Evaluation of the perfusion and viability of the femoral head after fracture of the femoral neck is important because the outcome of conservative treatment or joint-preserving surgery is adversely affected by the development of capital osteonecrosis. We evaluated the use of MR imaging, before and after IV administration of gadopentetate dimeglumine, for assessing perfusion of the femoral head in 13 patients with acute fracture of the femoral neck.Multiecho (1600/30-240 [TR/TE]) MR images were obtained before contrast administration and gradient-echo (315/14, 90 degrees flip angle) MR images were obtained both before and after contrast administration. MR findings were correlated with findings on superselective digital subtraction angiograms of the vessels supplying the femoral head and with clinical-radiographic follow-up for at least 12 months.Digital subtraction angiography showed impaired blood supply to the femoral head in five patients. On contrast-enhanced MR images of these patients, the femoral head did not enhance and was lower in signal intensity than were the enhancing femoral shaft and neck distal to the fracture and the enhancing femoral head on the unaffected side. In the patients with persistent perfusion, contrast-enhanced MR images showed a uniform increase in signal intensity in the femoral shaft and neck as well as the femoral head; the femoral head on the fractured side showed contrast enhancement similar to that on the healthy side.These preliminary results indicate that contrast-enhanced MR imaging may be useful for noninvasive evaluation of femoral head perfusion after fracture of the femoral neck. MR findings also may aid the clinician in deciding between joint-preserving therapy and hip arthroplasty.

After in-vivo labeling of red blood cells with 99mTcO4, the radioactivity of different parts of the body is proportional to the regional blood volume. This technique allows the simultaneous registration of drug-induced changes of the local blood volume. We examined the influence of nitroglycerin on the blood volume distribution in patients with coronary artery disease (n = 10) and chronic heart failure (n = 8). Compared with the controls (n = 8) we observed in the nitroglycerin-groups a significant decrease of blood volume over the heart and the liver, whereas there was an increase of the blood volume over the extremities. It is pointed out that functional whole-body scanning is a useful radiological technique for the non-invasive study of pharmacologically caused changes of the local blood volume in man.

61 patients with suggested pancreatic disease were examined by Hypotonic Duodenography (HD) and Computed Tomography (CT). After all examinations had been carried out, 45 patients were found to have an abnormal pancreas, and 16 patients had a normal pancreas. HD had a sensitivity of 60%, and a specificity of 94%, whereas CT had a sensitivity of 78% and a specificity of 100%. - HD is more sensitive than CT in detecting lesions originating from pancreatico-duodenal structures. In patients with uncertain clinical symptoms of the upper gastrointestinal tract HD is still a valuable basic examination. Only in patients with strongly suspected pancreatic carcinoma radiologic examination should start with CT because CT is more sensitive in detecting a pancreatic lesion and is helpful in assessing its extension and defining its etiology.

— Topographical information provided by brainstem auditory evoked potentials (BAEPs) was investigated in 43 patients by comparison with cerebral nuclear magnetic resonance imaging (NMR). Lesions in the region of the brainstem auditory pathways were demonstrated by BAEPs in 44.2%, and in 39.5% by NMR. As regards brainstem levels, in 15/21 (71.4%) with abnormal findings at least one lesion was verified by NMR-matched BAEP results. The study confirms the topographical information provided by the BAEPs on the different levels of the brainstem, but not the assumption that generation of the BAEPs is predominantly ipsilateral. BAEPs retain their importance for the detection of disseminated lesions in the diagnosis of multiple sclerosis (MS) in the era of expensive imaging methods.

The purpose of this study was to assess the role of MR imaging and the paramagnetic contrast agent Gadolinium-DTPA(Gd-DTPA) in the diagnosis of pituitary macroadenomas. 44 macroadenomas were examined with MRI before and after intravenous application of Gd-DTPA. Gd-DTPA produced excellent enhancement of solid adenoma. The best contrast between adenoma and surrounding structures could be gained on post-Gd T1-weighted images. Post-Gd images were equivalent to pre-Gd images in the evaluation of supra- and infrasellar extensions of macroadenomas. Post-Gd images had advantages in the evaluation of cavernous sinus invasion by adenoma. The difference in degree of contrast enhancement between adenoma and cavernous sinus facilitated the exact evaluation of lateral extension by adenoma in 18 cases. Almost equal degree of enhancement of both structures impaired tumor-sinus contrast in 2 cases. In the other 24 cases the tumor filled the cavernous sinus completely. It is our opinion that Gd-DTPA can be used on a widespread basis because of its excellent capability to highlight and delineate pituitary adenomas.
