Background Malnutrition has been shown to be a risk factor for postoperative complications after pancreatoduodenectomy (PD). In addition, patients needing a PD, such as patients with pancreatic cancer or chronic pancreatitis, often are malnourished. The best route of postoperative nutrition after PD remains unknown. The aim of this randomized controlled trial is to evaluate if early postoperative enteral nutrition can decrease complications after PD compared to oral nutrition. Methods This multicenter, open-label, randomized controlled trial will include 128 patients undergoing PD with a nutritional risk screening ≥3. Patients will be randomized 1:1 using variable block randomization stratified by center to receive either early enteral nutrition (intervention group) or oral nutrition (control group) after PD. Patients in the intervention group will receive enteral nutrition since the first night of the operation (250 ml/12 h), and enteral nutrition will be increased daily if tolerated until 1000 ml/12 h. The primary outcome will be the Comprehensive Complication Index (CCI) at 90 days after PD. Discussion This study with its multicentric and randomized design will permit to establish if early postoperative enteral nutrition after PD improves postoperative outcomes compared to oral nutrition in malnourished patients. Mediale Responsibility Frames können beeinflussen, wie Menschen über Gesundheitsthemen wie Demenzen denken. Obwohl sich die bisherige Forschung auf die Untersuchung textueller Frames konzentriert, können auch visuelle Inhalte Frames enthalten, die die Wirkung medialer Inhalte auf Rezipierende beeinflussen können. Visuelle Frames können somit hilfreich zur effektiven Gestaltung medialer Gesundheitsbotschaften sein. Die vorliegende Studie untersucht die Wirkung visueller Responsibility Frames auf verschiedenen Verantwortungsebenen im Kontext von Demenzen auf Verantwortungsattributionen und Verhaltensintentionen Rezipierender. Ein repräsentatives Online-Experiment (N = 1,059) zeigt, dass das reine Vorhandensein eines visuellen Responsibility Frames, unabhängig von der Ebene, auf der Verantwortung zugeschrieben wird, die Zuschreibung von Verantwortung bei Rezipierenden erhöht. Aim The aim of this study was to assess whether a preoperative HH≥3cm had an impact on histopathologic tumor response after neoadjuvant treatment, as well as on overall and disease-free survival. Background & Methods Hiatal hernia (HH) and long-term gastroesophageal reflux are known risk factors for esophageal cancer. Previous data suggest a negative impact of large hiatal hernias on survival after esophagectomy, as well as an increased toxicity after neoadjuvant treatment (1), although evidence remains scarce and the mechanism is not fully elucidated. All consecutive patients who underwent surgical esophagectomy for adenocarcinoma or squamous cell cancer of the esophagus and gastro-esophageal junction from 2012-2018 were assessed. Baseline oesogastroduodenoscopy reports and CT-scan images were retrospectively reviewed to identify the presence of a HH of ≥3cm (2). Response to neoadjuvant treatment as assessed by the Mandard score (3), postoperative outcomes and survival were compared between HH and non-HH patients (defined as HH Results Among the 174 included patients, 44 (25.3%) had a HH≥3cm upon diagnosis. Aim This study aimed to compare long-term survival and recurrence of patients depending on their R status: R0 (>1mm of clearance), R1 (positive margins with 0mm clearance) and R0+ (margins with Background & Methods Resection margin (R) status is a known prognostic factor after curative esophagectomy. Categorical variables were compared with the x2 or Fisher’s exact test, continuous variables with the ANOVA tests, whereas the Kaplan-Meier method with the log rank test was used for survival analyses. Results Among the 174 included patients, 126 (72.5%) resections were R0, 14 (8%) R1 and 34 (19.5%) R0+ resection. R0 patients had a better overall (OS) (median 59 months, 95%CI 34-84) and disease-free survival (DFS) (median not reached) compared to R1 (OS 20 months, 95%CI 8-32, DFS 9 months, 95%CI 7-11) and R0+ patients (OS 24months, 95% CI 19-29, DFS 13, 95%CI 3-23). Das Ziel der Akzeptanzstudie "Mieterstrom aus Mietersicht" war es, den Blick der Stromkundinnen und -kunden auf das dezentrale Versorgungskonzept "Mieterstrom" zu untersuchen und zu bewerten. Die Studie untersucht, inwiefern Mieterinnen und Mieter über das in ihrem Wohnhaus angebotene Mieterstromprodukt informiert sind, wie sie es annehmen und welche Faktoren die Akzeptanz beeinflussen. Dazu wurden 61 fragebogengestützte Face-to-Face-Interviews in sieben Mieterstromprojekten geführt. Die Bewohnerinnen und Bewohner gaben darin Auskunft zu ihren Motiven bei der Wahl ihrer Stromversorgung und ihrem Informationsstand. Außerdem führte der Autor der Studie leitfadengestützte Experteninterviews mit den Projektverantwortlichen der Mieterstromanbietern. Die Ergebnisse bestätigen in großen Teilen die zu Beginn der Forschung aufgestellten Thesen. Das Mieterstrommodell ist selbst in Wohnhäusern, in denen ein Mieterstromprodukt angeboten wird, noch häufig unbekannt. Dies ist nicht nur auf unzureichende Information durch die Anbieter zurückzuführen, sondern auch auf das Desinteresse der Stromkunden am Thema Energieversorgung allgemein und als kompliziert wahrgenommenen Konzepten wie Mieterstrom insbesondere. Hohe Beteiligungsquoten konnten nur in Projekten erzielt werden, in denen die Befragten Faktoren wie die Nutzung erneuerbarer Energien und lokale Erzeugung bei der Wahl Ihrer Energieversorgung klar über den Strompreis stellten, oder in denen Mieterstrom besonders günstig angeboten wurde. Allerdings gibt es auch bestimmte Vertriebsmethoden, die zu hohen Beteiligungsquoten führen können. Trotz der unterschiedlichen Rahmenbedingungen bei den untersuchten Projekten kann der Autor die Ergebnisse der Studie nutzen, um Handlungsempfehlungen für die Energiewirtschaft zur Erreichung höherer Beteiligungsquoten in Mieterstromprojekten zu formulieren.

BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12���g���dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (���week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g���dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g���dl-1 in week 2 and 8.0 [7.3 to 9.0] g���dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml���kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g���dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. In Gesundheitskrisen wie der COVID-19-Pandemie stellt die massenmediale Berichterstattung eine zentrale Informationsquelle dar. Sie wird dabei nicht nur individuell genutzt und verarbeitet, sondern auch gemeinsam rezipiert, im sozialen Umfeld besprochen oder in Sozialen Medien diskutiert. Wie sich die interpersonale Kommunikation über pandemiebezogene Berichterstattung gestaltet, ist jedoch selten beforscht worden. Der Beitrag widmet sich daher der Anschlusskommunikation zur Medienberichterstattung in der COVID-19-Pandemie. In einer qualitativen Interviewstudie mit 22 Teilnehmenden fragen wir nach (a) Kommunikationspartner:innen, (b) den kommunikativen Rollen innerhalb der Anschlusskommunikation, (c) den Funktionen der Anschlusskommunikation sowie (d) nach potentiellen Veränderungen im Zeitverlauf. Die Ergebnisse zeigen, dass die Anschlusskommunikation (a) insbesondere mit Familie und Freund:innen im direkten sozialen Umfeld stattfindet, aber auch zur Entstehung neuer Beziehungen beiträgt. Sie erfolgt (b) vor allem auf Augenhöhe, sodass die Rolle der Austauscher:innen dominiert, wobei manche Personen als Multiplikator:innen fungieren. Die Anschlusskommunikation erfüllt (c) die Funktion des Teilens und Erhaltens von Informationen, der Informationsvalidierung und gemeinsamen Wahrheitssuche sowie dem Coping mit negativen Emotionen. Veränderungen im Zeitverlauf (d) sind vor allem mit Blick auf das (Themen-)Interesse und die Kommunikationsrollen zu beobachten., In health crises such as the COVID-19 pandemic, mass media coverage is a key source of information. It is not only used and processed individually, but also used collectively, discussed within people’s personal social network, or on social media. However, how interpersonal communication about pandemic-related media coverage is shaped has rarely been researched. This chapter therefore addresses the follow-up communication about media coverage during the COVID-19 pandemic. In a qualitative interview study with 22 participants, we ask about (a) the communication partners, (b) the communicative roles within the follow-up communication, (c) the functions of the follow-up communication, as well as (d) potential changes over time. The results show that follow-up communication (a) takes place especially with family and friends in the immediate social network, but also contributes to the formation of new relationships. It takes place (b) primarily at eye level, so that the role of the exchanger dominates, with some people acting as multipliers. The follow-up communication fulfills (c) the function of sharing and receiving information, information validation and joint truth-seeking, as well as coping with negative emotions. Comics bieten ein vielversprechendes, aber bislang kaum genutztes Potenzial für die Kommunikation von Gesundheitsinformationen. Über die Wirkung von Comics bei der Aufklärung über psychische Erkrankungen wie Depressionen oder Burnout ist wenig bekannt. Depressionen werden von Laien oft als Burnout bezeichnet, was zu einer geringeren Stigmatisierung der Betroffenen, aber auch zu einer Verharmlosung depressiver Symptome führen kann. In der vorliegenden Studie (N = 1.201) wurde untersucht, wie das Geschlecht des Fallbeispiels und zusätzliche Sachinformationen in Comics die Stigmatisierung der Rezipierenden gegenüber Betroffenen beeinflussen und welche Rolle kognitive Prozesse dabei spielen. Die Ergebnisse bestätigen die destigmatisierende Wirkung des Burnout-Labels im Vergleich zu einem Depressions-Label. Die beobachteten Effekte sind jedoch von geschlechtsspezifischen Wahrnehmungen und in gewissem Maße von kognitiven Prozessen bei der Bildung stigmatisierender Einstellungen abhängig. Implikationen für die strategische Anti-Stigma- und Gesundheitskommunikation werden diskutiert., Comics are considered a promising, but often neglected medium for health information. However, little is known about the effects of comics providing health information about mental illnesses like depression or burn-out. Depression is often labeled by laypersons as burn-out, which can lead to reduced stigmatization of affected individuals, but likewise to a trivialization of depressive symptoms. The present study (N = 1,201) examined how the depiction of mental illness labels, exemplar's sex, and additional factual information in comics influences recipients' stigma-related perceptions about affected individuals and which role cognitive processes play in this regard. Results confirm the stigma-reducing effect of the burn-out label compared to a depression label. The observed effects, however, rely on sex-specific perceptions and, to some extent, on cognitive processes for the formation of stigmatizing attitudes. Die Repräsentation chronischer Erkrankungen in audiovisuellen und sozialen Medien ist ein bis dato wenig systematisch erfasstes Themenfeld der Gesundheitskommunikation. Der Beitrag stellt eine Studie vor, die diesen Aspekt mittels einer qualitativen Medieninhaltsanalyse empirisch exploriert. Es wurde ein Sample aus den Bereichen Film/TV/Streaming, Soziale Medien und Videospiele anhand von Spezifika, Relevanz, Verfügbarkeit sowie Aktualität ausgewählt und analysiert. Die untersuchten Medieninhalte weisen kaum stereotype Darstellungen, Stigmatisierung oder Tabuisierung chronischer Erkrankungen auf. Es wurden künstlerisch und gestalterisch innovative Formen der Darstellung erfasst. Deutlich wird, besonders im Bereich klassisch-audiovisueller Formate, dass die Beziehung Betroffener zu Angehörigen beziehungsweise zu Unterstützer:innen mit im Fokus der Narrationen steht. Die explorativen Ergebnisse bieten einen ersten Zugang zum Thema, dem weitere Studien folgen sollten., The contribution is presenting research on the representation of chronic illness in audio-visual and social media by a qualitative media content analysis. A sample from the fields of TV/Movie/Streaming, Social Media and Video Games were selected based on their specifics, relevance, accessibility and currentness. The analyzed media content includes little to no stereotypical depictions, stigmatization or tabooing of chronic illness. There are often used innovative forms in terms of design and art. Noticeably, the relationship between people affected and their dependents as well as their social support is often focused on. Informationsnormen stellen einen zentralen Einflussfaktor auf die Suche und Vermeidung von Informationen dar. Eine kombinierte Betrachtung von such- und vermeidungsbezogenen Informationsnormen als verschiedene Einflüsse auf Informationshandeln ist bislang jedoch nicht erfolgt. Zudem wurde kaum erforscht, inwieweit unterschiedliche Normtypen (deskriptive und injunktive Norm) und Bezugsgruppen bei der Betrachtung von Normeffekten auf Informationshandeln notwendig sind. Im Rahmen einer Längsschnittbefragung (N = 493) mit vier Wellen werden dazu mittels konfirmatorischer Faktoranalysen drei Normkonzepte verglichen. Dann wird der Einfluss verschiedener Informationsnormen auf Informationssuche und -vermeidung zu COVID-19 anhand von hybriden Within-Between Modellen betrachtet. Die Ergebnisse zeigen, dass keines der geprüften Normkonzepte zur Differenzierung von COVID-19 Informationsnormen geeignet ist. Bei der Betrachtung der Normeffekte auf Single Item Ebene zeigt sich, dass Suchnormen die Suche und Vermeidungsnormen die Vermeidung von Informationen stärker beeinflussen. Das persönliche Umfeld ist als Bezugsgruppe einflussreicher als die Gesellschaft. Deskriptive und injunktive Normen sind (unabhängig von der Bezugsgruppe) ähnlich bedeutsam. Auf der Within-Ebene lassen sich vielfältige Effekte feststellen, wobei die Normeffekte auf der Between-Ebene stärker sind. Die Studie biete somit Ansatzpunkte für ein detailliertes Verständnis von Normeffekten auf Informationshandeln., Information norms are one of the central predictors of information seeking and avoidance. However, a combined investigation of seeking-related and avoidance-related information norms as different effects on information behavior has not yet taken place. Furthermore, little research has been conducted on the importance of different norm types (descriptive and injunctive norm) and reference groups when considering norm effects on information behavior. Based on a longitudinal survey (N = 493) with four waves, three norm concepts are tested via confirmatory factor analyses. Subsequently, the influence of different norm types and reference groups on COVID-19 information seeking and avoidance is investigated using hybrid within-between models. None of the tested norm concepts are proven to be suitable. Examining norm effects based on single items reveals that seeking norms exert a stronger influence on information seeking, whereas avoidance norms influence information avoidance more strongly. The reference group of the personal environment is more influential than societal norms, while descriptive and injunctive norms appear to be similarly meaningful (regardless of the reference group). Multiple effects can be identified at the within-level, although norm effects are stronger at the between-level. Gesundheitsinformationen bilden die Grundlage für einen selbstbestimmten und kompetenten Umgang mit der eigenen Gesundheit. Die Nutzung von gesundheitsbezogenen Informationskanälen als Teil des individuellen Informationshandelns erfordert wiederum entsprechende (digitale) Gesundheitskompetenz. Obwohl beide Konstrukte verwandt scheinen, fehlt es bisher an einer theoriebasierten sowie empirischen Bestimmung ihrer Beziehung. Die vorliegende Studie exploriert deshalb, wie (digitale) Gesundheitskompetenz mit Gesundheitsinformationshandeln in Form der Nutzung verschiedener gesundheitsbezogener Informationskanäle zusammenhängt. Zur Beantwortung der Forschungsfrage wurde eine nach Alter, Geschlecht, Bildung und Region stratifizierte Querschnittsbefragung der deutschen Bevölkerung (N = 1.000) durchgeführt, die im Juni 2020 über ein Online-Access-Panel erhoben wurde. Die Ergebnisse zeigten, dass Personen mit höherer wahrgenommener (digitaler) Gesundheitskompetenz tendenziell häufiger gesundheitsbezogene Informationskanäle nutzen. The selection and use of health-related information channels as part of the individual's health information seeking behavior requires (digital) health literacy. Although health information seeking and (digital) health literacy both focus the handling of health information, a theory-based as well as empirical determination of their relationship has been lacking. Therefore, the present study explores how the perceived literacies interrelate with the use of different health-related information channels. To answer this research question, an online survey of a German population (N = 1,000) stratified by age, gender, education, and region was conducted via an online access panel in June 2020. The results indicated that perceived (digital) health literacy tends to be positively related to the use of health-related information channels. However, the explanatory power of perceived literacies was limited. Soziale Medien bieten Menschen mit psychischen Erkrankungen die Möglichkeit, das öffentliche Bild ihrer Krankheit zu gestalten und stereotypen Darstellungen entgegenzuwirken. In dieser Studie untersuchen wir, wie Instagram-Nutzende sich Sick-Style-Posts, d. h. die ästhetisierte und inszenierte Darstellung psychischer Krankheiten, aneignen, um herauszufinden, ob und welche Art von Sick-Style zur Entstigmatisierung beitragen oder möglicherweise zu einer noch stärkeren Stigmatisierung führen könnte. Auf der Grundlage von 18 qualitativen Interviews wurden vier Arten der Sick-Style-Aneignung identifiziert: Befürworter:innen, Gegner:innen, distanzierte und überforderte Nutzer:innen. Die Ergebnisse zeigen Entstigmatisierungspotential für verschiedene Typen der Sick-Style-Aneignung sowie für verschiedene Formen des Sick Styles, weisen aber in einigen Fällen auch auf die Gefahr einer stärkeren Stigmatisierung hin., Social media provide individuals with mental illness with the opportunity to shape the public image of their illness and counter stereotypical portrayals. SocieteInternationale de Chirurgie 2008 Definitive colostomy formation has become less frequent during the last two decades since abdominoperineal resection can now be omitted in many patients with rectal cancer, and Hartmann's operation for sigmoid diverticulitis is increasingly replaced by resection with primary anasto- mosis—with a loop ileostomy, if necessary. Nevertheless, there are still a number of patients who will receive a definitive colostomy due to different reasons. Long-term quality of life in those patients is significantly correlated to a ''good'' colostomy that comprises a proper localization in the abdominal wall, a non-impaired evacuation, and finally an easy management of the ostomy devices. While patients with a well functioning colostomy have few restrictions in their daily live, patients with ostomy problems may be heavily disturbed in their individual quality of life. Fur- thermore, patients with parastomal hernia often need additional ostomy devices, i.e. specially shaped faceplates, and repeated use of health care resources, both causing a significant increase of cost. Development of parastomal hernia is a common prob- lem that occurs after definitive colostomy creation. The reported incidence ranges up to 50%. Surgical repair is performed in at least 30% of all patients with parastomal hernia, whereby the postoperative recurrence rate is high. Colostomies are at particular risk to develop parastomal hernia since intraoperatively it is impossible to exactly determine the precise diameter of the hole; in reality it is always a compromise between keeping the opening narrow versus not impairing blood flow as well as stool evacuation. The passage of the colon through the abdominal wall represents therefore a kind of a ''planned'' weakness her- alding hernia formation. This is in contrast to ''normal'' incisions of the abdominal wall that are tightly closed after the intervention. The high incidence of parastomal hernia with its sub- sequent adverse effects on quality of life, the need for surgical repair, and increased cost provide the impetus to prevent parastomal hernia formation instead of just repairing the defect.
