Background: Developmental models suggest that the phenotypes may arise from an immediate or mediated adaptive metabolic response of the perinatal growth. Evidence on the cumulative effects of growth and factors associated with risk of insulin resistance in adolescents is lacking. Objective: To investigate the association between birthweight, weight gain during infancy, childhood and adolescence and the triglyceride-glucose index in adolescents. Methods: This is a cohort of 217 children born at term, followed for the first six months, and reassessed at 8 and 18 years of age. The variables of interest were birthweight, postnatal growth defined as rapid postnatal growth when the weight gain from birth to six months of age was greater than 0.67 z-score, and the same criterion was used for high BMI gain from ages 6 months to 8 years, and from 8 to 18 years. Socioeconomic condition, nutritional status, practice of physical exercises and consumption of ultra-processed foods were verified. Multivariate linear regression analysis was used to verify the effect of the variables on the triglyceride-glucose index. Results: Birthweight was not associated with triglyceride-glucose index in adolescence. Rapid postnatal growth during the first 6 months, higher BMI gain from 8 to 18 years and higher waist circumference contributed significantly to explain higher triglyceride-glucose index. Conclusion for Practice: Our findings suggest that rapid postnatal growth may be one of the first signs of a higher triglyceride-glucose index in adolescence and that attention should be paid to the greater gain in body mass between childhood and adolescence for the risk of a higher triglyceride-glucose index. Significance: What is Already Known on this Subject?: Metabolic disorders are associated with excess weight and body fat in adolescence, however, growth deviations in phases of rapid human development, for example, perinatal period, childhood and adolescence, may be the first signs of future metabolic changes. Childhood dyslipidaemia is associated with the occurrence of cardiovascular diseases in adulthood, so evaluating whether an individual has a genetic predisposition to this pathology is of great importance for early action of prevention and treatment. This study aimed to evaluate the association between the FTO (rs9939609), MC4R (rs17782313) and MTMR9 (rs2293855) polymorphisms, the obesity‐related genetic risk score and atherogenic risk in Brazilian children. This is a cross‐sectional study conducted in 544 children aged 4–9 years in the city of Viçosa, Minas Gerais state, Brazil. The single nucleotide polymorphisms rs9939609, rs17782313 and rs2293855, were identified by the system TaqMan SNP genotyping and the obesity‐related genetic risk score was determined. The lipid profile (serum total cholesterol [TC], high density lipoprotein [HDL] cholesterol, low density lipoprotein [LDL] cholesterol, triglycerides) was analysed and the atherogenic indices (Castelli I and II indices), atherogenic coefficient (AC), lipoprotein combined index (LCI) and plasma atherogenic index (PAI) were calculated. A semi‐structured questionnaire was applied, obtaining data on the sociodemographic, economic and lifestyle characteristics of the children. Weight and height measurements were performed in all children, and body composition was evaluated by Dual‐Energy X‐ray Absorptiometry (DXA). 55.5% of the sample had dyslipidaemia, while 28.5% of the sample had at least one polymorphism and 2.2% had three polymorphisms. Children with the AG/AA genotypes in the rs2293855 polymorphism had lower HDL cholesterol levels and higher TC/HDL cholesterol, LDL/HDL cholesterol ratios and AC. Those with one or more polymorphisms (rs9939609, rs17782313 and rs2293855) in the genetic risk score had lower HDL cholesterol levels and higher TC/HDL cholesterol ratios, AC, LCI and PAI. In conclusion, the risk allele of the rs2293855 polymorphism and a higher obesity‐related genetic risk score were positively associated with higher atherogenic risk in Brazilian children. Objective: To analyze the relation between alterations in the growth hormone (GH)/insulin-like growth factor 1 (IGF-1) axis during the first 6 months of life and weight in children born in the lower-middle São Francisco region. Methods: This is an analytical cohort and exploratory. Thirty children, were formed two groups, one of low birth weight children (LBW, n = 15) and another of normal weight (NBW = 15) were initially identified in a hospital and reapproached at 3 and 6 months of age. Birth weight and alterations in GH/IGF-1 curves were measured at birth and the third and sixth months of life. Results: Weight gain during the 6 months of follow-up in newborns with a low birth weight was greater compared to newborns with a normal birth weight. All children who were born with a low birth weight had an altered GH/IGF-1 curve at birth (p = 0.002). Most newborns with a low birth weight maintained the alteration in the GH/IGF-1 curve at the third month of life (p = 0.027). Regarding the GH/IGF-1 curve at the sixth month, alteration persisted in greater proportion among children with a low birth weight. Conclusions: Alterations in insulin resistance markers, demonstrated by increased GH without a proportional increase in IGF-1, were observed to be significant in children with a low birth weight with greater adiposity in this group which may increase the risk of metabolic diseases in later life. Aim: To analyse the etiological factors of the nursing diagnosis of impaired comfort in children and adolescents with cancer. Design: This cross‐sectional study was carried out in the referral unit for the treatment of childhood cancer in a tertiary hospital located in northeastern Brazil. Methods: A total of 200 children and adolescents who were undergoing cancer treatment were included in this study. Data collection instruments and protocols were constructed with operational and conceptual definitions of clinical indicators and etiological factors for the nursing diagnosis of impaired comfort. A latent class model with adjusted random effects was used to determine impaired comfort and measures of sensitivity and specificity of clinical indicators. A univariate logistic regression analysis was performed for each etiological factor of impaired comfort. Results: The analysis of etiological factors for the nursing diagnosis of impaired comfort in children and adolescents with cancer showed the high prevalence of four factors: noxious environmental stimuli, insufficient situational control, insufficient resources and insufficient environmental control. Illness‐related symptoms, noxious environmental stimuli, and insufficient environmental control increased the chance of impaired comfort occurring. Conclusion: The etiological factors with the highest prevalence and most significant impact on the occurrence of impaired comfort were noxious environmental stimuli, insufficient situational control and illness‐related symptoms. Impact: The results obtained in this investigation can support more accurate nursing diagnostic inference of impaired comfort in children and adolescents with cancer. Moreover, the results can inform direct interventions for the modifiable factors that trigger this phenomenon to avoid or minimize the signs and symptoms of the nursing diagnosis. Introduction: The frequent occurrence of adverse events during hospital admission demands proactive means of risk management, including checking trackers/triggers. Objective: To verify the factors associated with triggers and adverse events in pediatric hospitalization. Material and Methods: Cross-sectional research based on the Institute for Healthcare Improvement (IHI) methodology, through the application of the Pediatric Trigger Tool (PTT) to a sample (n= 194) from medical records of pediatric patients from a hospital in the Center-West of Brazil. Descriptive, inferential statistical analysis and Poisson regression were performed. Results: More than half (n=107; 55.15%) of patients had at least one trigger upon admission. 204 triggers were identified, with the highest occurrence of a drop in hemoglobin/hematocrit (9.80%), a drop in oxygen saturation (9.80%) and an increase in kidney function markers (9.20%). Of the total triggers, 64 (31.37%) adverse events were confirmed, which were mostly classified as temporary damage requiring patient support (65.62%). The length of stay (p-value=0.004) and the nature of the hospitalization (p-value<0.001) were variables associated with the occurrence of triggers. Character of hospitalization and admissions from other institutions were predictors of the occurrence of triggers and adverse events. Discussion: The study found 31.37% of triggers resulting in harm to the patient, early detection is essential in pediatric patient safety, prolonged hospitalizations are linked to infections and adverse events, patient transfers require rigorous and effective safety measures. Brazil has been severely affected by the COVID-19 pandemic with one of the largest numbers of youth impacted by school closure globally. This longitudinal online survey assessed emotional problems in children and adolescents aged 5–17 years living in Brazil during the COVID-19 pandemic. Recruitment occurred between June to November 2020 and participants were invited for follow-up assessments every 15 days until June 2021. Participants were 5795 children and adolescents living across the country with mean age of 10.7 (SD 3.63) years at recruitment; 50.5% were boys and 69% of white ethnicity. Weighted prevalence rates of anxiety, depressive and total emotional symptoms at baseline were 29.7%, 36.1% and 36%, respectively. Longitudinal analysis included 3221 (55.6%) participants and revealed fluctuations in anxiety and depressive symptoms during one year follow-up, associated with periods of social mobility and mortality. Emotional problems significantly increased in July and September 2020 and decreased from December 2020 to February 2021 and then significantly increased in May 2021 relative to June 2020. Older age, feeling lonely, previous diagnosis of mental or neurodevelopmental disorder, previous exposure to traumatic events or psychological aggression, parental psychopathology, and sleeping less than 8/h a day were associated with increased rates of anxiety and depressive symptoms at baseline and over time. Food insecurity and less social contact with family and peers were associated with baseline anxiety and depressive symptoms, and lowest socio-economic strata, chronic disease requiring treatment and family members physically ill due to COVID-19 were associated with increasing rates over time. The pandemic severely affected youth, particularly those from vulnerable populations and in moments of increased mortality and decreased social mobility. Purpose: The objective of the present study was to investigate oral health status and treatment needs of children with disabilities in Recife, Brazil.Materials and Methods: A cross-sectional study was carried out in the six administrative districts of Recife. The sample consisted of 366 children with disabilities and age between 3 and 12 years. The oral health conditions investigated were dental caries (CPOD index and dmft index), gingival state (IPV and IGC index) and dental trauma. Data analysis involved descriptive statistics, Pearson's chi-square test, Fisher's exact test, and Poisson regression models.Results: The prevalence of caries was 65% and was associated with age (p = 0.0027) and area of residence (p = 0.020). The prevalence of need for treatment was also 65%. The mean decayed, missing and filled teeth (DMFT)/DMFT index of the study population was 3.17/1.73. Their mean number of DMFT was 2.37, 0.55 and 0.25 for the deciduous dentition, as well as 1.56, 0.05 and 0.12 for the permanent dentition. Almost the entire sample (96.7%) had visible plaque, 77.3% had gingival bleeding and 27.6% had dental traumatism.Conclusion: Children with disabilities were found to have high rates of caries and gingivitis, as well as cumulative needs for preventive and curative treatment. The Apgar score is applied immediately after birth to clinically evaluate the newborn, at the first and fifth minutes of life. The Apgar score can help diagnose scenarios of neonatal anoxia or hypoxia. This is a retrospective, descriptive, and analytical study that used secondary data from DATASUS (Department of Informatics of the Unified Health System), from 1994 to 2018. The studied population includes all live births in Brazil during this period. The inclusion criteria were pregnant women over 15 years old, and the exclusion criteria were multiple pregnancies, pregnancies lasting less than 37 weeks, and newborns with congenital anomalies. These criteria were established to reduce potential confounding factors in the analysis that could lead to errors in interpretation of the results. The variables studied were place of delivery, type of delivery, and number of prenatal consultations. As results, having 7 or more prenatal consultations is a protective factor for alterations in the Apgar score, as well as cesarean delivery and hospital delivery. It was observed that pregnancy care, regarding the studied variables, influences the Apgar score. However, future studies in different populations are necessary to confirm these results. Omission of breakfast starts at school age and can be explained as a reflection of the independent behaviour acquired in this phase of life. Breakfast has been investigated for its association with healthier diet quality, but few studies have investigated this relationship in schoolchildren aged 7 to 13 years. The objective of this study was to assess breakfast consumption in schoolchildren aged 7 to 13 years and to examine associations with dietary patterns (DPs). It was a cross‐sectional study carried out in 2017 with 1069 students in Florianopolis, southern Brazil. Previous‐day food consumption, physical activity and screen activities were self‐reported. Factor analysis was used to identify DPs. Associations between breakfast consumption and overall DPs were evaluated using multivariate linear regression. Breakfast consumption was reported by 85% of the children, with the highest proportion observed among students on the afternoon school shift. Five DPs were identified explaining 41% of the total variance in daily food consumption: Ultra‐processed, Mixed, Traditional Brazilian Lunch, Healthy + Sweets and Traditional Brazilian Breakfast. Breakfast consumption was directly associated with a Traditional Brazilian Breakfast DP (β = 0.335; 95% CI = 0.227, 0.442), a Mixed DP (β = 0.241; 95% CI = 0.160, 0.323), and a Healthy + Sweets DP (β = 0.160; 95% CI = 0.061, 0.260), and inversely associated with an Ultra‐processed DP (β = −0.116; 95% CI = −0.207, −0.026). Breakfast consumption was associated with overall DPs, suggesting the importance of eating this meal by schoolchildren. We emphasise the relevance of offering a healthy breakfast at public schools for students who do not take this meal at home, contributing to health promotion. [ABSTRACT FROM AUTHOR]

Objective: The present study aimed to describe the clinical forms and the time taken to diagnose new tuberculosis cases and to statistically analyze the isolated and combined forms of the disease in children and adolescents treated at a university hospital in Rio de Janeiro during the first year of the COVID-19 pandemic in Brazil. Methods: This was a cross-sectional study that used retrospective data on children (0–9 years old) and adolescents (10–18 years old) with pulmonary (PTB), extrapulmonary (EPTB), and combined tuberculosis (PTB + EPTB) followed up at the outpatient clinic from January 2019 to March 2021. Categorical data were analyzed by descriptive statistics and expressed as frequency and proportions. Categorical variables were compared using the Chi-square test, and numerical variables using Student’s T-test. Results: A total of 51 cases were included, 63% (32/51) of which comprised patients in the year of the pandemic (group A), while 37% (19/51) were patients attended in previous years (group B). In group A, 19% (6/32) of the patients presented PTB, 59% (16/32) had EPTB, and 31% (10/32) had PTB+EPTB. In group B, 42% (8/19) of the patients presented PTB, 42% (8/19) had EPTB, and 16% (3/19) had PTB+EPTB. Conclusion: Our study revealed more tuberculosis cases in the first year of the pandemic than in the same period of the previous year, with greater variation of sites affected by the disease, including rarer and more severe forms. Objective: To verify whether access to information about oral hygiene influenced the practices of caregivers of children with microcephaly caused by Zika virus (ZIKV). Methods: A case series study was developed at the Mens Sana Rehabilitation Center, in the municipality of Arcoverde, Pernambuco, Brazil. The study universe included the mothers of 32 children with microcephaly caused by Zika virus. Data collection took place in June 2018 and was carried out through a structured questionnaire. The variables studied comprised socioeconomic issues, habits, diet, and knowledge about oral hygiene. Fisher's Exact and Pearson's Chi‐square tests were used with a significance level of 5%. Results: Out of 32 individuals eligible to participate in the study, only 27 were selected. Most mothers reported receiving information about oral hygiene from their infants (63.0%). Fisher's Exact and Pearson's Chi‐square tests showed that access to oral hygiene information was associated with mother's education, family income, regular visits to the dentist, frequency of tooth brushing, age at which the infant started brushing, and brushing time (p < 0.05). Conclusion: Access to information about oral hygiene positively influenced the oral health care of mothers with their children with microcephaly caused by ZIKV. [ABSTRACT FROM AUTHOR]

Few studies have assessed frequency of maltreatment and other factors associated with maltreatment of deaf and hard of hearing children. The present study's objective was to verify the frequency of physical and psychological maltreatment and associated factors experienced by DHH children. DHH children from low-income families in Maceió, Brazil, were studied (N = 265). The Parent-Child Conflict Tactics Scales (Straus et al. 1998) were used to identify nonviolent discipline, psychological aggression, and physical assault (minor, severe, very severe). The study found that 221 children (83.4%) experienced physical assault; 238 (89.8%) experienced psychological aggression. Both physical and psychological aggression was reported for 94.3%. Most mothers (98.1%) reported using nonviolent discipline. Maltreatment was associated with male children, mothers' job dissatisfaction, religiously nonobservant families, and children born of unintended pregnancy. Infant feeding practices impact children's nutritional and health status, influencing growth and development. This study aimed to analyse the evolution of infant feeding practices from 9 to 24 months of age, considering infant and young child feeding (IYCF) indicators and food processing. The infant feeding practices in children from the Brazilian site of the MAL‐ED study were evaluated at 9 (n = 193), 15 (n = 182) and 24 months (n = 164) using 24‐h dietary recalls. IYCF indicators were evaluated, and the extent of food processing was evaluated, using the NOVA classification. Breastfeeding declined significantly over time, from 77.6% at 9 months to 45.1% at 24 months. Although dietary diversity did not significantly change during the study period (80.5% at 24 months), the minimum acceptable diet significantly increased from 67.9% to 76.1% at 24 months (p < 0.0005). All the studied children consumed sweetened beverages from 9 months. Unhealthy food consumption and zero vegetable or fruit consumption significantly increased over time (p < 0.0005). Unprocessed food consumption decreased from 9 to 24 months of age (p < 0.0005), while ultra‐processed food consumption increased (p < 0.0005) during the study period. Logistic regressions showed that, at 9 months, breastfed children presented a lower risk for ultra‐processed food consumption (odds ratio [OR] = 0.31; 95% confidence interval [CI] = 0.13–0.77); and children reaching the minimum acceptable diet presented more risk for ultra‐processed food consumption (OR = 2.31; 95% CI = 1.01–5.27). In conclusion, data showed a reduction in the quality of infant feeding practices over the first 2 years of life, with a decrease in breastfeeding and an increase in the consumption of unhealthy and ultra‐processed foods. Key messages: Data showed a reduction in the quality of infant feeding practices over the first 2 years of life.Breastfeeding decreased over the studied period. There was an increase in the consumption of ultra‐processed and unhealthy foods, which occurred concomitantly to the increase of no consumption of fruits/vegetables.Breastfeeding was an important protective factor against a greater consumption of ultra‐processed food.National nutrition policy should consider promoting breastfeeding and increasing fresh and minimally processed food consumption in children under 2 years. Background: Medical adhesives are used to affix components to the skin. They are part of procedures performed by medical specialties because of their participation as constituents of different products, such as tapes, dressings, and electrodes. Aim: This study aims to assess the prevalence of, and factors associated with, the development of medical adhesive‐related skin injuries (MARSIs) in patients treated with medical tapes in the neonatology department of a large teaching hospital in Brazil. Study Design: Cohort study. Methods: All premature newborns (gestational age from 28 to <37 weeks) admitted in the neonatal intensive care unit of a teaching hospital, from March to August 2019, were followed up. Neonate skin condition was assessed based on the Neonatal Skin Condition Scale (NSCS). Data analyses were conducted in R software. Results: In total, 46 premature newborns were included in the study; 552 evaluations were performed—mean of 11.7 per patient. Most neonates (n = 41; 89.1%) used adhesive tapes, either paper tape (n = 37; 80.4%) or transparent film dressing (n = 34; 73.9%). Newborns' face and head were the most affected body regions (n = 125; 50.2%). Eight patients had MARSIs (19.5% of patients who used tape). NSCS scores (P value <.001) and the adopted warming system (P value =.01302) were associated with the occurrence of MARSIs. Incubators seem to be a protective factor for MARSI (OR = 0.048; IC95% = 0.0008‐0.75; P value =.013). Conclusion: Adhesive tapes in premature newborns should be considered a risk factor for injuries. Although NSCS showed mild‐to‐moderate impairment and lesion severity was low, this event is relatively frequent in neonatal units. Relevance to Clinical Practice: Awareness of the risk associated with adhesive tape application and removal in newborns allow health services to better address the problem by enforcing good practices, elaborating better protocols, qualifying the health care professionals, and potentially selecting softer tapes for neonates. Objective: Diarrhea still causes high morbidity and mortality in children under five, requiring constant interventions. Thus, the study aims to evaluate the effects of educational technologies on maternal self-efficacy to prevent childhood diarrhea. Methods: Randomized clinical trial carried out in Fortaleza, Ceará, Brazil, with four randomized groups of mothers of children under 5 years of age, as follows: control group (n = 61), group A (booklet, n = 60), group B (video, n = 60), and group AB (booklet and video, n = 60). From June to October 2015, data collection was carried out in three moments (the first in person and the others by telephone monitoring). Results: All groups improved their self-efficacy after the intervention, with higher scores in the group AB (booklet and video). A statistically significant association was found between the occurrence of diarrhea and maternal self-efficacy in groups A (booklet) (p = 0.023) and AB (booklet and video) (p = 0.042) at the second moment of data collection. From the second moment to the third moment, the risk of diarrhea decreased in group A, from 12.8 to 1.3, and in group AB, from 8.5 to 1.1. Conclusion: The technologies used, isolated or combined, were effective in improving maternal self-efficacy and decreasing the occurrence of childhood diarrhea; therefore, they can be used by nurses as health education tools. Background: Dietary patterns can influence maternal and child health outcomes. The study aims to characterize dietary patterns during pregnancy as well as to identify their associations with gestational weight gain (GWG) and anthropometric measurements at birth. Methods: A minimum sample size of 95 nursing mothers was estimated for this work. Socioeconomic, anthropometric, physical activity, obstetric and food consumption (food frequency questionnaire) data was collected from mothers and their newborns in immediate postpartum (n = 260) at the maternity hospital. Maternal pregestational weight and GWG were self-reported and the neonatal data was obtained from their records. The patterns were derived posteriori by Principal Component Analysis (PCA). Multinomial Logistic Regression and Poisson Regression with Robust Variance were applied. Findings: Four dietary patterns were derived: "pattern 1", characterized by the consumption of meat and eggs, processed meat, vegetables and olive oil; "pattern 2", consisting of sweets, snacks and cookies; "pattern 3", which includes cereals and breads, coffee and tea, and processed fats; and "pattern 4", characterized by soft drinks, tubers and instant noodles. Together, the dietary patterns account for 49.16% of the total variance for food intake. Higher adherence to "pattern 3" was associated with inadequate birth weight (low and excessive birth weight); while greater adherence to "pattern 2" was associated with lower chances of inadequate GWG. Conclusion: The dietary patterns presented mixed composition and predominance of ultra-processed foods. They were associated with the investigated outcomes, denoting the importance of strategies to promote healthy eating habits during pregnancy in order to avoid possible complications. This study aimed to compare the prevalence of food consumption assessment coverage in children under 10 years old before and after the implementation of the Healthy Growth Program (HGP). In this sense, a cross-sectional study was conducted using data from 548 municipalities registered in the HGP and available in the Food and Nutritional Surveillance System of the Brazilian Ministry of Health. The food consumption assessment coverage indicator was analyzed per geographic regions, a Chi-square test was performed and the Prevalence Ratios (PR) and their respective 95% confidence intervals (95% CI) were estimated. Food consumption assessment coverage increased from 0.25% in 2015 to 1.78% in 2018 (PR=7.09; 95%CI 7.00;7.18). The Southern region showed the highest and the Midwestern region the lowest increase in food consumption coverage. Although food consumption assessment coverage has increased less than the five-percent target set by the HGP, there has been an improvement in monitoring the nutritional status of children, which requires developing actions that support mapping the food and nutritional scenario of children in Brazil. Objective: to identify the frequency of reported cases of recurring violence against adolescents and their association with victim, violence, and aggressor characteristics. Method: cross-sectional study conducted with notified data on violence against adolescents, produced by Epidemiological Surveillance and registered in the Information System of Diseases and Notification (SINAN), from 2011 to 2018, in the state of Espírito Santo, Brazil. Results: the frequency of recurring violence against adolescents was 46.4%. Higher occurrence was observed among girls (PR: 1.26; 95%CI: 1.15 -- 1.38) between 10 and 14 years of age (PR: 1.20; 95%CI: 1.13 -- 1.28), and people with a disability or disorder (PR: 1.52; 95%CI: 1.42 -- 1.62). Psychological violence/neglect was 30% more prevalent in recurrence than self-harm. Most cases occurred at home (PR: 1.56; 95%CI: 1.37 -- 1.77). Results showed a 1.11 times higher prevalence of recurring violence perpetrated by aggressors aged 20 years or older and higher evidence in male aggressors (95%CI: 0.97 -- 1.17). Background Prevalence of cardiometabolic risk factors is increasing and vitamin D insufficiency/deficiency has become a worldwide public health problem, even in tropical countries. Therefore, we identified the prevalence of hypertriglyceridemic waist phenotype (HWP) and evaluate its relationship with vitamin D insufficiency/deficiency. Methods A cross-sectional study with 378 children aged 8 and 9 enrolled in all urban schools in the city of Viçosa, MG, Brazil. Anthropometric measurements, body composition (dual energy X-ray absorptiometry), biochemical tests and clinical evaluation were performed. Poisson regression was used to analyze the association between vitamin D and HWP. Results Prevalence of HWP was 16.4%. This prevalence was higher among children with vitamin D insufficiency and deficiency and in those with a greater number of other cardiometabolic risk factors. Multiple regression analysis showed that children with vitamin D insufficiency and deficiency had, respectively, prevalence 85% (95% CI: 1.03–3.30) and 121% (95% CI: 1.11–4.45) higher of HWP than the vitamin D sufficiency group. Conclusion Vitamin D insuffiency and deficiency were associated with a higher prevalence of HWP among children, regardless of the presence of other cardiometabolic risk factors, indicating an additional risk of inadequate vitamin D status to cardiometabolic health in childhood. The World Health Organization recommends deworming to reduce soil‐transmitted helminth (STH)‐attributable morbidity in women of reproductive age, including pregnant and lactating women, to reduce blood loss, iron deficiency anaemia and nutrient malabsorption. This study assessed the impact of maternal postpartum deworming with albendazole approximately 1 day after delivery on infant milk intake among a subset of 216 randomly selected mother–infant pairs recruited into a large trial in Peru. Infant milk intake was measured using the deuterium‐oxide method at 1‐ and 6‐month postpartum. Maternal STH infection was measured at 6‐month postpartum. At 1‐month postpartum, mean intake was 756 ± 16 and 774 ± 18 mL day−1 in the albendazole and placebo groups, respectively (mean difference: −18 mL day−1; 95% CI: −65, 30). At 6‐month postpartum, mean intake was 903 ± 16 and 908 ± 18 mL day−1 in the albendazole and placebo groups, respectively (mean difference: −5 mL day−1; 95% CI: −52, 43). There was no statistically significant difference in milk intake between groups at either time point. At 6‐month postpartum, mothers infected with Trichuris trichiura had infants with higher milk intakes (adjusted mean difference: 70 mL day−1; 95% CI: 20, 120) compared with uninfected mothers. However, there was no statistically significant difference in infant milk intake between mothers who had moderate‐and‐heavy intensity infection compared with the comparison group (mothers with no and light intensity infection). A lower prevalence and intensity of infection, and inclusion of uninfected mothers in both arms of the trial, resulting in effect dilution, may explain the null findings. [ABSTRACT FROM AUTHOR]

Objective: To know the profile of newborns hospitalized in the Intermediate Neonatal Care Unit and associate maternal and neonatal factors with the length of hospitalization. Methods: Cross-sectional, documentary research, carried out in a university hospital in the South Region of Brazil, with 384 newborns hospitalized. A descriptive analysis and the chi-square test were performed. Results: Most hospitalizations occurred in the early neonatal period, of male babies, white ethnicity, gestational age 36 weeks, weight 2,690g, Apgar score 6.77 in the first and 8.49 in the fifth minute, and average days of hospitalization 8.49. Prolonged period of hospitalization was associated with maternal intercurrence, prematurity, low birth weight, male gender, use of antibiotics and weight gain. Conclusion: Reaffirms the importance of knowing the causes of hospitalization of newborns and understanding the profile of illness in the neonatal period, to subsidize public policies and quality assistance to reduce hospitalization and perinatal morbidities. We examined whether reported higher frequencies of anxiety and depression symptoms are related to the presence of developmental coordination disorder in school-age Brazilian children. A total of 272 children were assigned to six groups according to age and motor performance. The Movement Assessment Battery for Children-Second Edition evaluated the motor performance. The Spence Children's Anxiety Scale and the Child Depression Inventory assessed anxiety and depression symptoms, respectively. Brazilian children are at high risk for anxiety, regardless of motor performance and age. However, children with developmental coordination disorder report significantly more depressive symptomatology in 10–12 years compared to typically developing children. [ABSTRACT FROM AUTHOR]

Background: Obtaining peripheral venous access, especially in children, is usually a challenging procedure for nursing professionals. In pediatric patients this is because of the small caliber of the vessels, deep localization in the tissue, and multiple previous attempts of venipuncture. This experiment compared the success in obtaining venous access in pediatric patients with the use of near-infrared light and with the conventional technique. Method: True experiment, developed in a large private general hospital and a public general hospital under the same management. The study included children aged 0 to 6 y (including newborns and pediatric patients up to 5 y, 11 mo and 29 d) under clinical or surgical treatment, undergoing peripheral venous puncture both for laboratory test collection and for infusion of medications and solutions. A total of 144 punctures were analyzed, 51.4% with the conventional technique and 48.6% using the device with near-infrared light. Results: The success for the conventional technique was 89.2%, and 97.1% on the technique with near-infrared light, P = 0.098, with no evidence of significant differences. Conclusion: The success rate for obtaining peripheral venous access in children was similar in the study groups. The aim of this study was to evaluate the association of oral parafunctions with the presence of signs and symptoms of craniomandibular disorder in children with cleft lip and palate (CLP). A cross-sectional study was performed in which one hundred and eighty-eight children with CLP and absence of associated syndromes or malformations were investigated. During clinical examination, the presence of wear facets and dental occlusion were observed and palpation was performed in the temporomandibular joint region (TMJ) and orofacial musculature. The children and their guardians were interviewed regarding the presence of oral parafunctional habits (bruxism, the act of biting the lips and/or cheeks, nail biting, sucking of fingers and pacifiers) and signs and symptoms of craniomandibular dysfunction (headache, ear and/or neckache, tiredness or crack in the TMJ during mouth opening and chewing movements). The data were analyzed by means of absolute and relative frequency measurements, and bivariate analysis using Pearson's chi-square test and likelihood ratio test were performed, considering the significance level of 5%. A higher prevalence of cleft lip and palate type (76.0%) and the presence of malocclusion (61.7%) was observed, highlighting the anterior crossbite (58.7%) and midline deviation (48.9%). There was a higher occurrence of teeth grinding (40.4%), onychophagy (28.7%) and biting lips and/or cheeks (28.7%). Onychophagia was associated with headache (p <0.05) and with pain or cracking during mouth opening (p <0.05). This study aimed to analyze the independent associations of accelerometer-determined sedentary behavior, physical activity, and steps/day with body composition variables in Brazilian children. 485 children wore accelerometers for 7 days. Variables included time in sedentary behavior and different physical activity intensities (light, moderate, vigorous, or moderate-to-vigorous) and steps/day. Body fat percentage was measured using a bioelectrical impedance scale, and BMI was calculated. Children spent 55.7% of the awake portion of the day in sedentary behavior, 37.6% in light physical activity, 4.6% in moderate physical activity, and 1.9% in vigorous physical activity. Moderate-to-vigorous physical activity and steps/day were negatively associated with body composition (BMI and body fat percentage) variables, independent of sex and sedentary behavior. Beta values were higher for vigorous physical activity than moderate physical activity. Vigorous physical activity was negatively associated with BMI (β. 1425) and body fat percentage (β-.3082;p < .0001). In boys, there were significant negative associations between moderate, vigorous, and moderate-to-vigorous physical activity and steps/day with body composition, and in girls, there was only a negative association with vigorous physical activity, independent of sedentary behavior. Moderate-to-vigorous physical activity and steps/day (in boys), but especially vigorous physical activity (in boys and girls), are associated with body composition, independent of sedentary behavior. Purpose: Child participation in type 1 diabetes (T1D) self‐care is needed in developing countries due to a lack of resources, especially during the school day. This pilot study evaluated the feasibility of a therapeutic play intervention (ITP) versus standard education (SE) on the ability of children with T1D to correctly perform insulin injection technique. Design and Methods: Children with T1D (7–12 years) were recruited at two diabetes clinics in Brazil and randomized to ITP or SE. Registered nurses received protocol training to deliver the intervention and perform data collection. ITP group received an education that included a story about a child with T1D who self‐injects insulin at school; SE group received routine clinic‐based education. Preintervention, children were video‐recorded giving insulin injections to a doll; postintervention, children were rerecorded giving the doll an injection. The research team reviewed the videos and assessed the injection technique using validated checklists. Parents reported children's self‐injection practices at baseline and 30 days. Results: Children (N = 20, 40% male) were 9.6 ± 1.3 years old and had T1D for 3.6 ± 2.3 years; HbA1c was 9.1 ± 2.0%; 20% of ITP and 50% of SE children used syringes (vs. pens) for injections. At baseline, 80% of both groups knew how to self‐inject; most were taught by a parent/relative. Injection technique scores were low in both groups; ITP group increased their scores significantly postintervention. Practices of self‐injection did not change in either group after 30 days. Practice Implications: The play‐based intervention appeared to improve the injection technique in the short‐term. Background & Aim: To investigate the association between the genetic polymorphisms FokI (rs2228570) and BglI (rs739837) in vitamin D receptor (VDR) with dental caries and gingivitis susceptibility. Design: This study included 353 Brazilian children (8 to 11 years old). Dental caries was assessed using ICDAS (International System for Detection and Assessment of Carious Lesions) and gingival bleeding using Community Periodontal Index (CPI). The presence of visible biofilm was also evaluated. DNA was extracted from saliva, and real‐time PCR was used to evaluate genetic polymorphisms in VDR: rs2228570 (FokI, A>G/Met>Thr) and rs739837 (BglI, G>T). Dental caries was evaluated as a continuous data (mean and standard deviation‐SD) and was also categorized (ICDAS0versus ICDAS1‐6 or ICDAS1‐2versus ICDAS3‐6). Gingivitis was categorized in with and without. One‐way ANOVA was used for comparisons of caries among genotypes. Chi‐square test, logistic regression, and haplotype analysis were performed (P <.05). Results: Biofilm was associated with dental caries susceptibility and gingivitis (P <.05). The mean distribution of the caries lesions and cavitated caries lesions among FokI and BgII genotypes were not statistically significant (P >.05). Genotype distributions among caries groups (in the two different cut‐offs) and among gingivitis and non‐gingivitis groups were not statistically significant (P >.05). Background: Studies that establish the indicators of clinical status deterioration of Ineffective airway clearance remain scarce. Prognostic studies provide data for nurses identify clinical indicators that suggest a higher chance of early development of a nursing diagnosis. Objective: To identify the prognostic indicators of short-term survival of ineffective airway clearance (IAC) in children with acute respiratory infection (ARI). Design: A prospective open cohort study. Methods: This study was with a group of 136 children with acute respiratory infection who were followed for a minimum of six and a maximum of ten consecutive days. Children who had not completed six days of monitoring or who were carriers of diseases that would alter the specific symptoms of respiratory infection were excluded. The survival rate of ineffective airway clearance was calculated using Nelson-Aalen's method. A Cox regression model was used to analyze the influence of clinical indicators on survival time of this diagnosis. Results: The diagnosis survival rate was extremely low (only 0.4% on the 5th day of follow-up). The greatest reduction in survival rate was observed in the first 48 h (survival rate = 2.5%). Four defining characteristics associated with a worse prognosis of IAC among children with ARI: ineffective cough (RR = 5.86; 95% CI: 3.53–9.72), absence of cough (RR = 2.92; 95% CI: 1.68–5.08), adventitious breath sounds (RR = 2.47; 95% CI: 2.01–3.03), and diminished breath sounds (RR = 1.23; 95% CI: 1.05–1.45). Conclusion: Four clinical indicators showed a strong relationship with an increased risk of worsening clinical status associated with this nursing diagnosis. Impact statement: Clinical deterioration related to ineffective airway clearance among children with acute respiratory infection is fast and requires particular attention from nurses. Aim: To investigate the validity and reliability of the Portuguese version of the Test of Infant Motor Performance (TIMP) for Brazilian infants. Methods: The study was conducted with 20 health professionals and 655 Brazilian infants (between 34 weeks of Post-Menstrual Age and 17 weeks Corrected Age). Results: (1) Content validity: High concordance among the experts, for language clarity and pertinence, were found (ICC from.74 to.100; AC1 from.87 to 1.00); (2) Inter-rater, intra-rater, and test-retest reliability: Strong inter (α from.65 to.99) and intra (α from.97 to.99) raters agreement, and high test-retest reliability (r = 1.00; ICC = 1.00); (3) Internal consistency: Excellent internal consistency (α from.71 to.98); (4) Discriminant validity: Significant power to identify groups at-risk for delays (p <.0001); (5) Predictive validity: Capability to predict motor delay (r.55 to.89) was found; and, (6) Concurrent validity: Significant (p <.05) and low correlations at the first months (τ.21 to.24) and weak to moderate correlations at 3 and 4 month-old (τ.42 and.44, respectively) were observed between the TIMP and Alberta Infant Motor Scale. This study aimed to examine the association between breastfeeding and childhood obesity. A multinational cross‐sectional study of 4,740 children aged 9–11 years was conducted from 12 countries. Infant breastfeeding was recalled by parents or legal guardians. Height, weight, waist circumference, and body fat were obtained using standardized methods. The overall prevalence of obesity, central obesity, and high body fat were 12.3%, 9.9%, and 8.1%, respectively. After adjustment for maternal age at delivery, body mass index (BMI), highest maternal education, history of gestational diabetes, gestational age, and child's age, sex, birth weight, unhealthy diet pattern scores, moderate‐to‐vigorous physical activity, sleeping, and sedentary time, exclusive breastfeeding was associated with lower odds of obesity (odds ratio [OR] 0.76, 95% confidence interval, CI [0.57, 1.00]) and high body fat (OR 0.60, 95% CI [0.43, 0.84]) compared with exclusive formula feeding. The multivariable‐adjusted ORs based on different breastfeeding durations (none, 1–6, 6–12, and > 12 months) were 1.00, 0.74, 0.70, and 0.60 for obesity (Ptrend =.020) and 1.00, 0.64, 047, and 0.64 for high body fat (Ptrend =.012), respectively. These associations were no longer significant after adjustment for maternal BMI. Purpose: To evaluate oral inclusion cysts in newborns and their association with birth weight, preterm birth, newborn health, maternal health, prenatal factors, and socioeconomic factors. Methods: The sample consisted of 411 pairs of mothers and their newborns born at a university hospital in Brazil. The oral inclusion cysts, diagnosed by clinical examination, were Epstein pearls, Bohn's nodules, and dental lamina cysts. Medical records and a questionnaire were used to collect data related to newborns and mothers. Data were analyzed using the chi-square, linear by linear, and Fisher's exact tests for bivariate analysis, while Poisson regression with robust variance was used for multivariate analysis (α=five percent). Results: The prevalence of inclusion cysts was 56.7 percent. A total of 42 (62.7 percent) low birth weight (LBW) newborns, 192 (56.0 percent) normal weight newborns, 38 (63.3 percent) preterm newborns, and 187 (56.7 percent) full-term newborn had inclusion cysts. Epstein pearls (n=164, 39.9 percent) were more prevalent and associated with prematurity (P=0.025), LBW newborns (P=0.033), and mothers who worked during pregnancy (P=0.019). Bohn's nodules (n=80, 19.9 percent) were more prevalent in newborns of mothers who did not receive welfare support (P=0.004). Dental lamina cysts occurred in 5.6 percent (n=23) of the sample and were not associated with independent variables. Conclusion: Epstein pearls were more common in LBW and preterm infants. A large number of children of depressed mothers have one or more mental disorders. This study aimed to evaluate the impact of maternal depression on the mental health of 4–5-year-old children of adolescent mothers, according to the hypotheses generated from the model of accumulation. Between October 2009 and March 2011, all pregnant adolescents who received prenatal care from the public health system in Pelotas (southern Brazil) were invited to participate in the study and have been prospectively followed. Of these individuals, 413 participants were evaluated in the postpartum period and when the child was 2–3 years old and 4–5 years old (current stage). The Strengths and Difficulties Questionnaire was used to assess mental health problems in children, and the Mini International Neuropsychiatric Interview (MINI)-Plus version was used to assess maternal depression. We applied a structured modeling approach to examine the relations between three different hypothesized life course models (accumulation, critical period, and mobility) and maternal depression. After selecting the most appropriate model, we used a logistic regression analysis to assess the effect of depression on mental health problems in 4–5-year-old children of adolescent mothers. We used the Chi square test to estimate the prevalence of mental health problems in 4–5-year-old children. The longer the time of exposure to maternal depression, the greater the probability that the child would present behavioral problems. Aim: Parental screening of children's motor skills can be helpful for early identification of Developmental Coordination Disorder (DCD). The present study examined the associations between the Developmental Coordination Disorder Questionnaire – Brazilian version (DCDQ-BR) and motor competence in school-age children. Methods: 707 children (332 boys, 375 girls) aged between 6- to 10-years and one of their parents participated in the study. Parents completed the DCDQ-BR and children's motor competence was determined using the Bruininks-Oseretsky Test of Motor Proficiency – 2nd edition (BOT-2). Results: The agreement between the BOT-2 and DCDQ-BR results was comparable across age, ranging from 74-84 percent. In general, there were low- to moderate correlations between all aspects of the parent report and children's motor competence. Sensitivity and specificity of the DCDQ-BR were 70% and 81%, respectively. Overall, boys had higher motor competence than girls, but parent reports were similar for gender. Conclusions: Parental assessment of their child's motor ability is moderately associated with motor competence, and this association is similar for boys and girls, even though boys scored higher in motor competence. Our findings suggest that it may be possible to reduce the length of clinical assessment by only testing children that flag as suspect for DCD in the DCDQ-BR. [ABSTRACT FROM AUTHOR]

Objective: to know the prevalence of occupational accidents in children and youth who work with their families in the rural environment and to identify the associated factors. Method: exploratory, descriptive and analytical study with quantitative approach, developed in three rural areas. Participants were 211 children and young people who assisted the family in rural work. Data collection was performed using a semi-structured questionnaire. Bivariate analysis was performed using Pearson's chi-square, Fisher's exact, Student's t and Mann-Whitney tests and multivariate analysis using Poisson regression. Results: the prevalence of self-reported occupational accidents was 55%. It was highlighted: insect bites (44%), burns (40.5%), falls (27.6%), injury with a working tool (16.4%), electric shock (15.5 %), burn by animal (8.6%), animal bite (6.9%) and pesticide poisoning (2.6%). These were related to shared housing, leisure activity - riding a motorcycle, product resulting from lettuce cultivation and use of personal protective equipment. Conclusion: it is believed that these findings may enhance the development of public policies aimed at preserving the health of these children and young people, regulate working conditions and reduce occupational risks in the rural environment. Introduction. Children and adolescents with physical disabilities (PD) still do not have the same opportunities as their non-disabled peers, which may be due to environmental factors and their effect on participation. Objective: The aim of this study was to evaluate the prevalence of environmental barriers in children and adolescents with PD and their associated factors in two states of Northeastern Brazil. Method. The parents or caregivers of 71 children and adolescents aged 18 years and younger with PD were interviewed, using a sociodemographic questionnaire and the Craig Hospital Inventory of Environmental Factors (CHIEF), which assesses the frequency and extent of environmental barriers. Results. The greatest environmental barriers were for Service and Assistance. In the Service and Assistance and Physical/Structural domains, the frequency of barriers was monthly, while Attitude and Support and Policies barriers were less frequent. In all the domains, the parents reported that the problems were serious. Children and adolescents with PD who walked faced barriers more frequently than those who used a wheelchair or were carried and those living in Rio Grande do Norte had more perceived barriers in the Policies domain. Conclusion. These results lead to a reflection on the role of the environment and how each state conducts national public policies aimed at children with disabilities. Aims: To evaluate the effects of a 16-week program based on Goals-Activity-Motor Enrichment (GAME) principles on infants with congenital Zika virus syndrome (CZS)'s mother report of functional goal achievement, motor and cognitive abilities, home enrichment, and parents' perceptions regarding the service provided. Methods: Quasi-experimental study with infants (n = 32) with CZS and their mothers. Twenty-two infants composed the GAME-based group and 10 were included in the control group. The primary outcome measure was the Canadian Occupational Performance Measure. Secondary outcome measures were the Bayley Scales of Infant and Toddler Development, the Affordances in the Home Environment for Motor Development-Infant Scale, and the Measure of Processes of Care. All measures were performed by blinded assessors. Results: Mothers of infants in the GAME-based group reported significant improvements in their infants' performance on functional priorities (p = 0.0001) and satisfaction with their infants' performance (p = 0.0001), the extent in which services promoted enabling and partnership (p = 0.021), provided general information (p = 0.039), specific information (p = 0.0001), and an enriched home environment (p = 0.0001). Infants in both groups did not improve in motor or cognitive abilities. Conclusions: A family-centered early intervention program based on GAME principles improved mothers' individualized outcomes and enriched infants with CZS's environment.
