This is the first Enhanced Recovery After Surgery (ERAS) guideline dedicated to standardizing and optimizing perioperative care for women undergoing minimally invasive gynecologic surgery. The guideline was rigorously formulated by an American Association of Gynecologic Laparoscopists Task Force of US and Canadian gynecologic surgeons with special interest and experience in adapting ERAS practices for patients requiring minimally invasive gynecologic surgery. It builds on the 2016 ERAS Society recommendations for perioperative care in gynecologic/oncologic surgery by serving as a more comprehensive reference for minimally invasive endoscopic and vaginal surgery for both benign and malignant gynecologic conditions. For example, the section on preoperative optimization provides more specific recommendations derived from the ambulatory surgery and anesthesia literature for the management of anemia, hyperglycemia, and obstructive sleep apnea. Recommendations pertaining to multimodal analgesia account for the recent Food and Drug Administration warnings about respiratory depression from gabapentinoids. The guideline focuses on workflows important to high-value care in minimally invasive surgery, such as same-day discharge, and tackles controversial issues in minimally invasive surgery, such as thromboprophylaxis. In these ways, the guideline supports the American Association of Gynecologic Laparoscopists and our collective mission to elevate the quality and safety of healthcare for women through excellence in clinical practice. With the growing global energy demand and limited production of conventional crude oil, the extraction of unconventional heavy and extra-heavy oil is receiving unprecedented attention. China is the fourth largest producer of heavy oil. High viscosity and complex formation environments increase the difficulty of exploiting heavy oil. To alleviate the economic cost and environmental pollution pressure brought by heavy oil production, China has achieved significant advancement in the innovation of heavy oil recovery, including accelerating the transformation of thermal technology to non-thermal technology and the use of new energy technology to achieve the goal of low-carbon economy and green environmental protection. Based on the background of heavy oil in China, this paper recharacterizes the high-viscosity mechanism of heavy oil. From different aspects, including basic principles, main characteristics, applicability, limitations, and challenges, this paper comprehensively reviews existing heavy oil recovery technologies, including thermal recovery, in-situ upgrading, cold recovery, and new energy technologies. Currently, in China, the main target of heavy oil recovery is achieving the "carbon peaking and carbon neutrality" goals, which can be described as low energy consumption, low emission, and low pollution. To facilitate the low-carbon transition in the extraction of heavy/extra-heavy oil and attain environmental sustainability goals, it is essential to increase the proportion of non-thermal factors in thermal recovery technologies, develop efficient in-situ catalysts, adopt clean cold extraction techniques, and advance the development of new energy technologies. Background: Multiple papers have purported the superiority of spinal anesthesia used in total joint arthroplasty (TJA). However, there is a paucity of data available for modern general anesthesia (GA) regimens used at high-volume joint replacement centers.Methods: We retrospectively reviewed a series of 1527 consecutive primary TJAs (644 total hip arthroplasties and 883 total knee arthroplasties) performed over a 3-year span at a single institution that uses a contemporary GA protocol and report on the length of stay, early recovery rates, perioperative complications, and readmissions.Results: From the elective TJAs performed using a modern GA protocol, 96.3% (n = 1471) of patients discharged on postoperative day 1, and 97.2% (n = 1482) of subjects were able to participate with physical therapy on the day of surgery. Only 6 patients (0.4%) required an intensive care unit stay postoperatively. The 90-day readmission rate over this time was 2.4% (n = 36), while the reoperation rate was 1.3% (n = 20).Discussion: Neuraxial anesthesia for TJA is commonly preferred in high-volume institutions utilizing contemporary enhanced recovery pathways. Our data support the notion that the utilization of modern GA techniques that limit narcotics and certain inhalants can be successfully used in short-stay primary total joint arthroplasty.Level Of Evidence: IV- Case series. Enhanced recovery after surgery (ERAS) initiatives in the fields of gastrointestinal and pelvic surgery have contributed to improved postoperative functional status for patients and decreased length of stay. A similar comprehensive protocol is lacking for patients undergoing craniotomy for tumor resection. A literature search was performed using PubMed. These references were reviewed with a preference for recent high quality studies. Cohort and retrospective studies were also included if higher levels of evidence were lacking. A literature search was conducted for scalp blocks and minimally invasive craniotomies. Papers were scored using Grading of Recommendations Assessment, Development and Evaluation (GRADE) criteria for evidence level and recommendation grade. Seventeen ERAS items were reviewed and recommendations made. The current body of evidence is insufficient to create a standardized protocol for craniotomy and tumor resection. However, this initial review of the literature supports pursuing future research initiatives that explore modalities to improve functional recovery and decrease length of stay in craniotomy patients. Summary: Background & Aims: Clinical care of patients undergoing colonic surgery differs between hospitals and countries. In addition, there is considerable variation in rates of recovery and length of hospital stay following major abdominal surgery. There is a need to develop a consensus on key elements of perioperative care for inclusion in enhanced recovery programmes so that these can be widely adopted and refined further in future clinical trials. Methods: Medline database was searched for all clinical studies/trials relating to enhanced recovery after colorectal resection. Relevant papers from the reference lists of these articles and from the authors’ personal collections were also reviewed. A combination of evidence-based and consensus methodology was used to develop the resulting enhanced recovery after surgery (ERAS) clinical care protocol. Results and Conclusions: Within traditional perioperative practice there is considerable evidence supporting a range of manoeuvres which, in isolation, may improve individual aspects of recovery after colonic surgery. The present manuscript reviews these issues in detail. There is also growing evidence that an integrated multimodal approach to perioperative care can result in an overall enhancement of recovery. However, effects on major morbidity and mortality remain to be determined. A protocol is presented which is in current use by the ERAS Group and may provide a standard of care against which either current or future novel elements of an enhanced recovery approach can be tested for their effect on outcome. This paper presents results of a comprehensive study involving analytical, numerical and experimental investigations into transverse fracture propagation from horizontal wells. The propagation of transverse hydraulic fractures from horizontal wells is simulated and investigated in the laboratory using carefully designed experimental setups. Closed-form analytical theories for Mode I (opening) stress intensity factors for idealized fracture geometries are reviewed, and a boundary element-based model is used herein to investigate non-planar propagation of fractures. Using the mixed mode fracture propagation criterion of the model, a reasonable agreement is found with respect to fracture geometry, net fracture pressures and fracture propagation paths between the modeled fractures and the laboratory tested fractures. These results suggest that the propagation of multiple fractures requires higher net pressures than a single fracture, the underlying reason of which is theoretically justified on the basis of local stress distribution.
