Background: The prognostic value of the neutrophil–lymphocyte ratio (NLR) in ischemic stroke remains debated due to cohort variability and treatment heterogeneity across studies. This study evaluates the relationship between admission NLR, stroke severity and 90-day outcomes in patients with anterior circulation large vessel occlusion (LVO) undergoing early, successful revascularization. Methods: A retrospective multicenter study was conducted with 1082 patients treated with mechanical thrombectomy for acute ischemic stroke. The relationship between admission NLR, baseline National Institutes of Health Stroke Scale (NIHSS), 24 h NIHSS and 90-day modified Rankin Scale (mRS) outcomes was analyzed using logistic regression. Results: Admission NLR correlated weakly but significantly with both baseline (p = 0.018) and 24 h (p = 0.005) NIHSS scores, reflecting stroke severity. However, multivariate analysis showed that higher 24 h NIHSS scores (OR 0.831, p = 0.000) and prolonged puncture-to-recanalization times (OR 0.981, p = 0.000) were independent predictors of poor 90-day outcomes, whereas NLR was not (p = 0.557). Conclusions: Admission NLR is associated with stroke severity but does not independently predict clinical outcomes at 90 days in patients achieving early and successful revascularization. These findings underscore the critical role of inflammation in the acute phase of stroke but suggest that its prognostic value for long-term outcomes is limited in this context. Our study aims to report the findings of transesophageal echocardiography (TEE), Risk of Paradoxical Embolism (RoPE) score, the Patent Foramen Ovale-Associated Stroke Causal Likelihood (PASCAL) classification analyses, and follow-up results after percutaneous PFO closure in patients diagnosed with cryptogenic stroke. The medical records of patients with acute arterial ischemic stroke who were diagnosed with PFO by TEE and applied percutaneous PFO closure between 2020-2023 were reviewed. The echocardiography (Echo) score, the RoPE score, and the PASCAL classification were calculated for all patients as indicated in the literature. Newly developed arrhythmia, recurrent stroke, and cardiovascular death were recorded during follow-up. A total of 65 patients were included in the study. The median age was 41 years [range:33 -47]), the median RoPE score was 8, and the Echo score was 3. While 29.2% of the patients were in the possible group according to the PASCAL classification, 70.8% were in the probable group. We did not have any patients in the unlikely group. Recurrent ischemic stroke was detected in only two patients at a median follow-up of 12.1 months. No cardiac arrhythmia or cardiovascular death was observed. PFO closure in PFO-associated stroke patients was safe, with a low incidence of arrhythmia and cardiovascular and cerebrovascular events during the short -term follow-up. [ABSTRACT FROM AUTHOR]

Moyamoya disease is a chronic, progressive hereditary disease characterized by narrowing of the vascular lumen because of hypertrophy of smooth muscles in the walls of the arteries that form the circle of Willis. Cerebral vessels may be encountered in bleeding and occlusion clinics. Although it is seen as predominant in Asian races, cases have been reported worldwide. Although its etiopathogenesis is not clear, genetics, some infectious agents, and autoimmune mechanisms are blamed. The gold standard in diagnosis is digital subtraction angiography. Surgical or endovascular interventions can be used in patients with recurrent progressive ischemic events and decreased cerebral perfusion. Objectives: Aim of the present study was to determine prevalence of Stevens-Johnson syndrome (SJS) in Aydın province epilepsy clinic patients due to antiepileptic drug use. Methods: Records of 2112 adult epileptic patients treated at outpatient clinics of Adnan Menderes University hospital departments of neurology, pediatric neurology, and dermatology were studied retrospectively. Results: Two of 2112 epileptic patients who had used lamotrigine had history of SJS. Conclusion: Prevalence of SJS due to antiepileptic drug therapy was 0.021% in Aydın province epilepsy clinic patients. [ABSTRACT FROM AUTHOR]

Rhinocerebral mucormycosis is a rare fungal infection which is capable of causing a fatal outcome and commonly affects diabetic and immunocompromised patients. Early symptoms are fever, nasal ulcerations or necrosis, periorbital or facial edema, and decreased vision. In the literature, there are only a few cases of rhinocerebral mucormycosis which presented with involvement of all cranial nerves unilaterally, known as Garcin syndrome. In this report, we presented a 65-year-old immunosupressed and diabetic female with rhinocerebral mucormycosis and left Garcin syndrome. It should not be forgotten that rhinocerebral mucormycosis can present with unilateral palsies of all cranial nerves and further investigations should be considered in suspected cases because of the life-threatening character of this infection. Introduction: Depression and dementia have been recognized as a common problem in PD. However there are debatable results about their nature and frequency of occurrence. Their effects on the PD clinic remain controversial. The aim of our study was to assess the frequency, risk factors, and interrelations between depression and dementia in PD patients. Materials and Method: We evaluated retrospectively 240 idiopathic PD patients who responded well to L-dopa treatment. Severity of disease was evaluated by using UPDRS, HY and WS. HAM-D, MMSE and a socio-demographic questionnaire were also performed on all patients. Results: Our patients (105 females, 135 males) were between 30 and 88 years of age (68.56±10.3). Depression was revealed in 43.6%, and dementia in 33.3%. Patients with demen-tia were older and had higher depression frequency and WS and UPDRS scores. Depressive patients had higher HY and WS scores also. Positive correlations were detected between disease duration and WS, UPDRS, HY. HAM-D had positive correlations with HY and UPDRS as well. Conclusion: Depression and dementia were correlated with the severity of PD disability. Therefore, regular mood and cognitive state screening and appropriate treatment should be con-sidered, especially in older PD patients with higher disease severity. [ABSTRACT FROM AUTHOR]

Catatonia is a syndrome characterized by mutism, immobility, negativism, stereotypy, mannerisms, echophenomena, perseveration and passive obedience. The underlying causes can be psychiatric or may be associated with general medical status or neurological diseases. Additionally catatonia has two subtypes named as malignant and nonmalignant catatonia. Main symptoms of malignant catatonia are hyperthermia and autonomic symptoms such as tachycardia, tachypnea and hyperhidrosis. According to International Classification of Headache Disorders (ICHD-III Beta version) headache attributed to hypoxia and/or hypercapnia is classified under the 4 title: 10.1.1 High altitude headache, 10.1.2 Airplane travel associated headache, 10.1.3 Diving headache and 10.1.4 Sleep apnoea headache. Headache associated with airplane travel is encountered infrequently in our clinical practice and firstly reported in 2004 as a case in the literature. The pathophysiology of headache associated with airplane travel is not yet clear. We presented this case in the aspect of the patient having both airplane travel and high altitude headaches and seen giant Virchow-Robin spaces in cranial MRI and disappearence of pain with a preventive treatment.
