Purpose This study aims to focus on the avoidance and anxiety attachment patterns among unaccompanied refugee minors (URMs) after resettlement in Norway. The authors explored the extent of stability and change in these attachment patterns and the role of demographic and interpersonal predictors of change in levels of attachment insecurity. Design/methodology/approach Three waves of data were analyzed with latent growth curve modeling. The sample consisted of 918 refugees who had arrived in Norway as unaccompanied minor asylum seekers and were granted residence. Of the initial sample, 82% were male and the mean age was 18.6 years (SD = 2.61). Findings Results revealed substantial stability in attachment patterns: anxiety patterns did not change over time, while avoidance patterns increased, but only marginally. Research limitations/implications A large sample of URMs revealed rather stabile attachment patterns over time, underlining the need for social support that fosters their adjustment processes best. More research is needed to clarify the mechanism underlying the stability and change in attachment patterns and to understand better how these young people can be supported emotionally during their resettlement process in the absence of parental care. Social implications Host societies could perhaps invest more in promoting successful and sustainable adult relationships for unaccompanied refugees approaching the age of 18 years. This may help them to cope better with acculturation stressors and negative past experiences and may thereby improve their future mental health and social relationships. Originality/value Targeting competencies to develop and maintain close relationships may be particularly important for URM’s, because this may alleviate risks for mental health related to past and current stressors as well as loneliness. To the best of the authors’ knowledge, the present study is the first to investigate stability and change in attachment insecurity among unaccompanied refugees.

In deze studie beschrijven we een single-case experimental design (SCED) onderzoeksprotocol. Daarvoor deden we een onderzoek naar de effectiviteit en mogelijke mechanismen van de Resolutions Approach (RA), een multidisciplinaire interventie voor het stoppen van kindermishandeling en voor het bevorderen van veiligheid binnen het gezin. Vanwege de heterogeniteit van de populatie en het nieuwe onderwerp, maakten we een opzet voor een SCED, met een baselineperiode (fase A) en een behandelperiode (fase B). De deelnemersgroep bestaat uit vijftien gezinnen met kinderen tussen acht en achttien jaar, waarbij door meer dan een informant specifieke signalen van kindermishandeling zijn gemeld. RA bestaat uit twintig sessies en deze interventie wordt aangeboden binnen de context van individu, gezin en sociaal netwerk. Metingen voor primaire uitkomsten (incidenten van kindermishandeling) en secundaire uitkomsten (emotionele en gedragsproblemen van de kinderen, ouderlijke stress, band tussen ouder en kind) vinden plaats bij aanvang van de baselineperiode, bij het begin en einde van de behandeling en bij de twee follow-ups. Gepersonaliseerde, idiosyncratische metingen van de belangrijkste problemen binnen het gezin vinden wekelijks plaats. Aan het einde van de behandeling wordt met het gezin een kwalitatief interview afgenomen, waarin wordt gekeken wat de krachtigste verandermechanismen en behandelcomponenten waren. Tijdens bijeenkomsten met het sociaal netwerk worden de ervaren veiligheid gemeten, en de openheid over kindermishandeling. Met deze studie komen we onzes inziens tegemoet aan de vraag van ggz-medewerkers naar meer evidence-based interventies voor kindermishandeling. Beperkingen van de studie, zoals vertekeningen in de rapportage van ouders over kindermishandeling, worden ook besproken.

BackgroundA growing body of literature shows profound effects of the COVID-19 pandemic on mental health, among which increased rates of posttraumatic stress disorder (PTSD) and adjustment disorder (AD). However, current research efforts have largely been unilateral, focusing on psychopathology and not including well-being, and are dominated by examining average psychopathology levels or on disorder absence/presence, thereby ignoring individual differences in mental health. Knowledge on individual differences, as depicted by latent subgroups, in the full spectrum of mental health may provide valuable insights in how individuals transition between health states and factors that predict transitioning from resilient to symptomatic classes. Our aim is to (1) identify longitudinal classes (i.e., subgroups of individuals) based on indicators of PTSD, AD, and well-being in response to the pandemic and (2) examine predictors of transitioning between these subgroups.Methods and analysisWe will conduct a three-wave longitudinal online survey-study of n ≥ 2000 adults from the general Dutch population. The first measurement occasion takes place six months after the start of the pandemic, followed by two follow-up measurements with six months intervals. Latent transition analysis will be used for data-analysis.Ethics and disseminationEthical approval has been obtained from four Dutch universities. Longitudinal study designs are vital to monitor mental health (and predictors thereof) in the pandemic to develop preventive and curative mental health interventions. This study is carried out by researchers who are board members of the Dutch Society for Traumatic Stress Studies and is part of a pan-European study (initiated by the European Society for Traumatic Stress Studies) examining the impact of the pandemic in eleven countries. Results will be published in peer-reviewed journals and disseminated at conferences, via newsletters, and media-appearance among (psychotrauma-)professionals and the general public.Strengths and limitations of this studyThis is one of the first studies examining the mental health impact of the COVID-19 pandemic by focusing on negative and positive mental health in the general population.A longitudinal research design is used, which enable us to examine predictors of transitioning between mental health classes over three time points.A limitation of this study is that we used self-report measures, instead of clinical interviews, to assess mental health.

The present study examined the relationship between cultural identity conflict and psychological well-being, as well as the role of self-concept clarity and self-esteem in mediating this linkage. Elevated cultural identity conflict was hypothesized to be associated with lower psychological well-being via both (lower) self-concept clarity and (lower) self-esteem, with self-concept clarity preceding self-esteem. In a cross-sectional design, 473 bicultural young adults (age range, 18-35) completed an online questionnaire assessing cultural identity conflict, self-concept clarity, self-esteem, emotional distress, psychopathological symptoms, and satisfaction with life. Correlation analyses revealed that elevated cultural identity conflict was positively associated with emotional distress and psychopathological symptoms, and negatively associated with satisfaction with life. Mediation analyses indicated that these associations were mediated by lower self-concept clarity and lower self-esteem. The results support the importance of interventions that foster the development of skills in bicultural young adults to obtain more self-concept clarity and promote self-esteem and psychological well-being.

Families provide children with crucial surroundings of social support, acknowledgment, and hope, creating a protective shield. When a family is confronted with adversities and long-lasting stress and faces adjustment difficulties, familial support may become inadequate, and the family environment is a source of additional stress for the child. It is therefore important to find ways to preserve and strengthen family support in times of difficulties. How do families adapt to disruptive circumstances and changed environments? What is known about families adapting to crises and turmoil? In which way can families be supported when confronted with stress and adversity? We will begin with a theoretical and conceptual overview of how to understand the experience of refugee families. Then we will discuss practical family-centered programs, such as multifamily groups that can foster adjustment during and after severe upheaval. Such groups have been applied in low-income as well as high-income countries with different types of populations.

Child maltreatment (CM) is highly prevalent and one of the most injurious conditions that children may experience. Because it is often-clandestine, it is difficult to assure that existing prevention and treatment mechanisms reach those in need. CM's consequences may take a long time to be recognized and expressed. We discuss the need to increase public health actions when the person reaches adulthood to mitigate the consequences of CM. We propose three intervention targets to alleviate CM-related difficulties in adults: emotion regulation, social functioning, and self-concept. By enhancing awareness and facilitating access to these interventions and using technology-based strategies, we expect this approach to benefit affected persons. It may also reduce the risk for transmission of CM consequences across generations.

Het is onbekend wat werkt voor minderjarige vluchtelingen in Nederland, als het gaat om traumabehandeling. De narratieve exposure therapie is speciaal ontwikkeld voor vluchtelingen, en EMDR is een bekende traumabehandeling. Zouden deze ook goed werken bij kinderen en jongeren die naar Nederland zijn gevlucht, met alle stress van dien? Die vraag is onderwerp van onderzoek.

Behandelingen van complexe problematiek doen vaak een ingewikkeld beroep op behandelaren, waardoor er in de therapeutische relatie allerlei lastige dynamieken kunnen ontstaan. In dit hoofdstuk wordt beschreven hoe de combinatie van de noodzakelijke compassie en noodzakelijke professionele distantie kan leiden tot onnodig lange behandelingen doordat de behandelaar zijn ‘weke hart’ te veel laat spreken. In dit hoofdstuk wordt ook beschreven wat de niet-functionele voordelen zijn voor zowel client als behandelaar om behandelingen onnodig lang te laten duren. Bij de behandeling van complexe problematiek is het onvermijdelijk dat behandelaren af en toe in deze lastige dynamiek terechtkomen. Dat is niet erg, mits een behandelaar maar kritisch naar zichzelf en zijn eigen handelen in de therapiesituatie kan kijken en regelmatig zijn behandelingen evalueert met de client en met collega’s in een multidisciplinair team.

In dit hoofdstuk worden verschillende psycho-educatieve interventies beschreven. Psycho-educatie over de problematiek gebeurt altijd aan het begin van een behandeling. Tussendoor kan het ook goed zijn om uitleg te geven over psychische verschijnselen. In dit hoofdstuk bespreken we ook het belang dat behandelaar en client uitgaan van een gelijkluidend ‘Verklarend Model’. Een aantal psycho-educatieve interventies worden beschreven: uitleg geven aan de hand van het modusmodel uit de gedragstherapie, uitleg over ‘kwetsbaarheid’, ‘toen’ en ‘nu’, controle, schuldontlasting van ouders door kinderen en niet kunnen afleren. Ook een aantal metaforen wordt besproken: zoals de metafoor van de bruiloft en van de kettingbotsing om traumafenomenen te verklaren. Afgesloten wordt met het benoemen van de retraumatiseringsdriehoek. Dit model wordt uitgebreid uitgelegd in het volgende hoofdstuk over emotieregulatie.

In dit hoofdstuk worden de richtlijnen voor de behandeling van angststoornissen en persoonlijkheidsstoornissen van het Trimbos-instituut besproken. Er wordt een poging gedaan beide richtlijnen te integreren. De rest van het hoofdstuk wordt besteed aan het uiteenzetten van handvatten voor de behandeling van complexe problematiek. Onder complexe problematiek verstaan we mengvormen van persoonlijkheidsstoornis en trauma en andere ernstige stoornissen. De handvatten bestaan uit (1) goede diagnostiek, (2) een goede theorie over de samenhang van symptomen, diagnostiek, levensloop en hulpvraag als kapstok van hulpverlening, (3) consensus/compromis met de client over diagnostiek, behandeldoelen en -methoden, (4) overzicht over verschillende behandelinterventies in overzichtgevend plan, (5) behandelfocus voor zover mogelijk op verwerking van trauma’s/schokkende gebeurtenissen, (6) de adequate therapeutische relatie en oppassen voor het ‘weke hart’. In de volgende hoofdstukken worden deze handvatten verder besproken.

In dit hoofdstuk is beschreven hoe belangrijk het is om bij complexe problematiek overzicht te bewaren over de interventies en traumagericht te behandelen. Op het driefasenmodel, dat hiervoor gebruikt werd, is de laatste tijd veel kritiek geweest. We introduceren daarom het model van de ‘interventiecirkel’ om dit overzicht te bewaren. De interventiecirkel bestaat uit zes segmenten, waarvan het centrale segment het verwerkingssegment is. Daarnaast zijn er het psycho-educatiesegment, het emotieregulatiesegment, het praktische-zakensegment en het integratiesegment. In de volgende hoofdstukken worden de verschillende segmenten van de interventiecirkel beschreven.

In dit hoofdstuk wordt beschreven waarom het belangrijk is om bij complexe problematiek aan het begin van de behandeling de verworven informatie in een overzichtelijk model onder te brengen, zodat op grond daarvan beredeneerde keuzes voor een behandelinterventie gemaakt kunnen worden. Er zijn binnen de cognitieve gedragstherapie verschillende modellen bekend waarin de samenhang tussen de psychische klachten, coping en de levensloop geordend wordt. Hier worden verschillende namen voor gebruikt: probleemsamenhang, holistische theorie of casusconceptualisatie. In dit hoofdstuk wordt een aantal van deze modellen beschreven en met een voorbeeld geillustreerd; de holistische theorie vanuit de klassieke gedragstherapie, de cognitieve casusconceptualisatie volgens Beck, de cognitieve casusconceptualisatie volgens de schematherapie, het door Sprey beschreven SORC-model en een hybride model. Er wordt geen voorkeur voor een bepaald model uitgesproken, want daarvoor is onvoldoende wetenschappelijke evidentie. Benadrukt wordt wel dat het belangrijk is dat een behandelaar een van deze modellen gebruikt.

In dit hoofdstuk bespreken we de mogelijkheden en beperkingen van de therapeutische relatie bij de behandeling van (vroegkinderlijk) trauma bij persoonlijkheidsproblematiek. In de manier waarop de client de therapeutische relatie aangaat, zal zich zijn problematiek herhalen. De therapeutische relatie kan zowel faciliterend werken, wat beoogd wordt door behandelaar en client, maar kan ook een frustrerend en belemmerend effect op de behandeling hebben. Dat wordt niet alleen bepaald door de overdracht van de client op de behandelaar, maar ook door de tegenoverdracht van de behandelaar op de client. Het werken met clienten met vroegkinderlijke traumatisering is soms emotioneel belastend. Dit kan leiden tot verschillende soorten problemen. Er worden verschillende functies en aspecten van de therapeutische relatie besproken: de voorwaardenscheppende functie, een constructivistische functie, omgaan met de problematische overdracht en omgaan met de belasting van de behandelaar. Het hoofdstuk wordt afgesloten met valkuilen voor de behandelaar per persoonlijkheidsstoornis.

De definitie van het begrip trauma en het begrip persoonlijkheid worden verkend en uitgediept. Bij persoonlijkheid worden verschillende perspectieven besproken: het perspectief van de karaktertrekken, het biologische perspectief, het intrapsychische perspectief, het cognitieve en experientiele perspectief en het perspectief van coping. Persoonlijkheidsstoornis wordt gedefinieerd, en de verschillende persoonlijkheidsstoornissen zoals die in de DSM-5 genoemd staan, worden uitgebreid beschreven. De dimensionele benadering van persoonlijkheidsstoornissen, zoals die in de DSM-5 wordt voorgesteld, wordt geschetst. Afgesloten wordt met een weergave van de overeenkomsten tussen persoonlijkheidsstoornis en complex trauma.

Recente studies tonen steeds sterkere evidentie voor de relatie tussen ouderlijk trauma, de ouder-kindinteractie en de ontwikkeling van het kind. In dit artikel wordt de behandelmethodiek Multifamily Therapy (mft) voor multi-probleemgezinnen gepresenteerd en de toepassing in de populatie ernstig getraumatiseerde asielzoeker- en vluchtelingengezinnen met jonge kinderen beschreven. Bij deze doelgroep worden relatieproblemen en ontwikkelingsproblematiek geconstateerd. De behandeling is dan ook gericht op het versterken van ouderlijke competenties en hechtingsrelaties. Een eerste evaluatie van deze behandelmethode wordt beschreven middels dossierstudie en een klinische indruk van de evaluatiegesprekken met de deelnemers. Deze indrukken laten een positief beeld zien: bij afsluiting van de behandeling is de kwaliteit van de ouder-kindinteractie subjectief verbeterd en is de psychopathologie bij kinderen verminderd. Behandeling met mft bij vluchtelingen- en asielzoekersgezinnen met complexe problematiek na traumatisering is goed uitvoerbaar en veelbelovend, maar verdient verder onderzoek.

Er bestaat onder clinici veel onduidelijkheid over wat de concepten complex trauma en complexe posttraumatische stressstoornis (PTSS) behelzen. Dit heeft ongewenste consequenties voor de behandeling, omdat de behandelrichtlijnen voor complexe PTSS afwijken van die voor PTSS. In dit artikel wordt verhelderd wat er onder complex trauma en complexe PTSS verstaan wordt, om zo bij te dragen aan een meer gefundeerd gebruik van deze termen. Complex trauma wordt gekenmerkt door elementen als herhaling, langdurigheid, interpersoonlijke context en verstoring van ontwikkelingsfasen. Complexe PTSS is niet als afgebakende classificatie opgenomen in het ‘Diagnostic and statistical manual of mental disorders’ (DSM) en kan niet als diagnose zonder PTSS gesteld worden. Het is van belang complex trauma en complexe PTSS te specificeren in een beschrijvende diagnose. Daarnaast is het belangrijk er niet automatisch van uit te gaan dat patienten met complex trauma wel complexe PTSS zullen hebben; bij veel patientengroepen is dit niet onderzocht of zijn de prevalenties relatief laag.

Beneficial effects of attentional bias modification have been claimed for a number of anxiety disorders, but study results are variable. A recent trial in patients with posttraumatic stress disorder (PTSD) showed no therapeutic effects. The use of personally relevant and verbal stimuli might increase the efficacy of attentional bias modification. In an A-B case series design, we hypothesized that individualized attentional bias modification would lead to reduction of attentional bias and a decrease in PTSD symptoms. Six Dutch male war veterans (mean age 39.33 years) who had developed PTSD after peacekeeping missions underwent the treatment. No therapeutic effects were observed. Inter- and intraindividual attentional bias scores varied widely and did not respond to attentional bias modification as hypothesized. This study provides no evidence that individualized attentional bias modification is an effective treatment for PTSD.

Many refugees resettled in Western countries suffer from an accumulation of traumatic and current stressors that contribute to mental health problems and may complicate trauma-focused treatment. Consequently, the acceptability, safety, and efficacy of trauma-focused treatment with refugees have been a matter of clinical and scientific interest. In recent years, the evidence has accumulated for narrative exposure therapy and culturally adapted cognitive behavioral therapy. Although eye movement desensitization and reprocessing (EMDR) is practiced with resettled refugees, only five small studies of limited quality have been conducted on EMDR with this population. In the absence of strong evidence, therapists practising EMDR with refugees may be aided by transcultural psychiatric principles, especially matching of explanatory models. In addition, high-quality research is needed to reliably determine acceptability, safety, and efficacy of EMDR with traumatized refugees.

Maternal traumatization has been proposed as a risk factor for child development, but the mechanisms involved are poorly understood. This study analyzed the interrelations among maternal posttraumatic stress symptoms, parent-child interaction (emotional availability), and infants' psychosocial functioning and development among 49 asylum-seeker and refugee mothers and their children (18-42 months). Measures included assessment of mothers' trauma and comorbid symptoms (Harvard Trauma Questionnaire: R.F. Mollica et al., 1992; Hopkins Symptom Checklist: L. Derogatis, R. Lipman, K. Rickels, E. Uhlenhuth, & L. Covi, 1974), emotional availability within parent-child interaction (Emotional Availability Scales: Z. Biringen, 2008), and infants' psychosocial functioning (Child Behavior Checklist: T.M. Achenbach & L.A. Rescorla, 2000) and development (Bayley Scales of Infant Development: B.F. van der Meulen, S.A.J. Ruiter, H.C. Spelberg, & M. Smrkovsky, 2000). The results show that higher levels of maternal posttraumatic stress symptoms are associated with a higher level of psychosocial problems of infants, but not with delays in their mental or psychomotor development. The results also show that higher levels of maternal posttraumatic stress symptoms are associated with higher levels of insensitive, unstructuring, or hostile, but not intrusive, parent-child interactions. Infants show lower levels of responsiveness and involvement to their traumatized mothers. Parent-child interaction did not function as a mediator between maternal trauma symptoms and infants' psychosocial functioning. Results are discussed in relation to the dyad's regulation of emotions. Results implicate a need to reestablish attunement between traumatized mothers and their nontraumatized children.

Parents and children together: Multifamily therapy for refugees with complex trauma symptoms

‘On the move’ – state of the art regarding mental health care for traumatized ethnic minorities

In de praktijk van de GGZ wordt bij meervoudige klachten na traumatiserende gebeurtenissen de term ‘complex trauma’ (of complexe posttraumatische stressstoornis [PTSS of DESNOS]) veelvuldig gebezigd. Dit is echter een warrig construct en het gebrek aan helderheid komt de indicatie of rationale voor behandeling veelal niet ten goede. Er zijn goed onderzochte behandelingen voor de posttraumatische stressstoornis voorhanden (Foa, Keane, Friedman & Cohen, 2008; Nice, 2005), maar die volstaan slechts ten dele voor de behandeling van meervoudige en onderling samenhangende klachten bij mensen die chronisch, meervoudig en interpersoonlijk geweld hebben meegemaakt (Cloitre, 2009; Crumlish & O’Rourke, 2010; Palic & Elklit, 2010). In deze bijdrage beschrijven wij een aanpak die als best practice kan worden beschouwd. Achtereenvolgens gaan we in op een conceptuele kwestie: wat verstaan we onder ‘complex trauma’ en waarin is het anders dan enkelvoudige posttraumatische stressstoornis (PTSS) of ‘simpel trauma’, welke diagnostische instrumenten zijn er voorhanden om complex trauma vast te stellen? Vervolgens worden aan de hand van het driefasenmodel verschillende behandeltechnieken globaal besproken.

Stichting Centrum ’45 is een zorginstelling die zich richt op de behandeling van psychotraumaklachten als gevolg van oorlog, vervolging en geweld. Het gaat om clienten met eerste- en tweede-generatieproblematiek, oude en jonge veteranen en vluchtelingen, maar ook clienten met beroepsgerelateerde traumaproblematiek. Bij al deze doelgroepen zijn regelmatig ook persoonlijkheidsproblemen aanwezig die binnen onze instelling steeds meer aandacht vragen.In dit artikel betogen we hoe in onze visie moet worden omgegaan met de comorbiditeit van trauma- en persoonlijkheidsproblematiek. Eerst gaan we in op de behandeling van psychotraumaklachten en op onze keuze van een persoonlijkheidsmodel voor de behandeling van persoonlijkheidsproblematiek. Door beide benaderingen op elkaar te betrekken komen we tot een integratief model voor de indicatiestelling en behandeling van zowel trauma als persoonlijkheidsproblematiek. Dit leidt tevens tot de conclusie dat een op het individu gerichte aanpak de beste is.

The meaning of threatening events: A questionnaire on assigning meaning after experiences of war and violence The Meaning of War Scale has been developed to examine cognitive assumptions related to threatening experiences specific to war or violence. To obtain the psychometric properties of this new scale, questionnaires filled out by two different samples have been analyzed: 1. refugees from Bosnia Herzegovina living in the Netherlands (N=365), and 2. Dutch veterans sent to different war and peacekeeping missions (N=1561). Factor analysis and construct validity were used to examine psychometric qualities of the Meaning of War Scale. Three out of five sub-scales showed good reliability and validity: distrust, personal growth and religious adherence. Moreover, these scales have been found to be related to stress responses and dispositional features of the cognitive adaptation process to war-related experiences.

De getuigenismethode is een therapievorm die is ontwikkeld op grond van de ervaringen met mensen die slachtoffer zijn geworden van georganiseerd politiek geweld. De client wordt begeleid bij het afleggen van een getuigenis: dit is een hoofdzakelijk chronologisch verhaal over de persoonlijke levensloop met nadrukkelijke aandacht voor de specifieke traumatische ervaringen. In dit artikel wordt ingegaan op de psychotherapeutische kracht van de getuigenismethode, afgezet tegen andere perspectieven op de behandeling van psychotrauma.

The Impact of Event Scale (IES; M. J. Horowitz, N. Wilner, & W. Alvarez, 1979) is a worldwide-used self-report measure to assess the frequency of intrusive and avoidant phenomena after a variety of traumatic experiences. The purpose of this article is to assess the psychometric value of the Dutch version of the IES (D. Brom & R. J. Kleber, 1985) in several samples of individuals who had experienced various traumatic stressors. The reliability and structure of the IES were evaluated in 3 different samples (total N 1,588). The reliability of the Dutch version of the IES was adequate across the various stressors. The construct validity was assessed by using confirmatory factor analyses. Outcomes revealed a robust structure over the various samples, generally supporting the composition of the original IES.

‘Behandeling na trauma’ is een uit het Engels vertaalde gids voor de behandeling van symptomen van traumatische stress. Het is geent op de cognitieve gedragstherapie en is tevens sterk gebaseerd op de neurofysiologie van de gevolgen van trauma.

Background:This study examines late consequences of war and migration in both non-clinical and clinical samples of child survivors of World War II. This is one of the very few studies on the mental health of children who were subjected to internment in camps, hiding, and violence under Japanese occupation in the Far East. It provides a unique case to learn about the significance of experiences of war and migration in later life.Methods:Long-term sequelae of the Japanese persecution in the Dutch East Indies (DEI) in child survivors were studied by analyzing sets of standardized questionnaires of 939 persons. Instruments dealt with post-traumatic responses, general health, and dissociation. Participants were recruited through community services and registers of clinical services. Discriminant analyses were conducted to evaluate the significance of early experiences in determining group belonging.Results:Compared with age-matched controls that lived through the German occupation in the Netherlands during World War II, the child survivors from the DEI reported both more trauma-related experiences and mental health disturbances in later life. In particular, the number of violent events during the war, among which especially internment in a camp, contributed to the variation among groups, in support of the significance of these disruptive experiences at older age.Conclusion:The results underline the long-term significance of World War II-related traumatic experiences in the population of elderly child survivors who spent their childhood in the former DEI.

Background: Traumatised asylum seekers and refugees are clinically considered a complex population. Discussion exists on whether with this population treatment guidelines for post-traumatic stress disorder (PTSD) should be followed and Trauma-Focused Cognitive-Behavioural Therapy (TF-CBT) or Eye Movement Desensitisation and Reprocessing (EMDR) should be applied, or whether a phased model starting with stabilisation is preferable. Some clinicians fear that trauma-focused interventions may lead to unmanageable distress or may be ineffective. While cognitive-behavioural interventions have been found to be effective with traumatised refugees, no studies concerning the efficacy of EMDR with this population have been conducted as yet. Objective: In preparation for a randomised trial comparing EMDR and stabilisation with traumatised refugees, a pilot study with 20 participants was conducted. The objective was to examine feasibility of participation in a randomised trial for this complex population and to examine acceptability and preliminary efficacy of EMDR. Design: Participants were randomly allocated to 11 sessions of either EMDR or stabilisation. Symptoms of PTSD (SCID-I, HTQ), depression and anxiety (HSCL-25), and quality of life (WHOQOL-BREF) were assessed at pre- and post-treatment and 3-month follow-up. Results: Participation of traumatised refugees in the study was found feasible, although issues associated with complex traumatisation led to a high pre-treatment attrition and challenges in assessments. Acceptability of EMDR was found equal to that of stabilisation with a high drop-out for both conditions. No participants dropped out of the EMDR condition because of unmanageable distress. While improvement for EMDR participants was small, EMDR was found to be no less efficacious than stabilisation. Different symptom courses between the two conditions, with EMDR showing some improvement and stabilisation showing some deterioration between pre-treatment and post-treatment, justify the conduct of a full trial. Conclusion: With some adaptations in study design, inclusion of a greater sample is justifiable to determine which treatment is more suitable for this complex population. In de laatste fase van de behandeling van complex trauma volgens het driefasenmodel hebben interventies een plaats die gericht zijn op verdergaan met het leven na het trauma. De focus van de behandeling in fase 1 is het stabiliseren van de symptomen, in fase 2 is dat het verwerken van het traumatische verleden en in fase 3 is de focus de toekomst: Hoe kan het leven weer worden opgepakt?

In dit hoofdstuk beschrijven we verschillende groepen clienten bij wie complex trauma kan optreden. De sociaal-maatschappelijke en historische context waarin ernstig geweld zich voordoet, is van belang voor een goede betekenisverlening; in de eerste plaats voor de client, maar ook voor de behandelaar. Enige kennis van de situatie van specifieke clientengroepen is onmisbaar voor het behandelproces.

De eerste fase van het driefasenmodel kent twee hoofddoelen (Van der Hart & Nijenhuis, 2007): (1) het vestigen van een veilige therapeutische relatie en (2) stabilisatie en symptoomreductie. Wanneer deze twee doelen bereikt zijn, kan de behandeling worden voortgezet met fase 2.

In de tweede fase van het driefasenmodel is de behandeling gericht op ‘verwerking’ van de traumatische herinneringen. In dit hoofdstuk worden enige algemene aspecten van verwerking besproken. Ook wordt een beslisboom geintroduceerd waarmee bepaald kan worden of begonnen wordt met globale traumaverwerkingstechnieken of met specifieke verwerkingstechnieken. Beide soorten technieken worden in de volgende hoofdstukken beschreven.
