My undergraduate thesis portfolio includes both a technical report and STS research paper. I wrote my technical report on an internship project from the summer of 2022, where I built a service development and production support API and web application. My motivations for writing about this project were that it demonstrates the development process and priorities of modern technology departments, and was the most formative engineering experience I had during my time at UVA. My STS research paper was written on how attention and engagement engineering in Meta’s social networking platforms has contributed to addiction among Generation Z, and was motivated by the increase in social media usage during the COVID-19 pandemic and the fact that I have a personal connection to the topic as a member of Gen Z. While the two papers are not connected in subject matter, they both draw attention to the way that the software architecture of modern companies is structured. The technical project is an example of one of these smaller systems that fit into a company’s suite of internal services, while the massive platforms that are the subject of the research are composed of a variety of these smaller services containing different functionalities. My technical report details a finance-area product team at a used car retailer and financer based out of Richmond, Virginia that decided to improve access and usability for various elusive, confusing, and time-consuming functionalities for service development and production support. The system falls under the category of information technology service management, which has become increasingly important to companies for increasing the efficiency of software development teams. As an intern, I achieved this using a custom API and web application that combined functionalities into a centralized suite of tools. The project, titled Service Operation Management (SOM), was built first as an ASP.NET API and later integrated with a web application. Using React and an internal design library, my mentor and I developed a user interface so these tools could be readily available for software developers in the finance area. The functionality of the tools in the SOM comes from both custom company systems and Microsoft Azure, and it has increased the agility and efficiency of the team that developed it. Plans include adding more tools, building out the administration system for customization purposes, as well as rolling the system out to other teams in the company. In the STS research paper I examined the addictive effects of Meta’s Facebook and Instagram platforms on members of Generation Z as a result of engagement and attention engineering practices, and investigated how society might remedy the negative social impacts it has had to protect future generations. Sources included research from academic databases, articles from reputable news outlets, proposed legislation, and books written by experts in the psychology or technology industries. This case study analysis covers the engineering tactics used by Meta and the relationship that they have with social media addiction in young users. Beyond that, I studied how the business models encourage Meta’s practices and how the engineering and widespread adoption of the platforms, for both personal and business uses, have formed a positive feedback loop that has caused the addiction problem to proliferate. This subject was analyzed through the lens of Actor Network Theory, where many relevant human and non-human actors were considered in addition to the ways in which the technology and end users interact with each other. I concluded by informing the reader about the failures of our legislative system, ideas for future work in the area, and recommending some paths forward to a solution. In producing these documents, I gained an immense amount of experience and skill in writing, research, and communication of technical subject matter. In producing these papers simultaneously, I was able to observe the differences in process and structure of papers that serve entirely different, yet both exist in the realm of engineering and computer science. Through writing the technical report, I improved my ability to communicate complex technical concepts to a non-technical audience. This is a valuable skill that I will leverage throughout my professional life, as I will often need to define requirements or discuss system plans with clients or management. As for the STS research paper, I will forever view technologies that I observe, use, or create with a new found understanding of their intricate construction and vast societal implications. Producing these two papers simultaneously has demonstrated the breadth of the purpose and format of engineering and technological knowledge production and communication, growing my competence and confidence as a young engineer just entering the industry.

The ongoing COVID-19 pandemic has motivated social workers to reckon with and transform traditions in service delivery. The development, application, and evaluation of technology-enhanced practices have become more vital than ever. Garden on the Balcony (GOB) was an innovative internet-based social work service designed to respond rapidly to the COVID-19 outbreak in Beijing. This paper introduces the underlying perspectives and design of GOB and reports participants’ reflections on the program to understand its mechanisms and implications. Interview data from GOB participants were collected 4 months after the program ended. Thematic analysis generated three major themes, suggesting that GOB had (a) promoted individual resilience and family cohesion; (b) built online and offline community bonds; and (c) cultivated a green lifestyle and spiritual reflection on life. This study demonstrates a practical example of the effective use of technology-enhanced practice.

Despite its importance, for micro, small, and, medium-sized enterprises (MSMEs), the impact of new service development performance (NSDP) on organizational performance (OP) and the significance of formal or standardized service development processes (SSDPs) remains unknown. This study proposes that NSDP is positively associated with OP and that SSDPs positively moderate this direct effect. These and other hypotheses are tested with data collected from 801 Irish service MSMEs using Partial Least Squares Structural Equation Modelling (PLS-SEM) and multigroup analysis. Our results reveal that a positive relationship exists between NSDP and OP and we report the magnitude of this relationship for micro, small, medium-sized organizations; while the moderating effect of SSDPs was not supported for each of these groups. We thus provide novel empirical insights into service development for MSMEs, where our findings suggest that SSDPs are of particular importance for small organizations.

Background/aims The rehabilitation community transition support team was created as a response to the COVID-19 pandemic, resulting in faster discharges from the inpatient rehabilitation service. The aim of this evaluation was to explore the perspectives of staff and patients on their experience of the rehabilitation community transition support team. Methods Staff and patients in the new team were interviewed using semi-structured interviews. The data were analysed using thematic analysis. Results Staff interviews generated seven main themes: positive staff experiences; defining the ‘team’; mode of working; link role for the team; technology; relationships with patients; and support from colleagues. The patient analysis generated five themes: positive experiences of the rehabilitation community transition support team; relationship with rehabilitation community transition support team worker; mode of working; handling the hurdles of discharge; and defining the ‘team’. There were crossovers of staff and patient themes, particularly surrounding around face-to-face visits, defining the team and relationships. Patients indicated that contact with the community team helped them to overcome both practical and emotional hurdles of discharge. Conclusions The presence of a team supporting the transition from hospital to a community setting may be helpful for people who have been discharged.

Aims Elective orthopaedic surgery was cancelled early in the COVID-19 pandemic and is currently running at significantly reduced capacity in most institutions. This has resulted in a significant backlog to treatment, with some hospitals projecting that waiting times for arthroplasty is three times the pre-COVID-19 duration. There is concern that the patient group requiring arthroplasty are often older and have more medical comorbidities—the same group of patients advised they are at higher risk of mortality from catching COVID-19. The aim of this study is to investigate the morbidity and mortality in elective patients operated on during the COVID-19 pandemic and compare this to a pre-pandemic cohort. Primary outcome was 30-day mortality. Secondary outcomes were perioperative complications, including nosocomial COVID-19 infection. These operations were performed in a district general hospital, with COVID-19 acute admissions in the same building. Methods Our institution reinstated elective operations using a “Blue stream” pathway, which involves isolation before and after surgery, COVID-19 testing pre-admission, and separation of ward and theatre pathways for “blue” patients. A register of all arthroplasties was taken, and their clinical course and investigations recorded. Results During a seven-month period, 340 elective arthroplasties were performed. There was zero mortality. One patient had a positive swab for COVID-19 while an inpatient, but remained asymptomatic. There were two readmissions within a 12-week period for hip dislocation. Patients had a mean age of 68 years (28 to 90), mean BMI of 30 kg/m2 (19.0 to 45.6), and mean American Society of Anesthesiologists grade of 2 (1 to 3). Conclusion Results show no increased morbidity or mortality in this cohort of patients compared to the same hospital’s morbidity and mortality pre-COVID-19. The screened pathway for elective patients is effective in ensuring that patients can be safely operated on electively in an acute hospital. This study should reassure clinicians and patients that arthroplasties can be carried out safely when the appropriate precautions are in place. Cite this article: Bone Jt Open 2021;2(11):940–944.

Purpose The purpose of this study is to gain a deeper understanding of female spouses’ lived experiences of changes in everyday life while living with a husband with dementia. Design/methodology/approach Nine individual interviews of female spouses were conducted in 2017. A phenomenological narrative approach was applied during data collection, and the analysis was inspired by Amedeo Giorgi’s analytic steps. Findings Female spouses experienced changes in their marital relationships, and found ways of managing these changes, although they realized life was marked by loneliness and distress. The identified themes reveal how female spouses experienced changes in everyday life as the disease progressed. Everyday routines gradually changed and they actively sought ways to uphold everyday life and a marital relationship. Research limitations/implications Research should focus on developing supportive interventions, where the people with the lived experiences in relation to dementia are involved in the research process, to better target the needs for support, when developing interventions. Practical implications Insight into everyday life can help health-care service providers to better the support to female spouses and contribute with more individualized support, which may contribute to the quality of care. Originality/value In this study, the authors disclose the invisible and silent work that takes place in an everyday life, when living with a husband with dementia during the time span of caregiving. Spouses’ experiences are important to include, when developing intervention to support spouses to better tailor the interventions.

The topicality of reforming the children palliative care (CPC) system due to the increase of the number of patients with cancer, congenital disorders, neurological damages, injuries consequences, genetically determined pathologies is discussed. The patterns of CPC service development, the importance to track those in need, to determine the required volume, forms, and methods of palliative care, and to develop human capacity were analyzed. The results of “Children's Hospice" (Saint Petersburg) activities in 2014-2019 years are presented alongside with the data on CPC development in Russia. This example shows the advantages of clear routing, establishment of indications for patients’ hospitalization, and availability of high-technology medical care and medications. All innovations have led to the significant change in medical conditions structure (over 80% of patients are children with neurological pathology), to the decrease in the number of hospitalizations, to the increase in the number of children who can get required respiratory support at home. The issues of high employees’ turnover and low staffing level in CPC institutions are discussed.

The Netherlands has an unique system of perinatal and postpartum healthcare. Pregnancy care is delivered predominantly by primary care midwives and childbirth services predominantly involve hospital care. The first week postpartum, all women receive daily care from maternity nurses. In addition, hospitals and out-patient clinics offer perinatal mental healthcare. More specifically, ‘POP care’ (psychiatry, obstetrics and paediatrics) was developed to promote multidisciplinary collaboration in this regard. Although clinical practices and government initiatives to improve pregnancy-related mental healthcare work well, they have yet to be fully described and evaluated. The current COVID-19 pandemic has an impact on health services and perinatal mental health.

The paper examines the formation of the contraceptive method mix within the framework of family planning programmes in developing countries, taking into account the influence of socio-demographic, economic, and behavioural factors. The study has shown that the impact of family planning programmes on changes in the method mix is ambiguous and depends on the time of the start of the programme, the region of the country, the level of medical service development, and other factors. The paper also analyzes the correlation between the peculiarities of the method mix in a country and abolishment of particular methods. The author reveals that in developing countries the poorest users of young reproductive age are the most vulnerable and prone to contraception discontinuation. It is these population groups that experience the most dissatisfaction with family planning.

Purpose This study aims to purpose a strategy for Financial Technology (Fintech) service development to increase user adoption of Fintech services. Design/methodology/approach A conceptual model based on a risk-benefit approach was adopted based on the theory of reasoned action, the theory of planned behaviour and technology acceptance models. The partial least squares structural equation modelling method was used to test the proposed model and estimate the relationship between constructs. The results are used to develop strategies, which are then categorised based on the integration of an importance-performance analysis and the Kano model. Findings The results show that trust, economic benefit and convenience significantly influence users to adopt Fintech services, whereas privacy awareness, financial risk and legal risk factors affect user trust. A strategy for consumer protection against security threats and financial losses warrants the highest priority for improvement. Practical implications The findings of this study can help Fintech product managers classify their service improvements and help regulators make the appropriate policy decisions related to customer security and the adoption of Fintech. Social implications There are some limitations of this research that should be noted. First, the research target respondents were millennials who have been statistically proven to have the highest affinity in using Fintech services. Second, the Fintech services used in this research were only mobile payments, peer-to-peer lending and crowdfunding. Future research could expand the number of research respondents to improve research validity, including other Fintech services or focus on one Fintech service at a time for targeted results and consider moderation variables, such as customer experience with Fintech services based on age and gender. Originality/value This study contributes to the development of new frameworks for evaluating Fintech adoption intention based on a risk-benefit approach. It also presents strategy recommendations to be implemented by Indonesian Fintech companies.

The study aimed to determine the role of community partnership in improving the quality of social services through: determining the role of community partnership in providing social services, achieving adequacy of social services, achieving fair distribution of social services, sustainability of social services, as well as identifying the obstacles facing community partnership in improving the quality of Social services, to reach a proposed vision to activate the role of community partnership in improving the quality of social services. To achieve the objectives of the study, it relied on a measurement form as a main tool prepared by the researcher by reviewing previous studies and consulting with experts. The tool consists of (42) phrases in addition to the primary data distributed as follows: primary data (6) questions, the role of community partnership in providing social services (7) phrases, achieving the adequacy of social services (7) phrases, achieving fair distribution of social services (7) phrases ,Sustainability of social services (7) Phrases, Obstacles facing community partnership (7) Phrases, reaching a proposed vision to activate the role of community partnership in improving the quality of social services (7) Phrases. Its validity and reliability were confirmed, and it was field-applied to a sample of (138) items distributed among community partnership projects in Fayoum Governorate. The study reached several results, which are: The community partnership has an important role in achieving the availability of social services by: providing these services, introducing them, and meeting the basic needs. Its role in achieving the adequacy of social services is represented in: providing services at a good level, facilitating the procedures for obtaining them, benefiting from them in improving income, diversifying. Its role in achieving fair distribution of social services has been: good treatment, free access to service, and service to those in need. As for its role in the sustainability of social services, it included: access to service continuously, tailored to needs, search for new services. As for the obstacles facing community partnership in improving the quality of social services, the most important of which are: lack of project development, disproportionate services to needs, services focus on one category. The proposals to activate the role of community partnership were: service development, continuous needs study, new services, interest in income improvement projects, and streamlined access to services. Our systematic review will synthesise empirical research literature describing how voluntary positive feedback from healthcare service users can create change in healthcare staff and systems. This registration is for a protocol describing how the systematic review will be conducted.

Introduction: In recent years, the mental health services have undergone much structural and organizational change. The battle between evidence-based practice and human-rights based approaches continues to rage onwards despite the growing evidence suggesting that both ideologies can be met through the adoption of a recovery-orientated service. There is much evidence available to suggest what such a service would look like if this became a reality. In Irish mental health services, ‘A National Framework for Recovery in Mental Health’ is the leading document supporting mental health services in their journey towards recovery orientation. Within the document, co-production is listed as a core recovery principle that underpins the organizational change necessary for recovery to become a reality. However, little empirical evidence is available to support its use within a mental health context. Therefore, this proposed scoping review aims to explore the concept of co-production within a mental health context in an effort to support a larger project into its implementation within such services. Method & Analysis: This proposed scoping review aims to investigate the concept of co-production within a mental health context. A PRISMA compliant scoping review is proposed, based on Arkzey and O’Malley’s framework. This framework documents a five staged approach to conducting scoping reviews. Search terms will be stated, and a variety of databases and repositories will be examined for relevant papers based on a stated inclusion/exclusion criteria. Discussion: The proposed scoping review will identify the various definitions of co-production whilst also identifying the potential effects co-production may have on mental health service provision. It will present gaps in the current literature which can be addressed by the larger project. Several limitations are also noted, including the inability to assess study quality and the amalgamation of two separate but interrelated research questions into one proposed scoping review.

Purpose Children are gradually attaining recognition as service users and their involvement in service development has been advanced in recent years. This study draws on empirical research in social and health-care services designed for children and families. The purpose of this paper is to analyse how professionals understand children’s involvement as experts by experience. The focus is on professionals’ views and intergenerational relations. Design/methodology/approach The research data comprise 25 individual and 10 group interviews with managers and professionals working in social and health-care services in one Finnish province. The data were analysed using qualitative thematic analysis. Findings The professionals recognised the value of children’s service user involvement. However, they concentrated more on the challenges than the possibilities it presents. Health-care professionals emphasised parental needs and children’s vulnerability. In turn, the professionals from social services and child welfare non-governmental organisations perceived children as partners, although with reservations, as they discussed ethical issues widely and foregrounded the responsibilities of adults in protecting children. In general, the professionals in both domains saw themselves as having ethical responsibility to support children’s service user involvement while at the same time setting limits to it. Originality/value This study confirmed the importance of taking intergenerational relations into account when developing children’s service user involvement. The results indicate that professionals also need to reflect on the ethical challenges with children themselves as, largely owing to the generational position of children as minors, they rarely perceive them as partners in ethical reflection.

The postnatal period is high-risk time for the first onset and recurrence of maternal mental health disorders. Untreated maternal mental illness can have significant adverse impacts on a woman, her baby, and the wider family unit. For women with mental illnesses that cannot be managed in the community, psychiatric inpatient mother-baby units are the gold standard treatment whereby mothers are co-admitted with their infant for specialist perinatal and infant mental health assessment and treatment. The study explores the model of care and examines the philosophies of care that are used within a psychiatric mother-baby unit. Purposive sampling was used to conduct semi-structured focus group and individual interviews with multidisciplinary staff members at a single mother-baby unit. Themes derived from these interviews were coded into two primary themes and a range of sub-themes. The first primary theme focused on the Model of Care consisting of the following sub-themes: mental health care, physical health care, babies’ care, building mother-baby relationship, fostering relationships with supports, and facilitating community support. The second primary theme centered around the Philosophy of Care comprising of: person-centered care, trauma-informed care, compassion-centered care, recovery-oriented care, attachment-informed care, non-judgmental care, strengths-based care and interdisciplinary care. The model can be used to provide consistency across mother-baby units and to support core capabilities of staff in providing an optimal level of care.

Background: The transition between child and adult services should aim to support young people into the next stage of their life in a way that optimises their function. Yet financial, organisational and procedural barriers to continuity of care often hamper smooth transition between child and adult services. Aim and method: We reviewed studies of transition from child to adult mental health services, focusing on: (i) rates of referrals and referral acceptance; (ii) barriers and facilitators of successful transition; (iii) continuity of care during and post-transition; and (iv) service users’ experience of transition. Studies were identified through systematic searches of electronic databases: PsychINFO, Medline, Embase and Child Development and Adolescent Studies. Findings: 47 papers describing 43 unique studies met inclusion criteria. Service provision is influenced by previous history and funding processes, and the presence or absence of strong primary care, specialist centres of excellence, and coordination between specialist and primary care. Provision varies between and within countries, particularly in whether services are restricted to “core” mental health or broader needs. Unsupportive organisational culture, fragmentation of resources, skills and knowledge-base undermine the collaborative working essential to optimise transition. Stigma and young people’s concerns about peers’ evaluation often prompts disengagement and discontinuation of care during transition, leading to worsening of symptoms and later, to service re-entry. Qualitative studies reveal that young people and families find the transition process frustrating and difficult, mainly because of lack of advanced planning and inadequate preparation. Conclusion: Despite increasing research interest over the last decade transition remains “poorly planned, executed and experienced”. Closer collaboration between child and adult services is needed to improve the quality of provision for this vulnerable group at this sensitive period of development.

Since the 1990s, New Zealand mental health policy has shifted from a focus on the management of symptoms and risk to the recovery of psychological, social, cultural, and physical well-being. Despite a vision for recovery-oriented services being integrated within national and regional policies, there is growing concern that barriers to recovery-oriented service provision continue to exist. Such barriers include the attitudes, skills, and knowledge of front-line staff, as well as system issues. This study explored the experience and meaning of recovery-oriented practice for 10 nurses working in an acute inpatient mental health service. A phenomenological and hermeneutic lens was used to explore the nurses' experience of working in a recovery-focused manner alongside service users. Stories of practice were collected from participants through open-ended conversational interviews. Transcribed narratives were analysed to explore taken-for-granted aspects of working in acute mental health care and to uncover the meaning of being recovery-oriented in this setting. Findings revealed that although the experience and meaning of recovery-focused care varied among nurses, there were common elements in the practice accounts. The accounts highlighted the nurses' role in creating different therapeutic spaces to promote safety, relational commitment, and healing for service users. However, the nurses faced challenges to recovery-oriented care within the team hierarchical culture and the broader service systems. The nurses were, at times, fearless in advocating for service users and recognized that this was essential for developing recovery-focused services. The findings have implications for nursing practice, as well as training and service development.

Background/aims This was a pilot study that was part of a Department of Health and Social Care initiative to improve access to cognitive behavioural therapies for patients with long-term health conditions. The service development work involved integrating an ‘Improving Access to Psychological Therapies’ service with a cardiac rehabilitation service, to treat patients with cardiac disease and comorbid anxiety and/or depression. The aim was to reduce levels of anxiety and depression, urgent care visits, inpatient stays and outpatient appointments, and secondary care service usage. Methods A repeated-measures design was implemented using clinical data collected as part of routine practice. A quasi-control group was used for the economic evaluation, comparing patients who entered into therapy with those who did not. Collaboration between an Improving Access to Psychological Therapies and cardiac rehabilitation service identified 222 cardiac patients (142 males; 80 females) with comorbid anxiety and/or depression to access the pathway over 29 months. Patients accessed psychological therapy using the Improving Access to Psychological Therapies stepped-care model, where therapy intensity and durations varied according to patient need and complexity. Psychological outcomes were measured at three time points: assessment; end of contact; and 3-month follow up. Secondary care service usage was measured for 6 months before and after therapy. Results Significant improvements on all psychological outcomes were found when comparing scores between the three time points. Patients with coronary heart disease and myocardial infarction who received Improving Access to Psychological Therapies treatments had decreased use of secondary care hospital services. Conclusions An integrated Improving Access to Psychological Therapies and cardiac rehabilitation service can provide effective, adapted evidence-based psychological treatments for cardiac patients with comorbid anxiety and depression.

EnglishVery Small Entities (VSE) are very important in the software product and service development sector. Therefore, it is necessary to implement models and/or standards according to their size and features. Moreover, this kind of enterprises face problems such as lack of knowledge, trained personnel and experience in the implementation of this type of norms even when there are already standards adjusted to their size such as ISO/IEC 29110. This article proposes a tool that supports VSEs to assets the processes and the project based on the ISO/IEC 29110 standard to be presented in a future certification. For this, the tool was implemented in a Software development VSE prior to starting a certification process in this standard. After the implementation of this tool, the VSE obtained the certification in the ISO/IEC 29110 standard in a more controlled and guided way, since it allowed it to solve the observations in advance before starting a certification process.

Despite the country's generous social welfare systems, perinatal mental health problems are prevalent in Norway. National guidelines recommend that health services identify women with perinatal mental conditions, but systematic screening and clear treatment pathways are not nationally endorsed, neither are recommendations for evaluating and treating possible parent–infant interaction difficulties of affected mothers. AIM: Early psychosis delivery models have proliferated worldwide, but there is limited research into establishing model fidelity. In this context, this article aims to describe the development and implementation of a fidelity tool in a national network of early psychosis services across Australia-the headspace Early Psychosis program. METHODS: Following a detailed consultation process, and based on the Australian Early Psychosis model, an 80-item Early Psychosis Prevention and Intervention Centre Model Integrity Tool (EMIT) was developed along with predefined thresholds for fidelity. The tool was used to assess adherence to the model in six clusters of service sites across Australia. Ratings on the EMIT were informed by interviews with site staff and young people receiving the service, routinely collected data and site policies and procedures. RESULTS: All six clusters of headspace Early Psychosis programs participated in five fidelity assessments across a period of two and a half years. This study sought to develop a framework for social media data mining and analysis for the betterment and advancement of products (art effects) and services (exhibitions) with in the contemporary art industry of Zimbabwe through a case study of the Zimbabwean National Art Gallery. The information for this paper was gathered through the use of in-depth interviews, questionnaires, key-word search and API from the organization’s social media twitter handle were used to get the data for analysis. Focus group participants were chosen from the National art gallery and a matching number was selected from the artist who makes the artworks which will be soles or exhibited through the national art gallery. The findings suggested that, yes it is possible to inform the next batch of products with information mined from analyzing the sentiments or reviews of the previous set of artworks. Hence with this in mind the researchers managed to develop a framework which can be used to implement social media mining in the art sector of Zimbabwe. PurposeThe SECURE STAIRS framework has promoted young person involvement as a key feature of innovative service provision. In the North East of England, Secure Children’s Homes (SCHs) have pioneered young person involvement in team formulation. Young people have also led on the development of formulation materials through a young person involvement project. Young people’s attendance at team formulation meetings has become embedded. These service developments have also begun to expand across broader SCHs, Secure Training Centres (STCs) and Young Offender Institutions (YOIs) and reflect the national impact of such pioneering developments. There is a notable lack of research examining the impact of young person involvement in formulation. The purpose of this study is to elucidate this area.Design/methodology/approachBetween April and August 2019, there were 86 staff attendances at formulation meetings where young people were present across two SCHs in the North East of England.FindingsPaired samplest-tests indicated staff-reported significant post-formulation improvements across all domains measured including knowledge [t(85) = 11.38,p< 0.001]; confidence working with the young person [t(85) = 5.87,p< 0.001]; motivation [t(85) = 3.58,p< 0.001]; understanding [t(85) = 9.03,p< 0.001]; and satisfaction with the treatment plan [t(85) = 8.63,p< 0.001].Research limitations/implicationsImplications of findings are discussed and further developments outlined.Practical implicationsYoung people attending team formulation is a new and evolving area, and to the best of the authors’ knowledge, there is no existing research evaluating this area. The current findings suggest that staff knowledge, motivation, confidence and satisfaction with the treatment plan is improved following attendance at team formulation where the young person is present and actively participating. Young people’s participation and involvement is central to planning, developing and progressing meaningful provision within children and young people secure provision.Social implicationsInvolving children and young people in decision-making, service development and evaluation are also key components of ensuring the voice of young people is central in our minds. It also supports empowering the young people we work with. Young people and their journey should always be at the heart of what we do.Originality/valueYoung people attending team formulation is a new and evolving area, and to the best of the authors’ knowledge, there is no existing research evaluating this area. This is the first study of its type evaluating formulation directions within the SECURE STAIRS framework.

Introduction The preliminary clinical evaluation (PCE) system involves the radiographer providing an immediate comment highlighting an abnormality on the x-ray image. This can be a valuable service development though it must be recognised that the structure of the PCE may impede its usefulness. This study aimed to assess radiographers' ability to form a concise description of radiographic abnormalities by evaluating their structure and brevity. Method A convenience sampling approach was used and the study was open to all radiographers (n = 48) in a United Kingdom (UK) hospital. Participants provided a PCE for 35 abnormal appendicular cases, which were assessed for the number of words used, lexical density and Gunning-Fog index; comparisons were made with a gold standard. PCE accuracy was evaluated with a scoring system and statistical analysis was completed with SPSS. Results 21 participants took part. The mean (SD,range) words used was greater than the gold standard (9.5 (3.89,14.9) vs 5.6 (1.46,7)). The mean (SD,range) lexical density was lower than the gold standard (73.8 (4.02,20.1) vs 100 (0,0)), and the mean Gunning-Fog index was also lower (15.1 (3.79,18.3) vs 20.7 (6.82,22.6)). The mean (SD,range) PCE score was 2.8 (0.34,1.17), compared to 5 (0,0) for the gold standard, and this was a statistically significant difference (t (21) = -29,p = .001). Conclusion Participants used too many words in their PCE comments with reduced descriptive content that did not match the reading level of the gold standard. Areas for suggested improvement in practice include introduction of a comment-forming model with additional education. These findings provide an interesting addition to the growing PCE knowledge base. Implications for practice Dedicated training prior to implementation and participation, to standardise comment structure, could improve the effectiveness of the PCE system.

Новизной проведенного исследования можно считать обобщение опыта развития придорожного сектора в России и зарубежных странах, позволившее выявить основные факторы, оказывающие влияние на уровень его развития, а также принципы, которыми необходимо руководствоваться при его размещении. The purpose of the article is to analyze the trends and possible directions for the development of roadside service as an element of Russia's spatial development policy, as well as factors affecting its placement. The authors investigate the main aspects of the modern state of the roadside service as part of the service sector, as well as the features of the territorial organization of the roadside service. It is argued that it is at the junction of such important and interconnected activities as transport, trade, tourism, household and technical services, public food. The novelty of the study can be considered a generalization of the experience of the development of the roadside sector in Russia and foreign countries, which made it possible to identify the main factors that affect its level of development, as well as the principles that should be guided when placing it. The article presents reasonable indicators that allow you to assess the existing and possible potential for the development of roadside service in a specific territory.

There is a trend of combining agile and traditional project management practices for technology-based product and service development in the search for more agility. Although there are, in the literature, hybrid models that propose combinations of traditional and agile approaches, there are no studies that discuss the impact of the adoption of this approach in organizations in practice. Consequently, guidance on the selection of the most appropriate project management approach has remained largely theoretical, rather than based on companies’ experiences. The objective of this research is to analyze how organizations that develop technology-based products and services apply hybrid approaches to project management, their characteristics, advantages, and disadvantages, conducting a literature review and multiple case studies as research methods. Results reveal that hybrid approaches to project management are currently fundamental for companies in order to deal with distinct organizational cultures, specific processes, customer contractual requirements, and project specificities. This study also led to a consolidated list of the characteristics of hybrid approaches to project management.

Objective: Due to advances in surgical techniques and anaesthesia, day surgery is now becoming the standard care pathway for many complex procedures traditionally treated through inpatient pathways. Our aim was to study outcomes for patients undergoing rigid and flexible ureteroscopy before and after moving this service from a district general hospital to a day surgery unit. Methods: This was a retrospective observational study of 136 consecutive patients undergoing elective ureteroscopy for stone treatment. Half of the patients ( n=68, inpatient group) underwent treatment according to the standard inpatient pathway, while the other half underwent treatment following relocation of this service to a day case unit ( n=68, day case group). Outcomes were length of stay, readmission rate and complications. Results: In the inpatient group, 12 patients (17.6%) were discharged home the day of surgery (day 0), 42 patients (61.8%) stayed in hospital for one night. In the day case group, 58 patients (85.3%) were discharged on day 0, nine patients (13.2%) stayed for one night. Length of stay in the day case group was significantly shorter ( PConclusion: Day case ureteroscopy for stone treatment is feasible and safe. It is associated with a significantly reduced length of stay without an increase in postoperative complications or readmission.

When developing new music therapy services in ID settings, music therapists are often required to introduce their role to other healthcare professionals (OHPs) who often have little or no experience of music therapy when developing new music therapy services in ID settings. While some research has explored how to successfully integrate music therapy services into established healthcare organisations and account for the current challenges of working in healthcare settings (Clarkson & Killick, 2016; Darsie, 2009; Miller, 2008; Ledger et al., 2013; Ledger 2016), to date no studies have considered development of music therapy services in ID contexts. This study addressed this issue through carrying out ethnographic inquiry into music therapy service development in ID organisations in Ireland. The overarching aim of this ethnographic study was to highlight the experiences of music therapists and OHPs related to developing music therapy services, with the intention of providing a valuable resource to inform development of future music therapy services in ID in Ireland. This began with a curiosity to explore my own experiences of developing a music therapy service in an ID setting and led to me keeping a reflective journal. As the research progressed, it became apparent that gleaning a wider range of perspectives on the topic of music therapy service development was needed to explore the unique working culture and contexts of ID organisations in Ireland. This was achieved through an online anonymous survey among OHPs at the ID organisation where I developed a music therapy service. It also involved semi-structured interviews with six music therapists and three managers in other ID settings across Ireland. This ethnographic study aimed to answer the following research questions: 1. What are the expectations of a music therapist when developing a service to support those with intellectual disabilities in Ireland, and to what extent are those expectations realised in the development of a new music therapy service? 2. What are other healthcare professionals’ values and practices that influence the development of a music therapy service in intellectual disability services in Ireland? 3. How does a music therapist navigate any differences in expectations, values and practices when developing a new music therapy service into intellectual disability services in Ireland? Upon compiling all data sources, findings revealed music therapists held expectations around a) a smooth transition from training to practice, b) a one-sided approach to education, c) being recognised and valued for their role and, d) the idea that hard work pays off. Furthermore, OHPs’ values and practices may further influence the development of a music therapy service. Findings suggested that such influences lie in OHPs’ care and protection for those they support, allocation of funding whereby different funds bring various implications to service development, in addition to the influence of a manager’s background which lead to different perspectives around the trajectory of music therapy service development. Finally, findings also highlighted how music therapists might navigate any differences in expectations, values and practices when developing a new music therapy service in ID settings in Ireland. It is recommend that music therapists: a) be flexible yet boundaried, b) embrace curiosity and action around organisational change and, c) work with others for better service provision. Due to the unique culture and context of each ID organisation, is it not possible to offer a definitive pathway towards successful music therapy service integration. Instead, this study puts forward a number of key recommendations to aid other music therapists as they embark on the task of music therapy service development in ID contexts in future.

Objectives:Photo at Discharge (PaD) is a nurse-led discharge strategy for enhanced wound care information for patients and healthcare providers. The purpose of this study is to describe implementation of PaD in three English cardiac centres.Methods:A prospective, cross-sectional design was used to evaluate implementation fidelity and sustainability of PaD on various geographical settings.Results:Three out of four hospitals (75%) approached agreed to complete surveys on implementation fidelity. Implementing the IT component took an average of 16 months (range 11–21 months). Across the three sites, 474 nursing staff have received training on PaD. Since implementing, a combined total of 9007 patients have received PaD. A 1-month compliance snapshot indicated mean of 96% (range 92–100%).Conclusions:PaD requires collaborative working, a change in behaviour and a change to the service. Despite these challenges, fidelity and sustainability scores across the sites were high. The findings from this study may help to increase implementation quality and dissemination of PaD.

Introduction Pediatric postmortem (PM) rates have significantly declined, creating a need for effective minimally invasive alternatives to correlate with parental wishes. We review the use of a minimally invasive fetal and neonatal PM service further to preliminary findings published in 2015. Materials and Methods Cases taken from the mortuary electronic database from 2012 to 2017 are analyzed. The minimally invasive service consisted primarily of external examination, magnetic resonance imaging (MRI), and placental examination. Any significant conditions found noted. All pathology reports include a Relevant Condition at Death (ReCoDe) obstetric classification. Reports analyzed to determine which aspects of the service provided positive information. Results Of 1498 perinatal postmortems, 105 (7%) were PM MRI, of which 75.24% were intrauterine fetal deaths. Relevant conditions were identified in 94 cases (89.52%), and ReCoDe categories in 80 cases (76.19%). Moreover, 90% of cases had a ReCoDe condition, with 10% unclassified. Seven cases had more than 1 ReCoDe. Main conditions related to placenta (32.5%) and umbilical cord (27.5%). The most informative elements were placental examination and MRI. Conclusion Minimally invasive PMs are a viable alternative to traditional autopsy when this option is refused. However, further case analysis is needed to determine potential bias toward certain classification codes.

Background Meeting the complex needs of pregnant migrants requires an innovative approach. Migrant women's experiences and opinions should directly guide service development if these needs are to be met effectively. Aims To evaluate the specialist migrant maternity service provided by Kings College Hospital London based upon users' experience and satisfaction. Methods A qualitative service evaluation via semi-structured, multi-lingual, in-depth interviews with 10 service users. Data was analysed using a general inductive approach for thematic analysis. Findings Positive aspects: access to midwife and referrals, provision of essentials and transport, respect and kindness of caregivers. Negative aspects: poor maternal nutrition, lack of access to hygienic infant-feeding equipment, lack of social support building and lack of service signposting. Conclusion Women conveyed a high level of satisfaction with the service. The negative aspects that persist are, arguably, not within the immediate remit of the maternity team, yet they negatively affect the health of these women, and therefore merit awareness raising if holistic care is to be achieved. Seven service recommendations have been made.

Climatic resources are vitally important for tourism, driving major intra- and inter-regional travel flows for sun-sand-surf (3S) tourism around the world. The development of climate indices to measure the suitability of climate for major tourism market segments has evolved over three decades. This study provides the first application of the holiday climate index (HCI):Beach specification in the Asia-Pacific tourism region. The HCI is designed from international tourist climate preference studies and is compared with the tourism climate index (TCI), which is widely applied, but not based on tourist climate preferences. The index inter-comparison is conducted at 14 of the most popular beach resort destinations in China, which include four geographic regions of China with four different Köppen classifications. The results show key differences between the two indices in rating the climatic suitability of the selected beach destinations in China, with the TCI rating beach destinations in the north and south higher during the spring and fall seasons, which is not consistent with beach tourism visits. During the summer months, southern destinations have much higher HCI:Beach rating, reflecting the 3S tourists' desire for higher temperatures. The findings reinforce those from other tourism regions that indicate the TCI is not appropriate for assessing 3S tourism potential and that additional cross-cultural studies of tourist climate indices are needed to better inform market segment climate service development and to understand the potential impacts of future climate change.

Background Social prescribing is a means of enabling primary care professionals to refer people to a range of local, non-clinical services. Medical students at a large GP surgery in Corby designed, implemented and led a social prescribing service for the practice's patients. Through the project students gained an understanding of social prescribing in an authentic setting. Methods During a 12 week GP placement students collated information on local organisations, charities and schemes into a social prescribing directory. A clinic was set up and a social prescribing protocol created to enable suitable patients to be referred to the service. Students educated staff and collected feedback on how the service should run. Patients referred to the service were seen by medical students, who identified suitable social prescribing opportunities. Follow up was arranged to encourage patient engagement with services. The student-led service has been successfully integrated with the work of the new PCN link worker. Outcome Medical students were able successfully identify social prescribing opportunities for patients referred to them in primary care. Experential learning enabled them to develop an understanding of social prescribing and its place in healthcare. Discussion Medical students successfully designed and delivered a social prescribing intervention providing authentic educational experience in real-life clinical practice. The introduction of a Primary Care Network link worker enhanced this work and student input has continued in the ongoing service. It is hoped the scheme will be rolled out across the Primary Care curriculum in Leicester.

Objectives. This paper outlines best practice recommendations for the clinical psychologist’s role in inpatient settings and also presents a methodology for deriving evidence-based recommendations, which may be replicated by healthcare professionals in other areas of service development. Methods. Twenty-three, nationally-drawn experts, deemed expert by profession and/or experience attended a one-day conference. Participants were presented with findings from previous research and key unanswered questions in terms of how psychological services should be delivered in inpatient settings. Participants discussed the information presented within small groups and then independently responded to the questions posed. Responses to questions either involved ranking priorities or choosing the best possible option in terms of what or how aspects of the model should be delivered. Results. We were able to identify referral priorities for formulation, nurse-led psychosocial interventions and psychological therapy using ranking questions. Consensus (defined as 70% agreement) was achieved within 1-3 voting rounds for all questions which involved selecting one of a range of possible options. These responses provided information about how to deliver formulation, supervision of nurse-led psychosocial interventions, staff well-being interventions and one-to-one therapy, as well as psychologists’ own training and supervisions needs. Conclusions. Findings provide recommendations relating to five different service areas for inpatient settings seeking to implement a psychological service model: formulation, nurse-led interventions, one-to-one therapy, staff well-being, and psychologist training and supervision. Additionally, we showcase a methodology for synthesising knowledge and decision-making that can be implemented by researchers and healthcare professionals seeking to design interventions or service models in other settings.
