1. Exploring the Value of Improvisational Theater in Medical Education for Advancing the Doctor–Patient Relationship and Health Equity

Abstract

PURPOSE: Health care inequities persist, and it is difficult to teach health professions students effectively about implicit bias, structural inequities, and caring for patients from underrepresented or minoritized backgrounds. Improvisational theater (improv), where performers create everything in a spontaneous and unplanned manner, may help teach health professions trainees about advancing health equity. Core improv skills, discussion, and self-reflection can help improve communication, build trustworthy relationships with patients, and address bias, racism, oppressive systems, and structural inequities. METHOD: Authors integrated a 90-minute virtual improv workshop using basic exercises into a required course for first-year medical students at University of Chicago in 2020. Sixty randomly chosen students took the workshop and 37 (62%) responded to Likert-scale and open-ended questions about strengths, impact, and areas for improvement. Eleven students participated in structured interviews about their workshop experience. RESULTS: Twenty-eight (76%) of 37 students rated the workshop as very good or excellent, and 31 (84%) would recommend it to others. Over 80% of students perceived their listening and observation skills improved, and that the workshop would help them take better care of non-majority- identifying patients. Six (16%) students experienced stress during the workshop but 36 (97%) felt safe. Eleven (30%) students agreed there were meaningful discussions about systemic inequities. Qualitative interview analysis showed that students thought the workshop helped develop interpersonal skills (communication, relationship building, empathy); helped personal growth (insights into perception of self and others, ability to adapt to unexpected situations); and felt safe. Students noted the workshop helped them to be in the moment with patients and respond to the unexpected in ways more structured, traditional communication curricula have not. The authors developed a conceptual model relating improv skills and equity teaching methods to advancing health equity. CONCLUSIONS: Improv theater exercises can complement traditional communication curricula to advance health equity.

Abstract

Reptiles are considered an important element of Saudi Arabia's wildlife to be conserved as a priority. However, the status and distribution of the Kingdom's reptile fauna is not well understood, thus hindering the conservation initiatives. Better understanding of the taxonomy and distribution of the Kingdom's reptile fauna is important for implementing effective conservation measures.Here, we provide the new distribution record of Pristurus minimus (Arnold, 1977; common name, Arnold's Gecko) from southern Saudi Arabia. The species was recorded from the Uruq Bani Ma'arid Protected Area (UBM) of southern Saudi Arabia. Four individuals were captured from different survey sites across the UBM and their species identification was confirmed through a mixture of physical and genetic examination. These results increase the number of species from the Pristurus genus to five for Saudi Arabia and improve the understanding of the Kingdom's reptile fauna and its distribution.

Abstract

This article focuses on the links between transatlantic relations—a structured array of markets, hierarchies, networks, ideas, and institutions—and broader elements of international structure and world order. It argues that the changing state of transatlantic relations reflects changes in the structure of the relations themselves, but also structural change in the global and domestic arenas and how such change shapes or reflects the actions of a wide variety of agents. The first part of the article briefly explores the importance of international structure in order to identify the global forces that shape the context for transatlantic relations. The article then examines the key mechanisms in transatlantic relations which interact to create forms of transatlantic order; these create spaces for a wide variety of agents, operating within broader elements of international and domestic structure, and the article illustrates this through the ways in which the EU’s “new agenda for EU–US relations” sought to shape transatlantic interactions during the first year of the Biden presidency. The article examines the implications of transatlantic responses to the Russian attack on Ukraine in February 2022, and concludes that despite the move to enhanced EU–US cooperation in the short term, the interaction of structures, mechanisms, and actors will contribute to continuing differentiation of transatlantic relations, at least in the medium term, whatever the preferences of US and EU policy-makers.

Abstract

Here the Human Pangenome Reference Consortium presents a first draft of the human pangenome reference. The pangenome contains 47 phased, diploid assemblies from a cohort of genetically diverse individuals1. These assemblies cover more than 99% of the expected sequence in each genome and are more than 99% accurate at the structural and base pair levels. Based on alignments of the assemblies, we generate a draft pangenome that captures known variants and haplotypes and reveals new alleles at structurally complex loci. We also add 119 million base pairs of euchromatic polymorphic sequences and 1,115 gene duplications relative to the existing reference GRCh38. Roughly 90 million of the additional base pairs are derived from structural variation. Using our draft pangenome to analyse short-read data reduced small variant discovery errors by 34% and increased the number of structural variants detectedper haplotype by 104% compared with GRCh38-based workflows, which enabled the typing of the vast majority of structural variant alleles per sample.

Abstract

Recurrent neural networks (RNNs) with rich feature vectors of past values can provide accurate point forecasts for series that exhibit complex serial dependence. We propose two approaches to constructing deep time series probabilistic models based on a variant of RNN called an echo state network (ESN). The first is where the output layer of the ESN has stochastic disturbances and a shrinkage prior for additional regularization. The second approach employs the implicit copula of an ESN with Gaussian disturbances, which is a deep copula process on the feature space. Combining this copula with a non-parametrically estimated marginal distribution produces a deep distributional time series model. The resulting probabilistic forecasts are deep functions of the feature vector and also marginally calibrated. In both approaches, Bayesian Markov chain Monte Carlo methods are used to estimate the models and compute forecasts. The proposed models are suitable for the complex task of forecasting intraday electricity prices. Using data from the Australian National Electricity Market, we show that our deep time series models provide accurate short term probabilistic price forecasts, with the copula model dominating. Moreover, the models provide a flexible framework for incorporating probabilistic forecasts of electricity demand as additional features, which increases upper tail forecast accuracy from the copula model significantly.

Abstract

We present a novel multipole formulation for computing the band structures of two-dimensional arrays of cylindrical Helmholtz resonators. This formulation is derived by combining existing multipole methods for arrays of ideal cylinders with the method of matched asymptotic expansions. We construct asymptotically close representations for the dispersion equations of the first band surface, correcting and extending an established lowest-order (isotropic) result in the literature for thin-walled resonator arrays. The descriptions we obtain for the first band are accurate over a relatively broad frequency and Bloch vector range and not simply in the long-wavelength and low-frequency regime, as is the case in many classical treatments. Crucially, we are able to capture features of the first band, such as low-frequency anisotropy, over a broad range of filling fractions, wall thicknesses and aperture angles. In addition to describing the first band we use our formulation to compute the first band gap for both thin- and thick-walled resonators, and find that thicker resonator walls correspond to both a narrowing of the first band gap and an increase in the central band gap frequency.

Abstract

We present a solution method which combines the technique of matched asymptotic expansions with the method of multipole expansions to determine the band structure of cylindrical Helmholtz resonator arrays in two dimensions. The resonator geometry is considered in the limit as the wall thickness becomes very large compared with the aperture width (the extremely thick-walled limit). In this regime, the existing treatment in Part I (Smith & Abrahams, 2022 Tailored acoustic metamaterials. Part I. Thin- and thick-walled Helmholtz resonator arrays), with updated parameters, is found to return spurious spectral behaviour. We derive a regularized system which overcomes this issue and also derive compact asymptotic descriptions for the low-frequency dispersion equation in this setting. We find that the matched-asymptotic system is able to recover the first few bands over the entire Brillouin zone with ease, when suitably truncated. A homogenization treatment is outlined for describing the effective bulk modulus and effective density tensor of the resonator array for all wall thicknesses. We demonstrate that extremely thick-walled resonators are able to achieve exceptionally low Helmholtz resonant frequencies, and present closed-form expressions for determining these explicitly. We anticipate that the analytical expressions and the formulation outlined here may prove useful in designing metamaterials for industrial and other applications.

11. Gratitude writing for positive affect: Moderation by life satisfaction

Abstract

Expressing gratitude is associated with increases in feelings of positive emotion. In a recent study, we found that writing a letter of gratitude, thanking a person for something they said or did in the past, was associated with an increase in positive affect. We also found that this effect was moderated by life satisfaction - the effect was only observed for participants with moderate and high levels of life satisfaction. This was an unregistered, exploratory study. It is now important that we replicate this finding, to show that it is a true effect and not something that we observed by chance. In the exploratory study, we also informed participants about the benefits of writing a gratitude letter - here, we aim to remove this information to investigate whether we can replicate the finding when these demand characteristics are removed.

Abstract

Condensation and sublimation of ices at the surface of the planet is a key part of both the Martian H$_2$O and CO$_2$ cycles, either from a seasonal or diurnal aspect. While most of the ice is located within the polar caps, surface frost is known to be formed during nighttime down to equatorial latitudes. Here, we use data from the Emirates Mars Infrared Spectrometer (EMIRS) onboard the Emirates Mars Mission (EMM) to monitor the diurnal and seasonal evolution of the ices at the surface of Mars over almost one Martian year. The unique local time coverage provided by the instrument allows us to observe the apparition of equatorial CO$_2$ frost in the second half of the Martian night around the equinoxes, to its sublimation at sunrise.

Abstract

Contrary to Google Search's mission of delivering information from "many angles so you can form your own understanding of the world," we find that Google and its most prominent returned results - Wikipedia and YouTube - simply reflect a narrow set of cultural stereotypes tied to the search language for complex topics like "Buddhism," "Liberalism," "colonization," "Iran" and "America." Simply stated, they present, to varying degrees, distinct information across the same search in different languages, a phenomenon we call 'language bias.' This paper presents evidence and analysis of language bias and discusses its larger social implications. Instead of presenting a global picture of a complex topic, our online searches and emerging tools like ChatGPT turn us into the proverbial blind person touching a small portion of an elephant, ignorant of the existence of other cultural perspectives. Piecing together a genuine depiction of the elephant is a challenging and important endeavor that will require collaborative efforts from scholars in both the humanities and technology.

As deep learning-based computer vision algorithms continue to improve and advance the state of the art, their robustness to real-world data continues to lag their performance on datasets. This makes it difficult to bring an algorithm from the lab to the real world. Ensemble-based uncertainty estimation approaches such as Monte Carlo Dropout have been successfully used in many applications in an attempt to address this robustness issue. Unfortunately, it is not always clear if such ensemble-based approaches can be applied to a new problem domain. This is the case with panoptic segmentation, where the structure of the problem and architectures designed to solve it means that unlike image classification or even semantic segmentation, the typical solution of using a mean across samples cannot be directly applied. In this paper, we demonstrate how ensemble-based uncertainty estimation approaches such as Monte Carlo Dropout can be used in the panoptic segmentation domain with no changes to an existing network, providing both improved performance and more importantly a better measure of uncertainty for predictions made by the network. Results are demonstrated quantitatively and qualitatively on the COCO, KITTI-STEP and VIPER datasets., Comment: 15 pages, 6 figures

Abstract

There is a major workforce crisis in the health and social care sector. This is driven in part by low morale and poor staff wellbeing, in turn leading to high staff turnover and chronic absenteeism. Consequently, quality of care and the experience of service users suffer. While major reforms and adequate funding is required to fully address this problem, the UK government has recognised the need for this sector to prioritise workplace wellbeing. A variety of employer and peer-led initiatives, such as meditation, yoga, physical exercise and mindfulness are suggested. However, many of these initiatives have issues with feasibility and lack the evidence base to support their efficacy. Our ongoing research programme provides evidence for the feasibility and efficacy of an alternative activity – expressive writing. Formal testing of expressive writing in the health and social care sector is now needed to determine the effectiveness and support future implementation of the approach. Here, we are interested in understanding how expressive writing practice can best be embedded in the daily lives of busy health and social care workers. Thus, we are keen to explore the barriers to uptake of these activities in relation to the COM-B (Capability, Opportunity, Motivation - Behaviour) model. This will include 1:1 interviews with health and social care workers, to seek their views on different expressive writing intervention approaches.

Abstract

Stochastic models with global parameters $\bm{\theta}$ and latent variables $\bm{z}$ are common, and variational inference (VI) is popular for their estimation. This paper uses a variational approximation (VA) that comprises a Gaussian with factor covariance matrix for the marginal of $\bm{\theta}$, and the exact conditional posterior of $\bm{z}|\bm{\theta}$. Stochastic optimization for learning the VA only requires generation of $\bm{z}$ from its conditional posterior, while $\bm{\theta}$ is updated using the natural gradient, producing a hybrid VI method. We show that this is a well-defined natural gradient optimization algorithm for the joint posterior of $(\bm{z},\bm{\theta})$. Fast to compute expressions for the Tikhonov damped Fisher information matrix required to compute a stable natural gradient update are derived. We use the approach to estimate probabilistic Bayesian neural networks with random output layer coefficients to allow for heterogeneity. Simulations show that using the natural gradient is more efficient than using the ordinary gradient, and that the approach is faster and more accurate than two leading benchmark natural gradient VI methods. In a financial application we show that accounting for industry level heterogeneity using the deep model improves the accuracy of probabilistic prediction of asset pricing models.

Form follows function throughout the development of an organism. This principle should apply beyond the organism to the nests they build, but empirical studies are lacking. Honeybees provide a uniquely suited system to study nest form and function throughout development because we can image the three-dimensional structure repeatedly and non-destructively. Here, we tracked nest-wide comb growth in six colonies over 45 days (control colonies) and found that colonies have a stereotypical process of development that maintains a spheroid nest shape. To experimentally test if nest structure is important for colony function, we shuffled the nests of an additional six colonies, weekly rearranging the comb positions and orientations (shuffled colonies). Surprisingly, we found no differences between control and shuffled colonies in multiple colony performance metrics—worker population, comb area, hive weight and nest temperature. However, using predictive modelling to examine how workers allocate comb to expand their nests, we show that shuffled colonies compensate for these disruptions by accounting for the three-dimensional structure to reconnect their nest. This suggests that nest architecture is more flexible than previously thought, and that superorganisms have mechanisms to compensate for drastic architectural perturbations and maintain colony function.

Abstract

In May 2023, Liverpool hosted the Eurovision Song Contest on behalf of Ukraine, involving 37 competing nations. With over 12,000 accredited staff involved in the planning and delivery of Eurovision, it is the largest and most complex multi-agency event that Liverpool has ever hosted. The organisations involved included Culture Liverpool, Merseyside Police, Liverpool City Council, North West Ambulance Service, Liverpool One, BBC, Maritime and Coastguard Agency, Camp and Furnace, Merseyside Fire and Rescue Service, and many more. This presented an exciting opportunity to explore multi-agency working for a major live event. The research will identify the key themes associated with a multi-agency approach, around relationship building, consistency, longevity, and trust. It will also investigate the ways in which multi-agency information sharing takes place in the preparation and delivery of a large-scale live event, informing a UK Research and Innovation Future Leaders Fellowship on law enforcement information exchange. To understand how agencies work together and share information for Eurovision 2023, the following data is collected: i.) field observations during multi-agency meetings, the multi-agency tabletop exercise, and the live events (i.e. opening ceremony, semi-finals, final); ii.) policy documents; iii.) post-event online surveys; and iv.) post-event semi-structured interviews. Participants include practitioners from a range of the organisations involved. Quantitative (descriptives) and qualitative (content, thematic) analyses will be conducted. As a ‘work-in-progress’, an overview of the research and headline findings will be presented. Abstract

BackgroundInebilizumab is an anti-CD19 antibody approved for the treatment of neuromyelitis optica spectrum disorder (NMOSD) in adults with aquaporin-4 autoantibodies. The relationship between B-cell, plasma-cell (PC), and immunoglobulin depletion with longitudinal reductions in NMOSD activity after inebilizumab treatment was characterised post hoc in an exploratory analysis from the N-MOmentum study (NCT02200770).MethodsPeripheral blood CD20+ B cells, PC gene signature, and immunoglobulin levels were assessed throughout N-MOmentum (follow-up ≥2.5 years); correlations with clinical metrics and magnetic resonance imaging (MRI) lesion activity were assessed.FindingsInebilizumab induced durable B-cell and PC depletion within 1 week versus placebo. Although no association was observed between B-cell counts at time of attack and NMOSD activity, depth of B-cell depletion after the first dosing period correlated with clinical outcomes. All participants receiving inebilizumab demonstrated a robust long-term therapeutic response, and participants with ≤4cells/μL after the first 6-month dosing interval had persistently deeper B-cell depletion, lower annualised attack rates (estimated rate [95% CI]: 0.034 [0.024-0.04] vs 0.086 [0.056-0.12]; p=0.045), fewer new/enlarging T2 MRI lesions (0.49 [0.43-0.56] vs 1.36 [1.12-1.61]; p&nbsp

Abstract

Aiming to investigate whether a positive writing intervention can alleviate the subjective and cardiovascular effects of acute stress when completing a critical social evaluation task based on the trier social stress test (TSST). Additionally, we will investigate the extent to which the effect of this positive writing intervention on acute stress reactivity is moderated by individual differences, specifically: ratings of self-esteem, perseverative thinking, and personality type.
