Purpose This study aimed to quantify sleeping heart rate (HR) and HR variability (HRV) alongside circulating tumor necrosis factor alpha (TNFα) concentrations during 12-week Basic Military Training (BMT). We hypothesised that, despite a high allostatic load, BMT would increase cardiorespiratory fitness and HRV, while lowering both sleeping HR and TNFα in young healthy recruits. Methods Sixty-three recruits (18–43 years) undertook ≥ 2 overnight cardiac frequency recordings in weeks 1, 8 and 12 of BMT with 4 h of beat-to-beat HR collected between 00:00 and 06:00 h on each night. Beat-to-beat data were used to derive HR and HRV metrics which were analysed as weekly averages (totalling 8 h). A fasted morning blood sample was collected in the equivalent weeks for the measurement of circulating TNFα concentrations and predicted VO2max was assessed in weeks 2 and 8. Results Predicted VO2max was significantly increased at week 8 (+ 3.3 ± 2.6 mL kg−1 min−1; p −1) was progressively reduced throughout BMT (wk8, 58 ± 6; wk12, 55 ± 6 b min−1; p p p = 0.79) but were significantly reduced at wk12 (8.0 ± 2.4 pg/mL; p Conclusion Increased predicted VO2max, HRV and reduced HR during overnight sleep are reflective of typical cardiorespiratory endurance training responses. These results indicate that recruits are achieving cardiovascular health benefits despite the high allostatic load associated with the 12-week BMT.

To investigate: (i) the chronicity and phasic variability of sleep patterns and restriction in recruits during basic military training (BMT); and (ii) identify subjective sleep quality in young adult recruits prior to entry into BMT.Prospective observational study.Sleep was monitored using wrist-worn actigraphy in Army recruits (n = 57, 18-43 y) throughout 12-weeks of BMT. The Pittsburgh Sleep Quality Index (PSQI) was completed in the first week of training to provide a subjective estimate of pre-BMT sleep patterns. A mixed-effects model was used to compare week-to-week and training phase (Orientation, Development, Field, Drill) differences for rates of sub-optimal sleep (6-7 h), sleep restriction (≤6 h), and actigraphy recorded sleep measures.Sleep duration was 06:24 ± 00:18h (mean ± SD) during BMT with all recruits experiencing sub-optimal sleep and 42% (n = 24) were sleep restricted for ≥2 consecutive weeks. During Field, sleep duration (06:06 ± 00:36h) and efficiency (71 ± 6%; p 0.01) were reduced by 15-18 min (minimum - maximum) and 7-8% respectively; whereas, sleep latency (30 ± 15 min), wake after sleep onset (121 ± 23 min), sleep fragmentation index (41 ± 4%) and average awakening length (6.5 ± 1.6 min) were greater than non-Field phases (p 0.01) by 16-18 min, 28-33 min, 8-10% and 2.5-3 min respectively. Pre-BMT global PSQI score was 5 ± 3, sleep duration and efficiency were 7.4 ± 1.3 h and 88 ± 9% respectively. Sleep schedule was highly variable at pre-BMT (bedtime: 22:34 ± 7:46 h; wake time: 6:59 ± 1:42 h) unlike BMT (2200-0600 h).The chronicity of sub-optimal sleep and sleep restriction is substantial during BMT and increased training demands exacerbate sleep disruption. Exploration of sleep strategies (e.g. napping, night-time routine) are required to mitigate sleep-associated performance detriments and maladaptive outcomes during BMT.

Traditional small mammal survey methods (e.g., Sherman traps) are beneficial in certain conditions but tend to require substantial effort and funds and can introduce various biases. The recently described AHDriFT Camera Trap System (i.e., camera traps combined with drift fences) can survey small terrestrial vertebrates and does not require much time in the field. Our objective was to compare the efficiency and effectiveness of AHDriFT systems and traditional Sherman traps for surveying small mammal communities. Surveys with both methods were conducted in four sites of varying habitat types at the Eagle Marsh Nature Preserve (Fort Wayne, IN, USA) from February to July in 2020 and May to August in 2021. We conducted 640 trap nights (one trap set on one calendar night) of Sherman trap surveys and 551 trap nights of AHDriFT system surveys. We captured 192 small mammals of 3 species with Sherman traps and obtained 532 images of unique small mammal individuals of 7 species with AHDriFT systems. Our AHDriFT systems resulted in two times greater species richness (Z = −6.21, P < 0.01), 16 times greater species evenness (Z = −4.83, P < 0.01), and 23 times greater Shannon’s diversity values (Z = −4.87, P < 0.01) than Sherman traps. The AHDriFT systems also documented the presence of four species that the Sherman traps did not (northern short-tailed shrew Blarina brevicauda, common shrew Sorex cinereus, long-tailed weasel Neogale frenata, eastern chipmunk Tamias striatus). Overall, AHDriFT system surveys provided 1.5–5 times more observations per dollar spent and required 90% less time in the field than Sherman trap surveys. These results suggest that AHDriFT systems may be a more efficient and effective method of surveying small mammal communities.

Military personnel are required to complete physically demanding tasks when performing work and training, which may be quantified through the physical stress imposed (external load) or the resultant physiological strain (internal load). The aim of this narrative review is to provide an overview of the techniques used to monitor work and training load in military settings, summarise key findings, and discuss important practical, analytical, and conceptual considerations. Most investigations have focused upon measuring external and internal load in military training environments; however, limited data exist in operational settings. Accelerometry has been the primary tool used to estimate external load, with heart rate commonly used to quantify internal load. Supplemental to heart rate, psychophysiological and biochemical measures have also been investigated to elucidate aspects of internal load. Broadly, investigations have revealed that military training requires personnel to perform relatively large volumes of physical activity (e.g. averaging ∼15,000 steps·day

Collaborative projects are commonplace in computing education. They typically enable students to gain experience building software in teams, equipping them with the teamwork skills they need to be competitive in the labour market. However, students often need encouragement to reflect upon and synthesise their experience to attain the most learning. Peer evaluation offers one such approach, but the conditions which facilitate effective peer evaluation have not yet been established. This paper seeks to provide insight into student experiences with peer evaluation. This builds upon a prior qualitative study, analysing quantitative data collected through a questionnaire taken by undergraduate students on a collaborate digital game development module. An exploratory factor analysis identifies seven dimensions of variance in the student experience: perceived impact; arbitrary influence; inconsistency; team cohesiveness; assessment reward; ease; as well as professionalism. Correlation analysis suggests some factors such as arbitrary influence, team cohesion, assessment pressure, and professionalism are associated with attained learning, whilst factors such as inconsistency and onerousness are not. This paper defines a conceptual framework, suggesting considerations for educators adopting peer evaluation. Expanding the framework and validating it across different demographics, contexts, and project types are suggested as avenues for further investigation.

BackgroundOur study addresses the gaps in knowledge of the characterizations of operations by remote tele-critical care medicine (tele-CCM) service providers interacting with the bedside team. The duration of engagements, the evolution of the tele-CCM service over time, and the distress during interactions with the bedside team have not been characterized systematically. These characteristics are critical for planning the deployment of teleICU services and preventing burnout among remote teleICU providers.MethodsREDCap self-reported activity logs collected engagement duration, triggers (emergency button, tele-CCM software platform, autonomous algorithm, asymmetrical communication platform, phone), expediency, nature (proactive rounding, predetermined task, response to medical needs), communication modes, and acceptance. Seven hospitals with 16 ICUs were overseen between 9/2020 and 9/2021 by teams consisting of telemedicine medical doctors (eMD), telemedicine registered nurses (eRN), and telemedicine respiratory therapists (eRT).Results39,915 total engagements were registered. eMDs had a significantly higher percentage of emergent and urgent engagements (31.9%) vs. eRN (9.8%) or eRT (1.7%). The average tele-CCM intervention took 16.1 ± 10.39 min for eMD, 18.1 ± 16.23 for eRN, and 8.2 ± 4.98 min for eRT, significantly varied between engagement, and expediency, hospitals, and ICUs types. During the observation period, there was a shift in intervention triggers with an increase in autonomous algorithmic ARDS detection concomitant with predominant utilization of asynchronous communication, phone engagements, and the tele-CCM module of electronic medical records at the expense of the share of proactive rounding. eRT communicated more frequently with bedside staff (% MD = 37.8%; % RN = 36.8, % RT = 49.0%) but mostly with other eRTs. In contrast, the eMD communicated with all ICU stakeholders while the eRN communicated chiefly with other RN and house staff at the patient's bedside. The rate of distress reported by tele-CCM staff was 2% among all interactions, with the entity hospital being the dominant factor.ConclusionsDelivery of tele-CCM services has to be tailored to the specific beneficiary of tele-CCM services to optimize care delivery and minimize distress. In addition, the duration of the average intervention must be considered while creating an efficient workflow.

Introduction The injury definitions and surveillance methods commonly used in Army basic military training (BMT) research may underestimate the extent of injury. This study therefore aims to obtain a comprehensive understanding of injuries sustained during BMT by employing recording methods to capture all physical complaints. Materials and methods Six hundred and forty-six recruits were assessed over the 12-week Australian Army BMT course. Throughout BMT injury, data were recorded via (1) physiotherapy reports following recruit consultation, (2) a member of the research team (third party) present at physical training sessions, and (3) recruit daily self-reports. Results Two hundred and thirty-five recruits had ≥1 incident injury recorded by physiotherapists, 365 recruits had ≥1 incident injury recorded by the third party, and 542 recruits reported ≥1 injury-related problems via the self-reported health questionnaire. Six hundred twenty-one, six hundred eighty-seven, and two thousand nine hundred sixty-four incident injuries were recorded from a total of 997 physiotherapy reports, 1,937 third-party reports, and 13,181 self-reported injury-related problems, respectively. The lower extremity was the most commonly injured general body region as indicated by all three recording methods. Overuse accounted for 79% and 76% of documented incident injuries from physiotherapists and the third party, respectively. Conclusions This study highlights that injury recording methods impact injury reporting during BMT. The present findings suggest that traditional injury surveillance methods, which rely on medical encounters, underestimate the injury profile during BMT. Considering accurate injury surveillance is fundamental in the sequence of injury prevention, implementing additional injury recording methods during BMT may thus improve injury surveillance and better inform training modifications and injury prevention programs.

Oesophageal adenocarcinoma (OAC) provides an ideal case study to characterize large-scale rearrangements. Using whole genome short-read sequencing of 383 cases, for which 214 had matched whole transcriptomes, we observed structural variations (SV) with a predominance of deletions, tandem duplications and inter-chromosome junctions that could be identified as LINE-1 mobile element (ME) insertions. Complex clusters of rearrangements resembling breakage-fusion-bridge cycles or extrachromosomal circular DNA accounted for 22% of complex SVs affecting known oncogenes. Counting SV events affecting known driver genes substantially increased the recurrence rates of these drivers. After excluding fragile sites, we identified 51 candidate new drivers in genomic regions disrupted by SVs, including ETV5, KAT6B and CLTC. RUNX1 was the most recurrently altered gene (24%), with many deletions inactivating the RUNT domain but preserved the reading frame, suggesting an altered protein product. These findings underscore the importance of identification of SV events in OAC with implications for targeted therapies.

The root mean square of successive differences (RMSSD) of R-R intervals, the most robust vagal modulation parameter of heart rate variability (HRV), has been proposed as a marker of global health and training adaptations. The weekly averaging of RMSSD and its natural logarithm (LnRMSSD) has been adopted to enhance detection of training-induced adaptations. J Strength Cond Res XX(X): 000-000, 2020-After basic military training, physical training practices among incumbent personnel differ substantially, potentially precipitating varied physical capacity and operational readiness. The purpose of this retrospective investigation was to compare physical characteristics and physical performance between recruits and incumbent personnel. Data were collected for 222 army recruits (REC: 197 men/25 women) nearing completion of basic training and 280 incumbent army personnel from combat arms (CA: 142 men) and noncombat arms trades (NCA: 113 men/25 women). Height, mass, and body mass index (BMI) were recorded together with performance measures including predicted V[Combining Dot Above]O2max and 1 repetition maximum box lift. Compared with REC, male incumbents were older (21.8 ± 0.6 vs. 27.6 ± 1.2 years; mean ± 95% confidence interval), had greater body mass (77.6 ± 1.3 vs. 82.6 ± 1.5 kg) and BMI (24.2 ± 0.3 vs. 25.8 ± 0.5 kg·m), similar relative V[Combining Dot Above]O2max, and lower box lift (49.7 ± 1.4 vs. 47.2 ± 2.4 kg). Male CA demonstrated greatest physical performance while male NCA demonstrated the highest BMI and poorest physical performance. Compared with REC, female incumbents were older (22.8 ± 1.6 vs. 31.9 ± 4.4 years), had similar body mass, higher BMI (24.1 ± 0.7 vs. 26.1 ± 2.4 kg·m), as well as lower V[Combining Dot Above]O2max (42.0 ± 1.3 vs. 37.9 ± 2.1 ml·kg··min) and box lift (27.8 ± 2.1 vs. 23.3 ± 2.4 kg). More than 97% of men and approximately 75% of women were able to achieve the box lift and V[Combining Dot Above]O2max baseline standards. The older age category (26 + vs. 18-25 years) typically demonstrated higher BMI and lower physical performance. Through-career maintenance of physical capacity is important for military personnel to support individual readiness and occupational performance, preserve health, and mitigate musculoskeletal injury risk.

We present photometric metallicity measurements for a sample of 2.6 million bulge red clump stars extracted from the Blanco DECam Bulge Survey (BDBS). Similar to previous studies, we find that the bulge exhibits a strong vertical metallicity gradient, and that at least two peaks in the metallicity distribution functions appear at b < -5. We can discern a metal-poor ([Fe/H] ~ -0.3) and metal-rich ([Fe/H] ~ +0.2) abundance distribution that each show clear systematic trends with latitude, and may be best understood by changes in the bulge's star formation/enrichment processes. Both groups exhibit asymmetric tails, and as a result we argue that the proximity of a star to either peak in [Fe/H] space is not necessarily an affirmation of group membership. The metal-poor peak shifts to lower [Fe/H] values at larger distances from the plane while the metal-rich tail truncates. Close to the plane, the metal-rich tail appears broader along the minor axis than in off-axis fields. We also posit that the bulge has two metal-poor populations -- one that belongs to the metal-poor tail of the low latitude and predominantly metal-rich group, and another belonging to the metal-poor group that dominates in the outer bulge. We detect the X-shape structure in fields with |Z| > 0.7 kpc and for stars with [Fe/H] > -0.5. COVID-19, an infectious pulmonary disease caused by the SARS-CoV-2 virus, has profoundly impacted the world, motivating researchers across a broad spectrum of academic disciplines to gain a deeper understanding and develop effective therapies to this disease. This article presents an engineering perspective on how microfluidic technologies may address some of the challenges presented by COVID-19 and other pulmonary diseases. In particular, this article highlights urgent needs in pulmonary medicine, with an emphasis on technological innovations in the microfluidic manipulation of particles and fluids, and how these innovations may contribute to the study, diagnosis, and therapy of pulmonary diseases.

A general testing approach is presented via a fracture mechanics study on the interfacial delamination behavior in overmolded composite materials using a variant of the double cantilever beam (DCB) geometry. Overmolding, a common injection molding process, is used to fabricate asymmetric DCB test specimens with Lexan™ 3414 resin overmolded onto commercially available TenCate Cetex® FST woven glass fiber/polycarbonate laminates. An analytical beam theory model is employed to partition the planar fracture modes at the overmold interface into mode I and mode II components, which are functions of material properties and relative beam thicknesses. Specimen curvature measurements are integrated into the beam theory model to estimate the residual stress effects on fracture mode mixity. We use the overmold thickness as a tunable variable to control fracture mode mixity, and target near mode I fracture conditions, where we find mode I fracture energy ( GIc) values of approximately 1 kJ/m2. Fiber bridging across the failure interface is observed, which is not expected at the nominal polymer/polymer overmold interface. Complementary scanning electron microscopy images of the failure surfaces indicate crack initiation at the overmold interface, followed by a change in locus of failure to the nearby polymer/glass fiber interface in the top layer of the composite laminate. Fiber bridging is observed in all specimens tested over a modest range of mode mixity, including specimens modified to the single leg bending geometry, suggesting that the polymer/glass interface is more susceptible to crack propagation than the desired overmold interface, which likely derives its strength from molecular interdiffusion during the overmolding process.

We present photometric evidence for multiple stellar populations (MPs) in 14 globular clusters (GCs) toward the southern Galactic bulge. The photometric data come as part of the Blanco DECam Bulge Survey, which is a deep, wide-field near-UV-near-IR (ugrizY) survey of the southern Galactic bulge. Here, we present the first systematic study of bulge GC multiple populations with deep photometry including the u band, which is a crucial indicator of the abundance of CNO-bearing molecules in stellar atmospheres. We identify cluster members using Gaia EDR3 proper motion measurements, and then isolate red giant branch stars using r versus u − r color–magnitude diagrams. We find evidence suggesting all 14 clusters host at least two populations, and NGC 6441, NGC 6626, and NGC 6656 appear to have at least three populations. Many of these clusters are not part of the Hubble Space Telescope (HST) surveys nor do they have comprehensive spectroscopic analyses so we are presenting the first evidence of MPs in several clusters. Not only do we find a strong anticorrelation between the fraction of first-generation stars and cluster absolute V magnitude, but the correlation coefficient and cluster-to-cluster scatter are similar to the results obtained from HST. Our ground-based data extend to much larger radial distances than similar HST observations, enabling a reliable estimate of the global fraction of first-generation stars in each cluster. This study demonstrates that ground-based u-band photometry as provided by DECam will prove powerful in the study of multiple populations in resolved GCs.

216 Background: Head and neck cancer (HNC) patients are at increased risk for suicide. Few studies have tested approaches to preventing suicide in this population, such as routine suicide risk screening. To address this issue, the current study pilots and assesses the feasibility and implementation of a suicide risk screening program for HNC patients at a National Cancer Institute (NCI)-designated Comprehensive Cancer Center. Methods: Routine screening was implemented at all in-person visits in the HNC clinic from February 1, 2021 – January 31, 2022 using the PHQ-9. Patients with passive suicidal ideation were consequently screened by social work for active suicidal ideation. Results: Most (92.6% or 1519/1641) HNC patients with a scheduled in-person visit completed at least one suicide risk screening; 9,968 screenings were completed (mean: 3.7; SD: 3.8 per patient) in total. Among patients who were screened, 3% (49/1519) of HNC patients reported passive suicidal ideation and 0.3% (5/1519) were identified as having active suicidal ideation. To explore synergistic effects related to skin regeneration, peptides with distinct biological mechanisms of action were evaluated in combination with different skin cell lines in the presence or absence of niacinamide (Nam). Furthermore, the synergistic responses of peptide combinations on global gene expression were compared with the changes that occur with fractional laser resurfacing treatment, a gold standard approach for skin rejuvenation, to further define optimal peptide combinations.Microarray profiling was used to characterize the biological responses of peptide combinations (+/- Nam) relative to the individual components in epidermal keratinocyte and dermal fibroblast cell lines. Cellular functional assays were utilized to confirm the synergistic effects of peptide combinations. Bioinformatics approaches were used to link the synergistic effects of peptide combinations on gene expression to the transcriptomics of the skin rejuvenation response from fractional laser treatment.Microarray analysis of skin cells treated with peptide combinations revealed synergistic changes in gene expression compared with individual peptide controls. Bioinformatic analysis of synergy genes in keratinocytes revealed the activation of NRF2-mediated oxidative stress responses by a combination of Ac-PPYL, Pal-KTTKS and Nam. Additional analysis revealed direct downstream transcriptional targets of NRF2/ARE exhibiting synergistic regulation by this combination of materials, which was corroborated by a cellular reporter assay. NRF2-mediated oxidative stress response pathways were also found to be activated in the transcriptomics of the early skin rejuvenation response to fractional laser treatment, suggesting the importance of this biology in the early stages of tissue repair. Additionally, the second combination of peptides (pal-KT and Ac-PPYL) was found to synergistically restore cellular ATP levels that had been depleted due to the presence of ROS, indicating an additional mechanism, whereby peptide synergies may accelerate skin repair.Through combinatorial synergy studies, we have identified additional in vitro skin repair mechanisms beyond the previously described functions of individual peptides and correlated these to the transcriptomics of the skin rejuvenation response of fractional laser treatment. Ces résultats suggèrent que des peptides spécifiques peuvent agir ensemble, par le biais de mécanismes complémentaires et synergiques, pour améliorer de manière globale la capacité régénérative des cellules cutanées in vitro.

In this study, student success metrics using a combined lecture/laboratory “atoms-first” approach will be compared to traditional teaching models in which lecture and laboratory are separate courses with separate meeting times. This model was piloted earlier with the introductory course CHEM 108, Forensic Chemistry, which demonstrated increased student engagement due to daily rather than weekly laboratory activities. A curricular change was implemented in the CHEM 101 course, General Chemistry I, in the Fall of 2016. This model included new experimental and computational modules incorporated around the course material each week in addition to changing to an “atoms-first,” structural model rather than a “stoichiometry-first” mathematical model over the first few chapters of the course. Hands-on molecular modeling assignments have been included along with the curricular changes, which have allowed students to better visualize atomic and molecular structure. Evidence for the efficacy of these newly-developed assignments is shown in the results of exams primarily covering topics of structure and bonding, which are used as a tool of quantitative assessment between traditional lecture and combined lecture/laboratory models. In the traditional lecture model, the average exam score for this learning outcome was 65.8% (N = 45), whereas for the combined lecture/laboratory model, the average exam score is 71.2% (N = 64). Additional student learning outcomes results for both General Chemistry and Forensic Chemistry will be discussed, and changes in performance and engagement will be outlined.

4587 Background: Upper tract urothelial carcinoma (UTUC) is an aggressive cancer for which use of neoadjuvant chemotherapy (NAC) is limited by suboptimal clinical staging prior to nephroureterectomy. Detection of circulating tumor DNA (ctDNA) is associated with locally advanced and metastatic urothelial carcinoma of the bladder and may help identify UTUC patients who would benefit from NAC. Here we examine the feasibility and utility of plasma ctDNA in the diagnosis of non-organ confined high-risk UTUC. Methods: Patients with high-grade cTa-T2 UTUC without radiographic evidence of metastatic disease undergoing up-front radical nephroureterectomy (RNU) were prospectively accrued for pre- and post-operative plasma collection. Blood was collected preoperatively on the day of surgery, and plasma and buffy coat were processed for extraction of cell-free DNA and genomic DNA, respectively. FFPE tumor samples from RNU were used for tissue genomic DNA extraction. Targeted next-generation sequencing (NGS) was used for variant profiling. ctDNA positivity was defined as the presence of plasma cell-free DNA variants concordant with tissue-based variants. Results: NGS analyses of matched FFPE and plasma samples were successfully performed for all 19 accrued UTUC patients. Alterations in the TERT promoter (74%), TP53 (58%), FGFR3 (53%), myc amplification (53%), and ATM (42%) were demonstrated in urothelial tumor tissue. Matched plasma ctDNA showed prevalent alterations in the TERT promoter (42%), TP53 (42%), PIK3CA (37%), ATM (32%) and CD274 (26%). Nine patients (47%) had detectable plasma ctDNA mutations concordant with tumor-specific variants using the targeted NGS panel. All patients with detectable preoperative ctDNA had advanced staging (≥pT2 or ≥pN1) and lymphovascular invasion on final pathology, resulting in a 90% sensitivity. The panel was 100% specific with no patients with pTis, pTa, pT1 and pN0 having detectable concordant ctDNA mutations. Concordant plasma ctDNA was detected in four of nine patients postoperatively. Two of three (67%) who developed metastatic disease had detectable ctDNA while neither of the two who developed non-muscle-invasive bladder recurrences did. Conclusions: Prospective ctDNA analysis using a targeted NGS panel can be used to predict muscle-invasive and non-organ-confined UTUC preoperatively. Detectable postoperative ctDNA may indicate residual disease and predate clinical recurrence.

4574 Background: CG0070 is a replication-competent oncolytic adenovirus engineered to target RB deficient tumor cells and to express GM-CSF. CG0070 has previously demonstrated safety and efficacy against BCG-exposed high risk non-muscle invasive bladder cancer through tumor lysis and anti-tumor immune activation. We tested the safety and efficacy of CG0070 in combination with N as neoadjuvant therapy for MIBC in C-ineligible patients in this study (NCT04610671). Methods: C-ineligible pts with MIBC (cT2-4a, N≤1) were enrolled. Pts received 6 weekly intravesical CG0070 (1x1012vp) and 2 doses of N at wks 2 and 6, followed by radical cystectomy (RC). The primary objective is safety of CG0070+N as measured by CTCAEv5.0. Secondary objective is to assess pathologic response (PaR) (pT0N0). Exploratory objectives include the assessment of correlation between PaR with baseline 1) E2F expression; 2) immune infiltration; 3) PD-L1 expression; and 4) TLS expression. Results from a prespecified interim analysis following accrual of 15 patients is reported herein. Results: Between Nov 2020 and Jan 2022, 15 pts were enrolled with median age 75.5yrs, 73% male, 47% > cT2; 1 patient refused RC but was included in the ITT population. No DLTs were encountered. The overall rate of grade 3-4 AEs was 10/15 (75%), and the vast majority were related to RC (90%). Immune related AEs were seen in one pt, who had grade 2 autoimmune thyroiditis. There was no delay in time to RC and no unexpected surgical complications from treatment. PaR was observed in 6/13 (46%) evaluable pts, and an additional pt had negative post-treatment biopsy but refused RC. Conclusions: Neoadjuvant CG0070+N is safe and effective in C-ineligible pts with MIBC. This combination was well tolerated without any delays in RC and induced an overall response rate of 54%. We present a 3D hydrodynamics study of gravitational instabilities (GIs) in a 0.14 M$_{\odot}$ circumbinary protoplanetary disc orbiting a 1 M$_{\odot}$ star and a 0.02 M$_{\odot}$ brown dwarf companion. We examine the thermodynamical state of the disc and determine the strengths of GI-induced density waves, nonaxisymmetric density structures, mass inflow and outflow, and gravitational torques. Results are compared with a parallel simulation of a protoplanetary disc without the brown dwarf binary companion. Simulations are performed using CHYMERA, a radiative 3D hydrodynamics code. The onset of GIs in the circumbinary disc is much more violent due to the stimulation of a strong one-armed density wave by the brown dwarf. Despite this early difference, detailed analyses show that both discs relax to a very similar quasi-steady phase by 2,500 years after the beginning of the simulations. Similarities include the thermodynamics of the quasi-steady phase, the final surface density distribution, radial mass influx, and nonaxisymmetric power and torques for spiral arm multiplicities of two or more. Effects of binarity in the disc are evident in gravitational torque profiles, temperature profiles in the inner discs, and radial mass transport. After 3,800 years, the semimajor axis of the binary decreases by about one percentage and the eccentricity roughly doubles. The mass transport in the outer circumbinary disc associated with the one-armed wave may influence planet formation.

OBJECTIVES:This study investigated indirect measures of post-exercise parasympathetic reactivation (using heart-rate-variability, HRV) and sympathetic withdrawal (using systolic-time-intervals, STI) following upper- and lower-body exercise. DESIGN:Randomized, counter-balanced, crossover. METHODS:13 males (age 26.4±4.7years) performed maximal arm-cranking (MAX-ARM) and leg-cycling (MAX-LEG). Subsequently, participants undertook separate 8-min bouts of submaximal HR-matched exercise of each mode (ARM and LEG). HRV (including natural-logarithm of root-mean-square-of-successive-differences, Ln-RMSSD) and STI (including pre-ejection-period, PEP) were assessed throughout 10-min seated recovery. MEG based on optically-pumped magnetometry (OP-MEG) operates with miniaturized, wearable insulation, in contrast to massive cryogenic dewars for SQUID-MEG, and allows placement of the sensors close to the scalp. This allows more natural head motion during data recording and localized signal quality comparable to, or surpassing, SQUID-MEG. However no OP-MEG system to date has offered full-head coverage with dense sensor packing, and existing systems - as with SQUID-MEG - require the subject to be sealed in a multilayer, passively-shielded vault in order to suppress ambient magnetic fields. Here we present Kernel Flux, which overcomes these limitations. Kernel Flux uses a collection of alkali vapor sensors in a unique array architecture to directly detect the magnetic fields generated by collective neural activity in the brain, while allowing for comfortable head motion. Each Kernel Flux OP-MEG system was designed from the ground up to work as an integrated system optimized around the user's experience, with relevance to natural home and office contexts.

As deep learning techniques and algorithms become more and more common in scientific workflows, HPC centers are grappling with how best to provide GPU resources and support deep learning workloads. One novel method of deployment is to virtualize GPU resources allowing for multiple VM instances to have logically distinct virtual GPUs (vGPUs) on a shared physical GPU. However, there are many operational and performance implications to consider before deploying a vGPU service in an HPC center. In this paper, we investigate the performance characteristics of vGPUs for both traditional HPC workloads and for deep learning training and inference workloads. Using NVIDIA's vDWS virtualization software, we perform a series of HPC and deep learning benchmarks on both non-virtualized (bare metal) and vGPUs of various sizes and configurations. We report on several of the challenges we discovered in deploying and operating a variety of virtualized instance sizes and configurations. Next-generation sequencing (NGS) was used for variant profiling. Plasma and somatic tissue mutations were called by comparing with matched buffy coat samples. Detection of cancer variants with a mutation allele frequency (MAF) ≥ 0.25% and hotspot variants with a MAF down to 0.1% were reported for plasma samples targeted by a NGS panel (PredicineCARE). Variants with a MAF ≥ 5% and hotspot variants with a MAF down to 2% were reported for FFPE samples. Results: NGS analyses of matched FFPE and plasma samples were successfully performed for all 15 accrued UTUC patients. Alterations in MYC amplification (62%), TERT promoter (62%), TP53 (38%), FGFR3 (31%), ERBB2 (25%), ARID1A (19%), and PIK3CA (19%) were demonstrated in urothelial tumor tissue. Matched plasma ctDNA showed prevalent alterations in the TERT promoter (47%), TP53 (30%), ARID1A (20%), ERBB2 (20%), FGFR3 (20%), and PIK3CA (17%). Five patients (33%) had detectable plasma ctDNA mutations concordant with tumor-based genotypes using the targeted NGS panel. All patients with detectable preoperative ctDNA had advanced staging (≥pT2 or ≥pN1) and lymphovascular invasion on final pathology, resulting in a 71.4% sensitivity. The panel was 100% specific with no patients with pTis, pTa, or pT1 and pN0 having detectable concordant ctDNA mutations. Conclusions: Prospective ctDNA analysis using a targeted NGS panel is a feasible nonsurgical approach to prediction of high-risk UTUC and has the potential for identification of upper tract urothelial cancer patients that may benefit from NAC.

Exotic forest insects and pathogens (EFIP) have become regular features of temperate forest ecosystems, yet we lack a long-term perspective on their net impacts on tree mortality, carbon sequestration, and tree-species diversity. Here, we analyze three decades (1987-2019) of forest monitoring data from the Blue Ridge Mountains ecoregion in eastern North America, including 67 plots totaling 29.4 ha, along with a historical survey from 1939. Over the past century, EFIP substantially affected at least eight tree genera. Tree host taxa had anomalously high mortality rates (≥6% yr-1 from 2008-2019, versus 1.4% yr-1 for less-impacted taxa). Following the arrival of EFIP, affected taxa declined in abundance (-25 to -100%) and live aboveground biomass (AGB; -13 to -100%) within our monitoring plots. We estimate that EFIP were responsible for 21-29% of ecosystem AGB loss through mortality (-87 g m-2 yr-1) from 1991-2013 across 66 sites. Over a century, net AGB loss among affected species totaled roughly 6.6-10 kg m-2. The affected host taxa accounted for 23-29% of genera losses at the plot-scale, with mixed net effects on α-diversity. Several taxa were lost from our monitoring plots but not completely extirpated from the region. Despite these losses, both total AGB and α-diversity were largely recovered through increases of sympatric genera. These results indicate that EFIP have been an important force shaping forest composition, carbon cycling, and diversity. At the same time, less-affected taxa in these relatively diverse temperate forests have conferred substantial resilience with regards to biomass and α-diversity., Files were extracted from a private GitHub repository that supported this analysis (https://github.com/EcoClimLab/Shenandoah). Background Macromolecules in skin cells are damaged when exposed to environmental stressors, leading to disrupted cellular function and homeostasis. While epidermal turnover can eliminate some of this damage, autophagy can rapidly remove these defective components. Niacinamide (Nam) is known to induce autophagy and optimizing formulations to maximize this response could provide improved homeostasis in stressed skin. Objective To determine (i) whether Nam can induce autophagy related 5 (ATG5), an autophagy marker, in human keratinocytes and (ii) whether optimized low pH Nam formulations can enhance the response in 3D skin models. Methods Human keratinocytes treated with Nam were evaluated for autophagosome accumulation and induction of ATG5 by gene expression, immunoblotting and immune-fluorescence microscopy. 3D skin equivalents were topically treated with Nam formulations at pH 5.8 and 3.8. Gene expression profiling and immunoblot analysis of ATG5 were performed. Results Nam treatment of keratinocytes led to an accumulation of autophagosomes with a maximal signal at 48 h. Gene expression of ATG5 was induced by Nam, and immunoblots stained for ATG5 showed a significant increase after 6 h of treatment. Gene expression profiling of 3D epidermal skin equivalents treated with Nam at pH 3.8 showed stronger induction of autophagy-related genes, including ATG5, compared with pH 5.8 formulas. Enrichment for gene ontology terms on autophagy showed an increased linkage with Nam formulas at pH 3.8. Conclusions We found that Nam induces autophagosome accumulation and ATG5 levels in keratinocytes. We also discovered that a Nam formulation at pH 3.8 can further increase levels of ATG5 in 3D skin models when compared to Nam at pH 5.8. These data support that Nam can induce autophagy in keratinocytes and formulations at pH 3.8 can enhance the impact. We hypothesize that optimized formulations at pH 3.8 can improve skin ageing appearance via autophagy induction.

Niacinamide has been suggested to impact hair biology via stimulation of VEGF synthesis. Testing in an in vitro VEGF synthesis assay, it was found that niacinamide cannot stimulate VEGF synthesis across a broad dose-response range.

The Blanco DECam Bulge Survey (BDBS) imaged more than 200 square degrees of the Southern Galactic bulge using the ugrizY filters of the Dark Energy Camera, and produced point spread function photometry of approximately 250 million unique sources. In this paper, we present details regarding the construction and collation of survey catalogs, and also discuss the adopted calibration and dereddening procedures. Early science results are presented with a particular emphasis on the bulge metallicity distribution function and globular clusters. A key result is the strong correlation (sigma ~ 0.2 dex) between (u-i)o and [Fe/H] for bulge red clump giants. We utilized this relation to find that interior bulge fields may be well described by simple closed box enrichment models, but fields exterior to b ~ -6 degrees seem to require a secondary metal-poor component. Applying scaled versions of the closed box model to the outer bulge fields is shown to significantly reduce the strengths of any additional metal-poor components when compared to Gaussian mixture models. Additional results include: a confirmation that the u-band splits the sub-giant branch in M 22 as a function of metallicity, the detection of possible extra-tidal stars along the orbits of M 22 and FSR 1758, and additional evidence that NGC 6569 may have a small but discrete He spread, as evidenced by red clump luminosity variations in the reddest bands. A system of a systems approach that analyzes energy and water systems simultaneously is called energy-water nexus. Neglecting the interrelationship between energy and water drives vulnerabilities whereby limits on one resource can cause constraints on the other resource. Power plant energy production directly depends on water availability, and an outage of the power systems will affect the wastewater treatment facility processes. Therefore, it is essential to integrate energy and water planning models. As mathematical energy-water nexus problems are complex, involve many uncertain parameters, and are large-scale, we proposed a novel multi-stage adjustable Fuzzy robust approach that balances the solutions' robustness against the budget-constraints. Scenario-based analysis indicates that the proposed approach generates flexible and robust decisions that avoid excessive costs compared to conservative methods. Driving may be detrimental to health, with one hypothesis suggesting that driving may elicit an acute stress response and, with repeated exposures, may become a chronic stressor. The present study examined the stress response to driving and the effectiveness of a prior exercise bout in dampening this response. Twenty healthy adults performed three tasks: control, driving and exercise plus driving. Heart rate (HR), heart rate variability (HRV), blood pressure (BP) and cortisol were measured to quantify the acute stress response to each condition. Data indicated a stress response to driving: HR was elevated and HRV was reduced during the driving task compared with control. HR was elevated and HRV was reduced comparing the exercise plus driving with the driving condition. BP and cortisol were not different among conditions. The potential of interventions, such as exercise, to counter daily stressors should be evaluated to safeguard long-term health. Practitioner Summary: this study confirms that driving induces a stress response, with the exercise intervention providing mixed results (an increase in cardiovascular measures and a decrease in cortisol measure trending significance). Given the known consequences of stress and evidence that exercise can mitigate acute stress, further evaluation of exercise interventions is recommended.
