3. La eficacia de la terapia cognitivo-conductual para el duelo prolongado

Abstract: Antecedentes: El Duelo Prolongado (DP) es una forma patológica del duelo dados los síntomas patentes y el tiempo que perduran, generando malestar y limitando la funcionalidad psicosocial. Durante años se ha empleado la Terapia Cognitivo-Conductual (TCC) para el tratamiento del DP, pero se sugiere que la activación conductual no específica es suficiente, se hallan fallos metodológicos en las muestras de TCC, o se cuestiona si es necesaria la exposición. Objetivo: Analizar si la TCC es realmente eficaz para mejorar el DP en adultos. Método: Se realizó una revisión sistemática mediante la búsqueda en las bases de datos Medline, PsycINFO, PubMed, Scopus y Web of Science desde junio de 2022 hasta febrero de 2023, así como mediante una comunicación personal. Se identificaron 9 estudios que cumplían los criterios de inclusión y exclusión establecidos. Resultados: Se comparan las evaluaciones basales, del postratamiento y seguimiento. Todos los estudios reflejan mejorías superiores en la condición de TCC frente a otras modalidades terapéuticas o frente a la propia TCC sin exposición, a excepción de uno que establece igualdad de eficacia con Eye Movement Desensitization and Reprocessing (EMDR). Se analiza el riesgo de sesgo siguiendo los criterios Cochrane, presentando una adecuada calidad metodológica; el nivel de evidencia científica, 1+; y el grado de recomendación, A. Conclusiones: Los resultados respaldan la eficacia de la TCC para el tratamiento del DP en la población adulta, aunque es fundamental realizar nuevos estudios que fortalezcan esta evidencia. Hasta entonces, se recomienda su aplicación a quienes padezcan DP.

4. Visual and Refractive Outcomes of a Trifocal Toric Intraocular Lens Implanted in Eyes With High Corneal Astigmatism

Abstract: Purpose: To analyze the refractive and visual outcomes following cataract surgery and implantation of a trifocal toric intraocular lens (IOL) in eyes with high degrees of corneal astigmatism. Methods: A total of 29 eyes from 21 patients who underwent implantation of a trifocal toric IOL (FineVision PODFT; PhysIOL) were evaluated in this study. Phacoemulsification with femto-second laser and intraoperative aberrometry were performed in all cases. All IOLs used exhibited a cylinder power of 3.75 diopters (D) or greater. The main outcome measures were refractive error and corrected distance visual acuity (CDVA) and uncorrected distance visual acuity (UDVA) values. Eyes were evaluated during a follow-up period of 5 years. Results: A total of 96.30%, 100%, 95.83%, and 89.47% of eyes were within ±1.00 D at 1, 2, 3, and 5 years postoperatively, respectively. Furthermore, 92.31%, 86.36%, 82.61%, and 84.21% of eyes showed a refractive cylinder value of ±1.00 D at 1, 2, 3, and 5 years postoperatively, respectively. During the entire follow-up period, between 81.48% and 91.30% of eyes showed a CDVA of 20/25 or better. The mean monocular Snellen decimal CDVA values were 0.90 ± 0.12, 0.90 ± 0.11, 0.91 ± 0.11, and 0.90 ± 0.12 at 1, 2, 3, and 5 years postoperatively, respectively. No significant rotation was reported in any eye during follow-up. Conclusions: The current study suggests that use of this trifocal toric IOL in eyes with high amounts of corneal astigmatism provides accurate refractive outcomes with good distance visual acuity. Abstract: Background: We aim to assess whether time of day modified the treatment effect in the RACECAT trial (Direct Transfer to an Endovascular Center Compared to Transfer to the Closest Stroke Center in Acute Stroke Patients With Suspected Large Vessel Occlusion Trial), a cluster-randomized trial that did not demonstrate the benefit of direct transportation to a thrombectomy-capable center versus nearest local stroke center for patients with a suspected large vessel stroke triaged in nonurban Catalonia between March 2017 and June 2020. Methods: We performed a post hoc analysis of RACECAT to evaluate if the association between initial transport routing and functional outcome differed according to trial enrollment time: daytime (8:00 am –8:59 pm ) and nighttime (9:00 pm –7:59 am ). Primary outcome was disability at 90 days, as assessed by the shift analysis on the modified Rankin Scale score, in patients with ischemic stroke. Subgroup analyses according to stroke subtype were evaluated. Results: We included 949 patients with an ischemic stroke, of whom 258 patients(27%) were enrolled during nighttime. Among patients enrolled during nighttime, direct transport to a thrombectomy-capable center was associated with lower degrees of disability at 90 days (adjusted common odds ratio [acOR], 1.620 [95% CI, 1.020–2.551]); no significant difference between trial groups was present during daytime (acOR, 0.890 [95% CI, 0.680–1.163]; P interaction =0.014). Influence of nighttime on the treatment effect was only evident in patients with large vessel occlusion(daytime, acOR 0.766 [95% CI, 0.548–1.072]; nighttime, acOR, 1.785 [95% CI, 1.024–3.112] ; P interaction P interaction >0.1 for all comparisons). We observed longer delays in alteplase administration, interhospital transfers, and mechanical thrombectomy initiation during nighttime in patients allocated to local stroke centers. Conclusions: Among patients evaluated during nighttime for a suspected acute severe stroke in non-urban areas of Catalonia, direct transport to a thrombectomy-capable center was associated with lower degrees of disability at 90 days. This association was only evident in patients with confirmed large vessel occlusion on vascular imaging. Time delays in alteplase administration and interhospital transfers might mediate the observed differences in clinical outcome. Abstract: 12 figures, 6 tables.-- Supplementary information available., Chemical looping combustion (CLC) is a novel technology for the combustion of fuels with inherent CO2 capture. CLC is based on the transference of oxygen from air to fuel by means of an oxygen carrier which is based on a metal oxide. CuO/Al2O3 (14 wt.% CuO) and Fe2O3/Al2O3 (20 wt.% Fe2O3) particles have been used in several CLC units with different results when methane combustion was evaluated. In order to shed light on the main processes affecting methane conversion in CLC, a mathematical model for a dual circulating fluidized bed (DCFB) system was developed to simulate the behavior of CuO/Al2O3 and Fe2O3/Al2O3 in a CLC unit. The model consists of the coupling of individual fuel and air reactor models to simulate steady state of the CLC unit. Individual models consider both the fluid dynamics of the fluidized beds at the high velocity regime and the corresponding kinetics of oxygen carrier reactions, that is, reduction in the fuel reactor and oxidation in the air reactor. The model was validated using results obtained in a 120 kWth CLC unit with CuO/Al2O3 and Fe2O3/Al2O3 particles. The validated model was used to simulate the performance of these materials in the 10 MWth CLC unit. Desing parameters of the fuel and air reactors as well as the suitable particle size of the oxygen carriers were determined in order to achieve the complete combustion of natural gas in the CLC unit as a function of the oxygen carrier properties., This work was supported by Grant funded by MCIN/AEI/10.13039/501100011033 and by the European Union NextGenerationEU/PRTR.

Abstract: La historia de los avances médicos está ligada al desarrollo de la ingeniería. No en pocas ocasiones, los avances en el conocimiento científico-médico han ido por delante de lo que la tecnología podía ofrecer y ha sido sólo, después de años de desarrollo, cuando se ha podido implementar el instrumento que satisfacía las necesidades concretas. En este artículo se pone de manifiesto la simbiosis entre la ingeniería y la medicina a lo largo de la historia y se centra en los avances significativos que han ocurrido en tres tecnologías clave en los últimos años: la robótica, las comunicaciones móviles 5G y la inteligencia artificial. Todo ello con aplicación, principalmente, al ámbito de la cirugía por ser, quizás, uno de los ámbitos donde ésta resulta más impactante. Abstract: El concepto de politainment alude a la progresiva adaptación por parte de actores políticos y mediáticos a la lógica de la cultura del entretenimiento. Este fenómeno ha sido extensamente estudiado en el medio en el que nació, el televisivo; sin embargo, conforme las plataformas digitales ganan un mayor protagonismo como fuente de información política, el politainment ha encontrado en ellas un nuevo espacio en el que desarrollarse. Así, ciertas técnicas relacionadas con este fenómeno se presentan ante los partidos políticos como una forma sencilla de acercar sus mensajes a nuevas audiencias. Este trabajo busca demostrar la creciente traslación y adaptación del politainment a plataformas digitales como YouTube, además de analizar su uso como recurso electoral por parte de los principales partidos políticos españoles. Se ha llevado a cabo en un análisis de contenido de una muestra de los vídeos subidos a YouTube por los cinco principales partidos políticos españoles durante el periodo electoral de las elecciones generales de abril de 2019. Los resultados obtenidos muestran que el uso de técnicas de politainment no es aún mayoritaria en la plataforma en su conjunto, pero que existen importantes diferencias en el grado en el que los distintos partidos recurren a las mismas.

Abstract: EEn este artículo se expone el estudio realizado en el Museo del Prado para la selección de materiales y sistemas de protección in situ ante emergencias de sus colecciones. Se parte de la premisa de una opción factible de protección in situ de las obras que por su naturaleza material, formal, significación o uso dentro de la colección lo permitiera o se aconsejara priorizar frente a un traslado por circunstancias ordinarias o frente a una evacuación, en caso de circunstancias extraordinarias. Inicialmente se definen las necesidades de protección de los bienes culturales distinguiendo las opciones con las que contamos para combatir algunos riesgos concretos o la mayoría de los más comunes. En un análisis posterior se comparan cuatro materiales desarrollados en el mercado centrándonos en los denominados “tejidos ignífugos”, apuntando sus características y propiedades. A continuación, se hace una valoración de los aspectos a considerar cuando se usa como fundas en caso de emergencia sobre obras de arte sensibles a temperaturas altas, llamas, agua y depósitos de partículas.

Abstract: Children with congenital heart disease (CHD) can face neurodevelopmental, psychological, and behavioural difficulties beginning in infancy and continuing through adulthood. Despite overall improvements in medical care and a growing focus on neurodevelopmental screening and evaluation in recent years, neurodevelopmental disabilities, delays, and deficits remain a concern. The Cardiac Neurodevelopmental Outcome Collaborative was founded in 2016 with the goal of improving neurodevelopmental outcomes for individuals with CHD and pediatric heart disease. This paper describes the establishment of a centralised clinical data registry to standardize data collection across member institutions of the Cardiac Neurodevelopmental Outcome Collaborative. The goal of this registry is to foster collaboration for large, multi-centre research and quality improvement initiatives that will benefit individuals and families with CHD and improve their quality of life. We describe the components of the registry, initial research projects proposed using data from the registry, and lessons learned in the development of the registry.

We hypothesize that during the swine flu pandemic in 2009-2010 and the COVID-19 pandemic in 2020, a variety of neologisms spread through newspapers. Employing corpus linguistics, we have extracted and analysed forty candidates to become neologisms and we have classified them according to several neologicity criteria.

Abstract: Mejores geodatos estadísticos implican mejores políticas y la experiencia de la Consejería de Salud de Andalucía es un ejemplo de ello. La incertidumbre en la disponibilidad de información precisa sobre el número y características de las poblaciones potencialmente afectadas por una intervención suele ser el mayor factor limitante de la efectividad de toda evaluación de impacto. Una de las claves del éxito de la implementación de la Evaluación de Impacto en la Salud (EIS) en Andalucía ha sido la disponibilidad de datos de población georreferenciada en cuadrículas que difunde el Instituto de Estadística y Cartografía de Andalucía. Estos datos permiten a la administración y a los agentes económicos y sociales afrontar los grandes retos que plantea la EIS: identificación y caracterización de la población vulnerable. La tecnología y el software libre han permitido dar un paso más allá, facilitando herramientas a la ciudadanía que faciliten esa caracterización y representación espacial de los indicadores necesarios para la elaboración de los informes de evaluación. Es en este contexto donde se ha desarrollado un complemento en QGIS mediante el lenguaje de programación Python que facilita la accesibilidad a los datos geoespaciales indicados y su posterior tratamiento

27. The arrow of time of brain signals in cognition: Potential intriguing role of parts of the default mode network

Abstract: A promising idea in human cognitive neuroscience is that the default mode network (DMN) is responsible for coordinating the recruitment and scheduling of networks for computing and solving task-specific cognitive problems. This is supported by evidence showing that the physical and functional distance of DMN regions is maximally removed from sensorimotor regions containing environment-driven neural activity directly linked to perception and action, which would allow the DMN to orchestrate complex cognition from the top of the hierarchy. However, discovering the functional hierarchy of brain dynamics requires finding the best way to measure interactions between brain regions. In contrast to previous methods measuring the hierarchical flow of information using, for example, transfer entropy, here we used a thermodynamics-inspired, deep learning based Temporal Evolution NETwork (TENET) framework to assess the asymmetry in the flow of events, ‘arrow of time’, in human brain signals. This provides an alternative way of quantifying hierarchy, given that the arrow of time measures the directionality of information flow that leads to a breaking of the balance of the underlying hierarchy. In turn, the arrow of time is a measure of nonreversibility and thus nonequilibrium in brain dynamics. When applied to large-scale Human Connectome Project (HCP) neuroimaging data from close to a thousand participants, the TENET framework suggests that the DMN plays a significant role in orchestrating the hierarchy, that is, levels of nonreversibility, which changes between the resting state and when performing seven different cognitive tasks. Furthermore, this quantification of the hierarchy of the resting state is significantly different in health compared to neuropsychiatric disorders. Overall, the present thermodynamics-based machine-learning framework provides vital new insights into the fundamental tenets of brain dynamics for orchestrating the interactions between cognition and brain in complex environments.

28. Outcomes of subsequent neoplasms after umbilical cord blood transplantation in Europe

Abstract: Subsequent neoplasms (SNs) compromise long-term survivors after hematopoietic cell transplantion. We performed a retrospective analysis of SNs in a cohort of 10358 recipients of umbilical cord blood transplantation (UCBT) reported to Eurocord/EBMT registries from 1988 to 2018. A total of 233 patients developed SNs. Median age at UCBT was 31 years (y) (0.3-69), and 84 were pediatric patients. Indications for UCBT were malignant hematological diseases in 199 patients (85%). Three groups of SNs were observed. Post-transplant lymphoproliferative disorders (PTLD) were reported in 145 patients in a median of 4 months after UCBT. Of these, 9/145 patients died from relapse, 83/145 from PTLD, and 24/145 from transplant-related causes. At last follow-up, 29/145 were alive; 5y-overall survival (OS) after PTLD diagnosis was 21±3%. Acute leukemia / myelodysplasia (AL/MDS) was diagnosed in 23 patients in a median of 28 months after UCBT and included 3 donor-cell AL. Four of 23 patients died from relapse of primary disease, 8/23 from progression of SNs, and 4/23 from TRM. Seven patients were alive at last follow-up; 5y-OS after AL/MDS diagnosis was 36±10%. Solid tumors (ST) were reported in 65 patients in a median of 54 months after UCBT. Most common tumor sites were lung, thyroid, bone and soft tissue. A total of 33/65 patients died (26 due to ST, 6 to relapse of primary disease, 1 cause missing). At last follow-up, 32/65 patients were alive; 5y-OS after the diagnosis of ST was 51±6%. In conclusion, despite their poor outcomes, SNs that occur after UCBT are extremely rare. Identification of associated risk factors and early detection may help to improve OS.

Abstract: PURPOSE Rituximab/chemotherapy is a cornerstone of treatment for Waldenström's macroglobulinemia (WM). In addition, bortezomib has shown significant activity in WM. This study evaluated the efficacy and safety of dexamethasone, rituximab, and cyclophosphamide (DRC) as first-line treatment in WM. METHODS In this European study, treatment-naïve patients were randomly assigned to DRC or bortezomib-DRC B-DRC for six cycles. The primary end point was progression-free survival. Secondary end points included response rates, overall survival, and safety. RESULTS Two hundred four patients were registered. After a median follow-up of 27.5 months, the estimated 24-month progression-free survival was 80.6% (95% CI, 69.5 to 88.0) for B-DRC and 72.8% (95% CI, 61.3 to 81.3) for DRC ( P = .32). At the end of treatment, B-DRC and DRC induced major responses in 80.6% versus 69.9% and a complete response/very good partial response in 17.2% versus 9.6% of patients, respectively. The median time to first response was shorter for B-DRC with 3.0 (95% CI, 2.8 to 3.2) versus 5.5 (95% CI, 2.9 to 5.8) months for DRC. This resulted in higher major response rates (57.0% v 32.5%; P < .01) after three cycles of B-DRC compared with DRC. At best response, the complete response/very good partial response increased to 32.6% for B-DRC. Both treatments were well tolerated: grade ≥ 3 adverse events occurred in 49.2% of all patients (B-DRC, 49.5%; DRC, 49.0%). Peripheral sensory neuropathy grade 3 occurred in two patients treated with B-DRC and in none with DRC. CONCLUSION This large randomized study illustrates that B-DRC is highly effective and well tolerated in WM. The data demonstrate that fixed duration immunochemotherapy remains an important pillar in the clinical management of WM.

Abstract: Antecedentes: La violencia de pareja (VP) es un fenómeno social complejo ampliamente estudiado. Sin embargo, pocos de estos estudios consideran el capital social y la interseccionalidad. Nuestro objetivo fue describir el efecto del capital social en la victimización por VP entre estudiantes de secundaria, considerando tres factores de interseccionalidad (sexo asignado al nacer, orientación sexual y origen migratorio) en dos ciudades de España en 2019-2020. Métodos: Se analizó una muestra transversal de 640 adolescentes con pareja, de 13 a 16 años, de un programa de promoción de relaciones equitativas en Alicante y Terrassa. La variable resultado fue haber sufrido violencia en la pareja (control, miedo, física o sexual) en algún momento de la vida. Se utilizaron variables individuales y relacionales para calcular la razón de prevalencia ajustada (RPa) a partir de modelos multivariable construidos mediante regresión robusta de Poisson, y estratificados por sexo, orientación sexual y origen migratorio. Resultados: La mayor prevalencia de VP (56,25%) se encontró en adolescentes lesbianas, gays o bisexuales (LGB) sin actividad social. La orientación LGB se asoció significativamente con la victimización por VP en los adolescentes con origen en países de bajos ingresos (PBI) (aPR: 1,93) y en las chicas (aPR: 1,53). Por último, la procedencia de un PBI fue un determinante significativo de la VP en los chicos (aPR: 1,76) y en los estudiantes, independientemente de la orientación sexual. Una mayor actividad social mostró un efecto protector para los estudiantes de origen en países de altos ingresos (PAI) y de orientación sexual LGB, independientemente del sexo y la orientación sexual. Conclusiones: La actividad social se asocia de forma desigual con una menor victimización por violencia de la pareja en algún momento de la vida adolescente. La fuerte asociación del origen migratorio y la orientación sexual con la VPI refuerza el enfoque de su prevención en los adolescentes desde una perspectiva de interseccionalidad. Background: Intimate partner violence (IPV) is a complex social phenomenon widely studied. However, few of these studies consider social capital and intersectionality. Our aim was to describe the effect of social capital in IPV victimisation among secondary students, considering three factors of intersectionality (sex assigned at birth, sexual orientation and migration background) in two cities of Spain in 2019-2020. Methods: We analysed a cross-sectional sample of 640 ever-partnered adolescents aged 13–16 years who had taken part in a programme for positive relationship. The main outcome was lifetime IPV (control, fear, physical or sexual violence). Individual and relational variables (bonding social capital) were used to calculate adjusted prevalence ratios (aPR) from multivariate Robust Poisson regression models stratified by sex assigned at birth, sexual orientation and migration background (factors of intersectionality). Results: The highest IPV prevalence (56.25%) was found in lesbian, gay or bisexual (LGB) adolescents without social activity. LGB orientation was significantly associated with IPV victimisation in adolescents with low-income country (LIC) backgrounds (aPR: 1.93) and in girls (1.53). Finally, a LIC background was a significant determinant of IPV in boys (aPR: 1.76) and in students independently of sexual orientation. Higher social activity showed a protective effect for students with HIC backgrounds and LGB-sexual orientations. A possible protective effect of social support in HIC backgrounds and regardless of sex and sexual orientation must be considered. Conclusions: Social activity is unequally associated with less lifetime IPV. The strong association of migration background and sexual orientation with IPV reinforces the approach to its prevention in adolescents from an intersectionality perspective. Se incide en que si se mantienen los estilos sociales, demográficas y migratorias, la población expuesta a infecciones de transmisión sexual seguirá aumentando espectacularmente. La carga de morbilidad es particularmente pesada en el mundo en desarrollo, pero también en los países industrializados cabe prever un aumento de la carga de enfermedad debido a la prevalencia de infecciones víricas incurables, a las tendencias en el comportamiento sexual y al incremento de los viajes.

Abstract: Molecular modification of compounds remains important strategy towards the discovery of new drugs. In this sense, this study presents a new pyrazole derivative 5-(1-(2-fluorophenyl)-1 H-pyrazol-4-yl)-1 H-tetrazole (LQFM039) and evaluated the anti-inflammatory, analgesic, and vasorelaxant effects of this compound as well the mechanisms of action involved in the pharmacological effects. For this, mice were orally treated with LQFM039 (17.5, 35, or 70 mg/kg) prior acetic acid-induced abdominal writhing, formalin, tail flick, and carrageenan-induced paw edema protocols. In addition, vascular reactivity protocols were made with aortic rings contraction with phenylephrine and stimulated with graded concentrations of LQFM039. Abdominal writhing and licking time in both neurogenic and inflammatory phases of formalin were reduced with LQFM039 without altering latency to nociceptive response in the tail flick test. Carrageenan-induced paw edema showed that LQFM039 reduces edema and cell migration. In addition, the mechanism of action of LQFM039 involves NO/cGMP pathway and calcium channels, since this new pyrazole derivate elicited concentration-dependent relaxation attenuated by Nω-nitro-l-arginine methyl ester and 1 H-[1,2,4] oxadiazolo [4,3-alpha]quinoxalin-1-one, and blockade of CaCl2-induced contraction. Altogether, our finding suggests anti-inflammatory, antinociceptive, and vasorelaxant effect of this new pyrazole derivative with involvement of NO/cGMP pathway and calcium channels.

43. MEG Oscillatory Slowing in Cognitive Impairment is Associated with the Presence of Subjective Cognitive Decline

Abstract: The mechanisms behind Alzheimer's disease are not yet fully described, and changes in the electrophysiology of patients across the continuum of the disease could help to understand them. In this work, we study the power spectral distribution of a set of 129 individuals from the Connectomics of Brian Aging and Dementia project. From this sample, we acquired task-free data, with eyes closed, and estimated the power spectral distribution in source space. We compared the spectral profiles of three groups of individuals: 70 healthy controls, 27 patients with amnestic MCI, and 32 individuals showing cognitive impairment without subjective complaints (IWOC). The results showed a slowing of the brain activity in the aMCI patients, when compared to both the healthy controls and the IWOC individuals. These differences appeared both as a decrease in power for high frequency oscillations and an increase in power in alpha oscillations. The slowing of the spectrum was significant mainly in parietal and medial frontal areas. We were able to validate the slowing of the brain activity in individuals with aMCI, appearing in our sample in areas related to the default mode network. However, this pattern did not appear in the IWOC individuals, suggesting that their condition is not part of the AD continuum. This work raises interesting questions about this group of individuals, and the underlying brain mechanisms behind their cognitive impairment.

44. Clinical and microbiological efficacy of indocyanine green-based antimicrobial photodynamic therapy as an adjunct to non-surgical treatment of periodontitis: a randomized controlled clinical trial

Abstract: Objectives The aim of the present randomized clinical trial (RCT) with a parallel arm design was to evaluate the clinical and microbiological efficacy of repeated ICG-aPDT as an adjunct to full-mouth subgingival debridement in the treatment of periodontitis. Materials and methods Twenty-four periodontitis patients were treated with full-mouth ultrasonic subgingival debridement (FMUD). Initial sites with probing depth (PD) > 4 mm were randomly assigned to receive the test (ICG-aPDT with an 810 nm diode laser) or the control treatment (off-mode aPDT) one and four weeks after FMUD. Clinical parameters were registered after 3 and 6 months. The presence of the main periodontal pathogens in subgingival samples was assessed with real-time PCR. Results Both treatment modalities resulted in significant clinical improvements at 3 and 6 months. The only significant differences in favour of the test group were found at 6 months for a higher PD reduction in initial deep pockets (PD ≥ 6 mm) and a higher percentage of closed pockets (PD ≤ 4 mm/no bleeding on probing). Limited microbiological changes were observed in both groups after treatment with no inter-group difference, except for a more significant reduction in Aggregatibacter actinomycetemcomitans and Parvimonas micra levels in the test group at 3 months. Conclusion The combination of repeated ICG-aPDT and FMUD provided no benefits except for selective clinical and microbiological improvements compared to FMUD alone. Clinical relevance Based on the obtained results, only limited adjunctive effects could be found for the combined use of ICG-aPDT and FMUD. Further, well-designed RCT with larger sample sizes are required to confirm these findings. Trial registration ClinicalTrials.gov NCT04671394.

Binocular contrast sensitivity with and without glare was measured under photopic conditions (85 cd/m2) and mesopic conditions (3 cd/m2). Patients were asked to complete the Catquest-9SF patient outcomes questionnaire and a visual symptoms questionnaire.Results: Photopic contrast sensitivity values were either within or above normal levels without glare; when glare was induced, the mean sensitivity values dropped just below normal range. Mesopic contrast sensitivity values were above or within normal range both with and without glare, except for 12 cpd with glare, where the mean fell just below the normal range. Differences in binocular contrast sensitivity threshold values with and without glare were significant for all spatial frequencies tested under both photopic and mesopic conditions (p< 0.05). The Catquest-9SF questionnaire outcomes showed that 88% of patients were either satisfied or very satisfied with their sight after the surgery, and in all cases, the results indicated no difficulty in performing different daily activities. The visual symptoms questionnaire indicated no relevant visual symptoms regarding frequency, intensity, or bothersomeness after implantation of the trifocal IOL.Conclusion: This novel bi-aspheric diffractive trifocal IOL provides good contrast sensitivity outcomes under bright and dim lighting conditions. Abstract: Transition metal nitrides have shown promise as electrocatalysts in proton exchange membrane fuel cells and electrolyzers, but the instability of these nitrides in acid has limited their function for such technologies. On the other hand, having fast, complete nitride dissolution and ammonia formation in acid can offer new opportunities for distributed, on-demand ammonia generation. Optimizing nitride chemistries for these clean energy applications requires design principles for nitride dissolution and ammonia formation in acid. Here, we report that lowering the nitrogen 2p band center of transition metal nitrides relative to the Fermi level weakens metal–nitrogen bonds and increases labile metallic character, reducing dissolution reaction barriers and boosting ammonia formation kinetics in acid. Increasing the solubility of dissolved metal cations further facilitates the decomposition of nitrides in acid by prohibiting surface oxide passivation. These findings highlight essential future directions for preventing nitride dissolution or facilitating ammonia production for diverse acidic applications.

Abstract: Background: We aim to compare the outcome of patients from urban areas, where the referral center is able to perform thrombectomy, with patients from nonurban areas enrolled in the RACECAT trial (Direct Transfer to an Endovascular Center Compared to Transfer to the Closest Stroke Center in Acute Stroke Patients With Suspected Large Vessel Occlusion). Methods: Patients with suspected large vessel occlusion stroke, as evaluated by a Rapid Arterial Occlusion Evaluation score of ≥5, from urban catchment areas of thrombectomy-capable centers during RACECAT trial enrollment period were included in the Stroke Code Registry of Catalonia. Primary outcome was disability at 90 days, as assessed by the shift analysis on the modified Rankin Scale score, in patients with an ischemic stroke. Secondary outcomes included mortality at 90 days, rate of thrombolysis and thrombectomy, time from onset to thrombolysis, and thrombectomy initiation. Propensity score matching was used to assemble a cohort of patients with similar characteristics. Results: The analysis included 1369 patients from nonurban areas and 2502 patients from urban areas. We matched 920 patients with an ischemic stroke from urban areas and nonurban areas based on their propensity scores. Patients with ischemic stroke from nonurban areas had higher degrees of disability at 90 days (median [interquartle range] modified Rankin Scale score, 3 [2–5] versus 3 [1–5], common odds ratio, 1.25 [95% CI, 1.06–1.48]); the observed average effect was only significant in patients with large vessel stroke (common odds ratio, 1.36 [95% CI, 1.08–1.65]). Mortality rate was similar between groups(odds ratio, 1.02 [95% CI, 0.81–1.28]). Patients from nonurban areas had higher odds of receiving thrombolysis (odds ratio, 1.36 [95% CI, 1.16–1.67]), lower odds of receiving thrombectomy(odds ratio, 0.61 [95% CI, 0.51–0.75]), and longer time from stroke onset to thrombolysis (mean difference 38 minutes [95% CI, 25–52]) and thrombectomy(mean difference 66 minutes [95% CI, 37–95]). Conclusions: In Catalonia, Spain, patients with large vessel occlusion stroke triaged in nonurban areas had worse neurological outcomes than patients from urban areas, where the referral center was able to perform thrombectomy. Interventions aimed at improving organizational practices and the development of thrombectomy capabilities in centers located in remote areas should be pursued. 48. Enhancing Biomolecular Simulations with Hybrid Potentials Incorporating NMR Data

Abstract: Some recent advances in biomolecular simulation and global optimization have used hybrid restraint potentials, where harmonic restraints that penalize conformations inconsistent with experimental data are combined with molecular mechanics force fields. These hybrid potentials can be used to improve the performance of molecular dynamics, structure prediction, energy landscape sampling, and other computational methods that rely on the accuracy of the underlying force field. Here, we develop a hybrid restraint potential based on NapShift, an artificial neural network trained to predict protein nuclear magnetic resonance (NMR) chemical shifts from sequence and structure. In addition to providing accurate predictions of experimental chemical shifts, NapShift is fully differentiable with respect to atomic coordinates, which allows us to use it for structural refinement. By employing NapShift to predict chemical shifts from the protein conformation at each simulation step, we can compute an energy penalty and the corresponding hybrid restraint forces based on the difference between the predicted values and the experimental chemical shifts. The performance of the hybrid restraint potential was benchmarked using both basin-hopping global optimization and molecular dynamics simulations. In each case, the NapShift hybrid potential improved the accuracy, leading to better structure prediction via basin-hopping and increased local stability in molecular dynamics simulations. Our results suggest that neural network hybrid potentials based on NMR observables can enhance a broad range of molecular simulation methods, and the prediction accuracy will improve as more experimental training data become available.

49. Evaluación del estado nutricional en la enfermedad de Alzheimer y su influencia en la progresión tras el diagnóstico

Abstract: Resumen Introduccion Las deficiencias nutricionales son frecuentes en la enfermedad de Alzheimer (EA), incluso en fases iniciales. El deterioro nutricional (DN) puede asociarse con una progresion mas rapida de la enfermedad. El objetivo fue describir la frecuencia y los factores de riesgo asociados a DN en el momento del diagnostico y analizar su influencia en la evolucion posterior. Metodos Estudio observacional, multicentrico, prospectivo. Se incluyeron sujetos recien diagnosticados de EA prodromica (EAp) o demencia por EA (EAd). Se realizaron dos evaluaciones en un periodo de 18 meses. Para estimar el estado nutricional se empleo el Mini Nutritional Assessment Test (MNA, rango 0-30; DN: MNA Resultados Se incluyeron 50 sujetos con EAp (edad 76,1 ± 5,3 anos; 68% mujeres) y 127 con EAd (edad 80 ± 5,9 anos; 72,4% mujeres); 141 (79,7%) completaron las dos evaluaciones. La prevalencia de DN fue del 28,2% (EAp 24%, EAd 29,9%; p = 0,43), la mayoria (92%) en riesgo de desnutricion. El DN se asocio con el sexo femenino (OR: 4,2; IC 95%: 1,7-10,5; p Conclusiones La prevalencia de DN en la EA es elevada. La evaluacion del estado nutricional en el momento del diagnostico puede permitir identificar pacientes con mayor riesgo de progresion de la enfermedad.
