Abstract

The Sicily Channel is an NW–SE trending area characterized by three primary tectonic depressions (i.e., Pantelleria, Linosa, and Malta basins). Five gravity cores collected in these basins were studied. Through geochemical analyses, the main elements characterizing the sampled sediments were recognized. The normalized values of Ti and K allowed identifying some changes in sediment composition and potential source areas controlling the more recent sedimentation. These source areas were identified as the Pantelleria soil and rocks, Linosa rocks, Sicily stream sediments, Libyan soil and Saharan dust, which differently occur as control factors of the sedimentation in different sectors.

Abstract

Atopic dermatitis (AD) therapeutic approach calls for a long-term treatment. Treatment options for AD have recently undergone a revolutionary change by the introduction of the first biologic drug. Availability in daily practice of the last version of international AD guidelines, taking peculiarities of the country into account, can contribute to good clinical practice in Italy. To adapt European Dermatology Forum (EDF) guidelines for AD to the Italian medical–legal context, the EDF guidelines were assessed independently by two independent Italian renowned experts in the field and further integrated with articles published and systematically reviewed before May 2019. The first draft was collegially corrected and updated by the members of the SIDEMAST, ADOI, and SIDAPA. Recommendation levels (A; B; C; D) were graded based on the evidence levels (1–4). The adapted guidelines presented here focus on topical and systemic therapies in AD patients, both children and adults. As opposed to previous Italian guidelines, they include indications about biologics. New relevant evidence available from very recent literature and peculiarities of the Italian medical and legal context have been integrated in the revision process. If compared to general guidelines for AD not adapted to a specific national and cultural context, a revision for specific Italian needs is now available: It comprises the option of implementing the new biologic treatments and is likely to provide an important contribution to the improvement of clinical practice in Italy. Cooperation between patients, dermatologists, allergologists, and pediatricians remains mandatory in AD management. The authors of the present revision recommend an update of the Italian guidelines to be performed at least every second year.

4. Sedimentological, geochemical and biogeochemical facies of Central and Southern Adriatic Sea

Abstract

The Adriatic Sea is a basin oriented in the NW-SE direction, which represents the result of filling the Apennine and Dinaric-Hellenic chain foredeep. The present surface sediment distribution is controlled by basin evolution, different sediment source areas and processes. The aims of this work were to identify sedimentary facies based on grain-size, geochemical and biogeochemical features and to describe their spatial distribution, to recognize regional patterns of sediment dispersal, geochemical signals for provenance and eventual local features. Grain-size, geochemical (major and trace elements) and biogeochemical (organic carbon) analysis (Droghini, 2017) carried out on 107 samples, collected by box-corer during the PERTRE cruise (Spagnoli, 2016). These data were elaborated with a Q-mode factor analysis to identify the sedimentary facies (Spagnoli et al., 2014). In particular the statistical processing of sediment properties highlighted 5 factors, which allowed characterizing different facies and to evaluate their provenance and surface distribution. In particular were identified: 1) Clayey facies, made up of clayey and clayey-silty sediments, originated from the contribution of the Po River and the Apennine rivers, and that are redistributed by the Adriatic circulation in a SE direction; 2) sandy facies, made up of sandy-silty sediments with silicate minerals and enriched in Magnesium; this facies is more abundant along the Italian coasts; 3) coarse carbonatic facies, made up of sandy-gravel sediments; this facies is more abundant in the outer areas of the Adriatic Basin, coinciding with the depositional areas of the relict sands; 4) silty facies with heavy minerals, characterized by silty sediments enriched in heavy minerals resulting from the action of waves and coastal currents; 5) sediment provenance, useful to identify the origin of sediments. This component shows high values in different areas mainly located offshore of northern Gargano Promontory and is due to sediment features mainly controlled by geochemistry rather than grain-size. The characterization and distribution of the above described facies allowed understanding in more detail the present hydrodynamic, sedimentological, geochemical and biogeochemical processes occurring in the Central and Southern Adriatic Sea, and to identify the main sediment source areas.

Abstract

Aims Patients with acute coronary syndromes (ACSs) who are managed without coronary revascularization represent a mixed and understudied population that seems to receive suboptimal pharmacological treatment. Methods and results We assessed patterns of antithrombotic therapies employed during the hospitalization and in-hospital clinical events of medically managed patients with ACS enrolled in the prospective, multicentre, nationwide EYESHOT (EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units) registry. Among the 2585 consecutive ACS patients enrolled in EYESHOT, 783 (30.3%) did not receive any revascularization during hospital admission. Of these, 478 (61.0%) underwent coronary angiography (CA), whereas 305 (39.0%) did not. The median GRACE and CRUSADE risk scores were significantly higher among patients who did not undergo CA compared with those who did (180 vs. 145, P < 0.0001 and 50 vs. 33, P < 0.0001, respectively). Antithrombotic therapies employed during hospitalization significantly differ between patients who received CA and those who did not with unfractioned heparin and novel P2Y12 inhibitors more frequently used in the first group, and low-molecular-weight heparins and clopidogrel in the latter group. During the index hospitalization, patients who did not receive CA presented a higher incidence of ischaemic cerebrovascular events and of mortality compared with those who underwent CA (1.6 vs. 0.2%, P = 0.04 and 7.9 vs. 2.7%, P = 0.0009, respectively). Conclusion Almost one-third of ACS patients are managed without revascularization during the index hospitalization. In this population, a lower use of recommended antiplatelet therapy and worse clinical outcome were observed in those who did not undergo CA when compared with those who did. Abstract

Aims In the last decades, several new therapies have emerged for the treatment of acute coronary syndromes (ACS). We sought to describe real-world patterns of use of antithrombotic treatments in the catheterization laboratory for ACS patients undergoing percutaneous coronary interventions (PCI). Methods EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units was a nationwide, prospective registry aimed to evaluate antithrombotic strategies employed in ACS patients in Italy. Results Over a 3-week period, a total of 2585 consecutive ACS patients have been enrolled in 203 cardiac care units across Italy. Among these patients, 1755 underwent PCI (923 with ST-elevation myocardial infarction and 832 with non-ST-elevation ACS). In the catheterization laboratory, unfractioned heparin was the most used antithrombotic drug in both ST-elevation myocardial infarction (64.7%) and non-ST-elevation ACS (77.5%) undergoing PCI and, as aspirin, bivalirudin and glycoprotein IIb/IIIa inhibitors (GPIs) more frequently employed before or during PCI compared with the postprocedural period. Any crossover of heparin therapy occurred in 36.0% of cases, whereas switching from one P2Y12 inhibitor to another occurred in 3.7% of patients. Multivariable analysis yielded several independent predictors of GPIs and of bivalirudin use in the catheterization laboratory, mainly related to clinical presentation, PCI complexity and presence of complications during the procedure. Conclusion In our contemporary, nationwide, all-comers cohort of ACS patients undergoing PCI, antithrombotic therapies were commonly initiated before the catheterization laboratory. In the periprocedural period, the most frequently employed drugs were unfractioned heparin, leading to a high rate of crossover, followed by GPIs and bivalirudin, mainly used during complex PCI. Abstract

International audience; Background and PurposeThe tachykinin NK2 receptor antagonist ibodutant is under Phase III clinical investigation to treat female patients with irritable bowel syndrome. The aim of this study was to investigate the NK2 receptor-related gender specificity in a model of colitis. Experimental ApproachColitis was induced by rectal instillation of 2,4,6-trinitrobenzenesulfonic acid (TNBS, 0.5mL, 30mgmL(-1) in 30% ethanol) in female and male guinea pigs. Electromyographic recording of the responses to colorectal distension (CRD) was made 3days later. Ibodutant (0.33 , 0.65, 1.9 and 6.5mgkg(-1)) was given s.c., 30min before CRD. Release of neurokinin A and substance P from isolated mucosal and smooth muscle tissues following treatment with KCl (80mM) or capsaicin (10M) was measured by EIA. Plasma pharmacokinetics of ibodutant following a single s.c. administration (0.73 or 2.1mgkg(-1)) were measured over 24h. Key ResultsIbodutant did not affect abdominal contractions in control animals. After TNBS-induced colitis, ibodutant prevented the increased visceral hypersensitivity to CRD in females, at lower doses than in males. Ibodutant pharmacokinetics did not differ between females and males. Tachykinins release was greater in smooth muscle than in mucosal samples. Capsaicin-stimulated release of tachykinins from inflamed mucosal samples from females was significantly lower than in males. Conclusions and ImplicationsIbodutant prevented abdominal nociception in a model of visceral hypersensitivity in guinea pigs with a greater efficacy in females than in males. Our results highlight a gender-related difference in colonic visceral hypersensitivity and mucosal nerve activation.

Abstract

We have compared the in vivo activity of the bradykinin B(2) receptor peptide antagonists MEN 11270 and Icatibant versus the nonpeptide antagonist FR 173657, after intravenous (i.v.) and intratracheal (i.t.) administration, on the bradykinin (BK)-induced bronchoconstriction and hypotension in anesthetized guinea pigs. We have also assessed the affinity of these antagonists for B(2) receptors in guinea pig lung membranes by radioligand binding and the metabolic stability of peptide antagonists in guinea pig plasma and tissue homogenates. The i.v. administration of MEN 11270, Icatibant, or FR 173657 induced a dose-dependent (10-100 nmol/kg) inhibition of both hypotension and bronchoconstriction induced by bradykinin (10 nmol/kg i.v.). The inhibitory effect of MEN 11270 and Icatibant was comparable both in terms of potency and time course, whereas FR 173657 was less potent and shorter acting. After i.t. administration MEN 11270 and Icatibant (10-100 nmol/kg) dose dependently inhibited both bronchoconstriction and hypotension, whereas FR 173657 (10-100 nmol/kg) reduced bronchoconstriction without affecting hypotension. The antibronchoconstrictor effect of MEN 11270 was more prolonged than that of Icatibant and FR 173657, whereas no differences were found between the peptide antagonists in inhibiting hypotension. These findings indicated that, in vivo, the peptide antagonists are more potent and longer lasting than FR 173657 acting on bradykinin B(2) receptors in guinea pig airways and in the vascular system. The greater efficacy of the antagonists in blocking airway compared with vascular B(2) receptors after topical administration suggests that they can block airway B(2) receptors with little systemic effects.

Abstract

An integration of seismological data with geological and geomorphological information aided by seismic interpretation was performed to characterise the Quaternary tectonic evolution of the Metauro River basin area (northern Marche) and adjacent off-shore sector of the external Northern Apennines. On-shore, along the Adriatic coast, the youngest age of thrusting and folding post-dates the Early-Middle Pliocene, while Pleistocene deposits appear to be, at least in part, not involved in the deformation. Recent (i.e. post-thrusting) tectonic structures have been recognised both in pre-Quaternary substratum rocks and in Upper Quaternary continental deposits (Upper Pleistocene terrace alluvium, Upper Pleistocene–Holocene slope deposits). These faults are all compatible with a WSW–ENE oriented extension. In the Metauro River basin area, preserved flights of stream terraces have been categorised according to the presence or absence of alluvial suites in relationship to each terrace level. Here, based on both the heights above the valley floor and the areal distribution of stream terraces, a generalised vertical tectonic uplift can be inferred, particularly during the Middle–Late Pleistocene. Moreover, the along-valley distribution of stream terraces provides further constraints on the age of thrusting and folding. In fact, the reconstructed terrace-levels are substantially parallel, and no evidence for any significant deformation by fold activity has been recognised. Local deformation displayed by both terrace surfaces and alluvial/slope-waste deposits suggests, instead, the intervening of some minor differential movements associated with the generalised uplift and/or to Middle–Late Pleistocene normal faulting. Their occurrence appears anyhow to be unrelated with the pattern of folds and associated thrusts. The present-day seismic activity of the study area was considered by analysing 83 seismic events that occurred from 1987 to 2000. The epicentre distribution is very scattered, and depths are generally comprised within the first 20 km of the crust, clustering between 0 and 15 km. Most of the events have magnitude
