Abstract

Risk for late-onset Alzheimers disease (LOAD), the most prevalent dementia, is partially driven by genetics. To identify LOAD risk loci, we performed a large genome-wide association meta-analysis of clinically diagnosed LOAD (94,437 individuals). We confirm 20 previous LOAD risk loci and identify five new genome-wide loci (IQCK, ACE, ADAM10, ADAMTS1, and WWOX), two of which (ADAM10, ACE) were identified in a recent genome-wide association (GWAS)-by-familial-proxy of Alzheimers or dementia. Fine-mapping of the human leukocyte antigen (HLA) region confirms the neurological and immune-mediated disease haplotype HLA-DR15 as a risk factor for LOAD. Pathway analysis implicates immunity, lipid metabolism, tau binding proteins, and amyloid precursor protein (APP) metabolism, showing that genetic variants affecting APP and Aβ processing are associated not only with early-onset autosomal dominant Alzheimers disease but also with LOAD. Analyses of risk genes and pathways show enrichment for rare variants (P = 1.32 ׫2009»10−7), indicating that additional rare variants remain to be identified. We also identify important genetic correlations between LOAD and traits such as family history of dementia and education.

Abstract

Next-generation genetic sequencing (NGS) technologies facilitate the screening of multiple genes linked to neurodegenerative dementia, but there is little guidance available about their use in clinical practice. Guidelines on which patients would most profit from testing, and information on the likelihood of discovery of a causal variant in a clinical syndrome, are conspicuously absent from the literature, mostly for a lack of large-scale studies. We applied a validated NGS dementia panel to 3241 patients with dementia and healthy aged controls; 13,152 variants were classified by likelihood of pathogenicity. We identified 354 deleterious variants (DV, 12.6% of patients); 39 were novel DVs. Age at clinical onset, clinical syndrome and family history each strongly predict the likelihood of finding a DV, but healthcare setting and gender did not. DVs were frequently found in genes not usually associated with the clinical syndrome. Patients recruited from primary referral centres were compared to those seen at higher-level research centres and a national clinical neurogenetic laboratory; rates of discovery were comparable, making selection bias unlikely and the results generalizable to clinical practice. We estimated penetrance of DVs using large-scale online genomic population databases and found 71 with evidence of reduced penetrance. Two DVs in the same patient were found more frequently than expected. These publicly-available data should provide a basis for informed counselling and clinical decision making.

Abstract

Eleven susceptibility loci for late-onset Alzheimer's disease (LOAD) were identified by previous studies; however, a large portion of the genetic risk for this disease remains unexplained. We conducted a large, two-stage meta-analysis of genome-wide association studies (GWAS) in individuals of European ancestry. In stage 1, we used genotyped and imputed data (7,055,881 SNPs) to perform meta-analysis on 4 previously published GWAS data sets consisting of 17,008 Alzheimer's disease cases and 37,154 controls. In stage 2, 11,632 SNPs were genotyped and tested for association in an independent set of 8,572 Alzheimer's disease cases and 11,312 controls. In addition to the APOE locus (encoding apolipoprotein E), 19 loci reached genome-wide significance (P < 5 x 10(-8)) in the combined stage 1 and stage 2 analysis, of which 11 are newly associated with Alzheimer's disease.

Abstract

Summary Background Surgical resection alone is regarded as the standard of care for patients with liver metastases from colorectal cancer, but relapse is common. We assessed the combination of perioperative chemotherapy and surgery compared with surgery alone for patients with initially resectable liver metastases from colorectal cancer. Methods This parallel-group study reports the trial's final data for progression-free survival for a protocol unspecified interim time-point, while overall survival is still being monitored. 364 patients with histologically proven colorectal cancer and up to four liver metastases were randomly assigned to either six cycles of FOLFOX4 before and six cycles after surgery or to surgery alone (182 in perioperative chemotherapy group vs 182 in surgery group). Patients were centrally randomised by minimisation, adjusting for centre and risk score. The primary objective was to detect a hazard ratio (HR) of 0·71 or less for progression-free survival. Primary analysis was by intention to treat. Analyses were repeated for all eligible (171 vs 171) and resected patients (151 vs 152). This trial is registered with ClinicalTrials.gov, number NCT00006479. Findings In the perioperative chemotherapy group, 151 (83%) patients were resected after a median of six (range 1–6) preoperative cycles and 115 (63%) patients received a median six (1–8) postoperative cycles. 152 (84%) patients were resected in the surgery group. The absolute increase in rate of progression-free survival at 3 years was 7·3% (from 28·1% [95·66% CI 21·3–35·5] to 35·4% [28·1–42·7]; HR 0·79 [0·62–1·02]; p=0·058) in randomised patients; 8·1% (from 28·1% [21·2–36·6] to 36·2% [28·7–43·8]; HR 0·77 [0·60–1·00]; p=0·041) in eligible patients; and 9·2% (from 33·2% [25·3–41·2] to 42·4% [34·0–50·5]; HR 0·73 [0·55–0·97]; p=0·025) in patients undergoing resection. 139 patients died (64 in perioperative chemotherapy group vs 75 in surgery group). Reversible postoperative complications occurred more often after chemotherapy than after surgery (40/159 [25%] vs 27/170 [16%]; p=0·04). After surgery we recorded two deaths in the surgery alone group and one in the perioperative chemotherapy group. Interpretation Perioperative chemotherapy with FOLFOX4 is compatible with major liver surgery and reduces the risk of events of progression-free survival in eligible and resected patients. La sélection des étudiants en médecine se fait à la Faculté de Médecine de Lausanne pendant les deux premières années du curriculum, sur la base d'examens portant sur les sciences de base. Le risque est par ce biais de privilégier le passage dans les années cliniques d'étudiants doués sur le plan scientifique, mais ne possédant pas nécessairement les qualités humaines que devrait avoir un bon médecin. La présente étude a porté sur une volée d'étudiants (n = 115) ayant réussi la première partie de l'examen final à la fin de la troisième année des études de médecine. A cette occasion les étudiants ont passé un examen oral de médecine psycho-sociale permettant d'évaluer leurs aptitudes à gérer la relation médecin-malade. Les notes obtenues à cet examen ont été mises en rapport avec celles obtenues en première année lors d'un examen écrit de physique et de physiologie. Les résultats ont montré que la performance à ces disciplines de base n'ont pas de valeur prédictive quant aux performances futures à l'examen de médecine psycho-sociale. Par ailleurs il est apparu que le type de maturité acquis pendant le gymnase n'a pas d'influence majeure sur la progression des étudiants pendant les années pré-cliniques des études de médecine.

Abstract

We have recently proposed the revised spherical cap harmonic analysis (R-SCHA) modeling technique. The new mathematical functions represent faithfully the spatial variations of potential fields in a restricted area. In this paper, we tackle the inverse problem and outline the efficiency of the new basis functions with respect to real magnetic data. Processing simultaneously repeat stations, observatory, aeromagnetic, andCHAMP satellite data provides our first vector lithospheric field model over France, which extends from surface to 500 km of altitude. The magnetic field is represented with a minimum horizontal spatial representation of 40 km at the mean Earth radius. The magnetic lithospheric map consistency is confirmed with a comparison to known geological features. The model variation with altitude also suggests that the major French magnetic anomaly, the Paris basin anomaly, is produced by a deep-rooted geological structure. These results demonstrate the superiority of regional modeling over global modeling for delineating small-scale details in the lithospheric field. In view of forthcoming satellite missions, like Swarm, the revised spherical cap harmonic analysis method will help to accurately represent the lithospheric field for more detailed geological interpretations.

Abstract

The infrared and Raman spectra of CaCO3 calcite substituted with 80% O-18 have been recorded. A detailed mode assignment is proposed for all the observed bands, including combinations and overtones. These data are used to propose a simplified model of the vibrational density of states (VDOS) from which the specific heat, the entropy, and the high-temperature equation of state are calculated. Excellent agreement between calculated and measured values of the thermodynamic properties is obtained when measured vibrational mode anharmonicity is included in the calculations. The model can be used to infer the properties of calcite at high pressures (less than or equal to 3 Gpa) and temperatures (less than or equal to 1200 K). The observed frequency shifts induced by O-18 substitution in both IR and Raman spectra are used to construct the VDOS of (CaCO3)-O-18 calcite. The reduced partition function of calcite is then calculated and the effects of anharmonicity are discussed. Finally the effect of pressure on the reduced partition function is calculated and is shown to be appreciable even at high temperatures.
