Aim: This article is a report of a study protocol designed to explore what factors inform healthcare professionals' clinical decision‐making when providing end of life care for people with dementia in primary care. Design: The proposed study will adopt a qualitative study design, utilizing an ethnographic approach. Methods: A mixed sample group of registered healthcare professionals, including district nurses, specialist nurses and general practitioners, will be purposively sampled and recruited from one health board in Wales, which will cover three settings in primary care. A multi‐method approach will be utilized to provide rich data, including non‐participant observation, semi‐structured interviews, and review of key documents. Data will be managed using NVivo 12 and analysed thematically. Ethics approval was gained in April 2022. Discussion: Little is known about the end of life care decision‐making process of healthcare professionals caring for people with dementia in primary care settings. This study will therefore address this pertinent gap. The study findings may also be transferable to healthcare professionals working in similar clinical settings across the UK. Impact It is anticipated that this study will inform the subsequent development of an intervention that can be used by healthcare professionals to support decision making and subsequently the provision of quality end of life care in primary care for people with dementia. In principle, it is possible for a couple to get married in a register office in England or Wales for £127 (including the cost of giving notice and a certificate). In this article, we draw on empirical research to show how limited this option is in practice. Its availability is constrained by the scarcity of register offices limited slots for weddings, and the addition of other fees not provided for in the regulations. Its accessibility is often not obvious from local authorities' websites, and the administration of such a wedding varies hugely, with some local authorities treating it as a no-frills legal procedure, and others regarding it as a significant ceremony that is incomplete without music or a reflective introductory speech by the superintendent registrar – even if the couple wanted neither. With significant numbers of couples having a register office wedding because the marriage ceremony they choose to have to reflect their beliefs is not legally binding, there is a need to address these issues of availability, accessibility and administration so that couples are not put off or discriminated against. The recording of Guardbridge Paper Mill was a joint field recording initiative between the Royal Commissions on the Ancient and Historical Monuments of Scotland and of Wales (RCAHMS and RCAHMW), to coincide with the centenary year of both organisations. Its aim was to foster an exchange of approaches to, and techniques of, site recording, in particular industrial process recording, and to explore the practicality of making a worthwhile record of operations of a large, complex working site within limited field time. The paper mill at Guardbridge was chosen for recording as it fulfilled several criteria: the works was considered to be of historical importance as it had been in existence on the same site since the 1870s; there were many and varied buildings and structures illustrating the continuous development of the site; it was easily accessible and relatively compact and it was still active manufacturing its original product. The site was unique, and it was decided that a record should be made on those grounds alone, yet typical in that standard items of plant were used here as elsewhere. The machinery in use spanned a wide date-range and, although everyone is familiar with the finished product and the process is broadly understood in general, it is a much more complex process than at first perceived and thus worth recording from an archaeological perspective so that others may use the results to identify features at similar types of site. As is so often the case with industrial sites, the buildings are generally of secondary importance to the processes carried on within and this should be reflected in the type of recording undertaken. In particular, full advantage should be taken of any opportunity to record while operations are still active. The RCAHMS has carried out surveys at large sites such as at the ICI Nobel's Explosives Factory, Ardeer, Ayrshire; the Lady Victoria Colliery at Newtongrange; Nairn's Linoleum Works, Kirkcaldy; and at Lochaline Silica Mine, Morvern, Argyll, with varying emphasis on process recording dependent on the time available for survey. The RCAHMW has made records of live industrial processes at several sites in Wales, for example, Taff Merthyr Colliery, Merthyr Tydfil; Cooke's Explosives Works, Penrhyndeudraeth, Gwynedd; Esgairmwyn Lead Re-processing Works, Ceredigion; Gywnfynydd Gold Mine, Gwynedd; Alwen Water Treatment Works, Conwy; and a partial record at Cwm Coking Works, Rhondda Cynon Taff. Adopting a children's rights perspective, a critique and analysis underpinned by documentary research methodology was undertaken in order to assess the extent to which the government's Green Paper (Department of Health and Social Care and Department of Education, 2017. Transforming children and young people's mental health provision: a green paper. Available at: https://www.gov.uk/government/consultations/transforming-children-and-young-peoples-mental-health-provision-a-green-paper (accessed 7 December 2017)) addresses the mental health and well-being needs of refugee children and young people in England and Wales, identifying strengths, limitations and challenges for future policy and practice. Findings suggest that there is much of potential benefit to refugee children and young people's future mental health and well-being. However, a paradigm shift, explicit in implications, scale and time frame, will be required, if the Green Paper is to achieve those changes in attitudes, practice and service delivery which it anticipates. We argue that this Green Paper's overarching challenge is that it is premised on Western-centric models in its understanding of the experiences of refugee children and young people, and management of trauma and mental health. It fails to recognize the meanings and significance of culture, and of diversity and difference, and the need to invest in all communities in facilitating engagement and support for children and young people's mental health issues.

Primary care services in or alongside emergency departments look and function differently and are described using inconsistent terminology. Research to determine effectiveness of these models is hampered by outdated classification systems, limiting the opportunity for data synthesis to draw conclusions and inform decision-making and policy. We used findings from a literature review, a national survey of Type 1 emergency departments in England and Wales, staff interviews, other routine data sources and discussions from two stakeholder events to inform the taxonomy. We categorised the forms inside or outside the emergency department: inside primary care services may be integrated with emergency department patient flow or may run parallel to that activity; outside services may be offered on site or off site. We then describe a conceptual spectrum of integration: identifying constructs that influence how the services function-from being closer to an emergency medicine service or to usual primary care. This taxonomy provides a basis for future evaluation of service models that will comprise the evidence base to inform policy-making in this domain. Short-fibre paper residuals (deinking paper fibre (DPF) or paper mill sludge) represents a major waste formed during the processing of recycled paper and is known to contain significant quantities of copper. It is often spread onto agricultural land to help increase soil pH and improve structure by adding soil organic matter (SOM). A number of agricultural sites in England and Wales that had received large and repeated applications were sampled to investigate the long-term effects of this practice on soil quality and plant copper content. We found that the composition of DPF waste has changed significantly between 1999 and 2006 with concentrations of Cu increasing and organic matter content declining. Whilst repeated additions of DPF to agricultural land always increased soil Cu, an associated increase in SOM was not always apparent. There was no link between SOM and bioavailable Cu nor between soil bioavailable Cu and plant Cu. In contrast to previous reports, our findings indicate that improvement in soil quality following the long-term application of DPF was site-specific and in some cases it may have reduced soil quality rather than enhanced it. The last 30 years has seen an exponential increase in Historical Institutional Abuse Inquiries.1 One feature of these has been to place adult survivor voices at the center of Inquiry work, meaning that child abuse victims and survivors2 are engaging with Inquiries, sharing their experiences, with this participation often presented as empowering and healing. This initiative challenges long held beliefs that child sexual abuse survivors are unreliable witnesses, which has led to epistemic injustice and a hermeneutical lacunae in survivor testimony. However to date there has been limited research on what survivors say about their experiences of participation. The Truth Project was one area of work of the Independent Inquiry into Child Sexual Abuse3 in England and Wales. It invited survivors of Child Sexual Abuse to share their experiences including the impacts of abuse and their recommendations for change. The Truth Project concluded in 2021 and heard from more than 6,000 victims of child sexual abuse. The evaluation of the Trauma Informed Approach designed to support survivors through their engagement with the project was a mixed methods, two phase methodology. A total of 66 survey responses were received. Follow-up interviews were conducted with seven survey respondents. The Trauma Informed Approach was found to be predominantly helpful in attending to victim needs and minimizing harm. However, a small number of participants reported harmful effects post-session. The positive impacts reported about taking part in the Truth Project as a one-off engagement challenges beliefs that survivors of child sexual abuse cannot safely talk about their experiences. It also provides evidence of the central role survivors should have in designing services for trauma victims. This study contributes to the epistemic justice literature which emphasizes the central role of relational ethics in the politics of knowing, and the importance of developing a testimonial sensibility when listening to marginalized groups. Purpose: Despite a known risk of cellulitis recurrence, the management of the wider impact and risk factors has been neglected. The innovative National Cellulitis Improvement Programme (NCIP) addresses this by providing evidence-based and individualised care to improve patient reported outcomes and reduce the risk of recurrence. The aim of this paper is to examine the longer-term impact of cellulitis and to identify a suitable and clinically relevant Patient Reported Outcome Measure (PROM). Methods: A review of existing cellulitis-specific PROMs was undertaken, alongside literature detailing the patient-focused impact of cellulitis, to identify a suitable PROM for clinical use. A group of expert therapists and patient representatives (n = 14) shared their individual and collective experiences over a series of events to discuss and debate the impact of cellulitis and review available PROMs. CELLUPROM© is introduced with anonymised PROM data and case study information reported to establish the impact of CELLUPROM© within usual NCIP care. Results: No cellulitis-specific PROMs were identified. Literature focused on the signs and symptoms of an acute episode of cellulitis, with outcome measures primarily used to evidence the impact of an intervention. An enduring physical, social and emotional impact of cellulitis was identified in this study, providing the basis for the new cellulitis-specific PROM (CELLUPROM©), which has been implemented with good effect in clinical care. Conclusion: This study has highlighted the lasting impact of cellulitis. Using CELLUPROM© within the risk-reduction NCIP has helped develop Value-Based Healthcare and support programme evaluation. This introduction outlines the research context from which this special number arises. It opens with a discussion of the interdisciplinary conference 'Reading, Writing and Collecting: Books and Manuscripts in Wales, 1450--1850' and our rationale in our selection of papers from that conference. There follows an exploration of relevant historiography on this topic, drawing out areas of interest but also those where further work is needed. Across the United Kingdom public (and private) resources have been targeted on improving broadband infrastructure. While this has served to provide new opportunities for households and firms, there has been some debate around the ability of firms to take full advantage of the opportunities that arise through this evolving infrastructure. In this respect, there has been particular debate on how far small- and medium-sized enterprises (SMEs) have taken up the challenge of effectively engaging with the resource. Drawing on the case of Wales, this paper explores the digital maturity of a sample of Welsh SMEs. The paper provides evidence of how far SMEs are successfully engaging with the new opportunities available through broadband, and develops a typology of firms according to their engagement. Background/aims: The value of using qualitative methods within clinical trials is widely recognised. How qualitative research is integrated within trials units to achieve this is less clear. This paper describes the process through which qualitative research has been integrated within Cardiff University's Centre for Trials Research (CTR) in Wales, UK. We highlight facilitators of, and challenges to, integration. Methods: We held group discussions on the work of the Qualitative Research Group (QRG) within CTR. The content of these discussions, materials for a presentation in CTR, and documents relating to the development of the QRG were interpreted at a workshop attended by group members. Normalisation Process Theory (NPT) was used to structure analysis. A writing group prepared a document for input from members of CTR, forming the basis of this paper. Results: Actions to integrate qualitative research comprised: its inclusion in Centre strategies; formation of a QRG with dedicated funding/roles; embedding of qualitative research within operating systems; capacity building/training; monitoring opportunities to include qualitative methods in studies; maximising the quality of qualitative research and developing methodological innovation. Facilitators of these actions included: the influence of the broader methodological landscape within trial/study design and its promotion of the value of qualitative research; and close physical proximity of CTR qualitative staff/students allowing sharing of methodological approaches. Introduction of innovative qualitative methods generated interest among other staff groups. Challenges included: pressure to under-resource qualitative components of research, preference for a statistical stance historically in some research areas and funding structures, and difficulties faced by qualitative researchers carving out individual academic profiles when working across trials/studies. Conclusions: Given that CTUs are pivotal to the design and conduct of RCTs and related study types across multiple disciplines, integrating qualitative research into trials units is crucial if its contribution is to be fully realised. We have made explicit one trials unit's experience of embedding qualitative research and present this to open dialogue on ways to operationalise and optimise qualitative research in trials. NPT provides a valuable framework with which to theorise these processes, including the importance of sense-making and legitimisation when introducing new practices within organisations. Rhinosinusitis is a common condition with adults experiencing 2-5 episodes per year. The European Positional Paper on Rhinosinusitis and Nasal Polyps (EP3OS) published in 2005 and updated in 2007 provided evidence-based guidelines on the management of sinus disease promoting a conservative approach to treatment. This study examines the effect of EP3OS on sinus surgery uptake and acute admissions for sinusitis in England and Wales. A retrospective study using the national electronic health databases of England (Hospital Episodes Statistics, HES online) and Wales (Patient Episodes Database of Wales, PEDW) was undertaken from 2000 to 2010 using the OPCS-4 codes E12-E17 (sinus surgery) and ICD10 code J01 (acute admission for sinusitis). Data were analysed for effect following the introduction of the EP3OS in 2005 using linear regression and Chi squared analysis. 116,370 sinus procedures and 10,916 acute admissions for sinusitis were made during the study period. No significant decrease in sinus surgery procedures occurred following the introduction of the EP3OS as may have been expected ( p > 0.05), although subgroup analysis suggested a significant increase in Wales ( p < 0.05). In addition, significant increases in acute admissions for sinusitis were observed following the introduction of EP3OS ( p < 0.05). However, subgroup analysis suggested this was not the case in Wales ( p > 0.05). The EP3OS appears to have had little impact on the rates of sinus surgery but more conservative approaches to managing of sinus disease may have led to an increase in acute admissions. Urine specimens from 135 295 infants have been collected on filter papers and tested for 7 abnormal urinary constituents using spot tests and paper chromatography. The method has detected 5 infants with phenylketonuria, 4 with histidinaemia, 5 with cystinuria, 5 with diabetes mellitus, and one with alcaptonuria. Transient abnormalities such as tyrosyluria, generalized aminoaciduria, cystinuria, and glycosuria have been noted. 2 phenylketonuric infants failed to excrete a detectable quantity of o-hydroxyphenlacetic acid at the time of testing. The findings show that the detection of this compound in urine is an unreliable method of screening for phenylketonuria.

Purpose: The purpose of this paper is to place on record the impact made on government policy and research by Autism Cymru, a small charity that existed in Wales between 2001 and 2014. The success of Autism Cymru resulted directly from philanthropic funding, applied with strategic vision and genuine ambition. Design/methodology/approach: A retrospective, chronological viewpoint highlighting the creative process, drawing upon records held by the charity. Findings: Autism Cymru initiated the concept and played a crucial role in steering the development of government policy for autism in Wales between 2001 and 2011. The charity also drove forward the initiative, which led to the establishment of the Wales Autism Research Centre at Cardiff University in 2010. This paper demonstrates that with astute philanthropic support, small learning disability/autism charities can elicit structural and sustainable change at the national level, leading to wide-ranging benefits for the communities they represent. Originality/value: The strategic approach taken over 20 years ago in Wales by Autism Cymru, which led to the Welsh Government's ASD Strategic Action Plan for Wales (Welsh Government, 2011), set in place a national policy model, which was then followed by The Scottish Strategy for Autism and the Northern Ireland Autism Strategy (Department of Health, 2013>; Scottish Government, 2011). The insightful and tenacious method used by Autism Cymru remains relevant today, demonstrating that any small charity supported by shrewd philanthropic funding can punch well above its weight by taking a planned, ambitious and strategic approach to policy, research and practice. Purpose: For over 15 years, Mental Health First Aid™ (MHFA) has successfully been delivered in Wales, United Kingdom, with growing interest in the MHFA programme and increasing course attendees. Trainers, aware of the need for support, know the importance of MHFA being accessible for different communities and learner groups. MHFA has always focused on increasing mental health literacy. One marginalised group, with lower mental health literacy than the general population, is Deaf people, a group with increased risk of mental health problems. This paper aims to provide insights about why Deaf people are twice as likely as hearing people to experience mental health problems. Design/methodology/approach: During this paper, the authors have used four focal points i) exploring situational contexts for Deaf people; ii) reasons why Deaf individuals are at greater risk of mental health problems; iii) the authors explore a project, "Hear Deaf", and implementation of MHFA Wales by Deaf MHFA trainers; and iv) initiatives to influence and impact on policymakers. Findings: During the project, nine MHFA courses were delivered to Deaf communities across different locations in Wales, often with information and advertisements circulated directly to Deaf communities through Deaf clubs, resulting in 120 Deaf people trained. The authors conclude with their own reflections as a researcher and an MHFA who work predominantly with Deaf communities. Originality/value: This paper provides a discussion on the specific risks for Deaf people around mental health and the importance of mental health promotion programmes for Deaf communities. Devolution to Scotland and Wales was a central pillar of the legislative agenda of the Labour government elected in 1997, yet despite the constitutional significance of this programme it was undertaken without particular enthusiasm by the then prime minister, Tony Blair. Nowhere was this blend of significant change, yet pervasive lack of passion (or interest) more apparent than devolution to Wales. Drawing upon recently released archival documents, this article will explore the New Labour government's approach to devolution in Wales. These papers provide fresh evidence of the second order nature, and treatment, of Welsh devolution compared to the attention provided to Scottish devolution, as well as of the narcissism of small details which pervaded Whitehall's discussions of Welsh devolution (with considerable energy, for example, dedicated to questions about the titles which leading figures in the Assembly would be given). These papers also bear testament to the struggles which Ron Davies, the then secretary of state for Wales, faced in securing even a minor scheme of devolution and the tensions he experienced with the centre of government. The Welsh Government has recently published a White Paper which sets out the key elements of the forthcoming Sustainable Development Bill. It is a significant stepping stone in incorporating sustainable development into dedicated primary legislation, and, in a wider sense, providing a governance framework for making strategic decisions on the part of the Welsh Government and the Welsh public service organisations. This article observes that the Welsh Government is diverting away from a meaning of sustainable development which emphasises the environmental pillar. The focus of the White Paper is to provide a framework for the exercise of discretion by public service organisations in order to increase the chances of mainstreaming sustainable development. Description Within the current climate of health care pressures, services are exploring ways in which to use resources to the best of their ability to ensure service users have timely, safe, and effective care as well as having positive outcomes and good experiences of the care they receive. The current paper explores a service level agreement between the private sector and a child and family service within the HSC. Within this agreement, has been the development of a pathway through which families on a psychological service waiting list, could be triaged, and allocated to a private clinic within which, psychological care could be provided from assessment through to discharge. The subjective experiences and initial outcomes of those involved have been largely positive. The paper describes, explores, and reflects on the process of collaborative practice. The papurau bro (Welsh language local newspapers) break all the rules in helping to keep the Welsh language alive - but is their revolution running out of steam? From a small seed sown in Cardiff in 1973, some 56 publications still exist, which implies they've earned their place in contemporary Wales. Yet they break every rule of design, of news values and competition. They rarely seek advertising and few have paid staff to produce the goods. They are furiously independent and regard the advent of "new" - for which read "relatively old" - technology with a perhaps not altogether healthy degree of scepticism... Yet the papers are being dragged slowly into the 21st century - some now have their own web presence. But even if, together, they form the largest single readership of anything published in the Welsh language, their future is not guaranteed. Research shows that 60 per cent of papers fear for their future. Their continued antipathy towards anything commercial and anything that might constitute a "scoop", plus that unbending independence, could spell doom for something that has been nothing short of a phenomenon. Purpose: This paper is a commentary that aims to address themes arising from the article by Morgan titled "Wales and Autism: The Impact of Philanthropy Matched with Ambition". Design/methodology/approach: This opinion piece considers the broader impact of stakeholder engagement in the field of autism, including how the dissemination of evidence-based support could be made more accessible for autistic people, their families and carers and philanthropists. Findings: This paper highlights the link between stakeholder engagement and meaningful change in policy and practice in the field of autism. It highlights the need for philanthropists to invest in evidence-based support but highlights the difficulty in identifying and accessing such support. This piece supports the use of evidence and gap maps (EGMs) to increase the accessibility of autism research and evidence. Originality/value: While stakeholder engagement is encouragingly rising in this field, most of the evidence is held in academic journals, which are behind paywalls and, therefore, inaccessible to many autistic people, their families, carers and associated professionals. Displaying evidence using EGMs provides an accessible way for autistic people and those who support them to identify possible avenues of support quickly and allows philanthropists to direct research funding to areas of high priority. Purpose: Parental advocacy is an emerging area of research and policy interest in Wales and across the UK. Although there is little research in the UK context to date, international research has indicated that parental advocacy can improve the relationship between parent and professional in the field of child protection social work. This paper aims to ascertain how the implementation of a parental advocacy programme supports parents to play a meaningful role in decision-making when children's services are working with them and their families. Design/methodology/approach: This study used interviews, surveys and focus groups to obtain qualitative data from 18 parents, seven parent advocates, two advocacy managers and four social workers, to explore the potential impact of parental advocacy on decision-making. The study identified challenges in implementing parental advocacy, particularly relating to awareness of the service. Participants also discussed experiences of the child protection system and how parents are supported by advocates. Findings: Despite challenges surrounding implementation, initial findings were encouraging, and generated examples of how parental advocacy services have helped parents to understand children's services and develop relationships of trust with social care professionals. In doing so, this study identified potential mechanisms that may be useful to support future service delivery. Originality/value: This paper and research is novel as it explores parental advocacy within the Welsh context. Although there has been research conducted into parental advocacy, this has largely come from the USA. Purpose: This study aims to critically examine the effects of COVID-19 social discourses and policy decisions specifically on older adult volunteers in the UK, comparing the responses and their effects in England, Scotland, Wales and Northern Ireland, providing perspectives on effects of policy changes designed to reduce risk of infection as a result of COVID-19, specifically on volunteer involvement of and for older adults, and understand, from the perspectives of volunteer managers, how COVID-19 restrictions had impacted older people's volunteering and situating this within statutory public health policies. Design/methodology/approach: The study uses a critical discourse approach to explore, compare and contrast accounts of volunteering of and for older people in policy, and then compare the discourses within policy documents with the discourses in personal accounts of volunteering in health and social care settings in the four nations of the UK. This paper is co-produced in collaboration with co-authors who have direct experience with volunteer involvement responses and their impact on older people. Findings: The prevailing overall policy approach during the pandemic was that risk of morbidity and mortality to older people was too high to permit them to participate in volunteering activities. Disenfranchising of older people, as exemplified in volunteer involvement, was remarkably uniform across the four nations of the UK. However, the authors find that despite, rather than because of policy changes, older volunteers, as part of, or with the help of, volunteer involving organisations, are taking time to think and to reconsider their involvement and are renewing their volunteer involvement with associated health benefits. Research limitations/implications: Working with participants as co-authors helps to ensure the credibility of results in that there was agreement in the themes identified and the conclusions. A limitation of this study lies in the sampling method, as a convenience sample was used and there is only representation from one organisation in each of the four nations. The 2019 ICC Cricket World Cup was held from May 30 to July 14 in England and Wales with a new tournament format being used. The competition was reduced to ten teams, in a single pool, with each team scheduled to play every one of the other nine teams. The purpose of such a tournament is to identify the strongest or most dominant teams, but this was thwarted in the 2019 Cricket World Cup by the scoring system, in particular the use of net run rate to rank teams tied on points awarded for wins, draws and ‘no result’ matches. Concepts from discrete mathematics, in particular graph theory, are applied to create a ranking system better suited to this type of tournament. The dominance graph for matches between the top five teams shows that the team which came fourth, under the current system, was dominated by the team placed in fifth position. This paper shows how the use of net run rate may be replaced by calculating one stage, two stages and if necessary three stage dominances, to rank teams. These calculations are easily done using a dominance matrix. Recommendations to ensure that the strongest teams play in the final match, for the cup, are made and it is suggested that the group stage of the FIFA Football World Cup use the scoring method explained in this paper. In an era of accelerating biomedical innovations, partial ectogenesis looms as a double-edged sword for female bodily autonomy and reproductive rights. Rather than simplifying or 'solving' abortion access and the ethical issues inherent to that right, this technology wields the alarming potential to curtail women's right to safe and legal abortions. This article investigates the potential of partial ectogenesis as a 'solution' to the abortion debate, focusing on two significant hypotheses. Foremost, the paper scrutinizes the dilemma of ectogenesis abortion; specifically, instances where a woman intending to terminate her pregnancy is obliged to undergo foetal transfer surgery. This scrutiny engages with Räsänen's concept of the right to the death of a foetus, yet ultimately concludes that this supposed right is unconvincing. Despite there being no such right, this paper argues that the prospect of partial ectogenesis is incapable of resolving the core issues of the abortion debate, not least because of the inherent conflict between a woman's right to her own body and the proposed technology or rather, compulsion of such surgeries. Instead, this article posits that a more significant ethical and logical concern lies in misconceptions surrounding the term 'foetus' and how such misconceptions impact considerations of autonomy. Thereafter, this paper examines the potential for partial ectogenesis to alter the legal viability threshold set by the Abortion Act 1967-in particular, this paper explores the impact a shift in the viability threshold would have on female bodily autonomy. In the aim of resolving these concerns, this article concludes by advocating for the decriminalisation of abortion to safeguard female bodily autonomy against constraints on abortion access were the viability threshold legally revised. By addressing these crucial topics, this article aims to provide a nuanced analysis of the dialogue surrounding reproductive rights, bodily autonomy, and the complex interplay between emerging biotechnologies, medical ethics and legislative frameworks. South Asians constitute Britain's largest ethnic minorities, yet very little is known about their migration history in the prewar era. Recent literature has begun to challenge the widely held view that South Asian migration to Britain was a post-war phenomenon in response to Britain's need for labour as it emerged from two costly wars. The literature has demonstrated that the South Asian presence in Britain can be traced back to the seventeenth century. This paper adds to that literature by analysing the occupation distribution and mobility of South Asian-born migrants during the early twentieth century. Regression results from the 1901 and 1911 Censuses of England and Wales suggest that relative to other foreign-born and British-born, South Asian-born immigrants were primarily employed in white-collar jobs, a position they largely retained until at least 1911. A growing body of scholarship draws attention to prisons and environmental justice, pointing out the propensity for prisons to be located on contaminated sites and to be in close proximity to polluting industries, as well as for prisons themselves to contribute to local environmental degradation. Prisoners' immobility renders them unable to relocate away from harmful environments, and there are now numerous suggestions that their wellbeing suffers as a result of the poor quality of many local environments. However, since a relationship between environmental quality and prisoner wellbeing is yet to be robustly demonstrated, there is currently no firm evidence base from which to argue for positive change. This paper therefore examines the effect of the environmental quality of the locations of prisons, approximated as the presence of greenspace in the immediate vicinity of prisons, local species biodiversity and local levels of air and noise pollution, on wellbeing outcomes in a set of prisons in England and Wales. It finds that good environmental quality, in the form of high biodiversity and/or low air pollution, enhances the already-recognized positive effects of greenspace on the wellbeing of incarcerated populations. An increasing body of research suggests that bilingualism is possible and perhaps even advantageous for autistic individuals. However, several factors might influence parents' decisions about raising their autistic child bilingually, including national language policies, educational contexts, advice received from key professionals, and the child's individual strengths and needs. Accordingly, there is a clear imperative to understand how the views of different stakeholders converge and diverge when language decisions are made in the context of autism. This paper brings new insights by synthesising the findings of three qualitative studies that used interpretative phenomenological analysis (IPA) to explore the perspectives and experiences of bilingual autistic children (n = 11), parents (n = 16), and educational practitioners (n = 13) of bilingualism in autism in England and Wales. Despite wide variation between and within groups, a striking tension emerged between individuals' beliefs about bilingualism in general, which were positive, and their experiences of bilingualism in autism specifically, which often foregrounded more monolingual approaches. This paper examines this tension, with a particular focus on stakeholders' attitudes towards the feasibility of bilingualism, families' language choices in the context of autism, and how notions of contextual linguistic diversity accentuated differences between England and Wales. We conclude by arguing that greater awareness of both bilingualism and neurodiversity in educational and clinical settings may improve the experiences of bilingual autistic children and, crucially, the language advice families receive. Motivated by demand prediction for the custodial prison population in England and Wales, this paper describes an approach to the study of service systems using infinite server queues, where the system has non-empty initial state and the elapsed time of individuals initially present is not known. By separating the population into initial content and new arrivals, we can apply several techniques either separately or jointly to those sub-populations, to enable both short-term queue length predictions and longer-term considerations such as managing congestion and analysing the impact of potential interventions. The focus in the paper is the transient behaviour of the M t / G / ∞ queue with a non-homogeneous Poisson arrival process and our analysis considers various possible simplifications, including approximation. Despite a widespread focus on grandparents, a large proportion of kinship care in the UK is provided by older siblings. What drives older siblings to become kinship carers, and how this might differ from other kinship carers, is not well represented in academic literature. In this study, narrative interviews were carried out with thirteen adults across England, Scotland, and Wales who had experience being the main carer for their younger sibling(s) when their parents could not care for them sufficiently. The narrative method elicited holistic accounts of participants experiences of being a sibling carer, and the analysis generated three groups of narrative accounts highlighting how and why some sibling kinship care arrangements come about—with siblings wanting to bring their younger siblings back into their family; siblings trying to keep their younger siblings in their family; or older siblings stepping in to fill a gap in parenting at home. The paper draws on the narrative accounts of participants to build the groups, presenting an illustrative narrative account to represent each group. Importantly, these accounts demonstrate how becoming a kinship carer as an older sibling may, or may not, be recognised or fit into wider narratives of what becoming a kinship carer looks like. It is hoped that these accounts will prompt practitioners and policymakers to look more closely at the role of siblings when considering who is and who should be involved in deciding how to support children to remain within their family network. Background: A COVID-19 hospital guideline was implemented across all 18 acute hospitals in Wales in March 2020, promoting ward management of COVID pneumonitis and data collected across the first 3 Waves of the pandemic (Wave 1 March 1st 2020 to November 1st 2020, Wave 2 November 2st 2020 to February 21st 2021 and Wave 3 June 1st 2021 to December 14th 2021). The aim of this paper is to compare outcomes for patients by admission setting and type of ventilatory support given, with a particular focus on CPAP therapy. Methods: This is a retrospective observational study of those aged over 18 admitted to hospital with community acquired COVID-19 between March 2020 and December 2021. The outcome of interest was in-hospital mortality. Univariate logistic regression models were used to compare crude outcomes across the waves. Multivariable logistic regression models were used to assess outcomes by different settings and treatments after adjusting for Wave, age, sex, co-morbidity and deprivation. Results: Of the 7,803 records collected, 5,887 (75.4%) met the inclusion criteria. Analysis of those cases identified statistically significant outcome improvements across the waves for all patients combined (Waves 1 to 3: 31.5% to 18.8%, p<0.01), all ward patients (28.9% to 17.7%, p<0.01), and all ICU patients (44.3% to 32.2%, p = 0.03). Sub group analyses identified outcome improvements in ward patients without any oxygen therapy (Waves 1 to 3: 22.2% to 12.7%, p<0.01), with oxygen therapy only (34.0% to 12.9%, p<0.01) and with CPAP only (63.5% to 39.2%, p<0.01). The outcome improvements for ICU patients receiving CPAP only (35.7% to 24.6%, p = 0.31) or invasive ventilation (61.6% to 54.6%, p = 0.43) were not statistically significant though the numbers being admitted to ICU were small. The logistic regression models identified important age and comorbidity effects on outcomes. The multivariable model that took these into account suggested no statistically significantly greater risk of death for those receiving CPAP on the ward compared to those receiving CPAP in ICU (OR 0.89, 95% CI: 0.49 to 1.60). Conclusions: There were successive reductions in mortality in inpatients over the three Waves reflecting new treatments and better management of complications. The dominant arrangement for governance in many States which are party to international human rights treaties is decentralisation. This puts implementation of human rights in the hands of institutions which are geographically closer to intended beneficiaries. Decentralisation to different levels of government introduces complexity, risk and opportunity to the governance of human rights. In response, UN Treaty Monitoring Bodies focus on State responsibility for implementation of human rights. The Committee on the Rights of the Child has emphasised the State as the coordinating body responsible for children's rights under the UN Convention on the Rights of the Child (CRC). In this paper, Wales is used as an example of a jurisdiction where decentralisation is a feature of children's rights, which has enabled a progressive approach to implementation. It explains the principled Children's Rights Approach which provides public authorities in Wales with a coherent framework to give effect to international children's rights policy planning and service delivery. As the contours of decentralisation in Wales are typical of arrangements in many States parties to the CRC, insights from Wales will be transferable to other jurisdictions.
