IFAT from combined control groups were 100%, 93.1% and 99.9%, respectively. The respective positive and negative predictive values of the tests were both 100% for rK39 ICT, 66.3% and 97.8% for p-ELISA and 99.3% and 98.8% for IFAT. The p-ELISA showed cross reactivity with 36.3% of sera positive for malaria and 28.6% of sera positive for Chagas disease while rK39 ICT and IFAT showed no cross reactivity. Conclusion: This study confirms the efficiency of rK39 ICT for rapid diagnosis of VL. The p-ELISA using mixed promastigote antigens did not perform well as a serological test for VL in Bangladesh. Due to high sensitivity and specificity of whole promastigote antigen of L. donovani complex utilized in IFAT, this test can be considered in combination with rK39 ICT to confirm VL diagnosis when clinical diagnosis cannot distinguish between other diseases.

The escalating dengue situation in Bangladesh has been emerging as a serious public health problem in terms of morbidity and mortality. Results of analysis of 40,476 cases of Bangladesh occurring during 2000–2017 indicated that 49.73% of the dengue cases occurred during the monsoon season (May–August) and 49.22% during the post-monsoon season (September–December). However, data also showed that, since 2014, these trends have been changing, and dengue cases have been reported during the pre-monsoon season. During 2015–2017, in the pre-monsoon season, the dengue cases were reported to be more than seven times higher compared to the previous 14 years. The findings closely correlate with those of the pre-monsoon Aedes vector survey which revealed the presence of high density of larva and pupa of the dengue vectors in the environment all the year round. In our study, climate changes, such as average rainfall, humidity, and temperature, after 2014, and rapid unplanned urbanization were the strong predictors of an imbalance in the existing ecology that has led to increase in dengue cases in 2016 and the emergence of the chikungunya virus for the first time in Bangladesh in 2017. Although 2018 dengue data are relevant but not included in this study due to study time frame, it is interesting to report an increase in the number of dengue cases in pre (2016) and post (2018, which is highest within 18 years) chikungunya outbreak, which favors the study hypothesis. Despite the efforts to control dengue, based primarily on the vector control and case management, the burden and costs of the disease and similar vector-borne diseases will continue to grow in future in our country. Developing a cost-effective vaccine against all the 4 strains of dengue remains a challenge. The CDC, in collaboration with other research organizations, may come forward to initiate and coordinate a large-scale randomized clinical trial of an effective dengue vaccine in Bangladesh.

Respiratory tract infections are prevalent among Hajj pilgrims with pneumonia being a leading cause of hospitalization. Streptococcus pneumoniae is a common pathogen isolated from patients with pneumonia and respiratory tract infections during Hajj. There is a significant burden of pneumococcal disease in India, which can be prevented. Guidelines for preventive measures and adult immunization have been published in India, but the implementation of the guidelines is low. Data from Bangladesh are available about significant mortality due to respiratory infections; however, literature regarding guidelines for adult immunization is limited. There is a need for extensive awareness programs across India and Bangladesh. Hence, there was a general consensus about the necessity for a rapid and urgent implementation of measures to prevent respiratory infections in pilgrims traveling to Hajj. About ten countries have developed recommendations for pneumococcal vaccination in Hajj pilgrims: France, the USA, Kuwait, Qatar, Bahrain, the UAE (Dubai Health Authority), Singapore, Malaysia, Egypt, and Indonesia. At any given point whether it is Hajj or Umrah, more than a million people are present in the holy places of Mecca and Madina. Therefore, the preventive measures taken for Hajj apply for Umrah as well. This document puts forward the consensus recommendations by a group of twenty doctors following a closed-door discussion based on the scientific evidence available for India and Bangladesh regarding the prevention of respiratory tract infections in Hajj pilgrims.

In order to explore novel agents for the treatment of PD, in the current study, we have evaluated the neuroprotective efficacy of ferulic acid (FA) using rotenone (ROT)-induced rat model of PD. ROT was administered 2.5 mg/kg body weight to male Wistar rats for 4 weeks to induce the PD. Since PD is progressive and chronic in nature, the paradigm for evaluating FA was based on chronic administration for 4 weeks at the dose of 50 mg/kg, 30 minutes prior to ROT administration. ROT administration caused significant reduction in endogenous antioxidants such as superoxide dismutase, catalase, and glutathione. ROT challenge-induced lipid peroxidation evidenced by increased malondialdehyde following perturbation of antioxidant defense. Apart from oxidative stress, ROT also activated proinflammatory cytokines and enhanced inflammatory mediators such as cyclooxygenase-2 and inducible nitric oxide synthase. The immunofluorescence analysis revealed a significant increase in the number of activated microglia and astrocytes accompanied by a significant loss of dopamine (DA) neurons in the substantia nigra pars compacta area upon ROT injection. However, treatment with FA rescued DA neurons in substantia nigra pars compacta area and nerve terminals in the striatum from the ROT insult. FA treatment also restored antioxidant enzymes, prevented depletion of glutathione, and inhibited lipid peroxidation. Following treatment with FA, the inflammatory mediators such as cyclooxygenase-2 and inducible nitric oxide synthase and proinflammatory cytokines were also reduced. Further, the results were supported by a remarkable reduction of Iba-1 and GFAP hyperactivity clearly suggests attenuation of microglial and astrocytic activation. Results of our study suggest that FA has promising neuroprotective effect against degenerative changes in PD, and the protective effects are mediated through its antioxidant and anti-inflammatory properties. BACKGROUND:Bangladesh had one of the highest burdens of lymphatic filariasis (LF) at the start of the Global Programme to Eliminate Lymphatic Filariasis (GPELF) with an estimated 70 million people at risk of infection across 34 districts. In total 19 districts required mass drug administration (MDA) to interrupt transmission, and 15 districts were considered low endemic. Since 2001, the National LF Programme has implemented MDA, reduced prevalence, and been able to scale up the WHO standard Transmission Assessment Survey (TAS) across all endemic districts as part of its endgame surveillance strategy. This paper presents TAS results, highlighting the momentous geographical reduction in risk of LF and its contribution to the global elimination target of 2020. METHODOLOGY/PRINCIPAL FINDINGS:The TAS assessed primary school children for the presence of LF antigenaemia in each district (known as an evaluation unit-EU), using a defined critical cut-off threshold (or 'pass') that indicates interruption of transmission. Since 2011, a total of 59 TAS have been conducted in 26 EUs across the 19 endemic MDA districts (99,148 students tested from 1,801 schools), and 22 TAS in the 15 low endemic non-MDA districts (36,932 students tested from 663 schools). All endemic MDA districts passed TAS, except in Rangpur which required two further rounds of MDA. In total 112 students (male n = 59; female n = 53), predominately from the northern region of the country were found to be antigenaemia positive, indicating a recent or current infection. However, the distribution was geographically sparse, with only two small focal areas showing potential evidence of persistent transmission. CONCLUSIONS/SIGNIFICANCE:This is the largest scale up of TAS surveillance activities reported in any of the 73 LF endemic countries in the world. Bangladesh is now considered to have very low or no risk of LF infection after 15 years of programmatic activities, and is on track to meet elimination targets. However, it will be essential that the LF Programme continues to develop and maintain a comprehensive surveillance strategy that is integrated into the health infrastructure and ongoing programmes to ensure cost-effectiveness and sustainability.

This study was carried out to see whether caregiver training to attendees during hospital stay can better address the stroke survivor at home. In total 119 admitted patients hailed from remote villages were randomly selected from a private neurology hospital in Dhaka and were divided into two groups. One group had caregiver training and another group without training. All the patients were assessed for Barthel index at discharge and after 2 months. Barthel index in Group A during discharge was 6.2 ± 4.0 and at follow-up was 56.4 ± 9.0. The index in Group B was 4.9 ± 4 during discharge and 28.2 ± 7.8 at follow-up. Results were statistically significant. In conclusion, this study reveals the positive effects of caregiver training in the post-acute care management of stroke survivors at home.

Background. Rosai-Dorfman disease (RDD) is a benign histiocytic proliferative disorder of unknown etiology. This rare condition commonly causes massive cervical lymphadenopathy. Intracranial RDD without any nodal involvement is extremely rare. Case Report. A young Bangladeshi male complained of bilateral complete blindness with left sided deafness for about three years. There was no lymphadenopathy. MRI and CT scan of brain suggested an inflammatory/neoplastic (?meningioma) lesion located at left parasellar region which extended frontally to encircle both optic nerves and also to left prepontine area. Histopathologically the lesion was diagnosed as RDD. The patient was treated with steroid and significant clinical improvement observed. Conclusion. The prognosis of intracranial RDD is not poor. It can be treated with surgery with or without corticosteroids, chemotherapy, and so forth. But as the condition is extremely rare and often misdiagnosed, the clinician, radiologist, and histopathologist should have a suspicion in their mind about the possibility of RDD.

Mediastinal lipoma (ML) is a rare entity. Though the mediastinum is the most common site of intrathoracic lipoma, ML constitutes less than 1% of all mediastinal tumours. ML frequently presents on incidental radiographic finding, CT scan is considered the investigation of choice. CT features of lipoma are quite characteristic. They are clinically significant because: (1) Despite their benign nature, these tumours tend to reach an enormous size and can cause compression of lungs and mediastinal structures; (2) It may not always be possible to differentiate a ML from a liposarcoma by CT or MRI alone. Aim: Aim of this study was to determine the seroprevalence of Salmonella along with pathological investigation in poultry. Materials and Methods: A total of 200 birds were selected randomly for collection of serum for serum plate agglutination test and 28 samples were collected from suspected birds for gross and histopathological study. Serum plate agglutination test was performed for detection of seroprevalence and routine histopathological method was used. Results: Out of 200 samples, 76 birds were Salmonellapositive giving the overall seroprevalence of 38.0%. Various macroscopically and microscopically pathologic lesions observed especially in the liver (haemorrhage, necrotic foci, heterophil infiltration) and intestine (goblet cell proliferation). Conclusion: Further studies are need for the determination of the presence of Salmonella infection in farms of Bangladesh.

OBJECTIVE: To evaluate the effect of pre-operative pain on the success of pulpotomy in primary teeth and to assess whether formocresol (FC) or MTA performs better in patients with pre-operative pain. METHODOLOGY: Data was collected retrospectively from the records of Department of Pediatric Dentistry, Children Hospital, Islamabad. Only patients with complete pre-operative and one-year follow-up clinical and radiographic records were included. A total of 60 teeth were selected for the study on which pulpotomy was performed. Among the selected teeth, thirty teeth were symptomatic at time of treatment and thirty were asymptomatic. Fifteen teeth in each group were treated with FC and fifteen with MTA as pulpotomy medicament. RESULTS: After one-year, the clinical and radiographic success of pulpotomized teeth with positive history of pain was found to be 70% and 53.3% respectively. Among the teeth with positive pre-operative pain, 60% of the teeth treated with FC and 80% of the teeth treated with MTA showed clinical success. The radiographic success rate was found to be 33.3% and 73.3% for FC and MTA respectively. CONCLUSION: The results from the current study suggest the presence of pre-operative pain negatively affects the success of pulpotomy in primary molars and could be used as an important clinical indicator to govern the choice of pulpotomy agent in clinical practice. Mutations in ATP13A2 (PARK9), an autophagy-related protein, cause Kufor–Rakeb syndrome, an autosomal recessive, juvenile-onset form of parkinsonism. α-Synuclein (α-syn) is a presynaptic neuronal protein that forms toxic aggregates in Parkinson's disease (PD). We studied α-syn aggregation and autophagic flux in ATP13A2-knockdown Drosophila expressing either wild-type (WT) or mutant α-syn. Dopaminergic (DA) neuron loss was studied by confocal microscopy. Sleep and circadian activity were evaluated in young and old flies using a Drosophila activity monitor. Thirty-day-old ATP13A2-RNAi A53T-α-syn flies had increased Triton-insoluble α-syn levels, compared to control A53T-α-syn flies without ATP13A2-RNAi. Whole-brain staining revealed significantly fewer dopaminergic (DA) neurons in the PPL2 cluster of 30-day-old ATP13A2-RNAi flies expressing WT-, A30P-, and A53T-α-syn than in that of controls. In ATP13A2-RNAi A53T-α-syn flies, autophagic flux was decreased, as indicated by increased accumulation of Ref(2)P, the Drosophila p62 homologue. ATP13A2 silencing decreased total locomotor activity in young, and enhanced sleep features, similar to PD (decreasing bout length), in old flies expressing A53T-α-syn. ATP13A2 silencing also altered the circadian locomotor activity of A30P- and A53T-α-syn flies. Thus, ATP13A2 may play a role in the autophagic degradation of A53T-α-syn. [ABSTRACT FROM AUTHOR]

Myofibroma is a rare benign spindle cell neoplasm which occurs predominantly in infants and young children. It can occur as a solitary mass or as a multicentric lesion, consisting of myofibroblasts. In oral cavity, the sites most commonly involved are the mandible, tongue, lips and buccal mucosa however, it rarely involves the maxilla. It has aggressive clinical presentation mimicking malignancies. We report a case of a male child seven years of age presented with intraoral swelling on the left palatal aspect of the maxilla that extended from deciduous upper left second molar to the distal of upper permanent left first molar causing the mobility of these teeth. Radiologically, the lesion was osteolytic causing marked resorption of alveolar bone and displacement of permanent maxillary left second molar. A differential diagnosis of peripheral giant cell granuloma, central giant cell granuloma, fibroma, aggressive fibromatosis, nodular fasciitis, peripheral ossifying fibroma and spindle cell neoplasm was made. After the excluding malignant and vascular involvement, the surgical excision of the lesion was planned and was carried out under general anesthesia. Histologically, benign proliferation of spindle cells was seen giving the diagnosis of benign spindle cell neoplasm. Reports also showed positive immunoreactivity with vimentin and aSMA while non-responsive to desmin, validates the diagnosis of myofibroma. [ABSTRACT FROM AUTHOR]

Objectives: This study aimed to compare the efficacy of low‐dose prednisolone with conventional high‐dose regimen in proliferative lupus nephritis (LN) for remission. Methods: This open‐label randomized clinical trial was conducted in the Department of Rheumatology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. A total of 32 LN patients were randomized into low‐dose (experimental) and high‐dose (control) groups. All subjects received standard dose of intravenous (I/V) methylprednisolone and pulse I/V cyclophosphamide. Oral prednisolone, 0.5 mg/kg/d and 1 mg/kg/d were given to experimental and control groups respectively for initially 4 weeks then tapered. The patients were followed for 24 weeks. The rates of renal remission (complete and partial) were assessed at 24 weeks. The disease activity, biochemical markers, and quality of life were evaluated at baseline and at 24 weeks. Results: Complete renal remission was achieved by 66.7% of patients in each group (P =.99). Renal remission (partial/complete) was achieved by 86.7% and 83.3% of patients in the prednisolone low‐dose group and high‐dose group respectively (P =.99). In between groups, no significant difference was observed in the improvement of active urinary sediments, serum creatinine level, anti‐double‐stranded DNA level, complements level, disease activity and Short Form‐12 score. The prednisolone dose‐related adverse events like cushingoid facies, abdominal stria, infections and serious adverse events like death occurred more in the high‐dose prednisolone group. Conclusions: It has been observed that low‐dose prednisolone regimen may be effective in LN. Steroid dose‐related side effects and rate of infections were lower in this group. [ABSTRACT FROM AUTHOR]

OBJECTIVE: Pediatric patients have high unmet treatment needs in Pakistan. There are only a handful of pediatric dentists in Pakistan. Thus, the dental services to child patients have to be dependent on general dentists and other dental specialists. The aim of this study was to assess the attitude and practice of the dentists towards clinical pediatric dentistry in Pakistan. METHODOLOGY: A cross-sectional, questionnaire-based study was conducted among the dentists in Pakistan. RESULTS: A total of 372 dentists participated in the study. These included 122 (32.8%) males and 250 (67.2%) females. The majority of the participants (n = 272, 73.1%) were between the ages of 25 to 35 years. As compared to other dentists, a significantly greater number of general dentists (p < 0.008) had received any training in the behavioral management of children. Operative dentists reported practicing more of the various procedures in child patients (pit and fissure sealants, stainless steel crowns, pulpectomies, fluoride varnishes, pulpotomies and restorations), as compared to the other dentists. Even though the operative dentists performed more of these procedures, more than 50% of them still did not frequently perform any of the procedures. A total of 244 (65.6%) dentists wanted to attend pediatric dentistry courses, if available. CONCLUSION: There is a major gap in the needs and availability of pediatric dentistry services. More training needs to be conducted at both the undergraduate and postgraduate levels in pediatric dentistry. [ABSTRACT FROM AUTHOR]
