- Abstract

Background: Distant metastasis (DM) development in Oropharyngeal Squamous Cell Carcinoma (OPSCC) represents an important prognostic factor. The identification of a phenotype of metastatic patients may better define therapeutic and follow-up programs. Methods: We included 408 patients with OPSCC, non-metastatic at the time of diagnosis, and treated with curative intent. The Overall Survival (OS) analyses were performed and the impact of developing DM on survival was analyzed through Cox proportional-hazard regression model. Results: 57 (14%) patients develop DM. 302 (74%) were p16+ OPSCC and 35 of them experienced DM. Advanced clinical stage, smoking, p16-status, response to primary treatment, and loco-regional relapse influence the DM rate. Only in the p16+ group, DM onset results in a greater impact on OS (p < 0.0001). Lung metastases have a better OS compared to non-pulmonary ones (p = 0.049). Conclusion: This retrospective study shows a possible stratification of OPSCC patients based on the risk of the development of DMs.

We present the results of a model-based search for continuous gravitational waves from the low-mass X-ray binary Scorpius X-1 using LIGO detector data from the third observing run of Advanced LIGO, Advanced Virgo and KAGRA. This is a semicoherent search which uses details of the signal model to coherently combine data separated by less than a specified coherence time, which can be adjusted to balance sensitivity with computing cost. The search covered a range of gravitational-wave frequencies from 25Hz to 1600Hz, as well as ranges in orbital speed, frequency and phase determined from observational constraints. No significant detection candidates were found, and upper limits were set as a function of frequency. The most stringent limits, between 100Hz and 200Hz, correspond to an amplitude h0 of about 1e-25 when marginalized isotropically over the unknown inclination angle of the neutron star's rotation axis, or less than 4e-26 assuming the optimal orientation. The sensitivity of this search is now probing amplitudes predicted by models of torque balance equilibrium. For the usual conservative model assuming accretion at the surface of the neutron star, our isotropically-marginalized upper limits are close to the predicted amplitude from about 70Hz to 100Hz; the limits assuming the neutron star spin is aligned with the most likely orbital angular momentum are below the conservative torque balance predictions from 40Hz to 200Hz. Assuming a broader range of accretion models, our direct limits on gravitational-wave amplitude delve into the relevant parameter space over a wide range of frequencies, to 500Hz or more., 19 pages, Open Access Journal PDF

Birds are highly visually oriented and use plumage coloration as an important signalling trait in social communication. Hence, males and females may have different patterns of plumage coloration, a phenomenon known as sexual dichromatism. Because males tend to have more complex plumages, sexual dichromatism is usually attributed to female choice. However, plumage coloration is partly condition-dependent; therefore, other selective pressures affecting individuals' success may also drive the evolution of this trait. Here, we used tanagers as model organisms to study the relationships between dichromatism and plumage coloration complexity in tanagers with parasitism by haemosporidians, investment in reproduction and life-history traits. We screened blood samples from 2849 individual birds belonging to 52 tanager species to detect haemosporidian parasites. We used publicly available data for plumage coloration, bird phylogeny and life-history traits to run phylogenetic generalized least-square models of plumage dichromatism and complexity in male and female tanagers. We found that plumage dichromatism was more pronounced in bird species with a higher prevalence of haemosporidian parasites. Lastly, high plumage coloration complexity in female tanagers was associated with a longer incubation period. Our results indicate an association between haemosporidian parasites and plumage coloration suggesting that parasites impact mechanisms of sexual selection, increasing differences between the sexes, and social (non-sexual) selection, driving females to develop more complex coloration.

Co28Cr6Mo alloy (ASTM F75 and F1537) is one of the standard biomaterials for permanent orthopedic implants, utilized especially in case of joint replacement, such as knee and ankle prostheses. At the present, innovative Additive Manufacturing (AM) technologies, such as laser-based powder bed fusion (LPBF), also known as selective laser melting (SLM), enable the production of customized medical devices with improved mechanical properties. When dealing with implants for joint replacement, wear resistance is critical and, unlike compressive and tensile properties, the knowledge on wear behavior of the LPBF Co28Cr6Mo alloy is currently limited. Furthermore, the effect of post-process heat treatment on tribological properties, that have to be customized on the peculiar microstructure induced by LPBF, needs to be assessed. In this view, the present work first focuses on a novel direct aging treatment of the LPBF Co28Cr6Mo alloy, performed in the range 600–900 °C up to 180 min, and investigates the effects on hardness and microstructural features, with the optimized heat-treated condition found in case of 850 °C for 180 min aging treatment. Then, the attention is driven to the dry sliding wear behavior of as-built and heat-treated LPBF Co28Cr6Mo alloy, considering the conventional wrought alloy as benchmark. For testing conditions closer to the in-service ones, the as-built LPBF alloy showed a wear resistance higher than the conventional wrought alloy. The optimized aging treatment significantly modified the as-built LPBF microstructure, it improved the alloy hardness and, in general, it positively affected its friction and wear behavior.

The efficacy of endoscopic resection in early-stage esophageal squamous cell carcinoma has not been defined. We queried the National Cancer Database to identify patients presenting with cT1N0M0 esophageal squamous cell cancer between 2004 and 2017. Transitive match methods were used to 1:1:1 propensity match patients undergoing endoscopic resection to patients undergoing esophagectomy and those undergoing definitive chemoradiotherapy. Kaplan Meier method was used to compare 5-year overall survival profiles for matched cohorts.301 patients (19%) underwent endoscopic resection; 497 (32%) esophagectomy; 767 (49%) chemoradiation. On comparison of matched cohorts, patients undergoing chemoradiation demonstrated lower rates of survival than those undergoing esophagectomy (32% vs. 59%, p 0.0001) while those undergoing endoscopic resection demonstrated rates comparable to patients undergoing esophagectomy (53% vs. 59%, p = 0.77).For cT1N0M0 esophageal squamous cell cancer, endoscopic resection is associated with rates of survival similar to those following esophagectomy and better than those following definitive chemoradiation.

Energy crops on marginal lands are seen as an interesting option to increase biomass contribution to the primary energy mix. However, in the literature there is currently a lack of integrated assessments of margin land availability, energy crop production potential and supply chain optimisation. Assessing the potential and the cost of these resources in a given region is therefore a difficult task. This work also emphasises the importance on a clear definition and discussion about marginal lands and the related ethical issues embedded in the concept to ensure positive societal impacts of the results. This study proposes a methodology to estimate and analyse, in terms of economic costs, the potential of miscanthus grown on marginal lands from the production to the final point of use. Different datasets are assembled and a supply chain optimisation model is developed to minimize the total cost of the system. Miscanthus is used as a representative energy crop for the Belgian and French case studies. High temperature heat demand is considered as final use. The miscanthus can be traded by truck either in the form of chips or pellets. The results show that the miscanthus on marginal lands could supply high temperature heat up to 38 TWh in France and 1.4 TWh in Belgium with an average cost of around 50 €/t. The different sensitivity analyses showed that the yield variation has the strongest influence on the final cost, together with the distances and the cost of production of miscanthus. The main pattern observed is the local consumption of miscanthus chips and export of the surplus (if any) to the neighbouring regions. Pellets are only of marginal interest for France and are never observed for Belgium. Distances and availability of sufficient feedstocks are the two main parameters impacting the production of pellets.

ABSTRACT This article is based on the need to understand to what extent, in Brazil, the teaching of English in early childhood education and the early years of elementary school has been finding room in the constitution of professionals in the area taking as well as other theoretical and epistemological positions and, thus reorienting the gaze involved in this context, including the author. By analyzing the data from two sources, a discipline offered in an undergraduate course and an extension course, both concerning the teaching of an additional language in childhood, the aim was to identify meanings attributed by teachers regarding assuming different attitudes and reasons for teaching English or language education in and for childhood. For this, we searched in the words of teachers who work in the area, linguistic marks that signal postures adopted in the activities developed in the context of teacher’s education that justify the usage of new terminologies. It was also verified the modes of organization of the referred discipline, given that changes made in its program can collaborate to better visualize emerging needs regarding discussions that make more sense to what has been experienced in English classes in the early years. It is concluded that the term ‘linguistic education in childhood with children’ has been preferred over ‘English teaching for children’ since reflections promoted in the field build an understanding that educating through language expands the possibilities of teaching it from a broader and sensitive linguistic approach.

Abstract: Nanomaterials have been offering improvements in different areas due to their unique characteristics, but cytotoxicity associated with their use is still a topic that concerns researchers. Causing cell death, at first glance, may seem to be a problem and the studies regarding signaling pathways involved in this toxicity are still in their infancy. However, there are scenarios in which this feature is desirable, such as in cancer treatment. Anti-cancer therapies aim to eliminate the cells of malignant tumors as selectively as possible. From this perspective, titanium dioxide (TiO2) nanoparticles (NPs) deserve to be highlighted as important and efficient tools. Besides being able to induce cell death, these NPs can also be used to deliver anti-cancer therapeutics. These drugs can originate from natural sources, such as paclitaxel (an antitumoral molecule derived from a vegetal source). The present review aims to explore the recent knowledge of TiO2 NPs as nanocarriers (promoting the nanodelivery of paclitaxel) and as nanosensitizers to be used in phototherapies and/or sonodynamic therapy aiming to treat cancer. Signaling pathways triggered by this nanomaterial inside cells leading to apoptosis (a desirable fate when targeting tumor cells) and challenges related to the clinical translation of these NPs will also receive attention in the future.

- Abstract

Sensory adaptation is a feature-specific modulation of neural responses and is potentially fundamental to maximizing perceptual sensitivity. Despite its function being unclear, it has been hypothesized that sensory adaptation modifies the neurons' response codes, increasing the ability to process sensory signals on a larger scale. To better understand how such flexibility of our brain is possible, we investigated the effect of high- and low-frequency vibrotactile adaptation on perceived tactile temporal frequency during childhood, a time known for the brain to experience varying levels of plasticity. We tested tactile temporal frequency discrimination thresholds in both children and adults before and after tactile adaptation. Our results demonstrate that sensory adaptation does not consistently change perceived tactile temporal frequency in younger children as it does in adults, as adult-like trends begin to emerge at around 8 years of age but consolidate only in 10-year-old children. The absence of adaptation aftereffects suggests that, under certain conditions, sensory history does not affect perception in younger children in a similar way to adults. Surprisingly, younger children proved to be less flexible in modulating neural responses after prolonged exposure to an adapting stimulus, a tendency conflicting with the high plasticity levels the brain experiences during the early stages of life. - Abstract
Phrenic nerve injury is considered a rare complication of pulmonary vein isolation ablation procedure that is used to treat refractory paroxysmal atrial fibrillation and has been reported in different settings in literature. In this case report, we discuss a case presented with dyspnea, cough, headaches, fatigue, and palpitation five months post pulmonary vein isolation ablation. This case has a medical history of several comorbidities that may contribute to the etiology of the above symptoms. Therefore, the prolonged presentation, late onset of symptoms, and multiple cofactors could be assumed to contribute to patient’s etiology of symptoms attributed to the challenge of the diagnosis. However, one should always keep phrenic nerve injury as a differential when dealing with post pulmonary vein isolation ablation dyspnea or exacerbation of co-existing respiratory symptoms, even when facing delayed presentations of symptoms.

Orthopedics and Sports Medicine ,Surgery - Abstract
Multiple diverse factors contribute to musculoskeletal pain, a major cause of physical dysfunction and health-related costs worldwide. Rapidly growing evidence demonstrates that the gut microbiome has overarching influences on human health and the body's homeostasis and resilience to internal and external perturbations. This broad role of the gut microbiome is potentially relevant and connected to musculoskeletal pain, though the literature on the topic is limited. Thus, the literature on the topic of musculoskeletal pain and gut microbiome was explored. This narrative review explores the vast array of reported metabolites associated with inflammation and immune-metabolic response, which are known contributors to musculoskeletal pain. Moreover, it covers known modifiable (e.g., diet, lifestyle choices, exposure to prescription drugs, pollutants, and chemicals) and non-modifiable factors (e.g., gut architecture, genetics, age, birth history, and early feeding patterns) that are known to contribute to changes to the gut microbiome. Particular attention is devoted to modifiable factors, as the ultimate goal of researching this topic is to implement gut microbiome health interventions into clinical practice.Overall, numerous associations exist in the literature that could converge on the gut microbiome's pivotal role in musculoskeletal health. Particularly, a variety of metabolites that are either directly produced or indirectly modulated by the gut microbiome have been highlighted. The review highlights noticeable connections between the gut and musculoskeletal health, thus warranting future research to focus on the gut microbiome's role in musculoskeletal conditions.

AbstractThe purpose of this investigation is to study, from a historical perspective, the urban formation that took place during 1897-1946 in Buenos Aires Province, which was affected by a tense relationship that, during this period, gave rise to paved and unpaved roads, trains, etc. The periodization that we propose covers the beginning of the railway boom, more specifically at the time that the Southern railway reached 25 de Mayo city from Empalme de Lobos (1897), until 1946, when the railways were nationalized. The area under study is more than 180 km away from the Capital city, and forms a quadrant that runs from such city up to the Provincial Route 65 line (North-South), and which is defined in the opposite direction by the National Route 205 and National Route 5 (East-West). In this opportunity, we will focus specifically on 25 de Mayo city.

Cardiology and Cardiovascular Medicine - Abstract
Pulmonary hypertension (PH) is defined as a mean pulmonary artery pressure (mPAP)20 mmHg and its presence is associated with worse outcomes. A comprehensive hemodynamic evaluation of the pulmonary circulation is essential for diagnosis, hemodynamic classification, and prognostication. A multitude of indices assess different aspects of the pulmonary circulation but there are no reviews that describe their specific value in PH.We performed a thorough literature search of relevant articles in English from 1970-2021 using PubMed.In this article, we present both static and dynamic indices used for the hemodynamic assessment of PH. While some of these indices are routinely used in clinical practice, including cardiac index (CI), stroke volume (SV), and pulmonary vascular resistance (PVR); others such as pulmonary artery compliance (PAC), pulmonary effective arterial elastance (Ea), and pulmonary artery pulsatility index (PAPi) are gaining popularity by enhancing the understanding of different aspects of the pulmonary circulation. We described the advantages and pitfalls of various indices, including when to use them in the hemodynamic evaluation of patients with PH.A variety of indices measuring different aspects of the right ventricle (RV)-pulmonary arteries (PA) system provide valuable information in patients with PH. However, it remains important to develop and validate indices that provide a comprehensive hemodynamic evaluation to improve outcomes in patients with PH.

This review addresses green algae-based gold (Au), iron (Fe), and silver (Ag) nanoparticles (NPs) as eco-friendly nanomaterials to deal with biological, organic, and inorganic environmental contaminants. Among nanotechnological tools that can fully degrade, adsorb, and/or convert pollutants into less harmful structures, AgNPs, AuNPs, and FeNPs deserve highlight for their efficiency and low cost. However, green protocols are preferable to produce them in an eco-friendly manner. Although phycosynthesis is still in its infancy, algae present various advantages as green raw materials to NPs’ synthesis; fast growth rate, low-energy input requirement, low costs, easy and eco-friendly cultivation, and high tolerance to metals are examples. To allow their large-scale application, however, challenges regarding obtaining sufficient biomaterial with good reproducibility, designing protocols to achieve desirable features on NPs, and recovering the biocompatible nanomaterial after use still need attention. Perspectives for the field involve surpassing these limitations, broadening knowledge on synthesis mechanisms, protocols, and new species useful to offer, in the future, commercial eco-friendly, and low-cost phycosynthesized AuNPs, AgNPs, and FeNPs to nanoremediation. The potential of these NPs to deal with environmental contaminants, their advantageous characteristics and biocompatibility, the main limitations associated with their large-scale application, and future prospects for the field will receive attention.

- Abstract
The design of nZEB buildings, as well as the implementation of retrofit interventions in existing structures, are essential tools for reducing energy consumption in buildings and increasing decarbonization of the building sector. To describe the effectiveness of a retrofit intervention, in addition to the analysis of the benefits in terms of costs and energy savings, an environmental analysis should also be performed, introducing various indicators, such as energy and environmental payback times. In this article, we considered a residential building located in Montemarcello (Liguria, North-west of Italy) that had been subjected to a refurbishment and an expansion, with the aim of evaluating the energy and carbon savings achievable due to the interventions carried out. The life cycle analysis approach was applied to calculate the environmental payback times. The main purpose of this work is the application of an integrated approach to assess the economic, energetic and environmental convenience of retrofit interventions during the entire life cycle of the building, underlining the importance of considering LCA and environmental aspects to achieve decarbonisation of the construction sector. The results show that energy and environmental payback times are lower than the useful life of the building and of its components, and that LCA proves to be a strategic methodology for studying the problems deriving from global warming and energy supply in the building sector.

- Abstract
Numerical simulations are widely used to evaluate the thermal comfort and energy savings in the retrofit of historic buildings. In most cases, however, no detailed data are available on the materials and stratigraphy of the building envelopes, and on-site measurements can be expensive and time consuming. The present work uses as a case study a university building characterized by a high thermal capacity in the city of Rome to verify whether the use of natural ventilation can be a practice of use in order to guarantee energy saving and natural comfort. To this end, in the summer of 2020, an experimental campaign was carried out aimed at acquiring thermofluximetric measurements through the vertical walls, the air temperature inside and outside the analyzed environment and the air velocity. Measurements were conducted under three different usage protocols, including night ventilation and 24-hour continuous ventilation. These measurements made it possible to identify the thermophysical characteristics of a wall considered "equivalent" to the real wall, allowing the realization of thermofluidodynamic computational models. In particular, in the study, 3 different stratigraphies were considered and compared, corresponding in the first case to the equivalent wall, in the second to that available from the Comsol software library, and, finally, in the third and last case, from literature data (Tabula project) for the building typology analyzed. From the analysis, it emerged that the 3 groups of parameters do not have a significant impact in terms of variation of internal comfort, confirming the reliability of the use of the literature values for these types of modeling.

Purpose This systematic literature review (SLR) aims to examine the extent of academic knowledge of sustainability materiality research. There is no academic review of this field; therefore, this study aims to close this research gap. Design/methodology/approach The paper systematically reviews the existing literature on sustainability materiality research. Papers were qualitatively classified and analysed in accordance with the theoretical underpinning, research methods and academic themes of sustainability materiality research. Findings The findings of the review show that scholarly work on sustainability materiality has increased exponentially since the 2010s. In terms of research methods, scholars have examined sustainability using content analysis techniques and qualitative approaches. A common theoretical foundation was missing, but an increasing number of articles have been anchored to stakeholder theory. The academic themes have progressively enriched empirical evidence on the evaluation of materiality in sustainability information. Research limitations/implications This review can be useful as an academic basis to open avenues for strengthening theoretical and empirical research on new emerging issues regarding double materiality and dynamic materiality. Originality/value This paper conducts the first SLR of academic knowledge on sustainability materiality research. Eight academic themes are proposed to classify sustainability materiality. Thus, it is an aid to future research in this area.

PurposeBased on a critical lens, this paper aims to empirically evaluate the adoption of programs to buy from minority suppliers (MS) and their outcomes in Brazil, considering a multi-stakeholder approach (buying companies, suppliers and third parties).Design/methodology/approachThe data were collected via an exploratory survey involving 109 buying companies and two case studies with 21 in-depth interviews.FindingsIt was revealed that supplier diversity (SD) in Brazil is still developing, and is more rhetoric than a practice promoting real change. The current traditional procurement mindset, the role of third parties, and the focus on supplier selection, rather than on supplier development prevents a social impact that could reduce inequality between MS and their counterparts.Originality/valuePrevious studies were focused on buying companies' perspective in advanced countries that have clear regulation for SD. By exploring the phenomenon in a country with great economic disparities and no regulation, and using a critical lens, this study highlights the difference between desired and effective implementation of social initiatives that promote inclusiveness.

Mutations enhancing the kinase activity of LRRK2 cause Parkinson’s disease (PD) and therapies that reduce LRRK2 kinase activity are being tested in clinical trials. Numerous rare variants of unknown clinical significance have been reported, but how the vast majority impact on LRRK2 function is unknown. Here, we investigate 100 LRRK2 variants linked to PD, including previously described pathogenic mutations. We identify 23 LRRK2 variants that robustly stimulate kinase activity, including variants within the N-terminal non-catalytic regions [ARM (E334K, A419V), ANK(R767H), LRR (R1067Q, R1325Q)], as well as variants predicted to destabilise the ROC:CORB interface [ROC (A1442P, V1447M), CORA (R1628P) CORB (S1761R, L1795F)] and COR:COR dimer interface [CORB (R1728H/L)]. Most activating variants decrease LRRK2 biomarker site phosphorylation (pSer935/pSer955/pSer973), consistent with the notion that the active kinase conformation blocks their phosphorylation. We conclude that the impact of variants on kinase activity is best evaluated by deploying a cellular assay of LRRK2-dependent Rab10 substrate phosphorylation, compared to a biochemical kinase assay, as only a minority of activating variants [CORB (Y1699C, R1728H/L, S1761R) and kinase (G2019S, I2020T, T2031S)], enhance in vitro kinase activity of immunoprecipitated LRRK2. Twelve variants including several that activate LRRK2 and have been linked to PD, suppressed microtubule association in the presence of a Type I kinase inhibitor [ARM(M712V), LRR(R1320S), ROC (A1442P, K1468E, S1508R), CORA(A1589S), CORB (Y1699C, R1728H/L) and WD40(R2143M, S2350I, G2385R)]. Our findings will stimulate work to better understand the mechanisms by which variants impact biology and provide rationale for variant carrier inclusion or exclusion in ongoing and future LRRK2 inhibitor clinical trials.
