Amaç: Araştırma, hemodiyaliz tedavisi alan hastaların fistül bakımı ve korunması ile ilgili bilgi ve davranışlarını belirlemek amacı ile planlandı. Gereç ve Yöntem: Tanımlayıcı ve kesitsel tipte olan bu araştırma 02 Eylül-12 Kasım 2019 tarihleri arasında bir üniversite hastanesinin hemodiyaliz ünitesinde yürütüldü. Araştırma 61 hasta ile tamamlandı. Veriler, hastaların sosyodemografik ve hastalık özelliklerini içeren 13 soru vehastaların fistül bakımı ve korunması konusunda bilgi ve davranışları ölçen 25 sorudan oluşan anket formları ile toplandı. Araştırmaya başlamadan önce etik kurul ve kurum izinleri alındı. Araştırmanın verileri yüz yüze görüşme yöntemiyle toplandı. Sosyodemografik verilerin sayı, yüzdelik ve aritmetikortalamaları alındı. Bilgi ve davranış anket formlarından elde edilen puanların değerlendirilmesi için One-Way ANOVA veIndependent-Samples t Testi kullanıldı. Veriler SPSS 22.0 paketprogramı kullanılarak değerlendirildi. The data was collected with a questionnaire consisting of 13 questions including the sociodemographic and disease characteristics of the patients, and 25 questions measuring the knowledge and behaviors of the patients about fistula care and protection. - Abstract
The progress in technological and scientific fields in the 20th century has transformed and shaped the lives of people remarkably. In healthcare, technology can be described as the application of the methods, systems, medicines, devices, vaccines and organized information to improve the quality of life by providing solutions for the needs of both professionals and patients. Besides its potential of reducing fragmentation and costs while increasing the safety, utilization of technology can be considered as the key to achieving a safe, sustainable, and person-centered healthcare environment. Virtual Reality (VR) is a computer-generated three-dimensional interactive environment that stimulates multiple sensory methods. Considering the fact that VR is one of the crucial technological trends of this century, it has the potential of affecting the health industry until 2025 extremely. It is a real-time graphic simulation that includes visual, auditory, and tactile experiences. In the field of health, VR is used for multiple purposes as training of medical students for critical surgeries, education of nursing candidates, and treatment of some diseases. As essential members of the healthcare team, nurses should be a strict follower of the rapidly evolving technology to provide efficient and quality care. The utilization of VR in nursing education has the advantage of decreasing faulty interventions, which enhances patient safety as well as the quality of the healthcare services in the future.

6. First Clinical Practice Experiences of Nursing First Class Students in Palliative Care

Amaç: Bu araştırmanın amacı,hemşirelik bölümü birinci sınıf öğrencilerinin palyatif bakım ünitesindekideneyimlerini incelemektir.Yöntem: Çalışma, nitel araştırmayöntemleri kapsamında yer alan fenomenoloji deseni kullanılarakgerçekleştirilmiştir. Araştırmanın örneklemini, araştırmaya katılmayı kabuleden öğrencilerden amaçlı örnekleme yöntemlerinden olan benzeşik (homojen)örnekleme yöntemi kullanılarak belirlenmiş yedi öğrenci oluşturmuştur.Çalışmanın gerçekleştirilmesi için gerekli izinler alınmıştır. Araştırmaverilerinin toplanmasında “Öğrenci Tanıtım Formu” ve “Yarı YapılandırılmışGörüşme Formu” kullanılmıştır. Görüşmeler ortalama 45 dakika sürmüştür.Görüşmelerin ses kayıtları araştırmacılar tarafından çözümlenmek üzerebelgelenmiş, elde edilen verilere onları açıklayabilecek kavramlara veilişkilere ulaşmak amacıyla araştırmacı tarafından içerik analizi uygulanmıştır.Bulgular: Araştırma kapsamına alınanyedi hemşirelik birinci sınıf öğrencisinden dördü kız, üçü erkektir ve yaşortalaması 20,57’dir. Verilerin içerik analizi doğrultusunda “hastayailişkin deneyimler”, “hemşirelik girişimlerine ilişkin deneyimler”, “yaşama ilişkindeneyimler” ve “hemşirelik mesleği algısına ilişkin deneyimler” olmak üzeredört tema belirlenmiştir. Temalar doğrultusunda ise “korku, umutsuzluk,ümitsizlik, öfke, zorluk, acı çekme, zamanın değerliliği, umut aşılama, meslekiözerkliğin anlamı, hemşire anahtar rol, insan onuru koruyucusu” olmak üzere 11alt tema saptanmıştır.Sonuç: Hemşirelik öğrencilerinin ilkklinik deneyimlerini yaşadıkları palyatif bakımda hasta kavramına vemesleklerine ilişkin yaşadıkları deneyimlerden, hastaların bağımlılık düzeylerininyüksek oluşu ve hastaların içinde bulundukları tedavi ve bakım sürecininhassasiyetine ilişkin düşünsel derinlik yaşadıkları ortaya konmuştur., Objective: The aim of this study was todetermine the experience of the first year nursing students in the palliativecare unit.Methods: The study was carried outusing the phenomenology design included in the qualitative research methods.The sample of the study consisted of seven students determined by usinghomogeneous sampling method which is one of the purposeful sampling methods.Necessary permits were obtained to carry out the study. “Student InformationForm” and “Semi-Structured Interview Form” were used to collect the data.Interviews lasted an average of 45 minutes. The audio recordings of theinterviews were analyzed by a researchers and content analysis was conducted bythe researcher in order to reach the concepts and relations that could explainthe data obtained.Results: Of the seven nursing firstyear students, four were female and three were male and the mean age was 20.57.According to the content analysis of the data, four themes that “experiencesrelated to patient”, “experiences related to nursing interventions”,“experiences related to life” and “experiences related to perception ofnursing”. In the direction of themes, 11 sub-themes were determined that“fear”, “hopelessness”, “despair”, “anger”, “difficulty”, “suffering”, “valueof time”, “instil hope”, “meaning of professional autonomy”, “nurse key role”,“human dignity protector”.Conclusion: It was revealed that nursingstudents experienced their first clinical experiences in the palliative care,the experiences they experienced in the concept of the patient and theirprofessions, and the diminishing depth related to the holistic and humanisticaspect of nursing.

Introduction: The purpose of this study is to determine behaviours of healthy life style of the old people with chronic disease. Methods: The study was carried out with old people aged 60 above who can perform daily life activities independently and having at least one of the diagnoses of cancer, hypertension and diabetes, staying at a residential and nursing home (n:304). Data were collected by use of old people interview form and scale of Healthy Life Style Behaviours-II (HLSB-II). Results: Age average of the individuals involved in the study is 78.9±7.6 and 67.1% were women. 5.3% of old people (n:16) had cancer, 26% (n:79) were DM, 49.7% (n:151) were HT patient and 19.1% (n:58) had more than one chronic disease. GIRIS ve AMAC: Calisma, hemsirelik egitim mufredatinda ilk defa uygulanmaya baslayan intornluk programina iliskin olarak, dorduncu sinif ogrencileri ile daha once intorn ogrencilik sisteminin olmadigi ve uygulamanin yapildigi bir hastanenin ilgili kliniklerinde gorev yapan hemsirelerin goruslerini belirlemek amaciyla yurutulmustur. YONTEM ve GERECLER: Tanimlayici tipteki bu calisma hemsirelik bolumu 4.sinifinda ogrenim goren intornluk programi uygulanan 101 ogrenci ve intornluk programinin uygulandigi hastanede, intornler ile birlikte calisan 100 hemsire ile yurutulmustur. Verilerin toplanmasinda literaturden faydalanilarak arastirmacilar tarafindan, ogrenci ve hemsireler icin ayri birer form olarak olusturulan Intornluk Degerlendirme Formu kullanilmistir. Verilerin degerlendirilmesi tanimlayici testler ile yapilmistir. BULGULAR: Ogrencilerin %96,4’u, intornluk uygulamalarinin hemsirelik meslegini tanimasina ve uyum saglamasina, %89,3’u el becerilerinin artmasina, %82,1’i sorumluluk duygusunun ve iletisim becerisinin gelismesine, %81’i kendine olan guveninin artmasina ve ekip calismasini ogrenmesine katki sagladigini belirtmistir. Hemsireler ise, intornluk uygulamalarinin ogrenciler acisindan, el becerilerinin (%97,9) ve teorik bilgilerinin (%91,8) artmasina, meslegi tanimalarina ve uyum saglamalarina (%97,9), iletisim becerilerinin gelismesine (%95,9), kendilerine guvenlerinin artmasina (%93,8) ve ekip calismasini ogrenmelerine (%91,8) katki sagladigini ifade etmistir. Ogrencilerin %72,6’si, hemsirelerin ise %98’i intornluk uygulamasinin devam etmesini istemektedir. TARTISMA ve SONUC: Hemsirelik egitiminde ogrencilerin mesleki becerilerinin gelismesine katki saglayan intornluk uygulamalarinin, ogrenci ve hemsirelerden goruslerinin alinarak duzenlenmesi, intornluk programinin daha etkin ve verimli olmasi acisindan onemlidir.

Amac: Bu arastirmanin amaci hemsirelerin bireysel ve profesyonel degerleri arasindaki kusaklararasi farkliliklari belirlemektir. Gerec ve Yontem: Tanimlayici ve karsilastirmali tipteki bu calisma, Izmir’deki bir universite hastanesinde calisan, X kusagindaki 85(dogum tarihi 1965-1979 yillari arasinda olanlar) ve Y kusagindaki 79 (dogum tarihi 1980-1999 yillari arasinda olanlar) olmak uzere toplam 164 hemsire ile yurutulmustur. Veriler, arastirmacilar tarafindan 30 dakikalik sure icerisinde yuz yuze gorusme yontemi ile Birey Tanitim Formu, Portre Degerler Anketi ve Hemsirelerin Profesyonel Degerleri Olcegi-Revize kullanilarak toplanmistir. Verilerin degerlendirmesinde tanimlayici istatistikler, Ki Kare, Kruskal Wallis, Mann-Whitney U testi ve spearman korelasyon analizi kullanilmistir. Bulgular: Arastirmaya katilan hemsirelerin %51.8’i X kusagi, %48.2’si ise Y kusagidir (n=164). X kusagindaki bireylerin %96.4’u kadin olup, %52.9’u universite mezunu, %72.6’si evli; Y kusagindaki bireylerin ise %93.7’si kadin, %51.9’u universite mezunu, %58.2’si evlidir ve bu ozellikler acisindan X ve Y kusagi arasinda istatistiksel olarak fark yoktur. X kusagindaki hemsirelerin Portre Degerler Anketi ve Hemsirelerin Profesyonel Degerler Olcegi puan ortalamalari arasindaki fark istatistiksel olarak anlamli degildir. Calismaya katilan X ve Y kusagindaki hemsirelerin Portre Degerler Anketi ile Hemsirelerin Profesyonel Degerleri Olcegi puan ortalamalari arasinda pozitif yonde anlamli bir iliski oldugu belirlenmistir. Sonuc: Calismada, X ve Y kusagindaki hemsirelerde bireysel ve profesyonel degerler acisindan fark olmadigi belirlenmistir. 11. An analysis of the cultural problems encountered during caregiving by the nurses working in two different regions of Turkey

The aim of this study was to identify the cultural problems encountered during caregiving by the nurses working in two university hospitals located in western and eastern Turkey. This descriptive, comparative study was conducted between July 2008 and October 2009 with 338 nurses who volunteered to take part in the study. The study data were collected using an individual description questionnaire consisting of 10 questions and another questionnaire consisting of 14 questions to identify the cultural problems encountered by nurses when giving care. The study showed with respect to training received on transcultural nursing that only 59 nurses had this training, but the percentage was higher in the nurses working at the hospital in the west (54.2%) (P > 0.05). It was found that a large number of nurses in the sample group (n = 286) gave care to at least one individual from another culture, but the percentage was significantly higher in the nurses working in the west (56.7%) than in the nurses working in the east (43.3%) (P 0.05). The problem experienced in this area was mostly because of the fact that patients 'did not speak Turkish' (63.8%). In conclusion, the nurses gave care to patients from different cultures, and most of them had trouble when giving care to patients from different cultures.

12. Investigation into the use of complementary and alternative medicine and affecting factors in Turkish asthmatic patients

Aims. The purpose of this study was to examine the frequency of complementary and alternative medicine usage in asthmatic patients living in the west of Turkey, the most frequently used complementary and alternative medicine methods and socio-demographic factors affecting this and factors related to the disease. Background. While the rate of complementary and alternative medicine usage in asthmatic patients and the reasons for using it vary, practices specific to different countries and regions are of interest. Differing cultural and social factors even in geographically similar regions can affect the type of complementary and alternative medicine used. Methods. Two hundred asthmatic patients registered in the asthma outpatient clinic of a large hospital in Turkey and who had undergone pulmonary function tests within the previous six months were included in this study, which was planned according to a descriptive design. The patients filled out a questionnaire on their demographic characteristics and complementary and alternative medicine usage. Results. The proportion of patients who reported using one or more of the complementary and alternative medicine methods was 63·0%. Of these patients, 61·9% were using plants and herbal treatments, 53·2% were doing exercises and 36·5% said that they prayed. The objectives of their use of complementary and alternative medicine were to reduce asthma-related complaints (58%) and to feel better (37·8%). The proportion of people experiencing adverse effects was 3·3% (n = 4). Factors motivating asthmatic patients to use complementary and alternative medicine were the existence of comorbid diseases and a long period since diagnosis (p 0·05). Relevance to clinical practice. Understanding by nurses of the causes and patterns of the use of complementary and alternative medicine in asthmatic patients helps them in directing patient care and patient safety. Nurses should conduct comprehensive diagnostics in the light of complementary and alternative medicine use, and they should be aware of the potential risks.

International Nursing Review58, 181–185 Background: The aims of this multi-centre cross-sectional survey were to detect Turkish nursing students' level of knowledge on viral hepatitis, to evaluate their rates of exposure to blood and to find out their hepatitis A (HAV), hepatitis B (HBV) vaccination status. Methods: This multi-centre cross-sectional study was conducted in 14 nursing schools located in the seven geographical regions of Turkey. A questionnaire composed of 47 questions on socio-demographic factors, level of knowledge on HAV, HBV, hepatitis C (HCV) immunisation status, exposure history and attitudes was applied to the study group. Results: A total of 1491 third- and fourth-year nursing students participated with an 89% response rate. The mean age of the participating students was 21.4 ± 1.3. Their mean knowledge score was 23.7 ± 4.6 (71/100) and was significantly higher in fourth-year students than third-year students. There were significant differences in mean scores among nursing schools. Among the participants, 85.3% had received HBV vaccine and 9.1% had received HAV vaccine. The percentage of students who signify themselves at increased risk of acquiring viral hepatitis was 97.3%. Of the students, 28.1% had sustained a needle-stick injury and 5.4% had experienced conjunctival exposure to blood. Conclusions: Curriculum differences among nursing schools have a significant effect on nursing students' level of knowledge. For this reason, nursing schools should organize HBV immunisation programmes for their students to increase vaccination coverage.

Aim. This study aims to determine the relationship between disability levels and self care agency scores in Turkish patients with rheumatoid arthritis and to investigate the factors affecting them. Background. Self-care agency is the complex acquired ability to meet one's constant requirements for care that regulates life processes, maintains or promotes the integrity of human structure, functioning and development, and promotes well-being. Design and methods. Forty-three consecutive patients with Rheumatoid Arthritis (M/F: 11/32; mean age 49·53 ± 13·37 years; disease duration 118·32 ± 76·55 months), being followed up by Ege University Rheumatology Outpatient Department and all fulfilling ACR 1987 criteria were included in this study. Data collection instruments used in this study were Informative-Demographic Data Form, Health Assessment Questionnaire (including disability index and pain scale) and Self-as-Carer Inventory. Results. The mean disability index, the mean pain score and the mean self-care agency score of patients were 0·53 (SD 0·45), 1·48 (SD 0·74) and 80·95 (SD 27·80) were respectively. The purpose of this study was to examine the complementary and alternative medicine (CAM) use in patients with rheumatoid arthritis living in different regions and cities of Turkey as well as the factors affecting the use of CAM modalities. Planned as a descriptive and cross-sectional study, this research was conducted as a study covering the rheumatology units of 10 university and 4 state hospitals in Turkey. A total of 594 patients meeting the research inclusion criteria comprised the sample. It was detected that 46.9% (n = 279) of the patients with rheumatoid arthritis whose average disease duration was 10.32 ± 9.05 years used CAM modalities. The most common 3 modalities used by these patients were herbs taken orally, nutritional supplements, and mind-body therapies, with rates of 54.5%, 41.2%, and 40.5%, respectively. It was determined that such variables as age, sex, marital status, education status, and economic situation did not affect the use of CAM (P > .05).

The Memorial Symptom Assessment Scale (MSAS) is a multidimensional tool developed to evaluate measure the prevalence, characteristics and distress of common symptoms related to cancer. A validated Turkish version has now become available. The aim of this study was to evaluate its reliability and validity.One hundred-twenty patients were included into this study. The MSAS, The Rotterdam Symptom Checklist (RSCL), and Karnofsky Performance Status Scale (KPSS) were used for data collection. Content and criterion validities were examined. Reliability analyses of the MSAS were performed using internal consistency reliability and test-retest reliability.The most frequently reported symptom (90%) was problems with sexual interest or activity. Item-total correlations ranged between 0.03 and 0.64. There was a high correlation between total MSAS and the RSCL (r=0.875, p0.01). The internal consistency reliabilities of subscales of the MSAS and total MSAS were moderately high, with Cronbach alpha coefficients ranging from 0.71 to 0.84. The MSAS's test - re-test reliability was 0.78.The MSAS for cancer patients was determined to be a valid and reliable instrument for the use in the Turkish population. It is recommended that the MSAS-Turkish version can be used as a tool for comprehensive symptom assessment in planning nursing care for cancer patients.

This study examined the effectiveness of a case management (CM) intervention in the care of patients with rheumatoid arthritis (RA) as a pilot study in a teaching hospital in Turkey. Two groups were compared with respect to disability, quality of life, cost, and patient satisfaction: RA patients who received CM plus usual nursing care and RA patients who received usual nursing care alone. All patients underwent follow-up interviews at 3 and 6 months after being discharged from the hospital. Disability scores were significantly better in the RA group receiving CM, but there were no significant differences between the two groups with regard to quality of life, patient satisfaction, and total healthcare costs. Using CM in the care of patients with RA may favorably affect disease-related outcomes.

The aims of this study were to investigate the effects of pain management education on the intensity of pain and frequency of utilization of pain management methods in two groups of patients with arthritis of different pathogenesis and clinical features, and to compare whether a significant difference existed between the two groups. The study was carried out between September 2007 and June 2008 on 30 female patients with gonarthrosis and 30 female patients with rheumatoid arthritis (RA) followed at the rheumatology outpatient clinic of a university hospital. Data on sociodemographic characteristics and those related with the illness were collected using a special survey. Each patient was given information about the features, causes, and treatment of the arthritis and how to cope with pain, emphasizing the importance of pain management methods. The intensity of pain and efficacy of pain management methods were assessed using the McGill Pain Questionnaire and the Pain Management Inventory at baseline and the second and sixth weeks after the education. The SPSS (v15.0) statistical package was used for statistical analysis. After education, significant improvements in pain intensity scores compared with baseline scores were observed in both groups (p < .05), and there was no significant difference between the RA and gonarthrosis groups. Among the various pain management methods, the education program led to significantly more utilization of massaging the painful area, exercising, and using complementary methods to control stress in both groups of patients, and there was no significant difference between the groups. In conclusion, the pain management education given in this study alleviated the intensity of pain and significantly increased the use of some pain management methods in both gonarthrosis and RA cases.

21. [Evaluation of anxiety and depression levels in patients with myocardial infarction]

Patients with myocardial infarction (MI) may experience anxiety and depression. This study was designed to determine anxiety and depression levels in patients with MI.The study included 506 patients (199 women, 307 men; mean age 55.7+/-6.9 years) who were admitted to the cardiology department for MI. Data were collected using a questionnaire to identify patient characteristics and the Hospital Anxiety and Depression Scale (HADS). The HADS has been validated as a sensitive screening tool for anxiety and depression in MI patients in the Turkish population.Clinically severe anxiety was found in all the patients (100%). Depression scores showed a clinically normal level in 45 patients (8.9%), borderline level in 289 patients (57.1%), and severe level in 172 patients (34%). The mean HADS score was 11.4+/-2.9, being 12.2+/-4.1 and 10.6+/-4.1 for anxiety and depression, respectively. Among patient characteristics, age was significantly associated with both anxiety and depression subscales (p0.001), and education level (p0.05), total family income (p0.01), health insurance (p0.05), and the number of myocardial infarctions (p0.05) were significantly associated with the depression subscale. In regression analysis, age (beta=-0.128), education level (beta=0.082), and working status (beta=-0.79) independently affected both anxiety and depression, and total family income (beta=-0.128) and health insurance (beta=-0.086) significantly affected depression.Our data suggest that planning nursing interventions to decrease anxiety and depression levels and implementation of cardiac rehabilitation programs are of particular importance in patients with MI.

Complementary and alternative medicine (CAM) therapies such as herbal therapy, acupuncture, yoga, chiropractic, relaxation techniques, nutrition and dietary supplements, continue to gain popularity as modalities for treatment of asthma. A number of study in this area has shown frequently used CAM therapies by patients with asthma. It has found that parents who have children with asthma preferred commonly CAM therapies such as massage, relaxation techniques, diet and vitamin and that adult patients with asthma used commonly nutrition and diet supplements, herbal therapies and homeopathy. Because of many studies had weak methodology and small sample; it hasn't exposed entirely effectiveness of CAM therapies in asthma treatment. In this respect, it is required qualitative researchs with strong methodology. Furthermore; it is suggested that health professionals have to much knowledge about CAM therapies in asthma and they have to an active role in development guidelines related to CAM interventions.
