2. Safety and Efficacy of Renuvion Helium Plasma to Improve the Appearance of Loose Skin in the Neck and Submental Region

Background Minimally invasive procedures that deliver thermal energy to subcutaneous tissue offer a solution when deciding between excisional and noninvasive options to address face and neck aging-related changes. A minimally invasive helium plasma device, Renuvion, was first utilized for subdermal tissue heating to reduce skin laxity under an FDA general clearance for cutting, coagulation, and ablation of soft tissue. Objectives The purpose of this study was to demonstrate the safety and effectiveness of the helium plasma device for improving the appearance of loose skin in the neck and submental region. Methods Patients undergoing the procedure with the helium plasma device in the neck and submentum were studied. They were seen for 6 months following the procedure. The primary effectiveness endpoint for improvement in lax skin in the treatment area was determined by 2 of 3 blinded photographic reviewers. The primary safety endpoint was the level of pain after treatment. Results The primary effectiveness endpoint was met; 82.5% demonstrated improvement at Day 180. The primary safety endpoint was met; 96.9% of patients experienced no pain to moderate pain to Day 7. There were no serious adverse events reported related to the study device or procedure. Conclusions The data demonstrate benefit to patients by improvement of the appearance of lax skin in the neck and submental region. Outcomes resulted in US Food and Drug Administration 510(k) clearance in July 2022, expanding indications for the device to include subcutaneous dermatological and aesthetic procedures to improve the appearance of loose skin in the neck and submental region. 5. A closed translocation channel in the substrate-free AAA+ ClpXP protease diminishes rogue degradation

- Abstract
Intracellular proteases must be specific to avoid degrading the wrong proteins. Here, we present cryo-EM structures of E. coli ClpXP, a AAA+ protease, which reveal that the axial channel of ClpX is closed prior to the binding and subsequent translocation of a protein substrate. An open-channel ClpX mutation stimulates degradation of casein, a non-specific substrate, indicating that channel closure contributes to increased degradation specificity. We demonstrate that ClpX activates ClpP cleavage of a degron-free decapeptide by a channel-independent mechanism, in which the peptide substrate appears to pass through a symmetry mismatched gap in the interface between ClpX and ClpP before entering the degradation chamber via the axial portal of ClpP. The peptide products of ClpXP protein degradation are likely to exit the chamber by the reverse route.

Background Nearly half of patients with hidradenitis suppurativa (HS) report dissatisfaction with their treatment. However, factors related to treatment satisfaction have not been explored. Objectives To measure associations between treatment satisfaction and clinical and treatment-related characteristics among patients with HS. Methods Treatment satisfaction was evaluated utilizing data from a cross-sectional global survey of patients with HS recruited from 27 institutions, mainly HS referral centres, in 14 different countries from October 2017 to July 2018. The primary outcome was patients’ self-reported overall satisfaction with their current treatments for HS, rated on a five-point scale from ‘very dissatisfied’ to ‘very satisfied’. Results The final analysis cohort comprised 1418 patients with HS, most of whom were European (55%, 780 of 1418) or North American (38%, 542 of 1418), and female (85%, 1210 of 1418). Overall, 45% (640 of 1418) of participants were either dissatisfied or very dissatisfied with their current medical treatment. In adjusted analysis, patients primarily treated by a dermatologist for HS had 1·99 [95% confidence interval (CI) 1·62–2·44, P < 0·001] times the odds of being satisfied with current treatment than participants not primarily treated by a dermatologist. Treatment with biologics was associated with higher satisfaction [odds ratio (OR) 2·36, 95% CI 1·74–3·19, P < 0·001] relative to treatment with nonbiologic systemic medications. Factors associated with lower treatment satisfaction included smoking (OR 0·78, 95% CI 0·62–0·99; active vs. never), depression (OR 0·69, 95% CI 0·54–0·87), increasing number of comorbidities (OR 0·88 per comorbidity, 95% CI 0·81–0·96) and increasing flare frequency. Conclusions There are several factors that appear to positively influence satisfaction with treatment among patients with HS, including treatment by a dermatologist and treatment with a biologic medication. Factors that appear to lower treatment satisfaction include active smoking, depression, accumulation of comorbid conditions and increasing flare frequency. Awareness of these factors may support partnered decision making with the goal of improving treatment outcomes. What is already known about this topic? Nearly half of patients with hidradenitis suppurativa report dissatisfaction with their treatments. What does this study add? Satisfaction with treatment is increased by receiving care from a dermatologist and treatment with biologics.Satisfaction with treatment is decreased by tobacco smoking, accumulation of comorbid conditions including depression, and higher flare frequency. What are the clinical implications of this work? Awareness of the identified factors associated with poor treatment satisfaction may support partnered decision making and improve treatment outcomes.

8. Impacts of Digital Care Programs for Musculoskeletal Conditions on Depression and Work Productivity: Longitudinal Cohort Study (Preprint)

BACKGROUND Comorbidity between musculoskeletal (MSK) pain and depression is highly common, and is associated with a greater symptom burden and greater loss of work productivity than either condition alone. Multimodal care programs tackling both physical and mental health components may maximize productivity recovery and return to work. Digital delivery of such programs can facilitate access, ensure continuity of care, and enhance patient engagement. OBJECTIVE The aim of this study was to assess the impact of a completely remote multimodal digital care program (DCP) for MSK pain on mental health and work-related outcomes stratified by baseline depression levels. METHODS Ad hoc analysis of an interventional, single-arm, cohort study of individuals with MSK pain undergoing a DCP was performed. Three subgroups with different baseline depression severity levels were established based on responses to the Patient Health Questionnaire (PHQ-9): cluster 1 (score RESULTS From a total of 7785 eligible participants, 6137 (78.83%) were included in cluster 1, 1158 (14.87%) in cluster 2, and 490 (6.29%) in cluster 3. Significant improvements in depression and anxiety scores were observed in clusters 2 and 3 but not in cluster 1, with average end-of-the program scores in clusters 2 and 3 below the initially defined cluster thresholds (score of 5 and 10, respectively). All clusters reported significant improvements in productivity impairment scores (mean changes from –16.82, 95% CI –20.32 to –13.42 in cluster 1 to –20.10, 95% CI –32.64 to –7.57 in cluster 3). Higher adherence was associated with higher improvements in depression in clusters 2 and 3, and with greater recovery in activities of daily living in cluster 3. Overall patient satisfaction was 8.59/10.0 (SD 1.74). CONCLUSIONS A multimodal DCP was able to promote improvements in productivity impairment scores comparable to those previously reported in the literature, even in participants with comorbid depression and anxiety. These results reinforce the need to follow a biopsychosocial framework to optimize outcomes in patients with MSK pain. Objectives: Shoulder and elbow injuries account for about one-fourth of the total injuries in professional baseball players. The loss of shoulder internal rotation range of motion (IR ROM) has been identified as a significant risk factor for upper extremity injuries in pitchers across all ages. Several anatomic structures have been independently associated with IR ROM. First, greater amounts of humeral retroversion (HR) have been related to decreased IR ROM. Second, increased posterior capsule thickness (PCT) has also been related to decreased IR ROM. Lastly, adaptations to the teres minor muscle pennation angle (PA) has been related to losses in IR ROM and the PA of both the infraspinatus and teres minor have been related to the acute decrease in IR ROM following pitching. However, these three anatomic structures have not been collectively measured in baseball pitchers to determine the underlying mechanisms responsible for IR ROM. Therefore, this study will determine the contributions of HR, PCT, and posterior rotator cuff PA on IR ROM in healthy professional baseball pitchers. We hypothesized that HR, PCT, and posterior rotator cuff PA would have a significant contribution to IR ROM. Methods: 51 healthy professional pitchers were recruited for this cross-sectional study at the beginning of the 2021 Major League Baseball spring training. Participants received a bilateral IR ROM assessment while laying supine with the shoulder at 90 degrees of abduction and the scapula stabilized. Ultrasound imaging was also performed bilaterally to assess HR, PCT, infraspinatus (superficial + deep) PA, and teres minor (superficial + deep) PA. All ultrasound imaging was performed utilizing a previously published standardized technique. A stepwise regression was performed which included both arms to determine the mechanisms of IR ROM. Results: The first variable to enter into the stepwise regression model was HR which had a moderate relationship with IR ROM (regression coefficient of -0.502). PCT also entered the model, as measuring both HR and PCT improved the relationship with IR ROM from moderate to strong (regression coefficient of -0.657). Infraspinatus and teres minor PA did not enter the predictive model of IR ROM. Conclusions: HR and PCT were found to be the primary anatomic mechanisms responsible for glenohumeral IR ROM. Surprisingly, the posterior rotator cuff PA was not found to be a significant predictor of IR ROM. The posterior rotator cuff may instead contribute more to the acute decrease in IR ROM following pitching, which has recently been reported. As deficits in IR ROM can have harmful effects in baseball pitchers, understanding which anatomic structures are most responsible for IR ROM is important for injury prevention and treatment. Objectives: The effect of platelet-rich plasma (PRP) treatment on recovery in acute hamstring injuries is controversial. Previous study results are inconsistent, and a standardized therapeutic approach has not been established yet. The objective of the study was to assess the treatment effect using a combination of hematoma aspiration and muscle strain PRP injection in partial hamstring muscle tears (grade 2 strains) in athletes. Methods: MRIs of athletes with grade 2 hamstring strains were reviewed from 2013 to 2018. From 2013 to 2015 athletes were treated conservatively and from 2016 to 2018 with a combination of ultrasound-guided hematoma aspiration and PRP muscle strain injection. The outcome, including return-to-play (in days) and recurrence rate, was compared retrospectively between both groups (conservative vs. aspiration/PRP) using ANOVA and Fisher’s Exact test. There was no significant difference in age, type of sport, and muscle involvement (including injury grade/location, hamstring muscle type, and length/cross-sectional area of the strain). Results: Fifty-five athletes (28 treated conservatively, 27 with hematoma aspiration/PRP injection) were included. Average return-to-play time (mean) was 32.4 days in the conservative group and 23.5 days in the aspiration/PRP group (pConclusions: Athletes with grade 2 hamstring strains treated with a combination of hematoma aspiration and PRP injection had a significantly shorter return-to-play and a lower recurrence rate compared to athletes receiving conservative treatment. 14. Poster 205: The Effect of Increased Tibial Tubercle Trochlear Groove Distance on Post-Operative Outcomes Following Anterior Cruciate Ligament Reconstruction

Objectives: Outcomes following anterior cruciate ligament reconstruction (ACLR) are generally good/excellent. While some risk factors for poor outcomes following ACLR have been determined, there are many that remain unknown. It is unclear if an elevated tibial tubercle trochlear groove (TT-TG) distance is a risk factor for poor outcomes following ACLR. Therefore, the purpose of this study was to determine if an elevated TT-TG increases risk of failure following ACLR. The authors hypothesized that patients with an elevated TT-TG will have an increased risk of failure following ACLR. Methods: All patients who underwent primary ACLR at a single institution between July 2018-June 2019 with a minimum of 2-year follow-up were eligible for inclusion. TT-TG distance was measured on preoperative magnetic resonance imaging (MRI) scans by 2 independent investigators. Clinical outcomes, complication rates, ability to RTS, and Lysholm scores were compared between patients with a TT-TG < 12.5mm (normal) and those with a TT-TG ≥ 12.5mm (elevated). Sub-analyses comparing normal and elevated TT-TG distance were also made after isolating athletes and bone-patellar tendon-bone (BTB) graft patients. Results: Overall, 159 patients were included, 98 with normal TT-TG distance and 61 with elevated TT-TG distance. There were no differences in demographics, concomitant procedures or injury mechanism between groups. There were also no differences in rates of reinjury, post-operative complications, need for revision ACLR, or RTS rates (all p>0.05). Patients with an elevated TT-TG distance had worse post-operative Lysholm scores than patients with a normal TT-TG distance (83.0 vs. 95.0, p=0.010), and also had differing rates of subjective instability (13.1% vs. 5.1%, p=0.083) and reoperation (13.1% vs. 4.1%, p=0.060) that trended towards significant. In patients who received a bone-patellar tendon-bone (BTB) graft, an elevated TT-TG distance was associated with higher rates of subjective instability (13.0% vs. 3.0%, p=0.041), reoperation (13.0% vs. 1.5%, p=0.012), and post-operative complications (25.0% vs. 8.2%, p=0.026), as well as lower ACL psychological readiness scores (324.1 vs. 446.7, p=0.015). Conclusions: Following ACLR, patients with an elevated TT-TG distance have worse Lysholm scores with a minimum 2-year follow-up. Patients with an elevated TT-TG who underwent ACLR with BTB grafts had significantly higher rates of subjective instability, reoperation, and post-operative complications, while no differences in reinjury, complication, revision, or RTS rates were observed. Objectives: Disruption of the medial patellofemoral ligament (MPFL) may lead to recurrent lateral patellar dislocation and patellofemoral chondral injury. Optimal graft choice for MPFL reconstruction in the surgical treatment of patellar instability is unknown. While autograft remains readily available, easily harvested, and of lower cost, allograft is a viable alternative and portends several benefits over autograft. Autograft may provide superior strength or lower re-tear rate, but allograft has been shown to have similar outcomes with less donor site morbidity in some series. Overall, there is a paucity of research comparing autograft vs allograft in MPFL reconstruction. Prior studies have been limited by small sample sizes, incomplete data, or heterogeneous patient population. The objectives of this study were to compare outcomes scores, return to activity and revision rates between allograft and autograft in MPFL reconstruction. The authors hypothesized that there would be no significant difference in outcomes between those who received an allograft and those who received an autograft in MPFL reconstruction. Methods: Patients who underwent isolated MPFL reconstruction with allograft or autograft at a single institution between 2013-2020 were retrospectively reviewed. Patients with a follow-up of less than 2 years were excluded, along with those who underwent concomitant tibial tubercle osteotomy or trochleoplasty. Patients were contacted to complete functional assessments including the Single Assessment Numeric Evaluation (SANE), Visual Analog Scale for pain (VAS), and International Knee Documentation Committee (IKDC) scores. Patient demographics and complications were also reviewed. Outcomes were compared between those that underwent allograft and autograft MPFL reconstruction Results: Out of the fourty-six patients that were retrospectively reviewed, thirty-one (67.4%) patients underwent allograft reconstruction, while fifteen (32.6%) patients underwent autograft reconstruction. There were no differences between the two groups in age at surgery (25.2 years old in allograft vs 26.3 years old in autograft group, P=0.726), sex ratio (19F/12M in allograft and 9F/6M in autograft, P=1.000), or BMI (27.0 in allograft and 24.8 in autograft, P=0.179). There was a mean follow up duration of 4.57 years in the allograft group and 4.83 years in the autograft group (P=0.672), with an overall response rate of 70.2%. There were no differences between the two groups when assessing subjective pain using SANE (66.7 in allograft vs 74.5 in autograft, P=0.334) or VAS score (35.7 in allograft vs 22.9 in autograft, P=0.110). Further there were no differences in patient outcomes as assessed by IKDC score (68.3 in allograft vs 76.2 in autograft, P=0.247). Patients who received an allograft returned to activity at an average of 5.9 months, while patients who received an autograft returned to activity at an average of 5.2 months (P=0.416). One patient in the allograft group required revision surgery (P=0.246). Conclusions: Both allograft and autograft remain viable options in MPFL reconstruction. There were no significant differences in failure rates, PROs, pain, or time to return to play between allograft and autograft MPFL reconstruction in this series.

Recent changes in the provision and organization of health care initiated by the Affordable Care Act have led to concerns about how these changes interact with the medical provider organizational characteristics to affect access, use, and expenditures for care. To examine the impact on individuals' use of care and costs, linked information is needed on both the characteristics of the medical providers' organizations and the individuals receiving care. There is currently no nationally representative data source that provides this essential information. Consequently, a Medical Organizations Survey (MOS) has been added as a component of the Medical Expenditure Panel Survey (MEPS) to address this limitation. The MEPS-MOS will obtain essential data on provider organizational characteristics, policies, and treatment protocols for a nationally representative sample of physicians providing care to MEPS participants. This paper describes the survey and sample design of the MEPS-MOS, survey content and precision targets and its analytical focus. This addition will substantially enhance the analytic capacity of the MEPS by facilitating assessments of how provider organizational structures and policies impact on health care utilization and expenditures, health status, patient safety and the health outcomes of individuals.

Growing population, increased urbanization, a growing global economy, and increased use of natural resources have created a demand for sustainability. Sustainability is an interdisciplinary field that requires collaboration from multiple perspectives. The planet’s resources are all being used at a ferocious pace. Energy is at the center of this issue; dependence on fossil fuels is causing extensive damage to ecosystems and public health and is a major cause of global climate change. The goal of a renewable resource-based economy is therefore important for the future, but rather than attacking the fossil fuel industry, it should be achieved through a positive vision of sustainability—one that includes the need to invest in basic and applied science of renewable energy in order to make renewables cheaper than fossil fuels. We will discuss the opportunities in sustainability-focused education and research that are needed for the transition to a renewable economy. We will also discuss the trend toward urbanization and the opportunities that exist in urban areas to close the cycle of production and consumption. Not only does most of the world’s population live in cities, but cities are paving the path toward a sustainable future. All of these factors contribute to a positive and creative vision of sustainability that focuses on building something new and clean rather than defeating something old and dirty.

A reduction in mean LDL cholesterol to 1.4-1.8 mmol/L with ezetimibe or statins reduces cardiovascular events in patients with an acute coronary syndrome and diabetes. However, the efficacy and safety of further reduction in LDL cholesterol with an inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9) after acute coronary syndrome is unknown. We aimed to explore this issue in a prespecified analysis of the ODYSSEY OUTCOMES trial of the PCSK9 inhibitor alirocumab, assessing its effects on cardiovascular outcomes by baseline glycaemic status, while also assessing its effects on glycaemic measures including risk of new-onset diabetes. Methods ODYSSEY OUTCOMES was a randomised, double-blind, placebo-controlled trial, done at 1315 sites in 57 countries, that compared alirocumab with placebo in patients who had been admitted to hospital with an acute coronary syndrome (myocardial infarction or unstable angina) 1-12 months before randomisation and who had raised concentrations of atherogenic lipoproteins despite use of high-intensity statins. Patients were randomly assigned (1: 1) to receive alirocumab or placebo every 2 weeks; randomisation was stratified by country and was done centrally with an interactive voice-response or web-response system. Alirocumab was titrated to target LDL cholesterol concentrations of 0.65-1.30 mmol/L. in this prespecified analysis, we investigated the effect of alirocumab on cardiovascular events by glycaemic status at baseline (diabetes, prediabetes, or normoglycaemia)-defined on the basis of patient history, review of medical records, or baseline HbA(1c) or fasting serum glucose-and risk of new-onset diabetes among those without diabetes at baseline. the primary endpoint was a composite of death from coronary heart disease, non-fatal myocardial infarction, fatal or non-fatal ischaemic stroke, or unstable angina requiring hospital admission. ODYSSEY OUTCOMES is registered with ClinicalTrials. gov, number NCT01663402. Findings At study baseline, 5444 patients (28.8%) had diabetes, 8246 (43.6%) had prediabetes, and 5234 (27.7%) had normoglycaemia. There were no significant differences across glycaemic categories in median LDL cholesterol at baseline (2.20-2.28 mmol/L), after 4 months' treatment with alirocumab (0.80 mmol/L), or after 4 months' treatment with placebo (2.25-2.28 mmol/L). in the placebo group, the incidence of the primary endpoint over a median of 2.8 years was greater in patients with diabetes (16.4%) than in those with prediabetes (9.2%) or normoglycaemia (8.5%); hazard ratio (HR) for diabetes versus normoglycaemia 2.09 (95% CI 1.78-2.46, p, Sanofi; Regeneron Pharmaceuticals, Sanofi and Regeneron Pharmaceuticals.

Project Data Sphere (PDS) is a research platform that provides the research community with broad access to both de-identified patient-level data from oncology clinical trials and related analytic tools. While these data are rich in measures that characterize the clinical trials under study, data providers are required to de-identify patient-level data by removing key demographic data. To address these analytic constraints, the data profiles in selected PDS patient-level cancer phase III clinical datasets have been augmented by linking the social, economic, and health-related characteristics of like cancer survivors from nationally representative health and health care-related survey data. Using statistical linkage and model-based techniques, patient-level records in selected PDS datasets have been linked to those of comparable cancer survivors, and are thereby augmented with survey content on social, economic, and health-related characteristics. These new analytically enhanced PDS data resources enable more targeted analyses designed to examine questions such as how disparities in cancer patients' access to health care and income impact patient outcomes in specific phase III clinical trials, and what variations in patient outcomes are associated with specific demographic, socioeconomic, and health-related factors. This study provides an overview of the methodologies used to connect patient-level clinical trial data with nationally representative health-related data on cancer survivors from the national Medical Expenditure Panel Survey (MEPS). MEPS was designed to provide national population-based health care use, expenditure, and source of payment estimates in addition to measures of health status, demographic characteristics, employment, health insurance coverage, and access to health care. Study findings include probabilistic assessments of the representation of the patients in the respective clinical trials relative to the characteristics of cancer survivors in the general population. The study also demonstrates how the augmented datasets serve to enable researchers to assess the impact of socioeconomic factors added through data integration on cancer survival and related outcomes of interest.

Purpose: c-KIT overexpression is well recognized in cancers such as gastrointestinal stromal tumors (GIST), small cell lung cancer (SCLC), melanoma, non–small cell lung cancer (NSCLC), and acute myelogenous leukemia (AML). Treatment with the small-molecule inhibitors imatinib, sunitinib, and regorafenib resulted in resistance (c-KIT mutant tumors) or limited activity (c-KIT wild-type tumors). We selected an anti–c-KIT ADC approach to evaluate the anticancer activity in multiple disease models. Experimental Design: A humanized anti–c-KIT antibody LMJ729 was conjugated to the microtubule destabilizing maytansinoid, DM1, via a noncleavable linker (SMCC). The activity of the resulting ADC, LOP628, was evaluated in vitro against GIST, SCLC, and AML models and in vivo against GIST and SCLC models. Results: LOP628 exhibited potent antiproliferative activity on c-KIT–positive cell lines, whereas LMJ729 displayed little to no effect. At exposures predicted to be clinically achievable, LOP628 demonstrated single administration regressions or stasis in GIST and SCLC xenograft models in mice. LOP628 also displayed superior efficacy in an imatinib-resistant GIST model. Further, LOP628 was well tolerated in monkeys with an adequate therapeutic index several fold above efficacious exposures. Safety findings were consistent with the pharmacodynamic effect of neutropenia due to c-KIT–directed targeting. Additional toxicities were considered off-target and were consistent with DM1, such as effects in the liver and hematopoietic/lymphatic system. Conclusions: The preclinical findings suggest that the c-KIT–directed ADC may be a promising therapeutic for the treatment of mutant and wild-type c-KIT–positive cancers and supported the clinical evaluation of LOP628 in GIST, AML, and SCLC patients. Massive rotator cuff tears, particularly those with significant retraction and associated muscle atrophy, can present a unique problem to the shoulder surgeon. In certain cases where repair of the tissue is not warranted or possible, debridement may offer a viable solution. This case highlights steps in treatment of a massive rotator cuff tear with atrophy of the supraspinatus musculature when the primary complaint is shoulder pain. The rotator cuff was noted to be significantly retracted and impossible to mobilize back to the footprint. The shoulder was moved immediately post-op and there was excellent pain relief for the first year postoperatively. In the low-demand patient with pain as a predominant debilitating symptom, arthroscopic debridement of massive, irreparable rotator cuff tears can offer a reliable alternative to formal repair.

Purpose: This phase I/II study evaluated safety, efficacy, and pharmacokinetics of escalating, multiple doses of siltuximab, a chimeric anti-interleukin (IL)-6 monoclonal antibody derived from a new Chinese hamster ovary (CHO) cell line in patients with advanced/refractory solid tumors. Experimental Design: In the phase I dose-escalation cohorts, 20 patients with advanced/refractory solid tumors received siltuximab 2.8 or 5.5 mg/kg every 2 weeks or 11 or 15 mg/kg every 3 weeks intravenously (i.v.). In the phase I expansion (n = 24) and phase II cohorts (n = 40), patients with Kirsten rat sarcoma-2 (KRAS)-mutant tumors, ovarian, pancreatic, or anti-EGF receptor (EGFR) refractory/resistant non–small cell lung cancer (NSCLC), colorectal, or H&N cancer received 15 mg/kg every 3 weeks. The phase II primary efficacy endpoint was complete response, partial response, or stable disease >6 weeks. Results: Eighty-four patients (35 colorectal, 29 ovarian, 9 pancreatic, and 11 other) received a median of three (range, 1–45) cycles. One dose-limiting toxicity occurred at 5.5 mg/kg. Common grade ≥3 adverse events were hepatic function abnormalities (15%), physical health deterioration (12%), and fatigue (11%). Ten percent of patients had siltuximab-related grade ≥3 adverse events. Neutropenia (4%) was the only possibly related adverse event grade ≥3 reported in >1 patient. Serious adverse events were reported in 42%; most were related to underlying disease. The pharmacokinetic profile of CHO-derived siltuximab appears similar to the previous cell line. No objective responses occurred; 5 of 84 patients had stable disease >6 weeks. Hemoglobin increased ≥1.5 g/dL in 33 of 47 patients. At 11 and 15 mg/kg, completely sustained C-reactive protein suppression was observed. Conclusions: Siltuximab monotherapy appears to be well tolerated but without clinical activity in solid tumors, including ovarian and KRAS-mutant cancers. This article reports the findings from a year-long study of 6 writing teachers in an urban elementary school who also received intensive professional development in writing instruction from a nonprofit organization. Repeated observations demonstrated that the teachers displayed consistency in their use of the core instructional elements associated with writing workshop, which aligned with the emphasis of the professional development support. However, the teachers exhibited substantial variability in their use of student engagement tactics, management techniques, and instructional supports. According to survey data, the teachers demonstrated a strong and relatively stable sense of teaching efficacy and held a fairly balanced view of the importance of explicit and incidental writing instruction, and these beliefs were related to their instructional practices. Interviews with the teachers highlighted the relevance of teachers’ own writing behaviors and attitudes.

43. Providing All-Inclusive Care for Frail Elderly Veterans: Evaluation of Three Models of Care

Frail elderly veterans aged 55 and older who met state nursing home admission criteria were enrolled in one of three models of all-inclusive long-term care (AIC) at three Veterans Affairs (VA) medical centers (n=386). The models included: VA as sole care provider, VA-community partnership with a Program of All-inclusive Care for the Elderly (PACE), and VA as care manager with care provided by PACE. Healthcare use was monitored for 6 months before and 6 to 36 months after enrollment using VA, DataPACE, and Medicare files. Hospital and outpatient care did not differ before and after AIC enrollment. Only 53% of VA sole-provider patients used adult day health care (ADHC), whereas all other patients used ADHC. Nursing home days increased, but permanent institutionalization was low. Thirty percent of participants died; of those still enrolled in AIC, 92% remained in the community. VA successfully implemented three variations of AIC and was able to keep frail elderly veterans in the community. Further research on providing variations of AIC in general is warranted.

Sensory experience is influenced by emotional context. Although perception of emotion and unpleasant visceral sensation are associated with activation within the insula and dorsal and ventral anterior cingulate gyri (ACG), regions important for attention to and perception of sensory and emotional information, the neural mechanisms underlying the effect of emotional context upon visceral sensation remain unexplored. Using functional MRI, we examined neural responses to phasic, non-painful oesophageal sensation (OS) in eight healthy subjects (seven male; age range 27-36 years) either during neutral or negative emotional contexts produced, respectively, by presentation of neutral or fearful facial expressions. Activation within right insular and bilateral dorsal ACG was significantly greater (P < 0.01) during OS with fearful than with neutral faces. In a second experiment, we measured anxiety, discomfort and neural responses in eight healthy male subjects (age range 22-41 years) to phasic, non-painful OS during presentation of faces depicting either low, moderate or high intensities of fear. Significantly greater (P < 0.01) discomfort, anxiety and activation predominantly within the left dorsal ACG and bilateral anterior insulae occurred with high-intensity compared with low-intensity expressions. Clusters of voxels were also detected in this region, which exhibited a positive correlation between subjective behaviour and blood oxygenation level-dependent effect (P < 0.05). We report the first evidence for a modulation of neural responses, and perceived discomfort during, non-painful visceral stimulation by the intensity of the negative emotional context in which the stimulation occurs, and suggest a mechanism for the effect of negative context on symptoms in functional pain disorders.

As a result of its accompanying co-morbidity, our lack of understanding regarding its mechanisms, and its resistance to conventional treatment, central pain is one of the most formidable challenges pain physicians are faced with. The objective of this review is to summarize recent advances in our understanding of the etiology, clinical presentation, and treatment of central pain, with special emphasis being placed on studies published within the past year.Recent evidence suggests that not only injuries commonly associated with central pain, such as strokes and spinal cord lesions, but also disorders such as fibromyalgia, phantom limb pain and tension-type headaches may involve central phenomena. Perhaps because of the lack of clinical trials, treatment is still largely based on traditional prescribing methods and anecdotal evidence. Recent studies have indicated possible roles for tricyclic antidepressants, anti-seizure medications, and motor cortex stimulation in the treatment of central pain.Injury to the spinothalamocortical pathways is a necessary, but not sufficient factor in the pathogenesis of central pain. Perhaps because of the similarities in mechanisms, there is considerable overlap between effective treatments for central pain and those for peripheral neuropathic pain. Our poor understanding of the etiology of central pain and the relative lack of effective treatments emphasize the need for further research into this elusive disorder.

The destruction of the World Trade Center on September 11, 2001, resulted in a rapid response by government to evaluate the area and, in subsequent days, to bring the city back to a semblance of normalcy. This article provides a case study of government action in New York city during and after the World Trade Center catastrophe. What is most striking is the skill and intensity of government's response to the emergency and the hard work, dedication, and bravery of New York City's government officials. This article presents government and public service at its finest, under some of the most difficult conditions one can imagine.
