De novo CD5-positive diffuse large B- cell lymphoma (CD5+ DLBCL) is a subtype of DLBCL found predominantly in older individuals. This particular subtype has been associated with a female pre- dominance and a more aggressive clinical course. Conversely, this entity has not been described in the pe- diatric population. We report a case of a 12 year-old boy who presented with an ileocecal intussusception. Radiologic, morphologic, and immunophenotypic analysis revealed an isolated extranodal mass consistent with a CD5+ DLBCL, germinal center cell phenotype. Fluorescent in situ hybridization analysis was nega- tive for cMYC, BCL6, BCL2, MLL, and IGH/CCND1 rearrangement and showed loss of one copy of MLL in 32% cells. The patient was treated with four cycles of cyclophosphamide, vincristine, prednisolone, methotrexate, and doxorubicin and achieved complete remission. To the best of our knowledge, this is the first detailed report of a de novo CD5+ DLBCL occurring in a child.

The ability to mount a strong immune response against pathogens is crucial for mammalian survival. However, excessive and uncontrolled immune reactions can lead to autoimmunity. Unraveling how the reactive versus tolerogenic state is controlled might point toward novel therapeutic strategies to treat autoimmune diseases. The surface receptor Toso/Faim3 has been linked to apoptosis, IgM binding, and innate immune responses. In this study, we used Toso-deficient mice to investigate the importance of Toso in tolerance and autoimmunity. We found that Toso(-/-) mice do not develop severe experimental autoimmune encephalomyelitis (EAE), a mouse model for the human disease multiple sclerosis. Toso(-/-) dendritic cells were less sensitive to Toll-like receptor stimulation and induced significantly lower levels of disease-associated inflammatory T-cell responses. Consistent with this observation, the transfer of Toso(-/-) dendritic cells did not induce autoimmune diabetes, indicating their tolerogenic potential. In Toso(-/-) mice subjected to EAE induction, we found increased numbers of regulatory T cells and decreased encephalitogenic cellular infiltrates in the brain. Finally, inhibition of Toso activity in vivo at either an early or late stage of EAE induction prevented further disease progression. Taken together, our data identify Toso as a unique regulator of inflammatory autoimmune responses and an attractive target for therapeutic intervention.

OBJECTIVE To assess the efficacy and safety of switching from sitagliptin to liraglutide in metformin-treated adults with type 2 diabetes. RESEARCH DESIGN AND METHODS In an open-label trial, participants randomized to receive either liraglutide (1.2 or 1.8 mg/day) or sitagliptin (100 mg/day), each added to metformin, continued treatment for 52 weeks. In a 26-week extension, sitagliptin-treated participants were randomly allocated to receive instead liraglutide at either 1.2 or 1.8 mg/day, while participants originally randomized to receive liraglutide continued unchanged. RESULTS Although 52 weeks of sitagliptin changed glycosylated hemoglobin (HbA1c) by −0.9% from baseline, additional decreases occurred after switching to liraglutide (1.2 mg/day, −0.2%, P = 0.006; 1.8 mg/day, −0.5%, P = 0.0001). Conversion to liraglutide was associated with reductions in fasting plasma glucose (FPG) (1.2 mg/day, −0.8 mmol/L, P = 0.0004; 1.8 mg/day, −1.4 mmol/L, P < 0.0001) and body weight (1.2 mg/day, −1.6 kg; 1.8 mg/day, −2.5 kg; both P < 0.0001) and with an increased proportion of patients reaching HbA1c

We present a scheme for robust and efficient projection measurement of a qubit consisting of the two magnetic sublevels in the electronic ground state of alkali earth metal ions. The scheme is based on two stimulated Raman adiabatic passages (STIRAP) involving four partially coherent laser fields. INTRODUCTION: As the only femoral stem, the Exeter stem has shown acceptable results with Boneloc cement, but no long-term results have been published. The aim of this study was to evaluate the 20-year performance of Boneloccemented primary Exeter hip arthroplasty. MATERIAL AND METHODS: From March 1992 to June 1994, a total of 200 consecutive Boneloc-cemented primary hip arthroplasties were performed in 183 patients aged 26-90 years, mean 71 years. At follow-up after 18-20 years, the primary endpoint was prosthetic survival assessed by the Kaplan-Meier analysis. Secondary endpoints were clinical and radiographic follow-up examination. RESULTS: After a mean 18.4 (17.6-19.6)-year period, 27 patients/32 hips had been revised. At endpoint revision for any reason the 20-year cumulative overall survival was 73% (95% confidence interval (CI): 49-89%); at endpoint revision for aseptic loosening the 20-year cumulative survival was 84% (95% CI: 68-96%) for the stem and 76% (95% CI: 6192%) for the cup. Cementing technique, implant size and metal backing of the cup were not associated with revision rates. A total of 23 patients/24 hips had clinical/radiographic examination, and 10 patients/12 hips were assessed by telephone. According to patient reported evaluation, all were very satisfied or satisfied. Radiographs showed loosening of two cups and possible loosening of two stems. CONCLUSION: This study provides the first long-term results on Boneloc-cemented primary hip arthroplasty. The Exeter prosthesis, especially the stem, maintains the previously published acceptable survival and results despite the documented inferior quality of the cement.

This work presents the early findings of methanol application in a direct injected compression ignited combustion engine. A two cylindered and four stroke BUKH DV24 engine with a geometric compression ratio of 18 is used. The engine operates at 1200 rpm, and one cylinder is fired with diesel to provide smooth engine rotation. The other cylinder - the experimental cylinder – is charged with air (at a slightly positive gauge pressure) and fired with methanol blended with a fuel additive. With 5 % (m/m) fuel additive and „early” fuel injections (around 45 crank angles before the top dead center) the fuel-blend provides a reasonable partially premixed combustion process. The combustion is, however, limited in terms of injection timing- and excess air ratio variations. Operation at later „diesel-like” injection timings requires a high compression ratio, which in this work is simulated by heating the charge air. With 5 % (m/m) fuel additive, a charge air temperature above 100 °C provides a less rapid combustion with lower efficiencies compared to the early injections.

The present study investigates three different types of mooring systems in order to establish potential cost reductions and applicability to wave energy converters (WECs). Proposed mooring systems for three existing WECs create the basis for this study, and the study highlights areas of interest using a preliminary cost estimation and discussion of buildability issues. Using synthetic rope and variations in the mooring configuration has the potential of influencing the cost significantly. In order to quantify this potential, a simple quasi-static analysis is performed, which shows that a SALM type system can provide a paramount cost reduction compared to a traditional CALM type system with chain lines. Similarly, use of nylon ropes similarly appears to provide low cost.

It is increasingly recognised that individuals with non-0 blood groups may be at elevated risk of venous thromboembolism (VTE) compared with blood group 0 individuals. Multiple studies suggest that individuals with a non-0 blood group have an approximately twofold increased risk of VTE. The largest risk is seen in individuals with combined factor VLeiden mutation and non-0 blood group. In this review, we outline an overview of the existing literature regarding AB0 blood groups and the risk of VTE.

Objectives: Respiratory movements may complicate diagnostic and therapeutic procedures such as biopsies and stereotactic irradiation therapy in lung cancer patients. An attempt to avoid respiratory movements, up to 30 minutes, long enough for procedures was performed in an animal study. Methods: Ten anaesthetized minipigs ~30 kg were intubated in the trachea and small NiTi-stents were placed in various parts of the lungs. Using a muscle relaxing drug, the pigs were deprived of the ability to breathe for 30 minutes, a longer time than normally used for positioning and irradiation or for biopsies. No attempt to hyperventilate the animals was made prior to the apneic period. After a lung recruitment manoeuvre, a constant oxygen pressure of 20 cm water was applied to the airways. Using X-ray fluoroscopy, the position of the stents and thereby the movements of the lung, were monitored. Arterial gas analyses were performed every 5 minutes during the apneic period. Results: All animals survived 30 minutes of apneic oxygenation. The median arterial oxygen partial pressure actually rose from 11.8 to 54.3 kPa and there were no changes in oxygen saturation. The median arterial carbon dioxide partial pressure rose from 6.9 to 18.7 kPa and the median pH fell from 7.41 to 7.04 during 30 minutes of apneic oxygenation. Our setup, or our strategy of anaesthesia, did not immobilise the internal parts of the lungs satisfactorily, and must be improved before it can be used in a clinical situation.
